Abstract

Abstract Lifestyle interventions are strategies used to self‐manage medical conditions, such as epilepsy, and often complement traditional pharmacologic and surgical therapies. The need for integrating evidence‐based lifestyle interventions into mainstream medicine for the treatment of epilepsy is evident given that despite the availability of a multitude of treatments with medications and surgical techniques, a significant proportion of patients have refractory seizures, and even those who are seizure‐free report significant adverse effects with current treatments. Although the evidence base for complementary medicine is less robust than it is for traditional forms of medicine, the evidence to date suggests that several forms of complementary medicine including yoga, mindfulness meditation, cognitive behavioral therapy, diet and nutrition, exercise and memory rehabilitation, and music therapy may have important roles as adjuncts in the treatment armamentarium for epilepsy. These topics were discussed by a diverse group of medical providers and scientists at the “Lifestyle Intervention for Epilepsy (LIFE)” symposium hosted by Cleveland Clinic. Plain Language Summary There are many people with epilepsy who continue to have seizures even though they are being treated with medication or brain surgery. Even after seizures stop, some may experience medication side effects. There is research to suggest that certain lifestyle changes, such as yoga, mindfulness, exercise, music therapy, and adjustments to diet, could help people with epilepsy, when used along with routine treatment. Experts discussed the latest research at the “Lifestyle Intervention for Epilepsy (LIFE)” symposium hosted by Cleveland Clinic.

Abstract

We measured the radiation tolerance of commercially available diamonds grown by the Chemical Vapor Deposition process by measuring the charge created by a 120 GeV hadron beam in a 50 μm pitch strip detector fabricated on each diamond sample before and after irradiation. We irradiated one group of samples with 70 MeV protons, a second group of samples with fast reactor neutrons (defined as energy greater than 0.1 MeV), and a third group of samples with 200 MeV pions, in steps, to (8.8±0.9) × 1015 protons/cm2, (1.43±0.14) × 1016 neutrons/cm2, and (6.5±1.4) × 1014 pions/cm2, respectively. By observing the charge induced due to the separation of electron–hole pairs created by the passage of the hadron beam through each sample, on an event-by-event basis, as a function of irradiation fluence, we conclude all datasets can be described by a first-order damage equation and independently calculate the damage constant for 70 MeV protons, fast reactor neutrons, and 200 MeV pions. We find the damage constant for diamond irradiated with 70 MeV protons to be 1.62±0.07(stat)±0.16(syst)× 10−18 cm2/(p μm), the damage constant for diamond irradiated with fast reactor neutrons to be 2.65±0.13(stat)±0.18(syst)× 10−18 cm2/(n μm), and the damage constant for diamond irradiated with 200 MeV pions to be 2.0±0.2(stat)±0.5(syst)× 10−18 cm2/(π μm). The damage constants from this measurement were analyzed together with our previously published 24 GeV proton irradiation and 800 MeV proton irradiation damage constant data to derive the first comprehensive set of relative damage constants for Chemical Vapor Deposition diamond. We find 70 MeV protons are 2.60 ± 0.29 times more damaging than 24 GeV protons, fast reactor neutrons are 4.3 ± 0.4 times more damaging than 24 GeV protons, and 200 MeV pions are 3.2 ± 0.8 more damaging than 24 GeV protons. We also observe the measured data can be described by a universal damage curve for all proton, neutron, and pion irradiations we performed of Chemical Vapor Deposition diamond. Finally, we confirm the spatial uniformity of the collected charge increases with fluence for polycrystalline Chemical Vapor Deposition diamond, and this effect can also be described by a universal curve.

Abstract

There are overwhelming data supporting the inflammatory origin of some epilepsies (e.g., Rasmussen's encephalitis and limbic encephalitis). Inflammatory epilepsies with an autoimmune component are characterized by autoantibodies against membrane-bound, intracellular or secreted proteins (e.g., voltage gated potassium channels). Comparably, little is known regarding autoantibodies targeting nuclear antigen. We tested the hypothesis that in addition to known epilepsy-related autoantigens, the human brain tissue and serum from patients with epilepsy contain autoantibodies recognizing nuclear targets. We also determined the specific nuclear proteins acting as autoantigen in patients with epilepsy.Brain tissue samples were obtained from patients undergoing brain resections to treat refractory seizures, from the brain with arteriovenous malformations or from post-mortem multiple sclerosis brain. Patients with epilepsy had no known history of autoimmune disease and were not diagnosed with autoimmune epilepsy. Tissue was processed for immunohistochemical staining. We also obtained subcellular fractions to extract intracellular IgGs. After separating nuclear antibody–antigen complexes, the purified autoantigen was analyzed by mass spectrometry. Western blots using autoantigen or total histones were probed to detect the presence of antinuclear antibodies in the serum of patients with epilepsy. Additionally, HEp-2 assays and antinuclear antibody ELISA were used to detect the staining pattern and specific presence of antinuclear antibodies in the serum of patients with epilepsy.Brain regions from patients with epilepsy characterized by blood–brain barrier disruption (visualized by extravasated albumin) contained extravasated IgGs. Intracellular antibodies were found in epilepsy (n = 13/13) but not in multiple sclerosis brain (n = 4/4). In the brain from patients with epilepsy, neurons displayed higher levels of nuclear IgGs compared to glia. IgG colocalized with extravasated albumin. All subcellular fractions from brain resections of patients with epilepsy contained extravasated IgGs (n = 10/10), but epileptogenic cortex, where seizures originated from, displayed the highest levels of chromatin-bound IgGs. In the nuclear IgG pool, anti-histone autoantibodies were identified by two independent immunodetection methods. HEp-2 assay and ELISA confirmed the presence of anti-histone (n = 5/8) and anti-chromatin antibodies in the serum from patients with epilepsy.We developed a multi-step approach to unmask autoantigens in the brain and sera of patients with epilepsy. This approach revealed antigen-bound antinuclear antibodies in neurons and free antinuclear IgGs in the serum of patients with epilepsy. Conditions with blood–brain barrier disruption but not seizures, were characterized by extravasated but not chromatin-bound IgGs. Our results show that the pool of intracellular IgG in the brain of patients with epilepsy consists of nucleus-specific autoantibodies targeting chromatin and histones. Seizures may be the trigger of neuronal uptake of antinuclear antibodies.

Abstract

A 30-year-old woman with intractable seizures evaluated as surgical candidate. During presurgical evaluation an invasive electroencephalogram was recommended to define the location and extent of epileptogenic zone and relationship to epileptogenic lesion. On third monitoring night the patient complained of persistent homonymous hemianopsia following a habitual seizure. Concurrently, persistent epileptic activity was evident in a small, restricted area around the right calcarine fissure. The ictal discharges persisted for the next 30 h despite high-dose administration of intravenous antiepileptic drugs, until patient was taken to operating room. Simple partial status epilepticus presenting with pure visual symptoms is rare and difficult to diagnose, even more so when presenting with negative visual phenomena. Epileptic etiology of unexplained, paroxysmal negative visual symptoms should be considered in the differential diagnosis in patients with pre-existing epilepsy, as well as patients with no prior history of epilepsy.

Abstract

The treatment of waste and especially solid waste, the type with the highest increase in terms of annual generation over the last decade, is a key issue in the Balkan and Mediterranean region. Piecemeal efforts to deal with it within the prevailing linear economy model were not successful since the techniques used such as recycling and reusing could not be effective with the existing products. A definitive solution requires the switch to a new model, the circular economy model, which will facilitate the tackling of the excessive use of virgin raw materials and waste generation. The design and development of a digital solid waste reuse platform in the context of the EU-funded Interreg Project SWAN involving four countries: Albania, Bulgaria, Cyprus and Greece, was a step in this direction. The present paper based on the evidence drawn from this project examines the current situation and the future trends in the solid waste reuse and industrial symbiosis schemes in this region.

Abstract

The wide implementation of circular economy and industrial symbiosis in the Balkan and Mediterranean region lags behind most of the other European countries. The economic crisis has had a significant impact to the local businesses, eliminating new investments. Regarding specifically solid waste, which is the type with the highest increase in terms of annual generation over the last decade, the situation is aggravated by the lack of national or regional strategies. In this paper, taking into consideration the results of the implementation of SWAN research project, which led to the design and development of a digital solid waste reuse platform for the Balkans, our objectives are: (a) to compare the current state of solid waste management in four different countries (Albania, Bulgaria, Cyprus, Greece), (b) analyse the opinions of various directly involved actors on solid waste management issues and on their familiarisation with new concepts such as circular economy and urban/industrial symbiosis; and (c) compare the feasible business models in all four countries, in terms of their quantitative and qualitative characteristics.

Abstract

Magnetic resonance fingerprinting (MRF) is a novel, quantitative, and noninvasive technique to measure brain tissue properties. We aim to use MRF for characterizing normal-appearing thalamic and basal ganglia nuclei in the epileptic brain.A three-dimensional (3D) MRF protocol (1 mmMRF revealed increased T1 mean value in the ipsilateral thalamus and nucleus accumbens; increased T1 CV in the bilateral thalamus, bilateral pallidum, and ipsilateral caudate; and increased T2 CV in the ipsilateral thalamus in patients compared to HCs (p .05, false discovery rate [FDR] corrected). The SVM classifier produced 78.2% average accuracy to separate individual patients from HCs, with an area under the curve (AUC) of 0.83. The logistic regression classifier produced 67.4% average accuracy to separate patients with left-sided and right-sided epilepsy, with an AUC of 0.72.MRF revealed bilateral tissue-property changes in the normal-appearing thalamus and basal ganglia, with ipsilateral predominance and thalamic preference, suggesting subcortical involvement/impairment in patients with medically intractable focal epilepsy. The individual-level performance of the MRF-based machine-learning models suggests potential opportunities for predicting lateralization.

Abstract

A large number of EU organisations already leverage Big Data pools to drive value and investments. This trend also applies to the banking sector. As a specific example, CaixaBank currently manages more than 300 different data sources (more than 4 PetaBytes of data and increasing), and more than 700 internal and external active users and services are processing them every day. In order to harness value from such high-volume and high-variety of data, banks need to resolve several challenges, such as finding efficient ways to perform Big Data analytics and to provide solutions that help to increase the involvement of bank employees, the true decision-makers. In this chapter, we describe how these challenges are resolved by the self-service solution developed within the I-BiDaaS project. We present three CaixaBank use cases in more detail, namely, (1)analysis of relationships through IP addresses, (2)advanced analysis of bank transfer payment in financial terminalsand (3)Enhanced control of customers in online banking, and describe how the corresponding requirements are mapped to specific technical and business KPIs. For each use case, we present the architecture, data analysis and visualisation provided by the I-BiDaaS solution, reporting on the achieved results, domain-specific impact and lessons learned.

Abstract

Manufacturing processes are highly complex. Production lines have several robots and digital tools, generating massive amounts of data. Unstructured, noisy and incomplete data have to be collected, aggregated, pre-processed and transformed into structured messages of a common, unified format in order to be analysed not only for the monitoring of the processes but also for increasing their robustness and efficiency. This chapter describes the solution, best practices, lessons learned and guidelines for Big Data analytics in two manufacturing scenarios defined by CRF, within the I-BiDaaS project, namely ‘Production process of aluminium die-casting’, and ‘Maintenance and monitoring of production assets’. First, it reports on the retrieval of useful data from real processes taking into consideration the privacy policies of industrial data and on the definition of the corresponding technical and business KPIs. It then describes the solution in terms of architecture, data analytics and visualizations and assesses its impact with respect to the quality of the processes and products.

Abstract

The increase in waste volume and greenhouse gas emissions and decrease in raw-material reserves are some of the serious problems that our planet is facing. The measures needed to address these issues cannot be implemented under the prevailing linear economy model; hence, the circular economy model has been introduced. The successful implementation of circularity, whose basic principles include waste reduction, reuse, and recycling, requires a change in the behaviour of all the parties involved and is expected to lead to industrial–urban symbiosis schemes. The present paper looks at the current state and future prospects of the circular economy in Cyprus, based on the evidence drawn from an EU-funded project entitled “a digital Solid Waste reuse plAtform for BalkaN” (Project Acronym: SWAN). The project’s main objective was the design and development of a digital solid waste reuse platform involving four countries: Greece, Albania, Bulgaria, and Cyprus. Using the data collected, in the context of this project, from a sample of Cypriot industries, we looked into the familiarisation of the respondents with the basic concepts of circularity and their willingness to participate in symbiotic value chains. Moreover, we examined the composition of the waste streams produced by those industries and proposed potential waste reuse business models and subsequent symbiotic clusters.

Abstract

International audience; We have measured the radiation tolerance of chemical vapor deposition (CVD) diamond against protons and neutrons. The relative radiation damage constant of 24 GeV protons, 800 MeV protons, 70 MeV protons, and fast reactor neutrons is presented. The results are used to combine the measured data into a universal damage curve for diamond material.

Abstract

International audience; In sight of the luminosity increase of the High Luminosity-LHC (HL-LHC), most experiments at the CERN Large Hadron Collider (LHC) are planning upgrades for their innermost layers in the next 5–10 years. These upgrades will require more radiation tolerant technologies than exist today. Usage of Chemical Vapor Deposition (CVD) diamond as detector material is one of the potentially interesting technologies for the upgrade. CVD diamond has been used extensively in the beam condition monitors of BaBar, Belle, CDF and all LHC experiments. Measurements of the radiation tolerance of the highest quality polycrystalline CVD material for a range of proton energies, pions and neutrons obtained with this material are presented. In addition, new results on the evolution of various semiconductor parameters as a function of the dose rate are described.

Abstract

The planned upgrade of the LHC to the High-Luminosity-LHC will push the luminosity limits above the original design values. Since the current detectors will not be able to cope with this environment ATLAS and CMS are doing research to find more radiation tolerant technologies for their innermost tracking layers. Chemical Vapour Deposition (CVD) diamond is an excellent candidate for this purpose. Detectors out of this material are already established in the highest irradiation regimes for the beam condition monitors at LHC. The RD42 collaboration is leading an effort to use CVD diamonds also as sensor material for the future tracking detectors. The signal behaviour of highly irradiated diamonds is presented as well as the recent study of the signal dependence on incident particle flux. There is also a recent development towards 3D detectors and especially 3D detectors with a pixel readout based on diamond sensors.

Abstract

Beam Loss Monitors (BLM) made of silicon are new devices for monitoring of radiation environment in the vicinity of superconductive magnets of the Large Hadron Collider. The challenge of BLMs is extreme radiation hardness, up to 10 16 protons/cm 2 while placed in superfluid helium (temperature of 1.9 K). CERN BE-BI-BL group, together with CERN-RD39 collaboration, has developed prototypes of BLMs and investigated their device physics. An overview of this development—results of the in situ radiation tests of planar silicon detectors at 1.9 K, performed in 2012 and 2014—is presented. Our main finding is that silicon detectors survive under irradiation to 1×10 16 p/cm 2 at 1.9 K. In order to improve charge collection, current injection into the detector sensitive region (Current Injection Detector (CID)) was tested. The results indicate that the detector signal increases while operated in CID mode.

22. A comprehensive clinico-pathological and genetic evaluation of bottom-of-sulcus focal cortical dysplasia in patients with difficult-to-localize focal epilepsy

Abstract

We comprehensively studied the clinical presentation, stereo-EEG and MRI findings, histopathological diagnosis, and brain somatic mutations in a retrospective series of drug-resistant patients with difficult-to-localize epilepsy due to focal cortical dysplasia at the bottom of a sulcus (BOS-FCD). We identified 10 patients with BOS-FCD from the Cleveland Clinic epilepsy surgery database submitted for intracranial video-EEG monitoring. Brain MRI, including voxel-based morphometric analysis and surgical tissue submitted for histopathology, was reviewed. Paraffin tissue samples from five patients were made available for targeted next-generation sequencing. Postsurgical follow-up was available in nine patients. BOS-FCD was identified in the superior frontal sulcus in six patients, inferior frontal sulcus in one patient, central sulcus in one patient, and intraparietal sulcus in two patients. All patients had stereotyped seizures. Intracranial EEG recordings identified ictal onset at the BOS-FCD in all 10 patients, whereas ictal scalp EEG had a localizing value in only six patients. Complete resection was achieved by lesionectomy or focal corticectomy in nine patients. Histopathologically, six patients had FCD type IIb and three had FCD type IIa. Next-generation sequencing analysis of DNA extracted from lesion-enriched (micro-dissected) tissue from five patients with FCD type II led to the identification of a germline frameshift insertion in DEPDC5, introducing a premature stop in one patient. Eight out of nine patients with available follow-up were completely seizure-free (Engel Class IA) after a mean follow-up period of six years. Our results confirm previous studies classifying difficult-to-localize BOS-FCD into the emerging spectrum of FCD ILAE type II mTORopathies. Further studies with large patient numbers and ultra-deep genetic testing may help to bridge the current knowledge gap in genetic aetiologies of FCD.

Abstract

Strengthening capacity of urban systems and their constituents in order to effectively tackle contemporary challenges and risks in a rapidly evolving, complex and uncertain global environment; and produce value for local communities in a sustainable and inclusive way, brings to the forefront the concept of smart governance. Tracking paths to smart governance, being the focus of this work, implies the need to conceptualize smartness and governance; and shed light on key organizational attributes that can pave the way for the transition from government to smart governance. Having identified these attributes by literature review, the paper highlights institutional, organizational, societal etc. developments in a Greek city, Korydallos Municipality, in order barriers and gaps in its trajectory to smart governance to be illuminated, both before and during the economic recession and austerity stress, faced by the Greek economy. Experiences gained by this pilot example, representing a rather typical small and medium-sized city in the Mediterranean context, provide useful inferences and evidence-based results for similar cities that strive to ride the smart governance wave.

Abstract

11th International Conference on Position Sensitive Detectors (PSD11),3–8 sept. 2017, The Open University, Milton Keynes, UK.; International audience; Beam test results of the radiation tolerance study of chemical vapour deposition (CVD) diamond against different particle species and energies is presented. We also present beam test results on the independence of signal size on incident particle rate in charged particle detectors based on un-irradiated and irradiated poly-crystalline CVD diamond over a range of particle fluxes from 2 kHz/cm2 to 10 MHz/cm2. The pulse height of the sensors was measured with readout electronics with a peaking time of 6 ns. In addition functionality of poly-crystalline CVD diamond 3D devices was demonstrated in beam tests and 3D diamond detectors are shown to be a promising technology for applications in future high luminosity experiments.

Abstract

Silicon detectors were proposed as novel Beam Loss Monitors (BLM) for the control of the radiation environment in the vicinity of the superconductive magnets of the High-Luminosity Large Hadron Collider. The present work is aimed at enhancing the BLM sensitivity and therefore the capability of triggering the beam abort system before a critical radiation load hits the superconductive coils. We report here the results of three in situ irradiation tests of Si detectors carried out at the CERN PS at 1.9–4.2 K. The main experimental result is that all silicon detectors survived irradiation up to 1.22× 1016 p/cm2. The third test, focused on the detailed characterization of the detectors with standard (300 μm) and reduced (100 μm) thicknesses, showed only a marginal difference in the sensitivity of thinned detectors in the entire fluence range and a smaller rate of signal degradation that promotes their use as BLMs. The irradiation campaigns produced new information on radiation damage and carrier transport in Si detectors irradiated at the temperatures of 1.9–4.2 K. The results were encouraging and permitted to initiate the production of the first BLM prototype modules which were installed at the end of the vessel containing the superconductive coil of a LHC magnet immersed in superfluid helium to be able to test the silicon detectors in real operational conditions.

Abstract

Study Design Serial screening of Roma children for idiopathic scoliosis. Objective To confirm or reject the observation that the prevalence of scoliosis is reduced in the Roma population and possibly to explain it. Materials and Methods The authors conducted serial screening for idiopathic scoliosis of 1,034 indigenous Roma children (542 boys and 492 girls), aged 4–18 years (857 children were aged 8–18 years) from 1997 to 2011. Age, height, weight, body mass index, years of schooling, and menarche for girls were recorded. Children were clinically examined for body asymmetries and a standing posteroanterior spinal radiograph was obtained in selected cases. Results Sixty Roma children (6%) had clinical humps. Single humps, according to location, were mostly benign and not related to progressive scoliotic curves. In children with right thoracic humps a left lumbar component could be overlooked. Of 60 children, only 4 (3 girls and 1 boy) with right thoracic and left lumbar or thoracolumbar humps had true progressive scoliotic curves with greater than 10° Cobb angle (prevalence rate, 0.35%). This is 5 times less than the rate of 1.5% in Greek children. One of these 4 children was young and had possible congenital scoliosis; the other 3 were early adolescents. Conclusions A substantial difference in the prevalence of scoliosis between Roma and Greek children was documented. The usual percentage of adolescent scoliosis found in the Greek population (approximately 15–17 cases/1,000 children) was not seen in this sample of Roma children.

Abstract

Stereo-electroencephalography (SEEG) enables precise recordings from deep cortical structures, multiple noncontiguous lobes, as well as bilateral explorations while avoiding large craniotomies. Despite a long reported successful record, its application in the United States has not been widely adopted. We report on our initial experience with the SEEG methodology in the extraoperative mapping of refractory focal epilepsy in patients who were not considered optimal surgical candidates for other methods of invasive monitoring. We focused on the applied surgical technique and its utility and efficacy in this subgroup of patients.Between March 2009 and May 2011, 100 patients with the diagnosis of medically refractory focal epilepsy who were not considered optimal candidates for subdural grids and strips placement underwent SEEG implantation at Cleveland Clinic Epilepsy Center. Demographics, noninvasive clinical data, number and location of implanted electrodes, electrophysiologic localization of the epileptic zone, complications, and short-term seizure outcome after resection were prospectively collected and analyzed.Mean age was 32 years (range 5-68 years); 54 were male and 46 female. The mean follow-up after resection was 15 months. In total, 1,310 electrodes were implanted. Analyses of the SEEG recordings resulted in the electrographic localization of the epileptogenic focus in 96 patients. In the group of 75 patients who underwent resection, only 53 had at least 12 months follow-up. From this group, 33 patients (62.3%) were seizure-free at the end of the follow-up period. The presence of abnormal pathologic finding was strongly associated with postoperative seizure control (p = 0.005). The risk of hemorrhagic complications per electrode was 0.2%.In patients who are not considered to be ideal candidates for subdural grids and strips implantation, the SEEG methodology is a safe, useful and reliable alternative option for invasive monitoring in patients with refractory focal epilepsy, providing an additional mean for seizure localization and control in a "difficult to localize" subgroup of patients.

Abstract

The study presented in the paper attempts to examine if the alleged effectiveness of the developmental funds - one of the main strategic tools of the EU - can be traced in the typical statistics provided by various authorities and organizations. The study focuses in the case of funds used in Greece during a period of years and it tries to provide insight in the issue, based on statistical and empirical data analysis methodologies. Available information regarding European programmes and corresponding funds is charted and linked to relevant official statistical data provided by Greek national authorities and Eurostat. Selected sectoral and regional actions are analyzed based on an optimal match between funding objectives and official statistics. The results, both statistical and empirical, appear to support the argument: assessment of EU regional policy based on statistics does not seem to be a conclusive method of evaluation. Abstract

[March 2008-Cleveland Case Report]. There is a well-described association between the occurrence of developmental tumors and the presence of cortical dysplasia in the neighboring brain tissue. The main surgical approaches in the treatment of medically refractory epilepsy related to such developmental tumors include a lesionectomy versus a tailored cortical resection, often guided by an invasive evaluation. This case report describes the surgical management of a 26-year-old female with olfactory auras evolving into automotor seizures and convulsions, occurring in the context of a right temporo-parietal developmental lesion. It illustrates the pros and cons of various surgical approaches, and discusses some pathophysiological aspects of developmental tumors, dysplasia and epilepsy. [Published with video sequences].

Abstract

International audience; Restraining excitatory neurotransmission within a seizure focus provides a nondestructive treatment strategy for intractable neocortical epilepsy. Clostridial toxin light chain (LC) inhibits synaptic transmission by digesting a critical vesicle-docking protein, synaptobrevin, without directly altering neuronal health. This study tests the treatment efficacy of adenoviral vector delivered LC (AdLC) on a model of seizures in rats induced by motor cortex penicillin (PCN) injection. LC expression significantly reduced electroencephalogram (EEG) frequency, amplitude, duration, and latency compared to control groups injected with either an adenoviral vector bearing green fluorescent protein (AdGFP) or phosphate buffered solution (PBS). Correspondingly, LC gene expression improved behavioral manifestations including seizure severity and latency. There was no statistical difference in motor function before and after vector administration between treatment and control groups. Histological analysis revealed spatially discrete LC expression with corresponding synaptobrevin depletion in the cortex surrounding the injection site. Thus, vector-mediated LC gene delivery is capable of improving both the EEG and behavioral manifestations of PCN-induced focal neocortical seizures through synaptobrevin depletion.
