Introduction Home-based records (HBRs) are widely used for recording health information including child immunisations. We studied levels and inequalities in HBR ownership in low-income and middle-income countries (LMICs) using data from national surveys conducted since 2010.Methods We used data from national household surveys (Demographic and Health Surveys (DHS) and Multiple Indicator Cluster Surveys (MICS)) from 82 LMICs. 465 060 children aged 6–35 months were classified into four categories: HBR seen by the interviewer; mother/caregiver never had an HBR; mother/caregiver had an HBR that was lost; and reportedly have an HBR that was not seen by the interviewer. Inequalities according to age, sex, household wealth, maternal education, antenatal care and giving birth in an institutional setting were studied, as were associations between HBR ownership and vaccine coverage. Pooled analyses were carried out using country weights based on child populations.Results An HBR was seen for 67.8% (95% CI 67.4% to 68.2%) of the children, 9.2% (95% CI 9.0% to 9.4%) no longer had an HBR, 12.8% (95% CI 12.5% to 13.0%) reportedly had an HBR that was not seen and 10.2% (95% CI 9.9% to 10.5%) had never received one. The lowest percentages of HBRs seen were in Kiribati (22.1%), the Democratic Republic of Congo (24.5%), Central African Republic (24.7%), Chad (27.9%) and Mauritania (35.5%). The proportions of HBRs seen declined with age and were inversely associated with household wealth and maternal schooling. Antenatal care and giving birth in an institutional setting were positively associated with ownership. There were no differences between boys and girls. When an HBR was seen, higher immunisation coverage and lower vaccine dropout rates were observed, but the direction of this association remains unclear.Interpretation HBR coverage levels were remarkably low in many LMICs, particularly among children from the poorest families and those whose mothers had low schooling. Contact with antenatal and delivery care was associated with higher HBR coverage. Interventions are urgently needed to ensure that all children are issued HBRs, and to promote proper storage of such cards by families.

To evaluate bidirectional and longitudinal association between physical activity (PA) and depressive symptoms (DS). This is a cohort study called “Como Vai?” (How are you?), conducted by older population representative sample of the city of Pelotas-Rio Grande do Sul. The analyses used data from the first interview (2014) and follow-up (2019/20). The long version of the International Physical Activity Questionnaire (IPAQ) was applied to assess PA in the leisure, commuting and total domains. The presence of DS was measured using the Geriatric Depression Scale (GDS-10). Linear regression models were used to evaluate bidirectional associations between PA and DS. Analytical sample included 515 individuals with a mean age of 70.7 (± 9.2) years. Each weekly minute of total PA (β: -0.0006; 95% CI: -0.0011; -0.0001) and commuting PA (β: -0.0008; 95% CI: -0.0016; -0.0001) in 2014 predicts worse DS scores in 2019/20. At the same time, DS score in 2014 (β: -9.79; 95% CI: -18.81; -0.76) was a negative predictor of leisure-time PA in the 2019/20 follow-up adjusted for sociodemographic and other health behaviors. However, when adjusted for the respective outcomes at baseline, the associations found were not statistically significant. The present study did not find evidence of bidirectionality between PA and DS and the prospective associations lost statistical significance after adjustment for the respective outcomes at baseline. Other investigations are necessary including more periods of follow-up, objective analyses of PA to complement analyses by domains and, therefore, provide a better understanding of this complex relationship.

The world is not on track to achieve universal access to safely managed water by 2030, and access is substantially lower in rural areas. This Sustainable Development Goal target and many other global indicators rely on the classification of improved water sources for monitoring access. We aimed to investigate contamination in drinking water sources, comparing improved and unimproved sources in urban and rural settings. We used data from Multiple Indicator Cluster Surveys, which tested samples from the household water source and a glass of water for Escherichia coli contamination across 38 countries. Contamination was widespread and alarmingly high in almost all countries, settings, and water sources, with substantial inequalities between and within countries. Water contamination was found in 51.7% of households at the source and 70.8% in the glass of water. Some improved sources (e.g., protected wells and rainwater) were as likely to be contaminated as unimproved sources. Some sources, like piped water, were considerably more likely to be contaminated in rural than urban areas, while no difference was observed for others. Monitoring water contamination along with further investigation in water collection, storage, and source classification is essential and must be expanded to achieve universal access to safely managed water. HIGHLIGHTS Water sources classified as improved – in particular, protected wells and rainwater – had high prevalence of E. coli contamination.; Piped water sources were much more likely to be contaminated in rural areas.; Contamination was more prevalent at the point of use than at the source, but this varied significantly according to the water source.; We provided a list of suggestions to improve monitoring of the Sustainable Development Goal 6.;

ObjectiveThis study aims to test the association of rest-activity rhythm (intradaily variability and interdaily stability) with all-cause mortality in an older adult cohort in Brazil. It also assesses whether the amount of time spent at each intensity level (i.e., physical activity and nocturnal sleep) interferes with this association.MethodsThis cohort study started in 2014 with older adults (≥60 years). We investigated deaths from all causes that occurred until April 2017. Rest-activity rhythm variables were obtained using accelerometry at baseline. Intradaily variability indicates higher rhythm fragmentation, while interdaily stability indicates higher rhythm stability. Cox proportional-hazard models were used to test the associations controlling for confounders.ResultsAmong the 1451 older adults interviewed in 2014, 965 presented valid accelerometry data. During the follow-up period, 80 individuals died. After adjusting the analysis for sociodemographic, smoking, morbidity score, and number of medicines, an increase of one standard deviation in interdaily stability decreased 26% the risk of death. The adjustment for total sleep time and inactivity did not change this association. On the other hand, the association was no longer significant after adjusting for overall physical activity and moderate to vigorous physical activity.ConclusionRest-activity rhythm pattern was not associated with mortality when physical activity was considered, possibly because this pattern could be driven by regular exercise. Promoting physical activity remains a relevant strategy to improve population health.

Abstract Background and Aims This study aims to describe inequalities in health indicators according to gender, area of residence, and socioeconomic position among Brazilian adolescents. Methods Cross‐sectional study using data from a school‐based survey carried out in Brazil in 2019. Twelve health outcomes were evaluated. Dimensions of inequality assessed were gender, area of residence, wealth and subnational region. Results This study comprises a sample of 124,898 adolescents. The most prevalent outcome was physical inactivity (71.9%) followed by thinking life is worthless (52.6%) and bullying (51.8%). Gender inequalities were more marked for physical inactivity and thinking life is worthless with girls presenting a prevalence more than 20 p.p. higher than boys. In zero‐dose HPV, however, the prevalence in girls was 17.7 p.p. lower than in boys. Area of residence and wealth inequalities were smaller than gender disparities. Context presented a relevant role in inequality with analysis stratified by states of the country, revealing high variability in estimates. Conclusions We highlight the need for attention to disparities between subgroups of the adolescent population, especially for gender inequalities that were the most marked for this age group.

Abstract Background Within-country inequalities in birth registration coverage (BRC) have been documented according to wealth, place of residence and other household characteristics. We investigated whether sex of the head of household was associated with BRC. Methods Using data from nationally-representative surveys (Demographic and Health Survey or Multiple Indicator Cluster Survey) from 93 low and middle-income countries (LMICs) carried out in 2010 or later, we developed a typology including three main types of households: male-headed (MHH) and female-led with or without an adult male resident. Using Poisson regression, we compared BRC for children aged less than 12 months living the three types of households within each country, and then pooled results for all countries. Analyses were also adjusted for household wealth quintiles, maternal education and urban-rural residence. Results BRC ranged from 2.2% Ethiopia to 100% in Thailand (median 79%) while the proportion of MHH ranged from 52.1% in Ukraine to 98.3% in Afghanistan (median 72.9%). In most countries the proportion of poor families was highest in FHH (no male) and lowest in FHH (any male), with MHH occupying an intermediate position. Of the 93 countries, in the adjusted analyses, FHH (no male) had significantly higher BRC than MHH in 13 countries, while in eight countries the opposite trend was observed. The pooled analyses showed t BRC ratios of 1.01 (95% CI: 1.00; 1.01) for FHH (any male) relative to MHH, and also 1.01 (95% CI: 1.00; 1.01) for FHH (no male) relative to MHH. These analyses also showed a high degree of heterogeneity among countries. Conclusion Sex of the head of household was not consistently associated with BRC in the pooled analyses but noteworthy differences in different directions were found in specific countries. Formal and informal benefits to FHH (no male), as well as women’s ability to allocate household resources to their children in FHH, may explain why this vulnerable group has managed to offset a potential disadvantage to their children.

Background: Regular physical activity (PA) is an important behavior in improving sleep health. However, the short-term effects of PA on sleep are still controversial. This study aimed to verify the effect of different intensities of PA practiced in different periods of the day on the subsequent sleep night in a population-based cohort of young adults. Methods: : Prospective analyses were conducted for PA performed during the day and its effect on the following sleep night using data from the 22-year follow-up of the 1993 Pelotas Birth Cohort in Brazil (mean age of participants = 22.6 years). Wrist-worn accelerometry was used to measure both PA and sleep parameters. Regarding intensity, we analyzed the sleep effect of light PA (LPA), moderate PA, and vigorous PA, stratified by sex. Sleep variables were sleep time window (STW; the difference between sleep onset and sleep end), total sleep time (TST; the sum of minutes classified as sleep in STW), and sleep percent (SP; SP = (TST/STW); expressed in percentage). We performed generalized estimating equations using Stata software. Results: : The means of STW, TST, and SP were 443.6 min/day, 371.1 min/day, and 84%, respectively. Time spent in moderate PA and vigorous PA in the morning and afternoon was not associated with sleep variables. Among men, 10 min/day of morning LPA increased TST by 2.56 min/day. Among women, 10 min/day of morning LPA increased SP by 0.15 percentage points. Afternoon LPA also increased SP by 0.09 percentage points for women. Night PA seems to have an inverse effect on sleep variables for any intensity and both sexes. Conclusion: : The effect of PA on sleep health is intrinsically related to the period of the day in which it is performed. The effect magnitude is different between sexes. For better sleep health, it is preferable that PA be performed during the day.

Purpose: The aims of this study were to (a) describe gender inequalities in physical activity (PA) among adolescents from Global South countries, and (b) investigate the relationship between gender inequalities in PA and contextual factors, such as geographic region, human development index, gender inequality index, and unemployment rates. Methods: We analyzed cross-sectional data from the Global School-Based Student Health Survey conducted in Global South countries between 2010 and 2020 among 13- to 17-year-old adolescents. Country-context variables were retrieved from secondary data sources (World Health Organization, World Bank, and Human Development Reports). PA was assessed by a self-administered questionnaire querying the number of days in the past week in which participants were physically active for a total of at least 60 min. PA absolute gender inequalities were evaluated by the differences in the prevalence between boys and girls, 95% confidence intervals (95%CIs) were estimated using the bootstrap method. Relative inequalities were obtained through Poisson regression. Meta-analyses with random effects were used to calculate pooled estimates of absolute and relative inequalities. Results: Based on 64 Global South countries/surveys, the prevalence of PA was 6.7 percentage points (p.p.) higher in boys than in girls, ranging from 0.5 p.p. in Afghanistan to 15.6 p.p. in Laos (I2= 85.1%). The pooled ratio for all countries showed that boys presented a PA prevalence 1.58 times higher than girls (95%CI: 1.47–1.70) on average. The highest absolute and relative inequalities were observed in high income countries. Countries with higher Human Development Index rankings and lower Gender Inequality Index rankings also presented greater gender differences. Conclusion: Given that girls are overall less active than boys across the globe, the findings of this study reinforce that macro- and micro-level changes should be actively sought if we aim to increase population levels of PA in adolescents and promote equity in PA.

ObjectiveTo evaluate changes in socioeconomic inequalities in food consumption in Brazil over a 10-year period.MethodsData on 24-h recalls of adults (aged 20 years or more) from the 2008/9 (n = 26,327) and 2017/8 (n = 37,689). Brazilian Dietary Survey were analyzed. We used the Nova classification system to group food items and estimate the percentage of total energy from ultra-processed foods and plant-based natural or minimally processed foods. For sex and area of residence, we calculated the percentage points (p.p.) difference between the estimates for women and men, and rural and urban populations. Negative values indicate higher consumption among men or urban residents, positive values indicate higher consumption among women or rural residents, and zero indicates equality. For education and wealth levels we calculated the slope index of inequality (SII). The SII varies from −100 to 100, with positive values indicating higher consumption among more educated or wealthiest groups, negative values indicating higher consumption among less educated or poorest groups, and zero equality.ResultsOver the period, we observed a reduction in the percentage of total energy from plant-based natural/minimally processed foods from 13.0 to 12.2% and an increase in that of ultra-processed foods from 17.0 to 18.3%. The urban population and those in the wealthier and more educated groups presented higher consumption of ultra-processed foods and lower consumption of plant-based natural/minimally processed foods in both survey years. Over the 10-year period, there was an overall reduction of the socioeconomic inequalities, mainly explained by the greater increase in ultra-processed food consumption by the rural population and those from the poorest and less educated groups (difference for area −7.2 p.p. in 2008/9 and −5.9 p.p. in 2017/8; SII for education 17.7 p.p. in 2008/9 and 13.8 p.p. in 2017/8; SII for wealth 17.0 p.p. in 2008/9 and 11.2 p.p. in 2017/8).ConclusionSocioeconomic inequalities in food consumption decreased in Brazil, but it may lead to the overall deterioration of the dietary quality of the more vulnerable groups.

Purpose: The aim of this study was to systematically review the literature about the relationship between sleep duration and cognitive, academic and socioeconomic outcomes. Methods: We performed a systematic search in PubMed, Psyinfo, Scopus, Web of Science, ERIC, and Socindex up to June 2022, independently by two researchers. Original studies testing the association between sleep duration (as exposure) and academic, cognitive and socioeconomic variables (as outcomes) among all age groups in population-based studies were included. We excluded studies assessing participants with specific diseases or specific populations. Quality assessment was evaluated considering three domains: internal validity, study design, and adjustments. Results: After the study selection process, a total of 56 manuscripts were selected. Most studies were from high-income countries with a large variability of instruments and cutoff points to measure sleep duration and outcomes. Cognitive outcomes were evaluated in 35 manuscripts, academic outcomes in 22, and socioeconomic outcomes in one. Long sleep seems to be associated with poor cognitive outcomes in older adults. In contrast, short sleep duration seems to be associated with poor cognitive and school outcomes among children and adolescents. Studies evaluating cognition in children and adolescents and academic outcomes presented lower scores in the quality assessment. Conclusion: More well designed and well-adjusted studies evaluating cognitive and academic outcomes in children, adolescents, and mainly, in adult population are necessary. Furthermore, studies from low- and middle- income countries, evaluating sleep duration and exploring the relationship between sleep duration as exposure variable and human capital characteristics as outcomes variables should be included in the research agenda.

BackgroundThe literature on the association between religion and immunization coverage is scant, mostly consisting of single-country studies. Analyses in low and middle-income countries (LMICs) to assess whether the proportions of zero-dose children vary according to religion remains necessary to better understand non-socioeconomic immunization barriers and to inform interventions that target zero-dose children.MethodsWe included 66 LMICs with standardized national surveys carried out since 2010, with information on religion and vaccination. The proportion of children who failed to receive any doses of a diphtheria-pertussis-tetanus (DPT) containing vaccine – a proxy for no access to routine vaccination or “zero-dose” status – was the outcome. Differences among religious groups were assessed using a test for heterogeneity. Additional analyses were performed controlling for the fixed effect of country, household wealth, maternal education, and urban-rural residence to assess associations between religion and immunization.FindingsIn 27 countries there was significant heterogeneity in no-DPT prevalence according to religion. Pooled analyses adjusted for wealth, maternal education, and area of residence showed that Muslim children had 76% higher no-DPT prevalence than Christian children. Children from the majority religion in each country tended to have lower no-DPT prevalence than the rest of the population except in Muslim-majority countries.InterpretationAnalyses of gaps in coverage according to religion are relevant to renewing efforts to reach groups that are being left behind, with an important role in the reduction of zero-dose children.

Introduction: This study aims to compare differences regarding physical activity (PA) indicators and TV viewing according to location of residence and sociodemographic variables in Brazil. Methods: This cross-sectional study with individuals aged 18 years or older using data from the Brazilian National Health Survey was carried out in 2013. PA (total and domains) and TV viewing (≥3 hours) were measured by self-report. Stratifiers were location of residence, sex, age, wealth quintiles and regions of the country. Results: This study included 60 202 individuals (49 245 from urban areas). In general, total PA was not different according to location of residence (urban 59.4%; rural 57.4%). Higher differences were found for leisure-time PA (urban 24.1%; rural 13.9%), work-based PA (urban 12.9%; rural 21.1%) and TV viewing of more than 3 hours (urban 30.1%; rural 21.4%). The differences between urban and rural areas were enhanced according to sex, age and wealth quintiles. Conclusion: The differences between urban and rural areas vary according to PA domains and population groups. To explore specific domains of PA in each location of residence may improve the understanding of the analyzed outcomes in different communities and consequently guide future interventions.

Background: Women’s empowerment may play a role in shaping attitudes towards female genital mutilation/cutting (FGM/C) practices. We aimed to investigate how empowerment may affect women’s intention to perpetuate FGM/C and the practice of FGM/C on their daughters in African countries.Materials and methods: We used data from Demographic and Health Surveys carried out from 2010 to 2018. The countries included in our study were Benin, Burkina Faso, Chad, Côte d´Ivoire, Ethiopia, Guinea, Kenya, Mali, Nigeria, Senegal, Tanzania, and Togo. This study included 77,191 women aged 15–49 years with at least one daughter between zero and 14 years of age. The proportion of women who reported having at least one daughter who had undergone FGM/C as well as the mother’s opinion towards FGM/C continuation were stratified by empowerment levels in three different domains (decision-making, attitude to violence, and social independence) for each country. We also performed double stratification to investigate how the interaction between both indicators would affect daughter’s FGM/C.Results: The prevalence of women who had at least one daughter who had undergone FGM/C was consistently higher among low empowered women. Tanzania, Benin, and Togo were exceptions for which no differences in having at least one daughter subjected to FGM/C was found for any of the three domains of women’s empowerment. In most countries, the double stratification pointed to a lower proportion of daughters’ FGM/C among women who reported being opposed to the continuation of FGM/C and had a high empowerment level while a higher proportion was observed among women who reported being in favor of the continuation of FGM/C and had a low empowerment level. This pattern was particularly evident for the social independence domain of empowerment. In a few countries, however, a higher empowerment level coupled to a favorable opinion towards FGM/C was related to a higher proportion of daughters’ FGM/C.Conclusion: Women’s empowerment and opinion towards FGM/C seems to be important factors related to the practice of FGM/C in daughters. Strategies to improve women’s empowerment combined with shifts in the wider norms that support FGM/C may be important for achieving significant reductions in the practice.

Studies of inequalities in child health have given limited attention to household structure and headship. The few existing reports on child outcomes in male and female-headed households have produced inconsistent results. The aim of our analyses was to provide a global view of the influence of sex of the household head on child health in cross-sectional surveys from up to 95 LMICs. Studied outcomes were full immunization coverage in children aged 12–23 months and stunting prevalence in under-five children. We analyzed the most recent nationally-representative surveys for each country (since 2010) with available data. After initial exploratory analyses, we focused on three types of households: a) male-headed household (MHH) comprised 73.1% of all households in the pooled analyses; b) female Headed Household (FHH) with at least one adult male represented 9.8% of households; and c) FHH without an adult male accounted for 15.0% of households. Our analyses also included the following covariates: wealth index, education of the child’s mother and urban/rural residence. Meta-analytic approaches were used to calculate pooled effects across the countries with MHH as the reference category. Regarding full immunization, the pooled prevalence ratio for FHH (any male) was 0.99 (0.97; 1.01) and that for FHH (no male) was 0.99 (0.97; 1.02). For stunting prevalence, the pooled prevalence ratio for FHH (any male) was 1.00 (0.98; 1.02) and for FHH (no male) was 1.00 (0.98; 1.02). Adjustment for covariates did not lead to any noteworthy change in the results. No particular patterns were found among different world regions. A few countries presented significant inequalities with different directions of association, indicating the diversity of FHH and how complex the meaning and measurement of household headship may be. Further research is warranted to understand context, examine mediating factors, and exploring alternative definitions of household headship in countries with some association.

The concept of multiple deprivation recognizes that the same individuals, households, and communities are often exposed to several forms of scarcity. We assessed whether lack of immunization is also associated with nutritional, environmental, and educational outcomes. We analyzed data from nationally representative surveys from 80 low- and middle-income countries with information on no-DPT (children aged 12–23 months without any doses of a diphtheria, pertussis and tetanus containing vaccine), stunting, wasting, maternal education and use of contraception, improved water and sanitation, and long-lasting insecticidal nets. Analyses of how these characteristics overlap were performed at individual and ecological levels. Principal component analyses (PCA) provided additional information on indicator clustering. In virtually all analyses, no-DPT children were significantly more likely to be exposed to the other markers for deprivation. The strongest, most consistent associations were found with maternal education, water, and sanitation, while the weakest associations were found for wasting and bed nets. No-DPT prevalence reached 46.1% in the most deprived quintile from first PCA component derived from deprivation indicators. All children were immunized in the two least deprived quintiles of the component. Our analyses provide strong support for the hypothesis that unimmunized children are also affected by other forms of deprivation.

Abstract: The aim of the study is: (a) investigate the racial inequalities as one specific dimension that affects dental pain in Brazilian adolescents; and (b) investigate the regional variations of dental pain. This cross-sectional study used data from Brazilian National Survey of School Health (PeNSE), carried out with adolescents in 2009, 2012 and 2015. Dental pain was evaluated through the question: “Did you have dental pain in the last six months?”. The main exposures were race and Brazilian regions, used to evaluate inequalities related to the outcome. Sex, age, school type and maternal education were used as covariables. The statistical significance of the trends in dental pain was tested using linear regression. The analysis was conducted in Stata 13.0 statistical package using the svy command. The standard prevalence of dental pain was 18.8%, 21.1% and 23.7%, showing an increasing trend over time (p < 0.001). We observed absolute inequalities in dental pain related to race and regions. A higher prevalence was found in non-white girls of public schools and in the Northern Region. The indexes of inequalities increased in the group of black girls, related to an increase of dental pain predominantly in girls whose mothers had lower educational level. It was observed that the prevalence of dental pain in Brazilian adolescents increased over time as well as its inequalities, which remained in marginalized populations and linked to Brazilian regions.

Estudos sobre desigualdades na prática de esporte e atividades físicas são uma importante ferramenta para embasar políticas públicas. Assim como abordagens de nível nacional, análises específicas de acordo com regiões se fazem necessárias. O objetivo deste estudo foi verificar as desigualdades em termos de sexo, cor da pele, área de residência e escolaridade na prática de esporte ou atividades físicas de acordo com as regiões do Brasil. Foram analisados os dados da investigação suplementar da Pesquisa Nacional por Amostra de Domicílios de 2015, uma amostra representativa da população de adultos brasileiros. As análises contemplaram a prática de esporte e atividades físicas durante o ano anterior à entrevista e análises simples e complexas de desigualdade foram realizadas de acordo com cada região do país. Homens, pessoas de cor de pele branca, moradores da zona urbana e com maior escolaridade apresentaram maior prática em todas as regiões. Em termos de magnitude das desigualdades, as regiões que apresentaram maiores desigualdades foram a norte (prática entre homens 12,9 pontos percentuais – p.p., maior que entre mulheres) e a sul (41,1 p.p. de diferença entre os grupos extremos de escolaridade e 13,4 p.p. entre zona urbana e rural). Pessoas de cor da pele branca apresentaram 4,9 p.p. a mais de prevalência em relação às pretas ou pardas, sem diferenças marcantes entre as regiões. Os resultados identificaram a presença de desigualdades sistematicamente na direção de grupos privilegiados social e economicamente em nossa sociedade em todas as regiões do país, embora magnitudes diferentes sejam observadas regionalmente.

Resumo Estudo de tendência temporal com o objetivo de avaliar as desigualdades no tabagismo e no consumo abusivo de álcool, considerando a escolaridade como proxy de nível socioeconômico, de acordo com sexo e regiões brasileiras. Foram utilizados dados do Vigilância de Fatores de Risco e Proteção para Doenças Crônicas por Inquérito Telefônico (Vigitel) coletados entre 2006 e 2017. Os desfechos foram o consumo abusivo de bebidas alcoólicas e o fumo. As desigualdades foram avaliadas com base na escolaridade, com dupla estratificação por sexo e regiões brasileiras. Para avaliar a tendência das prevalências dos desfechos e suas desigualdades foi usada a regressão linear ponderada pelos quadrados mínimos da variância. A prevalência de consumo abusivo de álcool aumentou em mulheres, de 7,8% (2006) para 12,2% (2017), e foi maior sempre entre os mais escolarizados. A prevalência de tabagismo diminuiu para homens e mulheres e foi maior sempre entre os menos escolarizados. A desigualdade no consumo de álcool entre os grupos de escolaridade parece ter aumentado ao longo do tempo, com maior magnitude entre as mulheres, mas a desigualdade no tabagismo parece estar diminuindo. Em algumas regiões do país, as medidas de desigualdade sugerem estabilidade e em outras um aumento da desigualdade para o consumo de álcool (Sudeste, Sul e Centro-oeste, entre as mulheres) e diminuição para o tabagismo (todas as regiões, exceto Sudeste, entre os homens; Nordeste e Centro-oeste, entre as mulheres). Apesar dos avanços na redução do tabagismo, as desigualdades na escolaridade persistem e estão presentes também em relação ao álcool. Os desfechos têm comportamentos opostos, portanto, estratégias devem ser focadas em cada um dos problemas, a fim de reduzir as desigualdades existentes.

Gender-related barriers to immunization are key targets to improve immunization coverage and equity. We used individual-level demographic and health survey data from 52 low- and middle-income countries to examine the relationship between women’s social independence (measured by the Survey-based Women’s emPowERment (SWPER) Global Index) and childhood immunization. The primary outcome was receipt of three doses of the diphtheria-tetanus-pertussis vaccine (DTP3) among children aged 12–35 months; we secondarily examined failure to receive any doses of DTP-containing vaccines. We summarized immunization coverage indicators by social independence tertile and estimated crude and adjusted summary measures of absolute and relative inequality. We conducted all analyses at the country level using individual data; median results across the 52 examined countries are also presented. In crude comparisons, median DTP3 coverage was 12.3 (95% CI 7.9; 16.3) percentage points higher among children of women with the highest social independence compared with children of women with the lowest. Thirty countries (58%) had a difference in coverage between those with the highest and lowest social independence of at least 10 percentage points. In adjusted models, the median coverage was 7.4 (95% CI 5.0; 9.1) percentage points higher among children of women with the highest social independence. Most countries (41, 79%) had statistically significant relative inequality in DTP3 coverage by social independence. The findings suggest that greater social independence for women was associated with better childhood immunization outcomes, adding evidence in support of gender-transformative strategies to reduce childhood immunization inequities.

This study aims to describe objectively measured physical activity (PA) in different periods of the day in young adults according to sex, socioeconomic position and during weekdays and weekends. This is a cross-sectional analysis carried out with the participants of the 1993 Pelotas Birth Cohort at 22 years. PA was assessed by triaxial accelerometer. Descriptive analyses were performed presenting the time spent on light PA (LPA) and bouted moderate to vigorous PA (MVPA) in different periods of the day (morning - 6am to 11:59 am, afternoon – 12pm to 7:59 pm and night – 8pm to 11:59pm). The present study included 2,766 individuals (48.2% male and 51.8% female). LPA was higher among women, while bouted MVPA levels were higher among men. The median of PA was higher on weekdays compared to weekends for all intensities. The bouted MVPA medians in the morning and at night were zero minutes for all days and both sexes. The richest group presented a higher percentage of individuals with zero minutes. PA may vary according to different periods of the day and intensity. The absence of PA practice was markedly influenced by sex and socioeconomic position.

Objective. To measure the prevalence of various care services offered to the elderly with diabetes mellitus in Brazil, and to assess the social inequalities in these services. Methods. This cross-sectional, population-based study was carried out in 2013. The care services offered were evaluated in terms of the following eight indicators: recommendations to lower carbohydrates, to measure blood glucose, and to examine the feet; requests made for blood tests, for glycated hemoglobin tests, and for glycemic curve tests; and whether service users had had their eyes or feet examined in the previous year. We used the slope index of inequality and the concentration index to assess the inequalities among wealth quintiles. Results. A total of 1 685 elderly persons with diabetes were evaluated. Overall, 41.7% of them had had their eyes examined in the preceding year, 35.4% had had their feet examined in the preceding year, and 10.9% had been offered all eight of the care services. The largest absolute differences (in percentage points) between the first (poorest) and fifth (richest) wealth quintiles in terms of the care services that were offered to the users were for: a recommendation to measure blood glucose (25.8), a glycated hemoglobin test request (27.4), a glycemic curve test request (31.9), having the eyes examined in the preceding year (29.3), and having the feet examined in the preceding year (27.0). Conclusion. There were notable inequalities in the prevalences of the care services. In the future, measurement of blood glucose and examination of the feet should be emphasized, ­especially for elderly persons in a lower socioeconomic level.

Abstract Background Physical exercise interventions have been extensively advocated for the treatment of obesity; however, clinical trials evaluating the effectiveness of exercise interventions on weight control show controversial results. Compensatory mechanisms through a decrease in energy expenditure and/or an increase in caloric consumption is a possible explanation. Several physiological mechanisms involved in the energy balance could explain compensatory mechanisms, but the influences of physical exercise on these adjustments are still unclear. Therefore, the present trial aims to evaluate the effects of exercise on non-exercise physical activity energy expenditure, energy intake and appetite sensations among active overweight/obese adults, as well as, to investigate hormonal changes associated with physical exercise. Methods This study is a randomized controlled trial with parallel, three-group experimental arms. Eighty-one overweight/obese adults will be randomly allocated (1:1:1 ratio) to a vigorous exercise group, moderate exercise group or control group. The trial will be conducted at a military institution and the intervention groups will be submitted to exercise sessions in the evening, three times a week for 65 min, during a 2-week period. The primary outcome will be total spontaneous physical activity energy expenditure during a 2-week period. Secondary outcomes will be caloric intake, appetite sensations and laboratorial biomarkers. Intention-to-treat analysis will be performed using linear mixed-effects models to evaluate the effect of treatment-by-time interaction on primary and secondary outcomes. PurposeThe present study aims to estimate the minimum number of accelerometer measurement days needed to estimate habitual physical activity (PA) among 6- (2010), 18- (2011) and 30- (2012) year-old participants, belonging to three population-based Brazilian birth cohorts.MethodPA was assessed by triaxial wrist-worn GENEActiv accelerometers and the present analysis is restricted to participants with at least 6 consecutive days of measurement. Accelerometer raw data were analyzed with R-package GGIR. Description of PA measures (overall PA, moderate-to-vigorous PA (MVPA), light PA (LPA)) on weekdays and weekend days were conducted, and statistical differences were tested with chi-squared and Kruskal-Wallis tests. Spearman Brown Formulae was applied to test reliability of different number of days of accelerometer use.ResultsDifferences between week and weekend days regarding LPA, MVPA and overall PA, were only observed among 30-year-olds. Higher levels of MVPA (p = 0.006) and overall PA (p0.70, two and three days of measurement were needed in adults and children, respectively. For LPA, a reliability coefficient >0.70 was achieved with five days in 6-year-old children, three days in 18-year-old young adults, and four days in 30-year-old adults. Considering MVPA, four days would be necessary to represent a week of measurement among all cohort groups.ConclusionOur results show that four and five measurement days are needed to estimate all habitual PA constructs, for children and adults, respectively. Also, among 30-year-old adults, it is important to make efforts towards weekend days measurement.

O objetivo do estudo foi verificar a distribuição dos diferentes tipos de práticas entre os praticantes de atividade física de uma amostra de adultos de acordo com características sociodemográficas. Estudo transversal utilizando dados da Pesquisa Nacional de Saúde (PNS) de 2013, com adultos de 18 anos ou mais, praticantes de atividade física (n = 17.350). As variáveis independentes foram sexo, faixa etária, escolaridade e regiões geográficas. As variáveis dependentes foram os tipos de atividade física mais praticadas na população. Para apresentar as frequências de prática de prática segundo região, escolaridade e idade as atividades as atividades foram divididas em três grupos: caminhada, exercício e esportes. Os dados foram analisados no STATA 12.1. Nos homens, as atividades mais comuns foram futebol (39,1%), caminhada (22,9%) e musculação (15,3%). Nas mulheres, foram a caminhada (48,9%), ginástica (19,2%) e musculação (14,2%). Nos homens, a prática de caminhada foi maior nos mais velhos, sem instrução e da região Centro Oeste (p < 0,001). Quanto a prática de exercícios, nos homens, este hábito foi mais comum entre os mais jovens, com maior escolaridade, e da região Sudeste (p < 0,001). Em relação aos esportes, a prática foi mais comum nos mais jovens, nos com maior escolaridade e da região Norte (p < 0,001), tanto nos homens quanto nas mulheres. A prática esportiva foi menos frequente nas mulheres. Os tipos de atividade têm grande influência da idade, escolaridade dos indivíduos e região do país. Estes aspectos devem ser levados em conta do planejamento de intervenções.

The aim of this study was to systematically examine the literature on physical activity and sleep in non-clinical and population-based settings. The inclusion criteria were original studies testing the association between physical activity (as exposure) and sleep (as outcome) in representative samples of the general population, workers, or undergraduate students. Sleep health included sleep duration, sleep quality and insomnia. Studies evaluating samples including only individuals with some disease or a health condition were excluded. A search was performed in the PubMed, Scopus, Lilacs, CINAHL, and SPORTdiscus databases in March 2018. Data extraction was performed using the following items: year, author, country, population, age group, sample size, study design, sleep measurement/definition, physical activity measurement/definition, adjustment and main results. A total of 57 studies were selected, which markedly used heterogeneous instruments to measure physical activity and sleep. The majority were conducted in high-income countries and with cross-sectional design. Physical activity was associated with lower odds of insomnia (observed in 10 of 17 studies), poor sleep quality (observed in 12 of 19 studies) and long sleep duration (observed in 7 of 11 studies). The results about short sleep or continuous sleep duration remain unclear. Physical activity seems to be associated with sleep quality and insomnia, especially among adult and elderly populations in which these outcomes are more usually measured. The short- and long-term effects of physical activity intensities and dose-response on sleep should be better evaluated.

Abstract: This study aims to describe the prevalence of sleep disturbances and daytime fatigue and their association with socio-demographic and behavioral factors. Data from the Brazilian National Health Survey conducted in 2013 with 60,202 adults (≥ 18 years old) were used. The outcomes evaluated were self-reported sleep disturbances and daytime fatigue in the last two weeks. Sleep disturbance was defined as the presence of difficulty to fall asleep, frequently waking up during the night or sleeping more than usual; daytime fatigue was defined as the presence of not feeling rested and motivated during the day, feeling tired and lacking energy. Sociodemographic, lifestyle and chronic health aspects were explored as exposures for both outcomes. Prevalence of sleep disturbances and daytime fatigue were 14.9% (14.4-15.4) and 11.9% (11.4-12.3), respectively. Both outcomes were more common in women, older people, people with no formal education, smokers and among physically inactive individuals. The association with education was inverse (the highest the level of education the lower the prevalence ratio - PR - of sleep disturbances and daytime fatigue; adjusted p-value for trend < 0.001). Prevalence of sleep disturbances combined with daytime fatigue was 6.7% (6.4-7.1) and was about 6 times higher among those with three or more chronic health disturbances (PR = 6.2; 95%CI: 5.3-7.2). Strategies to decrease the prevalence of sleep disturbances and daytime fatigue should be encouraged and focused on chronically ill individuals that share other modifiable risk factors.

Lower socioeconomic level is positively related to multimorbidity and it is possible that the clustering of health conditions carries the same association. The aim of this study was to identify prevalence of multimorbidity and clusters of health conditions among elderly, as well the underlying socioeconomic inequalities. This was a cross-sectional population-based study carried out with 60-year-old individuals. Multimorbidity was defined as the presence of 2+, 3+, 4+ or 5+ health conditions in the same individual. Schooling levels and the National Economic Index were used to investigate inequalities in the prevalence of multimorbidities among elderly. Slope and concentration indexes of inequality were used to evaluate absolute and relative differences. A factorial analysis was performed to identify disease clusters. In every ten older adults, about nine, eight, seven and six presented, respectvely, 2+, 3+, 4+ and 5+ health conditions. Three clusters of health conditions were found, involving musculoskeletal/mental/functional disorders, cardiometabolic, and respiratory factors. Higher inequalities were found the higher amount of health conditions (5+), when considering economic level, and for 3+, 4+ and 5+, when considering educational level. These findings show high multimorbidity prevalence among elderly, highlighting the persistence of health inequalities in Southern Brazil. Strategies by the health services need to focus on elderly at lower socioeconomic levels.

ABSTRACT OBJECTIVE To estimate the average of a sleep problems score and their associated factors in adults living in rural areas. METHODS A population-based cross-sectional study with individuals from the rural area of the city of Pelotas, Southern Brazil. Twenty-four of the 50 census tracts that make up the eight rural districts of the city were randomly selected. Individuals of 18 years of age or older residing in the households chosen were considered eligible. Sleep problems were measured using the Mini Sleep Questionnaire, which ranged from 10 to 70 points and the higher the score, the greater the sleep problems. The independent variables evaluated included socioeconomic, demographic, behavioral and health characteristics. In the analysis, linear regression was used, obeying a previous hierarchical model. RESULTS The sample consisted of 1,421 individuals. The average obtained for sleep problems was 29.4 points (95%CI 28.7–30.1). After adjusted analysis, the following variables were associated factors for greater sleep problems: female sex, age greater than or equal to 40 years, lower schooling level, depressive symptoms, pesticide poisoning, and poorer quality of life. CONCLUSIONS The Mini Sleep Questionnaire average in this study was 4.4 points above the cut-off point that established sleep problems. The total points found in the score was high for the rural population. Strategies to improve sleep for these individuals should be focused on higher-risk groups such as women and the elderly and those with pesticide poisoning.

O objetivo foi identificar desigualdades na prática de atividade física de lazer e deslocamento ativo para escola em adolescentes brasileiros, bem como suas tendências de acordo com o sexo, tipo de escola, escolaridade materna e regiões geográficas de 2009 a 2015. Estudo descritivo baseado em dados da Pesquisa Nacional de Saúde do Escolar (PeNSE) de 2009, 2012 e 2015. Foram considerados ativos no lazer aqueles que acumularam, no mínimo, 60 minutos por dia, em cinco ou mais dias da semana anteriores à entrevista. Para deslocamento ativo para escola, foi avaliado o transporte a pé ou de bicicleta para a escola na semana anterior à entrevista. Os desfechos foram estratificados pelo sexo, tipo de escola, escolaridade materna e regiões geográficas. As desigualdades foram avaliadas por meio de diferenças e razões entre as estimativas, bem como por índices sumários de desigualdade. Foram incluídos na PeNSE 2009, 2012 e 2015, 61.301, 61.145 e 51.192 escolares, respectivamente. A prevalência de atividade física de lazer foi 13,8% em 2009, 15,9% em 2012 e 14,7% em 2015; já para o deslocamento ativo para escola, foi 70,6%, 61,7%, 66,7%, respectivamente. Meninos apresentaram uma prevalência de 10 pontos percentuais (p.p.) maior de atividade física de lazer e cerca de 5p.p. no deslocamento ativo para escola do que as meninas. Escolares filhos de mães com maior escolaridade apresentaram, em média, uma prevalência de atividade física de lazer 10p.p. maior do que seu grupo extremo de comparação e cerca de 30p.p. menor com relação ao deslocamento ativo para escola. As desigualdades observadas permaneceram constantes ao longo do período avaliado. Foram identificadas desigualdades socioeconômicas e entre os sexos, que se mantiveram constantes ao longo do período analisado e que foram específicas para cada domínio de atividade física.

Estudo transversal com o objetivo de investigar a associação entre comportamento sedentário e consumo de alimentos ultraprocessados (AUP) em adolescentes brasileiros. Foram utilizados dados da Pesquisa Nacional de Saúde do Escolar (PeNSE) realizada em 2015. O consumo diário de pelo menos um grupo de AUP representou o desfecho, e a exposição principal foi o tempo diário de comportamento sedentário (horas em atividades sentado, excluído o tempo dispendido na escola). Foram calculadas prevalências, razões de prevalências e intervalos de 95% de confiança (IC95%). As análises foram ajustadas para sexo, idade, cor da pele, escolaridade materna, índice de bens, região geográfica e dependência administrativa da escola. Cerca de 40% dos escolares reportaram consumo diário de pelo menos um grupo de AUP (39,7%; IC95%: 39,2-40,3) e 68,1% (IC95%: 67,7-68,7) referiram > 2 horas/dia de comportamento sedentário. Entre os escolares com comportamento sedentário > 2 horas/dia, a prevalência de consumo diário de AUP foi de 42,8% (IC95%: 42,1-43,6%), maior do que entre os sem comportamento sedentário (29,8%; IC95%: 29,0-30,5%). Quanto maior o tempo de comportamento sedentário, maior a prevalência de consumo de AUP (valor de p para tendência linear < 0,001). Estratégias que promovam a alimentação saudável e a diminuição de comportamentos sedentários, bem como regulamentações da publicidade de AUP, tornam-se necessárias a fim de evitar que estilos de vida não saudáveis perdurem à idade adulta.

AIM:To evaluate the relationship between leisure-time activities at 11 years old and the incidence of school failures from 11 to 15 years in adolescents. METHODS:The sample comprised 4,090 adolescents from the 1993 Pelotas Birth Cohort, Brazil. The outcome was measured as the number of school failures from 11 to 15 years, based on reported information from cohort participants and their parents. The exposures were collected at 11 years old, as follows: reading; meeting friends; talking to parents; and dating. RESULTS:In the group from 11 to 15 years old, 53.3% failed at school at least once. Meeting friends 4-7 times/week (RR = 1.15) and dating 1-3 times/week (RR = 1.22) were associated with high risk for school failure. Reading showed an inverse relationship with school failures (1-3 times/week RR = 0.83; 4-7 times/week RR = 0.71). Reading at least once a week could prevent around 16% of school failures. CONCLUSION:The context in which adolescents are inserted plays a relevant role in school performance. Understanding these factors may help to propose actions to reduce school failure rates even further.

Objetivos: Descrever a cobertura da vacina meningocócica C em crianças menores de um ano de idade, em 2012, nos estados e regiões do Brasil, e correlacionar com variáveis socioeconômicas e cobertura da Estratégia de Saúde da Família. Métodos: Estudo ecológico. A cobertura vacinal foi calculada utilizando o número de doses da vacina em 2012, disponível no Sistema de Informação do Programa Nacional de Imunizações, como numerador, e população residente menor de um ano, também em 2012, como denominador. Ainda, foi realizada uma correlação de Pearson (r) entre a cobertura vacinal e as variáveis socioeconômicas. Resultados: As regiões Norte e Nordeste não atingiram a recomendação de 95% para cobertura. A cobertura vacinal apresentou correlação positiva moderada com renda média per capita (r=0,6) e negativa e moderada com o coeficiente de Gini (r=-0,5). Conclusão: São necessárias ações educativas e políticas para aumentar a cobertura vacinal com objetivo de reduzir a incidência da doença meningocócica, além de estudos com delineamentos mais robustos.

In epidemiological studies, measures of body fat generally are obtained through anthropometric indices such as the body mass index (BMI), waist (WC), and hip circumferences (HC). Such indices, however, can only provide estimates of a person's true body fat content, overall or by adipose compartment, and may have limited accuracy, especially for the visceral adipose compartment (VAT).To determine the extent to which different body adipose tissue compartments are adequately predicted by anthropometry, and to identify anthropometric measures alone, or in combination to predict overall adiposity and specific adipose tissue compartments, independently of age and body size (height).In a sub-study of 1,192 participants of the German EPIC (European Prospective Investigation into Cancer and Nutrition) cohorts, whole-body MRI was performed to determine adipose and muscle tissue compartments. Additional anthropometric measurements of BMI, WC and HC were taken.After adjusting for age and height, BMI, WC and HC were better predictors of total body volume (TBV), total adipose tissue (TAT) and subcutaneous adipose tissue (SAT) than for VAT, coronary adipose tissue (CAT) and skeletal muscle tissue (SMT). In both sexes, BMI was the best predictor for TBV (men: r = 0.72 [0.68-0.76], women: r = 0.80 [0.77-0.83]) and SMT (men: r = 0.52 [0.45-0.57], women: r = 0.48 [0.41-0.54]). WC was the best predictor variable for TAT (r = 0.48 [0.41-0.54]), VAT (r = 0.44 [0.37-0.50]) and CAT (r = 0.34 [0.26-0.41]) (men), and for VAT (r = 0.42 [0.35-0.49]) and CAT (r = 0.29 [0.22-0.37]) (women). BMI was the best predictor for TAT (r = 0.49 [0.43-0.55]) (women). HC was the best predictor for SAT (men (r = 0.39 [0.32-0.45]) and women (r = 0.52 [0.46-0.58])).Especially the volumes of internal body fat compartments are poorly predicted by anthropometry. A possible implication may be that associations of chronic disease risks with the sizes of internal body fat as measured by BMI, WC and HC may be strongly underestimated.

BACKGROUND:Compared to food patterns, nutrient patterns have been rarely used particularly at international level. We studied, in the context of a multi-center study with heterogeneous data, the methodological challenges regarding pattern analyses. METHODOLOGY/PRINCIPAL FINDINGS:We identified nutrient patterns from food frequency questionnaires (FFQ) in the European Prospective Investigation into Cancer and Nutrition (EPIC) Study and used 24-hour dietary recall (24-HDR) data to validate and describe the nutrient patterns and their related food sources. Associations between lifestyle factors and the nutrient patterns were also examined. Principal component analysis (PCA) was applied on 23 nutrients derived from country-specific FFQ combining data from all EPIC centers (N = 477,312). Harmonized 24-HDRs available for a representative sample of the EPIC populations (N = 34,436) provided accurate mean group estimates of nutrients and foods by quintiles of pattern scores, presented graphically. An overall PCA combining all data captured a good proportion of the variance explained in each EPIC center. Four nutrient patterns were identified explaining 67% of the total variance: Principle component (PC) 1 was characterized by a high contribution of nutrients from plant food sources and a low contribution of nutrients from animal food sources; PC2 by a high contribution of micro-nutrients and proteins; PC3 was characterized by polyunsaturated fatty acids and vitamin D; PC4 was characterized by calcium, proteins, riboflavin, and phosphorus. The nutrients with high loadings on a particular pattern as derived from country-specific FFQ also showed high deviations in their mean EPIC intakes by quintiles of pattern scores when estimated from 24-HDR. Center and energy intake explained most of the variability in pattern scores. CONCLUSION/SIGNIFICANCE:The use of 24-HDR enabled internal validation and facilitated the interpretation of the nutrient patterns derived from FFQs in term of food sources. These outcomes open research opportunities and perspectives of using nutrient patterns in future studies particularly at international level.

Resumo OBJETIVO: analisar a simultaneidade de dois grupos de comportamentos de risco para infecções sexualmente transmissíveis em adolescentes brasileiros. MÉTODOS: estudo transversal com dados da Pesquisa Nacional de Saúde do Escolar 2012; os desfechos foram (1) simultaneidade de consumo de álcool, fumo e drogas (pelo menos dois) e (2) não uso de camisinha e relação sexual com dois ou mais parceiros. RESULTADOS: a simultaneidade de consumo de álcool, fumo e drogas foi de 14,7% nos meninos e 21,5% nas meninas, mais prevalente em brancos, com mães mais escolarizadas, que não moravam com os pais; aproximadamente 12,0% dos adolescentes apresentaram simultaneidade de não uso de camisinha e dois ou mais parceiros, sendo mais frequente nos meninos mais novos, não brancos (RP=1,22; IC95% 1,10;1,36) e que não moravam com os pais (RP=1,78; IC95% 1,48;2,16), enquanto nas meninas, esse desfecho associou-se a maior idade. CONCLUSÃO: ambos desfechos associaram-se a diferentes características, dependendo do sexo do adolescente.

Resumo OBJETIVO: analisar a tendência da incidência de dengue no Brasil, no período de 2002 a 2012. MÉTODOS: estudo ecológico com dados do Sistema de Informação de Agravos e Notificação (Sinan); a taxa de incidência de dengue foi calculada segundo grupos etários, unidades da federação (UF) e grandes regiões do país, utilizando-se a regressão de Prais-Winsten. RESULTADOS: as taxas de incidência de dengue no Brasil, em 2002 e 2012, foram de 401,6 e 301,5 por 100 mil habitantes, respectivamente; as taxas de incremento anual revelaram-se estáveis (21,4%; IC95% -19,8;83,7) na maioria das UF, à exceção de Alagoas (38,9%; IC95% 5,1;83,5) e Tocantins (50,4%; IC95% 12,6;100,7); a região Norte foi a única a apresentar tendência de crescimento da incidência de dengue. CONCLUSÃO: embora as taxas tenham permanecido estáveis na maioria das UF, ainda são altas no país; políticas mais amplas com foco em novas estratégias de combate à dengue mostram-se necessárias.

ABSTRACT: Objective: The aims of this study were: 1) to estimate the prevalence of multimorbidity in 2013 and 2019 in adults aged 20–59 years; 2) to assess inequalities in the prevalence of multimorbidity in 2013 and 2019 according to educational level. Methods: Data from two cross-sectional surveys from the Brazilian National Health Survey in 2013 and 2019 were used. Multimorbidity was assessed from 14 lifetime self-reported morbidities (except back problems) and defined using the cutoff point of ≥2 diseases. The prevalence of multimorbidity and individual morbidities were described according to gender, age, skin color, and education. For education, crude, and relative inequalities in prevalence of multimorbidity were calculated using the Slope Index of Inequality and the Concentration Index, respectively. Results: The prevalence of multimorbidity increased from 18.7% (95%CI 18.0–19.3) in 2013 to 22.3% (95%CI 21.7–22.9) in 2019, being higher among women and adults between 30–59 years in both periods. Asthma/bronchitis, depression, and back problems were the conditions that increased the most in the study period. Absolute and relative inequalities by education status were observed in the study period, with worse multimorbidity profiles among the less educated. Conclusion: The prevalence of multimorbidity increased between 2013 and 2019. Inequalities in the prevalence of multimorbidity were observed according to educational level.

ABSTRACT Objective: To estimate the evolution of the prevalence of asthma and wheezing among schoolchildren in Brazil from 2012 to 2015, as well as to identify factors associated with both conditions. Methods: This was a cross-sectional study using data from the Brazilian National School-Based Adolescent Health Survey for 2012 and 2015. To characterize the evolution of the prevalence of asthma and wheezing, we used linear regression with weighted-least-squares estimation and presented the annual percent change (APC). Results: During the study period, there was a reduction in the prevalence of wheezing, from 23.2% in 2012 to 22.4% in 2015 (APC, −0.27). The prevalence of asthma increased from 12.4% in 2012 to 16.0% in 2015 (APC, 1.20). The increase in the prevalence of asthma was greatest in the southern region of the country (APC, 2.17). Having any history of smoking and having consumed alcohol in the last 30 days were factors that influenced the prevalence of wheezing and the prevalence of a self-reported diagnosis of asthma during the two years evaluated. Conclusions: There has been an increase in the prevalence of asthma in recent years in Brazil. Our data underscore the importance of improving health strategies and policies aimed at the control of asthma.

O objetivo foi avaliar a atenção oferecida por profissional de saúde aos idosos com hipertensão e as desigualdades, utilizando a escolaridade como indicador socioeconômico, no Brasil. Estudo transversal, com dados da Pesquisa Nacional de Saúde, em 2013. A atenção oferecida foi avaliada por meio dos indicadores: recomendações para alimentação saudável, manter o peso adequado, ingerir menos sal, praticar atividade física, não fumar, não ingerir bebida alcoólica em excesso, fazer acompanhamento regular e solicitação de exames de sangue e urina, eletrocardiograma e teste de esforço. As recomendações para ingerir menos sal (91%), alimentação saudável (88%) e fazer acompanhamento regular (88%) foram mais prevalentes. Idosos com ensino superior completo apresentaram maiores prevalências de recomendações para manter uma alimentação saudável (RP = 1,12), manter o peso adequado (RP = 1,21), praticar atividade física (RP = 1,29), acompanhamento regular (RP = 1,11), solicitação do teste de esforço (RP = 2,26) e recebimento de todos indicadores (RP = 2,14) em relação aos idosos sem instrução. A atenção oferecida por profissional de saúde está deficiente em todo o país. Além disso, as desigualdades ainda persistem. Tendo em vista o aumento da população idosa e das doenças crônicas não transmissíveis, são necessários esforços para melhorar a qualidade da atenção e diminuir as desigualdades.

Resumo: O objetivo do estudo foi descrever a simultaneidade de fatores de risco para doenças crônicas não transmissíveis em idosos (60 anos ou mais), residentes em uma cidade do Sul do Brasil. Trata-se de um estudo transversal, de base populacional, realizado em 1.451 idosos em 2013. Com uma abordagem de análise de clusters, foi avaliado o agrupamento entre os fatores de risco em estudo (tabagismo, consumo de álcool, excesso de peso e inatividade física). Para a avaliação da associação da simultaneidade dos fatores de risco com variáveis sociodemográficas, foi utilizada regressão logística. O agrupamento mais frequente entre os homens (18,1%) e mulheres (30,7%) foi inatividade física + excesso de peso. As combinações consumo de álcool + excesso de peso excederam o esperado entre os homens (O/E = 1,27; IC95%: 1,01; 1,59) e mulheres (O/E = 1,72; IC95%: 1,35; 2,20). A presença de dois ou mais fatores de risco na população idosa (88,1%) aponta para a necessidade de intervenções específicas para esta população voltadas ao combate simultâneo dos fatores de risco e não de forma isolada.

This study evaluated prospective associations between self-reported and objectively measured physical activity (PA) and risk of falls among older adults. A cohort study started in 2014 with 1,451 community-dwelling older adults living in Pelotas, Brazil. Leisure-time PA was obtained by the International Physical Activity Questionnaire and 7-day raw accelerometer data evaluated for total, light PA, and moderate to vigorous PA. In 2016–2017, participants recorded their falls in the previous 12 months. Around 23% of the 1,161 participants followed-up in 2016–2017 experienced a fall in the last 12 months. Participants who did not spend any time in self-reported leisure-time PA at baseline had on average 34% higher risk of falls, and individuals in the lowest tertile for moderate to vigorous PA had on average 51% higher risk of falls compared to those in the highest tertile. Low levels of self-reported and objectively measured moderate to vigorous PA were related to higher risk of falling among Brazilian older adults. [ABSTRACT FROM AUTHOR]
