1. Efficacy and Safety of Etelcalcetide in Hemodialysis Patients with Moderate to Severe Secondary Hyperparathyroidism

Abstract

Background. Secondary hyperparathyroidism (SHPT) is a major risk factor for cardiovascular events and all-cause mortality in hemodialysis (HD) patients. The purpose of our study was to evaluate the effects and tolerability of etelcalcetide in HD patients with SHPT. Methods. An observational study was conducted on 16 hemodialysis patients with SHPT treated with etelcalcetide. All patients were followed up for a duration of 6 months. The primary endpoints were the reduction in mean PTH ≥ 30% and ≥40% from baseline after 6 months of etelcalcetide. All patients were divided into two groups (group A versus group B) based on baseline serum PTH level prior to etelcalcetide: above and below the median serum PTH (1300 pg/mL), respectively. Results. After 6 months, a significant decrease in PTH levels was achieved by all patients receiving etelcalcetide (p = 0.015). Both primary endpoint of reduction in PTH ≥ 40% at 6 months (p = 0.01), and the secondary endpoint of reduction in median PTH values (p = 0.0001) and median percentage reduction in PTH values (p = 0.009) were significantly achieved in group A. In contrast, a greater decline of calcium (p = 0.028) and phosphorus was reached in group B than group A. Dialysis vintage ≥ 36 months, arteriovenous fistula (AVF)-based hemodialysis, post-diluition hemodiafiltration (HDF) method, and baseline values of PTH < 1300 pg/mL can positively influence the achievement of the endpoints. Furthermore, the baseline PTH < 1300 pg/mL, among these variables, was the only one showing statistically significant relevance (OR 2.28, 95% CI 1.32–3.96, p = 0.015). The history of cinacalcet use negatively correlated with the possibility to reach therapeutic targets with etelcalcetide (OR 0.47, 95% CI 0.26–0.85, p = 0.031). Treatment with etelcalcetide was well tolerated and no adverse effects were observed. Conclusions. In our study, patients with low baseline PTH levels showed a better response to etelcalcetide than patients with higher PTH levels. Consequently, the possibility to reach desirable therapeutic targets could depend on SHPT severity at the time of initiation of therapy.

Abstract

La DPI rappresenta un'opportunità per favorire il trattamento domiciliare. In questo momento la terapia sostitutiva domiciliare con DP rappresenta una sfida a causa dell'invecchiamento della popolazione e le difficoltà che abbiamo nel preparare tali pazienti a questa terapia. Nel paziente in uremia terminale la DPI rappresenta una opportunità di adattamento alla terapia, non sempre possibile; in caso invece di GFR superiore a 6 mL/min può rappresentare una possibilità di adattamento più semplice e graduale alla terapia, viste le migliori condizioni cliniche generali. Nel paziente con scompenso cardiaco refrattario riteniamo che sia il trattamento ideale nella gestione cronica di pazienti con frequenti episodi di scompenso acuto.

Abstract

Secondary hyperparathyroidism is a systemic disorder that associates with bone and cardiovascular disease, including arterial calcification. Treatment with calcitriol, the active form of vitamin D, reduces parathyroid hormone levels, but may result in elevations in serum calcium and phosphorus, increasing the risk of vascular calcification in dialysis patients. New vitamin D receptor activators (VDRAs) have been developed and investigated with the rationale to treat high serum PTH levels, with a reduced risk of hypercalcemia and hyperphosphatemia. Paricalcitol is a selective VDRA that suppresses PTH secretion with minimal increases on serum calcium and phosphate. Moreover, paricalcitol prevents vascular calcification in experimental models of renal failure, compared with calcitriol.

Abstract

Introduction: Predicting the timing and occurrence of kidney replacement therapy (KRT), cardiovascular events, and death among patients with advanced chronic kidney disease (CKD) is clinically useful and relevant. We aimed to externally validate a recently developed CKD G4+ risk calculator for these outcomes and to assess its potential clinical impact in guiding vascular access placement. Methods: We included 1517 patients from the European Quality (EQUAL) study, a European multicentre prospective cohort study of nephrology-referred advanced CKD patients aged ≥65 years. Model performance was assessed based on discrimination and calibration. Potential clinical utility for timing of referral for vascular access placement was studied with diagnostic measures and decision curve analysis (DCA). Results: The model showed a good discrimination for KRT and “death after KRT,” with 2-year concordance (C) statistics of 0.74 and 0.76, respectively. Discrimination for cardiovascular events (2-year C-statistic: 0.70) and overall death (2-year C-statistic: 0.61) was poorer. Calibration was fairly accurate. Decision curves illustrated that using the model to guide vascular access referral would generally lead to less unused arteriovenous fistulas (AVFs) than following estimated glomerular filtration rate (eGFR) thresholds. Conclusion: This study shows moderate to good predictive performance of the model in an older cohort of nephrology-referred patients with advanced CKD. Using the model to guide referral for vascular access placement has potential in combating unnecessary vascular surgeries.

Abstract

Introduction: Secondary hyperparathyroidism (SHPT) represents a complication of chronic kidney disease (CKD). Vitamin D system is altered since early CKD, and vitamin D deficiency is an established trigger of SHPT. Although untreated SHPT may degenerate into tertiary hyperparathyroidism with detrimental consequences in advanced CKD, best treatments for counteracting SHPT from stage 3 CKD are still debated. Enthusiasm on prescription of vitamin D receptor activators (VDRA) in non-dialysis renal patients, has been mitigated by the risk of low bone turnover and positive calcium-phosphate balance. Nutritional vitamin D is now suggested as first-line therapy to treat SHPT with low 25(OH)D insufficiency. However, no high-grade evidence supports the best choice between ergocalciferol, cholecalciferol, and calcifediol (in its immediate or extended-release formulation).Areas covered: The review discusses available data on safety and efficacy of nutritional vitamin D, VDRA and nutritional therapy in replenishing 25(OH)D deficiency and counteracting SHPT in non-dialysis CKD patients.Expert opinion: Best treatment for low 25(OH)D and SHPT remains unknown, due to incomplete understanding of the best homeostatic, as mutable, adaptation of mineral metabolism to CKD progression. Nutritional vitamin D and nutritional therapy appear safest interventions, whenever contextualized with single-patient characteristics. VDRA should be restricted to uncontrolled SHPT by first-line therapy.

Abstract

Background Various prediction models have been developed to predict the risk of kidney failure in patients with CKD. However, guideline-recommended models have yet to be compared head to head, their validation in patients with advanced CKD is lacking, and most do not account for competing risks. Methods To externally validate 11 existing models of kidney failure, taking the competing risk of death into account, we included patients with advanced CKD from two large cohorts: the European Quality Study (EQUAL), an ongoing European prospective, multicenter cohort study of older patients with advanced CKD, and the Swedish Renal Registry (SRR), an ongoing registry of nephrology-referred patients with CKD in Sweden. The outcome of the models was kidney failure (defined as RRT-treated ESKD). We assessed model performance with discrimination and calibration. Results The study included 1580 patients from EQUAL and 13,489 patients from SRR. The average c statistic over the 11 validated models was 0.74 in EQUAL and 0.80 in SRR, compared with 0.89 in previous validations. Most models with longer prediction horizons overestimated the risk of kidney failure considerably. The 5-year Kidney Failure Risk Equation (KFRE) overpredicted risk by 10%-\18%. The four- and eight-variable 2-year KFRE and the 4-year Grams model showed excellent calibration and good discrimination in both cohorts. Conclusions Some existing models can accurately predict kidney failure in patients with advanced CKD. KFRE performed well for a shorter time frame (2 years), despite not accounting for competing events. Models predicting over a longer time frame (5 years) overestimated risk because of the competing risk of death. The Grams model, which accounts for the latter, is suitable for longer-term predictions (4 years).

Abstract

Background Depressive symptoms are associated with adverse clinical outcomes in patients with end-stage kidney disease; however, few small studies have examined this association in patients with earlier phases of chronic kidney disease (CKD). We studied associations between baseline depressive symptoms and clinical outcomes in older patients with advanced CKD and examined whether these associations differed depending on sex. Methods CKD patients (≥65 years; estimated glomerular filtration rate ≤20 mL/min/1.73 m2) were included from a European multicentre prospective cohort between 2012 and 2019. Depressive symptoms were measured by the five-item Mental Health Inventory (cut-off ≤70; 0–100 scale). Cox proportional hazard analysis was used to study associations between depressive symptoms and time to dialysis initiation, all-cause mortality and these outcomes combined. A joint model was used to study the association between depressive symptoms and kidney function over time. Analyses were adjusted for potential baseline confounders. Results Overall kidney function decline in 1326 patients was –0.12 mL/min/1.73 m2/month. A total of 515 patients showed depressive symptoms. No significant association was found between depressive symptoms and kidney function over time (P = 0.08). Unlike women, men with depressive symptoms had an increased mortality rate compared with those without symptoms [adjusted hazard ratio 1.41 (95% confidence interval 1.03–1.93)]. Depressive symptoms were not significantly associated with a higher hazard of dialysis initiation, or with the combined outcome (i.e. dialysis initiation and all-cause mortality). Conclusions There was no significant association between depressive symptoms at baseline and decline in kidney function over time in older patients with advanced CKD. Depressive symptoms at baseline were associated with a higher mortality rate in men.

Abstract

Dabigatran overload has been reported in acute kidney injury (AKI), leading to occasional major bleeding. Haemodialysis (HD) was the method used for reversing dabigatran anticoagulant effects before the approval of idarucizumab, which is now indicated for dabigatran reversal in major bleeding or surgical emergencies. There have been reports of rebound of dabigatran levels following idarucizumab administration in AKI, requiring HD to achieve effective dabigatran clearance. However, a decisional algorithm to individualize treatments for dabigatran overload seems lacking. We present a case of dabigatran accumulation in obstructive AKI with minor bleeding that was successfully treated with HD and tranexamic acid without using idarucizumab, and propose a decision-making algorithm including different pathways in the management of suspected dabigatran overload in AKI.

Abstract

Background and Aims Chronic kidney disease (CKD), and especially end-stage renal disease (ESRD) are relevant worldwide public health problems, since they are linked to poor quality of life, serious adverse health outcomes (including cardiovascular diseases, infections, duration of hospitalization and, more importantly, overall mortality), as well as increased healthcare costs. Moreover, renal replacement therapy with either dialysis (both hemodialysis and peritoneal dialysis) or kidney transplantation represents the economic burden of ESRD, which continues to grow substantially. Future projections are for an increasing prevalence of ESRD globally. Worldwide, more than 2.5 million people were treated for ESRD in 2015, and this number is projected to more than double, thus reaching the astonishing number of 5 million by 2030. Method In our San Paolo Hospital, in Milan, a Division of Penitentiary Medicine has been established, allowing us to serve, with wide medical and surgical support, all the four prisons present in the Milan area, since several years. During the last decade, we were required to comprehensively manage renal patients as well. In particular, we followed also a number of ESRD patients on replacement treatment with hemodialysis thrice weekly. The Dialysis Unit received prisoner patients and treated them under the security control of several penitentiary policemen. Among them, we had also two high-surveillance prisoners who needed as many as six higly armed policemen each for security. The presence of these agents caused a relevant discomfort to both the other patients, as well the whole medical and nursing staff. After a long period of organization, we started an innovative project called ‘Home Jail Hemodialysis’. Since July 2018 until now, hemodialysis sessions for these two high-surveillance prisoners (on separate days) have been performed within one prison, with the need of just one policeman to control them. Nurses had to be provenly skilled for a comprehensive care for hemodialysis patients and were also specifically trained for the management of patients subjected to high-surveillance detention, as particular behavioral and relational precautions are obviously essential. After a run-in period, in which management protocols/procedures were shared with those of our hospital, all the activities started taking place only within the prison’s Health building. The care setting includes single security rooms, with a nurse present throughout the treatment period; once monthly, a Nephrologist re-evaluates the patients, updates the treatment, and shares with the nursing coordinator all eventual issues. Results The feasibility and safety of this approach, aimed at integrating the penitentiary structure with the hospital with a decentralized assistance, was tested using two indicators of performance: the number of adverse events reported in 12 months (about 310 dialyses), and costs. In 12 months of observation, there were no serious adverse events. Furthermore, in just one year approximately 1,000,000 € were saved (considering that the cost of traveling to the hospital and of high-level surveillance for detained patients is close to € 500,000 per year). Conclusion The ‘Home Jail Hemodialysis’ thus appears an easy way to save money and improve security.

Abstract

Cardiovascular disease (CVD) is a highly common complication and the first cause of death in patients with end-stage renal disease (ESRD) on haemodialysis (HD). In this population, mortality due to CVD is 20 times higher than in the general population and the majority of maintenance HD patients have CVD. This is likely due to ventricular hypertrophy as well as non-traditional risk factors, such as chronic volume overload, anaemia, inflammation, oxidative stress, chronic kidney disease–mineral bone disorder and other aspects of the ‘uraemic milieu’. Better understanding the impact of these numerous factors on CVD would be an important step for prevention and treatment. In this review we focus non-traditional CVD risk factors in HD patients.

Abstract

Background: Bone Gamma-carboxyglutamic acid (Gla)-protein (BGP or osteocalcin) is a vitamin K-dependent protein involved in the regulation of bone mineralization. Smoking is a risk factor for osteoporosis. Methods: We carried out a secondary analysis of the Vitamin K Italian (VIKI) study to investigate the association between cigarette smoking and BGP levels in patients with end stage renal disease. Data were collected in 370 haemodialysis patients, 37% (136) smokers (or ex-smokers) and 63% (234) nonsmokers. Vascular calcifications and vertebral fractures (quantitative morphometry) were identified on spine radiographs. Results: Smokers had significantly lower BGP levels (152 vs. 204 μg/L, p=0.003). Abstract

SETTING: The World Health Organization Commission on the Social Determinants of Health (SDoH) observes that building political will is central to all its recommendations, because governments respond to those who organize and show up. Since younger Canadians are less likely to vote or to organize in between elections, they are less effective at building political will than their older counterparts. This results in an age gap between SDoH research and government budget priorities. Whereas Global AgeWatch ranks Canada among the top countries for aging, UNICEF ranks Canada among the least generous OECD (Organisation for Economic Co-operation and Development) countries for the generations raising young children. INTERVENTION: A surgical intervention into the body politic. Guided by the “health political science” literature, the intervention builds a non-profit coalition to perform science-based, non-partisan democratic engagement to increase incentives for policy-makers to translate SDoH research about younger generations into government budget investments. OUTCOMES: All four national parties integrated policy recommendations from the intervention into their 2015 election platforms. Three referred to, or consulted with, the intervention during the election. The intervention coincided with all parties committing to the single largest annual increase in spending on families with children in over a decade. IMPLICATIONS: Since many population-level decisions are made in political venues, the concept of population health interventions should be broadened to include activities designed to mobilize SDoH science in the world of politics. Such interventions must engage with the power dynamics, values, interests and institutional factors that mediate the path by which science shapes government budgets.

Abstract

Background Tunneled central venous catheters (tCVCs) are considered inferior to arteriovenous fistulas (AVFs) and grafts in all nephrology guidelines. However, they are being increasingly used as hemodialysis vascular access. The purpose of this study was to document the natural history of tCVCs and determine the rate and type of catheter replacement. Methods This was a prospective study of 141 patients who underwent hemodialysis with tCVCs between January 2008 and December 2012. The patients used 154 tCVCs. Standard protocols about management of tCVCs, according to European Renal Best Practice, were well established. All catheters were inserted in the internal jugular vein. Criteria for catheter removal were persistent bloodstream infection, detection of an outbreak of catheter-related bloodstream (CRBS) infections, or catheter dysfunction. Event rates were calculated per 1,000 catheter days; tCVC cumulative survival was estimated by Kaplan-Meier analysis. Results Catheter replacement occurred in 15 patients (0.29 per 1,000 days); catheter dysfunction was the main cause of replacement (0.18 per 1,000 days), typically within 12 months of surgical insertion. A total of 53 CRBS events in 36 patients were identified (0.82 per 1,000 days); 17 organisms, most commonly Gram-positive pathogens, were isolated; 87% of CVC infections were treated by systemic antibiotics associated with lock therapy. tCVC cumulative survival was 91% at 1 year, 88% at 2 years and 85% at 4 years. Conclusions Our data show a high survival rate of tCVCs in hemodialysis patients, with low incidence of catheter dysfunction and CRBS events. These data justify tCVC use for hemodialysis vascular access, also as first choice, especially in patients with exhausted peripheral access and limited life expectancy.

Abstract

In chronic kidney disease (CKD) patients, cardiovascular (CV) morbidity and mortality rate is higher than in the general population, because of frequently concomitant hypertension, peripheral vascular disease, heart failure, vascular calcification (VC), diabetes and mineral bone disease. Recently, another important factor associated to CV risk in CKD has been deeply investigated: vitamin D deficiency. Vitamin D Receptors (VDRs) are present in several systems and tissues and VDR activation is associated to positive effects, resulting in better blood pressure control and prevention of diabetic nephropathy. Unfortunately, the natural, non-selective vitamin D receptor activator (VDRA), calcitriol, is associated to higher serum calcium and phosphate levels, thus worsening CV risk in CKD. Recent data showed that the selective VDRA paricalcitol might have ameliorative CV effects. The potential positive impact of the use of paricalcitol on diabetic nephropathy, cardiac disease, hypertension, and VC may open new paths in the fight against CV disease in CKD patients.

Vitamin K (phylloquinone or vitamin K1and menaquinones or vitamin K2) plays an important role as a cofactor in the synthesis of hepatic blood coagulation proteins, but recently has also aroused an increasing interest for its action in extra-hepatic tissues, in particular in the regulation of bone and vascular metabolism. The accurate measurement of vitamin K status in humans is still a critical issue. Along with indirect assays, such as the undercarboxylated fractions of vitamin K-dependent proteins [prothrombin, osteocalcin (OC), and matrix gla protein], the direct analysis of blood levels of phylloquinone and menaquinones forms might be considered a more informative and direct method for assessing vitamin K status. Different methods for direct quantification of vitamin K serum levels are available. High-performance liquid chromatography (HPLC) methods coupled with post-column reduction procedures and fluorimetric or electrochemical detection are commonly used for food and blood analysis of phylloquinone, but they show some limitations when applied to the analysis of serum menaquinones because of interferences from triglycerides. Recent advancements include liquid chromatography tandem mass spectrometry (LCMS/MS) detection, which assures higher specificity. The optimization and standardization of these methods requires specialized laboratories. The variability of results observed in the available studies suggests the need for further investigations to obtain more accurate analytical results.

Abstract

Patients with mitochondrial myopathies (MM) or myophosphorylase deficiency (McArdle's disease, McA) show impaired capacity for O(2) extraction, low maximal aerobic power, and reduced exercise tolerance. Non-invasive tools are needed to quantify the metabolic impairment. Six patients with MM, 6 with McA, 25 with symptoms of metabolic myopathy but negative biopsy (patient-controls, P-CTRL) and 20 controls (CTRL) underwent an incremental cycloergometric test. Pulmonary O(2) uptake (VO(2)) and vastus lateralis oxygenation indices (by near-infrared spectroscopy, NIRS) were determined. Concentration changes of deoxygenated hemoglobin and myoglobin (Delta[deoxy(Hb + Mb)]) were considered an index of O(2) extraction. Delta[deoxy(Hb + Mb)] peak (percent limb ischemia) was lower in MM (25.3 +/- 12.0%) and McA (18.7 +/- 7.3) than in P-CTRL (62.4 +/- 3.9) and CTRL (71.3 +/- 3.9) subjects. VO(2) peak and Delta[deoxy(Hb + Mb)] peak were linearly related (r(2) = 0.83). In these patients, NIRS is a tool to detect and quantify non-invasively the metabolic impairment, which may be useful in the follow-up of patients and in the assessment of therapies and interventions.

Abstract

With the aging of the population, the prevalence of chronic kidney disease (CKD) is increased. Measurement of glomerular filtration rate as a screening tool may over-diagnose CKD, especially when proteinuria is normal, but it can be very useful when considering drug metabolism. Renal dysfunction is a factor predisposing to potential adverse drug reactions, because drug can accumulate to toxic levels. In addition, some drugs are nephrotoxic and can more easily damage the kidneys in the elderly. Limited data are available on the risks of drugs in the elderly population with CKD. Drugs with no clear evidence-based indication, drugs with higher risks of adverse side effects compared to their benefits, and drugs which are not cost-effective, have been defined "potentially inappropriate medications" (PIMs). Even if criteria to evaluate PIMs and adverse drug reactions are available to clinical management, we strongly support the idea that the issue of PIMs in the elderly affected by CKD should be better studied and defined. The Italian website Slow Medicine, based on the US project "Choosing Wisely", opened a new strategy in this field, taking into consideration both economic issues and patients quality of life. The main goal of this initiative is avoiding wasteful or unnecessary medical tests, treatments and procedures.

Abstract

Chronic kidney disease (CKD) is characterized by phosphorus retention and, in more advanced stages, by high serum phosphorus (P) levels. During the last decade, it has been elucidated the central role of P in the pathogenesis of CKD mineral bone disorder (CKD-MBD), determining both renal osteodystrophy and cardiovascular disease. Unfortunately, at least one third of patients on chronic dialysis have high serum P levels, with a consequent higher serum PTH levels, commonly associated with vitamin D deficiency, increased vascular calcification and the highest ratios of morbidity and mortality. In patients with CKD stage 5 on dialysis, therapeutic approaches to reduce serum P levels should include restriction of dietary phosphate intake, optimal dialysis treatment, and use of P binders. In this context, the use of P binders appears to be an essential treatment to control P overload in CKD patients. In this review, we analyzed the use of calcium-based and calcium-free P binders in peritoneal dialysis patients.

Abstract

The involvement of vitamin D deficiency in cardiovascular morbidity and mortality is attracting great interest. In patients with chronic kidney disease this association is stronger because vitamin D levels decrease as a result of renal progressive impairment. In chronic kidney disease secondary hyperparathyroidism commonly occurs in response to persistent hypocalcemia and hyperphosphatemia; moreover, parathyroid gland volume increases, vascular calcification is accelerated, and structural and functional modifications of the left ventricle are observed. These alterations entail both cardiac and renal involvement, resulting in cardio-renal syndrome. Recent studies concluded that vitamin D administration seems to have cardioprotective and renoprotective effects and improve peripheral vascular disease, vascular calcification, cardiac outcome, and blood pressure control. In clinical practice, therefore, the use of this hormone may play an important role in cardio-renal syndrome prevention.

During the last decade, it has been elucidated the central role of P in the pathogenesis of CKD mineral bone disorder (CKD-MBD), determining both renal osteodystrophy and cardiovascular disease. Unfortunately, at least one third of patients on chronic dialysis have high serum P levels, with a consequent higher serum PTH levels, commonly associated with vitamin D deficiency, increased vascular calcification and the highest ratios of morbidity and mortality. In patients with CKD stage 5 on dialysis, therapeutic approaches to reduce serum P levels should include restriction of dietary phosphate intake, optimal dialysis treatment, and use of P binders. In this context, the use of P binders appears to be an essential treatment to control P overload in CKD patients. In this review, we analyzed the use of calcium-based and calcium-free P binders in peritoneal dialysis patients.

Abstract

Raised blood pressure (BP) is a major risk factor for cardiovascular disease. Previous studies have identified 47 distinct genetic variants robustly associated with BP, but collectively these explain only a few percent of the heritability for BP phenotypes. To find additional BP loci, we used a bespoke gene-centric array to genotype an independent discovery sample of 25,118 individuals that combined hypertensive case-control and general population samples. We followed up four SNPs associated with BP at our p < 8.56 x 10(-7) study-specific significance threshold and six suggestively associated SNPs in a further 59,349 individuals. We identified and replicated a SNP at LSP1/TNNT3, a SNP at MTHFR-NPPB independent (r(2) = 0.33) of previous reports, and replicated SNPs at AGT and ATP2B1 reported previously. An analysis of combined discovery, and follow-up data identified SNPs significantly associated with BP at p < 8.56 x 10(-7) at four further loci (NPR3, FIFE, NOS3, and SOX6). The high number of discoveries made with modest genotyping effort can be attributed to using a large-scale yet targeted genotyping array and to the development of a weighting scheme that maximized power when meta-analyzing results from samples ascertained with extreme phenotypes, in combination with results from nonascertained or population samples. Chromatin immunoprecipitation and transcript expression data highlight potential gene regulatory mechanisms at the MTHFR and NOS3 loci. Abstract

Secondary hyperparathyroidism is a systemic disorder that associates with bone and cardiovascular disease, including arterial calcification. Treatment with calcitriol, the active form of vitamin D, reduces parathyroid hormone levels, but may result in elevations in serum calcium and phosphorus, increasing the risk of vascular calcification in dialysis patients. New vitamin D receptor activators (VDRAs) have been developed and investigated with the rationale to treat high serum PTH levels, with a reduced risk of hypercalcemia and hyperphosphatemia. Paricalcitol is a selective VDRA that suppresses PTH secretion with minimal increases on serum calcium and phosphate. Moreover, paricalcitol prevents vascular calcification in experimental models of renal failure, compared with calcitriol.

Abstract

La DPI rappresenta un'opportunità per favorire il trattamento domiciliare. In questo momento la terapia sostitutiva domiciliare con DP rappresenta una sfida a causa dell'invecchiamento della popolazione e le difficoltà che abbiamo nel preparare tali pazienti a questa terapia. Nel paziente in uremia terminale la DPI rappresenta una opportunità di adattamento alla terapia, non sempre possibile; in caso invece di GFR superiore a 6 mL/min può rappresentare una possibilità di adattamento più semplice e graduale alla terapia, viste le migliori condizioni cliniche generali. Nel paziente con scompenso cardiaco refrattario riteniamo che sia il trattamento ideale nella gestione cronica di pazienti con frequenti episodi di scompenso acuto.

Abstract

Epidemiological and observational data indicate that there is a close relationship between progressive renal dysfunction in chronic kidney disease (CKD), cardiovascular disease, and mortality. In addition, deficits in vitamin D (25-hydroxyvitamin D) and vitamin D receptor (VDR) activation play a crucial role in adversely affecting cardiovascular health in CKD patients. Even in patients with mild CKD, renal dysfunction is associated with cardiovascular events. Modulation of vitamin D levels results in correlative regulatory effects on mineral homeostasis, hypertension, and vascular calcification. The use of VDR activators such as paricalcitol to treat these and other parameters outside of cardiovascular and renal disease not only results in enhanced patient health but significantly reduces the mortality risk in CKD patients.

Abstract

Background. Hyperhomocysteinaemia is an independent risk factor for the development of atherosclerosis. Furthermore, homocysteine induces endothelial dysfunction by an increased inactivation of nitric oxide. In patients with chronic renal failure, the administration of folic acid or its metabolites reduces but does not normalize plasma homocysteine concentrations. Methods. We examined the effect of oral treatment with 15 mg/daily of 5-methyltetrahydrofolate (5-MTHF) for 12 weeks, on homocysteinaemia and endothelial function in 19 patients undergoing peritoneal dialysis and compared them, for the same period of time, to a control group of patients on peritoneal dialysis. Endothelial function was evaluated by B-mode ultrasonography on the brachial artery. Flow-mediated dilation (FMD) was recorded during reactive hyperaemia produced by the inflation of a pneumatic tourniquet. Nitroglycerine-mediated dilation (NMD) was recorded after sublingual administration of glyceryl trinitrate. Finally, oxidative stress was assessed by evaluating the conjugated dienes plasma levels. Results. Plasma homocysteine concentrations fell by 30% after oral treatment with 5-MTHF. Endothelial function improved significantly after oral 5-MTHF treatment (13.8 � 1.2% vs 11.4 � 1.4%; P < 0.02) while in the control group we observed a worsening of basal values from 12.1 � 2.66% to 8.7 � 2.90% (P < 0.02). The conjugated dienes plasma levels did not change either. Conclusions. Our study demonstrated that 5-MTHF administration improves endothelial dysfunction in patients undergoing peritoneal dialysis. This effect appears to be independent of the reduction in homocysteine plasma levels.

Abstract

A proposta principal desta tese foi o desenvolvimento de novas composições vítreas teluritos e teluretos para obtenção de materiais amorfos úteis para fotônica. Primeiramente foram explorados novos sistemas binários e ternários baseados em TeO2, Sb2O3 e GeO2, contendo fluoreto de chumbo para obtenção de vidros termicamente e quimicamente estáveis. Estas composições vítreas foram caracterizadas utilizando técnicas tradicionais no estudo de materiais não-cristalinos como Análise Térmica (DSC), espectroscopia vibracional (IV e Raman) e espectroscopia eletrônica na região do ultravioleta, visível e infravermelho próximo. Os resultados obtidos por análise térmica permitiram a determinação das temperaturas características, que podem ser utilizadas para avaliar a estabilidade térmica dos vidros frente à cristalização. As investigações estruturais por espectroscopia vibracional, eletrônica e luminescencia de Eu³+ utilizado como sonda estrutural foram fundamentais para avaliar as mudanças estruturais na rede vítrea em função da composição. Estudos de luminescência de íons terras raras para amostras dopadas com Er³+, codopadas com Er³+/Yb³+ e dopadas com Eu³+ em composições mais promissoras também foram realizados com o intuito de investigar o efeito da composição na propriedade de emissão em comprimento de onda de interesse para aplicação em dispositivos ópticos. Uma outra linha de pesquisa desta tese foi a preparação de filmes finos em substratos flexíveis (celulose e fibroína), a partir da evaporação por diferentes técnicas de vidros calcogenetos com propriedades fotossensíveis já descritas na literatura. Neste caso, estudou-se a composição Ge2Sb2Te5 já estabelecida para armazenamento de dados em CDs e DVDs. Também foram obtidos filmes finos a partir das composições de vidros óxidos estudados neste trabalho de doutorado. Esta linha do trabalho permitiu a obtenção de um material totalmente novo, inédito e flexível. The main purpose of this thesis has been the development of new tellurite and telluride vitreous compositions to obtaining useful amorphous materials for optics. First new binary and ternary systems based on TeO2, GeO2, Sb2O3 and PbF2 were explored in order to prepare thermally and chemically stable vitreous compositions. These glasses were characterized using traditional techniques in the study of non-crystalline materials such as thermal analysis (DSC), vibrational spectroscopy (Raman and IR) and electronic spectroscopy in the ultraviolet, visible and near infrared spectral regions. The results of thermal analysis allowed the determination of the characteristic temperatures, which can be used to evaluate the thermal stability of the glass against crystallization. Structural investigations by electronic and vibrational spectroscopy, used as a structural probe were useful to assess the structural changes in the vitreous network according to the composition. Luminescence studies of glass samples doped with Er³+, codoped with Er³+/Yb³+ and doped with Eu³+, in the most promising compositions were also conducted in order to investigate the effect of composition on optical properties in wavelengths of interest for application as optical devices. Another research line in this thesis was the preparation of thin films on flexible substrates (cellulose and fibroin) from diverse evaporation techniques of chalcogenide glasses with photochromic properties already described in the literature, which in this case was the Ge2Sb2Te5 composition already established for data storage on CDs and DVDs. 39. Preparação e caracterização de vidros de fosfato de tungstênio contendo nanopartículas metálicas de prata, ouro e platina

Abstract

Foram preparadas amostras vítreas no sistema binário NaPO3-WO3 com porcentagens de WO3 de 30%, 40% e 50% em mol. Agentes oxidantes como CeO2 e Sb2O3 foram utilizados para obter vidros com maior transparência no visível. Foi possível obter amostras vítreas transparentes e homogêneas com porcentagem em mol de 0,1% de Au3+, 0,1% de Pt4+ e 0,1% de Ag+. Foi observado que a incorporação de agentes oxidantes influencia nas propriedades térmicas dos vidros obtidos e que o mecanismo predominante de cristalização é superficial em todas as amostras. Medidas de espalhamento Raman permitiram acessar parâmetros estruturais da rede vítrea, a qual é composta por cadeias mistas de tetraedros PO4 e octaedros WO6, com presença de agregados de octaedros WO6 para as amostras contendo 40% e 50% em mol de WO3. Os vidros dopados (Ag, Au e Pt) e tratados termicamente apresentaram uma intensa banda de absorção na região do visível atribuída à ressonância de plasmons de nanopartículas metálicas presentes na rede vítrea, visto que a amostra não dopada assim como as amostras não tratadas não apresentam os mesmos padrões de absorção nessa região. Os resultados de DSC em conjunto com resultados de difração de raios X sugerem que a presença de nanopartículas induz a precipitação volumétrica de WO3 monoclínico, o qual nucleia a fase principal de fosfato duplo de tungstênio e sódio NaWO2PO4 para cristalização da matriz vítrea. Finalmente, medidas de espalhamento Raman mostraram que a amostra dopada com nanopartículas de ouro apresenta um espectro Raman diferente das demais amostras (dopadas com Ag, Pt e não dopada), sugerindo a ocorrência de um possível efeito SERS (Surface Enhanced Raman Spectroscopy) nessa amostra dopada com ouro. Glass samples were prepared by the melt-quenching method using sodium polyphosphate and tungsten oxide with WO3 contents varying from 30 mole% to 50 mole%. Oxidizing agents (CeO2 and Sb2O3) have been used to avoid tungsten reduction and produce glass samples with higher transparency in the visible. These glasses were doped with noble metals using salts precursors Au2O3, PtCl4 and AgNO3. Transparent and homogeneous glasses containing 0,1 mole% of each metallic salt were successfully obtained. UV-visible spectra confirmed transparency of the doped and undoped glasses containing oxidizing agents CeO2 and Sb2O3. Raman spectroscopy was useful to access structural parameters of the glass network, which is built-up of PO4 and WO6 units, with WO6 clusters in the most WO3 concentrated samples. The doped glasses (Ag, Au and Pt) heat treated above the glass transition temperature exhibit an intense absorption band in the visible with a specific absorption maximum for each doped sample which has been attributed to surface plasmon resonance of metallic nanoparticles precipitated inside the glass network. DSC together with X-ray diffraction results allowed proposing a crystallization mechanism in which metallic nanoparticles are able to induce precipitation of monoclinic WO3 and further precipitation of tungsten sodium phosphate NaWO2PO4. Finally, Raman investigations showed that the gold-doped sample exhibits a different Raman spectrum when compared with the other doped and undoped samples, suggesting a possible SERS effect which can be related with a resonance between the laser wavelength (633nm) and the plasmon resonance absorption band in this gold-doped sample.
