International audience; In tandem with the ever-increasing aging population in low and middle-income countries, the burden of dementia is rising on the African continent. Dementia prevalence varies from 2.3% to 20.0% and incidence rates are 13.3 per 1000 person-years with increasing mortality in parts of rapidly transforming Africa. Differences in nutrition, cardiovascular factors, comorbidities, infections, mortality, and detection likely contribute to lower incidence. Alzheimer's disease, vascular dementia, and human immunodeficiency virus/acquired immunodeficiency syndrome-associated neurocognitive disorders are the most common dementia subtypes. Comprehensive longitudinal studies with robust methodology and regional coverage would provide more reliable information. The apolipoprotein E (APOE) ε4 allele is most studied but has shown differential effects within African ancestry compared to Caucasian. More candidate gene and genome-wide association studies are needed to relate to dementia phenotypes. Validated culture-sensitive cognitive tools not influenced by education and language differences are critically needed for implementation across multidisciplinary groupings such as the proposed African Dementia Consortium.

Introduction: Facial aging implies special care and personalized treatment. Thus, the new strand of Neomodern Dentistry seeks, through Orofacial Harmonization, the functional and aesthetic balance between the stomatognathic system and the facial aspect. Objective: This article seeks to disclose, through a literature review, the aesthetical consequences of the stomatognatic system repositioning and orofacial aging. Data source: The present literature review consisted in researches up to May 2019 using PubMed and Google Academic electronic databases. A 10-year publication limit was applied in the research. No language restriction was applied. Inclusion criteria were clinical investigations, books, dissertations, thesis or literature reviews that addressed the topics of interest. Data synthesis: A total of 231 articles were retrieved from databases. After applying a 10-year publication limit, 111 remained and, based on the inclusion and exclusion criteria, 20 articles were selected and included in this review. Conclusion: Considering the limitationsof the present study, it can be concluded that the aging process is natural and predictable and can be changeable and malleable through procedures that restore the support nutrients that were lost. The aesthetics can be achieved as a functional consequence of the stomatognathic system repositioning due to orofacial aging Introdução: O envelhecimento facial implica em cuidados especiais e um tratamento diferenciado. Desse modo, a nova vertente da Odontologia Neo moderna busca, por meio da Harmonização Orofacial, o equilíbrio funcional e estético entre o aparelho estomatognático e a face. Objetivo: Esse artigo buscacompreender, por meio de uma revisão de literatura, as consequências estéticas do reposicionamento do aparelho estomatognático e envelhecimento orofacial. Fonte dos dados: A presente revisão de literatura consistiu em um viés qualitativo nas plataformas PubMed e Google Acadêmico, nos ultimos 10 anos, sem restrição de idiomas. Os critérios de inclusão consistiram em estudos clínicos, livros, dissertações, teses ou revisões de literatura que abordavam os tópicos de interesse. Síntese dos dados: Foram recuperados nas bases de dados 231 artigos. Após a aplicação de um limite de publicação de 10 anos, 111 permaneceram e, com base nos critérios de inclusão e exclusão, 20 artigos foram selecionados e incluídos nesta revisão. Conclusão: Com as limitações do presente estudo, pode-se concluir que o processo de envelhecimento é natural e previsível e pode ser mutável e maleável por meio de procedimentos que restauram os nutrientes de suporte perdidos. A estética pode ser alcançada como uma consequência funcional do reposicionamento do sistema estomatognático e do envelhecimento orofacial.

Introdução: O sorriso gengival, ocorre durante a fala ou sorriso espontâneo, quando há aparência de mais de três milímetros de gengiva, tornando-se desarmônica a relação entre a proporção de dentes, lábios e gengivas. Nesses casos, o tratamento seria o aumento de coroa clínica, seguido ou não de aplicação de toxina botulínica do tipo A, dependendo do diagnóstico. Ferramentas como o Planejamento Digital do Sorriso e a Tomografia Computadorizada Cone Beam com afastadores labiais, são recursos atuais que possibilitam um bom e completo planejamento dos casos de sorriso gengival. Associando-se ambas as técnicas para o planejamento, o tratamento de correção do sorriso gengival torna-se mais simples e previsível. Objetivos: A presente pesquisa teve por objetivo reportar um caso clínico de sorriso gengival de etiologia múltipla, em que o planejamento e o tratamento foram multidisciplinares. Relato do Caso: Paciente do sexo feminino, 19 anos de idade, compareceu à Clínica de Odontologia da Faculdades Integradas Aparício Carvalho – FIMCA, com queixa de sorriso gengival, e desproporção de tamanho das coroas dos incisivos centrais superiores. Foram realizados anamnese, exame clínico e exame periodontal, tomografia computadorizada e planejamento digital do sorriso. Após minucioso planejamento foi inidicada a cirurgia de aumento de coroa clínica e a aplicação de toxina botulínica. Paciente, relatou satisfação pelo tratamento e novo sorriso. Conclusão: Quando se trata de estética do sorriso, é essencial um bom planejamento. O Planejamento Digital e a Tomografia Computadorizada Cone Beam com afastadores labiais são ferramentas fundamentais para os casos de sorriso gengival. Optar pelo tratamento multidisciplinar, avaliando a estética orofacial seré sempre mais seguro e com resultados precisos, previsíveis e satisfatórios. Introduction: The gummy smile, occurs during speech or spontaneous smile, when there is appearance of more than three millimeters of gingiva, making disharmonious the relation between the proportion of teeth, lips and gums. In these cases, the treatment would be the clinical crown increase, followed or not by the application of botulinum toxin type A, depending on the diagnosis. Tools such as Digital Smile Design and Cone Beam Computed Tomography with lip retractors are current features that enable good and complete planning of gummy smile cases. By associating both techniques in the planning, gingival smile correction treatment becomes simpler and predictable. Objectives: The present study aimed to report a clinical case of gummy smile of multiple etiology, in which planning and treatment were multidisciplinary. Case Report: A 19-year-old female patient attended the Dental Clinic of Faculdades Integradas Apari?cio Carvalho - FIMCA, with complaint of gummy smile, and disproportion of crown size of the upper central incisors. Anamnesis, clinical examination and periodontal examination, computed tomography and digital smile planning were performed. After careful planning, clinical crown augmentation surgery and the application of botulinum toxin were instituted. Patient reported satisfaction with treatment and new smile. Conclusion: When it comes to smile aesthetics, good planning is essential. Digital Smile Design and Cone Beam Computed Tomography with lip retractors are fundamental tools for gingival smile cases. Opting for multidisciplinary treatment, evaluating orofacial aesthetics will always be safer and will bring accurate, predictable and satisfactory results.

To develop a brief, culturally appropriate screening tool for identifying late life depression (LLD), for use by non-specialist clinicians in primary and out-patient care settings in sub-Saharan Africa (SSA). Depressive disorders are a leading contributor to the global health burden. LLD is common and cases will increase as populations' age, particularly in low- and middle-income countries (LMICs), such as those in SSA. A chronic mental health workforce shortage and the absence of culturally adapted LLD screening tools to aid non-specialist clinicians have contributed to a significant diagnostic gap. A systematic random sample of older people attending general medical clinics were interviewed using a 30-item LLD questionnaire, developed utilizing a Delphi consensus analysis of items from the Geriatric Depression Scale, Patient Health Questionnaire-2 and questions developed from a study of lay conceptualisations of depression in Tanzania. The items were assessed for validity against blinded DSM 5 diagnosis of depression by a research doctor. Factor and item analysis were then used to refine the questionnaire. The 12-item Maddison Old-age Scale for Identifying Depression (MOSHI-D) was developed. It has good internal consistency (Cronbach's α = 0.820) and construct and criterion validity (AUROC = 0.880). On initial evaluation, the MOSHI-D showed good internal validity. It should be easy for non-specialists to administer. External validation and further refinement will be conducted. A culturally-appropriate LLD screen may improve mental health care integration into existing healthcare settings within SSA and facilitate greater patient access to care, in accordance with current WHO strategy. Background: The majority of people with dementia live in low-and middle-income countries (LMICs). In sub-Saharan Africa (SSA) human-resource shortages in mental health and geriatric medicine are well recognized. Use of technological solutions may improve access to diagnosis. We aimed to assess the diagnostic accuracy of a brief dementia screening mobile application (app) for non-specialist workers in rural Tanzania against blinded gold-standard diagnosis of DSM-5 dementia. The app includes 2 previously-validated culturally appropriate low-literacy screening tools for cognitive (IDEA cognitive screen) and functional impairment (abbreviated IDEA-IADL questionnaire). Methods: This was a 2-stage community-based door-to-door study. In Stage1, rural primary health workers approached all individuals aged ≥60 years for app-based dementia screening in 12 villages in Hai district, Kilimanjaro Tanzania. In Stage 2, a stratified sub-sample were clinically-assessed for dementia blind to app screening score. Assessment included clinical history, neurological and bedside cognitive assessment and collateral history. Results: 3011 (of 3122 eligible) older people consented to screening. Of these, 610 were evaluated in Stage 2. For the IDEA cognitive screen, the area under the receiver operating characteristic (AUROC) curve was 0.79 (95% CI 0.74-0.83) for DSM-5 dementia diagnosis (sensitivity 84.8%, specificity 58.4%). For those 358 (44%) completing the full app, AUROC was 0.78 for combined cognitive and informant-reported functional assessment. Conclusions: The pilot dementia screening app had good sensitivity but lacked specificity for dementia when administered by non-specialist rural community workers. We varied the presence or absence of tooth preparation, made with a cilindric diamond bur and two distinct protocols for finishing and polishing, as follows: Protocol 1, finish with a diamond bur and abrasive rubber and polishing with abrasive rubbers, and Protocol 2 , finishing and polishing with abrasive rubbers. Images generated by scanning electron microscopy were performed, and arranged in boards to be analyzed by the evaluators by scores under the following parameters: presence of cracks, marginal regularity, surface and marginal adaptation of the restoration. The data obtained from the qualitative analyzes of the images were statistically analyzed using the Kruskal-Wallis test with a significance level of 5%. The data obtained from the measurement of time (seconds) required for the evaluation of two protocols for finishing and polishing were statistically analyzed by Student's t test with significance level of 5%. After analyzing the results, it was concluded that there was no influence of the preparation of the enamel (P>0,05). There was not a statistically signifcant difference. As there was no statistically significant difference between the two finishing and polishing protocols according to the parameters evaluated (P>0,05). However, it is clear that there was a statistically significant difference between the time required for performance of the two protocols (P0,05), não apresentando diferença estatisticamente significantiva. Assim como não houve diferença estatisticamente significativa entre os dois protocolos de acabamento e polimento quanto aos parâmetros avaliados (P>0,05). The aim of this study was to characterize and analyze the microstructure, composition and mechanical properties of these materials, testing the compatibility of the systems and the assumption that they all have excellent properties. This study characterized and compared the In-Ceram ceramic systems (Vita Zahnfabrik, Bad Säckingen, Germany), Vitro-Ceram (Angelus, Londrina, Brazil) and Alglass (EDG, São Carlos, Brazil) using the following methods: 1 - Analysis of the vitreous phase by X-ray fluorescence; 2 - Analysis of the particle size of the crystalline phase and the vitreous phase; 3 - Evaluation of resistance at the equibiaxial test; 4 - Analysis of the microstructure by scanning electron microscopy (SEM). The RX fluorescence analysis of the glass phase of the three systems showed that their compositions in relation to their main elements, such as lanthanum (La2O3), aluminum (Al2O3) and silicon (SiO2), are similar, differing in the additives which serves to give color and other settings. According to the results, it can be seen that Vitro-Ceram and In-Ceram have similar particle size, and the alumina from the Alglass system showed a more heterogeneous composition with grains larger than 10 µm and a large amount of grains below 0,6 µm. This allows a larger agglomeration of the grains, forming a more compact ceramic structure after pre-sintering, which creates pores of small diameter and without interconnection. This fact undermines vitreous infiltration in the final ceramic composite, which can result in a material with more defects and a reduction of the mechanical properties. Also, the glass was also characterized, which showed no similarity between the three systems, but its high grain facilitates its handling and the overlap over the structure of pre-sintered alumina, not interfering in their infiltration. In the bending test, the groups composed by the alumina present at the In-Ceram system had the highest average, being statistically different from the other groups. Despite showing higher values of resistance, the groups with the alumina present at the Vitro-Ceram system did not statistically differ from the groups of Alglass system s alumina. The infiltration of different glasses in the same alumina did not statistically influenced the mechanical strength of the materials. However, groups using the glassy phase of Alglass showed higher resistance. In the fractographic validation analysis none of the samples had to be discarded because of its fracture failure, in which 70% of the specimens of the three studied ceramic systems had low-power fractures, 30% suffered fractures of average strength and no sample suffered a high-strength fracture. Lower values found for the Vitro-Ceram ceramic system can be justified by the presence of much larger blisters and pores inside the material, thus, this feature translates into loss of mechanical strength of the ceramic. As much as all ceramic systems are suitable for clinical use, the In-Ceram material stands out for its features and reliability. A realização deste trabalho se deu justamente pela pouca informação sobre sistemas cerâmicos amplamente utilizados no mercado odontológico, sendo o objetivo deste estudo caracterizar e analisar a microestrutura, a composição e as propriedades mecânicas destes materiais, testando a compatibilidade entre os sistemas, além da hipótese de que todos eles apresentam ótimas propriedades. Este estudo caracterizou e comparou os sistemas cerâmicos In-Ceram (Vita Zahnfabrik, Bad Säckingen, Alemanha), Vitro-Ceram (Angelus, Londrina, Brasil) e Alglass (EDG, São Carlos, Brasil) utilizando as seguintes metodologias: 1 Análise da fase vítrea por fluorescência de raios-X; 2 Análise da granulometria da fase cristalina e da fase vítrea; 3 Avaliação da resistência ao ensaio equibiaxial; 4 Análise da microestrutura através de microscopia eletrônica de varredura (MEV). A análise por fluorescência de rx da fase vítrea dos três sistemas mostrou que as suas composições em relação aos elementos principais, como o lantânio (La2O3), alumínio (Al2O3) e silício (SiO2), são semelhantes, diferindo nos aditivos que servem para dar cor e outros ajustes. De acordo com os resultados da granulometria, pode-se observar que o In-Ceram e o Vitro-Ceram apresentam tamanhos de partículas semelhantes, e a alumina do sistema Alglass mostrou uma composição mais heterogênea com grãos maiores que 10 µm e uma grande quantidade de grãos inferiores a 0,6 µm. Isso permite uma maior aglomeração dos grãos, favorecendo a formação de uma estrutura cerâmica mais compacta após a pré-sinterização, o que gera poros de pequeno diâmetro e sem interconexão. Este fato prejudica a infiltração vítrea no compósito cerâmico final, o que pode resultar em um material com mais defeitos e uma redução nas propriedades mecânicas. Além disso, caracterizou-se também o vidro, que não apresentou semelhança entre os três sistemas, porém a sua granulometria elevada facilita seu manuseio e a justaposição sobre as estruturas de alumina pré-sinterizadas, não interferindo na sua infiltração. No ensaio de flexão, os grupos compostos pela alumina do sistema In-Ceram apresentaram as maiores médias, se diferenciando estatisticamente dos demais grupos. Os grupos com a alumina do sistema Vitro-Ceram, por mais que tenham apresentado valores maiores de resistência, não se diferenciaram estatisticamente com relação aos grupos da alumina do sistema Alglass. A infiltração de vidros diferentes em uma mesma alumina não influenciou estatisticamente a resistência mecânica dos materiais. Porém, os grupos que utilizaram a fase vítrea do Alglass apresentaram os maiores valores de resistência. Na análise de validação fractográfica nenhuma das amostras teve de ser descartada por falha na fratura, nos quais 70% dos corpos de prova dos três sistemas cerâmicos estudados tiveram fraturas de baixa força, 30% sofreram fraturas de média força e nenhuma amostra sofreu uma fratura de alta força. Os valores mais baixos encontrados para o sistema cerâmico Vitro-Ceram podem ser justificados pela presença muito maior de bolhas e poros na parte interna do material, consequentemente, esta característica se traduz em perda de resistência mecânica da cerâmica. Por mais que todos os sistemas cerâmicos estejam aptos para utilização clínica, o material In-Ceram de destaca pelas suas características e confiabilidade.
