In a contemporary scenario of globalization of assets, services and capital, followed by a notable fiscal decentralization and loss of nation-state hegemony, a considerable international participation of the subnational governments of countless countries has been clearly observed, developing the phenomenon of Paradiplomacy. Based on the analysis of the Brazilian paradiplomatic context, it can be observed that the increasing participation of the Federative Units (States and Municipalities) in international economic relations in search of financing for their investments is significant. The objective of this study is to model the credit disbursement of R$ 52 billion from international agencies to Brazilian state governments from 2009 to 2014. The model proposed by Matos and de Jesus Filho (2018) was designed to take into account the role of substitution variables of Paradiplomacy - export and import, both as a ratio of GDP - as causal causers useful to explain the credit heterogeneity in both cross state data and time series. The findings, based on a dynamic balanced panel, suggest that the need for state financing by these financial institutions does not exhibit inertial or explosive behavior. This model allows us to identify the role played by the Gini coefficient and the impact of a positive change in the subsequent state tax rating, which leads to a 1.25% increase in its debt capacity. A positive import elasticity of 0.5 was also measured, while the export elasticity was -0.3. Em um cenário contemporâneo, de globalização de bens, serviços e capitais, seguidos de notória descentralização fiscal e perda da hegemonia do Estado-Nação, tem se observado com clareza uma considerável participação internacional dos governos subnacionais de inúmeros países, desenvolvendo o fenômeno da Paradiplomacia. Desta feita, tendo como base de análise o contexto paradiplomático brasileiro, nota-se que a participação crescente por parte das suas Unidades Federativas (Estados e Municípios) nas relações econômicas internacionais em busca de financiamentos para os seus investimentos é significativa. O objetivo desse estudo é modelar o desembolso de crédito de R$ 52 bilhões de agências internacionais para governos estaduais brasileiros no período de 2009 a 2014. Buscou-se ampliar o modelo proposto por Matos e de Jesus Filho (2018) para levar em conta o papel de variáveis substitutas de Paradiplomacia - exportação e importação, ambas como uma razão do PIB - como causadores de causalidade úteis para explicar a heterogeneidade do crédito em ambos os dados de estado cruzado e séries temporais. As descobertas realizadas, baseadas em um painel balanceado dinâmico, sugerem que a necessidade de financiamento dos estados por essas instituições financeiras não apresenta comportamento inercial nem explosivo. Esse modelo nos permite identificar o papel desempenhado pelo coeficiente de Gini e o impacto de uma mudança positiva no subsequente rating fiscal estadual, o que leva a um aumento de 1,25% em sua capacidade de endividamento. Também foi medida uma elasticidade de importação positiva de 0,5, enquanto a elasticidade de exportação é de -0,3.

Background. Renal failure (RF) is reported to occur in 11-49% of the patients with decompensated end-stage liver disease (ESLD) and has been associated with increased mortality, particularly in the occurrence of hepatorenal syndrome (HRS) type 1. Aims. To evaluate the frequency and outcome of RF in patients admitted to the hospital due to decompensated ESLD and to assess the impact of the underlying cause of RF on survival. Material and methods. Four hundred and six patients (65% males, mean age 62 ± 12 years) with decompensated ESLD were evaluated for the occurrence of RF (defined as serum creatinine ³ 1.5 mg/mL). The underlying cause of RF was reckoned in each subject and compared to outcome. Results. Renal failure was observed in 39% of the patients at admission and in 10% of the subjects during hospitalization. Mortality was significantly higher in subjects with RF (26 vs. 1%, p < 0.000001). Hypovolemia, bacterial infections, parenchymal kidney diseases and HRS were identified as causes of RF in, respectively, 40, 32, 15 and 12% of the cases. Mortality was significantly higher in those subjects with HRS type 1 and bacterial infections, when compared to other causes of RF. Conclusions. Renal failure occurs in nearly half of the patients with decompensated ESLD. It is most commonly caused by hypovolemia and bacterial infections. Occurrence of RF has an adverse impact in patient survival, particularly in those subjects with bacterial infections and HRS type 1, prone to develop progressive renal dysfunction despite intensive medical care.

The immunological nuances resulting from the immunosenescence phenomenon may promote significant alterations in the clinical course of HIV infection, as well as in treatment response. The major purpose of this Thesis was to evaluate the impact of age on the functional immune recovery in aids patients aged more than 55 years, when adequately treated with anti-retroviral therapy, characterizing the immunological result of advanced age and HIV infection. Thus, four experimental groups were enrolled: healthy or HIV-infected young adults, and healthy or HIV-infected adults over 55 years old. All the HIV-infected patients had diagnosis of aids and were under anti-retroviral treatment with therapeutic success. In the first presented article, we evaluated the lymphoproliferative response and cytokine production in vitro and humoral response in vivo, after antigenic challenge with tetanus toxoid (TT) in previously immunized individuals against tetanus. The results revealed significant age-related immunological impairments concerning anti-TT IgG production, lymphoproliferative response and production of IFN-γ. On the other hand, the production of IL-10 significantly higher in individuals aged more the 55 years, HIV-infected or not. In the second article, T cell subsets were characterized after polyclonal activation or specific stimulus with antigens derived from the HIV envelope (Env). In fresh unstimulated PBMC cultures obtained from the aged aids patients, there was a reduced frequency of naïve and central memory T cells, associated with increased frequency of effector T cells. When polyclonally stimulated, these cultures showed deficient production of IFN-γ and hyperproduction of IL-10, like in response to TT. In Env-stimulated cultures, flow cytometry revealed high frequency of T CD4+FoxP3-CD152+ T cells with strong intracellular staining for IL-10, indicating a dominant Tr-1 phenotype, and not the classical Treg cells. Interestingly, in both articles, the viral replication in vitro was significantly lower aids patients over 55 years old, which is in consonance with their excellent virological response to anti-retroviral treatment. IL-10 neutralization with anti-IL-10 antibody in Env-activated cultures enhanced the viral replication in culture supernantants. Both in TT and in Env-peptides-stimulated cultures, the IL-10 blockade enhanced the levels of pro-inflammatory cytokines, but it did not improve IFN-γ production from aged aids patients. Altogether, the results reported in this Thesis reveal disturbances in several segments of the immune response, particularly in the Th1 compartment, of anti-retroviral-treated aids patients older than 55 years, suggesting that, for these patients, immune reconstitution after treatment does not occur with the same efficacy as in young patients. And although the enhanced IL-10 production probably contributes, at least in part, to the virological control, it can compromise the immune response both to HIV and to other antigenic challenges, such as tetanus toxoid. It is suggested, therefore, the need for specific recommendations regarding the clinical management of these patients A proporção de idosos portadores da síndrome da imunodeficiência adquirida (aids) tem aumentado de maneira importante nos últimos anos e, até a presente data, existem poucos estudos que abordam a infecção nessa população especial. As particularidades imunológicas decorrentes do fenômeno da imunossenescência podem acarretar mudanças significativas na evolução da infecção pelo HIV, bem como na resposta ao tratamento. O objetivo maior desta Tese foi avaliar o impacto da idade na recuperação funcional do sistema imune de pacientes com aids acima de 55 anos, quando tratados adequadamente com terapia anti-retroviral, caracterizando a resultante imunológica da idade avançada e da infecção pelo HIV. Para tanto, foram estudados quatro grupos experimentais: indivíduos jovens saudáveis ou com aids, e indivíduos acima de 55 anos saudáveis ou com aids. Todos os pacientes com aids estavam recebendo terapia anti-retroviral, em sucesso terapêutico. No primeiro artigo apresentado, avaliamos resposta linfoproliferativa e produção de citocinas in vitro e resposta humoral in vivo mediante desafio antigênico com toxóide tetânico (TT) em indivíduos previamente vacinados contra o tétano. Os resultados mostraram deficiências imunológicas significativas relacionadas à idade avançada no que diz respeito a produção de IgG anti-TT, resposta linfoproliferativa e produção de IFN-γ. Em contrapartida, a produção de IL-10 foi significativamente maior nos indivíduos acima de 55 anos, infectados ou não pelo HIV. No segundo artigo, foram caracterizadas as subpopulações de células T mediante estímulo policlonal ou específico com antígenos do envelope do HIV (Env). Em culturas não-estimuladas de PBMC do grupo com aids e idade avançada, observamos frequência reduzida de células T naive e de memória central, associada a aumento de células T efetoras. Quando estimuladas policlonalmente, essas culturas apresentaram deficiência na produção de IFN-γ e hiperprodução de IL-10, como na resposta ao TT. Mediante estímulo específico com Env, a citometria de fluxo revelou frequência elevada de células T CD4+FoxP3-CD152+ com forte marcação intracelular para IL-10, indicando predomínio do fenótipo Tr-1, e não das células Treg clássicas. Interessantemente, em ambos os artigos, a replicação viral in vitro foi significativamente menor nos pacientes com aids acima de 55 anos, condizendo com a excelente resposta virológica desses pacientes ao tratamento antirretroviral. A neutralização da IL-10 com anticorpo anti-IL-10 nas culturas ativadas pelos peptídeos Env aumentou de forma significativa a replicação viral no sobrenadante. Tanto na resposta ao TT quanto aos peptídeos Env, o bloqueio da IL-10 aumentou os níveis de citocinas pró-inflamatórias, mas não melhorou a produção de IFN-γ dos pacientes acima de 55 anos com aids. Coletivamente, os achados dessa Tese revelam distúrbios em vários segmentos da resposta imune, particularmente no compartimento Th1, de pacientes acima 55 anos com aids e adequadamente tratados, sugerindo que, para esses pacientes, a reconstituição imune pós-tratamento não ocorre com a mesma eficácia que no jovem. Apesar do aumento da produção de IL-10 provavelmente contribuir, ao menos em parte, para o controle virológico, pode comprometer a resposta tanto ao próprio HIV, quanto a outros desafios antigênicos, a exemplo do toxóide tetânico. Sugere-se, portanto, a necessidade de recomendações específicas de manejo clínico para esse grupo de pacientes

O planejamento de medidas preventivas requer estudos epidemiológicos em diferentes áreas geográficas, os quais ainda são insuficientes para a definição de ocorrência, distribuição e fatores de risco da doença. Este trabalho descreve o perfil epidemiológico do câncer infantil em uma unidade de oncologia pediátrica de Salvador. Foram tomados para o estudo os prontuários dos pacientes admitidos na instituição no período de 1995 a 2003. Dos 465 pacientes estudados, 57,8% eram do sexo masculino, 50,8% eram pardos e 40,1% eram naturais da Região Metropolitana de Salvador. Os tipos de câncer mais freqüentes foram as leucemiùs, seguidas pelos linfomas e osteossarcomas, 39,5%, 18,9% e 9,9% respectivamente. Como principais causas de óbito, destacaram-se, em ordem decrescente: leucemias, osteossarcomas, linfomas e neuroblastoma. A sobrevida global livre de doença no período representou 51% da população estudada, o número de óbitos 38,3% e o número de abandonos de tratamento, 2,4%. Em 21% dos casos, havia relatos de outros casos de câncer na família e em 4% foi registrada exposição anterior a algum tipo de agente reconhecidamente tóxico. No entanto, a análise de possíveis fatores de risco associados ao desenvolvimento de neoplasias foi prejudicada em virtude da falta de registro desses aspectos na maioria dos prontuários analisados. São necessários novos estudos sobre o tema, que possam contribuir para a epidemiologia do câncer infantil e associar possíveis fatores de risco em áreas geográficas específicas. Salvador

Nesta entrevista, Regis de Castro Andrade conta sua experiência na famosa greve de Osasco de 1968, quando era membro do POC e lá trabalhava auxiliando a organização das bases operárias. Realça sua visão de que o movimento teve seus fundamentos na própria organização interna dos operários, com a ajuda dos estudantes, em contraposição a outras que vêem aquele movimento como fruto da articulação do movimento estudantil. Conta também algumas de suas experiências de clandestinidade, prisão e exílio nos anos que se seguiram, até a reorganização do movimento operário no fim da década de 70. In this interview, Regis de Castro Andrade tells us about his experience in the famous strike in Osasco in 1968, in wich he was a member who helped in the organization of the moviment. He enfasizes his point of view that the moviment originated from an internal organization among the employees, who were only helped by the students. This idea is a contrary idea if compared to the statements wich asure that the movement was completelly organized by the students. Andrade also tells us about his experiences in the clandestinity, prison and expatriation wich last until the end of 1970’s.
