To analyze the proteomic profile of salivary pleomorphic adenoma (PA) and carcinoma ex pleomorphic adenoma (CXPA) samples and correlate them with the malignant transformation of the PA.Thirty samples (10 PA, 16 CXPA, and 4 residual PA) were microdissected and submitted to liquid chromatography-tandem mass spectrometry (LC-MS/MS). The proteomic data and protein identification were analyzed through LC-MS/MS spectra using the MaxQuant software.The proteomic analysis identified and quantified a total of 240 proteins in which 135 were found in PA, residual PA, and CXPA. The shared proteins were divided into six subgroups, and the proteins that showed statistically significant differences (p 0.05) and fold-changeor2.5 in one subgroup to another subgroup were included. Seven proteins (Apolipoprotein A-I-APOA1, haptoglobin-HP, protein of the synaptonemal complex 1-SYCP1, anion transport protein of band 3-SLC4A1, subunit μ1 of AP-1 complex-AP1M1, beta subunit of hemoglobin-HBB, and dermcidin-DCD) were classified as potential protein signatures, being HP, AP1M1, and HBB with higher abundance for PA to residual PA, APOA1 with higher abundance for PA to CXPA, SLC4A1 with lower abundance in the PA to CXPA, SYCP1with lower abundance for residual PA to CXPA, and DCD with higher abundance in the CXPA with epithelial differentiation to myoepithelial differentiation.In this work, we demonstrated the comparative proteomic profiling of PA, residual PA, and CXPA, and seven were proposed as protein signatures, some of which may be associated with the malignant phenotype acquisition.

O carcinoma ex-adenoma pleomorfo (CXAP) e uma neoplasia agressiva, cuja patogenese ainda nao esta esclarecida. Nos ultimos anos, muitos estudos sugeriram que a transformacao maligna estava relacionada com o acumulo de alteracoes geneticas em um adenoma pleomorfo (AP) pre-existente. Neste trabalho, analisamos os genes que se encontraram amplificados pela tecnica de Hibridizacao Genomica Comparativa em microarranjo (array Comparative Genomic Hibridization - CGH array) em amostras de AP residual e CXAP em diferentes fases da carcinogenese (intracapsulares, minimamente invasivos e francamente invasivos) subdivididos quanto ao subtipo histologico.

Tertiary hyperparathyroidism, an autonomous hyperproduction of parathyroid hormone (PTH), has a challenge in its treatment. This study asked whether the intraoperative PTH and calcium drop can confirm the resection of all parathyroid tissues.Case series with planned data collection.Tertiary referral medical center.The study assessed patients with tertiary hyperparathyroidism who were treated at the Hospital of the State University of Campinas from 2007 to 2015. All patients underwent total parathyroidectomy with autotransplantation of parathyroid fragments. PTH and calcium were collected during the preoperative period; at 10, 20, and 240 minutes after resection of the glands; and at 1 year after the procedure. Data were analyzed by analysis of variance and logistic regression analysis with statistical values ofThirty-five patients were assessed: 17 women (48.57%) and 18 men (51.43%). The percentage of PTH drop was statistically significant at all times, unlike the calcium analysis, but only PTH collected at 20 minutes was able to confirm the removal of all parathyroid tissues (Treatment success can be predicted by analyzing the decrease of intraoperative PTH and not by calcium. The 71.2% PTH drop at 20 minutes after parathyroidectomy had high sensitivity and specificity to predict surgical cure.

PLAG1 (pleomorphic adenoma gene 1) is frequently activated in pleomorphic adenoma (PA). Carcinoma ex pleomorphic adenoma (CXPA) arises in PA, and PLAG1 expression is believed to be maintained from PA to CXPA, as it can contribute to the carcinogenesis process. To evaluate if PLAG1 is a good marker of malignant transformation from PA to CXPA as well as to evaluate if PLAG1 expression is associated with progression and histopathologic subtype of CXPA. Forty PAs, 21 residual PAs (without malignant transformation), and 40 CXPAs were analyzed by immunohistochemistry with PLAG1 antibody. The proportion of positive neoplastic cells was assessed according to a 2-tiered scale: >10% to 50%, and >50% positive cells. The CXPA group was classified according to histopathologic subtype and invasiveness degree. Thirty-seven PAs (92.5%), 15 residual PAs (71%), and 14 CXPAs (35%) were positive for PLAG1. In relation to the CXPA group, among the intracapsular cases, myoepithelial carcinoma and epithelial-myoepithelial carcinoma showed the highest level of PLAG1 expression. PLAG1 expression is lost when PA undergoes malignant transformation, possibly due to other pathway activation and different clone cells. In addition, PLAG1 expression seems to be present mainly in low-grade carcinomas and in cases with early phase of invasion, due to its regulation of oncogene-induced cell senescence. In CXPA, PLAG1 expression was most associated with myoepithelial differentiation. This way, loss of PLAG1 expression can be considered a hallmark of CXPA carcinogenesis, mainly when there is only epithelial differentiation.

Este trabalho apresenta um novo modelo numérico para simulação de juntas de argamassa em estruturas de alvenaria no plano via método dos elementos finitos. Neste modelo, blocos de alvenaria e juntas de argamassa são representados separadamente. Elementos finitos com alta razão de aspecto são utilizados para representar as juntas de argamassa e são inseridos na malha de elementos finitos através de uma técnica de fragmentação de malha. A principal vantagem desta técnica consiste na utilização de modelos constitutivos contínuos para representar regiões descontínuas, uma vez que seu campo de deformações quando a altura do elemento de interface tende a zero é semelhante ao apresentado pela abordagem de aproximação contínua de descontinuidades fortes. Um modelo constitutivo contínuo baseado na mecânica do dano foi desenvolvido para representar o comportamento dos elementos de interface. Este modelo consegue representar a abertura e fechamento de fraturas, bem como o efeito de atrito em função da tensão de confinamento nas interfaces. Como o objetivo deste trabalho consiste na simulação da formação e propagação de fraturas ao longo das juntas de argamassa, comportamento elástico linear foi atribuindo aos elementos triangulares de três nós utilizados na discretização dos blocos de alvenaria. Vários exemplos numéricos são apresentados. Inicialmente, testes básicos são realizados para demonstrar as principais características do modelo quando submetido a carregamentos de tração, compressão e cisalhamento. Posteriormente, estruturas de alvenaria submetidas a carregamentos estáticos são analisadas e os resultados comparados com as respostas experimentais a fim de validar o modelo proposto. A técnica proposta se mostrou bastante promissora para simulação da formação e propagação de fratura em juntas de argamassa de estruturas de alvenaria. This work presents a novel numerical model to simulate the failure process in masonry structures subjected to static loads via finite element method. Brick and mortar joints are modeled separately with their own constitutive equations. Interface finite element with high aspect ratio are used to simulate the mortar interface and inserted by the mesh fragmentation technique. The main advantage of this strategy is supported by the fact that, as the aspect ratio of a standard low-order solid finite element increases, the element strains also increase, approaching the same kinematics as the Continuum Strong Discontinuity Approach. A constitutive model was developed, based on the continuum damage mechanics, in order to represent the behavior of the interface finite elements. This model is able to simulate the creation and propagation of cracks, as well as, the frictional effects in dependence on stress confinement on the interfaces. Furthermore, as the objective of this work aims to simulate the failure in the mortar joints, the brick elements are assumed as linear elastic material. Three node standard triangular finite element are used to represent the bricks. Several numerical models are carried out. Initially, basics tests are show in order to demonstrate the main characteristics of the proposed model subjected to tensile, compression and shear loads. Subsequently, masonry structures are subjected to static loads are analyzed and the results compared with the experimental responses in order to validate the proposed model. This technique proved to be very promising for the simulation of failure onset and propagation in mortar joints of masonry structures.

Introduction The carcinoma ex pleomorphic adenoma (CXPA) arises from a pleomorphic adenoma (PA) and is supposed to gain lipogenesis and glycogenesis during its malignant transformation. Increased lipogenesis is a characteristic of cancer cells, consisting of increased fatty acid synthesis by the enzyme fatty acid synthase (FASN) and by the accumulation of cytoplasmic lipid droplets, which express adipophilin. The increase in glycolytic metabolism has been associated with the activity of glycolytic enzymes and glucose transporters (GLUTs), whose expression is induced by HIF-1 (inducing factor of hypoxia 1). Objectives To analyze the gene expression of FASN, adipophilin, HIF-1α, and GLUT-1 in PA and CXPA samples. Methodology Gene expression analysis of the FASN, adipophilin, HIF1-α, and GLUT-1 genes was performed by the real-time PCR (qPCR) method. Fourteen cases of PA and 14 cases of CXAP were evaluated. Results FASN, adipophilin, HIF-1α, and GLUT-1 were more expressed in CXAP than in AP, although there was no significant difference between levels of adipophilin expression in CXPA and PA. Conclusion Increased expression of FASN, HIF1-α, and GLUT-1 in CXAP may be associated with malignant transformation of PA, while additional studies are needed to understand the role of adipophilin in PA carcinogenesis. Moreover, this is the first study to investigate the role of these genes in the malignant transformation of PA.

Introduction Pleomorphic adenoma (PA) is a benign lesion of the salivary glands that can suffer malignant transformation to carcinoma ex adenoma pleomorphic (CXPA). The pathogenesis of CXPA has been attributed to the accumulation of genetic disorders in preexisting PAs. However, there is no confirmation whether there is a common target gene involved in all histopathological subtypes or the decisive factors for malignant transformation in a histopathological subtype are specific. Objectives To further analyze genes found in PA and CXPA using the CGH-array technique. The genes found were analyzed using the InteractiVenn virtual tool (http://www.interactivenn.net/) and grouped into a Microsoft Excel worksheet. Results: Of the 460 genes amplified in the studied samples, 287 (62.4%) were related only to CXPA, whereas 144 (31.3%) were related to residual PA. Twenty-nine (6.3%) of these genes were common between residual PA and CXPA. Regarding the degree of invasion of CXPA, there was an increase in the number of genes amplified as the degree of invasion and aggression increased: 8 genes related to intracapsular CXPA, 65 to minimally invasive CXPA, and 373 to weakly invasive. Moreover, when comparing residual AP and intracapsular CXPA, two genes were common to these groups: ERRB2 and GRB7. As for the histological subtype, the high-grade samples had more amplifications (320 amplified genes) than the low-grade ones (129 genes). Three of these genes were common among residual PAs and CXPA: HMGA2, RPSAP52, and LOC100129940. As for the replicates, MYNC, ERBB2, BRIP1, and HMGA2 were the most repeated amplified genes in the residual PAs. HMGA2, ERRBB2, CDK12, RPSAP52, LOC100129940, and LOC100507250 were the genes with the most replicates in CXPA. Conclusion HMGA2, ERRB2, and RPSAP52 may play a key role in PA carcinogenesis, whereas GRB7, CDK12, MYNC, and BRIP1 appear to act as coadjutants.

Introduction: A key step of cancer development is the progressive accumulation of genomic changes resulting in disruption of several biological mechanisms. Carcinoma ex-pleomorphic adenoma (CXPA) is an aggressive neoplasm that arises from a pleomorphic adenoma. CXPA derived from a recurrent PA (RPA) has been rarely reported, and the genomic changes associated with these tumors have not yet been studied. Objective: We analyzed CXPA from RPAs and RPAs without malignant transformation using array-comparative genomic hybridization (array-CGH) to identify somatic copy number alterations and affected genes. Methods: DNA samples extracted from FFPE tumors were submitted to array-CGH investigation, and data was analyzed by Nexus Copy Number Discovery Edition v.7. Results: No somatic copy number alterations were found in RPAs without malignant transformation. As for CXPA from RPA, although genomic profiles were unique for each case, we detected some chromosomal regions that appear to be preferentially affected by copy number alterations. The first case of CXPA-RPA (frankly invasive myoepithelial carcinoma) showed copy number alterations affecting 1p36.33p13, 5p and chromosomes 3 and 8. The second case of CXPA-RPA (frankly invasive epithelial-myoepithelial carcinoma) showed several alterations at chromosomes 3, 8, and 16, with two amplifications at 8p12p11.21 and 12q14.3q21.2. The third case of CXPA-RPA (minimally invasive epithelial-myoepithelial carcinoma) exhibited amplifications at 12q13.3q14.1, 12q14.3, and 12q15. Conclusion: The occurrence of gains at chromosomes 3 and 8 and genomic amplifications at 8p and 12q, mainly those encompassing the HMGA2, MDM2, WIF1, WHSC1L1, LIRG3, CDK4 in CXAP from RPA can be a significant promotional factor in malignant transformation. Resumo Introdução: Uma etapa fundamental do desenvolvimento do câncer é o acúmulo progressivo de alterações genômicas, resultando na ruptura de vários mecanismos biológicos. Carcinoma ex-adenoma pleomórfico (CXAP) é uma neoplasia agressiva que surge a partir de um adenoma pleomórfico. O CXAP derivado de um AP recorrente (APR) foi raramente relatado e, até o momento, as alterações genômicas associadas a esses tumores não foram estudados. Objetivo: Avaliar as diferenças entre os CXAPs decorrentes de APRs e os APRs sem transformações malignas usando hibridização genômica comparativa em microarrays (array Comparative Genomic Hibridization - aCGH) a fim de identificar alterações no número de cópias somáticas e os genes afetados. Método: Amostras de DNA extraídas de tumores provenientes de tecido emblocado em parafina foram submetidos à investigação com a técnica aCGH, e os dados foram analisados com o Nexus Copy Number Discovery Edition v.7. Resultados: Não observamos alterações no numero de cópias somáticas nos APRs sem transformação maligna. Quanto ao CXAP de APR, embora os perfis genômicos sejam exclusivos para cada caso, detectamos algumas regiões cromossômicas que pareciam ser preferencialmente afetadas por alterações no número de cópias. O primeiro caso de CXAP-APR (carcinoma mioepitelial francamente invasivo) apresentou alterações no numero de cópias afetando 1p36.33p13, 5p e cromossomos 3 e 8. O segundo caso de CXAP-APR (carcinoma epitelialmioepitelial francamente invasivo) apresentou várias alterações nos cromossomos 3, 8 e 16, com duas amplificações em 8p12p11.21 e 12q14.3q21.2. O terceiro caso de CXAP-APR (carcinoma epitelial-mioepitelial minimamente invasivo) apresentou amplificações em 12q13.3q14.1, 12q14.3, e 12q15. Conclusão: A ocorrência de ganhos de cromossomos 3 e 8, e as amplificações genômicas em 8p e 12q, principalmente aquelas que englobam os HMGA2, MDM2, WIF1, WHSC1L1, RG3, CDK4 no CXAP decorrente de APR podem ser fatores promocionais significativos para a transformação maligna.

PURPOSE: To investigate the carcinogenic action of hydrochloric acid, pepsin and sodium nitrate on the oropharyngeal mucosa of rats, simulating the reflux of gastric contents. METHODS: Eighty-two Wistar rats were divided in seven groups and submitted to 2 or 3 weekly applications of hydrochloric acid, pepsin and sodium nitrate on the pharyngeal mucosa during six months. Study groups comprised 12 animals each. Rats in groups I and II were submitted to 2 (GI) or 3 (GII) weekly applications of 0.1N hydrochloric acid. Groups III and IV were submitted to 2 (GIII) or 3 (GIV) weekly applications of 0.1N hydrochloric acid solution with pepsin. Groups V and VI were submitted to 2 (GV) or 3 (GVI) weekly applications of 0.1N hydrochloric acid and treated with daily nitrate diluted in water. Group VII consisted of 10 animals submitted to 2 weekly applications of filtered water. RESULTS: No dysplasia, intra-epithelial neoplasia or invasive carcinomas were detected. Inflammatory changes were observed in varying degrees and mast cells were more common in Groups V and VI (p=0.006). CONCLUSION: The data of the current study could not corroborate the hypothesis that gastroesophageal and pharyngolaryngeal refluxes are carcinogenic factors to the laryngopharyngeal mucosa, and more studies are necessary in the future. OBJETIVO: Investigar a ação carcinogênica do ácido clorídrico, pepsina e nitrato de sódio na mucosa orofaríngea de ratos, simulando o refluxo do conteúdo gástrico à mucosa do faringo-laringea. MÉTODOS: Oitenta e dois ratos Wistar foram divididos em 7 grupos e submetidos a 2 ou 3 aplicações semanais de ácido clorídrico, pepsina e nitrato de sódio na mucosa orofaríngea durante 6 meses. Os grupos de estudo envolveram 12 animais cada. Os ratos nos grupos I e II foram submetidos à 2 (GI) ou 3 (GII) aplicações semanais de ácido clorídrico 0,1N. Nos grupos III e IV foram 2 (GIII) ou 3 (GIV) aplicações semanais de ácido clorídrico e pepsina. Nos grupos V e VI foram 2 (GV) ou 3 (GVI) aplicações semanais de ácido clorídrico além da oferta de nitrato diluído em água diariamente. Grupo VII era composto por 10 animais submetidos a 2 aplicações semanais de água filtrada. RESULTADOS: Não se observou displasia, neoplasia intra-epitelial ou neoplasia invasora. Alterações inflamatórias em graus variados foram observadas, com infiltrado mastocitário mais intenso nos grupos V e VI. (p=0,006). CONCLUSÃO: Os dados do presente estudo não confirmam a hipótese que o refluxo gastro-esofágico e faringo-laringeo são fatores carcinogênicos para a mucosa laringo-faringea e mais estudos são necessários no futuro.

Polymorphous low-grade adenocarcinoma (PLGA) is a malignant neoplasm of low aggressiveness that occurs almost exclusively in the minor salivary glands, primarily those in the palate. We report a case of PLGA that arose in the base of the tongue and subsequently metastasized to the neck. The tumor was resected through the oral cavity with wide margins and dissection. The neck metastasis was treated with radical neck dissection and radiotherapy. The patient recovered and remained disease-free at follow-up 30 months later. This case shows that PLGA, which has a variable morphologic appearance, can occur at sites other than the salivary glands.

O uso crônico da fenitoína ou intoxicação aguda por essa droga produzem lesão cerebelar permanente com atrofia do vermis e hemisférios cerebelares, que pode ser evidenciada através de exames de neuroimagem. O objetivo deste estudo foi avaliar a correlação entre a dosagem e o tempo de uso da fenitoína com a ocorrência de atrofia cerebelar. Foram realizados levantamento de prontuários para a obtenção de dados clínicos e análise de tomografias de crânio para avaliação de atrofia cerebelar. Dos 66 pacientes estudados, 18 apresentaram atrofia moderada a severa, 15 atrofia leve e 33 foram considerados normais. Os pacientes com atrofia cerebelar moderada a severa foram aqueles com maior exposição à fenitoína (uso prolongado e dose total), apresentando diferença estatisticamente significativa se comparados aos pacientes com atrofia leve ou sem atrofia (p=0.02). Além disso, no subgrupo de pacientes em uso de fenitoína, aqueles com atrofia moderada a severa possuíam níveis séricos de fenitoína significativamente mais elevados que os pacientes com atrofia leve ou sem atrofia (p=0.008). Não houve relação entre duração do tratamento e dose de outros anticonvulsivantes e presença e grau de atrofia cerebelar. Os pacientes mais velhos apresentaram maior grau de atrofia cerebelar, indicando que o fator idade ou tempo de epilepsia, ou ambos, pode ser importante na determinação de degeneração cerebelar. Concluímos que apesar da possibilidade de lesão cerebelar relacionada a crises epilépticas repetidas, a contribuição da fenitoína pode ser claramente estabelecida como um dos determinantes da atrofia cerebelar, sobretudo naqueles pacientes com altas doses por tempo prolongado e níveis séricos elevados. The chronic treatment with phenytoin or the acute intoxication by this drug may cause permanent cerebellar injury with atrophy of cerebellum vermis and hemispheres, which can be detected by neuroimaging studies. The aim of the present study was to investigate the correlation between the dosage and duration of treatment with phenytoin and the occurrence of cerebellar atrophy. Sixty-six patients were studied and had their tomographies analyzed for cerebellar atrophy. Of the 66 patients studied, 18 had moderate/severe atrophy, 15 had mild atrophy and 33 were considered to be normal. The patients with moderate/severe atrophy were those with higher exposure to phenytoin (longer duration of treatment and higher total dosage) showing statistically significant difference when compared to patients with mild atrophy or without atrophy (p=0.02). Further, the patients with moderate/severe atrophy had serum levels of phenytoin statistically higher than those of patients with mild atrophy or without atrophy (p = 0.008). There was no association between other antiepileptic drugs dosage or duration of treatment and degree of cerebellar atrophy. We also found that older patients had cerebellar atrophy more frequently, indicating that age or duration of the seizure disorder may also be important in the determination of cerebellar degeneration in these patients. We conclude that although there is a possibility that repeated seizures contribute to cerebellar damage, long term exposure to phenytoin, particularly in high doses and toxic serum levels, cause cerebellar atrophy.

CONTEXT AND OBJECTIVE: Schwannomas are benign neoplasms of the peripheral nerves originating in the Schwann cells. According to their cellularity, they can be subdivided into Antoni A or Antoni B types. They are rare and usually solitary, with clearly delimited capsules. They occur in the head and neck region in only 25% of the cases, and may be associated with Von Recklinghausen's disease. The present study retrospectively analyzed some data on this disease in the head and neck region and reviewed the literature on the subject. DESIGN AND SETTING: Retrospective study at Head and Neck Service, Universidade Estadual de Campinas. METHODS: Data on 21 patients between 1980 and 2003 were reviewed. The sites of cervical schwannomas and the intraoperative, histopathological and postoperative clinical status of these cases were studied. Diagnostic methods, type of surgery and association with neurofibromatosis were evaluated. RESULTS: The patients' ages ranged from 16 to 72 years. Four patients had a positive past history of type I neurofibromatosis or Von Recklinghausen's disease. The nerves affected included the brachial and cervical plexuses, vagus nerve, sympathetic chain and lingual or recurrent laryngeal nerve. The nerve of origin was not identified in six cases. Tumor enucleation was performed in 16 patients; the other five required more extensive surgery. CONCLUSION: Schwannomas and neurofibromas both derive from Schwann cells, but are different entities. They are solitary lesions, except in Von Recklinghausen's disease. They are generally benign, and rarely recur. The recommended surgical treatment is tumor enucleation. CONTEXTO E OBJETIVO: Schwannomas são neoplasias benignas dos nervos periféricos, originadas nas células de Schwann, podendo, baseado na sua celularidade, ser subdivididas em Antoni A ou B. São de ocorrência infreqüente, geralmente únicas, com cápsula bem delimitada; ocorrendo na região de cabeça em pescoço em apenas de 25% dos casos, podendo estar relacionadas à doença de Von Recklinghausen. Este estudo objetiva analisar retrospectivamente dados concernentes a esta afecção na região de cabeça e pescoço e rever os dados da literatura. TIPO DE ESTUDO E LOCAL: Estudo retrospectivo realizado no Serviço de Cirurgia de Cabeça e Pescoço, Universidade Estadual de Campinas. MÉTODOS: Revisamos dados de 21 pacientes tratados entre 1980 e 2003. Sítio do tumor, status intra e pós-operatório e laudo histopatológico foram estudados nos schwannomas cervicais. Métodos diagnósticos, tipo de cirurgia e associação com neurofibromatose foram avaliados. RESULTADOS: A idade variou entre 16 e 72 anos. Quatro pacientes tinham história positiva para neurofibromatose tipo I ou doença de Von Recklinghausen. Os nervos afetados incluíram os plexos braquial e cervical, os nervos vago, lingual e recorrente e a cadeia simpática. O nervo de origem não foi identificado em seis casos. Enucleação do tumor foi feita em 16 pacientes; os outros cinco requereram cirurgias mais extensas. CONCLUSÃO: Schwannomas e neurofibromas derivam das células de Schwann, mas são tumores diferentes. São lesões solitárias, exceto na doença de Von Recklinghausen, geralmente benignas e raramente recidivam. O tratamento cirúrgico recomendado é a enucleção do tumor.

OBJECTIVE: to develop an experimental model of exposure to tobacco burning (cigarette) products to assess the effects of its chronic use in relation to cancers of the bladder. METHODS: the animals were chronically exposed to the burning tobacco products in a semi-open chamber to simulate smoking. Thirty young Wistar rats were divided into two groups: one with 20 animals simulating smoking for six months, and ten not exposed control animals for the same period. After exposure by inhalation of cigarette smoke, animals were euthanized and subjected to histopathological study of the bladder wall. RESULTS: no tumor was found but mild and non significant alterations. The studies of hemo-oximetry (carboxyhemoglobin and methemoglobin) and the concentration of carbon dioxide (CO2) confirm that the animals were exposed to high concentrations of tobacco smoke and its derivatives. CONCLUSION: no bladder mucosal neoplasia was found in the pathological study of animals. The developed experimental models were highly efficient, practical and easy to use and can be used in other similar studies to determine the harmful effects caused by smoking. OBJETIVO: desenvolver modelo experimental de exposição aos produtos da queima do tabaco (cigarro) para avaliar os efeitos do seu uso crônico em relação às neoplasias de bexiga. MÉTODOS: Os animais foram expostos cronicamente aos produtos da queima do tabaco em câmara semi-aberta para simular o tabagismo. Trinta ratos jovens da raça Wistar foram distribuídos em dois grupos: um com 20 animais simulando o tabagismo por período de seis meses, e um com dez animais controle sem exposição por igual período. Após exposição por inalação da fumaça do cigarro, os animais foram eutanasiados e submetidos a estudo histopatológico da parede da bexiga. RESULTADOS: Não foi encontrada neoplasia e sim alterações leves e não significativas. Os estudos da hemo-oximetria (carboxiemoglobina e metemoglobina) e da concentração de dióxido de carbono (CO2) confirmam que os animais foram expostos a altas concentrações da fumaça do tabaco e de seus derivados. CONCLUSÃO: No estudo anatomopatológico dos animais não foi encontrada neoplasia na mucosa da bexiga. Os modelos experimentais desenvolvidos foram altamente eficientes, práticos e fáceis de usar podendo ser empregados em outros estudos semelhantes para determinar os efeitos nocivos causados pelo tabagismo.

CONTEXT: Lingual thyroid gland is a rare clinical entity that is caused by the failure of the thyroid gland to descend to a normal cervical location during embryogenesis. The occurrence of an ectopic thyroid gland located at the base of the tongue may cause problems for the patient, with symptoms of dysphagia, dysphonia, upper airway obstruction or even hemorrhage at any time from infancy through adulthood. CASE REPORT: We report on a case of lingual thyroid gland in a 41-year-old female patient. The embryology and diagnosis of ectopic thyroid are discussed and its management is outlined. Features of the diagnostic and therapeutic evaluation are described with attention to the clinical findings, laboratory tests, thyroid scan and computed tomography imaging studies employed in the confirmation of diagnosis and planning of appropriate treatment. The history of the condition is reviewed and a treatment strategy is outlined. Surgical excision of the gland is reserved for cases of gland enlargement that result in compromised airways (dysphagia or dysphonia) or recurrent hemorrhage.

Objective: Evaluate details of neck node metastasis (NM) and mediastinal node metastasis (MM) in carcinomas of the pharyngolaryngoesophageal (PLE) region.Method: Chart review of 44 patients with gastric pull-up for Ca of the PLE junction (cervical esophageal, hypopharyngeal and advanced laryngeal carcinomas). The mediastinal dissection (MD) resected mainly the paratracheal/paraesophageal lymph nodes down to the aortic arch.Results: Fifty neck dissections (ND) were performed in 28 patients (positive nodes in 47.7%). The NM occurred in 22.2%, 60%, 70%, and 66.6% of patients with esophageal, pyriform sinus, retrocricoid, and laryngeal carcinomas, respectively. The number of neck nodes averaged 22.8 to 27.5 nodes per ND (positive in 12.5%) and was concentrated in levels II, III, and IV (84.3%). Extra capsular invasion (IE) occurred in 80.9%. Outros fatores de risco podem estar envolvidos, como o refluxo faringolaríngeo (RFL). O objetivo desta pesquisa foi testar modelos experimentais de simulação do refluxo faringolaríngeo e gastro-esofágico e modelo de exposição à fumaça do cigarro em câmara fechada utilizando ratos Wistar, avaliando a injúria tecidual causada nas mucosas faringolaríngea e esofágica dos animais. Na Fase I, foram empregados cinquenta ratos Wistar divididos em 5 grupos submetidos à simulação do RFL, exposição à fumaça do cigarro, simulação do refluxo gastro-esofágico e simulação combinada do RFL e exposição tabágica, durante 6 meses. Na Fase II, oitenta e dois ratos Wistar foram divididos em 7 grupos e submetidos à simulação do RFL com "aplicações bi e tri-semanais" de ácido clorídrico, pepsina e nitrato de sódio durante 6 meses. Ambas as fases possuíam grupo controle expostos à apenas água. Não se observou displasia ou neoplasia. Other risk factors can be involved, as laryngopharyngeal reflux (LPR). The objective of this study was to test experimental models that simulate laryngopharyngeal and gastroesophageal refluxes and a tobacco exposition chamber, analyzing the tissue injury caused to the pharyngolaryngeal and esophageal mucosa using Wistar rats. In Fase I fifty Wistar rats were divided in five groups submitted to LPR, smoke exposition, gastroesophageal reflux and combined reflux and smoke exposition, during 6 months. In Fase II eighty-two Wistar rats were divided in seven groups and submitted to 2 or 3 weekly applications of hydrochloric acid, pepsin and sodium nitrate during 6 months. Both fases had control groups exposed to water alone. Neither dysplasia nor neoplasia were detected. Inflammatory changes were observed in varying degrees with lymphocytes and mast cells, and focal epithelial hyperplasia was observed. The data of the current study could not corroborate the hypothesis that gastroesophageal and pharyngolaryngeal refluxes are carcinogenic factors to UADT mucosa. Myxomas are benign neoplasms of uncertain origin and etiology. First described by Virchow in 1863, they are derived from primitive mesenchymal structures and feature components of the umbilical cord. More recently, in 1995, Takahashi et al., through extensive research confirmed the fibroblastic and histiocytic origin of the tumor. We present a case in a female infant whose outcome and follow-up are discussed as well as a literature review in order to discuss many features of this rare pathology.

CONTEXT AND OBJECTIVE: Salivary gland tumor management requires long-term follow-up because of tumor indolence and possible late recurrence and distant metastasis. Adenoid cystic carcinoma (ACC) accounts for 10-15% of such tumors. The aim here was to evaluate surgical and clinical management, staging and follow-up of ACC patients in one academic institution. DESIGN AND SETTING: Retrospective study at Head and Neck Service, Universidade Estadual de Campinas. METHODS: Data on 21 patients treated between 1993 and 2003 were reviewed. Management utilized clinical staging, histology and imaging. Major salivary gland tumor extent was routinely assessed by preoperative ultrasonography. Diagnosis, surgery type, margin type (negative/positive), postoperative radiotherapy and recurrence (presence/absence) were evaluated. RESULTS: There were eleven major salivary gland tumors (52.3%), seven submandibular and four parotid. Ten patients (47.7%) had minor salivary gland ACC (all in palate), while the submandibular was the most frequently affected major one. Diagnoses were mostly via fine-needle aspiration (FNA) and incision biopsy. Frozen sections were used for six patients. There was good ultrasound/FNA correlation. Sixteen (76%) had postoperative radiotherapy. One (4.7%) died from ACC and five now have recurrent disease: three (14.2%) locoregional and two (9.5%) distant metastases. CONCLUSION: Adenoid cystic carcinoma has locally aggressive behavior. In 21 cases, of ACC, the facial nerve was preserved in all except in the few with gross tumor involvement. Treatment was defined from physical examination, imaging, staging and histology. CONTEXTO E OBJETIVO: Tumores de glândulas salivares exigem acompanhamento clínico por longos períodos devido à evolução indolente dessas neoplasias e à possibilidade tardia de recidivas locais e metástases a distância. O carcinoma adenóide cístico (CAC) compreende 10% a 15% dos tumores de glândulas salivares. O objetivo deste trabalho é analisar a conduta terapêutica, o estadiamento e o acompanhamento de pacientes com CAC em cabeça e pescoço em instituição de ensino. TIPO DE ESTUDO E LOCAL: Estudo retrospectivo realizado no Serviço de Cirurgia de Cabeça e Pescoço, Universidade Estadual de Campinas. MÉTODOS: Revemos o tratamento do CAC de 21 pacientes tratados entre 1993 e 2003. Os pacientes foram estadiados conforme os critérios clínicos da União Internacional Contra o Câncer (UICC) de 1998 e o tratamento foi baseado na avaliação clínica e imagenológica. RESULTADOS: 11 tumores estavam localizados nas glândulas salivares maiores (52,3%), sendo sete em glândula submandibular e quatro em parótida. Em 10 pacientes (47,7%), os tumores estavam em glândulas salivares menores (todos em palato). Quando ocorreram em glândula salivar maior, a submandibular foi a mais acometida. O diagnóstico foi realizado quase sempre por punção aspirativa por agulha fina (PAAF) ou biópsia. Biópsia por congelação no intraoperatório foi realizada em seis pacientes. Obtivemos boa correlação entre o ultra-som e a PAAF. Em 16 pacientes (76%), a radioterapia pós-operatória foi realizada. Um dos pacientes (4,7%) foi a óbito pelo CAC e cinco tiveram recidivas da doença, sendo três locoregionais (14,2%) e dois (9,5%) por metástases a distância. CONCLUSÃO: Carcinoma adenóide cístico apresenta comportamento agressivo localizado. Em 21 casos, o nervo facial foi preservado, exceto nos raros pacientes com invasão grosseira pelo tumor. O tratamento baseou-se no exame físico, exames de imagem, estágio do tumor e diferenciação histológica.

CONTEXT AND OBJECTIVE: The use of pedicled myocutaneous flaps in head and neck reconstruction is widely accepted. Here we describe our experience with infrahyoid flaps (IHFs) employed to cover surgical defects in the oral cavity and oropharynx in patients with benign and malignant tumors. The aim was to evaluate the success rate for infrahyoid myocutaneous flap procedures performed at a single institution. DESIGN AND SETTING: Retrospective study, at the Head and Neck Surgery Service, Unicamp. METHODS: Fourteen IHFs were used to reconstruct surgical defects in eleven men (78.5%) and three women (21.5%) with a mean age of 66.4 years. The anterior floor of the mouth was reconstructed in nine patients (64.2%), the base of tongue in three (21.4%), the lateral floor in one (7.1%), and the retromolar area (7.1%) in one. Thirteen patients (92.8%) had squamous cell carcinoma (SCC) and one (7.2%) ameloblastoma. The disease stage was T3 in eight (61.5%) of the SCC cases and T4 in five (38.5%). RESULTS: No patient presented total flap loss or fistula. The most common complication was epidermolysis, which delayed the beginning of oral ingestion. The patients with SCC received postoperative radiotherapy without major consequences to the flap. CONCLUSION: IHF is a safe and reliable procedure for reconstructing head and neck surgical defects. Due to its thinness and malleability, its use for oral cavity and oropharynx defects provides favorable cosmetic and functional outcomes. Complications, when present, are easy to manage.

CONTEXT: Adenoid cystic carcinomas are malignant tumors that occur in both the major and the minor salivary glands. A laryngeal location is rare because of the paucity of accessory salivary glands in this area. Adenoid cystic carcinomas account for less than 1% of all malignant tumors in the larynx, and only about 120 cases have been reported in the literature. These tumors have a slight female predisposition, and their peak incidence is in the fifth and sixth decades of life. In this article, we describe a case of laryngeal adenoid cystic carcinoma and discuss its clinical characteristics and treatment. CASE REPORT: We report on a case of laryngeal adenoid cystic carcinoma in a 55 year-old female patient who presented with dyspnea and hoarseness. Features of the diagnostic and therapeutic evaluation are described and the clinical management of such cases is outlined. The clinical course, definitive treatment strategy and surgical procedure, and also adjuvant treatment with irradiation are discussed. Although the tumor is radiosensitive, it is not radiocurable. CONTEXTO: Carcinoma adenóide cístico (CAC) é um tumor maligno que ocorre tanto nas glândulas salivares maiores quanto nas menores. Localização laríngea é rara devido à paucidade de glândulas salivares acessórias nesta região. O carcinoma adenóide cístico representa menos de 1% das lesões malignas da laringe e apenas 120 casos foram relatados na literatura. O CAC tem freqüência discretamente superior no sexo feminino e seu pico de incidência ocorre entre a quinta e sexta décadas de vida. Neste artigo, descrevemos um caso de CAC laríngeo e discutimos suas características clínicas e seu tratamento. RELATO DE CASO: Relatamos o caso de CAC numa paciente de 55 anos que apresentava disfonia e dispnéia. Características diagnósticas e avaliação terapêutica são descritas e a conduta clínica definida. Evolução clínica, estratégia terapêutica e procedimento cirúrgico são discutidos, bem como o tratamento adjuvante com radioterapia. O tumor, apesar de radiossensível, não é radiocurável.

Methods Twenty-four IHF were used to reconstruct surgical defects in twenty men (83.3%) and four women (16.7%) with a mean age of 61.8 years. The anterior floor of the mouth was reconstructed in 14 patients (58.3%), the lateral floor in five (20.8%), the base of tongue in 3 (12.5%), the retro molar area and oro-posterior wall of hypopharynx in one case each (4.1%). Twenty-two patients (91.6%) had squamous cell carcinoma (SCC) and two (8.3%) ameloblastoma. Of the SCC cases, the disease stage was T3 in 14 (63.6%) and T4 in 8 (36.3%). All patients were submitted to selective unilateral or bilateral neck dissections. Results No patient presented total flap loss or fistula. The most common complication was epidermolysis, which was important in five (20.8%) patients, delaying the beginning of oral ingestion. However, oral feeding was started on average at eight days. Four (16.6%) patients required postoperative speech therapy. The patients with SCC received postoperative radiotherapy with no major consequences to the flap. Conclusions IHF is a safe and reliable procedure for reconstructing head and neck surgical defects. Due to its thinness and malleability, its use for oral cavity and oropharynx defects provides favorable cosmetic and functional outcomes. Complications, when present, are easy to manage.

PURPOSE: To develop experimental models to evaluate the effects of hydrochloric acid associated with the pepsin instilled in the mucosa of the upper esophagus and the esophagogastric junction of young male rats Wistar, simulating injury caused by gastroesophageal reflux on the mucosa of aero-digestive tract in humans as well as the action of the risk exposure of mucosa to cigarette smoke. METHODS: Fifty young male Wistar rats divided in 5 groups with 10 animals each one, respectively simulating pharyngo-laryngeal reflux and gastroesophageal reflux, pharyngo-laryngeal reflux and smoking, smoking only, gastroesophageal reflux and control group. RESULTS: The histopathologic studies no recorded neoplasias, only mild changes and no significant alterations. The hemo-oximetry (carboxyhemoglobin and methemoglobim) and CO2 concentration confirm that the animals were submitted to high intensity of exposure to carcinogens in tobacco and its derivatives. CONCLUSION: The experimental models were highly efficient, practical, easy to use and economical and can be employed in other similar studies to determine the harmful effects by smoking and reflux.
