Changes in the indices of microcirculation and lipid metabolism were assessed in the process of treatment of patients with obliterating atherosclerosis of the lower extremity vessels and in combination with ischemic heart disease, with the II degree of ischemia those who had undergone revascularizing operations. It was found that plasmapheresis followed by plasmosorbtion improved the state of peripheral blood flow and decreased the concentration of blood lipoproteins of both low and high density, not decreasing the atherogenicity coefficient, and less effective in combination with ischemic heart disease, which was due to low selectivity of the carbofibrilar sorbent.

The state of microcirculation was studied in 35 patients with obliterating atherosclerosis of the lower extremity vessels (1st group) and 36 patients with extensive atherosclerosis having a combined impairment of the peripheral basin: arteries of lower extremities and coronary arteries (2nd group) treated using the method of plasmapheresis (PPh) and PPh combined with intravascular laser irradiation of blood (IVLIB). The isolated use of PPh in patients of the 1st group was established to be effective, while combination of PPh and IVLIB was necessary for the patients of the 2nd group.

The state of microcirculation in the ischemized extremity fingers was studied in 89 patients with obliterating atherosclerosis of the lower extremity vessels treated by conservative rheological therapy with rheopolyglucin and intravascular laser irradiation of blood in order to define their application alone, or in combination or in succession. The work has shown evident effectiveness of their using alone and in succession, while a combined use of the methods in question was found to be not expedient. The successive use of these methods can be considered expedient when monotherapy had proved to be helpless.

Under investigation there were 152 patients aged 61 +/- 16 years who were conditionally divided into three groups according to the severity of ischemia. Plasmaphersis was carried out by a standard method. The monitoring of changes of microcirculation was done with the help of high-frequency ultrasonic doppler. Plasmapheresis is considered to be an effective method of treatment of the peripheral artery occlusive disease, it improves the microcirculation in ischemic tissues in critical ischemiae of the legs, normalizes the arterial perfusion due to the disappearance of arterio-venous dump at the microcirculatory level.
