Depression is one of the most important psychiatric comorbidities in multiple sclerosis and is associated with increased disability and poor quality of life. The diagnosis is challenging because of the overlapping symptomatology. The pathogenesis is complex and controversial, the incidence is high, but is frequently under-diagnosed and under-treated. The aim of this article is to review the most important aspects of depression in multiple sclerosis as: epidemiology, pathogenesis, assessment, correlates with disability, cognition, fatigue, quality of life, suicide, the relationship with the immunomodulatory therapy, neuroimaging and treatment options.

ABSTRACT. In the current study we investigated the efficiency of two different types of training on the mathematical performance of students with low achievement in Mathematics. We chose to select a direct training of domain-specific working memory and a training of metacognitive skills as applied to Mathematics. It seems that domain-specific working memory deficits are encountered in children with Math learning difficulties. Moreover, it seems also reasonable to consider a program that is based on metacognition, as many mathematical activities are approached in a systematic and algorithmic manner. Both programs were efficient in enhancing operation fluency in simple and complex math problems. Results can be used in the direction of adding to the behavioral profile of children with Math learning disabilities, but also in designing efficient intervention programs for poor Mathematicians.

4. Subclavian double steal syndrome presenting with cognitive impairment and dizziness

Background. Atherosclerotic occlusion of the brachiocephalic artery is a very rare condition resulting in a distinctive pattern of hemodynamic disturbances in the cerebral circulation described as subclavian double steal syndrome. Case report. We present the case of a 66-year-old male patient with atherosclerotic occlusion of the brachiocephalic artery with double steal syndrome presenting with cognitive impairment and dizziness. The diagnosis was based on duplex ultrasound examination and was confirmed with angiography. The patient was referred for revascularization surgery, with extra-anatomical bypass. The outcome was favourable regarding the vertebrobasilar symptomatology, but mild cognitive impairment persisted.

BACKGROUND There were gaps between guidelines and practice when surgery was the only treatment for aortic stenosis (AS). OBJECTIVES This study analyzed the decision to intervene in patients with severe AS in the EORP VHD (EURObservational Research Programme Valvular Heart Disease) II survey. METHODS Among 2,152 patients with severe AS, 1,271 patients with high-gradient AS who were symptomatic fulfilled a Class I recommendation for intervention according to the 2012 European Society of Cardiology guidelines; the primary end point was the decision for intervention. RESULTS A decision not to intervene was taken in 262 patients (20.6%). In multivariate analysis, the decision not to intervene was associated with older age (odds ratio [OR]: 1.34 per 10-year increase; 95% CI: 1.11 to 1.61; P = 0.002), New York Heart Association functional classes I and II versus III (OR: 1.63; 95% CI: 1.16 to 2.30; P = 0.005), higher age adjusted Charlson comorbidity index (OR: 1.09 per 1-point increase; 95% CI: 1.01 to 1.17; P = 0.03), and a lower transaortic mean gradient (OR: 0.81 per 10-mm Hg decrease; 95% CI: 0.71 to 0.92; P < 0.001). During the study period, 346 patients (40.2%, median age 84 years, median EuroSCORE II [European System for Cardiac Operative Risk Evaluation II] 3.1%) underwent transcatheter intervention and 515 (59.8%, median age 69 years, median EuroSCORE II 1.5%) underwent surgery. A decision not to intervene versus intervention was associated with lower 6-month survival (87.4%; 95% CI: 82.0 to 91.3 vs 94.6%; 95% CI: 92.8 to 95.9; P < 0.001). CONCLUSIONS A decision not to intervene was taken in 1 in 5 patients with severe symptomatic AS despite a Class I recommendation for intervention and the decision was particularly associated with older age and combined comorbidities. Simultaneous determination of caffeine, cotinine, and 1N-methyluric acid in urine has numerous applications in determining patterns of use by athletes, according to the World Anti-Doping Agency (WADA) which includes these substances on the Monitoring List. The method can provide information on the enzyme-inducing activity on CYP1A2 of polycyclic aromatic hydrocarbons from cigarette smoke. Urine samples from 30 people (15 smokers and 15 non-smokers) were sampled 6 hours after having consumed a beverage with a total caffeine content of 200 mg. A HPLC with UV detection method was used which allowed concomitant determination of the three analytes. The results obtained show different values of caffeine urine concentrations (6.47 � 3.63 mM / L in smokers vs 10.09 � 5.68 in non-smokers, p [0.05) and a higher elimination of N1-methyluric acid by over 50% in smokers (identifiable due to the presence of cotinine, the main metabolite of nicotine).

Transthoracic echocardiographic examination is known to be a safe, non-invasive and reproducible method, used in every day clinical practice to obtain important information about cardiac structure and function. Unfortunately, a significant proportion of studies have highlighted the considerable technically difficultly in producing diagnostic images due to a poor acoustic window and more than 33% of patients undergoing stress echocardiography have suboptimal echocardiographic images. All these limitations have led to the use of contrast agents to improve the quality of standard ultrasound examination to provide a better delineation of left ventricle endocardial borders or to obtain information that cannot be achieved by using standard echocardiography, such as assessing myocardial microcirculation and therefore perfusion. This paper sought to review the clinical efficacy and safety of ultrasound contrast agents focusing on stress echocardiography.

The aim of the present study was to assess the prevalence of depression in a large multiple sclerosis (MS) patient group and to analyse the physical and psycho-socio-economic factors that influence its frequency and severity. In total, 351 consecutive patients with a diagnosis of MS were included in this study. All the patients completed a survey about their demographic characteristics, marital status, presence of children, employment status, educational level, duration of disease, disease course, duration of treatment and type of disease modifying therapies (DMT). Their disability level was evaluated using the Expanded Disability Status Scale (EDSS). Their depression level was evaluated using the Romanian Version (2012) of the Beck Depression Inventory-II (BDI-II). There was a significant positive correlation between the BDI-II score and the actual age of the patients, their disease duration, EDSS and the number of total relapses. From the EDSS functional scores, only the pyramidal score and the sensitive score presented a significant association with BDI-II in the logistic regression analysis. The BDI-II values decreased significantly with increasing education level. In the case of the retired patients, the BDI-II values were significantly higher. The BDI-II scores were also higher in widowers and divorced patients and in patients with children. There was no significant difference in the BDI-II values between different DMT groups. The main physical and psycho-socio-economic determinants of depression in patients with MS are the physical disability, disease duration, number of relapses, marital status, education level, employment status, and the presence of children, but many unsolved questions remain about the interrelations and relative contributions of these factors, which necessitates further research. The DMTs did not significantly influence the depression level.

The aim of this article is to review the most important aspects of depression in multiple sclerosis as: epidemiology, pathogenesis, assessment, correlates with disability, cognition, fatique, quality of life, suicide, the relationship with the immunomodulatory therapy, neuroimaging and treatment options.

Introduction: Depression and cognitive impairment are the most frequent mental disorders in multiple sclerosis (MS) and represent an important cause of morbidity and mortality. The aim of the study was to analyse the main determinants of depression in multiple sclerosis. Materials and methods: Thirty-two patients with relapsing remitting multiple sclerosis (RRMS), treated with Interferon Beta 1a, without relapses and corticosteroid treatment in the last 30 days, were included in the study. The mean age of the patients was 35.4±9.2 years, M/F ratio 0.33. Depression level was evaluated by the Romanian version of Beck Depression Inventory (BDI) and the cognitive function with Paced Auditory Serial Addition Test 3 (PASAT 3), Symbol Digit Modalities Test (SDMT). The functional status and disability level of the patients were evaluated with Multiple Sclerosis Functional Composite and Expanded Disability Status Scale. In all patients a cerebral MRI with intravenous contrast administration was performed using a 1.5T MRI device. Results: Twenty-three patients were free of depression (score 1-10), 4 patients presented mild mood disturbance (score 11-16), 3 borderline clinical depression (score 17-20) and 2 moderate depression (score 21-30). The mean BDI score was 8.71±7.16. BDI score correlated significantly with EDSS (R=0.38, p=0.03), PASAT 3 (R=-0.42, p=0.01), SDMT (R=-0.58, p=0.0007), Timed 25-Foot Walk (R=0.43, p=0.01) and 9-Hole Peg Test (R=0.45, p=0.008). From the EDSS functional scores, significant correlations were found with the urinary score (R=0.4, p=0.01) and sensitive score (R=0.49, p=0.004). BDI score correlated significantly with the total number of T2 lesions (R=0.31, p=0.05) while there was no correlation with the number of active lesions. Conclusions: The main determinants of depression in RRMS patients are the cognitive impairment, the affection of fine hand movements (9-HP), gait impairment (T25FT) and bladder and sensitive dysfunction.

11. Constrictive pericarditis - a difficult diagnosis

Constrictive pericarditis is an uncommon condition, often of unknown etiology. The diagnosis of constrictive pericarditis can be difficult and is often delayed, because the signs and symptoms of this disease can be falsely attributed to other causes. We report the case of a 62-year-old woman presented with a one year history of progressively worsening dyspnea, peripheral oedema, prominent jugular distension, hepatomegaly, ascites. The patient is known with a history of effusive pericarditis. Blood test showed a normal white cell count, anaemia, raised CRP. Chest X-Ray shows a normal sized heart and without calcification of the pericardium. The echocardiographic exam showed septal bounce-abrupt transient rightward movement, left, right ventricular size decreased-heart tubularin shape, mild atrial enlargement, IVC plethoric and unresponsive to respiration, hepatic veins dilated. Doppler echocardiographic findings were consistent with constrictive pericarditis. Cardiac catheterization showed elevation and equalization of diastolic filling pressures, and dips and plateau configuration of ventricular pressure during diastole (square root sign). Based on these results pericardiectomy was necessary. Constrictive pericarditis was also confirmed at the time of surgery. The pericardium was found with thickening of up to 30 mm in some areas. Histopathological exam showed fibrosis and calcification within the pericardium with no evidence of malignancy or tuberculosis. The patient was discharged 10 days later. At 3 months she had no significant symptoms. Diagnosis of constrictive pericarditis remains challenging. Constrictive pericarditis should be suspected in patients with clinical features of right-sides heart failure. Echocardiography and cardiac catheterisation are important investigation in diagnosis of constrictive pericarditis and avoiding unnecessary treatments.

Neuromyelitis optica (NMO) is a chronic inflammatory disease of the central nervous system that preferentially affects the optic nerves and spinal cord, and is characterised by the presence of autoantibody to aquaporin-4 (AQP4) [1–4]. Movement disorders have been described in demyelinating diseases, mainly in multiple sclerosis (MS). The presence of these complications in NMO is documented only in case presentations and small case series. The most frequent movement disorders in demyelinating diseases are paroxysmal dystonias (PD), also called tonic spasms (TS) [4]. The original description by Matthews WB was of ‘‘tonic seizures’’ [5]. Obstructive Sleep Apnea (OSA), characterized by airflow cessation (apnea) or reduction (hypopnea) due to repeated pharyngeal obstructions during sleep, causes frequent disruption of sleep and hypoxic events. The condition is linked to many adverse health related consequences, such as neurocognitive and cardiovascular disorders, and metabolic syndrome. OSA is a chronic condition requiring long-term treatment, so treatment using continuous positive airway pressure (CPAP) has become the gold standard in cases of moderate or severe OSA. However, its effectiveness is influenced by patients’ adherence. Surgery for OSA or treatment with oral appliances can be successful in selected patients, but for the majority, lifestyle changes such as exercise and dietary control may prove useful. However, exercise training remains under-utilized by many clinicians as an alternative treatment for OSA. Other interventions such as oral appliance (OA), upper way stimulation, and oropharyngeal exercises are used in OSA. Because the benefit of all these techniques is heterogeneous, the major challenge is to associate specific OSA therapies with the maximum efficacy and the best patient compliance.

Abstract Non-Cystic Fibrosis Bronchiectasis (NCFB) are characterised by abnormal, permanently damaged and dilated bronchi due to the innapropiate clearence of various microorganisms and recurrent chronic infections.The diagnosis is suggested by the clinical presentation and is confirmed by multiple investigations. There are some comorbidities associated with bronhciectasis, such as chronic obstructive pulmonary disease (COPD), cardiovascular disorders, gastro-esophageal reflux disease (GERD), psychological illnesses, pulmonary hypertension, obstructive apnea syndrome(OSA). The condition has a substantial socioeconomic impact because it requests a multidisciplinary management and periods of exacerbations are common. The aims of the management of bronchiectasis are to reduce symptoms (such as sputum volume and purulence, cough and dyspnea), reduce the frequency and severity of exacerbations, preserve lung function and improve health-related quality of life. The multidisciplinary approach of bronchiectasis patients require along with the medical treatment, a specific plan of nonphamarcological strategies, including balneological intervention. There are a lot of techniques improving the airway clearence, such as: active cycle of breathing techniques (which include breathing control, thoracic expansion exercises, forced expiratory technique), oscilatting possitive expiratory pressure, autogenic drainage, gravity-assisted-positioning, modified postural drainage. Together with specific medication, these techniques can diminuate symptoms and improve the quality of life.

16. Rehabilitation therapies in stable chronic obstructive pulmonary disease

Introduction. Chronic Obstructive Pulmonary Disease (COPD) is a common, preventable and treatable disease. The pulmonary rehabilitation (PR) is a mutidisciplinary and comprehensive intervention in symptomatic patients with COPD. Objective. This review aims to synthesize evidence from available studies on the relative efficacies of different methods of rehabilitation therapies in patients with stable COPD. Material and Methods. A search was performed on the databases Pubmed, Embase, ResearchGate. Of the 410 articles retrieved from databases, only 20 met the inclusion criteria. Two reviewers independently reviewed selected eligible studies. Results. Rehabilitation is a multidisciplinary intervention in symptomatic patients with COPD, including speleotherapy, halotherapy, muscular training, soft tissue manual therapy and neuromuscular electrostimulation. All of the case-control studies using speleotherapy reported improved respiratory function to varying degrees and there were improvements in lung functional tests including forced vital capacity (FVC), forced expiratory volume in 1 second (FEV 1), oxygen saturation, partial pressure of oxygen in arterial blood, and partial pressure of carbon dioxide in arterial blood. In addition, halotherapy has been associated with relief of respiratory conditions such as COPD, asthma and cystic fibrosis by its bactericidal effect, improvement of immunity and rheological properties of secretions. Respiratory muscle training is a part of rehabilitation for COPD subjects. In patients who can not perform physical activity, neuromuscular electrostimulation (NMES) increased 6 minute walking distance and time to symptom limitation exercising at a submaximal intensity and reduced the severity of leg fatigue on completion of exercise testing. Conclusion. The management of COPD should include a multidisciplinary therapy, including rehabilitation therapies as an adjuvant to the medical treatment, especially because due to the high prevalence, mortality, and morbidity, COPD will be one of the biggest public health challenges in the next century.

Introduction: Atrial myxomas are the most common primary heart tumors. Although quite rare, left atrial myxomas account for 80% of all cardiac tumors. Diagnosis is often difficult due to the wide array of presenting symptoms. This case report discusses an unusual presentation of left atrial myxoma in an elderly patient. Case presentation: A 73-year old woman with a history of hypertension, dyslipidemia and hyperthyroidism treatment presented to the emergency department with a new onset episode of palpitations. The electrocardiogram revealed atrial fibrillation. Rate control was achieved with beta-blockers and sinus rhythm transition was achieved shortly after admission. Transthoracic echocardiography revealed a heterogeneous mass in the left atria with a villous surface, occupying more than 50% of the left atrial cavity. Surgery was recommended because of the embolic potential of such a mass and tumor excision was performed. Microscopic pathology showed typical histological features of cardiac myxoma with no atypia or malignancy, and the patient was discharged in sinus rhythm 7 days after surgery. Discussion and conclusions: Left atrial myxoma presenting in the seventh decade of life is rare. Elderly patients often present with nonspecific symptoms that are often overlooked in the absence of a supporting cardiac history, which makes an early diagnosis challenging. We conclude that the majority of myxomas mimic many cardiovascular diseases and were detected in symptomatic patients, so a high index of clinical suspicion is important for its early and correct diagnosis. Two-dimensional echocardiography provides substantial advantages in detecting intracardiac tumors. [ABSTRACT FROM AUTHOR]

19. EVALUATION OF DEPRESSION IN A LARGE ROMANIAN COHORT OF MULTIPLE SCLEROSIS PATIENTS.

Background: Depression is the most common psychiatric disease seen in multiple sclerosis (MS) patients. It represents one of the most important determinants of the quality of life of MS patients having a significant negative impact on cognitive functions, vocational status, social and family relationships, adherence to treatment, and it also represents a risk factor for suicide. Material and methods: The study included a total of 351 patients with clinically isolated syndrome, relapsing-remitting and secondary progressive MS being in evidence in the Regional MS Center Targu Mures in the period September 2014-February 2015. Depression level was assessed by using the Beck Depression Inventory-Version II (BDI-II). All patients completed a questionnaire with data regarding the level of education, occupational status, marital status, presence or absence of children, demographic and medical informations. Neurological examination was performed in all patients and the level of disabilty was expressed by EDSS score. Results: In our group of patients, 191 had BDI-II scores within the normal range, 65 had mild mood disorders, 36 borderline depression, 35 moderate depression, and 24 severe and extreme depression. Age at disease onset, the actual age of the patient, disease duration, number of recurrences and the degree of disability were positively correlated with the severity of depression. Patients with low education levels, pensioners, patients with children had the highest values of the BDI-II scores. With regard to marital status, the highest levels of depression were retrieved in divorced and widowed patients, followed by married patients. Conclusions: The prevalence of depression in our group of patients was 27.05%. Determinant factors of depression in this group of patients were age, duration of illness, occupational and marital status, education level and degree of disability. 20. Contrast Echocardiography to Evaluate Myocardial Perfusion During Percutaneous Transluminal Septal Myocardial Ablation.

Background: Contrast echocardiography is frequently used to enhance endocardyal definition, Doppler signals, and to assess myocardial perfusion in percutaneous transluminal septal myocardial ablation. Material and method: We followed clinically and echocardiographic 5 female patients with Septal Obstructive Hypertrophic Cardiomyopathy, which observed symptoms despite undergoing maximal medical therapy - angina, functional class NYHA III dyspnea, which underwent septal alcohol ablation. Clinical and echocardiographical examinations were performed at admission, before, during and after the procedure, at discharge, at 6 months and at 1 year. During the ablation contrast echocardiography was used. Results: Maximum subaortic gradient was reduced by 30 mmHg in all cases, immediately after the procedure, with no increase in further examinations and reduced mitral regurgitation (grade I or minor after the procedure). Symptoms resolved in all patients with NYHA functional class I transition. Transient complications of the procedure in the first 24 hours after ablation were paroxysmal atrial fibrillation in 2 cases, atrioventricular block 2nd degree in 1 case and 3rd degree in 2 cases. Permanent complications were right bundle branch block 3 cases, left bundle branch block 1 case, 1st degree atrioventricular block 1 case. Conclusions: All 5 patients opted for percutaneous transluminal septal myocardial ablation, although, according to guidelines, they had indication for septal myomectomy. Contrast echocardiography has proven to be a useful addition to percutaneous transluminal septal myocardial ablation. [ABSTRACT FROM AUTHOR]

Background: Individualized therapy for population groups including poor, fast and ultra rapid metabolisers, is based on pharmacogenetic tests. Determination of metabolism products of various non-toxic substrates of cytochrome P450 isoforms may be a simple alternative for selecting patients who require further genetic investigations. Material and methods: The N1-methyl uric acid/caffeine ratio was determined in a group of 20 subjects after administration of a standard amount of caffeine to discriminate the CYP1A2 poor/fast metaboliser status. Simultaneously, the presence of cotinine was determined as a marker of smoking habits that induce CYP1A2. A similar method was used for a substrate of CYP3A4 (Testosterone). Results: The subjects number did not permit identification of poor metabolisers but emphasized the inducing effect of cigarette smoke on CYP1A2 reflected in the significant changes of N1-methyl uric acid/caffeine ratio. Conclusions: The analytical methods are cheap alternatives to choosing personalized treatment. In addition, enzyme induction and inhibition phenomenon can be identified even if the concomitenet intake of drugs/xenobiotics is denied by the patient. Acknowledgement.
