Osteosarcoma is a malignant tumor of mesenchymal cells that have the ability to produce osteoid matrix or mineralized bone. They can involve any bone, but in the jaw region they are uncommon and account for about 6% of all cases. These tumors present as painful and progressively growing masses. Often a sudden, non-traumatic fracture is the first significant clinical symptom. This paper reports a clinical case of osteosarcoma in the body of the mandible. A 25-year-old male patient came to the clinic reporting an intra and extra-oral swelling in the left lower jaw, associated with pain and paresthesia, which lasted four months. The patient reported a previous history of mucoepidermoid carcinoma in the left retroauricular region treated with parotidectomy when he was six years old. The diagnostic hypotheses of Odontogenic Myxoma and Osteosarcoma were raised. Thus, an incisional biopsy was performed, which showed histopathological examination compatible with osteosarcoma. After that, it was decided to perform a left segmental pelvemandibulectomy with histopathological study that confirmed the diagnosis. The case report contributes to the knowledge of health professionals about the osteosarcoma of the mandible since many times this diagnostic hypothesis is not raised because it is a rare situation.

Introdução: Os Programas de Residência Multiprofissional constituem-se uma estratégia fundamental para a ampliação da capacidade para o enfrentamento do câncer. Objetivo: Analisar o panorama atual dos Programas de Residência Multiprofissional em Oncologia para cirurgiões-dentistas no Brasil, bem como o número de profissionais egressos desses cursos, a partir de dados informados pelo Ministério da Educação (MEC). Método: Trata-se de um estudo quantitativo, exploratório. A primeira fase consistiu na solicitação à Equipe Técnica da Residência Multiprofissional do MEC do Brasil da relação de todos os Programas de Residência Multiprofissional em Oncologia que incluíssem a categoria profissional da odontologia. Adicionalmente, foi realizada busca manual. Na segunda fase, realizaram-se contato por e-mail e busca pelos editais disponíveis na Internet dos casos em que não houve resposta. Os dados investigados foram a presença de Programa ativo, inclusão da categoria profissional da odontologia, número de vagas ofertadas e de egressos para cada ano entre 2010 e 2020, que foram analisados descritivamente. Resultados: Foram identificados 13 Programas de Residência Multiprofissional em Oncologia para cirurgiões-dentistas, distribuídos em 12 centros, situados em dez cidades de oito Estados brasileiros, sobretudo, na Região Sudeste. Seis Programas foram implementados em 2010, seis em 2016 e um em 2020. No cenário Brasileiro, entre 2010 e 2020, foram ofertadas 216 vagas e formados pouco mais de 114 profissionais especializados nessa área. Conclusão: Ressalta-se a necessidade do incremento de centros, Programas e vagas para Residências Multiprofissionais em Oncologia para cirurgiões-dentistas no Brasil, bem como do reconhecimento da Oncologia como uma especialidade odontológica.

The report describes a case of mandibular osteosarcoma in a 25-year-old black male patient complaining of increase of volume in mandible with evolution of 4 months. The anamnesis stated a previous history of mucoepidermoid carcinoma in the left retroauricular region treated by parotidectomy 18 years ago. Physical examination revealed the swelling of the left body of the mandible with face asymmetry and an expansive increase of volume in the buccal region of the mandible, extending to vestibule depth, associated with pain on palpation and paresthesia. Panoramic radiograph revealed a poorly defined radiolucent image without sclerotic border, with destruction of the periodontal ligament corresponding to tooth 36. Incisional biopsy was performed, and the histopathologic report was compatible with osteosarcoma. The patient was referred for oncology treatment. Osteosarcomas in the mandible are extremely rare, and this case justifies investigation as the patient had already presented another type of tumor in the proximal region.

Oropharyngeal squamous cell carcinoma (OSCC) accounts for 30% of head and neck neoplasms. Tobacco and alcohol consumption are classic risk factors for this neoplasm. However, human papillomavirus (HPV) is an additional risk. This report describes the case of OSCC in a 60-year-old black man, a smoker and chronic alcoholic, who complained of “pain when swallowing.” A left cervical lymph node measuring 3.0 cm that was fixed and painless was observed at the extraoral examination. Intraoral examination revealed a bleeding hard ulcer with raised edges that was painful to palpation, 6.0 cm in diameter, and located in the left soft palate with 6 months of evolution. An incisional biopsy was performed with well-differentiated OSCC stage III diagnosis and negative immunohistochemistry for p16. The patient was submitted to radiotherapy and neoadjuvant chemotherapy. Classical carcinogenesis induced by tobacco and alcohol appears to be associated with this case rather than HPV infection.
