1. Cyclosporine A in hospitalized COVID-19 pneumonia patients to prevent the development of interstitial lung disease: a pilot randomized clinical trial

Abstract

Abstract Post-COVID-19 interstitial lung disease (ILD) is a new entity that frequently causes pulmonary fibrosis and can become chronic. We performed a single-center parallel-group open-label pilot randomized clinical trial to investigate the efficacy and safety of cyclosporine A (CsA) in the development of ILD in the medium term among patients hospitalized with COVID-19 pneumonia. Patients were randomized 1:1 to receive CsA plus standard of care or standard of care alone. The primary composite outcome was the percentage of patients without ILD 3 months after diagnosis of pneumonia and not requiring invasive mechanical ventilation (IMV) (response without requiring IMV). The key secondary composite outcomes were the percentage of patients who achieve a response requiring IMV or irrespective of the need for IMV, and adverse events. A total of 33 patients received at least one dose of CsA plus standard of care (n = 17) or standard of care alone (n = 16). No differences were found between the groups in the percentage of patients who achieved a response without requiring IMV or a response requiring IMV. A higher percentage of patients achieved a response irrespective of the need for IMV in the CsA plus standard of care group although the RR was almost significant 2.833 (95% CI, 0.908–8.840; p = 0.057). No differences were found between the groups for adverse events. In hospitalized patients with COVID-19 pneumonia, we were unable to demonstrate that CsA achieved a significant effect in preventing the development of ILD. Abstract

Background: We evaluated the prevalence and characteristics of persistent signs and/or symptoms in children and young people (CYP) one year after hospitalization for acute COVID-19 compared with a control group of CYP hospitalized for other conditions. Methods: We conducted an observational study in three hospitals in Madrid, which included a group of children aged between 1 month and 18 years who were hospitalized due to acute COVID-19 from March 2020 to December 2021. We also selected a comparison group of patients hospitalized for other, unrelated conditions within the same month. Eligible participants had no history of COVID-19 at recruitment or during follow-up. Data were collected from clinical records and a standardized questionnaire completed by the patients’ families. The primary outcome was the presence of persistent symptoms one year after hospitalization. Results: A total of 96 patients were enrolled and analyzed (50 acute COVID-19 patients and 46 non-COVID-19 participants). Of these, 34/96 (35%) met the criteria for persistent symptoms (CYP: 17/50 (34%) COVID-19 participants and 17/46 (37%) non-COVID-19 participants (p = 0.767)). Symptoms persisted ≥12 months in 14/50 (28%) COVID-19 participants and in 7/46 (15%) non-COVID-19 participants (p = 0.140). Both before and after admission, all of the participants provided similar ratings for all of the specific items related to emotional welfare, social relationships, and current activities. Readmissions occurred in 11/50 (22%) COVID-19 participants and in 6/46 (13%) non-COVID-19 participants (p = 0.267). Conclusions: We identified a non-significant difference in the prevalence of persistent symptoms 1 year after hospitalization between children and young people (CYP) with acute COVID-19 and those hospitalized for non-COVID-19-related conditions.

Abstract

The emergence of SARS-CoV-2 in 2019 and its rapid spread throughout the world has caused the largest pandemic of our modern era. The zoonotic origin of this pathogen highlights the importance of the One Health concept and the need for a coordinated response to this kind of threats. Since its emergence, the virus has caused >7 million deaths worldwide. However, the animal source for human outbreaks remains unknown. The ability of the virus to jump between hosts is facilitated by the presence of the virus receptor, the highly conserved angiotensin-converting enzyme 2 (ACE2), found in various mammals. Positivity for SARS-CoV-2 has been reported in various species, including domestic animals and livestock, but their potential role in bridging viral transmission to humans is still unknown. Additionally, the virus has evolved over the pandemic, resulting in variants with different impacts on human health. Therefore, suitable animal models are crucial to evaluate the susceptibility of different mammalian species to this pathogen and the adaptability of different variants. In this work, we established a transgenic mouse model that expresses the feline ACE2 protein receptor (cACE2) under the human cytokeratin 18 (K18) gene promoter's control, enabling high expression in epithelial cells, which the virus targets. Using this model, we assessed the susceptibility, pathogenicity, and transmission of SARS-CoV-2 variants. Our results show that the sole expression of the cACE2 receptor in these mice makes them susceptible to SARS-CoV-2 variants from the initial pandemic wave but does not enhance susceptibility to omicron variants. Furthermore, we demonstrated efficient contact transmission of SARS-CoV-2 between transgenic mice that express either the feline or the human ACE2 receptor.

Abstract

ABSTRACT Coronaviruses (CoVs) of genera α, β, γ, and δ encode proteins that have a PDZ-binding motif (PBM) consisting of the last four residues of the envelope (E) protein (PBM core). PBMs may bind over 400 cellular proteins containing PDZ domains (an acronym formed by the combination of the first letter of the names of the three first proteins where this domain was identified), making them relevant for the control of cell function. Three highly pathogenic human CoVs have been identified to date: severe acute respiratory syndrome coronavirus (SARS-CoV) and Middle East respiratory syndrome coronavirus (MERS-CoV), and SARS-CoV-2. The PBMs of the three CoVs were virulence factors. SARS-CoV mutants in which the E protein PBM core was replaced by the E protein PBM core from virulent or attenuated CoVs were constructed. These mutants showed a gradient of virulence, depending on whether the alternative PBM core introduced was derived from a virulent or an attenuated CoV. Gene expression patterns in the lungs of mice infected with SARS-CoVs encoding each of the different PBMs were analyzed by RNA sequencing of infected lung tissues. E protein PBM of SARS-CoV and SARS-CoV-2 dysregulated gene expression related to ion transport and cell homeostasis. Decreased expression of cystic fibrosis transmembrane conductance regulator (CFTR) mRNA, essential for alveolar edema resolution, was shown. Reduced CFTR mRNA levels were associated with edema accumulation in the alveoli of mice infected with SARS-CoV and SARS-CoV-2. Compounds that increased CFTR expression and activity, significantly reduced SARS-CoV-2 growth in cultured cells and protected against mouse infection, suggesting that E protein virulence is mediated by a decreased CFTR expression. IMPORTANCE Three highly pathogenic human CoVs have been identified: SARS-CoV, MERS-CoV, and SARS-CoV-2. The E protein PBMs of these three CoVs were virulence factors. Gene expression patterns associated with the different PBM motifs in the lungs of infected mice were analyzed by deep sequencing. E protein PBM motif of SARS-CoV and SARS-CoV-2 dysregulated the expression of genes related to ion transport and cell homeostasis. A decrease in the mRNA expression of the cystic fibrosis transmembrane conductance regulator (CFTR), which is essential for edema resolution, was observed. The reduction of CFTR mRNA levels was associated with edema accumulation in the lungs of mice infected with SARS-CoV-2. Compounds that increased the expression and activity of CFTR drastically reduced the production of SARS-CoV-2 and protected against its infection in a mice model. These results allowed the identification of cellular targets for the selection of antivirals.

Abstract

Coronaviruses (CoVs) have the largest genome among RNA viruses and store large amounts of information without genome integration as they replicate in the cell cytoplasm. The replication of the virus is a continuous process, whereas the transcription of the subgenomic mRNAs is a discontinuous one, involving a template switch, which resembles a high frequency recombination mechanism that may favor virus genome variability. The origin of the three deadly human CoVs SARS-CoV, MERS-CoV and SARS-CoV-2 are zoonotic events. SARS-CoV-2 has incorporated in its spike protein a furine proteolytic site that facilitates the activation of the virus in any tissue, making this CoV strain highly polytropic and pathogenic. Using MERS-CoV as a model, a propagation-deficient RNA replicon was generated by removing E protein gene (essential for viral morphogenesis and involved in virulence), and accessory genes 3, 4a, 4b and 5 (responsible for antagonism of the innate immune response) to attenuate the virus: MERS-CoV-Δ[3,4a,4b,5,E]. This RNA replicon is strongly attenuated and elicits sterilizing protection after a single immunization in transgenic mice with the receptor for MERS-CoV, making it a promising vaccine candidate for this virus and an interesting platform for vector-based vaccine development. A strategy could be developed for the design of RNA replicon vaccines for other human pathogenic coronaviruses.

La microangiopatía trombótica tumoral pulmonar es una enfermedad infrecuente con un diagnóstico de elevada complejidad, que requiere una alta sospecha clínica. La detección precoz es importante debido al mal pronóstico en estadios avanzados. Presentamos el caso de un varón de 39 años ingresado en el hospital para estudio por tos, disnea progresiva, hipertensión pulmonar y adenopatías abdominales. Durante su ingreso sufrió parada cardiorrespiratoria que no respondió a maniobras de reanimación cardiopulmonar avanzadas. El examen post mortem mostró hallazgos compatibles con microangiopatía trombótica tumoral pulmonar secundaria a adenocarcinoma gástrico en estadio avanzado.

Abstract

Background. The risk of herpes zoster reactivation is increased in immunocompromised patients, especially in those with immune-mediated inflammatory diseases (IMIDs) on Janus kinase inhibitor (JAKi) treatment. The recombinant subunit herpes zoster vaccine (RZV) is a non-live vaccine, recently approved for this subgroup of patients, which shows high rates of vaccine effectiveness, with few adverse effects reported in clinical trials. Purpose. The aim of this real-world study was to determine the immunogenicity and safety of RZV in IMID patients on JAKi treatment. Methods. The increase in the concentration of anti-gE antibody for varicella zoster virus post-vaccination, compared to the pre-vaccination concentration, was analyzed to test the humoral immune response. Adverse effects after the first and second vaccine doses were registered. Results. In total, 49 patients were analyzed, and a fourfold increase in antibody concentration was achieved in almost 40% of subjects, with only one serious local adverse effect. Discussion. The resulting immunogenicity was lower than that observed in clinical trials, probably due to the presence of immune disease and immunosuppressive treatment, and to the fact that this was a real-world study. No differences in response according to age, previous virus zoster reactivation, or concomitant treatments were found. Conclusions. RZV was well tolerated and reached the immune response objective in 40% of patients. These results reinforce the importance of including RZV vaccination for immunosuppressed patients. Real-world studies regarding vaccine effectiveness are still needed in order to gain a full understanding of the response to RZV in this group of patients.

Abstract

ObjectiveTo evaluate changes in blood long-chain polyunsaturated fatty acid (LCPUFA) and oxylipin concentrations in very preterm infants from birth to 36 weeks’ postmenstrual age (WPA) after providing an emulsified arachidonic acid (ARA):docosahexaenoic acid (DHA) supplement at two different concentrations.Study designThis prospective, randomized trial assigned infants to receive a supplement (1) 80:40 group (80 mg/kg/day ARA and 40 mg/kg/day DHA, n = 9) or (2) 120:60 group (120 mg/kg/day ARA and 60 mg/kg/day DHA, n = 9). Infants received supplement daily from birth until 36 WPA. At baseline, 21 days of life and 36 WPA, the LCPUFAs were measured in plasma by gas chromatography/mass spectrophotometry. Additionally, LCPUFAs and oxylipins were analyzed in whole blood by ultra-high-performance liquid chromatography-tandem mass spectrometry. Furthermore, a sample of oral mucosa was obtained to analyze single-nucleotide polymorphism located in the FADS1 gene by PCR.ResultsGestational age was similar between groups (80:40 = 28+6 [27+3; 30+3] completed weeks+days; 120:60 = 29+6 [27+3; 30+5] completed weeks+days, p = 0.83). At 36 WPA, the change in plasma ARA was significantly different between groups (80:40 group = 0.15 [−0.67; 0.69] %nmol, 120:60 = 1.68 [1.38; 3.16] %nmol, p = 0.031). In whole blood, the levels of ARA-derived oxylipins (5-, 8-, 9-, 11-, 15-HETE and 8,9-EET) and EPA-derived oxylipins (18-HEPE) significantly increase from baseline to 36 WPA in the 120:60 group than the 80:40 group.ConclusionSupplementation at high doses (120:60 mg/kg/day) increased levels of ARA, and EPA- and ARA-derived oxylipins compared to low doses (80:40 mg/kg/day). Differences were detected in EPA metabolites without a significant increase in plasma DHA.

Abstract

El encuentro entre Antonio Bonet y Eladio Dieste con motivo de la construcción de la Casa Berlingieri (1947) supuso el inicio del uso de un nuevo sistema constructivo: la cerá­mica armada. Inspirado en la tradición cerámica mediterránea, pero sustentado en la reali­dad latinoamericana, la cerámica armada es probablemente uno de los mejores ejemplos de alteridad constructiva, de voluntad de entendimiento y diálogo entre dos ámbitos a menudo tan dispares como Europa y Latinoamérica. En dicho encuentro primó el equilibrio frente a la apropiación, ninguno de los protagonistas impuso su ideario al otro y ambos aprendieron. Tanto fue así que, en cierta medida, la obra posterior de ambos dio frutos muy significativos fundamentados en ese momento de intercambio entre ambas orillas del Atlántico.

Abstract

This paper is displayed like a journey across the work of Christine Dalnoky, key referent for contemporary landscape architecture. Taking part of the critical movement towards a new landscape architecture focused on memory recovering of the site –in contrast with architectonical theories focused on program–, the analysis is presented as a crossing between ideological positions, spatial dialectic and prevalence or rhythms, in coherence with her personal evolution. Christine Dalnoky biography is divided in two periods: first settled in Paris and identified with her association from 1988 with Michel Desvignes; second moved to a little town in Provence, where she founded a new practices with Patrick Solvet. This radical removal from metropolis is shown in parallel with her projects development, in special with this of Parque del Agua in Zaragoza, where slow rhythm of landscape is imposed definitely over urban agitation.

Abstract

In 1959, the year of the dissolution of CIAM, although Spain was not completely connected with international architecture, its architects were certainly at a competitive level, having, for example, won a gold medal in the recent Brussels World’s Fair. At that time, the ideas and demands of the groups that instigated the demise of the CIAMs found an echo in publications around the world, particularly in European ones. But did the same thing happen in Spain? Is the indisputable quality of Spanish architecture in the 1960s a reflection of its awareness of international trends? This paper aims to review what was published about Team 10 and its members in the main Spanish architecture journals during the first years after its creation and, therefore, to what extent the articles contributed to the knowledge and dissemination of the group among Spanish architects. What was more interesting – the group, the characters or the ideas? Was the coverage constant or was it the result of specific activities or events? What reasons lead to the publication of certain information? Was there a theme or architect that stood out among the rest? Who were the Spaniards that were most interested in the group? The intention is not to offer a closed work on the subject, but to formulate a series of questions that helps us clarify whether there was real awareness of Team 10 in Spain in the 1960s.

Informes de la Construcción es una revista de indudable valor para documentar la historia de la construcción del siglo XX. Su incuestionable carga técnica es sobradamente conocida para los profesionales del sector. Sin embargo, en la medida en la que uno se sumerge entre sus páginas llama la atención otro aspecto: la especial atención que mostró durante sus primeras dos décadas de existencia a la divulgación de la arquitectura coetánea más señera. El presente trabajo trata de reivindicar el papel que jugó esta revista como agente de difusión de la producción foránea de las décadas de 1950 y 1960 en el ámbito arquitectónico madrileño. Es verdad que cuando se pregunta al respecto a los protagonistas del momento, Informes no es la primera publicación que recuerdan, pero también es cierto que las cifras que arroja el análisis de los artículos que incluye y la comparación con otras publicaciones especializadas de la época resultan 'apabullantes'. De la totalidad de reseñas en torno a la arquitectura o la actividad arquitectónica extranjera localizadas en un conjunto considerable de ellas, Informes de la Construcción acapara casi el 50 por ciento. Abstract

La fotografía es una de las maneras más eficaces de representar los objetos arquitectónicos. Es una herramienta óptima de comunicación gráfica. Sin embargo, en numerosas ocasiones su función excede la mera documentación y adquiere un papel primordial en la definición y difusión de una determinada arquitectura. Este trabajo pretende presentar la importancia que tuvo la fotografía en la definición y consolidación de la imagen de la arquitectura moderna española en las décadas de 1950 y 1960. En un momento en que los contactos y viajes resultaban relativamente escasos todavía, las fotografías publicadas en los medios permitieron que los arquitectos españoles mostraran sus avances dentro y fuera de las fronteras del país. Seguramente, sus responsables no eran conscientes de lo que hacían, pero, visto con cierta perspectiva, queda claro que la selección y mayor difusión de una serie de instantáneas marcó el desarrollo de la “nueva arquitectura” española.

A pesar de que durante el transcurso del siglo XX la arquitectura y el urbanismo se han identificado habitualmente con la ciudad, también hubo momentos en los que, coincidiendo con periodos críticos de la historia, se difundieron las que podríamos definir como 'ideologías antiurbanas', que llegaron a afectar a la creación artística y arquitectónica tanto como al pensamiento urbanístico. Sobre estas 'ideologías antiurbanas' y su oposición (o su derivación) a las 'urbanas' versó el III Congreso Internacional de arquitectura moderna española organizado por la Escuela Técnica Superior de Arquitectura de la Universidad de Navarra. Del desarrollo de dicha reunión, centrándonos esencialmente en las partes del mismo no publicadas en las actas del congreso, tratarán las siguientes líneas.

Abstract

La vivienda social como paradigma de la modernidad ha sido ampliamente revisada en las publicaciones periódicas del siglo XX. En ellas se legitimaron los avances urbanos, tipológicos y formales del Movimiento Moderno. Por otra parte, Latinoamérica fue tierra fértil para el desarrollo de grandes conjuntos habitacionales a partir de la segunda mitad de siglo. En ese contexto, este artículo se centra en un caso particular, el de la vivienda social mexicana, y lo hace a través del reflejo que tuvo en los medios impresos en el momento de su edificación. Durante unos años, magníficas unidades habitacionales poblaron las páginas de las revistas foráneas en una época (1950-1968) en que la arquitectura mexicana gozó de enorme difusión internacional en las publicaciones no latinoamericanas. Se certificaba así al mundo que una de las principales demandas del momento —la vivienda— podía solucionarse desde los programas estatales de edificación social.

Abstract

The popularity of the flipped classroom has been rising. This pedagogical model emphasizes active and peer-assisted learning and problem solving within the classroom and leaves the transmission of conceptual knowledge to individual tasks outside the classroom. This research evaluates the implementation and results of gamification, game-based learning, and active methodologies used in the development of the flipped methodology. The team of researchers has been applying this model since 2017. The sample consisted of students from different subjects, Management and Administration Business, Finance and Accounting, Marketing and Market Research, and Chemistry, at the University of Malaga and in Accounting and Administration and Marketing at the Polytechnic of Porto. A statistical analysis of the degree of students' satisfaction with the application of the pedagogical model has been carried out, considering the general aspects and the level of application within each subject. The data show a very positive assessment of the teaching achieved and the strategies applied in the model.

Abstract

[Objective]: To evaluate changes in blood long-chain polyunsaturated fatty acid (LCPUFA) and oxylipin concentrations in very preterm infants from birth to 36 weeks’ postmenstrual age (WPA) after providing an emulsified arachidonic acid (ARA):docosahexaenoic acid (DHA) supplement at two different concentrations., [Study design]: This prospective, randomized trial assigned infants to receive a supplement (1) 80:40 group (80 mg/kg/day ARA and 40 mg/kg/day DHA, n = 9) or (2) 120:60 group (120 mg/kg/day ARA and 60 mg/kg/day DHA, n = 9). Infants received supplement daily from birth until 36 WPA. At baseline, 21 days of life and 36 WPA, the LCPUFAs were measured in plasma by gas chromatography/mass spectrophotometry. Additionally, LCPUFAs and oxylipins were analyzed in whole blood by ultra-high-performance liquid chromatography-tandem mass spectrometry. Furthermore, a sample of oral mucosa was obtained to analyze single-nucleotide polymorphism located in the FADS1 gene by PCR., [Results]: Gestational age was similar between groups (80:40 = 28+6 [27+3; 30+3] completed weeks+days; 120:60 = 29+6 [27+3; 30+5] completed weeks+days, p = 0.83). At 36 WPA, the change in plasma ARA was significantly different between groups (80:40 group = 0.15 [−0.67; 0.69] %nmol, 120:60 = 1.68 [1.38; 3.16] %nmol, p = 0.031). In whole blood, the levels of ARA-derived oxylipins (5-, 8-, 9-, 11-, 15-HETE and 8,9-EET) and EPA-derived oxylipins (18-HEPE) significantly increase from baseline to 36 WPA in the 120:60 group than the 80:40 group., [Conclusion]: Supplementation at high doses (120:60 mg/kg/day) increased levels of ARA, and EPA- and ARA-derived oxylipins compared to low doses (80:40 mg/kg/day). Differences were detected in EPA metabolites without a significant increase in plasma DHA.

Abstract

BackgroundIncidence of anal cancer (AC) caused by persistent human papillomavirus (HPV) infection has risen in the last years in men who have sex with men (MSM) living with HIV. There is consensus that this population should be screened for anal precancerous lesions, but the role of HPV DNA testing in AC screening programmes is still under debate.ObjectivesThis study employed two molecular test to detect anal HPV DNA and compared assay performance and prognostic value for the diagnosis of histology proven high-grade intraepithelial anal lesions.MethodsMSM living with HIV attended their regular check-up visits consisting of detection of anal HPV infection, anal cytology, digital anorectal examination and high resolution anoscopy. HPV DNA was detected using Hybrid Capture 2 High-Risk test (HC2, total assay) and LINEAR ARRAY HPV Genotyping Test (LA, type-specific assay)ResultsAmong 274 participant, prevalence of HPV DNA was 48.5% by HC2 and 89.4% by LA. HPV16 (30.6%) and HPV6 (19.6%) were the most common genotypes identified. Prevalence of multiple HPV infections was 56.2%. Agreement between HPV DNA assays was 75.2% (κ=0.51; 95% CI 0.42 to 0.60). Total HPV detection demonstrated high sensitivity (90%; 95% CI 68.3 to 98.8) and moderate specificity (58.4%; 95% CI 50.2 to 66.3), while type-specific HPV16/18 genotyping provided an increase in specificity and showed the highest area under the curve (0.81; 95% CI 0.74 to 0.89) and Youden’s index (0.63).ConclusionsBoth methodologies identified a high prevalence of anal HPV infection and multiple HPV infections in MSM living with HIV, showing a moderate overall agreement between them. Either total HPV detection or type-specific HPV16/18 detection together with a threshold ≥atypical squamous cells of undetermined significance for abnormal cytology showed an acceptable diagnostic accuracy.

Abstract

ArchiteXt Mining: Spanish Modern Architecture through Its Texts (1939–1975) is a research project funded by the Government of Spain through the 2015 Call for “Excellence Projects” of the Ministry of Economy and Competitiveness. This project aims to explore a new viewpoint and look into the special features of Spanish modern architecture. Despite the increasing success of using data analysis as a tool in a variety of disciplines, research on architectural theory has never made the most efficient use of these technologies. The Spanish and international circumstances of modern architecture development have been scrutinized through qualitative research, which has established a shared theoretical ground. It is now time to start a new in-depth research based on objective data. To address this challenge, we propose the application of text mining techniques to take advantage of the best data source in the field: architectural periodicals. The purpose is to create a powerful database hosted on a public website for the scientific community. Thus, this project fulfils several e-Research objectives: to facilitate the computerization of data research, to support every stageof data collection, and to manage big data analyses with thehelp of specific tools.

Abstract

REIPI-SEIMC COVID-19 group and COVID@HULP group., [Background] The clinical presentation of COVID-19 in patients admitted to hospital is heterogeneous. We aimed to determine whether clinical phenotypes of patients with COVID-19 can be derived from clinical data, to assess the reproducibility of these phenotypes and correlation with prognosis, and to derive and validate a simplified probabilistic model for phenotype assignment. Phenotype identification was not primarily intended as a predictive tool for mortality., [Methods] In this study, we used data from two cohorts: the COVID-19@Spain cohort, a retrospective cohort including 4035 consecutive adult patients admitted to 127 hospitals in Spain with COVID-19 between Feb 2 and March 17, 2020, and the COVID-19@HULP cohort, including 2226 consecutive adult patients admitted to a teaching hospital in Madrid between Feb 25 and April 19, 2020. The COVID-19@Spain cohort was divided into a derivation cohort, comprising 2667 randomly selected patients, and an internal validation cohort, comprising the remaining 1368 patients. The COVID-19@HULP cohort was used as an external validation cohort. A probabilistic model for phenotype assignment was derived in the derivation cohort using multinomial logistic regression and validated in the internal validation cohort. The model was also applied to the external validation cohort. 30-day mortality and other prognostic variables were assessed in the derived phenotypes and in the phenotypes assigned by the probabilistic model., [Findings] Three distinct phenotypes were derived in the derivation cohort (n=2667)—phenotype A (516 [19%] patients), phenotype B (1955 [73%]) and phenotype C (196 [7%])—and reproduced in the internal validation cohort (n=1368)—phenotype A (233 [17%] patients), phenotype B (1019 [74%]), and phenotype C (116 [8%]). Patients with phenotype A were younger, were less frequently male, had mild viral symptoms, and had normal inflammatory parameters. Patients with phenotype B included more patients with obesity, lymphocytopenia, and moderately elevated inflammatory parameters. Patients with phenotype C included older patients with more comorbidities and even higher inflammatory parameters than phenotype B. We developed a simplified probabilistic model (validated in the internal validation cohort) for phenotype assignment, including 16 variables. In the derivation cohort, 30-day mortality rates were 2·5% (95% CI 1·4–4·3) for patients with phenotype A, 30·5% (28·5–32·6) for patients with phenotype B, and 60·7% (53·7–67·2) for patients with phenotype C (log-rank test p, [Interpretation] Patients admitted to hospital with COVID-19 can be classified into three phenotypes that correlate with mortality. We developed and validated a simplified tool for the probabilistic assignment of patients into phenotypes. 47. Revisión del tratamiento del uso conjunto del tabaco y del cannabis

Abstract

El consumo conjunto y paralelo de tabaco y cannabis es un fenómeno de tendencia creciente, especialmente entre los jóvenes, y que contiene importantes implicaciones clínicas que conviene clarificar a la vista de la diversidad de pautas de abordaje existentes. Se realizó una revisión bibliográfica de carácter descriptivo o cualitativo sobre el tratamiento del consumo de tabaco y cannabis. De la revisión realizada se concluyen varias recomendaciones: el abordaje conjunto de ambos consumos, la orientación a la abstinencia completa e indefinida, la individualización del tratamiento y la atención a los diversos objetivos parciales. Entre los tratamientos psicológicos, está bien establecida la efectividad de la entrevista motivacional, la terapia cognitivo-conductual y la terapia de incentivo con refuerzo comunitario. Entre los tratamientos farmacológicos, cuenta con evidencia el uso de sustitutivos de nicotina, bupropion y vareniclina para la dependencia del tabaco; en el caso del cannabis, no existen medicamentos aprobados, si bien existen líneas de trabajo interesantes con naltrexona, n-acetilcisteína, gabapentina y agonistas cannabinoides. Finalmente, los últimos años han visto una eclosión de aplicaciones móviles (mHealth) que ofrecen importantes ventajas y que han sido ya objeto de revisión sistemática.

Tanzania has one of the lowest levels of access to sanitation, with increasing open defecation. Ending open defecation has been identified as a top priority for reducing global inequalities in WASH (water, sanitation and hygiene). In this context, the coordinated work of NGOs, universities and governments developed a hygiene and sanitation programme (UMATA), whose methodologies and lessons learnt are ready to be scaled up. This research analyses the information collected from more than 21,000 households and discusses the challenges of monitoring and tracking the degree of achievement of the Sustainable Development Goals and the human right to sanitation. A characterisation of the hygiene and sanitation situation and the degree of adoption of different latrine types was assessed before (baseline) and after (follow-up) the implementation of the UMATA programme using the WHO/UNICEF JMP (Joint Monitoring Programme for Water Supply, Sanitation and Hygiene) metrics and approach. This allowed evaluating the efficacy of the strategy implemented in Tanzania to reduce open defecation that led to a decrease from 40.7% to 9.70% in the monitored period. Finally, we discuss the critical economic factors affecting the adoption and sustainability of improved sanitation systems.
