ABSTRACT Objective To describe the prevalence of hormone use, sociodemographic profile, and access to health services among the transgender, transvestite, and nonbinary population in Porto Alegre, capital city of Rio Grande do Sul state. Methods A cross-sectional study was conducted between September and November 2021, using a snowball sampling method (n=65). Data were collected through a self-administered questionnaire with transgender people aged 18 years or older. Results High hormone use was observed (n=47), with a higher occurrence of self-medication among transgender women, transvestites, and transfeminine people (n=10). Health monitoring predominantly occurs in gender identity outpatient clinics (n=56). Episodes of transphobia when accessing health services were reported by 28 individuals. Conclusion This study demonstrated high prevalence of hormone use, highlighted specific patterns of use across different gender identities, and the need for improvements in access to healthcare services.

Introdução: Porto Alegre é a capital com um dos cenários epidemiológicos mais críticos no que tange aos indicadores de HIV e Aids no Brasil. Em 2022, ocupou o terceiro lugar no Boletim Epidemiológico Nacional de 2023 na taxa de detecção para o HIV (47,9 casos por 100 mil habitantes) e o primeiro no coeficiente de mortalidade por Aids (23,8 óbitos por 100 mil habitantes). O acesso às estratégias de prevenção combinada, como a Profilaxia Pós-Exposição ao HIV (PEP), é central para promover mudanças neste cenário. Tendo em vista que a PEP é uma urgência médica que deve ser iniciada em até 72 horas após a exposição, torna-se fundamental a ampliação e descentralização do seu acesso por meio da oferta em outros locais além dos Serviços de Atenção Especializada (SAE), como as unidades de Atenção Primária à Saúde (APS). Objetivo: Qualificar os profissionais da APS de Porto Alegre sobre a PEP para promover a ampliação do acesso a esta estratégia de prevenção combinada no município. Método: Trata-se de um relato de experiência sobre o processo de educação continuada realizado com os médicos e enfermeiros da APS de Porto Alegre em 2023 realizados pela Coordenação de Atenção à Tuberculose, IST, HIV/Aids e Hepatites virais com participação dos médicos e enfermeiros dos SAE. Resultados: A capacitação foi realizada nas 04 coordenadorias (Norte, Leste, Sul e Oeste) e contemplou as 16 unidades de saúde da APS de turno estendido (Funcionamento até 22 horas) do município. Ao todo, foram capacitados 74 profissionais de saúde, entre médicos(as) e enfermeiros(as). Após a capacitação, foi oportunizado o matriciamento dos profissionais capacitados com especialistas dos SAEs do município para sanar possíveis dúvidas durante os atendimentos. Pode-se observar, de acordo com o painel PEP, uma tendência no aumento das prescrições desta profilaxia quando comparado aos anos de 2022 (3.182) e 2023 (3.673). Conclusão: Tendo em vista o grave cenário epidemiológico de Porto Alegre frente ao HIV e a Aids, faz-se de suma importância a expansão e diversificação do acesso à PEP. Portanto, a capacitação de 100% das US de turno estendido configura-se como uma importante via de redução dos novos casos de HIV no município.

Abstract Basil has aroused biological, pharmacological, agronomic and industrial interests, and stand out in the essential oil production. This study aimed to conduct cytogenetoxicity analysis through the Allium cepa L. test, of aqueous extracts and essential oil of basil (Ocimum basilicum L. “Basilicão”) grown under the conditions of presence or absence of induction to saline stress. The basil was grown in a greenhouse in two seasons, summer and winter, in which some of the plants received induction to saline stress (solution of electrical conductivity of 5 dSm-1) and the other part did not receive induction to saline stress (electric conductivity 1 dSm-1). With the collected plants, was prepare the aqueous extracts of the leaves (5 e 10 gL-1) and the extraction of essential oil of the leaves and inflorescences (0.10%) of basil, which were used for the cytogenetoxicity analysis by A. cepa test. Only the treatment with aqueous extract (5 gL-1) of basil grown under induction to saline stress and the treatment with aqueous extract (10 gL-1) of basil grown without induction to saline stress (both during the winter) are shown to have antiproliferative potential. As for the genotoxic potential, only the treatment with essential oil (0.10%) of plants which received induction to saline stress during the summer has that effect. The other treatments have not cytogenotoxic potential.

Background During a quit attempt, cues from a smoker’s environment are a major cause of brief smoking lapses, which increase the risk of relapse. Quit Sense is a theory-guided Just-In-Time Adaptive Intervention smartphone app, providing smokers with the means to learn about their environmental smoking cues and provides ‘in the moment’ support to help them manage these during a quit attempt. Objective To undertake a feasibility randomised controlled trial to estimate key parameters to inform a definitive randomised controlled trial of Quit Sense. Design A parallel, two-arm randomised controlled trial with a qualitative process evaluation and a ‘Study Within A Trial’ evaluating incentives on attrition. The research team were blind to allocation except for the study statistician, database developers and lead researcher. Participants were not blind to allocation. Setting Online with recruitment, enrolment, randomisation and data collection (excluding manual telephone follow-up) automated through the study website. Participants Smokers (323 screened, 297 eligible, 209 enrolled) recruited via online adverts on Google search, Facebook and Instagram. Interventions Participants were allocated to ‘usual care’ arm (n = 105; text message referral to the National Health Service SmokeFree website) or ‘usual care’ plus Quit Sense (n = 104), via a text message invitation to install the Quit Sense app. Main outcome measures Follow-up at 6 weeks and 6 months post enrolment was undertaken by automated text messages with an online questionnaire link and, for non-responders, by telephone. Definitive trial progression criteria were met if a priori thresholds were included in or lower than the 95% confidence interval of the estimate. Measures included health economic and outcome data completion rates (progression criterion #1 threshold: ≥ 70%), including biochemical validation rates (progression criterion #2 threshold: ≥ 70%), recruitment costs, app installation (progression criterion #3 threshold: ≥ 70%) and engagement rates (progression criterion #4 threshold: ≥ 60%), biochemically verified 6-month abstinence and hypothesised mechanisms of action and participant views of the app (qualitative). Results Self-reported smoking outcome completion rates were 77% (95% confidence interval 71% to 82%) and health economic data (resource use and quality of life) 70% (95% CI 64% to 77%) at 6 months. Return rate of viable saliva samples for abstinence verification was 39% (95% CI 24% to 54%). The per-participant recruitment cost was £19.20, which included advert (£5.82) and running costs (£13.38). In the Quit Sense arm, 75% (95% CI 67% to 83%; 78/104) installed the app and, of these, 100% set a quit date within the app and 51% engaged with it for more than 1 week. The rate of 6-month biochemically verified sustained abstinence, which we anticipated would be used as a primary outcome in a future study, was 11.5% (12/104) in the Quit Sense arm and 2.9% (3/105) in the usual care arm (estimated effect size: adjusted odds ratio = 4.57, 95% CIs 1.23 to 16.94). There was no evidence of between-arm differences in hypothesised mechanisms of action. Three out of four progression criteria were met. The Study Within A Trial analysis found a £20 versus £10 incentive did not significantly increase follow-up rates though reduced the need for manual follow-up and increased response speed. The process evaluation identified several potential pathways to abstinence for Quit Sense, factors which led to disengagement with the app, and app improvement suggestions. Limitations Biochemical validation rates were lower than anticipated and imbalanced between arms. COVID-19-related restrictions likely limited opportunities for Quit Sense to provide location tailored support. Conclusions The trial design and procedures demonstrated feasibility and evidence was generated supporting the efficacy potential of Quit Sense. Future work Progression to a definitive trial is warranted providing improved biochemical validation rates. Trial registration This trial is registered as ISRCTN12326962. Plain language summary Smokers often fail to quit because of urges to smoke triggered by their surroundings (e.g. being around smokers). We developed a smartphone app (‘Quit Sense’) which learns about an individual’s surroundings and locations where they smoke. During a quit attempt, Quit Sense uses in-built sensors to identify when smokers are in those locations and sends ‘in the moment’ advice to help prevent them from smoking. We ran a feasibility study to help plan for a future large study to see if Quit Sense helps smokers to quit. This feasibility study was designed to tell us how many participants complete study measures; recruitment costs; how many participants install and use Quit Sense; and estimate whether Quit Sense may help smokers to stop and how it might do this. We recruited 209 smokers using online adverts on Google search, Facebook and Instagram, costing £19 per participant. Participants then had an equal chance of receiving a web link to the National Health Service SmokeFree website (‘usual care group’) or receive that same web link plus a link to the Quit Sense app (‘Quit Sense group’). Three-quarters of the Quit Sense group installed the app on their phone and half of these used the app for more than 1 week. We followed up 77% of participants at 6 months to collect study data, though only 39% of quitters returned a saliva sample for abstinence verification. At 6 months, more people in the Quit Sense group had stopped smoking (12%) than the usual care group (3%). It was not clear how the app helped smokers to quit based on study measures, though interviews found that the process of training the app helped people quit through learning about what triggered their smoking behaviour. The findings support undertaking a large study to tell us whether Quit Sense really does help smokers to quit. Scientific summary Background Three million UK smokers attempt to stop smoking each year, but over 80% relapse. A lapse (any smoking) early on in a quit attempt is strongly associated with subsequent relapse back to smoking. Approximately half of all lapses are due to environmental smoking cues which can elicit cravings to smoke, such as the presence of cigarettes or being in a location which a smoker associates with smoking. Few interventions are effective at targeting cue-induced cravings. Smokers are more likely to prevent lapses if they better understand their smoking cues and use lapse prevention strategies, such as controlled deep breathing or avoiding other smokers, to combat these. This would likely help them avoid or cope with cue-induced cravings, but smokers typically lack skills in applying these effectively. If smokers trying to quit used effective lapse prevention strategies, this would very likely increase their chances of success. With patient and public involvement, we developed, refined and piloted a theory-guided Just-In-Time Adaptive Intervention smartphone app called Quit Sense that provides support to help smokers manage environmental cues to smoke as they arise. Pilot studies have shown that Quit Sense can provide ‘in the moment’ support to smokers, including lapse prevention strategies, and that users will engage with the app and find it acceptable. As Quit Sense is such a novel intervention, before a ‘definitive’ randomised controlled trial (RCT) can be conducted to test effectiveness, it is important to first undertake a RCT to establish whether such a study might be feasible. Objectives The main objective was to conduct a feasibility RCT of Quit Sense to inform a definitive effectiveness trial, by estimating: completion rates for the anticipated primary outcome for a full trial (6-month self-reported abstinence with biochemical validation, based on the Russell standard); usual care arm cessation rate; cost of recruitment using online advertising; rates of app installation, use and acceptability; completion of smoking cessation-related resource use and quality-of-life data; intervention effect on anticipated primary outcome; intervention effect on hypothesised mechanisms of action of app at 6 weeks post enrolment; participant views of the app, as part of a qualitative process evaluation. Design A parallel, two-arm RCT with an embedded qualitative interview process evaluation was undertaken. The evaluation included a preliminary cost-effectiveness analysis and a nested randomised study [Study Within A Trial (SWAT)] assessing the effects of different financial incentives on 6-month follow-up rates. Setting The study setting was online and trial procedures were primarily automated. Participants Participants were recruited via online adverts on Google search, Facebook and Instagram, screened for eligibility on the study website and enrolled if they were regular smokers, aged 16 years and above, were willing to make a quit attempt and owned an Android smartphone. Interventions After completing the baseline questionnaire participants were then randomly allocated to a ‘usual care’ arm (text message referral to NHS SmokeFree website) or a ‘usual care’ plus Quit Sense arm, via a text message invitation to install the Quit Sense app. The Quit Sense app required users to set a quit date and then, leading up to their quit date, alongside other support features, invited them to report their smoking in real time and indicate the presence of key environmental smoking cues using a smoking behaviour logging tool. Once their quit date arrived, the app used location sensing and what it had learnt about the individual’s smoking behaviour to tailor the timing and content of support messages when they spent time in self-identified high-risk locations. The support and advice aimed to help smokers learn about their smoking behaviour, prepare for their quit attempt and promoted the use of effective lapse prevention strategies during their quit attempt. Main outcomes measures All participants were contacted at 6-week and 6-month follow-up. This was done initially by text message with an embedded link to the follow-up questionnaires or, if no response, manually by telephone. As part of a SWAT, participants underwent secondary randomisation to receive either a £10 or £20 incentive for completion of the 6-month questionnaire. Four progression criteria and corresponding thresholds for seeking to undertake a definitive trial were set a priori. Criteria were met if each threshold was included in or lower than the 95% confidence interval (CI) of the estimate. Trial measures included health economic and outcome data completion rates (progression criterion #1 threshold: ≥ 70%), including biochemical validation rates (progression criterion #2 threshold: ≥ 70%), recruitment costs, app installation (progression criterion #3 threshold: ≥ 70%) and engagement rates (progression criterion #4 threshold: ≥ 60%), the app’s effect on biochemically verified abstinence at 6 months (anticipated primary outcome of definitive trial) and hypothesised mechanisms of action. The qualitative process evaluation sought to identify participants’ views of the trial and the Quit Sense app. The protocol and statistical and health economics analysis plan were pre-specified and published (open access). Results Of the people screened (N = 323), 299 (93%) were eligible and 209 (65% of screened) were consented and randomised either to the usual care arm (n = 105) or to the Quit Sense arm (n = 104). Completion of self-reported smoking questions was 71% (149/209; 95% CI 65% to 77%) and 77% (160/209; 95% CI 71% to 82%) at 6 weeks and 6 months, respectively, and at 6 months 70% provided both cessation support resource use data and EuroQol-5 Dimensions five level (EQ-5D-5L) data. Among those self-reporting abstinence at 6 months follow-up, 39% (95% CI 24% to 54%) returned a viable sample for biochemical verification, although return rates were higher in the Quit Sense than the usual care arm (52% vs. 19%). Online recruitment was completed successfully within the planned 6-week period, split into two campaigns, with a per-participant cost of £19.20, split into advert (£5.82) and running costs (£13.38). In the Quit Sense arm, 75% (95% CI 67% to 83%) of participants installed the app and, of these, 100% set a quit date within the app and 51% (95% CI 39% to 63%) engaged with it for more than 1 week. At final follow-up, the rate of 6-month biochemically verified sustained abstinence, which we anticipated would be used as a primary outcome in a future study, was 11.5% (12/104) in the Quit Sense arm and 2.9% (3/105) in the usual care arm (estimated effect size: adjusted odds ratio = 4.57, 95% CI 1.23 to 16.94). Similar effects were observed for biochemically verified secondary abstinence outcomes at 6-month follow-up and sensitivity analyses. Effect sizes were smaller and non-significant for self-report only abstinence and there was no evidence of a between-arm difference in abstinence at 6-week follow-up. Participants in the Quit Sense arm had higher rates of lapse avoidance in the first 2 weeks of their quit attempt or post enrolment compared to usual care though this was not statistically significant (29.6% vs. 19.2%; p = 0.14). There was no evidence of between-arm differences in hypothesised mechanisms of action of the app, including the mean frequency of lapse prevention strategy use [mean difference (MD) −0.07; p = 0.46], smoking cessation self-efficacy (MD 0.18; p = 0.39), strength (p = 0.23) and frequency (p = 0.83) of urges to smoke or the Wisconsin Inventory of Smoking Dependence Motives subscales of automaticity (p = 0.51) and associative processes (p = 0.58). The qualitative process evaluation identified several potential pathways to abstinence among Quit Sense arm participants. Interviewed participants reported finding the insights gained from engaging with the smoking behaviour learning tool before their quit attempt started particularly valuable. They reported that this reinforced their commitment to quit, helped them better understand the drivers of their smoking behaviour and challenged the need for them to smoke. Some also reported finding regular and location-specific support messages encouraging and motivational and that this reinforced the goal of quitting and made them feel equipped when they spent time in locations that they used to smoke in. Participants highlighted several factors which led to disengagement with the app, including relapse, no longer feeling they needed support, not finding the app met their needs and technical issues. Participants also provided suggestions for app improvement relating to logging smoking, adding gamification elements and improved support triggering. Interviewed participants from both arms highlighted how COVID-19 measures had affected their smoking behaviour and restricted the time they spent outside of their home, which would have had implications for location-based cessation support. In terms of the main cost-drivers, e-cigarettes/vaporisers and nicotine replacement therapy accounted for more than 70% of the total non-intervention costs. The total intervention cost (recruitment advertising and maintaining the Quit Sense app) was estimated to be £28.51 per participant. In both groups, the mean EQ-5D-5L score at both 6-week and 6-month follow-up was lower than that at baseline. A preliminary cost-effectiveness analysis using EQ-5D-5L score as the outcome estimated that the intervention was both more costly and less effective, compared to standard care, though there was no significant difference in cost or effect between the two groups. The SWAT analysis indicated that increasing the incentive for completing follow-up from £10 to £20 did not increase response rates at 6 months (74% vs. 79%; p = 0.36) but did reduce the proportion of participants requiring manual follow-up (62% vs. 46%; p = 0.018) and the median response time to responding (15 days vs. 7 days; p = 0.016). Conclusions The Quit Sense RCT design and procedures demonstrated feasibility and generated preliminary efficacy evidence of the app on abstinence at 6-month follow-up, although how the app may achieve this is not clear. Three out of the four pre-specified feasibility progression criteria for moving to a definitive trial – completion rate of self-reported abstinence at final follow-up and the rate of Quit Sense installation and engagement – were met. The return of saliva samples among quitters was lower than anticipated, though this can likely be increased in a future trial through increased incentivisation and enhanced procedures. Progression to a definitive trial is warranted. Trial registration This trial is registered as ISRCTN12326962. Introduction Over 50% of older adults are prescribed a medicine where the risk of harm outweighs the chances of benefit. During a hospital admission, older adults and carers expect medicines to be reviewed for appropriateness and any inappropriate medicines proactively deprescribed. While the principle of proactive deprescribing is an expectation of good prescribing practice, it is yet to become routine. The CompreHensive geriAtRician-led MEdication Review (CHARMER) study aims to develop and test a five-component behaviour change intervention to equip geriatricians and pharmacists to proactively deprescribe inappropriate medicines with older adults in hospital. This study aims to test the feasibility and acceptability of study processes and CHARMER implementation.Methods and analysis A two-arm purposive allocation feasibility study is being undertaken at four acute hospitals in England, UK (three intervention and one control). The target sample is 400 patients across all hospitals. Primary outcome measures are: (1) participant recruitment rate and (2) participant attrition rate. Secondary outcome measures are: (1) hospital readmission rate; (2) mortality rate and (3) quality of life. Quantitative data will be checked for completeness and quality, and practitioner and patient demographics descriptively analysed. We will undertake a rapid qualitative analysis on observations, interviews and study meeting minutes data. A subsequent thematic analysis will be undertaken with codes mapped to the Theoretical Domains Framework and Normalisation Process Theory. Triangulation of qualitative and quantitative data will be undertaken.Ethics and dissemination Ethics approval was obtained from Wales Research Ethics Committee 1 (IRAS ID 312494) and study approval from the Health Research Authority (22/WA/0087). Informed consent will be sought from all hospital staff involved in data collection activities and for patients involved in enhanced data collection activities. The findings of this study will be disseminated in peer-reviewed journals and conference presentations.Trial registration ISRCTN11899506.

Introdução: A Profilaxia Pós-Exposição (PEP) de risco à infecção pelo Vírus da Imunodeficiência Humana (HIV) é fornecida no Brasil com o objetivo de ampliar as formas de intervenção para evitar novas infecções pelo HIV, sendo necessário iniciar o uso em período inferior a 72 horas após a exposição sexual, uma vez que é considerada uma emergência médica. Conforme Protocolo Clinico e Diretrizes Terapêuticas, o esquema preferencial em adultos constitui-se de 1 comprimido de Dolutegravir 50mg+ Tenofovir 300mg +Lamivudina 300mg por 28 dias. O farmacêutico é um profissional estratégico dentro dos serviços de saúde e possui suas atividades clínicas, incluindo a prescrição de medicamentos, respaldadas através de legislações específicas como a Resolução do Conselho Federal de Farmácia 713/2021.Objetivos: Descrever a experiência da implantação do serviço de prescrição farmacêutica de PEP em uma instituição pública de Porto Alegre. Métodos: Nesse protocolo, são elegíveis pessoas que atendam os seguintes requisitos: maiores de 18 anos de idade, que tenham tido relação sexual consentida desprotegida ou com rompimento de preservativo, sem contraindicação ao esquema preferencial e sorologia negativa para o HIV. A coleta de dados foi realizada com uso de dados secundários obtidos de prontuários farmacêuticos, fichas de atendimento e banco de dados do Centro de Testagem e Aconselhamento (CTA) de usuários que acessaram o serviço para realização de PEP no período de 01 de dezembro de 2021 até 06 de julho de 2022.Resultados: Desde então, 86 usuários já foram atendidos. Apenas uma pessoa apresentou intercorrências durante o tratamento ocasionadas por efeitos adversos requerendo atendimento médico. Não houve necessidade de interrupção da PEP em nenhuma das pessoas prescritas , não houve relato de soroconversão do HIV e mais de 50% dos usuários atendidos retornaram ao CTA para testagem após conclusão do uso dos medicamento. O monitoramento dos pacientes está sendo feito através de indicadores como o retorno para realização do teste rápido de HIV , avaliação de efeitos adversos e recorrência ao uso de PEP , o qual sugere abordagem de indicação para Profilaxia PréExposição (PrEP).Conclusão: A iniciativa, que é pioneira no estado do Rio Grande do Sul, contribui para consolidação da prescrição farmacêutica nos serviços públicos e para aumentar o acesso da população às ações de prevenção do HIV

Early puberty has been found to be associated with adverse health outcomes such as metabolic and cardiovascular diseases and hormone-dependent cancers. The decrease in age at menarche observed during the past decades has been linked to an increased exposure to endocrine-disrupting compounds (EDCs). Evidence for the association between PFAS and phthalate exposure and menarche onset, however, is inconsistent. We studied the association between PFAS and phthalate/DINCH exposure and age at menarche using data of 514 teenagers (12 to 18 years) from four aligned studies of the Human Biomonitoring for Europe initiative (HBM4EU): Riksmaten Adolescents 2016–2017 (Sweden), PCB cohort (follow-up; Slovakia), GerES V-sub (Germany), and FLEHS IV (Belgium). PFAS concentrations were measured in blood, and phthalate/DINCH concentrations in urine. We assessed the role of each individual pollutant within the context of the others, by using different multi-pollutant approaches, adjusting for age, age- and sex-standardized body mass index z-score and household educational level. Exposure to di(2-ethylhexyl) phthalate (DEHP), especially mono(2-ethyl-5-hydroxyhexyl) phthalate (5OH-MEHP), was associated with an earlier age at menarche, with estimates per interquartile fold change in 5OH-MEHP ranging from −0.34 to −0.12 years in the different models. Findings from this study indicated associations between age at menarche and some specific EDCs at concentrations detected in the general European population, but due to the study design (menarche onset preceded the chemical measurements), caution is needed in the interpretation of causality.

Contrary to the initial hypothesis, Flemish adolescents who reported consuming organic food at least 7.5 times per week did not exhibit reduced internal exposure to the tested recently used pesticides. After adjustment for gender, age, country of origin, socioeconomic status, body mass index, consumption of high-fat foods and foods linked to organic food consumption, and concerning organochlorine derivatives and lead, additional adjustment for the duration of breastfeeding expressed in weeks, they displayed slightly elevated internal exposure to organochlorine derivatives, lead, methyl arsenate, and toxic relevant arsenic. A comparison was also made between the correlation of internal exposure to pollutants with the frequency of organic food consumption on one hand and the total consumption of equivalent products from all sources on the other. Regarding potatoes, vegetables, and fruits, no clear trends were observed. Regarding eggs, there was a trend towards higher internal exposures with organic food consumption, significant for trans-nonachlor, PCB118, and 2,4-dichlorophenoxyacetic acid, and marginally significant for glyphosate. For dairy, there was a trend towards higher internal exposures with organic food consumption, significant for perfluorononanoic acid and marginally significant for PCB153. Regarding nuts and seeds, the higher internal exposure to dichlorophenoxyacetic acid and the lower exposure to 3-phenoxybenzoic acid were marginally significant, while there was also a trend towards higher internal exposure to other pollutants with organic food consumption, significant for PCB118, PCB153, and sum PCBs, and marginally significant for trans-nonachlor. Introduction Poor retention in clinical trials can impact on statistical power, reliability, validity, and generalizability of findings and is a particular challenge in smoking cessation studies. In online trials with automated follow-up mechanisms, poor response also increases the resource need for manual follow-up. This study compared two financial incentives on response rates at 6 months follow up, in an online, automated smoking cessation feasibility trial of a cessation smartphone app (Quit Sense). Aims and Methods A study within a trial (SWAT), embedded within a host randomized controlled trial. Host trial participants were randomized 1:1 to receive either a £10 or £20 voucher incentive, for completing the 6-month questionnaire. Stratification for randomization to the SWAT was by minimization to ensure an even split of host trial arm participants and by 6-week response rate. Outcome measures were: Questionnaire completion rate, time to completion, number of completers requiring manual follow-up, and completeness of responses. Results Two hundred and four participants were randomized to the SWAT. The £20 and £10 incentives did not differ in completion rate at 6 months (79% vs. 74%; p  = .362) but did reduce the proportion of participants requiring manual follow-up (46% vs. 62%; p  = .018) and the median completion time (7 days vs. 15 days; p  = .008). Measure response completeness rates were higher among £20 incentive participants, though differences were small for the host trial's primary smoking outcome. Conclusions Benefits to using relatively modest increases in incentive for online smoking cessation trials include more rapid completion of follow-up questionnaires and reduced manual follow-up. Implications A modest increase in incentive (from £10 to £20) to promote the completion of follow-up questionnaires in online smoking cessation trials may not increase overall response rates but could lead to more rapid data collection, a reduced need for manual follow-up and reduced missing data among those who initiate completing a questionnaire. Such an improvement may help to reduce bias, increase validity and generalizability, and improve statistical power in smoking cessation trials. Abstract Background Exposure to air pollution during pregnancy has been associated with adverse pregnancy outcomes in studies worldwide, other studies have described beneficial effects of residential greenspace on pregnancy outcomes. The biological mechanisms that underlie these associations are incompletely understood. A biological stress response, which implies release of cortisol, may underlie associations of air pollution exposure and access to neighborhood greenspaces with health. Methods We explored residential exposure to air pollution and residential access to neighborhood greenspaces in relation to hair cortisol concentrations of participants in a prospective pregnancy cohort study in Flanders, Belgium. Hair samples were collected at the end of the second pregnancy trimester (n = 133) and shortly after delivery (n = 81). Cortisol concentrations were measured in 3-cm scalp-near hair sections, to reflect second and third pregnancy trimester cortisol secretion. We estimated long-term (3 months before sampling) residential exposure to fine particulate matter (PM2.5), nitrogen dioxide (NO2) and black carbon (BC), assessed residential distance to major roads and residential access to neighborhood greenspaces (NHGS). Associations between residential exposures and hair cortisol concentrations were studied using linear regression models while adjusting for season of sampling. Results Three-month mean residential NO2 and BC concentrations were positively associated with third pregnancy trimester hair cortisol concentrations (p = 0.008 and p = 0.017). Access to a large NHGS (10 ha or more within 800 m from residence) was negatively associated with third trimester hair cortisol concentrations (p = 0.019). Access to a large NHGS significantly moderated the association between residential proximity to major roads and second trimester hair cortisol concentrations (p = 0.021). Residential distance to major roads was negatively associated with second trimester hair cortisol concentrations of participants without access to a large NHGS (p = 0.003). Chronic biological stress may adversely affect adolescents' physical and mental health, but insight in the personal and environmental factors that determine chronic stress is limited. We measured 3-month cumulative hair cortisol concentration (HCC) in 419 adolescents, participating in the Flemish Environment and Health Study. Adolescents' health and lifestyle characteristics, household and neighborhood socio-economic status as well as neighborhood urbanicity were assessed as potential determinants of HCC, using multiple linear regression models. We additionally explored heterogeneity of our results by sex. HCC were significantly higher in boys from densely populated neighborhoods, the association was not significant in girls. Accordingly, boys living outside cities had significantly lower HCC than boys, living in cities. HCC was significantly lower in adolescents with an optimal vitality, a measure of a positive mental health status. In adolescent girls, menarcheal status (pre-/postmenarche) was a significant determinant of HCC. Our findings are the first to suggest that residential urbanicity may have an impact on chronic biological stress in a general population of adolescent boys.

Background: During a quit attempt, cues from a smoker's environment are a major cause of brief smoking lapses, which increase the risk of relapse. Quit Sense is a theory-guided Just-In-Time Adaptive Intervention smartphone app, providing smokers with the means to learn about their environmental smoking cues and provides 'in the moment' support to help them manage these during a quit attempt. Objective: To undertake a feasibility randomised controlled trial to estimate key parameters to inform a definitive randomised controlled trial of Quit Sense. Design: A parallel, two-arm randomised controlled trial with a qualitative process evaluation and a 'Study Within A Trial' evaluating incentives on attrition. The research team were blind to allocation except for the study statistician, database developers and lead researcher. Participants were not blind to allocation. Setting: Online with recruitment, enrolment, randomisation and data collection (excluding manual telephone follow-up) automated through the study website. Participants: Smokers (323 screened, 297 eligible, 209 enrolled) recruited via online adverts on Google search, Facebook and Instagram. Interventions: Participants were allocated to 'usual care' arm (n = 105; text message referral to the National Health Service SmokeFree website) or 'usual care' plus Quit Sense (n = 104), via a text message invitation to install the Quit Sense app. Main outcome measures: Follow-up at 6 weeks and 6 months post enrolment was undertaken by automated text messages with an online questionnaire link and, for non-responders, by telephone. Definitive trial progression criteria were met if a priori thresholds were included in or lower than the 95% confidence interval of the estimate. Measures included health economic and outcome data completion rates (progression criterion #1 threshold: ≥ 70%), including biochemical validation rates (progression criterion #2 threshold: ≥ 70%), recruitment costs, app installation (progression criterion #3 threshold: ≥ 70%) and engagement rates (progression criterion #4 threshold: ≥ 60%), biochemically verified 6-month abstinence and hypothesised mechanisms of action and participant views of the app (qualitative). Results: Self-reported smoking outcome completion rates were 77% (95% confidence interval 71% to 82%) and health economic data (resource use and quality of life) 70% (95% CI 64% to 77%) at 6 months. Return rate of viable saliva samples for abstinence verification was 39% (95% CI 24% to 54%). The per-participant recruitment cost was £19.20, which included advert (£5.82) and running costs (£13.38). In the Quit Sense arm, 75% (95% CI 67% to 83%; 78/104) installed the app and, of these, 100% set a quit date within the app and 51% engaged with it for more than 1 week. The rate of 6-month biochemically verified sustained abstinence, which we anticipated would be used as a primary outcome in a future study, was 11.5% (12/104) in the Quit Sense arm and 2.9% (3/105) in the usual care arm (estimated effect size: adjusted odds ratio = 4.57, 95% CIs 1.23 to 16.94). There was no evidence of between-arm differences in hypothesised mechanisms of action. Three out of four progression criteria were met. The Study Within A Trial analysis found a £20 versus £10 incentive did not significantly increase follow-up rates though reduced the need for manual follow-up and increased response speed. The process evaluation identified several potential pathways to abstinence for Quit Sense, factors which led to disengagement with the app, and app improvement suggestions. Limitations: Biochemical validation rates were lower than anticipated and imbalanced between arms. COVID-19-related restrictions likely limited opportunities for Quit Sense to provide location tailored support. Conclusions: The trial design and procedures demonstrated feasibility and evidence was generated supporting the efficacy potential of Quit Sense. Future work: Progression to a definitive trial is warranted providing improved biochemical validation rates. Trial registration: This trial is registered as ISRCTN12326962. Plain language summary: Smokers often fail to quit because of urges to smoke triggered by their surroundings (e.g. being around smokers). We developed a smartphone app ('Quit Sense') which learns about an individual's surroundings and locations where they smoke. During a quit attempt, Quit Sense uses in-built sensors to identify when smokers are in those locations and sends 'in the moment' advice to help prevent them from smoking. We ran a feasibility study to help plan for a future large study to see if Quit Sense helps smokers to quit. This feasibility study was designed to tell us how many participants complete study measures; recruitment costs; how many participants install and use Quit Sense; and estimate whether Quit Sense may help smokers to stop and how it might do this. We recruited 209 smokers using online adverts on Google search, Facebook and Instagram, costing £19 per participant. Participants then had an equal chance of receiving a web link to the National Health Service SmokeFree website ('usual care group') or receive that same web link plus a link to the Quit Sense app ('Quit Sense group'). Three-quarters of the Quit Sense group installed the app on their phone and half of these used the app for more than 1 week. We followed up 77% of participants at 6 months to collect study data, though only 39% of quitters returned a saliva sample for abstinence verification. At 6 months, more people in the Quit Sense group had stopped smoking (12%) than the usual care group (3%). It was not clear how the app helped smokers to quit based on study measures, though interviews found that the process of training the app helped people quit through learning about what triggered their smoking behaviour. The findings support undertaking a large study to tell us whether Quit Sense really does help smokers to quit. Scientific summary: Background: Three million UK smokers attempt to stop smoking each year, but over 80% relapse. A lapse (any smoking) early on in a quit attempt is strongly associated with subsequent relapse back to smoking. Approximately half of all lapses are due to environmental smoking cues which can elicit cravings to smoke, such as the presence of cigarettes or being in a location which a smoker associates with smoking. Few interventions are effective at targeting cue-induced cravings. Smokers are more likely to prevent lapses if they better understand their smoking cues and use lapse prevention strategies, such as controlled deep breathing or avoiding other smokers, to combat these. This would likely help them avoid or cope with cue-induced cravings, but smokers typically lack skills in applying these effectively. If smokers trying to quit used effective lapse prevention strategies, this would very likely increase their chances of success. With patient and public involvement, we developed, refined and piloted a theory-guided Just-In-Time Adaptive Intervention smartphone app called Quit Sense that provides support to help smokers manage environmental cues to smoke as they arise. Pilot studies have shown that Quit Sense can provide 'in the moment' support to smokers, including lapse prevention strategies, and that users will engage with the app and find it acceptable. As Quit Sense is such a novel intervention, before a 'definitive' randomised controlled trial (RCT) can be conducted to test effectiveness, it is important to first undertake a RCT to establish whether such a study might be feasible. Objectives: The main objective was to conduct a feasibility RCT of Quit Sense to inform a definitive effectiveness trial, by estimating: completion rates for the anticipated primary outcome for a full trial (6-month self-reported abstinence with biochemical validation, based on the Russell standard); usual care arm cessation rate; cost of recruitment using online advertising; rates of app installation, use and acceptability; completion of smoking cessation-related resource use and quality-of-life data; intervention effect on anticipated primary outcome; intervention effect on hypothesised mechanisms of action of app at 6 weeks post enrolment; participant views of the app, as part of a qualitative process evaluation. Design: A parallel, two-arm RCT with an embedded qualitative interview process evaluation was undertaken. The evaluation included a preliminary cost-effectiveness analysis and a nested randomised study [Study Within A Trial (SWAT)] assessing the effects of different financial incentives on 6-month follow-up rates. Setting: The study setting was online and trial procedures were primarily automated. Participants: Participants were recruited via online adverts on Google search, Facebook and Instagram, screened for eligibility on the study website and enrolled if they were regular smokers, aged 16 years and above, were willing to make a quit attempt and owned an Android smartphone. Interventions: After completing the baseline questionnaire participants were then randomly allocated to a 'usual care' arm (text message referral to NHS SmokeFree website) or a 'usual care' plus Quit Sense arm, via a text message invitation to install the Quit Sense app. The Quit Sense app required users to set a quit date and then, leading up to their quit date, alongside other support features, invited them to report their smoking in real time and indicate the presence of key environmental smoking cues using a smoking behaviour logging tool. Once their quit date arrived, the app used location sensing and what it had learnt about the individual's smoking behaviour to tailor the timing and content of support messages when they spent time in self-identified high-risk locations. The support and advice aimed to help smokers learn about their smoking behaviour, prepare for their quit attempt and promoted the use of effective lapse prevention strategies during their quit attempt. Main outcomes measures: All participants were contacted at 6-week and 6-month follow-up. This was done initially by text message with an embedded link to the follow-up questionnaires or, if no response, manually by telephone. As part of a SWAT , participants underwent secondary randomisation to receive either a £10 or £20 incentive for completion of the 6-month questionnaire. Four progression criteria and corresponding thresholds for seeking to undertake a definitive trial were set a priori. Criteria were met if each threshold was included in or lower than the 95% confidence interval (CI) of the estimate. Trial measures included health economic and outcome data completion rates (progression criterion #1 threshold: ≥ 70%), including biochemical validation rates (progression criterion #2 threshold: ≥ 70%), recruitment costs, app installation (progression criterion #3 threshold: ≥ 70%) and engagement rates (progression criterion #4 threshold: ≥ 60%), the app's effect on biochemically verified abstinence at 6 months (anticipated primary outcome of definitive trial) and hypothesised mechanisms of action. The qualitative process evaluation sought to identify participants' views of the trial and the Quit Sense app. The protocol and statistical and health economics analysis plan were pre-specified and published (open access). Results: Of the people screened (N = 323), 299 (93%) were eligible and 209 (65% of screened) were consented and randomised either to the usual care arm (n = 105) or to the Quit Sense arm (n = 104). Completion of self-reported smoking questions was 71% (149/209; 95% CI 65% to 77%) and 77% (160/209; 95% CI 71% to 82%) at 6 weeks and 6 months, respectively, and at 6 months 70% provided both cessation support resource use data and EuroQol-5 Dimensions five level (EQ-5D-5L) data. Among those self-reporting abstinence at 6 months follow-up, 39% (95% CI 24% to 54%) returned a viable sample for biochemical verification, although return rates were higher in the Quit Sense than the usual care arm (52% vs. 19%). Online recruitment was completed successfully within the planned 6-week period, split into two campaigns, with a per-participant cost of £19.20, split into advert (£5.82) and running costs (£13.38). In the Quit Sense arm, 75% (95% CI 67% to 83%) of participants installed the app and, of these, 100% set a quit date within the app and 51% (95% CI 39% to 63%) engaged with it for more than 1 week. At final follow-up, the rate of 6-month biochemically verified sustained abstinence, which we anticipated would be used as a primary outcome in a future study, was 11.5% (12/104) in the Quit Sense arm and 2.9% (3/105) in the usual care arm (estimated effect size: adjusted odds ratio = 4.57, 95% CI 1.23 to 16.94). Similar effects were observed for biochemically verified secondary abstinence outcomes at 6-month follow-up and sensitivity analyses. Effect sizes were smaller and non-significant for self-report only abstinence and there was no evidence of a between-arm difference in abstinence at 6-week follow-up. Participants in the Quit Sense arm had higher rates of lapse avoidance in the first 2 weeks of their quit attempt or post enrolment compared to usual care though this was not statistically significant (29.6% vs. 19.2%; p = 0.14). There was no evidence of between-arm differences in hypothesised mechanisms of action of the app, including the mean frequency of lapse prevention strategy use [mean difference (MD) −0.07; p = 0.46], smoking cessation self-efficacy (MD 0.18; p = 0.39), strength (p = 0.23) and frequency (p = 0.83) of urges to smoke or the Wisconsin Inventory of Smoking Dependence Motives subscales of automaticity (p = 0.51) and associative processes (p = 0.58). The qualitative process evaluation identified several potential pathways to abstinence among Quit Sense arm participants. Interviewed participants reported finding the insights gained from engaging with the smoking behaviour learning tool before their quit attempt started particularly valuable. They reported that this reinforced their commitment to quit, helped them better understand the drivers of their smoking behaviour and challenged the need for them to smoke. Some also reported finding regular and location-specific support messages encouraging and motivational and that this reinforced the goal of quitting and made them feel equipped when they spent time in locations that they used to smoke in. Participants highlighted several factors which led to disengagement with the app, including relapse, no longer feeling they needed support, not finding the app met their needs and technical issues. Participants also provided suggestions for app improvement relating to logging smoking, adding gamification elements and improved support triggering. Interviewed participants from both arms highlighted how COVID-19 measures had affected their smoking behaviour and restricted the time they spent outside of their home, which would have had implications for location-based cessation support. In terms of the main cost-drivers, e-cigarettes/vaporisers and nicotine replacement therapy accounted for more than 70% of the total non-intervention costs. The total intervention cost (recruitment advertising and maintaining the Quit Sense app) was estimated to be £28.51 per participant. In both groups, the mean EQ-5D-5L score at both 6-week and 6-month follow-up was lower than that at baseline. A preliminary cost-effectiveness analysis using EQ-5D-5L score as the outcome estimated that the intervention was both more costly and less effective, compared to standard care, though there was no significant difference in cost or effect between the two groups. The SWAT analysis indicated that increasing the incentive for completing follow-up from £10 to £20 did not increase response rates at 6 months (74% vs. 79%; p = 0.36) but did reduce the proportion of participants requiring manual follow-up (62% vs. 46%; p = 0.018) and the median response time to responding (15 days vs. 7 days; p = 0.016). Conclusions: The Quit Sense RCT design and procedures demonstrated feasibility and generated preliminary efficacy evidence of the app on abstinence at 6-month follow-up, although how the app may achieve this is not clear. Three out of the four pre-specified feasibility progression criteria for moving to a definitive trial – completion rate of self-reported abstinence at final follow-up and the rate of Quit Sense installation and engagement – were met. The return of saliva samples among quitters was lower than anticipated, though this can likely be increased in a future trial through increased incentivisation and enhanced procedures. Despite continued efforts to improve biosecurity protocols, Campylobacter continues to be detected in the majority of commercial chicken flocks across Europe. Using an extensive data set of Campylobacter prevalence within a chicken breeder flock for over a year, multiple Bayesian models are presented to explore the dynamics of the spread of Campylobacter in response to seasonal variation, species-specificity, bird health, and total colonization prevalence. These models indicated that birds within the flock varied greatly in their response to bacterial challenge, and that this phenomenon had a large impact on the overall prevalence of different species of Campylobacter. Campylobacter jejuni appeared more frequently in the summer, while Campylobacter coli persisted for a longer duration, amplified by the most susceptible birds in the flock. Our study suggests that strains of Campylobacter that appear most frequently likely possess no demographic advantage, but are instead amplified due to the health of the birds that ingest it.

During the last 15 years, VITO has established an infrastructure for biobanking a collection of biological samples from the general population in Flanders (Belgium). This biobank was set up to contribute to future, yet unspecified, research questions in the field of environment and health. Biobank@VITO is a population biobank in which bio-specimen including human peripheral blood, cord blood, and blood derivatives (e.g., serum, plasma, cells, RNA, DNA), urine, hair, nails, exhaled breath condensate, saliva DNA, and human breast milk collected from non-diseased populations are preserved. Currently, the biobank stores about 70,000 samples from 7,700 individuals. These biospecimen were collected since 2002 in different human biomonitoring studies comprising European (e.g., DEMOCOPHES, HBM4EU), national (e.g., WHO human breastmilk studies), Flemish (Flemish Environment and Health Study (FLEHS) campaigns), and local (e.g., hotspots, 3xG project) well-defined and ethically approved research projects. Participants to the surveys included different age groups (newborns, children, adolescents, and adults) and were representatively selected with regard to gender, age class, residence, and/or socioeconomic status (SES). In each campaign, samples were stored in the Biobank@VITO. The registration, preservation, and management of the samples in the biobank were done in a qualitative and uniform manner which guarantees the traceability of all samples. The samples in the biobank have an extended information backbone on the lifestyle, environment, and health status of the donor. The biological samples in the biobank are an invaluable archive that can be used to address specific policy and research questions in the future, to test old samples with new technology and according to the latest methods and insights or to measure newly identified pollutants in old samples looking for long-term trends.
