OBJECTIVE To report the outcome of locally administered antibiotic-impregnated poloxamer 407 (P407) hydrogel in dogs diagnosed with orthopedic surgical site infections (SSIs) and to identify risk factors for treatment failure. ANIMALS 34 client-owned dogs diagnosed with an orthopedic surgical site infection treated with local antibiotic-impregnated P407 hydrogel. PROCEDURES Medical records were reviewed of dogs receiving antibiotic-impregnated P407 hydrogel for an active orthopedic SSI between March 2018 and December 2020. The rate of successful infection clearance was calculated. Risk factors for failed treatment were evaluated with statistical analyses. RESULTS 34 dogs met the inclusion criteria. Vancomycin-impregnated P407 hydrogel (20 mg/mL) was implanted in all dogs. The rate of infection clearance was 77%. Each unit increase in the number of surgeries performed at a site before gel implantation decrease the chance of successful infection clearance by 25% (P = .005; unit OR, 0.25; 95% CI, 0.08 to 0.81). Presence of multidrug or methicillin resistance increased risk for treatment failure by 7.69 times (P = .042; OR, 0.13; 95% CI, 0.01 to 1.14). No adverse events related to gel administration were seen. CLINICAL RELEVANCE Treatment outcomes were negatively impacted by the presence of multidrug or methicillin resistance and by an increased number of surgeries before gel implantation. Local administration of antibiotic-impregnated P407 hydrogel had a high success rate with no adverse effects in this population. Local antibiotic gel administration may improve treatment outcomes in dogs with complicated SSI.

OBJECTIVE Determine whether dogs with well-functioning orthopedic metal implants can develop metal reactivity. SAMPLE Client-owned dogs that had tibial plateau leveling osteotomy (TPLO) or total hip replacement (THR) implants for 12 months or more and control dogs with no implants. PROCEDURES Lymphocyte transformation testing was performed by exposing peripheral blood lymphocytes to nickel (Ni), chromium (Cr), cobalt (Co), or a combination of these metals. Lymphocyte proliferation was assessed with flow cytometry. Lymphocyte stimulation indexes (SIs) were calculated. A SI > 2 was considered reactive. Median SIs of dogs in response to metal exposure were compared statistically. RESULTS Samples from 10 dogs with TPLO, 12 dogs with THR, and 7 control dogs were analyzed. Six dogs out of 22 with metal implants had a reactive SI to 1 or more metals, while 2 of 7 control dogs had a SI > 2 when exposed to nickel only. When all metals were considered, no differences in metal reactivity were found between TPLO, THR, and control groups. CLINICAL RELEVANCE Metal reactivity is present in dogs and can be identified using lymphocyte transformation testing. Reactivity to Ni is present in dogs with and without metal implants. Reactivity to Co and Cr occurs in some dogs with metal implants.

Objective The aim of this study was to assess bone density, bone architecture and clinical function of canine nonunion distal appendicular long bone fractures with a defect treated with fixation, compression-resistant matrix and recombinant human bone morphogenetic protein-2 (rhBMP-2). Study Design Prospective cohort study with dogs at least 1-year post treatment. Computed tomography was performed and quantitative measurements from previous fracture sites were compared with measurements from contralateral limbs. Subjective evaluation included gait assessment and palpation. Results Six patients met the inclusion criteria. The rhBMP-2 treated bone exhibited higher density at the periphery and lower density in the centre, similar to the contralateral limb. All patients were weight bearing on the treated limb and all fractures were healed. Conclusion The rhBMP-2-treated bone underwent restoration of normal architecture and density. Acceptable limb function was present in all patients. The results of this study can serve as a basis for long-term response in treating nonunion fractures in veterinary patients.

Objective This study aimed to analyse the epidemiological data of canine appendicular avulsion fractures from three academic veterinary hospitals in the United States. Study Design A total of 114 avulsion fractures that presented to three study centres (teaching hospitals in New York, California, and Michigan) between 2008 and 2018 were analysed for avulsion type, breed, sex, body weight, age and affected side. Results The mean age for avulsion fractures was 1.6 years (±2.3 standard deviation), and side and sex were comparable (53% females and 54% left sided, only 2% bilateral). Tibial tuberosity fracture was the most common type, comprising 54% of all avulsion fractures, significantly more prevalent in younger and lighter dogs (p Conclusion The most common avulsion fractures were tibial tuberosity fractures, which tend to occur in young, small dogs. There is no sex or side predilection for avulsion fractures noted in this study, although the effect of neuter could not be determined. There were no differences in fracture characteristics among the three different geographical locations in the United States.

Objectives The aim of this study was to evaluate the susceptibility pattern of the incisional site Staphylococcus spp. to three commonly used perioperative antimicrobials in dogs. Materials and Methods A cohort of client-owned dogs was included in this prospective observational study. After induction of general anaesthesia and hair clipping, culture samples were collected from the incisional site of healthy dogs before orthopaedic surgery. The isolated colonies were identified. The susceptibility pattern of staphylococcal isolates was determined using Clinical and Laboratory Standard Institute standards. The staphylococcal susceptibilities to amoxicillin/clavulanic acid and cefpodoxime were compared with cefazolin using two-sided equality of proportions test. Results A total of 391 bacterial isolates were identified in 115 dogs. Bacillus spp. (n = 200) and Staphylococcus spp. (n = 95) were the most common bacteria isolated. Coagulase- positive and -negative staphylococci were isolated in 44 and 38% of dogs respectively. Cefazolin, amoxicillin/clavulanic acid and cefpodoxime staphylococcal susceptibilities were 68, 77 and 76% respectively. There were no significant differences in Staphylococcus spp. susceptibility between amoxicillin/clavulanic acid or cefpodoxime and cefazolin (p = 0.19, p = 0.25). Thirty-six per cent of coagulase-negative staphylococci and 6% of coagulase-positive staphylococci isolated were methicillin resistant. Conclusion Cefazolin, amoxicillin/clavulanic acid and cefpodoxime had similar susceptibilities for staphylococcal isolates. Routine use of amoxicillin/clavulanic acid or cefpodoxime in replace of cefazolin should be practiced with caution.

OBJECTIVE To compare the torsional mechanical properties of 2 external skeletal fixators (ESFs) placed with 2 intramedullary pin (IP) and transfixation pin (TP) size combinations in a model of raptor tibiotarsal bone fracture. SAMPLE 24 ESF-synthetic tibiotarsal bone model (polyoxymethylene) constructs. PROCEDURES Synthetic bone models were fabricated with an 8-mm (simulated fracture) gap. Four types of ESF-synthetic bone model constructs (6/group) were tested: a FESSA with a 1.6-mm IP and 1.6-mm TPs, a FESSA with a 2.0-mm IP and 1.1-mm TPs, an acrylic connecting bar with a 1.6-mm IP and 1.6-mm TPs, and an acrylic connecting bar with a 2.0-mm IP and 1.1-mm TPs. Models were rotated in torsion (5°/s) to failure or the machine angle limit (80°). Mechanical variables at yield and at failure were determined from load deformation curves. Effects of overall construct type, connecting bar type, and IP and TP size combination on mechanical properties were assessed with mixed-model ANOVAs. RESULTS Both FESSA constructs had significantly greater median stiffness and median torque at yield than both acrylic bar constructs; FESSA constructs with a 1.6-mm IP and 1.6-mm TPs had greatest stiffness of all tested constructs and lowest gap strain at yield. No FESSA constructs failed during testing; 7 of 12 acrylic bar constructs failed by fracture of the connecting bar at the interface with a TP. CONCLUSIONS AND CLINICAL RELEVANCE Although acrylic bar ESFs have been successfully used in avian patients, the FESSA constructs in this study were mechanically superior to acrylic bar constructs, with greatest benefit resulting from use with the larger TP configuration.

This study in dogs explored the feasibility of using cartilage fragments removed and discarded during routine palliative surgery for osteochondritis dissecans (OCD) as a source of primary chondrocytes for scaffold-free cartilage tissue-engineering. Primary chondrocytes were obtained from three OCD donors and one age-matched healthy articular cartilage (HAC) donor. After monolayer expansion of primary cells, a three-dimensional spherical suspension culture was implemented. Following this stage, cells were seeded at a high density into custom-made agarose molds that allowed for size and shape-specific constructs to be generated via a method of cellular self-assembling in a scaffold-free environment. Fifty-eight neocartilage constructs were tissue-engineered using this methodology. Neocartilage constructs and native cartilage from shoulder joint were subjected to histological, mechanical, and biochemical testing. OCD and HAC chondrocytes-sourced constructs had uniformly flat morphology and histology consistent with cartilage tissue. Constructs sourced from OCD chondrocytes were 1.5-times (32%) stiffer in compression and 1.3 times (23%) stronger in tension than constructs sourced from HAC chondrocytes and only 8.7-times (81%) less stiff in tension than native tissue. Constructs from both cell sources consistently had lower collagen content than native tissue (22.9%/dry weight [DW] for OCD and 4.1%/DW for HAC vs. 51.1%/DW native tissue). To improve the collagen content and mechanical properties of neocartilage, biological and mechanical stimuli, and thyroid hormone (tri-iodothyronine) were applied to the chondrocytes during the self-assembling stage in two separate studies. A 2.6-fold (62%) increase in compressive stiffness was detected with supplementation of biological stimuli alone and 5-fold (81%) increase with combined biological and mechanical stimuli at 20% strain. Application of thyroid hormone improved collagen content (1.7-times, 33%), tensile strength (1.8-times, 43%), and stiffness (1.3-times, 21%) of constructs, relative to untreated controls. Collectively, these data suggest that OCD chondrocytes can serve as a reliable cell source for cartilage tissue-engineering and that canine chondrocytes respond favorably to biological and mechanical stimuli that have been shown effective in chondrocytes from other animal species, including humans.

Working relationships between veterinary medical teaching hospitals, animal shelters, and rescue groups are one way to increase veterinary students’ and residents’ hands-on training. The goal of this study is to describe the use of a shelter fracture program to improve the surgical skills of surgical residents. In this program, the participating shelter and rescue organizations electronically submit cases. Following evaluation of radiographs and case approval by the orthopedic faculty, the case is scheduled for a physical evaluation. A resident takes primary surgical care together with a fourth-year student rotating through the orthopedic surgery service to ensure the proper pre-, peri-, and post-operative standard of care. All care is overseen by the orthopedic faculty. A veterinary student–run fracture foster program allows students to gain additional experience in the pre-, peri-, and post-operative care of shelter animals. The total number of shelter animals treated during a 9-year period was 373, with a mean annual case load of 41.1 cases (± 10.3). During the same time period, a total of 435 client-owned cases underwent surgical fracture treatment, with a mean annual case load of 48 cases (± 11.7). Surgical resident and student surveys show that this program contributes to their knowledge, skills, and confidence in treating fracture patients. A successful cooperative program provides advanced surgical fracture treatment of shelter animals, improving animals’ quality of life as well as surgical residents’ and veterinary students’ skills training.

An 11-month-old intact female Golden Retriever presented for a 3- to 4-week history of right thoracic limb lameness. Computed tomography of the limb showed a sequestrum with a periosteal and endosteal reaction at the level of the nutrient foramen of the radius. Septic osteomyelitis was suspected based on cytology. Repeat examination and imaging revealed improvement in the lesion and resolution of the lameness. This lesion is similar to reports of radioulnar ischaemic necrosis, but is the first report with concurrent osteomyelitis and sequestration.

OBJECTIVE To evaluate radiation exposure of dogs and cats undergoing procedures requiring intraoperative fluoroscopy and for operators performing those procedures. SAMPLE 360 fluoroscopic procedures performed at 2 academic institutions between 2012 and 2015. PROCEDURES Fluoroscopic procedures were classified as vascular, urinary, respiratory, cardiac, gastrointestinal, and orthopedic. Fluoroscopy operators were classified as interventional radiology-trained clinicians, orthopedic surgeons, soft tissue surgeons, internists, and cardiologists. Total radiation exposure in milligrays and total fluoroscopy time in minutes were obtained from dose reports for 4 C-arm units. Kruskal-Wallis equality of populations rank tests and Dunn pairwise comparisons were used to compare differences in time and exposure among procedures and operators. RESULTS Fluoroscopy time (median, 35.80 minutes; range, 0.60 to 84.70 minutes) was significantly greater and radiation exposure (median, 137.00 mGy; range, 3.00 to 617.51 mGy) was significantly higher for vascular procedures than for other procedures. Median total radiation exposure was significantly higher for procedures performed by interventional radiology-trained clinicians (16.10 mGy; range, 0.44 to 617.50 mGy), cardiologists (25.82 mGy; range, 0.33 to 287.45 mGy), and internists (25.24 mGy; range, 3.58 to 185.79 mGy). CONCLUSIONS AND CLINICAL RELEVANCE Vascular fluoroscopic procedures were associated with significantly longer fluoroscopy time and higher radiation exposure than were other evaluated fluoroscopic procedures. Future studies should focus on quantitative radiation monitoring for patients and operators, importance of operator training, intraoperative safety measures, and protocols for postoperative monitoring of patients.

OBJECTIVE To determine the influence of interlocking screw threads on the biomechanical properties of repaired canine humeral condylar fractures. STUDY DESIGN Ex vivo biomechanical study. SAMPLE POPULATION Thirty-six humeral condyles. METHODS Simulated fractures of the lateral aspect of the humeral condyle were stabilized by a 3.5 mm interlocking thread screw (ITS) or 3.5 mm buttress thread screw placed in lag (BTS-L) or positional fashion (BTS-P) and axially loaded at a walk, trot, 2-mm displacement, and failure cycles. Compact flute drill bits (CFBs) were used for ITS constructs and standard flute drill bits (SFB) for BTS constructs. The effects of bit type on drilling parameters and screw type on screw insertion properties and fragment stability were assessed. RESULTS CFB produced a 6°C greater temperature increase (p = .042) and required 20 N higher torque (p = .003) than SFB. Insertional torque was greater for ITS than BTS-P (p = .001) and BTS-L (p = .001). Condylar fragment rotation at failure was lower in ITS (lsmean ± SE, 8.3° ± 1.9°) than BTS-L constructs (14.5° ± 2.3°, p = .011). ITS resisted greater loads (1503 ± 105 N) than BTS-P (1189 ± 99 N, p = .038) but not BTS-L (1249 ± 123 N, p = .121) constructs. CONCLUSION Biomechanical performance of constructs was improved with ITS rather than BTS fixation. CLINICAL SIGNIFICANCE ITS can be considered for stabilization of humeral condylar fractures in adult dogs.

Computed tomographic images were reviewed by one board-certified radiologist. Reviewers were blinded to surgical and clinical findings. Surgical findings from real-time TAR with palpation were used as the gold standard. Receiver operating characteristic (ROC) curves and concordance statistics tests for the diagnostic accuracy of MCP fissure, MCP fragment, medial compartment condition, and cartilage score were calculated.ResultsImages of 27 elbows joints were reviewed. For MCP fissure detection, areas under the ROC curves for CT (0.84), STAR (0.73), and SNAR (0.57) did not differ. For the detection of MCP fragment, STAR had a larger area under the ROC curve (0.93) compared with SNAR (0.74, Pn=n.015) and CT (0.54, P l .001). Still images of TAR and SNAR had comparable concordance for cartilage score (0.80 and 0.77, respectively) and medial compartment pathology (0.80 and 0.73, respectively).ConclusionStill images of NAR, STAR, and CT had similar diagnostic value to identify MCP fissures. Still images of TAR was superior to SNAR and CT to identify MCP fragments.Clinical significanceThe diagnostic accuracy of SNAR varied on the basis of the coronoid lesion being evaluated.

18F-Sodium Fluoride (18F-NaF) positron emission tomography (PET) provides high resolution functional information about bone activity and can be fused with CT images to improve three-dimensional localization and characterization of lesions. This prospective, observational study assessed 18F-NaF PET-CT for imaging of canine elbows, compared PET with CT findings, and assessed correlation with lameness. Twelve patients with elbow pain were included. Cases included primarily young, large breed dogs. A three-level clinical lameness score was assigned to each forelimb. All dogs had bilateral elbow joints imaged with CT and PET under general anesthesia, approximately 1.5 h after intravenous injection of 3 MBq/kg of 18F-NaF. Imaging findings were independently reviewed by two radiologists using a three-level scoring scheme over nine anatomical regions in the elbow. PET imaging identified areas of bone activity where minimal change was identified on CT. PET imaging also demonstrated absence of uptake in areas where modeling was present on CT. A stronger correlation was observed between clinical grades and PET scores (r2 = 0.38, P = .001) than between clinical grades and CT scores (r2 = 0.17, P = .048). The total PET scores were significantly different for each clinical grade (P = .013) but total CT scores did not differ (P = .139). This exploratory study suggests that PET improves the ability to detect lesions and to determine the clinical significance of CT findings in dogs with elbow pain.

Female physicians rarely choose spine surgery as their specialty. Although the specialty's nature and its associated lifestyle are potential barriers, gender-related issues may play an important part.To evaluate the gender discrimination among spine surgeons across Latin America.Cross-sectional survey.The participants in this study were 223 AO Spine Latin America (AOSLA) registered members who answered the web-based survey.Personal and professional demographics; gender-related objective and subjective experiences regarding career and personal life.A survey link containing a 24-item questionnaire was sent to the members' e-mails in September 2019. The survey was designed to evaluate the perception of gender discrimination by spine surgeons during their academic and professional lives.Out of 223 members who answered the survey, 196 (87.96%) were male and 27 (12.11%) female. Most were orthopedic surgeons (64.13%), ≥40 years of age (55.16%), and had20 years of experience (69.95%). Gender discrimination was more frequent among women than among men (66.67% vs. 1.02%), as did discouragement from becoming a spine surgeon, orthopedic surgeon, or neurosurgeon (81.48% vs. 0.51%). Females reported higher rates of sexual harassment (44.44% vs. 7.65%) and more often felt disadvantaged because of gender (55.56% vs. 2.55%). Working harder than men to achieve the same prestige and lack of female mentorship were the most common obstacles reported by women (55.56%). Residency/fellowship influenced the decision to postpone/avoid having children for 66.67% of women but only 37.75% of men. Creation of a Women's Committee in AO Spine was supported by 74.07% of women and 38.78% of men.Gender-based discrimination affects women more frequently than men in spine surgery. These experiences likely contribute to the low prevalence of female spine surgeons. Efforts to mitigate bias and support the professional development of women in neurosurgery, orthopedics and spine communities are encouraged.

Fracture of the ulnar styloid process can result in clinically debilitating antebrachiocarpal instability. Previously described repair methods include Kirschner wire and figure of eight tension band wire and/or intramedullary pinning. This case report describes surgical repair of a short oblique ulnar styloid process fracture in a cat using two cortical screws placed in lag fashion. Recheck evaluation confirmed lameness-free full return to function with anatomical fracture healing.

OBJECTIVE To describe academic rank, gender, surgical career length, and publication record of academic veterinary surgeons and to estimate the association between gender and higher academic rank. STUDY DESIGN Cross-sectional study. SAMPLE Residency-trained surgeons at US veterinary schools in 2019. METHODS Surgeons were identified via institutional websites. Data including surgeon gender, academic title, and year of board certification were collected from public resources. Publication record was measured by using author h-indices obtained from Scopus. Data were analyzed with descriptive and inferential statistics. RESULTS Three hundred eighteen surgeons were identified from 30 institutions, including 162 (51%) women and 156 (49%) men. Women represented 66% of instructors and assistant professors, and men represented 60% of associate and full professors. This distribution differed significantly (P < .001). Author h-index was associated with career length but not gender. Men were 2.5 times more likely than women to be associate or full professors (odds ratio 2.52, 95% CI 1.03-6.14, P = .042) after adjustment for career length and h-index. CONCLUSION Female surgery faculty at US veterinary schools in 2019 were concentrated in lower academic ranks and were less likely than male surgery faculty to be associate or full professors after adjustment for career length and publication record. IMPACT A gender gap exists in academic veterinary surgery in the United States. It is critical to recognize that women's increasing participation in veterinary medicine has not been matched by equal representation in all areas. Additional efforts are warranted to identify contributing factors and implement strategies to improve gender inclusion.

A retrospective case series that included 253 free-ranging birds of prey admitted to a rehabilitation center was conducted to describe the treatment and outcome of antebrachial fractures. Medical records from birds of prey belonging to 21 species admitted with antebrachial fracture between 1989 and 2015 at the University of California, Davis, were reviewed. Species distribution on admission, treatment, outcome, and complications were described by fracture category and species. Among 134 birds treated after initial triage on the day of admission, 4 bone/ wing categories were identified: 83 birds had an ulnar fracture only; 18 birds had a radial fracture only; 28 birds had a concomitant fracture of the radius and ulna on the same wing; and 5 birds had bilateral antebrachial fractures. Logistic regressions were performed to determine which factors were associated with a positive outcome within each of these 4 categories. Among birds having only an ulnar fracture, those with a closed fracture were significantly more likely to be released than birds with open fractures (P = .03; odds ratio = 5.43, 95% confidence interval: 1.29–28.12). In addition, birds with a fracture of the middle third of the ulna were significantly more likely to be released than birds diagnosed with a single fracture of the proximal third of the ulna (P = .02; odds ratio = 4.54, 95% confidence interval: 1.35–16.64). No significant prognostic factor was detected in other fracture categories.

Objective To compare the stiffness, angular deformation, and mode of failure of lumbar vertebral column constructs stabilized with bilateral pins and polymethylmethacrylate (Pin-PMMA) or with a unilateral (left) locking compression plate (LCP) with monocortical screws. Study Design Ex vivo biomechanical, non-randomized. Samples: Cadaveric canine thoracolumbar specimens (n=16). Methods Thoracolumbar (T13-L3) vertebral specimens had the L1-L2 vertebral motion unit stabilized with either Pin-PMMA or LCP. Stiffness in flexion, extension, and right and left lateral bending after nondestructive testing were compared between intact (pretreated) specimens and Pin-PMMA, and LCP constructs. The Pin-PMMA and LCP constructs were then tested to failure in flexion and left lateral bending. Results Both the Pin-PMMA and LCP constructs had reduced range of motion at the stabilized L1-L2 vertebral motion unit compared to intact specimens. The Pin-PMMA constructs had less range of motion for the flexion elastic zone than LCP constructs. The Pin-PMMA constructs were stiffer than intact specimens in flexion, extension, and lateral bending, and stiffer than LCP constructs in flexion and left lateral bending. The Pin-PMMA constructs had less angular deformation at construct yield and lower residual deformation at L1-L2 than LCP constructs after destructive testing to failure in flexion. The Pin-PMMA constructs were stiffer, stronger, and had less deformation at yield than LCP constructs after destructive testing to failure in lateral bending. Most constructs failed distant to the implant and fixation site. Conclusions Pin-PMMA constructs had greater lumbar vertebral stiffness and reduced ROM than LCP constructs; however, both Pin-PMMA and LCP constructs were stronger than intact specimens.

Objective The aim of this study was to evaluate and compare the effect of three clinically applicable screw hole diameters on rabbit femoral torsional structural properties. Sample Eighteen pairs of skeletally mature New Zealand White rabbit femora (36 bones). Materials and Methods Femora with a bicortical hole at mid-diaphysis from one of the 3-drill bit sizes, 1.1 mm, 1.5 mm, 2.0 mm, and intact bones were studied. Each bone was bi-axially loaded in a servo-hydraulic load frame with the bone positioned so the neutral axis of torsion was aligned with the centre of the bone diaphysis. Axial compression to 35% body weight was applied to represent compression at stance, and rapid external torsion was applied to failure. Torque and angular deformation data were plotted for each test, with pre-yield and post-yield stiffnesses derived. Yield and failure torques and angles were determined, along with calculated yield, failure and post-yield energies. Results Failure torque was reduced compared with that of intact bone; weakened by 37% in 1.1-mm hole models, 53% in 1.5-mm hole models and 65% in 2.0-mm hole models. The torque angular deformation curves lacked plastic deformation. Conclusions and Clinical Relevance This study demonstrates the unique, brittle biomechanics of rabbit bone. Based on data from other species that strength loss of no more than 50% is acceptable when placing orthopaedic implants, no defect greater than 1.1 mm (15% bone diameter) is recommended in rabbit femora.

Objective To measure the density of cellular phenotypes in canine caudal cruciate ligament (CaCL), cranial cruciate ligament (CrCL), medial collateral ligament (MCL), and long digital extensor tendon (LDET). Study design Ex-vivo study. Methods Ten CaCL, CrCL, MCL, and LDET obtained from 1 stifle of 10 dogs with no gross pathology were analyzed histologically. The density of cells with 3 nuclear phenotypes (fusiform, ovoid, and spheroid) was determined within the core region of each specimen. Results Cells with fusiform nuclei were most dense in the MCL (median [range], 319 [118-538] cells/mm2 ) and LDET (331 [61-463]), whereas cells with ovoid nuclei were most dense in the CaCL (276 [123-368]) and CrCL (212 [165-420]). The spheroid nuclear phenotype had the lowest density in all structures (31 [5-61] in CaCL, 54 [5-90] in CrCL, 2 [0-14] in MCL, and 5 [0-80] in LDET); however, the CrCL contained a denser population of spheroid cells compared with MCL and LDET (P .05). Conclusion Phenotype density varied within the ligaments and tendon tested here. The cell population of CaCL and CrCL differed from that of dense collagenous tissues such as MCL and LDET. Clinical significance The relatively higher density of spheroid phenotype in CrCL may reflect a distinctive native cellular population or a cellular transformation secondary to unique mechanical environment or hypoxia. This intrinsic cellular population may explain altered tissue properties prone to pathological rupture or poor healing potential of the canine CrCL.

OBJECTIVE To describe the torsional and axial compressive properties of tibiotarsal bones of red-tailed hawks (Buteo jamaicensis). SAMPLE 16 cadaveric tibiotarsal bones from 8 red-tailed hawks. PROCEDURES 1 tibiotarsal bone from each bird was randomly assigned to be tested in torsion, and the contralateral bone was tested in axial compression. Intact bones were monotonically loaded in either torsion (n = 8) or axial compression (8) to failure. Mechanical variables were derived from load-deformation curves. Fracture configurations were described. Effects of sex, limb side, and bone dimensions on mechanical properties were assessed with a mixed-model ANOVA. Correlations between equivalent torsional and compressive properties were determined. RESULTS Limb side and bone dimensions were not associated with any mechanical property. During compression tests, mean ultimate cumulative energy and postyield energy for female bones were significantly greater than those for male bones. All 8 bones developed a spiral diaphyseal fracture and a metaphyseal fissure or fracture during torsional tests. During compression tests, all bones developed a crushed metaphysis and a fissure or comminuted fracture of the diaphysis. Positive correlations were apparent between most yield and ultimate torsional and compressive properties. CONCLUSIONS AND CLINICAL RELEVANCE The torsional and axial compressive properties of tibiotarsal bones described in this study can be used as a reference for investigations into fixation methods for tibiotarsal fractures in red-tailed hawks. Although the comminuted and spiral diaphyseal fractures induced in this study were consistent with those observed in clinical practice, the metaphyseal disruption observed was not and warrants further research.

SummaryObjectives: To report the use of compression resistant matrix (CRM) infused with recombinant human bone morphogenetic protein (rhBMP-2) prospectively in the healing of non union long-bone fractures in dogs.Methods: A longitudinal cohort of dogs that were presented with nonunion fractures were classified and treated with CRM soaked with rhBMP-2 and fracture fixation. They were followed with serial radiographs and evaluated for healing times and complications according to the time frame and definitions previously established for orthopaedic clinical cases.Results: Eleven nonunion fractures in nine dogs were included. Median healing time was 10 weeks (range: 7–20 weeks). Major perioperative complications due to bandage morbidity were encountered in two of 11 limbs and resolved. All other complications were minor. They occurred perioperatively in eight of 11 limbs. Minor follow-up complications included short-term in one of two limbs, mid-term in one of three, and long-term in four of five limbs. Nine limbs returned to full function and two limbs returned to acceptable function at the last follow-up.Clinical significance: Nonunion fractures given a poor prognosis via standard-of-care treatment were successfully repaired using CRM with rhBMP-2 accompanying fixation. These dogs, previously at high risk of failure, returned to full or acceptable function.

Fracture healing involves rapid stem and progenitor cell migration, homing, and differentiation. SDF-1 (CXCL12) is considered a master regulator of CXCR4-positive stem and progenitor cell trafficking to sites of ischemic (hypoxic) injury and regulates their subsequent differentiation into mature reparative cells. In this study, we investigated the role of SDF-1/CXCR4 signaling in fracture healing where vascular disruption results in hypoxia and SDF-1 expression. Mice were injected with AMD3100, a CXCR4 antagonist, or vehicle twice daily until euthanasia with the intent to impair stem cell homing to the fracture site and/or their differentiation. Fracture healing was evaluated using micro-computed tomography, histology, quantitative PCR, and mechanical testing. AMD3100 administration resulted in a significantly reduced hyaline cartilage volume (day 14), callus volume (day 42) and mineralized bone volume (day 42) and reduced expression of genes associated with endochondral ossification including collagen Type 1 alpha 1, collagen Type 2 alpha 1, vascular endothelial growth factor, Annexin A5, nitric oxide synthase 2, and mechanistic target of rapamycin. Our data suggest that the SDF-1/CXCR4 signaling plays a central role in bone healing possibly by regulating the recruitment and/or differentiation of stem and progenitor cells.

Objective To compare (1) proximal femoral axial strains, (2) femoral head deflection, and (3) failure mechanical properties, between Helica head and neck prosthesis implanted femora and normal femora. Study Design In vitro study. Sample Population Cadaveric canine femora (n = 5 pair). Methods Femoral bone strains and head displacement during in vitro simulation of midstance of the gallop were evaluated using cadaveric femurs cyclically loaded in vitro. Strains and displacements were compared within femurs, before and after, prosthesis implantation; and throughout cycling to seek evidence of movement with cyclic loading. Subsequently, implanted femurs and contralateral, intact femurs were loaded to failure to compare failure mechanical properties and modes of failure. Results Proximal femoral axial strains were significantly different between intact and implanted femora on all 4 cortical surfaces (P .05). Vertical head displacement was ∼0.1 mm greater for implanted femora than intact femora (P < .05). Yield and failure loads and yield energy of implanted femora were 39–54% lower than those for intact femora (P < .05). Mode of failure for both the intact and implanted femora did not appear to be different. Conclusion Helica femoral prosthesis alters strain distribution in the proximal aspect of the femur and exhibits initial micromotion. Failure load in axial compression of the Helica-implanted femur is less than that of the normal femur, but greater than that expected in vivo.

Objectives: To (1) describe vascular distribution in the grossly intact canine cranial cruciate ligament (CCL) using immunohistochemical techniques specific to 2 components of blood vessels (factor VIII for endothelial cells, laminin for basement membrane); and (2) compare the vascularity in different areas of interest (craniomedial versus caudolateral bands; core versus epiligamentous regions; and proximal versus middle versus distal portions) in the intact normal canine CCL. Study Design: In vitro study. Animals: Large, mature dogs (n=7) of breeds prone to CCL disease that were euthanatized for nonorthopedic conditions. Methods: Intact CCL were collected from fresh canine cadavers free from stifle pathology. CCL tissue was processed for immunohistochemistry and stained for factor VIII and laminin. Vascular density was determined by histomorphometric analysis. Results: Specific vascular staining was sparsely identified throughout the CCL; however, the proximal portion of the CCL appears to have a greater number of vessels than the middle or distal portion of the ligament. Conclusions: The CCL is a hypovascular tissue and its vascular distribution is not homogeneous.

Objective: To (1) determine the microanatomic vascular distribution in ruptured canine cranial cruciate ligaments (CCL) using specific vascular immunohistochemical techniques, and (2) compare vessel density between ruptured and intact canine CCL and between different areas of interest in ruptured CCL using histomorphometric analysis. Study Design: In vitro study. Animals: Dogs (n=41) admitted for surgical treatment of ruptured CCL and 19 dogs euthanatized for nonorthopedic conditions. Methods: Diseased (variable CCL rupture) and intact (normal control) CCL were processed for immunohistochemical staining specific to vessels (factor VIII, laminin). Mean vascular density was assessed and compared in areas of interest (torn end versus remaining core regions of CCL, proximal femoral versus distal tibial core CCL regions). Results: Ruptured CCL was more vascular than intact CCL; however there was no difference in vascular density between the torn end and the remaining core area of the ruptured CCL. Ruptured CCL was vascularized to a greater degree at the proximal portion than the distal portion of the CCL. Partially ruptured CCLs had a higher vessel density than completely ruptured CCLs. Conclusions: Vascular density is increased in diseased CCL compared with intact CCL. It remains to be determined whether this finding is associated with the cause of CCL rupture or is a result of CCL degeneration and rupture.

Objective: To evaluate the use of a locking compression plate (LCP) with monocortical screw purchase for stabilization of the canine cervical spine. Study Design: Experimental study. Animals: Cadaveric canine cervical spine specimens (n=7). Methods: Flexion and extension bending moments were applied to canine cadaveric specimens (C3–C6) in 4-point bending, before and after creation of a ventral slot at C4–C5, and after fixation with a 5 hole, 3.5 mm LCP with monocortical screw placement. Screw placement and penetration into the vertebral canal were determined by radiography. Range of motion, stiffness, and energy for passive physiologic loads were determined for the C3–C4, C4–C5, and C5–C6 vertebral motion units (VMU). Monotonic failure properties were determined for cervical extension. Effects of treatments on biomechanical variables were assessed using repeated measures analysis of variance and least square means (P≤.05). Results: The ventral slot procedure increased range of motion at the treated VMU. Plate fixation decreased range of motion, increased stiffness, and decreased energy at the treated VMU. No changes were observed at adjacent VMUs. None of the screws penetrated the vertebral canal. Mean (± SD) yield bending moment of plate stabilized, slotted spines was 15.6 ± 4.6 N m. Conclusion: LCP fixation with monocortical screws stabilized the canine cervical spine.

Objective— To determine the clinical value of a novel osteoarthritis (OA) biomarker in detecting canine cruciate disease. Study Design— Cross sectional clinical study. Animals— Dogs (n=22) with cranial cruciate ligament (CCL) rupture and 12 control dogs. Methods— Concentrations of collagenase-generated cleavage epitope of type II collagen (Col2-3/4Clong mono, or C2C) in serum, urine, and joint fluid were compared between a group of dogs with CCL rupture and a control group. Correlation of C2C concentrations to the clinical stage of stifle OA was also evaluated. Results— There were no significant differences in C2C concentrations in serum, urine, and joint fluid between groups (P>.05). Subjective scores of lameness, joint effusion, osteophytosis were significantly more severe in the CCL rupture group compared with the control group (P .05). Conclusion— This OA biomarker did not detect pathology associated with CCL rupture. Our results suggest that collagenase-specific degradation of type II collagen in articular cartilage may not be involved in the early stage of naturally occurring canine cruciate disease, and that pathology associated with naturally occurring CCL rupture is different from that of experimental OA model. Clinical Relevance— C2C is not clinically useful in detecting CCL rupture in dogs.

This study was performed to determine the effect of shoulder extension and flexion, imaging plane, and intra-articular paramagnetic contrast medium administration on the visibility of soft tissue structures surrounding the canine shoulder joint using magnetic resonance (MR) imaging. Eight dogs without shoulder disease were imaged using a 1.5 T MR scanner. In all dogs, T1- and T2-weighted spin-echo sequences in sagittal, transverse, and dorsal planes, and a sagittal short tau inversion recovery (STIR) sequence were acquired with the shoulder joint in extension. In five dogs, postintravenous contrast medium T1-weighted images were obtained and in five dogs, MR arthrography was performed in flexion and extension. Each specific soft tissue structure was assigned a visual assessment score of 0–3 and descriptive statistics were calculated. The biceps brachii, supraspinatus, and teres minor muscles and tendons, and the biceps tendon sheath were best seen in the sagittal and transverse planes. The infraspinatus was well-visualized in all planes, the subscapularis was best seen in the transverse and dorsal planes, and the capsuloligmentous complexes were best seen in the transverse and dorsal planes. Limb extension improved conspicuousness of the medial capsuloligamentous stuctures. Arthrography resulted in greater conspicuousness of the biceps brachii tendon, biceps tendon sheath, infraspinatus, supraspinatus, subscapularis, medial joint capsule, medial glenohumeral ligament, and the lateral joint capsule–glenohumeral ligament complex. The major periarticular anatomic structures of the normal canine shoulder were consistently identified using the protocols described in this investigation, but arthrography with the joint in extension resulted in the best visualization of all structures.

Objectives— To compare (1) pullout properties between 3.5 mm cortical and locking screws, and (2) mechanical properties and gap displacements between the 3.5 mm broad limited-contact dynamic compression plate (LC-DCP), broad dynamic compression plate (DCP), and narrow locking compression plate (LCP), during axial loading of plate-stabilized diaphyseal fragments with an interfragmentary gap. Study Design— In vitro mechanical testing of implanted polyurethane foam (PUF) hollow cylinders that simulated compact or osteopenic diaphyseal bone. Sample Population— (1) Five cortical and locking screws and (2) 4 PUF-plate constructs for each plate type; using high- and low-density (0.8 and 0.32 g/cm3) cylinders. Methods— (1) Screws were completely extracted at 5 mm/min. (2) Plated constructs were axially compressed at 300 N/s for 10 cycles from 5 to 355 N to determine gap displacement during physiologic loading, followed by single cycle increasing load to failure. Results— Pullout properties were not different between screw types. All plate constructs had yield loads over 3 times trotting loads. Gap closure occurred with LC-DCP and DCP constructs, but not LCP constructs. LCP construct properties were most similar to LC-DCP and DCP construct properties in the low-density model. Conclusion— All plate systems sustained physiologic limb loads. Only LCP constructs maintained some gap integrity, although LC-DCP and DCP screws were placed in neutral position. Clinical Relevance— The LCP system is more likely than LC-DCP and DCP systems, with neutrally positioned screws, to maintain a planned interfragmentary gap, although gap strains range from 0% to 15% across the 2 mm gap during a trot load.

Objective— To compare failure mode and bending moment of a canine pancarpal arthrodesis construct using either a 2.7 mm/3.5 mm hybrid dynamic compression plate (HDCP) or a 3.5 mm dynamic compression plate (DCP). Study Design— Paired in vitro biomechanical testing of canine pancarpal arthrodesis constructs stabilized with either a 2.7/3.5 HDCP or 3.5 DCP. Sample Population— Paired cadaveric canine antebrachii (n=5). Methods— Pancarpal arthrodesis constructs were loaded to failure (point of maximum load) in 4-point bending using a materials-testing machine. Using this point of failure, bending moments were calculated from system variables for each construct and the 2 plating systems compared using a paired t-test. The optical properties (absorption [mu(a)], transport scattering [mu('s)] and effective attenuation [mu(eff)] coefficients) of normal canine prostate were measured in vivo using interstitial isotropic detectors. Measurements were made at 732 nm before, during and after motexafin lutetium (MLu)-mediated photodynamic therapy (PDT). They were derived by applying the diffusion theory to the in vivo peak fluence rates measured at several distances (3, 6, 9, 12 and 15 mm) from the central axis of a 2.5 cm cylindrical diffusing fiber (CDF). Mu(a) and mu('s) varied between 0.03-0.58 and 1.0-20 cm(-1), respectively. Mu(a) was proportional to the concentration of MLu.Mu(eff) varied between 0.33 and 4.9 cm(-1) (mean 1.3 +/- 1.1 cm(-1)), corresponding to an optical penetration depth (8 = 1/(mu(eff)) of 0.5-3 cm (mean 1.3 +/- 0.8 cm). The mean light fluence rate at 0.5 cm from the CDF was 126 +/- 48 mW/cm2 (N = 22) when the total power from the fiber was 375 mW (150 mW/cm). This study showed significant inter- and intraprostatic differences in the optical properties, suggesting that a real-time dosimetry measurement and feedback system for monitoring light fluences during treatment should be advocated for future PDT studies. However, no significant changes were observed before, during and after PDT within a single treatment site.

Summary Objectives: To determine if a single low dose of radiation therapy in dogs with osteoarthritis of the elbow joint was associated with a detectable improvement in their lameness and pain as documented by force platform gait analysis. Methods: In this cohort longitudinal observational study, five Labrador Retrievers with lameness due to elbow osteoarthritis that was unresponsive to medical treatment were removed from all non-steroidal anti-inflammatory and analgesic medications. A single treatment of radiation therapy delivering 10 Gray was performed on the affected elbow joint(s). Force platform gait analysis was used to assess the ground reaction forces of a limb affected with elbow osteoarthritis both before and after radiation therapy. Results: Significant differences occurred in the weight-bearing on an affected limb with elbow osteoarthritis after radiation therapy at weeks six and 14. Change due to treatment was particularly apparent in dogs with unilateral elbow osteoarthritis. Clinical significance: Administering a single low dose of radiation therapy may have a short-term benefit in dogs with elbow osteoarthritis, which is similar to the evidence supporting the use of radiation therapy in horses with orthopaedic disease.

SummaryObjective: To compare pelvic limb kinetic and kinematic gait parameters between Dachshunds six months following hemilaminectomy for treatment of thoracolumbar disc extrusion (post-hemilaminectomy; PHL) and Dachshunds without history and clinical evidence of spinal cord disease (control; CON).Methods: The CON (n = 8) and PHL (n = 6) Dachshunds were recruited for objective gait evaluation. Kinetic data collected included peak vertical force (PVF), stance phase duration and swing phase duration. Kinematic data collected included tarsal, stifle and hip range of motion (ROM) during stance and swing phases of the trot, tail ROM, and horizontal and vertical components of pelvis ROM.Results: No significant differences were identified between tarsal, stifle, hip, and tail ROM between CON and PHL Dachshunds. Although PVF was not significantly different between CON and PHL Dachshunds, PVF varied on average by 14% between the pelvic limbs in PHL Dachshunds (p Clinical significance: Six months after decompressive hemilaminectomy, Dachshunds have abnormal pelvic motion and asymmetric pelvic limb weight bearing. Pelvic sway (ROM) may be a more sensitive indicator of myelopathy than pelvic limb joint ROM and may serve as a useful objective tool to characterize response to treatment in patients with spinal cord disease.

Purpose Local recurrence of rectal cancer remains a significant clinical problem despite multi-modality therapy. Photodynamic Therapy (PDT) is a cancer treatment which generates tumor kill through the production of singlet oxygen in cells containing a photosensitizing drug when exposed to laser light of a specific wavelength. PDT is a promising modality for prevention of local recurrence of rectal cancer for several reasons: tumor cells may selectively retain photosensitizer at higher levels than normal tissues, the pelvis after mesorectal excision is a fixed space amenable to intra-operative illumination, and PDT can generate toxicity in tissues up to 1 cm thick. This study evaluated the safety, tissue penetration of 730 nm light, normal tissue toxicity and surgical outcome in a dog model of rectal resection after motexafin lutetium-mediated photodynamic therapy. Methods Ten mixed breed dogs were used. Eight dogs underwent proctectomy and low rectal end to end stapled anastomosis. Six dogs received the photosensitizing agent motexafin lutetium (MLu, Pharmacyclics, Inc., Sunnyvale, CA) of 2 mg/kg preoperatively and underwent subsequent pelvic illumination of the transected distal rectum of 730 nm light with light doses ranging from 0.5 J/cm 2 to 10 J/cm 2 three hours after drug delivery. Two dogs received light, but no drug, and underwent proctectomy and low-rectal stapled anastomosis. Two dogs underwent midline laparotomy and pelvic illumination. Light penetration in tissues was determined for small bowel, rectum, pelvic sidewall, and skin. Clinical outcomes were recorded. Animals were sacrificed at 14 days and histological evaluation was performed. Results All dogs recovered uneventfully. No dog suffered an anastomotic leak. Severe tissue toxicity was not seen. Histological findings at necropsy revealed mild enteritis in all dogs. The excitation light penetration depths were 0.46 ± 0.18, 0.46 ± 0.15, and 0.69 ± 0.39 cm, respectively, for rectum, small bowel, and peritoneum in dogs that had received MLu. For control dogs without photosensitizer MLu, the optical penetration depths were longer: 0.92 ± 0.63, 0.67 ± 0.10, and 1.1 ± 0.80 cm for rectum, small bowel, and peritoneum, respectively. Conclusion Low rectal stapled anastomosis is safe when performed with MLu-mediated pelvic PDT in a dog model. Significant tissue penetration of 730 nm light into the rectum and pelvic sidewall was revealed without generation of significant toxicity or histological sequelae. Penetration depths of 730 nm light in pelvic tissue suggest that microscopic residual disease of less than 5 mm are likely to be treated adequately with MLu-mediated PDT. This approach merits further investigation as an adjuvant to total mesorectal excision and chemoradiation for rectal cancer.

SummaryCemented total hip replacement (cTHR) is commonly performed to treat intractable coxofemoral pain in dogs. While owners generally perceive a good outcome after the procedure, the longevity of the implant may be limited by complications such as infection and aseptic loosening. The objective of this retrospective study was to identify the prevalence of complications and radiographic changes following cTHR, and to identify factors that may predispose to a need for revision surgery. Medical records and radiographs from 97 dogs that underwent cTHR were evaluated for signalment, preoperative degree of osteoarthritis, technical errors, intra-operative culture results, and the post-operative radiographic appearance of the implant. The complications occurring in the intra-operative and short-term (eight week) time period were recorded. Mean (±SD) follow-up time was 1.1 ± 1.6 years (range: 0–7.7 years). Seven dogs had a short-term complication and a revision surgery was performed in eleven dogs. Osseous or cement changes were radiographically detectable in the majority of cTHR. Eccentric positioning of the femoral stem and the presence of radiolucent lines at the femoral cement-bone interface were positively associated with the occurrence of revision surgery. The clinical significance of the periprosthetic radiographic changes is unclear and further investigation is warranted.

Objective—To evaluate hip joint laxity in 10 breeds of dogs via 2 radiographic techniques. Animals—500 clinically normal dogs. Design—Prospective study. Procedure—Radiographs obtained via routine hip joint evaluations performed in sedated dogs of 10 popular breeds were randomly selected from a database. Measurements of distraction index (DI) and hipextended index (HEI) on 1 hip joint radiograph randomly chosen from each dog were made. Results—Mean age of dogs was 20.7 months. Mean HEI was 0.17 (range, 0.0 to 0.72) and mean DI was 0.44 (range, 0.07 to 0.96). Borzois had uniformly tight hip joints as judged by use of both methods and were considered the gold standard by which hip joint laxity was judged (all Borzois had DI ≤ 0.32). Overall, DI was significantly greater than HEI. Within each breed, mean DI was always greater than mean HEI. Significant differences were detected among breeds for HEI; however, compared with DI, the magnitude of differences among breeds was less. Conclusions and Clinical Relevance—Distraction radiography detected the greatest range and magnitude of passive hip laxity in the 10 breeds of dogs. The difference in values between breeds known to have high prevalence of canine hip dysplasia and those in Borzois was greater for DI than for HEI. Breeds must be evaluated individually because of inherent differences in hip joint laxity. ( J Am Vet Med Assoc 2004; 224:542–546)
