Background: The aim of this study was to assess the clinical efficacy of coronally advanced flap (CAF) with or without connective tissue graft (CTG) for the treatment of multiple adjacent gingival recessions in the upper arch., Material and Methods: Thirty-two patients with a total of 74 gingival recessions were randomly allocated to the two groups. Outcome measures, collected by a blind examiner, included complete root coverage (CRC), recession reduction (RecRed), keratinized tissue (KT) gain, increase in gingival thickness (GT), patient satisfaction and root coverage esthetic score (RES)., Results: An interaction between treatment and baseline GT was detected. At 1 year, CAF + CTG resulted in better outcomes in terms of CRC (p = 0.0016) and RecRed (p < 0.0001) than CAF alone at sites with thin gingiva (thickness ≤ 0.8 mm). No difference was found between CAF alone and CAF + CTG at sites with thick gingiva (>0.8 mm). CAF resulted in higher aesthetic scores (RES) than CAF + CTG at sites with thick gingiva. CAF + CTG was associated with greater KT gain (p < 0.0001) and greater post-operative morbidity (p < 0.0001)., Conclusion: Connective tissue graft under CAF results in increased probability of CRC only at sites with thin baseline gingiva. Objective: To analyze any differences in the orthodontic treatment between children belonging to a particular subgroup of subjects with special health care needs (SHCN), children with craniofacial anomalies (CFA), and children not diagnosed with SHCN (NO SHCN)., Materials and Methods: The study sample consisted of 50 children with SHCN and a confirmed diagnosis of CFA (SHCN/CFA); the control group consisted of 50 NO SHCN children fully matched for age, gender, and type of appliance used. The differences between the two groups were analyzed retrospectively: pre-, posttreatment scores, and score reduction of the Peer Assessment Rating Index (PAR), dental health component (DHC), and aesthetic component (AC) of Orthodontic Treatment Need Index (IOTN), number of appointments, number of simple or complex chair-time appointments, overall treatment time, and age at treatment start and end., Results: There were no statistically significant differences between the SHCN/CFA and NO SHCN groups for number of appointments, overall treatment time, age at treatment start, and age at treatment end (P  =  .682, .458, .535, and .675, respectively). There were statistically significant differences between groups in PAR, DHC, AC pre- and posttreatment, and number of simple and complex chair-time appointments (P  =  .030 and .000; .020 and .023; .000 and .000; .043; and .037; respectively). The reduction of PAR, DHC, and AC scores was not significantly different between groups (P  =  .060, .765, and .825, respectively)., Conclusion: The treatment of children with SHCN, in general, and with CFA, in particular, on the one hand involves a higher rate of using complex chair time appointments and an inferior treatment outcome, by the other side implies an overall treatment time and a reduction of PAR, DHC or AC scores similar to the treatment of children not diagnosed with SHCN.

Background: The chemical, physical, and morphologic characteristics of the implant surface play a fundamental role during the osteointegration process. Implant design is of paramount importance in determining implant primary stability and implant ability to sustain loading during and after osteointegration. LASER treatment of the surface allows defining the precise parameters of roughness to obtain a regular and repeatable surface in total absence of contamination., Purpose: The aim of this study was to analyze the behavior of implant stability of LASER-treated surface implants by repeated resonance frequency analysis (RFA) measurements during 2 years in patients with complete upper maxilla edentulism subject to an immediate-loading protocol., Methods: Ten patients were included, and each treated with the insertion of 6 or 8 LASER surface implants according to the individual surgical-prosthetic planning. During the bone drilling and implant insertion, torque values were monitored with a specific handpiece and software. All implants were loaded within 24 hours from the insertion with the application of a temporary full-arch prosthesis. Subsequent follow-up has been done at 24 months from the loading with radiographic controls (OPT) and RFA measurement on all implants at time of implant insertion and at 1, 3, 6, 12, and 24 months from loading., Results: Resonance frequency analyses at 3 and 6 months from the implant loading have shown a rapid increment of implant stability quotient (ISQ) values in the first phases of bone remodeling, subsequent to the peri-implant bone remodeling. The paired comparisons between mean ISQ values by patient showed a statistically significant decrease in primary stability from baseline up to 1 month (P = 0.0039). Subsequent measurements revealed a statistically significant increase in implant stability from 1 up to 3 months (P = 0.0156), from 3 up to 6 months (P = 0.0020), from 6 up to 12 months (P = 0.020), and 12 up to 24 months (P = 0.0391)., Conclusions: Resonance frequency analysis of the LASER-treated surface implants showed good ISQ values at all time point measurements. These results are consistent with data from literature on the analysis of RFA in protocols of immediate loading at the upper maxilla.

Osteoporosis is a systemic disease in which the skeletal condition is characterized by a decreased mass of normally mineralized bone, due to an augmentation of bone resorption processes. Bone biomarkers serum are used for the diagnosis. On the other hand the main cause of the resorption in the bone jaws are periodontitis, inflammatory cysts, developmental cysts, odontogenic neoplasms. Periodontal diseases can be localized to a single site of the jaws or can affect all the teeth, with a massive bone resorption. The cysts are classified in developmental and inflammatory. They caused a local bone resorption in the jaws. Keratocystic odontogenic tumor produces a large bone resorption for its local aggressive nature. Their diagnosis is clinical and radiological.The aim of our review is to find a correlation between bone biomarkers serum and periodontitis, inflammatory cists, developmental cysts, odontogenetic neoplasms.The RANK/RANKL/OPG system is the most studied not only in osteoporosis but also in the periodontitis, inflammatory cysts, developmental cysts, odontogenic neoplasms. In the last years osteoimmunology was used to study the periodontal disease progression, because the immunity cells start the bone resorption processes.A lot of studies analyze the biomarkers present in the biofluids, as saliva and gingival crevicular fluid, but not the correlation with serum biomarkers.Future studies must be organized to deepen the correlation between bone biomarkers and bone jaws resorption and to allow diagnosis and prognosis of periodontitis, inflammatory cysts, developmental cysts, odontogenic neoplasms.

Light and electron microscopy were used to analyze the epithelial lining of odontogenic cysts excised from edentulous regions of the jaws. Clinically, three cases were identified as keratocysts, and 21 cases as cysts other than keratocysts ("non-keratocysts"). The epithelium of the former was found to achieve keratinization over most of the surface and to never contain mucus secreting cells. The epithelium of the latter appeared to be in part stratified squamous, with cells loosely connected to each other, and in part stratified columnar, with superficial cells connected to each other by tight junctions and secreting mucus. The results suggest that cysts arising from edentulous regions of the jaws may be either keratocysts or cysts with heterogeneous, non-keratinizing epithelium; the content of keratocysts can be formed mainly by shedding of cornified epithelial layers, and that of non-keratocysts by mucus secretion from columnar epithelium associated to fluid filtration through non-keratinizing squamous epithelium.

The presence of salivary tissue into the mandible represents often the submaxillary gland dislocation into the maxillary bones. Together with a literature revision, we have analysed the histogenetic hypothesis of this eterotopia and its prognostic implication. At last it has been described a clinical case treated in our Institute.

The hemophilic syndrome (hemophilia A, B, C) represent about the 90-95% of all the congenital coagulopathy; in order to avoid immediate or belated hemorrhagic complications it is necessary to give these patients all the care they need. When the hemophiliacs have to be submitted to oral surgery operations we can use various therapies according to the seriousness and to the type of the disease. We have also taken into consideration the ways to grant a local hemostatic control.
