Abstract

Item does not contain fulltext, Parkinson's disease (PD) is the fastest-growing neurodegenerative disorder, currently affecting ~7 million people worldwide. PD is clinically and genetically heterogeneous, with at least 10% of all cases explained by a monogenic cause or strong genetic risk factor. However, the vast majority of our present data on monogenic PD is based on the investigation of patients of European White ancestry, leaving a large knowledge gap on monogenic PD in underrepresented populations. Gene-targeted therapies are being developed at a fast pace and have started entering clinical trials. In light of these developments, building a global network of centers working on monogenic PD, fostering collaborative research, and establishing a clinical trial-ready cohort is imperative. Based on a systematic review of the English literature on monogenic PD and a successful team science approach, we have built up a network of 59 sites worldwide and have collected information on the availability of data, biomaterials, and facilities. To enable access to this resource and to foster collaboration across centers, as well as between academia and industry, we have developed an interactive map and online tool allowing for a quick overview of available resources, along with an option to filter for specific items of interest. This initiative is currently being merged with the Global Parkinson's Genetics Program (GP2), which will attract additional centers with a focus on underrepresented sites. This growing resource and tool will facilitate collaborative research and impact the development and testing of new therapies for monogenic and potentially for idiopathic PD patients.

The objective of this study was to assess worldwide access to genetic tests for movement disorders and factors impacting their utilization.MethodsThe Rare Movement Disorders Study Group of the International Parkinson and Movement Disorder Society designed an online survey electronically mailed to all 7815 members.ResultsSurvey data completed by 1269 participants from 109 countries were analysed. Limited access to geneticists and genetic counsellors was reported in many world regions compared to Europe and North America. Availability of genetic testing was limited, with rates of access lower than 50%. Genetic testing for chorea was the most commonly available. For parkinsonism, dystonia, ataxia, hereditary spastic paraplegias and metabolic disorders, there was limited access to genetic testing in all countries compared to Europe and North America, with significant differences found for Africa, Central/South America, Asia. In many regions, genetic testing was supported by either private or public funding. Genetic testing was free of charge in Europe according to 63.5% of respondents. In North America, Africa, Central/South America, Asia and the Middle East access to free of charge genetic testing was by far significantly lower compared to Europe.ConclusionsThis survey highlights difficulties in accessing genetic testing and individuals with expertise in genetics at the worldwide level. In addition, major disparities in genetic testing amongst world regions are highlighted, probably due to a variety of factors including financial barriers.

Abstract

Background Parkinson's disease is the second most common neurodegenerative disorder and affects people from all ethnic backgrounds, yet little is known about the genetics of Parkinson's disease in non-European populations. In addition, the overall identification of copy number variants at a genome-wide level has been understudied in Parkinson's patients. The objective of this study was to understand the genome-wide burden of copy number variants in Latinos and its association with Parkinson's disease. Methods We used genome-wide genotyping data from 747 Parkinson's disease patients and 632 controls from the Latin American Research Consortium on the Genetics of Parkinson's disease. Results Genome-wide copy number burden analysis showed that patients were significantly enriched for copy number variants overlapping known Parkinson's disease genes compared with controls (odds ratio, 3.97; 95%CI, 1.69-10.5; P = 0.018). PRKN showed the strongest copy number burden, with 20 copy number variant carriers. These patients presented an earlier age of disease onset compared with patients with other copy number variants (median age at onset, 31 vs 57 years, respectively; P = 7.46 × 10-7 ). Conclusions We found that although overall genome-wide copy number variant burden was not significantly different, Parkinson's disease patients were significantly enriched with copy number variants affecting known Parkinson's disease genes. We also identified that of 250 patients with early-onset disease, 5.6% carried a copy number variant on PRKN in our cohort. Our study is the first to analyze genome-wide copy number variant association in Latino Parkinson's disease patients and provides insights about this complex disease in this understudied population. © 2020 International Parkinson and Movement Disorder Society.

Abstract

Introdução: O cancro da próstata é uma importante causa de morbilidade e mortalidade. Em Portugal, de acordo com um estudo publicado em 2003, a taxa de novos casos de cancro na próstata era de 53 por 100.000 homens, em 2000, com uma taxa de mortalidade padronizada pela idade de 28 por 100.000 (em 1995). As metástases ósseas múltiplas são uma das principais complicações associadas ao cancro de próstata avançado. O Samário-EDTMP mostrou ser uma alternativa segura e eficaz no tratamento paliativo da dor associada a metástases ósseas. O objectivo deste estudo económico é avaliar o custo-efectividade do tratamento da dor associada a metástases ósseas múltiplas com Samário-153-EDTMP versus terapêutica convencional da dor, no carcinoma da próstata hormono-refractário, em Portugal. Metodologia: Estudo de custo-efectividade que compara os custos directos do tratamento de doentes com múltiplas metástases ósseas dolorosas com Samário-153-EDTMP versus terapêutica convencional para a dor, na perspectiva do Sistema Nacional de Saúde, em Portugal, num horizonte temporal de quatro meses. Resultados: Os custos directos totais num período de quatro meses são 2.311,91 € para um doente tratado com Samário-153-EDTMP versus 2.450,74 € para um doente sob tratamento padrão. De acordo com o modelo, um doente tratado com Samário-153-EDTMP representa um decréscimo de custos de 138,83 €. Conclusão: O Samário-153-EDTMP é não só um método terapêutico muito eficaz mas também uma solução com custos reduzidos quando comparado à terapêutica convencional da dor, em doentes com dor devida a metástases ósseas múltiplas, em Portugal.

Abstract

Conclusions: This study suggests that patients with Parkinson's disease (PD), even in the early stages, have decreased body limits of stability (LOS) and changes in the visual input impair their postural control., Objective: To assess the LOS and the postural responses after changes in visual input in a group of PD patients in stage 1 of the Hoehn and Yahr classification., Subjects and Methods: Twenty PD patients in stage 1 and a group of 24 normal subjects as control were assessed in two tests: (1) the LOS and (2) measurement of the body center of pressure area (COP) 10 s before and after sudden change in visual flow velocity. We also investigated labeling of the COP trajectory in these two periods. The stimulation paradigm was a horizontal optokinetic stimulation (60 degrees /s and suddenly stopped) using a virtual reality system., Results: LOS showed significant decrease in PD patients as compared with the control group (p<0.001, Kruskal-Wallis and Wilcoxon ranked test). The COP values increased significantly (p<0.001, Wilcoxon signed rank test) after sudden changes in the visual flow velocity in relation to the control group. After the visual stop the PD patient showed a spatial 'roaming' approaching the limits of stability and therefore impairing the postural control.

Abstract

Introduction: Prostate cancer is a significant cause of morbidity and mortality. In Portugal alone, according to a study published in 2003, the rate of new prostate cancer cases were 53 per 100,000 men (in 2000), with an age-standardized mortality rate of about 28 per 100,000 (in 1995). Multiple bone metastases are one of the major complications of advanced prostate cancer. Samarium-EDTMP showed to be a safe and effective alternative for palliative treatment of bone metastases. The goal of this economic study is to assess the cost-effectiveness of Samarium-153-EDTMP for the treatment of pain due to multiple bone metastases in hormone-refractory prostate cancer versus conventional pain therapy, in Portugal., Methodology: Cost-effectiveness study that compares the expected direct costs to the National Health System of managing patients with painful multiple bone metastases with Samarium-153-EDTMP versus conventional pain therapy, in Portugal, in a 4-moths period., Results: The total direct 4 months cost was 2,311.91 euro for a patient treated with Samarium-153-EDTMP versus 2,450.74 euro for a patient under standard treatment. According to the model a patient treated with Samarium-153-EDTMP represents a 138.83 euros saving., Conclusion: Samarium-153-EDTMP was not only a very effective therapeutic option but also an option with less cost than the conventional pain therapy, in patients with pain due to multiple bone metastases, in Portugal.

Abstract

Erysipelas is an acute dermo-hypodermal infection (non necrotizing) of bacterial origin, mainly group A beta-haemolytic streptococcus. The lower limbs are affected in more than 80% of the cases and the identified risk factors are disruption of cutaneous barrier, lymphoedema and obesity. Diagnosis is clinical and based upon the association of an acute inflammatory plaque with fever, lymphangitis, adenopathy and leukocytosis. Bacteriology is usually not helpful because of low sensitivity or delayed positivity. In the atypical forms erysipelas must be distinguished from necrotizing fasciitis and acute vein thrombosis. Penicillin remains the gold standard treatment, although new drugs, given their pharmacodynamic profile, may be used. Recurrence is the main complication, being crucial the correct treatment of the risk factors.
