Purpose: To compare the position and direction of femoral and tibial tunnels for both the anteromedial bundle (AMB) and posterolateral bundle (PLB) among three different femoral tunnel drilling techniques, transtibial (TT), transportal (TP), and outside-in (OI) techniques, in anatomic double-bundle ACL reconstruction to clarify advantages and disadvantages of each technique.Methods: One-hundred and thirty-nine patients underwent primary ACL reconstruction with an autologous semitendinosus tendon in our institution between 2014 and 2016. Thirteen patients were excluded according to the exclusion criteria. Of the 126 patients, 98 patients agreed to be included in this study. Patients were then randomized into three groups according to the femoral tunnel drilling technique; the TT, TP, and OI groups. Femoral and tibial tunnel angles and positions were measured using three-dimensional computed tomography.Results: Of patients who agreed to be included in this study, eight patients (seven in TT and one in OI) were excluded since the femoral tunnel could not be created at the intended position. Eighty-six patients (29 in TT, 29 in TP, and 28 in OI) were included for the analyses. Tunnel angles, as well as tunnel lengths, had significant differences among different techniques depending on each technique's characteristics. In terms of tunnel position, femoral tunnel positions of both the AMB and PLB in the TT group were significantly higher than those in the TP group (AMB: p = 0.003, PLB: p = 0.001), and the PLB tunnel position in the TP group had significantly smaller vaciance than that in the TT group (p = 0.004) and OI group (0.002).Conclusions: The femoral tunnel positions created by the TT technique were significantly higher, with larger variance, than the TP technique in double-bundle ACL reconstruction, although the positions seemed to be within anatomical footprint. Precise frequency comparisons are becoming more urgent given the recent rapid progress in next-generation frequency standards. This paper describes a new type of bidirectional optical amplifier that overcomes the fiber loss limits that have prevented accurate frequency comparisons between widely separated places; such comparison is realized by bidirectionally transmitting wavelength-division-multiplexed (WDM) signals along a single fiber. The proposed optical amplifier has an optical isolator in each two-way channel divided by wavelength filters to suppress the optical reflection that causes amplification instability. The additional insertion optical loss due to this method is about 1.5 dB. The optical gain greater than 30 dB is obtained for both signals with good optical isolation of 65 dB. A radio-frequency reference signal can directly be sent by simple intensity modulation and direct detection (IM-DD) devices in the 1550-nm region widely used in telecommunication networks. Phase comparisons of the received signals and the frequency standards at each terminal are used for frequency comparisons. The amplifier is tested in the field using two hydrogen masers. A 120-km fiber with loss of 52.5 dB is used to connect the National Metrology Institute of Japan (NMIJ) to the University of Tokyo. Because of this loss, an amplifier is needed to realize sufficient receiving power. The frequency stability of the system with a 10-MHz direct transmission is evaluated by returning the optical signal from a halfway point [55 km from the NMIJ] where the amplifier is installed. The result is 2.6 x 10-16 (Allan deviation) with the averaging time of 7 x 104 s. The laboratory result is 8.7 x 10-17 (τ = 4 x 104 s). This paper describes a simple and cost-effective method of frequency dissemination. In current digital communication networks, node clocks are hierarchically synchronized to the atomic master clock through fiber links. This synchronized network is used as an intermediate link for remote calibration services like the global positioning system common-view method. A prototype reference signal generator has been developed for recovering the communication clock signal and synthesizing a 10-MHz signal from it. The generator output frequency at the client site can be traced to coordinated universal time (UTC) National Metrology Institute of Japan (NMIJ) with some uncertainty, depending on the stability of the node clocks and the distance from the master clock. The stability performance of the generated reference signal has been tested at Okinawa-the farthest prefecture from Tokyo, where the master clock is located (baseline distance of 1500 km). The primary rate (1.544 MHz) for telecommunication services was chosen for the 10-MHz signal generation in the experiment. A sinusoidal phase fluctuation within a one-day period is dominantly observed. This fluctuation is mainly caused by fiber expansion and contraction due to normal daily temperature changes. It degrades the stability (Allan deviation) to the level of 5 × 10-13 τ = 40 000 s, which is almost half a day). However, the major part of the phase fluctuation can be canceled by averaging a full day's data. In this case, the Allan deviation becomes 1 ×-13 which is obtained at Okinawa over ten consecutive days of measurement. The worst average frequency offset relative to UTC (NMIJ) (one-day averaging) is -6.3 × 10-13. The results indicate that this method promises to be suitable for most applications, providing an uncertainty of less than 1 × 10-12 at an averaging time of one day. [ABSTRACT FROM AUTHOR]

We performed spectroscopic observations of the 698-nm clock transition in 87Sr confined in an optical lattice using a laser linewidth transfer technique. A narrow-linewidth laser interrogating the clock transition was prepared by transferring the linewidth of a master laser (1064 nm) to that of a slave laser (698 nm) with a high-speed controllable fiber-based frequency comb. The Fourier-limited spectrum was then observed for an 80-ms interrogating pulse. We determined that the absolute frequency of the 5s21S0–5s5p 3P0 clock transition in 87Sr is 429 228 004 229 872.0 (1.6) Hz referenced to the SI second. [ABSTRACT FROM AUTHOR]

We demonstrate an improved absolute frequency measurement of the 1S0–3P0 clock transition at 578 nm in 171Yb atoms in a one-dimensional optical lattice. The clock laser linewidth is reduced to ≈2 Hz by phase-locking the laser to an ultrastable neodymium-doped yttrium aluminum garnet (Nd:YAG) laser at 1064 nm through an optical frequency comb with an intracavity electrooptic modulator to achieve a high servo bandwidth. The absolute frequency is determined as 518 295 836 590 863.1(2.0) Hz relative to the SI second, and will be reported to the International Committee for Weights and Measures. [ABSTRACT FROM AUTHOR]

Background: Delayed anterior cruciate ligament (ACL) reconstruction often causes residual anterolateral rotatory instability (ALRI) and consequent knee osteoarthritis, warranting the recommendation of early ACL reconstruction within 6 months after injury. Nonetheless, some cases show notable instability, even shortly after injury. The purpose of this study was to identify risk factors for an increase in quantitatively measured pivot shift in patients with ACL-deficient knees within 6 months after injury., Methods: Patients with primary ACL reconstruction within 6 months after injury and quantitative triaxial accelerometer measurements of preoperative pivot shift were included. Descriptive statistics were calculated for 11 independent variables (age, gender, time from injury to surgery, KT-1000, knee extension angle, lateral and medial posterior tibial slope angle, medial and lateral meniscus tears, ramp lesion and Tegner active scale). A single regression analysis was performed on the 11 items and acceleration during the pivot shift, and a multiple regression analysis was performed for items with p value less than 0.1., Results: Overall, 111 patients met the inclusion criteria. Single regression analysis showed that medial posterior tibial slope angle, medial meniscus tear and ramp lesion were significantly correlated with acceleration during the pivot shift ( p  < 0.001, p  = 0.04 and p  = 0.001). Multiple regression analysis identified medial posterior tibial slope angles and ramp lesions as independent factors for increased preoperative pivot shift ( p  = 0.005 and 0.01)., Conclusion: A steep medial posterior tibial slope angle and ramp lesion were independent risk factors for increased quantitatively measured pivot shift in patients with ACL-deficient knees., Level of Evidence: Level IV, case series study., Competing Interests: The authors declare no conflict of interest., (© 2024 The Author(s). Purpose: The correlation of cartilage thickness measured by three-dimensional (3D) magnetic resonance imaging (MRI) and the medial meniscal coverage ratio (MMCR), which presented pathology of the medial meniscus extrusion (MME) in 3D MRI, has not yet been elucidated. The study's purpose was to retrospectively verify whether the average cartilage thickness calculated by the automatic MRI 3D analysis system for each subregion was correlated with MMCR., Methods: A total of 60 patients underwent medial meniscus repair or high tibial osteotomy to treat their medial knee osteoarthritis. Cartilage thickness and MMCR were automatically calculated using 3D MRI software. The MMCR was defined as the ratio of the area covered by the meniscus within the medial tibial cartilage area to the total medial tibial cartilage area. The association between MMCR and the average cartilage thickness at 15 subregions in the medial femoral condyle (MFC) and medial tibial plateau (MTP) was evaluated using Spearman's rank correlation coefficient., Results: Kellgren-Lawrence grade exhibited a negative correlation with MMCR and a positive correlation with MME width. Cartilage thickness in the MTP had a moderately positive correlation with MMCR at four subregions and a weakly positive correlation at another subregion. Purpose: The correlation between cartilage thickness evaluated by 3D MRI and knee alignment has not been elucidated. The study's purpose was to retrospectively verify whether average cartilage thickness calculated by the automatic MRI 3D analysis system for each subregion was correlated with knee alignment., Methods: A total of 53 patients underwent medial meniscus repair or high tibial osteotomy for medial knee osteoarthritis. Cartilage thickness was automatically calculated using 3D MRI software. Knee alignment, weight-bearing line ratio (WBLR), joint line convergence angle (JLCA), medial proximal tibial angle (MPTA), and lateral distal femoral angle (LDFA) were calculated on a weight-bearing long-leg radiograph using digital planning software. The association between knee alignment and the average cartilage thickness at 18 subregions in the medial femoral condyle (MFC) and medial tibial plateau (MTP) was evaluated using Spearman's rank correlation coefficient., Results: Cartilage thickness of the MFC had moderately positive correlations with WBLR at four subregions and weak correlations at two subregions. Cartilage thickness of the MTP had moderately positive correlations with WBLR at four subregions. Cartilage thickness of the MFC had moderately negative correlations with JLCA at six subregions. Cartilage thickness of the MTP had moderately negative correlations with JLCA at four subregions and a weak correlation at one subregion. Conversely, cartilage thickness had weak correlations with MPTA or LDFA., Conclusions: In knees with pathological conditions in the medial compartment, cartilage thicknesses by 3D MRI had positive correlations with WBLR and JLCA at almost all subregions of the MFC and at the anterior-middle and central-external of the MTP.
