Patients with stable heart failure and their caregivers were consecutively enrolled from March 2021 to April 2022. Participants were managed by nurses in a community health center through education and monitoring with a mobile app. The outcomes were feasibility outcomes, self-care outcomes, and qualitative acceptability and satisfaction. Quantitative and qualitative outcomes were linked to understanding how the model of care might benefit patients.Results: Twenty-six patients and nine of their caregivers were enrolled. Ten participants used the mobile app. Nineteen patients and eight caregivers were interviewed. Participants who improved their self-care appreciated the help in finding coping strategies, being close to the clinic, and feeling cared for. Participants with fewer improvements in self-care perceived the model of care as useless and were far from the centre. Participants decided to use the app mainly for usefulness prevision, and most of them were satisfied.Conclusion: The model of care was not successful in recruiting patients, and adjustments are needed to improve the recruitment strategy and to engage people who perceive the model of care as not useful or unable to use the app.Keywords: nursing, primary care, heart failure, cardiology, general practice

Abstract

The accretion flow / jet correlation in neutron star (NS) low-mass X-ray binaries (LMXBs) is far less understood when compared to black hole (BH) LMXBs. In this paper we will present the results of a dense multi-wavelength observational campaign on the NS LMXB 4U 1820-30, including X-ray (Nicer, NuSTAR and AstroSAT) and quasi-simultaneous radio (ATCA) observations in 2022. 4U 1820-30 shows a peculiar 170 day super-orbital accretion modulation, during which the system evolves between "modes" of high and low X-ray flux. During our monitoring, the source did not show any transition to a full hard state. X-ray spectra were well described using a disc blackbody, a Comptonisation spectrum along with a Fe K emission line at 6.6 keV. Our results show that the observed X-ray flux modulation is almost entirely produced by changes in the size of the region providing seed photons for the Comptonisation spectrum. This region is large (about 15 km) in the high mode and likely coincides with the whole boundary layer, while it shrinks significantly (<10 km) in low mode. The electron temperature of the corona and the observed RMS variability in the hard X-rays also exhibit a slight increase in low mode. As the source moves from high to low mode, the radio emission due to the jet becomes about 5 fainter. These radio changes appear not to be strongly connected to the hard-to-soft transitions as in BH systems, while they seem to be connected mostly to variations observed in the boundary layer., Comment: 12 pages, 6 figures. Abstract

We report the discovery of Swift J221951-484240 (hereafter: J221951), a luminous slow-evolving blue transient that was detected by the Neil Gehrels Swift Observatory Ultra-violet/Optical Telescope (Swift/UVOT) during the follow-up of Gravitational Wave alert S190930t, to which it is unrelated. Swift/UVOT photometry shows the UV spectral energy distribution of the transient to be well modelled by a slowly shrinking black body with an approximately constant temperature of T~2.5x10^4 K. At a redshift z=0.5205, J221951 had a peak absolute magnitude of M_u,AB = -23 mag, peak bolometric luminosity L_max=1.1x10^45 erg s^-1 and a total radiated energy of E>2.6x10^52 erg. The archival WISE IR photometry shows a slow rise prior to a peak near the discovery date. Spectroscopic UV observations display broad absorption lines in N V and O VI, pointing toward an outflow at coronal temperatures. The lack of emission in the higher H~Lyman lines, N I and other neutral lines is consistent with a viewing angle close to the plane of the accretion or debris disc. The origin of J221951 can not be determined with certainty but has properties consistent with a tidal disruption event and the turn-on of an active galactic nucleus., Comment: 37 pages (25 main + 12 supplementary), submitted to MNRAS

Abstract

Ultraluminous X-ray sources (ULXs) are a class of accreting compact objects with X-ray luminosities above 10$^{39}$ erg s$^{-1}$. The average number of ULXs per galaxy is still not well constrained, especially given the uncertainty on the fraction of ULX transients. Here, we report the identification of a new transient ULX in the galaxy NGC 55 (which we label as ULX-2), thanks to recent XMM-Newton and the Neil Gehrels Swift Observatory observations. This object was previously classified as a transient X-ray source with a luminosity around a few 10$^{38}$ erg s$^{-1}$ in a 2010 XMM-Newton observation. Thanks to new and deeper observations ($\sim$ 130 ks each), we show that the source reaches a luminosity peak $>1.6 \times 10^{39}$ erg s$^{-1}$. The X-ray spectrum of ULX-2 is much softer than in previous observations and fits in the class of soft ULXs. It can be well described using a model with two thermal components, as often found in ULXs. The time scales of the X-ray variability are of the order of a month and are likely driven by small changes in the accretion rate or due to super-orbital modulations, attributed to precession of the accretion disc, which is similar to other ULXs., Comment: 6 pages, including 4 figures and 2 tables, accepted for pubblication in MNRAS

Abstract

Soft Ultra-Luminous X-ray (ULXs) sources are a subclass of the ULXs that can switch from a supersoft spectral state, where most of the luminosity is emitted below 1 keV, to a soft spectral state with significant emission above 1 keV. In a few systems, dips have been observed. The mechanism behind this state transition and the dips nature are still debated. To investigate these issues, we obtained a long XMM-Newton monitoring campaign of a member of this class, NGC 247 ULX-1. We computed the hardness-intensity diagram for the whole dataset and identified two different branches: the normal branch and the dipping branch, which we study with four and three hardness-intensity resolved spectra, respectively. All seven spectra are well described by two thermal components: a colder ($kT_{\rm bb}$ $\sim$ 0.1-0.2 keV) black-body, interpreted as emission from the photo-sphere of a radiatively-driven wind, and a hotter ($kT_{\rm disk}$ $\sim$ 0.6 keV) multicolour disk black-body, likely due to reprocessing of radiation emitted from the innermost regions. In addition, a complex pattern of emission and absorption lines has been taken into account based on previous high-resolution spectroscopic results. We studied the evolution of spectral parameters and the flux of the two thermal components along the two branches and discuss two scenarios possibly connecting the state transition and the dipping phenomenon. Abstract

In this paper, we report on the observational performance of the Swift Ultra-violet/Optical Telescope (UVOT) in response to the Gravitational Wave alerts announced by the Advanced Laser Interferometer Gravitational Wave Observatory and the Advanced Virgo detector during the O3 period. We provide the observational strategy for follow-up of GW alerts and provide an overview of the processing and analysis of candidate optical/UV sources. For the O3 period, we also provide a statistical overview and report on serendipitous sources discovered by Swift/UVOT. Swift followed 18 gravitational-wave candidate alerts, with UVOT observing a total of 424 deg^2. We found 27 sources that changed in magnitude at the 3 sigma level compared with archival u or g-band catalogued values. Swift/UVOT also followed up a further 13 sources reported by other facilities during the O3 period. Using catalogue information, we divided these 40 sources into five initial classifications: 11 candidate active galactic nuclei (AGN)/quasars, 3 Cataclysmic Variables (CVs), 9 supernovae, 11 unidentified sources that had archival photometry and 6 uncatalogued sources for which no archival photometry was available. We have no strong evidence to identify any of these transients as counterparts to the GW events. The 17 unclassified sources are likely a mix of AGN and a class of fast-evolving transient, and one source may be a CV., Comment: 25 pages, 6 figures and 5 tables. Submitted to MNRAS. Abstract

Most ultraluminous X-ray sources (ULXs) are believed to be stellar mass black holes or neutron stars accreting beyond the Eddington limit. Determining the nature of the compact object and the accretion mode from broadband spectroscopy is currently a challenge, but the observed timing properties provide insight into the compact object and details of the geometry and accretion processes. Here we report a timing analysis for an 800 ks XMM-Newton campaign on the supersoft ultraluminous X-ray source, NGC 247 ULX-1. Deep and frequent dips occur in the X-ray light curve, with the amplitude increasing with increasing energy band. Power spectra and coherence analysis reveals the dipping preferentially occurs on $\sim 5$ ks and $\sim 10$ ks timescales. The dips can be caused by either the occultation of the central X-ray source by an optically thick structure, such as warping of the accretion disc, or from obscuration by a wind launched from the accretion disc, or both. This behaviour supports the idea that supersoft ULXs are viewed close to edge-on to the accretion disc., Comment: Accepted MNRAS version following minor comments. 8 pages, 6 figures

Abstract

Most ULXs are believed to be powered by super-Eddington accreting neutron stars and, perhaps, black holes. Above the Eddington rate the disc is expected to thicken and to launch powerful winds through radiation pressure. Winds have been recently discovered in several ULXs. However, it is yet unclear whether the thickening of the disc or the wind variability causes the switch between the classical soft and supersoft states observed in some ULXs. In order to understand such phenomenology and the overall super-Eddington mechanism, we undertook a large (800 ks) observing campaign with XMM-Newton to study NGC 247 ULX-1, which shifts between a supersoft and classical soft ULX state. The new observations show unambiguous evidence of a wind in the form of emission and absorption lines from highly-ionised ionic species, with the latter indicating a mildly-relativistic outflow (-0.17c) in line with the detections in other ULXs. Strong dipping activity is observed in the lightcurve and primarily during the brightest observations, which is typical among soft ULXs, and indicates a close relationship between the accretion rate and the appearance of the dips. The latter is likely due to a thickening of the disc scale-height and the wind as shown by a progressively increasing blueshift in the spectral lines., Comment: 18 pages, 16 figures, accepted for publication in MNRAS

Abstract

THESEUS is a medium size space mission of the European Space Agency, currently under evaluation for a possible launch in 2032. Its main objectives are to investigate the early Universe through the observation of gamma-ray bursts and to study the gravitational waves electromagnetic counterparts and neutrino events. On the other hand, its instruments, which include a wide field of view X-ray (0.3-5 keV) telescope based on lobster-eye focusing optics and a gamma-ray spectrometer with imaging capabilities in the 2-150 keV range, are also ideal for carrying out unprecedented studies in time domain astrophysics. In addition, the presence onboard of a 70 cm near infrared telescope will allow simultaneous multi-wavelegth studies. Abstract

Ultraluminous X-ray sources are considered amongst the most extremely accreting objects in the local Universe. The recent discoveries of pulsating neutron stars in ULXs strengthened the scenario of highly super-Eddington accretion mechanisms on stellar mass compact objects. In this work, we present the first long-term light curve of the source NGC 4559 X7 using all the available Swift, XMM-Newton, Chandra and NuSTAR data. Thanks to the high quality 2019 XMM-Newton and NuSTAR observations, we investigated in an unprecedented way the spectral and temporal properties of NGC 4559 X7. The source displayed flux variations of up to an order of magnitude and an unusual flaring activity. We modelled the spectra from NGC 4559 X7 with a combination of two thermal components, testing also the addition of a further high energy cut-off powerlaw. We observed a spectral hardening associated with a luminosity increase during the flares, and a spectral softening in the epochs far from the flares. Narrow absorption and emission lines were also found in the RGS spectra, suggesting the presence of an outflow. Furthermore, we measured hard and (weak) soft lags with magnitudes of a few hundreds of seconds whose origin is possibly be due to the accretion flow. Abstract

The Neil Gehrels Swift Observatory followed up 18 gravitational wave (GW) triggers from the LIGO/Virgo collaboration during the O3 observing run in 2019/2020, performing approximately 6500 pointings in total. Of these events, four were finally classified (if real) as binary black hole (BH) triggers, six as binary neutron star (NS) events, two each of NSBH and Mass Gap triggers, one an unmodelled (Burst) trigger, and the remaining three were subsequently retracted. Thus far, four of these O3 triggers have been formally confirmed as real gravitational wave events. While no likely electromagnetic counterparts to any of these GW events have been identified in the X-ray data (to an average upper limit of 3.60 x 10^{-12} erg cm^{-2} s^{-1} over 0.3-10 keV), or at other wavelengths, we present a summary of all the Swift-XRT observations performed during O3, together with typical upper limits for each trigger observed. The majority of X-ray sources detected during O3 were previously uncatalogued; while some of these will be new (transient) sources, others are simply too faint to have been detected by earlier survey missions such as ROSAT. The all-sky survey currently being performed by eROSITA will be a very useful comparison for future observing runs, reducing the number of apparent candidate X-ray counterparts by up to 95 per cent., Comment: 23 pages (including 4 pages of references, and a 4 page table in the appendix), 5 figures (4 in colour), accepted for publication in MNRAS. Abstract

We report on our observing campaign of the compact binary merger GW190814, detected by the Advanced LIGO and Advanced Virgo detectors on August 14th, 2019. This signal has the best localisation of any observed gravitational wave (GW) source, with a 90% probability area of 18.5 deg$^2$, and an estimated distance of ~ 240 Mpc. We obtained wide-field observations with the Deca-Degree Optical Transient Imager (DDOTI) covering 88% of the probability area down to a limiting magnitude of $w$ = 19.9 AB. Nearby galaxies within the high probability region were targeted with the Lowell Discovery Telescope (LDT), whereas promising candidate counterparts were characterized through multi-colour photometry with the Reionization and Transients InfraRed (RATIR) and spectroscopy with the Gran Telescopio de Canarias (GTC). We use our optical and near-infrared limits in conjunction with the upper limits obtained by the community to constrain the possible electromagnetic counterparts associated with the merger. A gamma-ray burst seen along its jet's axis is disfavoured by the multi-wavelength dataset, whereas the presence of a burst seen at larger viewing angles is not well constrained. Although our observations are not sensitive to a kilonova similar to AT2017gfo, we can rule out high-mass (> 0.1 M$_{\odot}$) fast-moving (mean velocity >= 0.3c) wind ejecta for a possible kilonova associated with this merger., Comment: 17 pages, 11 figures, 5 tables; updated acknowledgement section. Abstract

We report on the serendipitous discovery of a new transient in NGC 5907, at a peak luminosity of 6.4x10^{39} erg/s. The source was undetected in previous 2012 Chandra observations with a 3 sigma upper limit on the luminosity of 1.5x10^{38} erg/s, implying a flux increase of a factor of >35. We analyzed three recent 60ks/50ks Chandra and 50ks XMM-Newton observations, as well as all the available Swift observations performed between August 2017/March 2018. Until the first half of October 2017, Swift observations do not show any emission from the source. The transient entered the ULX regime in less than two weeks and its outburst was still on-going at the end of February 2018. The 0.3-10 keV spectrum is consistent with a single multicolour blackbody disc (kT~1.5 keV). The source might be a ~30 solar mass black hole accreting at the Eddington limit. Abstract

Ultraluminous X-ray sources (ULXs) are a class of accreting compact objects with X-ray luminosities above 1039 erg s-1. The average number of ULXs per galaxy is still not well-constrained, especially given the uncertainty on the fraction of ULX transients. Here, we report the identification of a new transient ULX in the galaxy NGC 55 (which we label as ULX-2), thanks to recent XMM-Newton and the Neil Gehrels Swift Observatory observations. This object was previously classified as a transient X-ray source with a luminosity around a few 1038 erg s-1 in a 2010 XMM-Newton observation. Thanks to new and deeper observations (∼130 ks each), we show that the source reaches a luminosity peak >1.6 × 1039 erg s-1. The X-ray spectrum of ULX-2 is much softer than in previous observations and fits in the class of soft ULXs. It can be well-described using a model with two thermal components, as often found in ULXs. The time-scales of the X-ray variability are of the order of a month and are likely driven by small changes in the accretion rate or due to super-orbital modulations, attributed to precession of the accretion disc, which is similar to other ULXs.

Abstract

BACKGROUND: The aim of this study was to measure and compare jump load and dynamic performance in elite volleyball athletes under varied conditions over an entire season of practices and games. Jump load and dynamic performance were compared among best jump height, mean jump height, as well as according to the number of jumps per game or practice session and the proportion of jumps higher than 50 cm relative to the total number of jumps in a practice or game. METHODS: Every jump performed by each of 12 players, in all practices and regular games (813 player-sessions in total), was measured by a particle accelerometer in accordance with a validated protocol (Vert, Fort Lauderdale, FL, USA). Data were collected and analyzed using STATA (SataCorp, College Station, TX, USA); the significance level for definition of confidence intervals was set to 95%, unless otherwise specified. Statistical analysis and comparison of means and proportions between groups was based on standard t-tests. RESULTS: Among player positions, the middle blocker consistently presented the greatest jump loads during the season; by comparison, the smallest jump loads were observed in the setter. CONCLUSIONS: Monitoring players’ jump loads and performance using a simple accelerometer provides evidence which can be used to plan individual player activity, roster composition, the season calendar and furthermore increase knowledge to reduce over-training and recurrence of injuries.

Abstract

Introduction. This study aimed to estimate the incidence of SARS-CoV-2 infection among pregnant women during the first pandemic wave in Italy, and to describe CO-VID-19 disease characteristics and maternal and perinatal outcomes. Materials and methods. National population-based prospective cohort study collecting information on women with SARS-CoV-2 diagnosis, confirmed within 7 days from hospital admission. Results. The national SARS-CoV-2 rate was 6.04 per 1,000 births (95% CI 5.62-6.49) among pregnant women and 7.54 (95% CI 7.47-7.61) among women in reproductive age. 72.1% of the cohort developed mild COVID-19 disease without pneumonia nor need for ventilatory support. Severe disease was significantly associated with women’s previous comorbidities (OR 2.55; 95% CI 0.98-6.90), obesity (OR 4.76; 95% CI 1.79-12.66) and citizenship from High Migration Pressure Countries (OR 3.43; 95% CI 1.27-9.25). Conclusions. During the first pandemic wave in Italy, the SARS-CoV-2 rate among pregnant women was lower compared to that detected among women of reproductive age, and risks of severe COVID-19 disease and adverse maternal and perinatal outcomes were rare.

Abstract

The new coronavirus emergency spread to Italy when little was known about the infection’s impact on mothers and newborns. This study aims to describe the extent to which clinical practice has protected childbirth physiology and preserved the mother–child bond during the first wave of the pandemic in Italy. A national population-based prospective cohort study was performed enrolling women with confirmed SARS-CoV-2 infection admitted for childbirth to any Italian hospital from 25 February to 31 July 2020. All cases were prospectively notified, and information on peripartum care (mother–newborn separation, skin-to-skin contact, breastfeeding, and rooming-in) and maternal and perinatal outcomes were collected in a structured form and entered in a web-based secure system. The paper describes a cohort of 525 SARS-CoV-2 positive women who gave birth. At hospital admission, 44.8% of the cohort was asymptomatic. At delivery, 51.9% of the mothers had a birth support person in the delivery room; the average caesarean section rate of 33.7% remained stable compared to the national figure. On average, 39.0% of mothers were separated from their newborns at birth, 26.6% practised skin-to-skin, 72.1% roomed in with their babies, and 79.6% of the infants received their mother’s milk. The infants separated and not separated from their SARS-CoV-2 positive mothers both had good outcomes. At the beginning of the pandemic, childbirth raised awareness and concern due to limited available evidence and led to “better safe than sorry” care choices. An improvement of the peripartum care indicators was observed over time.

Abstract

Purpose Therapeutic strategies for prostate cancer (PCa) have been evolving dramatically worldwide. The current article reports on the evolution of surgical management strategies for PCa in Italy. Methods The data from two independent Italian multicenter projects, the MIRROR-SIU/LUNA (started in 2007, holding data of 890 patients) and the Pros-IT-CNR project (started in 2014, with data of 692 patients), were compared. Differences in patients' characteristics were evaluated. Multivariable logistic regression models were used to identify characteristics associated with robot-assisted (RA) procedure, nerve sparing (NS) approach, and lymph node dissection (LND). Results The two cohorts did not differ in terms of age and prostate-specific antigen (PSA) levels at biopsy. Patients enrolled in the Pros-IT-CNR project more frequently were submitted to RA (58.8% vs 27.6%,p < 0.001) and NS prostatectomy (58.4% vs. 52.9%,p = 0.04), but received LND less frequently (47.7% vs. 76.7%,p < 0.001), as compared to the MIRROR-SIU/LUNA patients. At multivariate logistic models, Lower Gleason Scores (GS) and PSA levels were significantly associated with RA prostatectomy in both cohorts. As for the MIRROR-SIU/LUNA data, clinical T-stage was a predictor for NS (OR = 0.07 for T3, T4) and LND (OR = 2.41 for T2) procedures. As for Pros-IT CNR data, GS >= (4 + 3) and positive cancer cores >= 50% were decisive factors both for NS (OR 0.29 and 0.30) and LND (OR 7.53 and 2.31) strategies. Conclusions PCa management has changed over the last decade in Italian centers: RA and NS procedures without LND have become the methods of choice to treat newly medium-high risk diagnosed PCa.

Abstract

Introduction. This study aimed to estimate the incidence of SARS-CoV-2 infection among pregnant women during the first pandemic wave in Italy, and to describe CO-VID-19 disease characteristics and maternal and perinatal outcomes. Materials and methods. National population-based prospective cohort study collecting information on women with SARS-CoV-2 diagnosis, confirmed within 7 days from hospital admission. Results. The national SARS-CoV-2 rate was 6.04 per 1,000 births (95% CI 5.62-6.49) among pregnant women and 7.54 (95% CI 7.47-7.61) among women in reproductive age. 72.1% of the cohort developed mild COVID-19 disease without pneumonia nor need for ventilatory support. Severe disease was significantly associated with women’s previous comorbidities (OR 2.55; 95% CI 0.98-6.90), obesity (OR 4.76; 95% CI 1.79-12.66) and citizenship from High Migration Pressure Countries (OR 3.43; 95% CI 1.27-9.25). Conclusions. During the first pandemic wave in Italy, the SARS-CoV-2 rate among pregnant women was lower compared to that detected among women of reproductive age, and risks of severe COVID-19 disease and adverse maternal and perinatal outcomes were rare.

Abstract

BACKGROUND: We assessed patients and tumor characteristics, as well as health-related quality of life (HRQoL) items, associated with curative intent treatment decision-making in clinically localized prostate cancer (PCa) patients. METHODS: Clinically localized PCa treated with either radical prostatectomy (RP) or radiation therapy (RT) within 12 months from diagnosis were abstracted from The PROState cancer monitoring in ITaly, from the National Research Council (Pros-IT CNR) database. Multivariable logistic regression (MLR) models predicting RT vs. RP were fitted, after adjustment for HRQoL items, patients and tumor characteristics. RESULTS: Of 1041 patients, 631 (60.2%) were treated with RP and 410 (39.8%) with RT. Relative to RT, RP patients were younger age (mean age 64.5±6.6 vs. 71.4±4.9, P<0.001) and had higher rates of D’Amico low-intermediate risk groups (31.8 vs. 21.9% low, 46.3% vs. 43.5% intermediate and 21.9% vs. 34.6% high risk, P<0.001). Overall, 93.2% of RP patients were enrolled by urologists and 82.7% of RT patients by radiation oncologists. RP patients had generally higher means values of HRQoL items. In MLR models, higher RT rates were independently associated with more advanced age (odds ratio [OR] 6.14, P<0.001) and BMI≥30 kg/m2 (OR 1.78, P<0.001). Conversely, lower rates of RT were independently associated with married (OR 0.55, P=0.01) and worker status (OR 0.52, P=0.004), enrollment in academic centers (OR 0.59, P=0.005) and higher physical composite score (OR 0.88, P=0.03) and baseline sexual function items (OR 0.92, P<0.001). CONCLUSIONS: Most patients with clinically localized prostate cancer undergoing definitive treatment at Italian institutions receive RP instead of RT. Moreover, those who are younger, married, working, as well as those with better physical and sexual function are more likely to undergo surgery.

32. Investigating the nature of the ultraluminous X-ray sources in the galaxy NGC 925

Abstract

Variability is a powerful tool to investigate properties of X-ray binaries (XRB), in particular for Ultraluminous X-ray sources (ULXs) that are mainly detected in the X-ray band. For most ULXs the nature of the accretor is unknown, although a few ULXs have been confirmed to be accreting at super-Eddington rates on to a neutron star (NS). Monitoring these sources is particularly useful both to detect transients and to derive periodicities, linked to orbital and super-orbital modulations. Here, we present the results of our monitoring campaign of the galaxy NGC 925, performed with the Neil Gehrels Swift Observatory. We also include archival and literature data obtained with Chandra, XMM-Newton, and NuSTAR. We have studied spectra, light curves, and variability properties on days to months time-scales. All the three ULXs detected in this galaxy show flux variability. ULX-1 is one of the most luminous ULXs known, since only 10 per cent of the ULXs exceed a luminosity of ∼5 × 1040 erg s-1, but despite its high flux variability we found only weak spectral variability. We classify it as in a hard ultraluminous regime of super-Eddington accretion. ULX-2 and ULX-3 are less luminous but also variable in flux and possibly also in spectral shape. We classify them as in between the hard and the soft ultraluminous regimes. ULX-3 is a transient source: by applying a Lomb-Scargle algorithm, we derive a periodicity of ∼126 d, which could be associated with an orbital or superorbital origin.

Determining the nature of the compact object and the accretion mode from broad-band spectroscopy is currently a challenge, but the observed timing properties provide insight into the compact object and details of the geometry and accretion processes. Here, we report a timing analysis for an 800 ks XMM–Newton campaign on the supersoft ultraluminous X-ray source, NGC 247 ULX-1. Deep and frequent dips occur in the X-ray light curve, with the amplitude increasing with increasing energy band. Power spectra and coherence analysis reveals the dipping preferentially occurs on ∼5 and ∼10 ks time-scales. The dips can be caused by either the occultation of the central X-ray source by an optically thick structure, such as warping of the accretion disc, or from obscuration by a wind launched from the accretion disc, or both. This behaviour supports the idea that supersoft ULXs are viewed close to edge-on to the accretion disc.

Abstract

ABSTRACT Soft Ultra-Luminous X-ray (ULXs) sources are a subclass of the ULXs that can switch from a supersoft spectral state, where most of the luminosity is emitted below 1 keV, to a soft spectral state with significant emission above 1 keV. In a few systems, dips have been observed. The mechanism behind this state transition and the dips nature are still debated. To investigate these issues, we obtained a long XMM–Newton monitoring campaign of a member of this class, NGC 247 ULX-1. We computed the hardness–intensity diagram for the whole data set and identified two different branches: the normal branch and the dipping branch, which we study with four and three hardness–intensity resolved spectra, respectively. All seven spectra are well described by two thermal components: a colder (kTbb ∼ 0.1–0.2 keV) blackbody, interpreted as emission from the photosphere of a radiatively driven wind, and a hotter (kTdisc ∼ 0.6 keV) multicolour disc blackbody, likely due to reprocessing of radiation emitted from the innermost regions. In addition, a complex pattern of emission and absorption lines has been taken into account based on previous high-resolution spectroscopic results. We studied the evolution of spectral parameters and flux of the two thermal components along the two branches and discuss two scenarios possibly connecting the state transition and the dipping phenomenon. One is based on geometrical occultation of the emitting regions, the other invokes the onset of a propeller effect.

Abstract

Aims: The aims of the study were (a) to assess whether the proportion of female nurses and female physicians in a given unit influences the attitude of collaboration between nurses and physicians as reported by nurses, and (b) to examine how these two dimensions interact to influence attitudes towards cooperation. Background: Available studies have documented that gender influences the collaboration between physicians and nurses, but no have explored the influence of specific combinations—such as a high proportion of female nurses and a high proportion of female physicians. Methods: A cross-sectional study involving 700 nurses working in 36 hospital units in Italy. The validated Italian version of the Jefferson Scale of Attitudes towards Physician–Nurse Collaboration was used. A three stages hierarchical linear regression was performed by entering: (a) the control variables at the individual and at the unit levels, (b) the proportion of female nurses and physicians and (c) the two-way interaction. Results: A total of 430 nurses participated; the average Jefferson Scale of Attitudes towards Physician–Nurse Collaboration total score was 48.64 ± 5.27. Abstract

Background Implementation of high-quality national audits for perinatal mortality are needed to improve the registration of all perinatal deaths and the identification of the causes of death. This study aims to evaluate the implementation of a Regional Audit System for Stillbirth in Emilia-Romagna Region, Italy. Methods For each stillbirth (≥ 22 weeks of gestation, ≥ 500 g) occurred between January 1, 2014 to December 1, 2016 (n = 332), the same diagnostic workup was performed and a clinical record with data about mother and stillborn was completed. Every case was discussed in a multidisciplinary local audit to assess both the cause of death (ReCoDe classification) and the quality of care. Data were reviewed by the Regional Audit Group. Stillbirth rates, causes of death and the quality of care were established for each case. Results Total stillbirth rate was 3.09 per 1000 births (332/107,528). Late stillbirth rate was 2.3 per 1000 (251/107,087). Sixteen stillbirths were not registered by the Regional Birth Register. The most prevalent cause of death was placental disorder (33.3%), followed by fetal (17.6%), cord (14.2%) and maternal disorders (7.6%). Unexplained cases were 14%. Compared to local audits, the regional group attributed different causes of death in 17% of cases. At multivariate analysis, infections were associated with early stillbirths (OR 3.38, CI95% 1.62–7.03) and intrapartum cases (OR 6.64, CI95% 2.61–17.02). Placental disorders were related to growth restriction (OR 1.89, CI95% 1.06–3.36) and were more frequent before term (OR 1.86, CI95% 1.11–3.15). Stillbirths judged possibly/probably preventable with a different management (10.9%) occurred more frequently in non-Italian women and were mainly related to maternal disorders (OR 6.64, CI95% 2.61–17.02). Conclusions Regional Audit System for Stillbirth improves the registration of stillbirth and allows to define the causes of death. Moreover, sub-optimal care was recognized, allowing to identify populations which could benefit from preventive measures.

Abstract

Background There are several treatments available to newly diagnosed prostate cancer (PCA) patients. Although surgery and radiotherapy (RT) with or without androgen deprivation therapy (ADT) are widely adopted treatment options for localized PCA together with active surveillance (AS), there is no consensus nor randomised trials on treatment selection, prospective quality of life (QOL), along with toxicity outcomes and according to treatment modality in the Italian population. The current study aimed to describe clinical-therapeutic features and QOL at PCA diagnosis, according to different treatment patterns in a large prospective, Italian population, enrolled in the Pros-IT CNR study. Methods The Pros-IT CNR is an on-going national, multicenter, observational, prospective study on patients affected by PCA who have been referred by 97 Italian Urology, Radiation Oncology and Medical Oncology facilities participating in the project. The possible relationships between the treatment patterns reported in the 6 month follow-up case report form and patients’ features at diagnosis were evaluated using exploratory multiple correspondence analysis (MCA) and other data analysis method. Results At diagnosis, surgery and AS patients were significantly younger, had fewer comorbidities, lower PSA levels and Gleason Score (GS) values; they were also diagnosed at an earlier stage of disease with respect to the RT or ADT patients who showed significantly worse QoL scores at the time of diagnosis. Conclusions An analysis of the data collected at baseline and 6 months later uncovered substantial differences in ages, comorbidities, clinical and QOL features in the various treatment groups. These findings do not fully reflect the current PCA treatment guidelines and suggest the need for a multidisciplinary consensus guideline to ameliorate both the counselling and treatments of PCA patients.

Abstract

Background: To report health-related quality of life outcomes as assessed by validated patient-reported outcome measures (PROMs) after radical prostatectomy (RP).-Methods: This study analyzed patients treated with RP within The PROState cancer monitoring in Italy, from the National Research Council (Pros-IT CNR). Italian versions of Short-Form Heath Survey and university of California los Angeles-prostate cancer index questionnaires were administered. PROMs were physical composite scores, mental composite scores and urinary, bowel, sexual functions and bothers (UF/B, BF/B, SF/B). Baseline unbalances were controlled with propensity scores and stabilized inverse weights; differences in PROMs between different RP approaches were estimated by mixed models. Results: Of 541 patients treated with RP, 115 (21%) received open RP (ORP), 90 (17%) laparoscopic RP (LRP) and 336 (61%) robot-assisted RP (RARP). At head-to-head-comparisons, RARP showed higher 12-month UF vs. LRP (interaction treatment ∗ time p = 0.03) and 6-month SF vs. ORP (p < 0.001). At 12-month from surgery, 67, 73 and 79% of patients used no pad for urinary loss in ORP, LRP and RARP respectively (no differences for each comparison). Conversely, 16, 27 and 40% of patients declared erections firm enough for sexual intercourse in ORP, LRP and RARP respectively (only significant difference for ORP vs. RARP, p = 0.0004). Conclusions: Different RP approaches lead to significant variations in urinary and sexual PROMs, with a general trend in favour of RARP. However, their clinical significance seems limited.

Abstract

Background There are several treatments available to newly diagnosed prostate cancer (PCA) patients. Although surgery and radiotherapy (RT) with or without androgen deprivation therapy (ADT) are widely adopted treatment options for localized PCA together with active surveillance (AS), there is no consensus nor randomised trials on treatment selection, prospective quality of life (QOL), along with toxicity outcomes and according to treatment modality in the Italian population. The current study aimed to describe clinical-therapeutic features and QOL at PCA diagnosis, according to different treatment patterns in a large prospective, Italian population, enrolled in the Pros-IT CNR study. Methods The Pros-IT CNR is an on-going national, multicenter, observational, prospective study on patients affected by PCA who have been referred by 97 Italian Urology, Radiation Oncology and Medical Oncology facilities participating in the project. The possible relationships between the treatment patterns reported in the 6 month follow-up case report form and patients' features at diagnosis were evaluated using exploratory multiple correspondence analysis (MCA) and other data analysis method. Results At diagnosis, surgery and AS patients were significantly younger, had fewer comorbidities, lower PSA levels and Gleason Score (GS) values; they were also diagnosed at an earlier stage of disease with respect to the RT or ADT patients who showed significantly worse QoL scores at the time of diagnosis. Conclusions An analysis of the data collected at baseline and 6 months later uncovered substantial differences in ages, comorbidities, clinical and QOL features in the various treatment groups. These findings do not fully reflect the current PCA treatment guidelines and suggest the need for a multidisciplinary consensus guideline to ameliorate both the counselling and treatments of PCA patients.

Abstract

Ultraluminous X-ray sources (ULXs) are a class of accreting compact objects with X-ray luminosities above 1039 erg s−1. The average number of ULXs per galaxy is still not well-constrained, especially given the uncertainty on the fraction of ULX transients. Here, we report the identification of a new transient ULX in the galaxy NGC 55 (which we label as ULX-2), thanks to recent XMM–Newton and the Neil Gehrels Swift Observatory observations. This object was previously classified as a transient X-ray source with a luminosity around a few 1038 erg s−1 in a 2010 XMM–Newton observation. Thanks to new and deeper observations (∼130 ks each), we show that the source reaches a luminosity peak >1.6 × 1039 erg s−1. The X-ray spectrum of ULX-2 is much softer than in previous observations and fits in the class of soft ULXs. It can be well-described using a model with two thermal components, as often found in ULXs. The time-scales of the X-ray variability are of the order of a month and are likely driven by small changes in the accretion rate or due to super-orbital modulations, attributed to precession of the accretion disc, which is similar to other ULXs.
