4. Control and treatment of type 2 diabetes in private patients in the Egyptian "DIACONTROL study".

Abstract

OBJECTIVE: This study aims to identify the characteristics of Egyptian patients suffering from type 2 diabetes mellitus (T2DM), determine disease control rates, and gain insights into clinical treatments. PATIENTS AND METHODS: A total of 2,516 patients with T2DM were recruited from 244 private clinics across Egypt in a one-month period from May to June 2017. Data collected from patients included glycemic control parameters of glycosylated hemoglobin, fasting plasma glucose, and postprandial glucose. Additional information gathered included patients' weight, age, level of physical activity, smoking habits, presence of comorbidities, type of treatment received for type 2 diabetes, number and severity of hypoglycemic events, as well as treatment modification by the physician in the last visit. The type of statistics used for the analysis is descriptive statistics and regression model. RESULTS: Only 18.4% of participating patients achieved the target level of glycosylated hemoglobin of 7% or below. The mean age of these patients was 54±11.2 years, and the mean duration since the first diagnosis was 6.6±6.4 years. A total of 33.4% of all patients had no known comorbidity, while the rest had one or more known and treated comorbidities. A total of 76% of patients received sulfonylurea either as monotherapy or in combination with other treatments. In addition, no treatment modifications or adjustments were provided for 32% of the study participants who did not reach their glycemic control target. CONCLUSIONS: In Egypt, there is a low rate of glycemic control among private patients and a high prevalence of comorbid conditions. This is likely to cause a significant health burden to people with T2DM, the healthcare system, and the economy due to a loss in productivity. This study presented an argument for better-managed measures to improve glycemic control in the population, such as patient education to increase patient awareness and adherence to treatment protocols as well as improved adherence to guidelines by clinicians. 5. New Media and Crime among Adolescents in Anaocha LGA of Anambra Nigeria

Abstract

New media use has been recently understood as an important issue to be looked into among adolescents, parents and others who deal with adolescents. This study sought to examine the new media and crime among adolescents in Anaocha Local Government Area of Anambra State. The study employed the cross-sectional descriptive design method involving a quantitative approach which included a survey of 224 adolescents between the ages of 13-19 years selected by cluster and simple random sampling techniques from five communities in the LGA. The instrument for data collection was interviewer-administered questionnaire while the Statistical Packages for Social Sciences (SPSS) was used in analyzing the data. Findings revealed that phones were the most prevalent new media in use among adolescents particularly the male ones. Also, majority expressed negative view about the influence of the new media on adolescents, with sexting and cyber-bullying as the most common crimes associated with new media. Furthermore, psychological trauma was identified as the major effect of cyber-bullying on adolescent victims. This study recommended enlightenment of adolescents and other stakeholders in order to curtail the level of crimes committed through the new media. Parents and significant others are advised to keep a close watch on their adolescent children and wards while they are browsing the internet, and this will hopefully control criminal activities associated with new media use among adolescents in Anaocha LGA in particular and Nigeria in general.

10. Changes in Antibiotic Use and Disruptions to Antimicrobial Resistance Detection in South Africa and Uganda, 2019 – 2020

Abstract

Background: The COVID-19 pandemic disrupted routine health services worldwide, including systems to detect antimicrobial resistance (AR). AR is a mounting global health threat with some studies showing the highest mortality rate from AR infection is in Sub-Saharan Africa (SSA). Antibiotic use is a major contributor to AR. We sought to characterize COVID-19-related changes to antibiotic use and AR detection capacity in two countries in SSA from 2019 to 2020. Methods: Health facilities (HF) in South Africa and Uganda were surveyed as part of a larger study assessing disruptions to essential health services in SSA in the context of COVID-19. Modified stratified random sampling of HF by facility level was conducted in regions with high COVID-19 cumulative prevalence. Hospital pharmacists were surveyed to identify perceived changes in antibiotic use. Among facilities with the capacity to detect AR, surveys were conducted with AR laboratory managers to identify perceived changes in staff, equipment, training, and supplies. Descriptive data analysis was conducted using frequencies and proportions. Results: A total of 39 HFs in South Africa and 45 HFs in Uganda responded to the antibiotic use survey. Increases in total antibiotic use from 2019 to 2020 were reported by 82% (23/28) of HF in South Africa and 68% (27/40) in Uganda. Increased use of antibiotics for multi-drug resistant bacteria (per World Health Organization Reserve classification) was reported by 36% (9/25) and 38% (8/21) of HFs in South Africa and Uganda, respectively. 19 HFs in South Africa and 12 HFs in Uganda responded to the AR detection capacity survey. HFs in both countries reported decreases in laboratory staff responsible for AR (33% [13/40] in South Africa and 31% [11/35] in Uganda). Decreased availability of reagents and consumables for bacteriology and antimicrobial susceptibility testing was reported by 50% (8/16) and 33% (4/12) of HFs, and decreased availability of specimen collection supplies for bacterial cultures was reported by 41% (7/17) and 42% (5/12) of HFs in South Africa and Uganda, respectively. Diversion of laboratory supplies was reported in both countries (32% [6/19] in South Africa and 25% [3/12] of HF in Uganda). Conclusions: HFs in South Africa and Uganda reported increases in antibiotic prescribing, a risk factor for increased AR, concurrently with disruptions in AR detection capacity during the early phases of the COVID-19 pandemic. These findings emphasize the importance of investing in bacteriology and AR testing in SSA and maintaining support during infectious disease pandemics.

11. Investigation of Healthcare-Associated Infection Risks from Ice: Summary of CDC Consultations 2016-2023

Background: Nonsterile ice is frequently used in healthcare settings for a wide array of patient care activities and clinical procedures. However, this ice can harbor pathogenic organisms which can threaten patient safety and cause outbreaks. We sought to characterize recent Centers for Disease Control and Prevention (CDC) consultations involving ice leading to healthcare-associated infections (HAIs). Methods: We reviewed internal CDC records from the Division of Healthcare Quality Promotion (DHQP) to identify investigations of outbreaks and potential outbreaks involving the use of ice in healthcare facilities. We searched records from January 1, 2016, through November 30, 2023, for keywords related to ice. We excluded consultations in which ice was not thought to be a potential transmission pathway as well as those in which only sterile ice products (e.g., saline slush) were investigated. Results: We identified 45 consultations for ice-related investigations, involving a total of 533 patients. Nontuberculous mycobacteria were the most frequently implicated organisms, appearing in 40% (n=18) of investigations. Eighty-four percent (n=38) of investigations occurred in acute care hospitals. The most frequently implicated hospital settings were intensive care units (13%, n=6), operating rooms (13%, n=6), and bronchoscopy suites (13%, n=6). We identified a variety of plausible exposure pathways, including direct ingestion of ice by patients, use of ice during the bronchoscopy procedure, use of nonsterile ice in heater-cooler devices during cardiothoracic surgery, and the use of ice to chill saline for respiratory care. Environmental sampling directly of ice machines was performed in 62% of investigations (n=28) and nonsterile ice from these machines was sampled in 9% of investigations (n=4). Among those investigations in which ice machines were sampled, the organism implicated in the outbreak was isolated in 54% of investigations (n=15). Among those investigations in which ice itself was sampled, the organism implicated in the outbreak was isolated in 75% of investigations (n=3). These organisms included Mycobacterium mucogenicum, Burkholderia multivorans, and Acanthamoeba spp. Conclusions: The use of nonsterile ice during clinical care is a potential source of pathogens that cause patient infections and HAI outbreaks. Healthcare personnel should be aware of the risk posed by nonsterile ice and consider avoiding its use, especially when caring for patients who are critically ill or immunocompromised. Healthcare facilities should ensure regular cleaning and disinfection of ice machines to decrease their microbial burden. When HAI outbreaks involving water-associated organisms are identified, nonsterile ice should be considered as a potential mode of transmission.

Abstract

Transactive Energy (TE) has brought exciting opportunities for all stakeholders in energy markets by enabling management decentralization. This new paradigm empowers demand-side agents to play a more active role through coordinating, cooperating, and negotiating with other agents. Nevertheless, most of these agents are not used to process market signals and develop optimal strategies, especially in the residential sector. Accordingly, it is indispensable to create tools that automate and facilitate demand-side participation in TE systems. This paper presents a new methodology for residential automated agents to perform two key tasks: prevision and planning. Specifically, the proposed method is applied to a forward market where agents' planning is a fundamental step to maintain the dynamic balance between demand and generation. Since planning depends on future demand, agents' prevision of consumption is an inevitable part of this step. The procedures for automating the targeted tasks are developed in a general way for residential prosumers and consumers, interacting at the distribution level. These players are managed by a demand aggregator as the leader by means of the Stackelberg game. The suggested process results in a TE setup for multi-stage single-side auctions, useful to manage future Smart Energy Markets. Through simulated transactions, this paper examines the market clearing mechanism and the convenience of agents' planning. The results show that customers with higher price-elasticity leverage lower costs periods. However, they make it harder to reduce the peak-to-average ratio of the aggregated demand profile since a unique price signal can create prisoner's dilemma conditions.

Abstract

The human prolactin-inducible protein/gross cystic disease fluid protein-15 (hPIP/GCDFP-15) is a secretory glycoprotein found primarily in apocrine tissues including the breast and salivary glands. With largely unknown functions, PIP has been implicated in breast cancer and metastasis, host defense processes and T lymphocyte apoptosis. To begin to address PIP function in vivo, we generated the PIP null mouse ([Pip.sup.-/-] mouse). Additionally, to determine the effect of the loss of PIP on gene expression and to gain insight into some of the molecular mechanisms underlying PIP function, microarray analysis of the submandibular gland was also undertaken. [Pip.sup.-/-] mice developed normally with no overt differences in behaviour or gross morphology and were fertile. However, histological examination of 3-month-old [Pip.sup.-/-] mice sometimes showed enlarged submandibular lymph nodes, lymphocytic aggregations within the prostate lobes, and enlarged medulla in the thymus. Functional analysis of gene expression revealed sets of multiple differentially expressed genes associated with cell death and survival, lipid metabolism, inflammation, immune disease, and cancer, as a consequence of mPIP abrogation. Taken together, these studies lend support to an immunomodulatory role for PIP in vivo and provide further insights into potentially novel signaling pathways and regulatory networks for PIP. Afin d'examiner la fonction de la PIP in vivo, nous avons produit une souris [Pip.sup.-/-] . De plus, pour determiner l'effet de l'absence de la PIP sur l'expression genique et pour mieux comprendre certains des mecanismes moleeculaires a la base de la fonction de la PIP, nous avons aussi analyse la glande sous-mandibulaire au moyen de microreseaux. 14. Infrastructure Development in West Africa: What Is the Role of Concessionary External Debt?

Abstract

Using two key components of Africa Infrastructure Index—energy and capital—we ask the data whether and how concessionary public external debt may have impacted infrastructure outcomes in West Africa. The study relies on the fixed effect panel data model and covers the period of 1995–2018 for 14 West African Economies. The key results are as follows: (i) we found fairly broad and robust support for a positive overall contribution of “concessionary” external debt to infrastructure endowment and economic development; (ii) the sign of the interactive factor (debt/institutions) turned out to be negative and significant—suggesting that the quality of institution is a key determinant of how external debt impact infrastructure outcome and development for countries in West Africa; (iii) external debt is found to be broadly higher in regressions that exclude investment implying that the primary contribution of external debt has been through investment efficiency. The policy import of the latter is that reduction in binding constraints to investments along with institutional reforms such as better control of corruption and improvement in government effectiveness would improve the positive contribution of concessionary external debt to infrastructure outcome and economic development in West Africa.

Abstract

Background: The coronavirus disease 2019 (COVID-19) pandemic severely affected Southeast Asia, with >35 million cases and ~360,000 deaths. Despite relatively low rates of secondary bacterial infection among inpatients with COVID-19, several countries reported increased antibiotic use; raising concerns for worsening antimicrobial resistance. We assessed the impact of the COVID-19 pandemic on the use of antibiotics commonly used to treat respiratory infections in Southeast Asia. Methods: We evaluated intravenous antibiotic use among hospitalized adults in acute-care wards in 6 hospitals; 3 in Indonesia and 3 in the Philippines. We abstracted data on antibiotics that are commonly used to treat respiratory infections in these hospitals. We calculated antibiotic use rates for the 25 included antibiotics as monthly defined daily dose per 1,000 patient days (or patient discharges where patient days was unavailable) using data from pharmacy dispensing records and administrative records. Median antibiotic use rates for the prepandemic period (March 2018–February 2020) and the pandemic period (March 2020–February 2021) were compared, and percentage changes were calculated for (1) all 25 antibiotics combined; (2) ceftriaxone; (3) vancomycin and linezolid combined (anti-MRSA); and (4) broad-spectrum antibiotics with activity against Pseudomonas aeruginosa (anti-PSA). Monthly antibiotic use and COVID-19 patient discharges were graphed over the 36-month study period (March 2018–February 2021) to visualize trends (Fig. 1). The Wilcoxon rank-sum test was used to determine whether differences in median antibiotic use rates were statistically significant (2-tailed P < .05). Results: Overall, trends in antibiotic use were higher during months with increased COVID-19 patient discharges (Fig. 1). Use of all 25 antibiotics combined significantly increased in 4 of 6 hospitals (6.9%–63.6%) during the pandemic period compared to the prepandemic period. Ceftriaxone use significantly increased in 3 of 6 hospitals (37.1%–55.4%) and decreased in 3 of 6 hospitals (15.9%–31.9%). L’intervento progettuale, che stabilisce un corretto rapporto tra architettura e preesistenza, si basa su un profondo senso della misura, intendendo con questo termine la capacità di dosare il gesto architettonico rispetto alle condizioni di contorno che sono assunte come prioritarie nell’intervento.

Abstract

Dietary antioxidants may be useful in counteracting the chronic inflammatory status in neurodegenerative diseases by reducing oxidative stress due to accumulation of reactive oxygen species (ROS). In this study, we newly described the efficacy of a number of dietary antioxidants (polyphenols, carotenoids, thiolic compounds, and oligoelements) on viability of neuronal PC12 cells following Nerve Growth Factor (NGF) deprivation, a model of age-related decrease of neurotrophic support that triggers neuronal loss. Neuroprotection by antioxidants during NGF deprivation for 24 h was largely dependent on their concentrations: all dietary antioxidants were able to efficiently support cell viability by reducing ROS levels and restoring mitochondrial function, while preserving the neuronal morphology. Moreover, ROS reduction and neuroprotection during NGF withdrawal were also achieved with defined cocktails of 3-6 different antioxidants at concentrations 5-60 times lower than those used in single treatments, suggesting that their antioxidant activity was preserved also at very low concentrations. Overall, these data indicate the beneficial effects of antioxidants against oxidative stress induced by decreased NGF availability and suggest that defined cocktails of dietary factors at low concentrations might be a suitable strategy to reduce oxidative damage in neurodegenerative diseases, while limiting possible side effects.

Abstract

Biodiesel wastewater generally contains high levels of oils, soaps and glycerol residues. This needs wastewater treatment. In this study, the biodiesel wastewater treatment was tested (industrial wastewater (EFID) and laboratory wastewater (EFLB) from biodiesel) by performing flocculation and dual-flocculation with renewable polymers. Tannin and cationic hemicellulose (CH) were used as cationic flocculant, and cellulose acetate sulfate (CAS) was used as an anionic flocculant. Polyacrylamide (PAM) was used as a reference anionic flocculant for result efficiencies analysis obtained with CAS (renewable source flocculant). The treatment efficacy in wastewater was evaluated by: turbidity removal, sludge volume formed, chemical oxygen demand (COD) and total suspended solids (TSS). The obtained sludge was studied using thermogravimetric analysis (TG). The dual-flocculation application condition of the 25% proportion of tannin (T) and 75% proportion of cationic hemicelluloses (i.e., T25/CH75) showed EFLB turbidity removal of 89.1% and 89.5% for CAS and PAM additions respectively, and for EFID of 67% and 41% for CAS and PAM additions respectively. The dual-flocculation performance suggested that the polyelectrolytes obtained from renewable sources can be used for treating biodiesel wastewater. [ABSTRACT FROM AUTHOR]

Abstract

Background: The genome of living organisms is constantly exposed to several damaging agents that induce different types of DNA lesions, leading to cellular malfunctioning and onset of many diseases. To maintain genome stability, cells developed various repair and tolerance systems to counteract the effects of DNA damage. Here we focus on Post Replication Repair (PRR), the pathway involved in the bypass of DNA lesions induced by sunlight exposure and UV radiation. PRR acts through two different mechanisms, activated by mono- and poly-ubiquitylation of the DNA sliding clamp, called Proliferating Cell Nuclear Antigen (PCNA).Results: We developed a novel protocol to measure the time-course ratios between mono-, di- and tri-ubiquitylated PCNA isoforms on a single western blot, which were used as the wet readout for PRR events in wild type and mutant S. cerevisiae cells exposed to acute UV radiation doses. Stochastic simulations of PCNA ubiquitylation dynamics, performed by exploiting a novel mechanistic model of PRR, well fitted the experimental data at low UV doses, but evidenced divergent behaviors at high UV doses, thus driving the design of further experiments to verify new hypothesis on the functioning of PRR. The model predicted the existence of a UV dose threshold for the proper functioning of the PRR model, and highlighted an overlapping effect of Nucleotide Excision Repair (the pathway effectively responsible to clean the genome from UV lesions) on the dynamics of PCNA ubiquitylation in different phases of the cell cycle. In addition, we showed that ubiquitin concentration can affect the rate of PCNA ubiquitylation in PRR, offering a possible explanation to the DNA damage sensitivity of yeast strains lacking deubiquitylating enzymes.Conclusions: We exploited an in vivo and in silico combinational approach to analyze for the first time in a Systems Biology context the events of PCNA ubiquitylation occurring in PRR in budding yeast cells. Abstract

The current COVID-19 pandemic represents an acute threat to the health of adults and children across the globe. In addition, it has the potential to worsen the health of future generations through intergenerational health effects. Examples from history, including the Dutch famine (Hongerwinter), suggest that in utero and early life environments may have significant implications for health outcomes throughout the lifespan and are important in determining risk of chronic disease in adulthood. Parental health status, stress, and nutrition appear to affect offspring health and are all affected by the COVID-19 pandemic. Thus, it is critical that we consider the potential impacts of the current pandemic on pregnant women, infants and children and take public health and medical actions to mitigate risk and promote health in future generations.
