Up to 30% of ischemic stroke cases are due to large vessel occlusion (LVO), causing significant morbidity. Studies have shown that the collateral circulation of patients with acute ischemic stroke (AIS) secondary to LVO can predict their clinical and radiological outcomes. The aim of this study is to identify baseline patient characteristics that can help predict the collateral status of these patients for improved triage. In this IRB approved retrospective study, consecutive patients presenting with AIS secondary to anterior circulation LVO were identified between September 2019 and August 2021. The baseline patient characteristics, laboratory values, imaging features and outcomes were collected using a manual chart review. From the 181 consecutive patients initially reviewed, 54 were confirmed with a clinical diagnosis of AIS and anterior circulation LVO. In patients with poor collateral status, the body mass index (BMI) was found to be significantly lower compared to those with good collateral status (26.4 ± 5.6 vs. 31.7 ± 12.3; p = 0.045). BMI of >35 kg/m2 was found to predict the presence of good collateral status. Age was found to be significantly higher (70.5 ± 9.6 vs. 58.9 ± 15.6; p = 0.034) in patients with poor collateral status and M1 strokes associated with older age and BMI.

Background and SignificanceAutoimmune encephalitis (AE) is a rare group of diseases that can present with stroke-like symptoms. Anti-leucine-rich glioma inactivated 1 (LGI1) encephalitis is an AE subtype that is infrequently associated with neoplasms and highly responsive to prompt immunotherapy treatment. Therefore, accurate diagnosis of LGI1 AE is essential in timely patient management. Neuroimaging plays a critical role in evaluating stroke and stroke mimics such as AE. Arterial Spin Labeling (ASL) is an MRI perfusion modality that measures cerebral blood flow (CBF) and is increasingly used in everyday clinical practice for stroke and stroke mimic assessment as a non-contrast sequence. Our goal in this preliminary study is to demonstrate the added value of ASL in detecting LGI1 AE for prompt diagnosis and treatment.MethodsIn this retrospective single center study, we identified six patients with seropositive LGI1 AE who underwent baseline MRI with single delay 3D pseudocontinuous ASL (pCASL), including five males and one female between ages 28 and 76 years, with mean age of 55 years. Two neuroradiologists qualitatively interpreted the ASL images by visual inspection of CBF using a two-point scale (increased, decreased) when compared to both the ipsilateral and contralateral unaffected temporal and non-temporal cortex. The primary measures on baseline ASL evaluation were a) presence of ASL signal abnormality, b) if present, signal characterization based on the two-point scale, c) territorial vascular distribution, d) localization, and e) laterality. Quantitative assessment was also performed on postprocessed pCASL cerebral blood flow (CBF) maps. The obtained CBF values were then compared between the affected temporal cortex and each of the unaffected ipsilateral parietal, contralateral temporal, and contralateral parietal cortices.ResultsOn consensus qualitative assessment, all six patients demonstrated ASL hyperperfusion and corresponding FLAIR hyperintensity in the hippocampus and/or amygdala in a non-territorial distribution (6/6, 100%). The ASL hyperperfusion was found in the right hippocampus or amygdala in 5/6 (83%) of cases. Four of the six patients underwent initial follow-up imaging where all four showed resolution of the initial ASL hyperperfusion. In the same study on structural imaging, all four patients were also diagnosed with mesial temporal sclerosis (MTS). Quantitative assessment was separately performed and demonstrated markedly increased CBF values in the affected temporal cortex (mean, 111.2 ml/min/100 g) compared to the unaffected ipsilateral parietal cortex (mean, 49 ml/min/100 g), contralateral temporal cortex (mean, 58.2 ml/min/100 g), and contralateral parietal cortex (mean, 52.2 ml/min/100 g).DiscussionIn this preliminary study of six patients, we demonstrate an ASL hyperperfusion pattern, with a possible predilection for the right mesial temporal lobe on both qualitative and quantitative assessments in patients with seropositive LGI1. Larger scale studies are necessary to further characterize the strength of these associations.

The preoccupation with architecture, geography, and borders in the work of Anglo-Sudanese writers Leila Aboulela and Jamal Mahjoub is to a large extent tied to the postcolonial mindset the two authors share and the minority status of the Anglophone Arab literary tradition. This tradition aims, among other things, at rewriting space to negotiate questions of identity, power, and resistance. Drawing on recent research on the intersections between the postcolonial field and the field of space studies, this paper argues that, although Aboulela and Mahjoub both seek to expose the spatial organization of social reality, that is to say the ways in which space is both conceived and shaped to reinforce existing power differentials, they diverge on the esthetic and political strategies to challenge this power configuration. Therefore, by comparing Aboulela’s Minaret (2006) and Mahjoub’s A Line in the River: Khartoum, City of Memory (2018), it will be argued that, while Aboulela displaces the larger geographies of the nation and the city in favor of urban microstructures that become the site of dissent and empowerment for the alienated migrant subject, Mahjoub embraces the geography of the nation as holding the key both to the collective project of nation-building and the more personal task of coming to terms with the plurality of postcolonial identity. Despite its richness and diversity, Anglo-Arab fiction is often haunted by the themes of loss and displacements that are often associated with the experience of exile. This preoccupation with loss resonates with recent postcolonial re-appropriations of the Freudian theory of melancholia, that are aimed at uncovering the theory’s potential for explaining the phenomenon of unresolved integration at the heart of several diasporic narratives of migration. One such revisionary model, Sara Ahmed’s figure of the melancholic migrant, provides a useful interpretive framework with which to explain the elegiac mood that pervades a number of Anglo-Arab novels. This paper focuses on two key works from this tradition, Fadia Faqir’s My Name is Salma and Leila Aboulela’s Minaret, to argue that these texts employ the archetypal figure of the melancholic migrant both as a psychic response to loss and a strategy of resistance and empowerment. Coronary artery disease (CAD) is one of the chief causes of death in the world. Several hypotheses have been promoted as for the origin of the disease, among which are genetic predispositions and/or environmental factors. The aim of this study was to determine the effect of factor V (FV) gene polymorphisms (Leiden, G1691A [FVL] and HR2 A4070G) and to analyze their association with traditional risk factors in assessing the risk of CAD. Our study population included 200 Tunisian patients with symptomatic CAD and a control group of 300 participants matched for age and sex. All participants were genotyped for the FVL and HR2 polymorphisms. Multivariate logistic regression was applied to analyze independent factors associated with the risk of CAD. Our analysis showed that the FVL A allele frequency (P < 10-3, odds ratio [OR] = 2.81, 95% confidence interval [CI] = 1.6- 4.9) and GA genotype (P < 10-3, OR = 4.03, 95% CI = 2.1-7.6) are significantly more prevalent among patients with CAD compared to those controls and may be predisposing to CAD. We further found that the FVL mutation is an independent risk factor whose effect is not modified by other factors (smoking, diabetes, hypertension, dyslipidemia, and a family history of CAD) in increasing the risk of the disease. However, analysis of FV HR2 variation does not show any statistically significant association with CAD. The FVL polymorphism may be an independent risk factor for CAD. However, further investigations on these polymorphisms and their possible synergisms with traditional risk factors for CAD could help to ascertain better predictability for CAD susceptibility. Background: Coronary artery disease (CAD), also known as atherosclerotic heart disease, is a leading cause of mortality and morbidity throughout the world. The role of insertion/deletion (I/D) polymorphisms of the angiotensin-converting enzyme (ACE) gene in the etiology of CAD remains to be more completely clarified. The aim of this study was to determine the role of the ACE I/D polymorphism in patients with CAD and to study the association together with traditional risk factors in assessing the risk of CAD. Methods: Our study population included 145 Tunisian patients with symptomatic CAD and a control group of 300 people matched for age and sex. All participants in the study were genotyped for the ACE I/D polymorphisms obtained by polymerase chain reaction amplification on genomic DNA. Results: Our analysis showed that the ACE D allele frequency (P < 10−3; odds ratio [OR] = 5.2; 95% confidence interval [CI] = 3.6-7.6) and DD genotype (P < 10−3; OR = 6.8; 95% CI = 4.4-10) are significantly more prevalent among patients with CAD than in controls and may be predisposing to CAD. We further found that the risk of CAD is greatly potentiated by several concomitant risk factors (smoking, diabetes, hypertension, dyslipidemia, and a family history of CAD). Conclusion: The ACE D allele may be predictive in individuals who may be at risk of developing CAD. Further investigations of these polymorphisms and their possible synergisms with traditional risk factors for CAD could help to ascertain better predictability for CAD susceptibility. Research Problem: Hypertension is a multifactorial disease that affects approximately one third of the Tunisian adult population. It is a major risk factor for stroke and cardiovascular disease. Environmental and psychosocial factors play an important role in hypertension onset and control. The prevalence of depression among hypertensive patients is 26, 8% and its presence is associated with increased risk of cardiovascular related morbi-mortality. Our study aims to evaluate the role of depression in blood pressure control among ambulatory hypertensive patients. Investigative process: This study is a cross-sectional, multicentric and descriptive study. We intend to include three hundred and two patients. A 24-hour ambulatory blood pressure monitor will be used to evaluate blood pressure control. Depression will be assessed by the 9-item Patient Health Questionnaire (PHQ-9) in Tunisian dialect. Clinical, socio-environmental, psychosocial and therapeutic and prognosis data will be collected from medical records. Patients will be classified into two groups: Controlled versus non-controlled hypertension. PHQ-9 scores will be then compared between the two cohorts., Research Plan: Ethical considerations will be undertaken and respected. Introduction: The conventional radial approach (CRA), the gold standard approach for percutaneous coronary interventions (PCI), is associated with the risk of radial artery occlusion (RAO). The distal radial approach (DRA) is an effective alternative with fewer complications., Aim: To evaluate the efficacy in terms of puncture success and safety by RAO rate of the DRA in elective PCI in Tunisian patients., Methods: It was a randomized controlled non-inferiority trial including patients hospitalized for elective PCI. The protocol was previously published (Tunis Med 2022; 100(3): 192-202). The primary endpoints were puncture success and RAO rate at 30 days., Results: Overall, 250 patients were included and the groups were comparable. The preprocedural examination of the radial pulse and the Barbeau test were similar. The majority of PCIs were coronary angiography (82%). In ITT, respectively in CRA versus DRA, puncture success rates were similar (97.6% versus 96.8%; p≤0.500). RAO rates were similar (2.4% versus 3.2%; p≤0.500). Crossovers were similar. PCI through DRA lasted longer but was not more irradiating, however it required more contrast. Overall bleeding and vascular complications were similar., Conclusion: This study demonstrated the non-inferiority of DRA compared to CRA for elective PCIs in a Tunisian population regarding puncture success and RAO rate at 30 days. Multicenter trials including urgent PCI with systematic ultrasound screening for RAO are needed.

Introduction: Simulation-based education (SE) in interventional cardiology improves knowledge acquisition and mastery of procedures including the conventional radial access (CRA)., Aim: To evaluate the contribution of SE in CRA compared with theoretical learning alone., Methods: This is a prospective randomized controlled study including cardiology residents and patients with normal radial pulses. Experienced residents as well as patients with contraindications to CRA, requiring urgent intervention, or with hemodynamic instability were not included. Missing the teaching sessions was the exclusion criteria. Residents were randomized into two groups: simulation versus control. They attended a theory lecture explaining CRA and were evaluated Only the simulation group attended an SE session with measurement of heart rate (HR) and stress level. A real application was then performed with measurement of HR and stress level. The primary outcome was success puncture rate., Results: The success of the puncture was similar between the two groups (p=0.651). In the practical application, the stress level was significantly different before the procedure but similar after the end of the procedure. The stress level varied significantly within the same group before and after the procedure The simulation group was significantly less tachycardic both before and during the procedure with a significant difference between the two groups (p <10-3)., Conclusion: This study demonstrated the interest of SE in novice residents for CRA as a complement to theoretical learning upstream of the real-life procedure.

Research Problem: Drug-eluting balloon (DEB) angioplasty is a well-established treatment modality for in-stent restenosis, however its safety and efficacy in de-novo lesion especially in large vessel remains undetermined. Theoretically, DEB sight to eliminate stent thrombosis and reduce restenosis rates by leaving no metal behind., Aim: To compare the results of angioplasty of de novo lesions by DEB (SEQUENT PLEASE) versus DES (Promus Premier and Promus Elite) in a Tunisian population. THE ENDPOINTS will be primarily the Late Lumen Loss at 12 months and secondarily the Major Cardiovascular Event rate (MACE) at 12 months., Investigative Process: This is a randomized controlled non-inferiority trial including 290 patients with chronic coronary disease or non-ST elevation myocardial infarction with de novo lesions. After coronarography, angiographic parameters concerning lesion location and quantitative analysis will be collected. Patients will be treated with DEB or DES according to their allocation group. Before removal of the guide, post-procedural angiographic parameters will be evaluated. Follow-up will be performed for 12 months and an angiographic examination will be performed either as an emergency or at 12 months. The significance level will be 5%. Research Problem: The conventional radial approach is the recommended vascular access for percutaneous coronary interventions. It is effective and feasible but associated with a risk of occlusion of the radial artery. The distal radial approach is proposed as a new approach to reduce complications and preserve the radial artery. However, few clinical trials in real life were conducted in North African patients. This trial aims to evaluate the efficacy and safety of the distal radial approach versus the conventional radial approach. Investigative process : This trial is a non-inferiority, randomized controlled trial with two parallel arms: distal radial approach and conventional radial approach. Two hundred fifty patients scheduled for percutaneous coronary intervention will be included. The two main endpoints are the puncture success rate with a non-inferiority margin of 10% and the occlusion rate of the punctured radial artery attributed to the end of hemostasis and to 30 days. Secondary Endpoints : catheterization success, crossover rate, procedure time, radial artery spasm, bleeding complications, QuickDASH pain score, Operator satisfaction. A single blind analysis will be led according to the per-protocol and intention-to-treat methods., Research Plan: Ethical considerations will be undertaken and respected.
