Background and Aims: Functional dyspepsia, gastroesophageal reflux disease and peptic ulcer are common chronic diseases in the community. Different symptoms may sometimes be observed within these different groups of patients and this kind of overlap may create difficulties in the management and follow-up of these individuals. We aimed to determine the presence, frequency and severity of upper gastrointestinal system complaints and the range of overlapping symptoms among women who applied to primary care centers with complaints of the upper gastrointestinal system. Materials and Methods: All women who applied to the mobile primary care polyclinic in the areas of Gaziosmanpaşa and Kartal on the same day with upper gastrointestinal complaints were included in the study. Patients were questioned regarding the presence, frequency and severity of upper gastrointestinal system complaints by the researcher using 45 close-ended questions. The research was descriptive and the results were analyzed by SPSS 8.0 version. Results: There were 346 women with an average age of 43.3±12.5. The most common symptom was regurgitation followed by heartburn, retrosternal pain and epigastric fullness. 88.5% of all cases had at least two of the reflux symptoms (heartburn, regurgitation, retrosternal pain or dysphagia). Among the patients with at least two of the reflux symptoms, 47.5% and 59% of them also had epigastric pain and fullness, respectively. There was a correlation between epigastric pain and reflux symptoms other than regurgitation, and also between epigastric fullness and reflux symptoms other than heartburn. Symptoms severe enough to require drug usage were related with epigastric symptoms. Conclusions: Gastroesophageal reflux disease is recognized as the most prevalent upper gastrointestinal system complaint of women applying to first-step health care polyclinics. However, half of the women's complaints overlap with ulcer-like or dysmotility- like dyspepsia. In order to differentiate symptom groups, more objective tools than history taking are needed., Giriş ve Amaç: Gastroözofageal reflü, fonksiyonel dispepsi, ve peptik ülser sık karşılaşılan kronik hastalıklardır. Bu hastalıklara ait yakınmaları n benzemesi ve birlikte bulunması en uygun tanı ve tedavi yaklaşımını belirlemekte zorluklara neden olabilir. Bu çalışmanın amacı birinci basamak sağlık kuruluşlarına üst gastrointestinal sistem şikayeti ile başvuran kadınlarda reflü semptomlarının sıklığı, ağırlığı ve türünü saptamak ve hastaların ne kadarında eş zamanlı gastroduodenal hastalık semptomu olduğunu belirlemektir. Yöntem: İstanbul'un Gaziosmanpaşa ve Kartal semtlerinde gezici sağlık taraması sırasında gastrointestinal yakınmalar nedeniyle bir günde başvuran bütün kadınlar çalışmaya alındı. Üst gastrointestinal sistem şikayetlerinin varlığı, sıklığı ve ağırlığı 45 soruluk bir anketle sorgulandı. Veriler SPSS programı ile değerlendirildi. Bulgular: Yaşları 21 ile 87 arasında değişen (ortalama 43.3±12.5) 346 kadın sorgulandı. En sık rastlanan şikayet acı regurjitasyonu idi. Bunu retrosternal yanma, retrosternal ağrı ve epigastrik şişkinlik izledi. Olguların %88.5'inde en az iki reflü semptomu (restrosternal ağrı, yanma, acı regurjitasyonu veya yutma güçlüğünden herhangi ikisi) vardı. En az iki reflü semptomu olanların %47.5'inde epigastrik ağrı, %59'unda epigastrik şişkinlik vardı. Epigastrik ağrı; retrosternal yanma, retrosternal ağrı, yutma güçlüğü gibi acı regurjitasyonu dışındaki reflü semptomları ile ilişkili bulundu. Epigastrik şişkinlik ise acı regurjitasyonu, retrosternal ağrı, yutma güçlüğü gibi retrosternal yanma dışında kalan reflü semptomları ile ilişkili bulundu. Semptomları ilaç kullanacak kadar şiddetli olanlarda epigastrik yakınmalar daha sık görüldü. Sonuç: Birinci basamak sağlık kuruluşlarında üst gastrointestinal sistem şikayetleri ile başvuran kadınlarda en sık rastlanan semptomlar reflüye aittir. Ancak olguları n yarısında semptomlar ülser ya da fonksiyonel dispepsi semptomları ile örtüşür. Hastalık gruplarını ayırmak için anamnezden daha objektif araçlara ihtiyaç vardır.

AMAÇ:Yurdumuzda yaygın olarak yetiştirilen, 'Corylus avellana' cinsinden olan fındık; E vitamini, niasin,kalsiyum (Ca), magnezyum (Mg) ve potasyum (K) gibi mineralleri içeren önemli bir besin olup son yıllardafındığın kalp damar hastalıkları açısından olumlu etkileri çeşitli çalışmalarda gösterilmiştir. Amacımızliteratürde sık fındık tüketimi olarak belirtilen miktardaki fındığın 7 gün boyunca tüketiminin serum sodyum(Na), klor (Cl), demir (Fe), K ve Ca düzeylerine ve lökosit, eritrosit, hemoglobin, hematokrit, fibrinojen gibihematolojik parametreler üzerine etkisini incelemekti.GEREÇ ve YÖNTEMLER:25 sağlıklı bireyin gönüllü olarak 7 gün boyunca günde ortalama 20 g fındıkyemesi sağlandı. Diyet kısıtlaması yapılmadı. Fındık tüketme alışkanlığı olan ve olmayan kişiler ayrı ayrıdeğerlerlendirildi. Fındık tüketimi öncesi ve sonrasında sabah açlık kan örnekleri alındı. Serum parametreleri747 Hitachi otoanalizöründe Roche kitleri, hematolojik testler ise BCS tam otomatik koagulometre ve DadeBehring kitleri kullanılarak tayin edildi.BULGULAR:Fındık tüketme alışkanlığı olan ve olmayan kişilerde, 7 günlük düzenli fındık tüketiminden sonraserum Na, Cl, Ca değerleri anlamlı derecede azalırken, total demir bağlama kapasitesi (TDBK) anlamlı derecedearttı. Yedi günlük düzenli fındık tüketimi sonucu, fındık tüketme alışkanlığı olmayanlarda hemoglobin vehematokrit değerleri azalırken, fındık tüketme alışkanlığı olanlarda fibrinojen değerlerinde anlamlı artışgötürüldü.Ancak bütün bu değişimler normal sınırlar içindeydi.SONUÇ:Fındığın, kan lipidleri dışında, hemostatik sistem ve mineral dengesi üzerine etkilerini inceleyen uzundönemli çalışmalar, ülkemizde yaygın olarak yetişen ve tüketilen bu besin maddesinin etkilerinin açığa çıkmasınısağlayacaktır, OBJECTIVE: Hazelnut (Corylus avellana) widely raised and used in Turkey, is an important source of vitamin E, niacin, calcium (Ca), magnesium (Mg) and potassium (K). Recently, beneficial effects of hazelnut consumption in the prevention of cardiovascular diseases have been shown in several studies. Our aim was to investigate the effects of frequent hazelnut consumption on serum sodium (Na), K, chloride (Cl), iron (Fe) and Ca levels and hematological parameters such as leucocyte, eritrocyte, hemoglobin, hematocrit and fibrinogen levels. MATERIAL and METHODS: 25 volunteer healthy subjects were advised to eat 20 g/day hazelnut for 7 days without diet restriction. Subjects that consume nut habitually and non-consumers were evaluated separately. Fasting blood samples prior and after hazelnut consumption were taken. Blood parameters were analyzed using Roche kits in 747 Hitachi autoanalyzer, and hematological analysis were determined using Dade-Behring kits in BCSautomatic coagulometer. RESULTS: Hazelnut consumption, decreased serum Na, Cl, Ca and increased total iron binding capacity in hazelnut consumers and non-consumers. At the end of 7 days, hemoglobin and hematocrit levels decreased in non-consumers and fibrinogen levels were increased in hazelnut consumers. However all these changes were in normal limits. CONCLUSION: Studies on the effects of hazelnut consumption on hematological parameters and mineral balance may enlighten the effects this nutrient widely raised and consumed inTurkey.

Med., 2007, 213 (3), 241-248 - Accumulation of oxidized proteins and impaired antioxidant system have been shown to be associated with arthritis. Serum sialic acid (SA) is known as a parameter of inflammation. In the present study, to explore the potential role of SA in arthritis, we measured serum SA levels, plasma protein oxidation, and antioxidant status in patients with primary osteoarthritis (POA) and inactive rheumatoid arthritis (RA). Inactive RA (iRA) was defined upon the American College of Rheumatology criteria for clinical remission of RA. A total of 40 patients (20 POA patients, including 4 male subjects, and 20 iRA female patients) and 20 healthy female subjects were included in this study. SA, antioxidants, and protein oxidation levels were determined spectrophotometrically in serum or plasma samples. Serum SA levels were significantly increased in POA (3.34 +/- 0.37 mM, p < 0.0001) and 1RA (3.11 +/- 0.47 mM, p < 0.05), compared with healthy controls (2.41 +/- 0.16 mM). Plasma total antioxidant activity, plasma superoxide dismutase activity and serum reduced glutathione levels were significantly decreased in patients with POA and those with iRA, whereas plasma carbonyl content and serum total protein were increased in those patients. Moreover, plasma total thiol levels were significantly increased in iRA and decreased in POA. Thus, increased SA and protein oxidation levels are associated with the decreased antioxidant levels in POA and iRA patients. These results suggest that SA may be considered as a potent defense molecule against oxidative damage in arthritis.
