- Abstract
Adaptor chimeric antigen receptor (CAR) T-cell therapy offers solutions for improved safety and antigen escape which represent main obstacles for the clinical translation of CAR T-cell therapy in myeloid malignancies. The adaptor CAR T-cell platform “UniCAR” is currently under early clinical investigation. Recently, first proof-of-concept of a well-tolerated, rapidly switchable, CD123-directed UniCAR T-cell product treating patients with acute myeloid leukaemia (AML) was reported. Relapsed and refractory AML is prone to a high plasticity under therapy pressure targeting one single tumour antigen. Thus, targeting of multiple tumour antigens seems to be required to achieve durable anti-tumour responses, underlining the need to further design alternative AML-specific target modules (TM) for the UniCAR platform. We here present the preclinical development of a novel FMS-like tyrosine kinase 3 (FLT3)-directed UniCAR T-cell therapy, which is highly effective for in vitro killing of both AML cell lines and primary AML samples. Furthermore, we show in vivo functionality in a murine xenograft model. PET analyses further demonstrate a short serum half-life of FLT3 TMs, which will enable a rapid on/off-switch of UniCAR T-cells. Overall, the presented preclinical data encourage the further development and clinical translation of FLT3-specific UniCAR T-cells for the therapy of AML.

- Abstract
Precision medicine can significantly improve outcomes for patients with cancer, but implementation requires comprehensive characterization of tumor cells to identify therapeutically exploitable vulnerabilities. Here, we describe somatic biallelic TET2 mutations in an elderly patient with acute myeloid leukemia (AML) that was chemoresistant to anthracycline and cytarabine but acutely sensitive to 5 '-azacitidine (5 '-Aza) hypomethylating monotherapy, resulting in long-term morphological remission. Given the role of TET2 as a regulator of genomic methylation, we hypothesized that mutant TET2 allele dosage affects response to 5 '-Aza. Using an isogenic cell model system and an orthotopic mouse xenograft, we demonstrate that biallelic TET2 mutations confer sensitivity to 5 '-Aza compared with cells with monoallelic mutations. Our data argue in favor of using hypomethylating agents for chemoresistant disease or as first-line therapy in patients with biallelic TET2-mutated AML and demonstrate the importance of considering mutant allele dosage in the implementation of precision medicine for patients with cancer.

- Abstract
The major cause of death after allogeneic Hematopoietic Stem Cell Transplantation (HSCT) for acute myeloid leukemia (AML) is disease relapse. We investigated the expression of Inhibitory Receptors (IR; PD-1/CTLA-4/TIM-3/LAG-3/2B4/KLRG1/GITR) on T cells infiltrating the bone marrow (BM) of 32 AML patients relapsing (median 251 days) or maintaining complete remission (CR; median 1 year) after HSCT. A higher proportion of early-differentiated Memory Stem (T SCM ) and Central Memory BM-T cells express multiple IR in relapsing patients than in CR patients. Exhausted BM-T cells at relapse display a restricted TCR repertoire, impaired effector functions and leukemia-reactive specificities. In 57 patients, early detection of severely exhausted (PD-1 + Eomes + T-bet − ) BM-T SCM predicts relapse. Accordingly, leukemia-specific T cells in patients prone to relapse display exhaustion markers, absent in patients maintaining long-term CR. These results highlight a wide, though reversible, immunological dysfunction in the BM of AML patients relapsing after HSCT and suggest new therapeutic opportunities for the disease.

Acute myeloid leukemia (AML) is a hematological malignancy with an undefined heritable risk. Here we perform a meta-analysis of three genome-wide association studies, with replication in a fourth study, incorporating a total of 4018 AML cases and 10488 controls. We identify a genome-wide significant risk locus for AML at 11q13.2 (rs4930561; P = 2.15 × 10 -8 ; KMT5B). We also identify a genome-wide significant risk locus for the cytogenetically normal AML sub-group (N = 1287) at 6p21.32 (rs3916765; P = 1.51 × 10 -10 ; HLA). Our results inform on AML etiology and identify putative functional genes operating in histone methylation (KMT5B) and immune function (HLA).

Background: In the past years, the treatment of acute myeloid leukemia (AML) has been significantly shifted towards the development of targeted approaches. Nonetheless, clinical translation of novel immunotherapeutic strategies such as chimeric antigen receptor (CAR) T-cells in AML is still at an early stage. Given the heterogeneity of such disease, major challenges include immune escape and disease relapse, which demand for further improvements in the CAR design. To overcome such hurdles, we have invented the switchable, flexible and programmable adaptor RevCAR platform. This consists of T-cells engineered with RevCARs that are primarily inactive as they express an extracellular short peptide epitope incapable of recognizing surface antigens. RevCAR T-cells can be redirected to tumor antigens and controlled by bispecific antibodies cross-linking RevCAR T- and tumor cells resulting in tumor lysis. Remarkably, the RevCAR platform enables combinatorial tumor targeting following Boolean logic gates in which two separate RevCARs with different specificities can be simultaneously expressed and used to accomplish dual gated targeting of prominent AML antigens such as CD33 and CD123. We herein show for the first time the applicability of the RevCAR platform to target myeloid malignancies such as AML. Methods: Binding, functionality and proof-of-concept for combinatorial tumor targeting using the RevCAR system was assessed using both in vitro and in vivo models in different settings. For that, flow cytometry-based, cytokine release and cytotoxicity assays were performed using established AML cell lines or patient-derived material. Results: We have proven that AML cell lines as well as patient-derived AML blasts could be efficiently killed by redirected RevCAR T-cells targeting CD33 and CD123 in a flexible manner. Furthermore, by targeting both antigens, an AND gate logic targeting could be achieved using the RevCAR platform. This is a particular important approach to overcome existing or treatment related tumor escape variants and to tackle AML cancer heterogeneity. Conclusions: These accomplishments validate the preclinical versatility and controllability of the RevCAR platform embedded in one single system thereby paving the way for an improved and personalized immunotherapy of AML patients.

Background Induction chemotherapy is currently the standard of care for treatment of acute myeloid leukemia (AML) with 5-year disease-free survival of 33%. Given the large proportion of non-responders and relapsed patients, novel adjuvant drugs are urgently needed. Especially, targeted therapies including small molecules and T cell based immunotherapies are under intensive preclinical and clinical investigation. The tyrosine kinase inhibitor Midostaurin recently received approval for treatment of FLT3-positive AML. In addition to chemotherapy, it significantly deepens remission rates and improves overall survival of patients. In light of future combinatorial approaches, simultaneous application of different targeted therapies should theoretically augment anti-tumor effects. Aims Therefore, we questioned whether Midostaurin could strengthen cytotoxic effector mechanisms of redirected switchable UniCAR T cells or bispecific antibody-redirected T cells against primary AML cells. Methods/Results By performing in vitro co-cultivation assays with patient-derived AML cells, it was shown that Midostaurin concentrations ≥ 1 µM significantly impair the activation, proliferation, cytokine production and cytotoxicity of autologous and allogeneic T cells after engagement via bsAb or the UniCAR system. Data could be also verified in a solid tumor model. Summary/Conclusion At concentrations ranging between 0.1 and 10 M, it was shown that Midostaurin and its metabolites are indeed able to inhibit several components of the TCR signaling pathway including LcK, Zeta-chain-associated protein kinase 70 (ZAP-70), mitogen-activated protein kinase (MAPK) and Protein kinase C (PKC) in vitro. Therefore, we argue that the observed T cell inhibition by Midostaurin in our studies is caused by the inhibition of several of these kinases. This hypothesis is supported by the work of two individual research groups that were able to show synergistic effects by combining FLT3 selective TKIs with different T cell-based immunotherapies. Because Midostaurin through concentrations above ≥ 1 µM have been observed in earlier performed dose finding studies, we speculate that current standard Midostaurin therapy will inhibit T cell function in vivo. In summary, our data underline that combination of Midostaurin and T cell-based immunotherapies in FLT3-positive AML patients is not recommended due to the suppressive effect of Midostaurin on T cells. Therefore, more selective TKI or other small molecules should be chosen to avoid impairment of T cell functions.

- Abstract
Combinatory therapeutic approaches of different targeted therapies in acute myeloid leukemia (AML) are currently under preclinical and early clinical investigation. To enhance anti-tumor effects, we combined tyrosine kinase inhibitor (TKI) Midostaurin and T cell-mediated immunotherapy directed against CD33. We show that clinically relevant concentrations of Midostaurin abrogate T cell-mediated cytotoxicity both after activation with bispecific antibodies (bsAbs) and chimeric antigen receptor (CAR) T cells. This information is of relevance for clinicians exploring T cell-mediated immunotherapy in early clinical trials. Given the profound inhibition of T cell functionality and anti-tumor activity, we recommend favoring specific fms-like tyrosine kinase 3 (FLT3) TKIs for further clinical testing of combinatory approaches with T cell-based immunotherapy.

Following the completion in May 2011 of the shutdown for the installation of the beryllium wall and the tungsten divertor, the first set of JET campaigns have addressed the investigation of the retention properties and the development of operational scenarios with the new plasma-facing materials. The large reduction in the carbon content (more than a factor ten) led to a much lower Zeff (1.2-1.4) during L- and H-mode plasmas, and radiation during the burn-through phase of the plasma initiation with the consequence that breakdown failures are almost absent. Gas balance experiments have shown that the fuel retention rate with the new wall is substantially reduced with respect to the C wall. The re-establishment of the baseline H-mode and hybrid scenarios compatible with the new wall has required an optimization of the control of metallic impurity sources and heat loads. Stable type-I ELMy H-mode regimes with H98,y2 close to 1 and βN ∼ 1.6 have been achieved using gas injection. ELM frequency is a key factor for the control of the metallic impurity accumulation. Pedestal temperatures tend to be lower with the new wall, leading to reduced confinement, but nitrogen seeding restores high pedestal temperatures and confinement. Compared with the carbon wall, major disruptions with the new wall show a lower radiated power and a slower current quench. The higher heat loads on Be wall plasma-facing components due to lower radiation made the routine use of massive gas injection for disruption mitigation essential. © 2013 IAEA, Vienna.
