Neuroendocrine carcinomas (NECs) of the gallbladder are very rare and aggressive tumors with poor prognosis. Most of them are poorly differentiated and belong to the small cell type. We report a case of a 59-year-old woman who presented with abdominal pain and distension. Contrast-enhanced computed tomography revealed a large heterogeneous mass in the liver, adjacent to the gallbladder, and omental nodules. CA 19-9 level was elevated and ascitic fluid cytology was suspicious for malignancy. Percutaneous biopsy of the liver mass confirmed the diagnosis of small cell NEC of the gallbladder. The patient was considered inoperable and planned for chemotherapy, but she died 20 days after admission. This case illustrates the diagnostic challenges and the dismal outcome of small cell NEC of the gallbladder. Early detection and multimodal treatment are essential for improving the survival of these patients. [ABSTRACT FROM AUTHOR]

Acute appendicitis is the most common ethiology of the acute abdomen syndrome. Diagnose may be difficult for atipical cases. Some imaging methots may be helpful to diagnose for these type of cases. We aimed to evaluate the use of ultrasound for 287 acute appendicitis cases in our hospital emergency service retrospectively in this study., Akut apandisit tüm dünyada en sık karşılaşılan ve cerrahi müdahale gerektiren akut batın sendromu etyolojisidir. Fakat akut apandisit tanısı her zaman kolaylıkla koyulabilen bir tanı değildir. Fizik muayene ve laboratuar tetkikleri ile tanı koyulmakta zorlanılan olgularda görüntüleme yöntemlerinden faydalanılmaktadır. Bu çalışmada acil servise başvuran ve akut apandisit ön tanısı ile ameliyata alınan 287 olgunun kayıtları retrospektif olarak incelendi ve akut apandisit tanısı koymakta ultrasonografinin etkinliği araştırıldı.

Primary intestinal lymphomas (IL) are rare clinical entities despite the gastrointestinal tract is the most common localization for primary extra nodal lymphomas. Generally IL presents with non-specific and chronic gastrointestinal symptoms. We aimed to present a perforated small intestinal lymphoma case who admitted emergency service with the complaint of sudden onset abdominal pain., Gastrointestinal sistem primer ekstranodal lenfomaların en sık görüldüğü yer olmasına rağmen primer intestinal lenfomalar nadir karşılaşılan klinik durumlardır. İntestinal lenfomalar genellikle non-spesifik ve kronik abdominal şikayetlerle ortaya çıkarlar. Bu yazıda acil servise ani başlayan şiddetli karın ağrısı şikâyeti ile başvuran perfore olmuş ince barsak lenfoma olgusunu sunmayı amaçladık.

Objective: Granulomatous mastitis is a rare, benign, chronic inflammatory disease of the breast of unknown etiology. This study evaluated bacteriologic agents that might play a role in the etiology of granulomatous mastitis using a molecular method with a universal primer after isolating deoxyribonucleic acid (DNA) from pathology specimens from patients diagnosed with granulomatous mastitis. Materials and Methods: Breast biopsy material in the pathology department obtained between July 2008 and June 2013 was analyzed. The history of the granulomatous mastitis patients was examined in detail and paraffin block sections of the biopsy material were used to determine the presence of bacteria with a universal DNA primer. Results: This study examined 45 granulomatous mastitis patients who had been diagnosed using excisional, incisional, or core biopsies. We evaluated multiple bacterial taxa, but obtained no positive result using a nucleic-acid-based assay with a universal primer. Conclusion: The etiology of idiopathic granulomatous mastitis remains unclear. Further studies with a large number of patients should aim to identify the causative agent. [ABSTRACT FROM AUTHOR]

Minimally invasive esophagectomy is an increasing trend in surgery. Thoracoscopic esophagectomy is applicable and an alternative procedure to conventional esophagectomy in patients especially with end-stage benign diseases like caustic stricture. A 33-year-old female patient was admitted to the department of general surgery with dysphagia. The patient was suffering from caustic stricture due to ingestion of hydrochloric acid. A totally thoracoscopic and laparoscopic vagalsparing esophagectomy and colonic interposition was performed. Amaç: Akut apandisit, günlük uygulamada en sık karşılaşılan cerrahi hastalıkların başında gelir. Konvansiyonel laparoskopiye göre daha minimal invazif bir yöntem olan tek insizyonlu laparoskopik cerrahi, son zamanlarda yaygınlaşmaya başlamıştır. Bu çalışmada tek insizyonlu laparoskopik apandektomide edindiğimiz tecrübeyi paylaştık. Gereç ve Yöntem: Nisan 2010 – Nisan 2012 tarihleri arasında kliniğimizde, toplam 15 tek insizyonlu laparoskopik (TİL) apandektomi gerçekleştirdik. SILS (Single incision laparoscopic surgery) port (Covidien) direkt görüş altında göbekten batın içine yerleştirildi. Açı verilebilir endograsper, endodisektör ile apandiks ortaya konuldu. Apandektomi materyali SILS port ile birlikte göbekten çıkarıldı. Ameliyat süresi, ağrı skoru, hastanede kalış süresi ve komplikasyonlar kayıt altına alındı. Bulgular: Hastaların 11'sı (% 73) erkek, 4'ü (% 27) kadın idi. 1 hastada (% 6) 3 port laparoskopiye dönüldü. Ortalama yaş 35,25 (28-43)±7,36 yıl ortalama vücut kitle indeksi 30,75 (23-41)±8,34 kg/m2 idi. Anestezi skoru 1,25(1-2)±0,5 idi. Ortalama ameliyat süresi 59,33(40-90)±19,09. dakikaydı. Ameliyat sırasında ve sonrasında komplikasyonla karşılaşılmadı. Tüm hastalar ameliyat sonrası 24.saatte taburcu edildi. Ortalama ağrı skoru 2 (1-3) ±0,81 idi. Sonuç: TİL apandektomi, 3 port laparoskopik apandektomi kadar etkin bir yöntemdir. Laparoskopik cerrahi konusunda tecrübeli ellerde uygulandığında güvenli bir girişimdir. Erken dönem tecrübemizde, mükemmel kozmetik sonuçlar elde ettik, ancak yararlar, ağrı düzeyi ve iyileşme açısından daha geniş serilere ve prospektif çalışmalara ihtiyaç bulunmaktadır., Aim: Acute appendicitis is one of the most commonly encountered surgical problems in everyday practice. Single-incision laparoscopic operations have recently emerged as a less invasive alternative to conventional laparoscopy. The aim of this study was to describe our initial experience with single-incision laparoscopic appendectomy. Patients and methods: Between April 2010 – April 2012, we performed 15 single incision laparoscopic appendectomy in our clinic. SILS port (Covidien) was inserted under direct vision into the peritoneal cavity. Appendix was dissected with articulated endograsper and endodissector. Appendectomy was performed and appendix removed from umbilicus with SILS port. Outcome measures include operative morbidity, operative time, pain score, hospital stay. Results: Eleven of patients (73%) were men, four (27%) were women. Conversion to open surgery were required for one case. The mean age was 35,25 (28- 43)±7,36 years and mean body mass index was 30,75 (23-41)±8,34 kg/m2. Anestesia score was 1,25(1- 2)±0,5 and mean operating time was 59,33(40- 90)±19,09 minutes. No intraoperative or postoperative complications were encountered. Conclusion: Single incision laparoscopic appendectomy is an effective alternative to standard three -incision laparoscopic appendectomy in selected patients. This early experience suggests that outcomes are comparable to standard laparoscopic surgery but with improved cosmesis, however, a larger series is necessary to confirm these findings and to determine if there are any benefits in pain or recovery.

Biz de torakal epidural blok altında axiller diseksiyonlu modifiye radikal mastektomi uygulanan bir hastada epidural yayılım sonucu gelişen bilateral epidural blok olgusunu sunduk. The aim of this study was to determine that very rare anecdotal reports such as intra-neuronal and spinal cord injections, local anesthetic toxicity, and epidural spread of local anesthetic despite it is known that continuous catheter implementation is safer than paravertebral techniques, primary central neuraxial techniques. We presented a bilateral epidural block case developed due to epidural spread in a patient performed modified radical mastectomy with axillary dissection under thoracic epidural block.

Primeri bilinmeyen malign melanom metastazı nadir görülen bir klinik durumdur.Kırkiki yaşında erkek hasta sağ aksiller bölgede hızla büyüyen bir kitle şikayeti ile başvurdu. Fizik muayenede; sağ aksiller bölgeden dışarıya doğru büyümüş, 24-25 cm boyutunda, düzensiz sınırlı ve fuziform şekilli bir kitle saptandı. Kitle eksizyonu ile birlikte level I-II aksiller diseksiyon ameliyatı yapıldı. Histopatolojik ve immünohistokimyasal incelemeler sonucu malign melanom metastazı saptandı. A 42-year old man was admitted with the complaint of a rapidly growing mass in his right axillar region. In physical examination, a mass that 24-25 cm in diameter, irregular border, fusiform shaped and enlarged of outward from the axilla was detected. Mass excision and level I-II axillary dissection was performed. Malignant melanoma metastasis was found as result of histopathologic and immunohistochemical staining examination of this mass. This case has been reported by reason of its rare incidence and interesting clinical view.

AMAÇ: Nadir görülen travma dışı ince barsak delinmelerinin nedenlerini belirlemek ve cerrahi tedavi sonuçlarını sunmak. MATERYAL ve METOD: İki farklı cerrahi merkezinde akut karın sendromu öntanısı ile ameliyat edilen ve ince barsak perforasyonu saptanan hastaların kayıtları retrospektif olarak incelendi. Travma, mezenter vasküler hastalık, herniler, yapışıklıklar ve iatrojenik nedenli olan perforasyonlar inceleme dışı bırakıldı. Kalan 30 hastanın kayıtları demografik özellikler, etyoloji ve tedavi sonuçları açılarından değerlendirildi. SONUÇLAR: Olguları 17’si erkek (%56.6), 13’ü kadın (%43.3) ve yaş ortalaması 50.4 (18-88) idi. Karın ağrısı tüm olgularda olan ve en sık karşılaşılan başvuru şikayetiydi (%100). İlave olarak 9 olguda kusma (%30), 1 olguda kabızlık (%3.3) ve 1 olguda siyah renkli dışkılama (%3.3) şikayeti mevcuttu. On olguda (%33.3) en sık kardiyovasküler sistemle ilgili olmak üzere eşlik eden sistemik hastalık, 9 olguda (%30) geçirilmiş abdominal girişim anamnezi mevcuttu. Yirmibir olguda (%70) lökositoz, 1 olguda (%3.3) lökopeni mevcuttu. Ondokuz olguda (%63.3) bilgisayarlı tomografi, 8 olguda (%26.6) ultrasonografi incelemesi yapılırken, 7 olgu (%23.3) fizik muayene, laboratuar sonuçları ve ayakta direk karın grafisi bulgularına dayanılarak ameliyata alındı. En sık etyolojinin malign hastalıklar olduğu saptandı (7 olgu, %23). Ameliyatta 18 olguya (%60) primer tamir veya rezeksiyon sonrası anastomoz işlemleri uygulanırken, 12 olguya (%40) çeşitli stoma prosedürleri uygulandı. En sık karşılaşılan morbidite yara yeri enfeksiyonuydu (7 olgu, %23.3). Toplam 7 olguda (%23.3) mortalite görülürken bunların sadece 1’i erken post-operatif mortaliteydi. TARTIŞMA: Barsak delinmeleri acil servislerde ve cerrahi servislerinde sık karşılaşılan problemlerdir. Etyolojileri çok farklı ve genellikle bilinen nedenlerdir. Fakat bu resimden en sık karşılaşılan etkenler çıkartılırsa sonra çok farklı bir manzara ile karşılaşılmaktadır

The aim of this study is to evaluate the cases of intesitinal obstruction due to bezoars. The patients’ files who had been operated for intestinal obstruction between March 2004 and March 2010 were evaluated retrospectively. The patients’ records who detected of bezoar as the etiologic factor were reviewed. Two hundred and seventyeight cases that had been operated for intestinal obstruction were inspected. Adhesions were the most common etiology with 121 cases (44%). 24 cases which bezoars were detected as the etiology of obstruction has been included in the study. 19 patients (79%) had abdominal operations history. 13 patients (68%) into this 19 patients had been operated due to peptic ulcus before average 12 (9-25) years ago. Bezoars leading to obstruction were located in terminal ileum in 19 (79%) cases, jejunum in 5 (21%) cases. Bezoars were teared into pieces mechanically and moved forward to the caecum in 16 (67%) cases and removed by enterotomy in 8 (33%) cases. In conclusion; the probability of bezoar formation should be considered in case of intestinal obstruction, although it is a very rare condition. Especially, in patient with intestinal obstruction who had been abdominal operations due to peptic ulcer, history must be questioned carefully. Preoperative orally contrasted abdominal computerized tomography can be helpful to make a diagnosis of bezoar formation in order not to lose time by conservative therapy instead of operation., Bu çalışmada, akut mekanik intestinal obstrüksiyona neden olmuş bezoar olgularımızı değerlendirmeyi amaçladık. Mart 2004-Mart 2010 yılları arasında Sakarya Eğitim ve Araştırma Hastanesi’nde intestinal obstrüksiyon tanısı ile acil şartlarda ameliyat edilen hastaların kayıtları retrospektif olarak incelendi. Obstrüksiyon etyolojisi olarak bezoar saptanan olgular demografik özellikler, başvuru şikayeti, başvuru anındaki fizik muayene bulguları, radyolojik bulgular, geçirilmiş abdominal girişim anamnezi, ameliyat öncesi lökosit değerleri, ameliyata alınma zamanı, ameliyat bulguları, uygulanan ameliyat yöntemleri ve gelişen komplikasyonlar açılarından incelendi. İkiyüzyetmişsekiz olgunun kayıtları incelendi. En sık etyolojinin adezyonlar olduğu saptandı (121 olgu / %44). Obstrüksiyon etyolojisi olarak bezoar saptanan 31 olgudan (%11) kayıtları tam olan ve ulaşılabilen 24’ü değerlendirmeye alındı. Ondokuz olguda (%79) geçirilmiş karın ameliyatı anamnezi mevcuttu. Olguların 13’ünde (%68) geçirilmiş ameliyat peptik ülser ameliyatıydı ve ameliyatın üzerinden ortalama 12 (9-25) yıl geçmişti. 19 olguda (%79) terminal ileumda, 5 olguda (%21) jejunumda obstrüksiyona neden olmuş bezoar saptandı. 16 olguda (%67) bezoar mekanik olarak parçalanarak çekuma ilerletilirken, 8 olguda (%33) enterotomi ile çıkartıldı. Sonuç olarak; nadir bir etyoloji de olsa mekanik intestinal obstrüksiyonlu olgularda bezoar olasılığı mutlaka düşünülmelidir. Özellikle geçirilmiş peptik ülser nedenli abdominal girişim anamnezi olan intestinal obstrüksiyonlu olgularda bu olasılık üzerinde mutlaka ısrarla durulmalıdır. Ameliyat öncesi dönemde yapılacak oral kontrastlı karın bilgisayarlı tomografi (BT) incelemesi bezoar olasılığı düşünülerek incelenirse, bazı olgularda gereksiz konservatif tedavi uygulanarak kaybedilecek tedavi zamanının geri kazanılabileceğini düşünüyoruz.

Idiopathic granulomatous mastitis (IGM) is a rare benign disease of the breast with an unknown etiology, which mimicks the carcinoma of the breast. Its shows an increased incidence among women of the breeding ages usually developing in a single breast as an isolated lesion, clinically revealing dermal findings similar to the infl ammatory carcinoma of the breast. Radiologically, the differential diagnosis between IGM and cancer remains to be a challenge for the physicians. Histopathologic examinations reveal intense inflammation accompanied by non-caseified granulomas constituting the characteristic features of this disease. The presence of other etiologies associated with histopathologic findings of granulomatous reactions must be excluded to achieve the diagnosis of IGM. A thirty fi ve years old female patient admitted to our clinic with complaints of an erythematous tender mass, which she had realized to be present in her right breast for the last fifteen days. Despite the two-week-convenient medical therapy, the clinical fi ndings revealed no improvement. Since the diagnostic procedures could not exclude malignancy, surgery was performed. Following resection, the specimen was evaluated histopathologically revealing the diagnosis of granulomatous mastitis. No evidences of recurrence were met in the postoperative sixth month. This case is presented to emphasize that the possibility of IGM should be kept in mind as an entity for differential diagnosis during the clinical evalutions of women of the breeding ages admitting with complaints of isolated breast masses that bring up the suspicion of breast cancer either clinically, or radiologically. Isolated rectal injury during parturition is a rare clinical entity and it is very important to determine this complication immediately. A 29 year-old pregnant woman was admitted to our hospital to give birth. The dilatation period lasted 9 hours. In the pelvic examination performed after parturition, there was a rectal injuly 3 cm in length including the posterior vaginal wall with intact perineum. Thoracic paravertebral block (TPVB) is an alternative method to general anesthesia because of provides a safe anesthesia with balanced hemodynamic response, allows postoperative pain control by means of catheter and has low side effect profile. TPVB performed safely for the patients undergoing breast cancer surgery with the same reason, has used in too few center instead of general anesthesia. This technique provides an adequate anesthesia for the patients undergoing breast surgery and in addition provides stable hemodynamic status with unilateral somatic and sympathetic blockade, near-perfect control of postoperative pain, minimal nausea and vomiting rate, early discharge and low cost. For this reason, thoracic paravertebral block which is a standard method in breast surgeries for some centers should be known by all anesthesiologists. 47. IDIOPATHIC PARTIAL GASTRIC NECROSIS: A CASE REPORT.

- Abstract
Objective: In the present case report, we emphasise that gastric necroses should be considered among acute abdominal syndromes, regardless of the aetiology. No clinical, radiological, or pathological finding to explain the gastric necrosis can be detected in some cases. Such cases are referred to as idiopathic gastric necrosis. Case: A 62-year-old male patient who had stomach ache, nausea, and vomiting for 7 days was evaluated in the emergency ward. It was ascertained from the histoiy of the patient that he had diabetes, hypertension, chronic obstructive pulmonary disease, and below-the-knee dry gangrene. Because standing direct abdominal radiography revealed free air under the diaphragm, and computed tomography revealed generalised intra-abdominal fluid and air, the patient was explored with a pre-diagnosis of organ perforation. A total gastric resection and Roux-en-Y oesophagojejunostomy were performed on the patient. Discussion: Gastric necrosis is a rare and potentially fatal condition. Several aetiological factors have been identified. Vascular causes may include arterial embolism-thrombosis, and vasculitides and diabetes due to autoimmune diseases resulting in microcirculatory disorders. Diabetes can affect multiple systems. Especially in diabetic patients, the rate for peripheral vascular disease increases in proportion to the duration. The impaired microcirculation in diabetic patients due to microangiopathy reduces the blood supply to tissues. Tissue necrosis, the end-point of damage, is due to the reduced perfusion. Conclusion: In the present case report, we emphasise that gastric necroses should be considered among acute abdominal syndromes, regardless of the aetiology, which can be recovered with early intervention, but which have a mortality of up to 90% when intervention is delayed. Background: Stump appendicitis is inflammation of remnant appendix tissue due to incomplete removal of the appendix. Due to appendectomy history, stump appendicitis diagnosis is usually delay and that can cause increase morbidity. Methods: Medical records of patients who had surgery for acute appendicitis at a single center from 2008 to 2017 were retrospectively reviewed. During the evaluation of medical records, patients that had a previous operation for acute appendicitis or had "stump appendicitis" as an exploratory finding in operation notes were included. Results: Appendectomy was performed in 3130 patients (2630 open surgeries and 380 laparoscopic surgeries). Stump appendicitis was diagnosed in five patients (0.15%). The appendectomies had been performed 4, 5, 7, 7, and 11 years previously. Mean time taken for surgery was 36 h after symptoms began. Open surgery was performed in three patients, laparoscopic procedures in others. Conclusion: Awareness of stump appendicitis before radiological examinations may facilitate accurate diagnosis and decrease the duration of the decision-making process, leading to decreased morbidity. GİRİŞ VE AMAÇ: Barsak arteriyel kan akımının kısmen veya tamamen tıkanması sonrası intestinal iskemi; kan akımının yeniden sağlanması ile reperfüzyon hasarı ortaya çıkmaktadır. İntestinal İskemi Reperfüzyon (İİR) hasarı çoklu organ yetmezliğine ve ölüme neden olabilir. Bu çalışmanın amacı İİR hasarına İloprost (İL) ve N- asetilsistein'in (NAC) etkisini araştırmaktır.GEREÇ VE YÖNTEM: Ağırlıkları 200-300 g arasında değişen 30 adet Sprague Dowley rat her birinde 6'şar denek olacak şekilde 5 gruba ayrıldı. Ketamin ksilazin verilerek supin pozisyonda batın traşı yapılıp povidon iyodür ile boyanan ratlara median insizyon yapılarak ile batına girildi. Sham grubuna herhangi bir cerrahi işlem yapılmadan sadece barsak örnekleri ve kan alındı. İİR grubunda KMA diseke edilip klemp yardımıyla barsak iskemisi yapıldı. Cilt cilt stapleri ile kapatıldı. 45 dakikalık iskemi süresi tamamlanınca batın açıldı. KMA deklempe edilerek ince barsak 120 dakıka reperfuzyona bırakmak üzere cilt cilt stapleri ile kapatıldı. 120 dakika tamamlandıktan sonra batın tekrar açılarak hayvanlar sakrifiye edilip barsak örnekleri ve kan alındı.İİR+ İL grubunda reperfüzyondan yarım saat önce 2mcg/kg İloprost İ.P verilerek aynı işlemler uygulandı. İİR+NAC grubunda N-Asetilsistein reperfüzyondan yarım saat önce 300 mg/kg İ.P olarak verilip aynı işlemler yapıldı.İİR+NAC+İL grubunda ise yine reperfüzyondan yarım saat önce 2mcg/kg İloprost İ.P ve N-Asetilsistein 300 mg/kg İ.P verilerek aynı işlemler uygulandı reperfüzyon tamamlandıktan sonra diğer gruplardaki gibi hayvanlar sakrifiye edilerek barsak ve kan örnekleri alındı.BULGULAR: İİR+NAC grubunun Chiu Skoru ortalaması İİR grubuna ve İİR+İL grubuna göre daha düşük bulundu, ancak bu gruplar arasında istatiksel anlamlı farklılık saptanmadı(p>0.05). Sham grubu skorlarının İİR, İİR+İL, İİR+NAC, İİR+NAC+İL skorlarına göre belirgin düşük ve istatiksel olarak anlamlı olduğu belirlendi. NAC, İL ve ikisinin birlikte verildiği gruplarda Redükte Tiyol, NT/TT oranlarının İİR grubuna göre artmış olarak ve Okside Tiyol, Disülfit/TT ve Disülfit/NT oranlarının İİR gruplarına göre azalmış olarak bulundu (p0,05).SONUÇ: İİR hasarı modelinde İloprost ve N- Asetilsistein'in birlikte kullanımının tek başına N-Asetilsistein kullanımından üstün olmadığı bulunmuştur. Çalışmamızın İİR hasarında Tiyol-Disülfit hemostazı parametrelerinin antioksidan mekanizmayla ilgili belirteç olarak kullanılabileceğini ve sonuçlarımızın yeni çalışmalara katkıda bulunacağını düşünmekteyiz. SUMMARYINTRODUCTION AND PURPOSE: Intestinal ischemia occurs after partial or complete obstruction of the intestinal arterial blood flow and reperfusion injury occurs with the re-supply of blood flow. Intestinal Ischemia Reperfusion (IRR) damage can cause multiple organ failure and death. The aim of this study is to investigate the effect of Iloprost (IL) and N- acetylcysteine (NAC) on IRR damage.MATERIALS AND METHODS: 30 Sprague Dowley rats weighing between 200-300 g were divided into 5 groups, each with 6 subjects. The rat was shaved in a supine position by giving ketamine xylazine, and the rats were stained with povidone iodide and entered into the abdomen by making a median incision. Only intestinal samples and blood were collected from the Sham group without any surgical procedures. In IIR group, KMA was dissected and intestinal ischemia was performed with a clamp. The skin was closed with skin staples. When the 45-minute ischemia time was completed, the abdomen was opened. KMA declamped and the skin was closed with skin staples to allow for 120 minutes of reperfusion. After reperfusion the abdomen was reopened and animals were sacrificed and intestinal samples and blood were collected. In IIR + IL group, 2mcg / kg Iloprost I.P was given half an hour before reperfusion and the same procedures were applied. In IIR + NAC group, N-Acetylcysteine was given as 300 mg / kg I.P half an hour before reperfusion and the same procedures were performed. In IIR + NAC + IL group, the same procedures were applied by giving 2mcg / kg Iloprost I.P and N-Acetylcysteine 300 mg / kg I.P, half an hour before reperfusion.RESULTS: The average of Chiu Score of the IIR + NAC group was lower than the IIR group and the IIR + IL group, but there was no statistically significant difference between these groups(p>0,05). Sham group scores were determined to be significantly lower and statistically significant compared to IIR, IIR + IL, IIR + NAC, IIR + NAC + IL scores(p0,05). Similarly the Oxidized Thiol, Disulfide / TT and Disulfide / NT results of the NAC group were found to be lower than the İİR + İL and İİR + İL + NAC groups, but there was no statistically significant difference between these rates(p>0,05).CONCLUSION: It was found that the use of Iloprost and N-Acetylcysteine in the IIR injury model was not superior to the use of N-Acetylcysteine alone. We think that Thiol-Disulfide hemostasis parameters can be used as an indicator of antioxidant mechanism in IIR damage and our results will contribute to new studies 65

Background: Postmastectomy pain syndrome (PMPS) is defined as a chronic (continuing for 3 or more months) neuropathic pain affecting the axilla, medial arm, breast, and chest wall after breast cancer surgery. The prevalence of PMPS has been reported to range from 20% to 68%. In this study, we aimed to determine the prevalence of PMPS among mastectomy patients, the severity of neuropathic pain in these patients, risk factors that contribute to pain becoming chronic, and the effect of PMPS on life quality. Methods: This cross-sectional study was approved by the Sakarya University, Medical Faculty Ethical Council and included 146 patients ranging in age from 18 to 85 years who visited the pain clinic, general surgery clinic, and oncology clinic and had breast surgery between 2012 and 2014. Patients were divided into two groups according to whether they met PMPS criteria: pain at axilla, arm, shoulder, chest wall, scar tissue, or breast at least 3 months after breast surgery. All patients gave informed consent prior to entry into the study. Patient medical records were collected, and pain and quality of life were evaluated by the visual analog scale (VAS) for pain, a short form of the McGill Pain Questionnaire (SF-MPQ), douleur neuropathique-4 (DN-4), and SF-36. Results: Patient mean age was 55.2 ± 11.8 years (33.0–83.0 years). PMPS prevalence was 36%. Mean scores on the VAS, SF-MPQ, and DN-4 in PMPS patients were 1.76 ± 2.38 (0–10), 1.73 ± 1.54 (0–5), and 1.64 ± 2.31 (0–8), respectively. Of these patients, 31 (23.7%) had neuropathic pain characteristics, and 12 (9.2%) had phantom pain according to the DN-4 survey. Patients who had modified radical mastectomy were significantly more likely to develop PMPS than patients who had breast-protective surgery (P = 0.028). Only 2 (2.4%) of PMPS patients had received proper treatment (anticonvulsants or opioids). Conclusions: PMPS seriously impacts patients’ emotional situation, daily activities, and social relationships and is a major economic burden for health systems. We conclude that the rate of PMPS among patients receiving breast cancer surgery in Turkey is 64.1% and that challenges to the proper treatment of these patients deserve further investigation. © 2015 Chinese Medical Journal.
