Background: Tuberculosis (TB) is long standing disease and SARS-COV-2 being global pandemic in current era. Interactions between SARS-COV-2 and TB needs analysis, as both has major infection-related morbidity and mortality worldwide. This study was conducted to evaluate impact of super imposing viral pandemic over chronic diseases like tuberculosis. Methodology: A Retrospective study done in 75 patients registered under RNTCP program at our centre. Epidemiological data, diagnostic timing of TB, duration of AKT, history of SARS-COV-2 symptoms and SARS-COV-2 diagnostic result were evaluated. Analysis conducted based on incidence of COVID-19 infection in active cases of TB. Results: Male: Female ratio was 1.88:1 with majority belongs to age group of 35 to 55years. Only 3 patients developed influenza like symptoms who tested negative for SARS-COV-2. None COVID-19 suspected or positive patient were reported amongst laboratory confirmed TB patients taking AKT, indicating TB drugs definitely have some impact on SARS-COV-2 virus. Conclusion: TB and COVID-19 co-infection has limited cases. Very few individuals who were exposed to SARS-COV-2 during AKT treatment developed influenza like symptoms, however tested negative for SARS-COV-2. A larger study is needed to understand any role played by TB infection or AKT drugs on covid-19 disease.

Background: Chronic otitis media with ossicular discontinuity is most commonly faced otological condition which causes conductive hearing loss. It presents mostly with ear discharge and decreased hearing. The use of cartilage graft material in ossiculoplasty markedly improve the result in hearing. In this study we have used autologous (tragal/conchal) cartilage or homologous (septal) cartilage for ossiculoplasty and temporalis fascia graft for tympanoplasty.Aims: Aim of the study was to compare and analyze the outcome of ossiculoplasty using autologous conchal/tragal cartilage and homologous septal cartilage in terms of hearing results and graft uptake rates. Materials and method: The study was conducted in E.N.T. Department at S.C.L. Hospital, NHLMMC Ahmedabad. 50 patients were randomly selected and divided in 2 groups. 25 patients underwent homologous and 25 patients underwent autologous cartilage reconstruction. All patients underwent detailed ENT examination followed by audiological and radiological assessment of temporal bone, and those patients with evidence of ossicular erosion were subjected to ossiculoplasty with autologous cartilage and homologous cartilage randomly. Results: Majority of the patients have central perforation and retraction pockets in middle ear pathology in both groups. Overall graft taken up rate was 96% in autologous while 92% in homologous ossiculoplasty. Our study shows hearing improvement up to 20-25 dB in 80% and 88% in autologous and homologous ossiculoplasty. Conclusion: Both autologous and homologous cartilage have excellent outcomes and gives equivalent results irrespective of age or sex of the patient or middle ear pathology. In both the groups hearing improvement is similar. Subjective hearing is also improved. No post-op complications like wound infection, facial palsy, dead ear have occurred in either case.

p-Nitroaniline (PNAN) and nitrobenzene (NB) being important raw materials and intermediates for the production of a wide range of chemical products have the potential of constituting water pollutants. Hence, the development of the means of removing these chemicals from water would go a long way to safeguard the health of the environment. The goal of this research was to transform bagasse fly ash (BFA), solid waste from the sugar industry into porous zeolitic adsorbents MgFZBFA and MgMZBFA and examine them for their adsorptive removal of PNAN and NB from aqueous solutions. The syntheses of the sorbents involve alkali fusion technique and microwave hydrothermal treatment for the desired properties. Advanced characterization techniques such as FTIR (Fourier transform infrared spectroscopy), XRF (X-ray fluorescence spectroscopy), XRD (X-ray diffraction spectroscopy), SEM (scanning electron microscopy), Brunauer–Emmett–Teller (BET)/Barrett–Joyner–Halenda (BJH) Method, and TGA (thermogravimetric analysis) were used for the characterization and evaluation of the sorbents’ properties. The adsorptive removal of PNAN and NB from aqueous solutions by MgFZBFA and MgMZBFA were investigated. Various parameters such as pH, adsorbent dosage, initial sorbate concentration were optimized during the adsorption experiment to achieve best performance. Adsorption isotherm, kinetics and dynamics were studied. It was found that Langmuir adsorption isotherm model better represents the adsorption processes and that the processes follow pseudo-second-order kinetics. More so, the sorption processes were most possibly completed by both surface sorption (liquid-film diffusion) and intra-particle diffusion. The maximum sorption capacities observed with MgFZBFA for PNAN and NB are 30.86 mg g−1and 19.92 mg g−1, while with MgMZBFA the values are 12.72 mg g−1and 10.20 mg g−1, respectively. The performances of MgFZBFA and MgMZBFA for the sorption of PNAN and NB were compared with some adsorbents previously studied for the same purpose, and results show that the adsorbents in the present study exhibit better performances. The application as materials of cheap source for the removal of PNAN and NB from contaminated water could be considered.

Tonsillectomy is one of the most common surgical procedures carried out in ENT since ancient time, is associated with several morbidities in which the pain and bleeding being the commonest and can cause considerable delay in starting oral intake and discharge from the hospital. Different methods have been used to reduce posttonsillectomy pain including use of opioids, sucralfate and local anaesthetics. Local anaesthetics in the form of pre-incisional or post-incisional peritonsillar infiltration and also topical post-incisional spray or packing are some of the most effective methods for post-tonsillectomy pain management. In our hospital, a study was carried out for preincisional peritonsillar infiltration of 0.5% bupivacaine in tonsillectomy patients for post operative pain relief. Written informed valid consent was taken, all routine investigations were done. Pre anaesthetic check up was done and bupivacaine test dose was given, none of the patients showed allergic reactions. Our study showed that this is effective method of controlling post operative pain. Patients receiving bupivacaine showed lower pain scores 6 h post operatively. The mean pain scores for Bupivacaine group were 2.85, 5.52 and 7.04 versus 5.04, 7.04 and 7.61 in saline group at 2, 4, and 6 h post operatively (t value significant). Pre incisional peritonsillar infiltration of 0.5% bupivacaine significantly reduces postoperative pain till 6 h, thereby reducing the need of analgesics intraoperatively and post operatively. Oral intake was also earlier without any adverse effects in our study.

Background: The objective of the study was to identify various lesions of neck swellings and to study their distributions according to age, sex and site and to find out distribution of malignant, inflammatory and infective lesions in neck swellings.Methods: The prospective study was conducted in the department of ENT, SCL General Hospital, NHL MMC, Saraspur, Ahmedabad from July 2014 to July 2016. All indoor and outdoor 400 patients of SCL hospital presenting with palpable head and neck swellings were referred to the cytology section at SCL hospital for FNAC and included in present study. Results: In our study out of 400 cases, lymph nodes constituted 276 cases with female preponderance. Among the inflammatory lesions, commonest lesion was tuberculosis which comprised of 147 cases followed by 64 cases of reactive lymphadenitis. In thyroid lesions the male: female ratio was 1:18, with maximum incidence in age group of 20-50 years. Total 95 cases were observed, out of which benign thyroid lesions (Bethesda grade II) were most common (92.63%). Salivary gland comprised of 11 cases, Out of 10 benign neoplastic lesions, 9 cases of pleomorphic adenoma and 1 cases of Warthin’s tumor were found.Conclusions: FNAC is a very simple and expeditious procedure which can be carried out without much problem. It reduces the necessities to perform excisions biopsy in many cases. Thus, FNAC can be recommended as a first line of investigation in the diagnosis of neck swellings and can be adopted as an outpatient procedure, thus reducing the cost and time of hospitalization.

Background: Thyroid gland is unique among endocrine organs as it is the largest endocrine gland in the body and the first to develop in the fetal life. There is approximately 4-5% incidence of clinically apparent thyroid lesions in general population. The goal of USG and FNAC diagnosis work up now is to select those patients for surgery who have a high likelihood of harboring malignancy in the nodule. Ultrasonography is the single most valuable imaging modality in the evaluation of the thyroid gland. Indications for thyroid USG include evaluation for palpable thyroid lesion or suspected thyroid enlargement and workup of thyroid lesions discovered incidentally.The present study is undertaken to evaluate the utility of FNAC in preoperative diagnosis of various thyroid lesions and to evaluate the efficacy of in USG and FNAC differentiating between benign and malignant lesions.Methods: A retrospective clinical study, 100patients, in the age group of equal to or above 18 years, with thyroid swellings, referred to the department of ENT, sent for USG and FNAC at radiology and pathology department during the period from July 2014 to July 2016. Results: Out of 100 cases, 6% were malignant, 94% were benign on grey scale ultrasound. Out of 6 malignant cases 2 cases were confirmed malignant by FNAC.Conclusions: High resolution grey scale ultrasound has emerged as initial imaging modality of choice for the evaluation of patients with thyroid enlargement. Ultrasound can detect solitary nodule, multiple nodules and diffuse thyroid enlargement. It can also differentiate solid and cystic lesions.

Background: Dacrocystitis refers to a series of clinical entities characterized by inflammation of lacrimal sac which leads to obstruction at the level of drainage of lacrimal system. Owing to the initial encouraging results and simplicity of the operation, it is decided that all symptomatic patients with lacrimal drainage obstruction would be treated initially by endoscopic endonasal dacrocystorhinostomy method, irrespective of the level of obstruction. Methods: A total 50 patients were included in the study over a period of 3-4 months. They were subjected to endonasal dacrocystorhinostomy and were evaluated for postoperative complications. Results: Very few complications were seen postoperatively suggesting that this newer technique may be useful. Conclusions: Endonasal dacrocystorhinostomy may prove better in coming years.

We present a case of Papillary Thyroid Carcinoma (PTC) in a 4- year- old boy. The very young age of the patient and unusual presentation with respiratory distress prompted us to report this case.
