We aimed to ascertain the chronologicage at which these sonographic measurements begin to show a significantdifference. The study included 100 women (78, aged 19-44 and 22, aged 45-50)menstruated spontaneously and without thyroid disease, diabetes mellitus,hyperprolactinaemia, hypogonadotropic hypogonadism or polycystic ovary syndromeand any gynaecologic surgery. They all underwent sonographic measurement oftotal antral follicle (TAF) count and mean ovarian volume (MOV) on the secondor third days of menstrual cycle. Then, the women were assigned to 5 groups: group1, 19–24 years; group 2, 25–29 years; group 3, 30–37 years; group 4, 38-44years; and group 5, 45-50 years. Therewas a significant association between chronological age and TAF number (r² =0.328, p < 0.001). No significant difference was noted in TAF number amongthe first three groups (² = 0.58, p > 0.05). TAF number was significantly different between groups 3 and 4(Z: -3.463, p < 0.001), but not between groups 4 and 5 (Z: -1.698, p >0.05). There was a significantrelationship between chronological age and MOV (r² = 0.149, p < 0.001). MOVvalues did not show a significant difference among the first 4 groups(² = 0.58, p> 0.05; ² = 7.87, p > 0.05 respectively). The MOV of group 5 was significantly different from those ofgroups 1, 2, 3, and 4 (Z: -2.75, p < 0.01; Z: -4.351, p < 0.01; Z:-2.722, p < 0.01; and Z: -2.829, p < 0.01 respectively). In conclusion, theTAF count decreases significantly beginning at 38 years of age, while MOVdecreases significantly beginning at 45 years of age. The aim of this study was to evaluate the validity of platelet count (PC) and indices, including mean platelet volume (MPV), platelet distribution width (PDW) and platelet crit (PCT), in identifying the pregnancies at risk for preeclampsia. Demographic data and laboratory tests for PC, MPV, PDW and PCT of 250 pregnant women (100 normotensive pregnants, 121 pregnants with severe preeclampsia and 29 pregnants with mild preeclampsia) were retrospectively analyzed. Platelet counts and indices were compared between the study groups. MPV and PDW were significantly higher in preeclamptic pregnant women when compared to the normal group (p=0.006, p=0.046) and area under ROC curve for MPV was found statistically significant. Amaç: Bu çalışma gebelikte uygulanan tarama testlerinin hasta anksiyetesi üzerine olan etkisini araştırmak amacıyla planlanmıştır. Gereç -Yöntem: Çalışmamız 16- 20. haftalar arası 300 gebeyi ve görüşlerine başvurduğumuz 35 kadın hastalıkları ve doğum uzmanını kapsadı. Durumsal anksiyeteyi ölçen STAI anksiyete testi tüm olgulara uygulandı. Gebelerin Down sendromu, tarama testleri ve amniosentez hakkında bilgi düzeyleri, hekimle olan iletişimleri için anket formuyla sorular soruldu. Hekimlerin gebelerin tarama testleri ile olan tutumları yine anket ile sorgulandı. Veriler hasta, hekim sayısı ve yüzde değer olarak ifade edildi. Değerlendirmede nonparametrik ki kare testi uygulandı. Gruplar arası farklılığın değerlendirilmesinde ise çok gözlü düzeyde ki kare testi, Zafson Programı ile kullanıldı. P değeri 0.05’in altında olan değerler istatistiksel olarak anlamlı kabul edildi. Bulgular: Hastaların ortalama yaşı 285 di. %37,3’ü primipar, %62,7’si ise multipardı. Paritelerine göre değerlendirildiğinde ise primipar hastaların multipar hastalardan daha fazla oranda tarama testleri hakkında bilgi sahibi olduğu teste verdikleri cevaplardan görüldü. (p0,05). Objective: We aimed at measuring levels of knowledge of pregnant about Down Syndrome, triple screening test, amniocentesis, and other tests performed during pregnancy their anxiety. Material and Method: Our study includes 300 pregnant women between gestational weeks 16 and 20 and 35 gynecologists that we applied to their opinions. STAI anxiety test measuring conditional anxiety was performed. Questions related to the levels of information of the pregnant women about Down syndrome, screening tests and amniocentesis and their communication with physicians. Attitude of physicians about the screening tests were questioned. Findings: The average age of patients was 28±5 and of them, a group comprising 37,3% was primipara while the other group comprising 62,7% had been multipara. Amaç: Bu çalışmanın amacı 50 gr glukoz tarama testinin sadece gestasyonel diabetes mellitus için risk faktörü taşıyan gebelere uygulanmasının yeterli ve etkili olup olmadığını saptamaktır. Gereç ve yöntem: Bu çalışma Ondokuz Mayıs Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Kliniğine Kasım 2005 - Kasım 2006 tarihleri arasında gebelik takibi nedeniyle başvuran 24-28. gebelik haftalarındaki 426 gebede, prospektif olarak yapıldı. Gebeler, GDM için risk faktörüne sahip olup olmamalarına göre iki gruba ayrıldı. Her iki gruba da 50 gr GTT uygulanıp, 50 gr GTT sonucu yüksek çıkan gebelere 100 gr oral glukoz tolerans testi (100 gr OGTT) yapıldı. Her iki grup için 50 gr GTT’nin pozitif prediktif değeri (PPD) hesaplandı. Bulgular: Çalışmaya dahil edilen 426 gebeden, 222 gebe risk faktörü taşımazken, 204 gebe en az bir risk faktörüne sahipti. Risk faktörü taşımayan ve 100 gr OGTT uygulanan 18 gebenin 4’üne GDM tanısı konuldu. Prevalans %1,8 olarak hesaplandı. Risk faktörü taşıyan ve 100 gr OGTT uygulanan 44 gebenin 18’ine GDM tanısı konuldu. Prevalans %8,8 olarak hesaplandı. Çalışmaya alınan 426 gebenin toplam 22 tanesinde GDM saptanmış olup, GDM’nin prevelansı %5,2 olarak hesaplandı. Risk faktörleri taşıyan gebelerde 50 gr GTT’nin pozitif prediktif değeri %40,9 iken risk faktörleri taşımayanlarda %22,2 olup aralarında istatistiksel olarak anlamlı fark bulundu. (p0.05). Sonuç: Bu çalışma ile GDM taramasında 50 gr GTT’nin sadece risk faktörüne sahip gebelere uygulanmasının daha uygun olacağı sonucuna varılmıştır. Objective: Our objective is to investigate whether 50 gr GCT is enough and effective when performed for pregnant women only carrying the risk factors for GDM. Material and methods: A total of 426 pregnant women having 24-28 weeks of pregnancy were enrolled to this prospective study between November 2005 and November 2006 in Gynecology and Obstetrics Departments of Ondokuz Mayıs University. Pregnant women were divided into two groups according to the presence of risk factors for GDM. 50 gr GCT was applied to both groups. 100 gr oral glucose tolerance test (OGTT) was applied to the cases with positive 50 gr GCT. Positive predictive value (PPV) of 50 gr GCT was calculated for both groups. Results: Of 426 evaluated pregnant women, 222 pregnant women had no risk factor for GDM and the rest 204 pregnant women had at least one risk factor. 4 out of 18 pregnant women who do not carry risk factors, and who had 100 gr OGTT had the diagnosis of GDM, in prevalance of 1,8%. 18 out of 44 pregnant women who carry risk factors, and who had 100 gr OGTT had the diagnosis of GDM, in prevalance of 8,8%. Of 426 pregnant women, 22 had diagnosis of GDM and prevalance of GDM was found to be as 5,2%. Amaç: Bu çalışmanın amacı pre-eklampsili olgularda plazma thrombomodulin seviyelerinin hastalığın patogenezi ve şiddeti ile ilişkisi olup olmadığını araştırmaktı. Gereç ve Yöntem: Çalışma 85 pre-eklampstik ve 85 sağlıklı gebeden oluşan kontrol grubu üzerinde yapıldı. Kontrol grubu, hasta grubu olguların gebelik haftalarına uygun gelecek şekilde sağlıklı gebelerden oluşturuldu. Thrombomodulin için alınan plazma örnekleri ELİSA yöntemiyle çalışıldı. Elde edilen veriler istatistiksel olarak Student T, x2, Mann Whitney U testleri ile değerlendirildi. Bulgular: Hafif ve şiddetli preeklamptik olgular ile kontrol grubu arasında plazma thrombomodulin seviyeleri yönünden istatistiksel olarak anlamlı fark mevcuttu (p0.01). Ancak eklampsi olguları ile kontrol grubu arasında anlamlı fark yoktu (p0.05). Şiddetli preeklampsili olgularda plazma thrombomodulin düzeyleri ile serum kreatinin düzeyleri arasında anlamlı korelasyon mevcutken, hafif preeklampsili ve eklampsili olgularda bu ilişki mevcut değildi. Sonuç: Plazma thrombomodulini pre-eklampsili olgularda endotel harabiyetinin bir göstergesi olarak kullanılabilir. Ancak hastalığın şiddetinin belirlenmesinde güvenilir değildir. Bu çalışmada eklampsi ile endotel hasarı ve plazma thrombomodulin düzeyleri arasında ilişki olmadığı görülmüştür. Ayrıca pre-eklampsili hastalarda plazma thrombomodulin seviyelerindeki yüksekliğin yalnızca renal fonksiyonların bozukluğuna bağlı olmadığı söylenebilir. Objective: The aim of this study is to investigate whether plasma thrombomodulin levels are related to the pathogenesis or the severity of the disease in preeclamptic patients. Material and Methods: The study group comprised of 85 pregnant women with preeclampsia and a total of 85 healthy pregnant controls who attended the Obstetrics and Gynecology department. The control case with a similar gestational age was selected for each study case among the healthy pregnant women. The plasma samples levels were tested by the ELİSA method. The data were analyzed by Student T, x2, Mann Whitney U tests. Results: Mild preeclampsia and severe preeclampsia groups had significantly higher plasma thrombomodulin levels when compared to the levels in their control groups respectively (p0.05). While there was a significant correlation between the plasma thrombomodulin and serum creatinine levels in patients with severe preeclampsia, this was not so in patients with mild preeclampsia and eclampsia. Conclusion: Plasma thrombomodulin may be an indicator of endothelial damage in preeclampsia. However it is not reliable in determining severity of the disease. AMAÇ: Bu çalışma, over volümünün hiperandrojenemideki önemini, polikistik over sendromundaki (PKOS) over volüm artışının bazal hormon parametreleri ile ilişkilerini araştırmak amacıyla yapıldı. MATERYAL ve METOD: PKOS tanısı konulan 40 hasta ile 40 kontrol olgusu çalışmaya dahil edildi. PKOS ve kontrol grubunun her ikisinden de son menstrüel siklusun 3. gününde hormon düzeylerini ölçmek için kan örnekleri alındı. Tüm olguların vücut kitle indeksleri (VKI) vücut ağırlığı (kg.)/boy (metre) formülüyle hesaplandı. Pelvik abdominal veya vajinal ultrasonografi (USG) ile over volümleri ise üç ayrı planda boyutları ölçülerek (p/6 D1*D2*D3) formülü ile hesaplandı. İstatistiksel değerlendirmelerde Mann Whitney U, student t testi ve regresyon analizi kullanıldı. BULGULAR: Ultrasonografik değerlendirmelerde PKOS ve kontrol grubu total over volümü arasında fark anlamlıydı (p 0.05). Bazal FSH, LH, insülin, serbest testosteron ve östrojen düzeyi ortalamaları gruplar arasında fark göstermedi (p 0.05). Total testosteron ve DHEA-S düzeyleri PKOS'lu grupta anlamlı olarak yüksekti (p0.05). SONUÇ: Total over volümü ile PKOS arasında pozitif bir ilişki bulunmaktadır. Overin periferinde bulunan çok sayıdaki atrezik foliküller over boyutlarında artışa katkıda bulunmaktadırlar. Kontrol grubuna göre, PKOS grubunda bulunan yüksek androjen düzeyleri, over volümü ile hormon parametreleri arasında uyumlu değişiklikler olduğunu göstermiştir. OBJECTIVE: The aim of this study was to evaluate the significance of ovarian volume in hyperandrogenemia and to investigate the relationship between the ovarian volume and hormone parameters in polycystic ovarian syndrome (PCOS). STUDY DESING: Fourty healthy women and 40 women with PCOS included the study. Blood sampling was performed in the early follicular phase (on the day 3 of the spontan or induced cycle) for measurement of the hormone levels in both PCOS and control women. Amaç: Bu çalışmanın amacı; pre-eklampsili olgularda serum total hCG ve serbest ß-hCG seviyelerinin hastalığın patogenezi ve şiddeti ile ilişkisi olup olmadığını araştırmaktır. Gereç ve Yöntem: 75 pre-eklampsili ve gebelik haftaları hasta grubuna uygun toplam 75 sağlıklı gebeden oluşan kontrol grubu olgularda total hCG ve serbest ß-hCG değerleri karşılaştırıldı. Alınan serum örnekleri ELİSA yöntemi ile çalışıldı. Elde edilen veriler istatistiksel olarak Student T, x2 ve Mann Whitney U testleri ile değerlendirildi. Bulgular: Hafif preeklampsi ile kontrol grubu arasında total hCG ve serbest ß-hCG düzeyleri yönünden istatistiksel olarak anlamlı fark bulunmadı (p0.05). Ancak şiddetli preeklampsi ve eklampsi ile kontrol grupları arasında fark anlamlı olarak saptandı (p0.05). Ancak, şiddetli preeklampsi ve eklampsi grubunda, kontrol grubu hastalara göre istatistiksel olarak anlamlı düzeyde yüksektir. Bütün bu bulgular serum total hCG ve serbest ß-hCG düzeylerinin preeklampsi gelişiminin öngörülmesinde bir belirteç olarak fazla bir anlam ifade etmediğini ancak hastalığın şiddetini destekleyici belirteçlerden biri olabileceğini göstermektedir. Results: There were no significant differences in serum levels of total hCGand free ß-hCG between patients with mild preeclampsia and those of their macthed controls (p>0.05). Patients with preeclampsia and eclampsia had significantly different serum levels of total hCG when compared to their control group (p0.05). Conclusion: As a conclusion serum total hCG and free ß-hCG levels could be supportive markers in the diagnosis of severe preeclampsia and eclampsia. Increased serum total hCG and free ß-hCG levels might indicate the severity of the disease but they could not be used as early predictive markers or determinant of preeclampsia in general sense.

Amaç: Bu çalışmanın amacı, Ondokuz Mayıs Üniversitesi Hastanesi'ne başvuran gebelerde, demir eksikliği anemi prevalansını, yerleşim yeri, eğitim düzeyi ve vücut demir depolarının durumuna göre belirlemek amacıyla yapıldı. Gereç ve Yöntem: Kesitsel tipteki bu çalışmaya, Nisan 2003-Nisan 2004 tarihleri arasında kadın doğum kliniğine başvuran 590 gebe alındı. Gebeler tam kan sayımı ve anemi ile ilişkili diğer parametreler açısından araştırıldı. Hb düzeyi, 10.5 g/dl altında olanlar anemik olarak kabul edildi. İstatistiksel değerlendirmelerde student t ve ki-kare testleri kullanıldı. Bulgular: 90 (%15.3) gebede demir eksikliği anemisi saptandı. Eğitim düzeyine göre, üniversite mezunu gebelerin %9 kadarı anemik iken lise eğitimi ve altındakilerde %16.3 oranında anemi olduğu saptandı. Şehir merkezinde oturan gebelerde anemi prevalansı %14.4 iken, kırsal yerleşimde %27.8 olarak bulundu. İstatistiksel olarak total demir bağlama kapasitesi, transferrin ve ferritin değerleri açısından anlamlı farklılık bulunamadı. Sonuç: Çalışmamızda demir depolarının, anemik olan ve olmayan olgu grupları arasında farklılık göstermemesi nedeniyle, anemik olmayan gebelere de demir desteği verilmesinin, demir depolarının doyurulması açısından yararlı olabileceği kanısına varıldı. Objective: The aim of this study was to investigate the prevalence of iron deficiency anemia according to living place and educational level and iron body reserve among the pregnant women who attended to 19 Mayıs University Hospital. Materials and Methods: Between April 2003-April 2004, a total of 590 pregnant women were included to this cross sectional study. They evaluated performing complete blood count and diagnostic tests with regard to anemia. The cut-off hemoglobin level for anemia was accepted as 10.5 g/dl. Student t test and chi-square tests were used in statistical analysis. Results: Ninety (15.3%) were considered to be anemic. Anemia rates were 9% and 16.3% for university education and high school graduated or below, respectively. The prevalence of anemia was 14.4% for patients who had been settling in the city center and 27.8% for patients living in the countryside. Total iron binding capacity, serum ferritin, transferrin and were not different between anemic and non-anemic groups. Conclusion: In our study, since there was no difference in iron body reserve between anemic and non-anemic pregnant groups, it was thought that iron supplementation may be helpful in saturation of the iron body reserve for non-anemic pregnant.

Amaç: Bu çalışmanın amacı, serum leptin düzeyi, insülin rezistansı ve preeklampsi arasındaki ilişkiyi araştırmaktır. Gereç ve Yöntem: Bu çalışma Mayıs 2000-Haziran 2001 tarihleri arasında, 85 preeklampsili gebe olgu, ve hasta grubu olguların gebelik haftalarına uygun olacak şekilde toplam 85 sağlıklı gebeden oluşan kontrol grubu üzerinde yapıldı. Leptin ölçümleri Active Human Leptin IRMA DSL-23100 (Diagnostic Systems Laboratories, Inc. Texas/USA) leptin kiti ile, insülin ölçümleri Immulite 2000 analizör kullanılarak yapıldı. İnsülin rezistansının derecesi HOMA (homeostazis model assesment) katsayısı ile ifade edildi. İstatistiksel değerlendirmelerde, Student T, Mann Whitney U testleri kullanıldı. Bulgular: Serum leptini preeklampsi grubunda kontrol grubuna göre belirgin olarak yüksek bulundu (sırasıyla 46.131.1 ve 27.721.1; p0.001). İnsülin düzeyleri ve HOMA IR gruplar arasında fark göstermedi (P0.05). Sonuç: Çalışmamızın bulgularına göre, serum leptin düzeylerinin preeklampside yükselmesine rağmen, preeklampsinin şiddetinde bir beliteç olmadığı görülmüştür. Toplam hasta grubu şiddetli preeklampsi alt grubunda, leptin düzeyi ile HOMA IR ve insülin düzeyleri arasında ilişki bulunmaması insülin rezistansının preeklampsi patogenezinde rol alabileceği fikrini desteklememektedir. Serum insulin levels were tested by the Immulite 2000 analyzer method. The degree of insulin resistance was demonstrated by HOMA coefficient. The data were analyzed by student T and Mann Whitney U tests. Results: Serum leptin levels of the preeclamptic group were significantly higher than the total control group (46.1±31.1 and 27.7±21.1; p0.05). Conclusion: According to these results, although serum leptin levels increase in preeclampsia, it was not an indicator in determining the severity of the disease. Since serum leptin levels were not related with HOMA IR and serum insulin levels in the overall preeclampsia group, the idea that insulin resistance has a role in the pathogenesis of preeclampsia could not be supported.

Objective: To assess the recovery, maintenance, and quality of ovarian function by detecting hormonal status and hystopathologic examination of autotransplanted ovary in abdominal wall location in syngeneic winstar rats. Study Design: This experimental study was conducted in the Unit of Experimental Research at the Ondokuz Mayıs University, School of Medicine. Autologous heterotopic transplantation of ovarian tissue was performed without vascular pedicle in rats. Hormone levels were measured on the basal day and at the end of the 12th week. Also, hystopathologic examination of the transplanted ovaries were performed at the end of the experiment. Results: All of the transplanted ovaries were successfully vascularised and there was no necrosis among ovarian species on hystopathological evaluation. It was found out that serum hormone levels of control and transplant group did not show a significant difference in and between the groups. Conclusion: A heterotopic autotransplant of ovary without vascular pedicle in syngeneic Wistar rats was 100% successful without any necrosis in our animals.

Bu yazıda, başlangıçta transvaginal ultrasonografi yardımıyla tanı konulan ve sistemik tek doz metotreksat tedavisi uyguladığımız iki olguyu sunuyoruz. Bu olgulardandan bir tanesi tek doz metotreksat ile tedavi olurken diğer hastada tedavinin 19. gününde şiddetli hemoraji gelişti ve serviko-vajinal tampon yardımıyla durduruldu. We present two cases of cervical pregnancy diagnosed by transvaginal ultrasonography. Initially, they were treated with systemic single dose methotrexate. One of the patients was treated successfully with only single dose methotrexate, however the other patient developed severe hemorrhage on the 19th day of the treatment, which was controlled by uterine cervico-vaginal tamponade together with uterine packing.
