Aim: To share our experience with the management of Fournier gangrene (FG) using the bagging technique of the testes, and to highlight the importance of implementing a multidisciplinary approach in managing FG. Case presentation: A 58-year-old male with type 2 diabetes mellitus (DM) was brought to the emergency department (ED) with necrotizing fasciitis involving the genitalia; he was managed in the ED with Intrave (IV) fluid resuscitation and IV antibiotics. The surgical team was consulted and multiple debridement procedures were done. Healthy granulation tissue was formed within one month of the serial debridement. A split-thickness skin graft using bagging technique of the testes and vacuum-assisted closure (VAC) were applied. The patient was reassessed one year following presentation, and a result with a near normal appearance was achieved with complete preservation of functional outcome.Conclusion: FG is a type of necrotizing fasciitis that could be managed either conservatively with IV antibiotics and/or hyperbaric oxygen, or surgically by debridement and applying VAC.In our case, the testes were debrided and bagging technique of the testes was used. It’s believed that with this technique, the overall cosmetic and functional results are superior.

Background: Type 1 diabetes mellitus (T1DM) is on the rise globally. Patient education is a major cornerstone in diabetes management. Diabetes educators play a pivotal role in educating patients with diabetes mellitus. Objectives: To estimate the impact of a diabetes educator-led clinic on the control of T1DM among adults. This was measured through mean changes in HbA1c levels and alterations in perceived dietary and physical activity compliance among patients attending a Diabetes Centre in the Qassim region of Saudi Arabia. Methods: We enrolled 97 adult T1DM patients in a quasi-experimental study, conducted at a certified diabetes educator-led clinic. All patients were followed up for 6 months pre-intervention and for a minimum of 6 months post-intervention. Patients' weight, self-monitoring of blood glucose, hypoglycaemic events frequency, HbA1c level and lipid profiles were routinely recorded. In addition, compliance with advice on dietary plan and physical activity recommendations was also assessed at the pre-intervention, baseline and post-intervention stages. Results: Post-intervention, participants reported better compliance with dietary and physical activity advice. Fewer hypoglycaemic events and higher self-monitoring of blood glucose frequencies were also noted. While there was no significant difference between pre-intervention and baseline mean HbA1c levels, mean baseline and post-intervention HbA1c levels showed a clinical and statistically significant difference of 1.01 (95% confidence interval [CI]: 0.74-1.28). Mean low-density lipoprotein (LDL) and triglyceride differences also showed small but significant changes. Conclusion: Attendance at a diabetes educator-led clinic had a positive and significant impact on the control of T1DM among adults. We recommend diabetes educator employment at outpatient clinics for adults with T1DM.

One of the environmental problems that affects negatively orange productivity is drought because it greatly minimizes the growth attributes, photosynthetic process, water uptake, percentage of fruit set, and productivity; meanwhile, it raises the rates of respiration and transpiration, as well as the premature and preharvest fruit drop percentages. In addition, drought creates osmotic stress, affects the relationship between plants and water, reduces the amount of water in shoots, and prevents plant cell development and expansion. It is very important to search for a solution to minimize the effect of drought stress; therefore, the present study has investigated the effect of the application of humic acid (HA) at 0, 1 and 2 kg per tree and spraying of seaweed extract (SWE) at 0.2, 0.3 and 4% in combination with moringa leaf extract (MLE) at 2, 4 and 6%, respectively, on the productivity, fruit quality and nutritional status of navel orange cv. Washington during the 2022 and 2023 seasons. The results proved that the application of the biostimulants individually or in combination significantly positively changed the vegetative growth, productivity, fruit quality parameters and leaf mineral content of macro- and micronutrients of the treated trees compared to untreated trees. The superior treatments which gave the best results were 2 kg HA + 0.4% SWE + 6% MLE followed by 2 kg HA + 0.3% SWE + 4% MLE over the rest of the applied treatments. [ABSTRACT FROM AUTHOR]

This study presents an efficient vibration-based fault detection method for rotating machines utilising the poly-coherent composite spectrum (pCCS) and machine learning techniques. pCCS combines vibration measurements from multiple bearing locations into a single spectrum, retaining amplitude and phase information while reducing background noise. The use of pCCS significantly reduces the number of extracted parameters in the frequency domain compared to using individual spectra at each measurement location. This reduction in parameters is crucial, especially for large industrial rotating machines, as processing and analysing extensive datasets demand significant computational resources, increasing the time and cost of fault detection. An artificial neural network (ANN)-based machine learning model is then employed for fault detection using these reduced extracted parameters. The methodology is developed and validated on an experimental rotating machine at three different speeds: below the first critical speed, between the first and second critical speeds, and above the second critical speed. This range of speeds represents the diverse dynamic conditions commonly encountered in industrial settings. This study examines both healthy machine conditions and various simulated fault conditions, including misalignment, rotor-to-stator rub, shaft cracks, and bearing faults. By combining the pCCS technique with machine learning, this study enhances the reliability, efficiency, and practical applicability of fault detection in rotating machines under varying dynamic conditions and different machine conditions. One of the biggest problems that threaten the production of strawberry in the world is the rapid damage and high rate of deterioration after harvest or during cold storage. Therefore, the current study was conducted to investigate the possibility of decreasing the post-harvest damage percentages and increasing the fruit quality of Fragaria x ananassa cv. 'Estavana' after harvest immediately or during the cold storage period. The strawberry plants were dipped for 3 to 5 minutes at 25 α in a solution of 10 L made from distilled water and containing salicylic acid (SA) at 0, 250, 500, and 1000 mg/L or chitosan (CHIT) at 0, 2.5, 5, and 10 mg/L during the period of 0, 3, 6, 9, 12, and 16 days after harvesting. The results showed that the fruit firmness was notably decreased, and the loss and decay percentages were increased by increasing the period of storage, but it could be decreased by using SA or CHIT. Fruit content from soluble solids, total sugars and anthocyanin was significantly increased in the 16 days stored fruits treated with 500 mg/L SA or 50mg/L CHIT. Treating the fresh harvested without or with SA or CHIT increased the fruit content from vitamin C. The highest fruit content from titratable acidity was in the fresh harvested fruits compared with treated fruits with SA or CHIT. [ABSTRACT FROM AUTHOR]
