Abstract

The massive deployment of Machine Learning (ML) models raises serious concerns about data protection. Privacy-enhancing technologies (PETs) offer a promising first step, but hard challenges persist in achieving confidentiality and differential privacy in distributed learning. In this paper, we describe a novel, regulation-compliant data protection technique for the distributed training of ML models, applicable throughout the ML life cycle regardless of the underlying ML architecture. Designed from the data owner's perspective, our method protects both training data and ML model parameters by employing a protocol based on a quantized multi-hash data representation Hash-Comb combined with randomization. The hyper-parameters of our scheme can be shared using standard Secure Multi-Party computation protocols. Our experimental results demonstrate the robustness and accuracy-preserving properties of our approach.

2. Validity of Using Still Images for Written Competency Assessments for Point-of-Care Ultrasound among Non-Radiologists-pilot study.

Abstract

Background and Objective: Point-of-care ultrasound (POCUS) has become an integral part of the curricula of various spe-cialty residency programs. POCUS education requires learners to be proficient in image acqui-sition, image interpretation, and integration into clinical practice. The assessment of POCUS skills and competency is variable. This study aimed to assess the validity and reliability of us-ing still ultrasound images as examination test items without providing clinical vignettes or anatomical locations. Methods: This is a proof-of-concept survey pilot study, residents from the Middle East who had com-pleted their training in emergency medicine were surveyed during their competency assess-ment examination. They were provided with a single ultrasound image of an abdominal aortic aneurysm in both print and PowerPoint slide format. The task was to identify the diagnosis based on this image, which was taken from a reference emergency medicine textbook. Results: The study showed that only 50% of the residents correctly identified the diagnoses based on the still images. Many of the incorrect answers included diagnoses that were not related to the anatomical structure that was scanned, indicating that the residents were not able to identify the anatomical locations, which highlighted the challenges of interpreting a single still image without clinical context and emphasized the need for valid and reliable test items in point-of-care ultrasound (POCUS) education and assessment. Conclusions: National and international emergency medicine societies should establish standards for writ-ing POCUS examination test items to ensure their validity and reliability. Abstract

Background: Cardioversion is the treatment of choice in an unstable patient with arrhythmia. It requires analgesia and sedation, which is usually given via intravenous route. However, when there is a failure to get vascular access, alternatives routes to intravenous (IV) sedation and analgesia are needed. Case Presentation: A 59 year old male patient was presented to the emergency department (ED) complaining of chest pain and palpitation. His electrocardiogram (ECG) showed new onset rapid narrow complex atrial fibrillation, which then converted to wide complex tachycardia. Initially, the patient remained hemodynamically stable, but his blood pressure dropped, when cardioversion was indicated. In this case, intranasal (IN) and intraosseous (IO) routes were used, both of which are uncommon to be used for sedation in an unstable patient. The sedatives bioavailability and dosage via non intravenous (non-IV) routes are not commonly used, hence, literature was reviewed to find out the appropriate sedative dosage via non-IV routes specifically in IN and IO route for moderate sedation in ED, to facilitate procedures like cardioversion. Conclusion: It was believed that IN and IO routes are good alternatives to intravenous routes when timely vascular access can't be secured and when emergency medication administration is required. Abstract

The present study performs the design of a novel Gudermannian neural networks (GNNs) for the nonlinear dynamics of prey-predator system (NDPPS). The process of GNNs is applied using the global and local search approaches named as genetic algorithm and interior-point algorithms, i.e., GNNs-GA-IPA. An error-based merit function is constructed using the NDPPS and its initial conditions and then optimized by the hybrid of GA-IPA. Six cases of the NDPPS using the variable coefficients have been presented and the correctness is observed through the overlapping of the obtained and Runge-Kutta reference results. The results of the NDPPS have been performed between 0 and 5 using the step size 0.02. The graph of absolute error are performed around 10−06 to 10−08 to check the consistency of the proposed GNNs-GA-IPA. The statistical analysis based minimum, median and semi-interquartile ranges have been performed for both predator and prey dynamics of the model. Moreover, the investigations through the statistical operators are performed to validate the reliability of the obtained outcomes based on multiple trials.

Abstract

Aim:Mycobacterium tuberculosis (MTB) is a leading cause of death worldwide. The World Health Organization (WHO) recommends X-pert MTB/RIF or X-pert Ultra as the initial test for pulmonary MTB diagnosis. While several studies have explored the cost-effectiveness of this technology, none have specifically looked at its use in the United Arab Emirates (UAE). To evaluate the average estimated cost and length of stay for suspected MTB patients admitted from the emergency department to the respiratory isolation rooms to rule out MTB using the MTB classic diagnosis pathway of 3 AFB smear and MTB cultures compared to the estimated cost if the WHO X-pert MTB/RIF outpatient pathway is implemented for suspected MTB.Materials and Methods:A quality improvement project was conducted with a retrospective audit and data analysis of suspected pulmonary MTB infection at a secondary care hospital in Abu Dhabi, UAE. We report the true accrued costs of the current admission practice for management of suspected pulmonary MTB. We also report the estimated cost of working up these same patients with the WHO pathway using X-pert MTB/RIF testing.Results:Data analysis demonstrated that 62% of the cost of working up suspected pulmonary MTB was accumulated during admissions for patients who ultimately proved to be MTB negative. Cost evaluation of study data suggests that using the WHO X-pert MTB/RIF clinical pathway would cost approximately one-tenth as much as the current practice.Conclusion:This analysis presents evidence for cost savings associated with the use of the WHO X-pert MTB/RIF clinical pathway in a low MTB incidence area such as the UAE. Further analysis to assess how the pulmonary MTB diagnostic pathway was influenced by COVID-19 is needed.

11. Association between Vegetarian Diet Consumption and Academic Performance, Sleep Quality, and Health-Related Quality of Life among Female Undergraduate College Students

Abstract

Purpose. This cross-sectional survey study aimed to examine the association between vegetarian diet consumption and sleep quality, academic performance, and health-related quality of life among female undergraduate college students. Method. A sample of 158 undergraduate female college students was recruited using a convenience sampling approach. Data collection utilized reliable and validated English-language instruments including the Vegetarian Quality of Life Questionnaire (VEGQOL), Pittsburgh Sleep Quality Index (PSQI), and health-related quality of life scale (HRQoL). The data were analyzed using one-way analysis of variance (ANOVA) and appropriate descriptive statistics. Results. Vegetarians exhibited a significantly lower BMI compared to nonvegetarians (F (1, 156) = [6.09], p=0.015). Those strictly adhering to a vegan diet (48.79 ± 9.41) had the lowest vegetarian quality of life among all participants following various forms of a vegetarian diet (F (3, 68) = [2.78], p=0.04). The majority of female college students reported good to excellent general health (91.7%), with 53.2% perceiving their sleep quality as fairly good. However, the mean PSQI global score of 8.04 (±3.35) indicated poor sleep quality. No significant association was found between diet type (vegetarian vs. nonvegetarian) and academic performance (cGPA) (χ2 (4, N = 158) = 2.92, p=0.57). There was no significant relationship between diet types and academic performance, HRQoL, and sleep quality. Conclusion. Despite a significant association between vegetarian diet and lower BMI, surprisingly, no substantial relationships were identified between diet type and academic performance, HRQoL, and sleep quality. These findings contribute to the ongoing discourse on the potential impacts of a vegetarian diet on various facets of female college students’ well-being and highlight the need for further exploration in this field.

Abstract

BackgroundEarly identification of a patient with infection who may develop sepsis is of utmost importance. Unfortunately, this remains elusive because no single clinical measure or test can reflect complex pathophysiological changes in patients with sepsis. However, multiple clinical and laboratory parameters indicate impending sepsis and organ dysfunction. Screening tools using these parameters can help identify the condition, such as SIRS, quick SOFA (qSOFA), National Early Warning Score (NEWS), or Modified Early Warning Score (MEWS). We aim to externally validate qSOFA, SIRS, and NEWS/NEWS2/MEWS for in-hospital mortality among adult patients with suspected infection who presenting to the emergency department.Methods and analysisPASSEM study is an international prospective external validation cohort study. For 9 months, each participating center will recruit consecutive adult patients who visited the emergency departments with suspected infection and are planned for hospitalization. We will collect patients' demographics, vital signs measured in the triage, initial white blood cell count, and variables required to calculate Charlson Comorbidities Index; and follow patients for 90 days since their inclusion in the study. The primary outcome will be 30-days in-hospital mortality. The secondary outcome will be intensive care unit (ICU) admission, prolonged stay in the ICU (i.e., ≥72 hours), and 30- as well as 90-days all-cause mortality. The study started in December 2021 and planned to enroll 2851 patients to reach 200 in-hospital death. The sample size is adaptive and will be adjusted based on prespecified consecutive interim analyses.DiscussionPASSEM study will be the first international multicenter prospective cohort study that designated to externally validate qSOFA score, SIRS criteria, and EWSs for in-hospital mortality among adult patients with suspected infection presenting to the ED in the Middle East region.Study registrationThe study is registered at ClinicalTrials.gov (NCT05172479).

AbstractThe purpose of this study was to examine the relationship between career adaptability, job crafting, and job embeddedness, and it was predicted that career adaptability promotes job embeddedness in self-initiated expatriates by increasing their job crafting behavior. Data were collected from 881 self-initiated expatriates from multinational companies in the United Arab Emirates, which were analyzed using structural equation modeling. The structural equation modeling results support the proposed hypotheses and suggest that career adaptability and job crafting are associated with job embeddedness. Specifically, career adaptability is effective in promoting job embeddedness through job crafting. Therefore, career adaptability is an important antecedent for self-initiated proactivity and job embeddedness in organizations.

Abstract

Mesenteric cysts are rare benign intra-abdominal pediatric lesions. It has a variable clinical presentation from an asymptomatic mass to an acute abdomen that appears in the omentum or mesentery of the gastrointestinal tract. Abdominal ultrasound and complete surgical resection are the modality of choice for diagnosis and treatment. Here, we present 15 months old boy who presented to the emergency department with constipation and abdominal pain and was found to have a huge mesenteric cyst that was excised surgically.

Abstract

The report focuses on three areas in particular: the first is the Received Signal Strength indicator technique, Direction of Arrival technique, and the integration of two algorithms, RSS and DOA, in order to build a hybrid, more robust algorithms. In the Received Signal Strength (RSS), the unknown node location is estimated using trilateration. This report examines the performance of different estimators such as Least Square, Weighted Least Square, and Huber robustness in order to obtain the most robust performance. In the direction of arrival (DOA) method, the estimation is carried out using Multiple Signal Classification (MUSIC), Root-MUSIC, and Estimation of Signal Parameters Via Rotational Invariance Technique (ESPRIT) algorithms. We investigate multiple signal scenarios utilizing various antenna geometries, which includes uniform linear array (ULA) and uniform circular array (UCA). Specific attention is given for multipath scenarios in which signals become spatially correlated (or coherent). This required the use of pre-processing techniques, which include phase mode excitation (PME), spatial smoothing (SS), and Toeplitz. Further improvements of existing localization techniques are demonstrated through the use of a hybrid approach in which various combinations of RSS and DOA are explored, simulated, and analyzed. This has led to two major contributions: the first contribution is a combined RSS/DOA method, based on UCA, which has the tolerance of detecting both uncorrelated and coherent signals simultaneously. Abstract

Background: Burnout among health-care workers have become a notable global health and safety challenge. This phenomenon reflects mental and physical burden on healthcare staff. The Maslach Burnout Inventory tool is a structured and validated tool that has been used to objectively assess burnout. The aim of this study was to determine the prevalence of burnout among physicians working in Mafraq Hospital, Abu Dhabi, United Arab Emirates (UAE) and to understand the possible underlying factors. Methods: A cross-sectional survey study was conducted incorporating the Maslach Burnout Inventory Human Services Survey for Medical Personnel tool. A questionnaire was sent via an anonymous e-mail to all physicians at Mafraq Hospital. Participating physicians were categorized as part of a 5-class profile continuum ranging from "engaged" to "burnout". Results: Burnout prevalence among Mafraq physicians was 11.24% (19 of the 169). The rate of burnout was higher among resident physicians (17%) than non-resident physicians (8.5%). Majority of physicians were "overextended" (55 of 169; 32.5%). Mafraq physicians were noted to be more emotionally exhausted than the healthcare worker comparison group provided by Mind Gardens. It was also found that males, young participants, unmarried, and smokers had a higher rate of burnout. Conclusion: The burnout among Mafraq physicians using the person centered five burnout profiles model was analyzed which helped in mapping out employee from burnout to engaged. A prevalence of 1 in 10 Mafraq physicians for burnout was determined. A strategy to proactively prevent burnout among physicians and healthcare workers in UAE is needed at an organizational level. Abstract

Fine jewelry is a unique class of ornaments composed of precious metals and gemstones. Premium-grade metals such as gold, platinum, and sliver, and gemstones such as pearls, diamonds, rubies, and emeralds are used use to make fine jewelry. Paper-based certificates are typically issued by retailers and producers for fine jewelry and gemstones as a proof of origin, sale, ownership, history, and quality. However, paper certificates are subject to counterfeiting, loss, or theft. In this paper, we show how non-fungible tokens (NFTs) and Ethereum blockchain can be used for digital certification, proof of ownership, sale history, and quality, as well as proof of delivery for fine jewelry and gemstones. We present the proposed system design and architecture with sequence diagrams covering key interactions for jewelry production, purchase, and sale, along with algorithms related to NFT minting, auctioning, ownership management, and physical delivery. We demonstrate that our proposed NFT and blockchain-based solution can provide superior alternative in terms of verifiability, traceability, immutability, and security when compared with paper-based certification and traditional auctioning, delivery and ownership management. We make our developed smart contracts and testing scripts publicly available on GitHub.

19. What are the predictors of survival among patients who suffer cardiac arrest in the ED? A single-center chart review

Abstract

Background: Cardiac arrest in the emergency department (ED) has not been well studied; however, the existing literature shows that there are better survival and neurological outcomes with cardiac arrest in the ED than out-of-hospital cardiac arrest (OHCA) and in-hospital cardiac arrests. The aim of this study was to examine the cardiac arrests managed in a tertiary care center ED and identify the characteristics of patients who had good outcomes. Methods: This was a retrospective chart review of patients who had cardiac arrest in ED from January 2018 to June 2019 Results: There were 209 cardiac arrests reported in the ED. A total of 189 patients were excluded because they presented with OHCA. Twenty patients had cardiac arrests in the ED. The survival to discharge from the hospital for the total patients analyzed was 40% (8/20). 60% (12/20) of patients were male with a median age of 60.5 years. Altered mental status or drowsiness was the presenting complaints of nearly 40% (8/20) of the patients, followed by dyspnea in 20% (4/20). Only 10% (2/20) reported having chest pain at presentation. 45% (9/20) of the patients had peri-intubation arrest. The most common initial rhythm was pulseless electrical activity (PEA) (70%, 14/20), followed by asystole (25%, 5/20) and ventricular fibrillation (VF), which was only 5% (1/20). 95% (19/20) of the patients had a return of spontaneous circulation with an average duration of cardiopulmonary resuscitation (CPR) of only 5 minutes. CPR duration and heart rate (HR) on arrival were the potential predictors of survival as per the classification and regression trees method. Age, gender, HR as a continuous variable, systolic blood pressure, diastolic blood pressure, mean arterial blood pressure >70 mm hg, the Modified Early Warning Score, or National Early Warning Score 2 were not statistically significant by Cox regression analysis as predictors of survival. Conclusion: Cardiac arrests occurring in ED are associated with better outcomes than OHCAs. CPR duration of less than 5 minutes coupled with presenting HR of > 110 b/min might be a predictor of survival. Larger database analyses of cardiac arrest in EDs are needed to validate this finding. Data collection utilized reliable and validated English‐language instruments including the Vegetarian Quality of Life Questionnaire (VEGQOL), Pittsburgh Sleep Quality Index (PSQI), and health‐related quality of life scale (HRQoL). The data were analyzed using one‐way analysis of variance (ANOVA) and appropriate descriptive statistics. Results. Vegetarians exhibited a significantly lower BMI compared to nonvegetarians (F (1, 156) = [6.09], p = 0.015). Those strictly adhering to a vegan diet (48.79 ± 9.41) had the lowest vegetarian quality of life among all participants following various forms of a vegetarian diet (F (3, 68) = [2.78], p = 0.04). The majority of female college students reported good to excellent general health (91.7%), with 53.2% perceiving their sleep quality as fairly good. However, the mean PSQI global score of 8.04 (±3.35) indicated poor sleep quality. No significant association was found between diet type (vegetarian vs. nonvegetarian) and academic performance (cGPA) (χ2 (4, N = 158) = 2.92, p = 0.57). There was no significant relationship between diet types and academic performance, HRQoL, and sleep quality. Conclusion. Despite a significant association between vegetarian diet and lower BMI, surprisingly, no substantial relationships were identified between diet type and academic performance, HRQoL, and sleep quality. These findings contribute to the ongoing discourse on the potential impacts of a vegetarian diet on various facets of female college students' well‐being and highlight the need for further exploration in this field. 26. Intravenous fluid therapy in the noncritically ill patient. Our experience at a tertiary care emergency department in Abu Dhabi

Abstract

Background: Intravenous (IV) fluids are a commonly prescribed treatment in the emergency department (ED). Often the prescription of a treatment to be administered in the ED prolongs the patients' length of stay (LOS). We hypothesize that IV fluids are overprescribed in the ED, mainly in patients classified as Emergency Severity Index (ESI) triaged level 3 (T3) and level 4 (T4). Methods: We conducted a quality audit to assess the appropriateness of IV fluid use in adult (age > 16 years) hemodynamically (HD) stable ESI triaged T3 and T4 patients at a tertiary hospital ED during two 6-month periods. Appropriate use of IV fluids was defined from the published literature and our own clinical experience. The primary outcome looked at was the number of patients appropriately prescribed IV fluids before and after implementing a list of indications for IV fluid use in HD stable patients. Results: Analysis showed 62.8% of T3 triaged patients and 74.2% of T4 patients were inappropriately prescribed IV fluids before the implementation of our indication list. This compared similar to 63.6% of T3 triaged patients and 71.4 % of T4 triaged patients after implementation. Median LOS of patients who received IV fluids was 3.5 and 3 hours for T3 and T4 patients, respectively. Conclusion: The data obtained in this quality audit demonstrated no difference in the use of IV fluids before and after implementing a list of indications for fluid therapy in HD stable T3 and T4 patients in our ED. We believe that developing a structured education program for caregivers in addition to patient education may help improve utilization of IV fluid based on true clinical indication. 29. ‘See and Treat’ Clinic Service Evaluation at a Tertiary Care Hospital in Abu Dhabi

Abstract

Objectives: We sought to evaluate the performance provided at a ‘See and Treat’ (ST) clinic at a tertiary hospital emergency department (ED) in Abu Dhabi, UAE, and to assess its impact on ED crowding. Methods: We conducted a retrospective electronic medical chart review and database analyses. We included patients triaged as triage level 4 (T4) and triage level 5 (T5) as per the Emergency Severity Index treated at ED in the ST clinic and other ED areas, including the off-site Urgent Care Centre (UCC) between 1 June 2016 and 30 June 2017. We analyzed a group of process and outcome measures at our ST clinic and compared them to the same measures in other areas of our ED and the co-located UCC. The process measure analyzed was the door-to-doctor time. In addition, the outcome measures analyzed were the door-to-door time, unplanned return within 72 hours, and feedback from T4 and T5 triaged patients treated at the clinic. Results: The number of patients enrolled in the study was 43 109. Of these, 11329 (26.3%) patients were treated at the ST clinic, 6328 (14.7%) were treated at the UCC, and 25 452 (59.0%) were treated at the main ED. The door-to-doctor time was within 30 minutes for 89.0% of ST clinic patients, and 94.0% of patients experienced a doorto-door time of within two hours; 2.1% of these patients returned within 72 hours. Among these, 78.7% returned for an issue related to their first visit. However, none of the patients were admitted on their return visit. For patients presenting to UCC and other parts of our ED, we recorded a door-to-doctor time of within 30 minutes for 80.5% of patients and a door-to-door time of within two hours for 73.0% of patients. We found the difference in waiting times (i.e., door-to-doctor times between ST clinic patients and the rest of ED) to be statistically significant (p < 0.001, 95% confidence interval (CI): 0.56–0.63). However, on comparing door-to-door times, we found the difference between ST clinic patients and the rest of ED patients was not statistically significant. Conclusions: Door-to-doctor times were shorter in ST clinics compared to other parts of our ED, but there was no statistically significant difference in door-to-door times when comparing ST clinics to the rest of the ED. ST clinic patients had a lower rate of unplanned return within 72 hours, of which, none required admission on the return attendance. We believe ST clinics have a positive impact on reducing ED crowding but acknowledge they are one of the many plausible solutions attributing to optimized patient flow in the ED.

Abstract

In relation to “objective risk” or “subjective risk”, a bibliometric analysis was performed using documents found in the Scopus database. A search for related documents was narrowed down to 192 documents and these were considered in this study. The results of this study suggest that the use of the ranking method and descriptive statistics is not sufficient in presenting a concise bibliometric analysis. To create a more in-depth bibliometric analysis, the results of this study have to be analyzed together with a visualization map using VOSviewer software. This way, researchers can easily locate a specific gap in the literature, understand the relation between the papers on the same subject, and cite the literature studies based on their effectiveness.

Abstract

Green development across the world has brought forth the idea of Green Human Resource Management (green HRM), which helps sustainability. Companies can get benefits from green HRM by increasing brand awareness by customers through implementing the right practices. The purpose of the current investigation was further to extend the research on the effects of green HRM. This research focuses on the impact of green HRM on the sustainable performance of the manufacturing industry in the United Arab Emirates (UAE). Social, environmental, and economic performance indicators were identified. A survey was used, where the number of respondents was 111 manufacturing employees in the UAE. The data gained was analyzed using different statistical methods such as t-test, Analysis of Variance (ANOVA), and Pearson correlation. Results showed a generally accepted level of green practices and sustainable performance. However, about 16% of the respondents reported low performance and application levels in their companies. Results also showed a positive impact of the green HRM on environmental, economic, and social performance indicators. The value of this paper comes from being the first study that investigates the effect of green HRM on performance in the UAE.

Abstract

Background: Urine output is widely used as one of the criteria for the diagnosis and staging of acute renal failure, but few studies have specifically assessed the role of oliguria as a marker of acute renal failure or outcomes in general intensive care unit (ICU) patients. Using a large multinational database, we therefore evaluated the occurrence of oliguria (defined as a urine output < 0.5 ml/kg/h) in acutely ill patients and its association with the need for renal replacement therapy (RRT) and outcome. Methods: International observational study. All adult (> 16 years) patients in the ICON audit who had a urine output measurement on the day of admission were included. To investigate the association between oliguria and mortality, we used a multilevel analysis. Results: Of the 8292 patients included, 2050 (24.7%) were oliguric during the first 24 h of admission. Patients with oliguria on admission who had at least one additional 24-h urine output recorded during their ICU stay (n = 1349) were divided into three groups: transient - oliguria resolved within 48 h after the admission day (n = 390 [28.9%]), prolonged - oliguria resolved > 48 h after the admission day (n = 141 [10.5%]), and permanent - oliguria persisting for the whole ICU stay or again present at the end of the ICU stay (n = 818 [60.6%]). ICU and hospital mortality rates were higher in patients with oliguria than in those without, except for patients with transient oliguria who had significantly lower mortality rates than non-oliguric patients. In multilevel analysis, the need for RRT was associated with a significantly higher risk of death (OR = 1.51 [95% CI 1.19-1.91], p = 0.001), but the presence of oliguria on admission was not (OR = 1.14 [95% CI 0.97-1.34], p = 0.103). Conclusions: Oliguria is common in ICU patients and may have a relatively benign nature if only transient. The duration of oliguria and need for RRT are associated with worse outcome.

Abstract

Background: Massive pulmonary embolism (PE) is a deadly disease that can be managed by thrombolytic therapy to save patients' lives. A case of postpartum woman Who presented to the emergency department (ED) with cardiac arrest due to massive PE. Case presentation: We report a case of 34-year-old woman who presented to the ED with cardiac arrest due to massive PE 2 weeks post-uncomplicated spontaneous vaginal delivery. She was managed by a comprehensive multidisciplinary team approach, cardio-cerebral resuscitation (CCR), and systemic thrombolysis. The thrombolysis resulted in massive vaginal bleeding, which led to a prolonged cardiac arrest. This was recognized timely and managed adequately. The patient survived and was discharged home with a good neurological and cardiovascular outcome. Conclusion: Vaginal bleeding may occur post thrombolysis in post-partum life-threatening PE, which can be resolved by a multidisciplinary team-based approach coupled with CCR.

Abstract

Introduction Early identification of a patient with infection who may develop sepsis is of utmost importance. Unfortunately, this remains elusive because no single clinical measure or test can reflect complex pathophysiological changes in patients with sepsis. However, multiple clinical and laboratory parameters indicate impending sepsis and organ dysfunction. Screening tools using these parameters can help identify the condition, such as SIRS, quick SOFA (qSOFA), National Early Warning Score (NEWS), or Modified Early Warning Score (MEWS). We aim to externally validate qSOFA, SIRS, and NEWS/NEWS2/MEWS for in-hospital mortality among adult patients with suspected infection who presenting to the emergency department. Methods and analysis PASSEM study is an international prospective external validation cohort study. For 9 months, each participating centre will recruit consecutive adult patients who visited the emergency departments with suspected infection and are planned for hospitalisation. We will collect patients' demographics, vital signs measured in the triage, initial white blood cell count, and variables required to calculate Charlson Comorbidities Index; and follow patients for 90 days since their inclusion in the study. The primary outcome will be 30-days in-hospital mortality. The secondary outcome will be intensive care unit (ICU) admission, prolonged stay in the ICU (i.e., >72 hours), and 30- as well as 90-days all-cause mortality. The study started in December 2021 and planned to enrol 2851 patients to reach 200 in-hospital death. The sample size is adaptive and will be adjusted based on prespecified consecutive interim analyses. Ethics and dissemination The Aseer Regional Committee for Research Ethics in the General Directorate of Health Affairs-Aseer Region has approved this study. Informed consent is not required for this study due to its complete observational nature and absence of interventions or invasive procedures. We will publish study's results in peer-reviewed journals and may present them at scientific conferences.

Abstract

Background: Intravenous (IV) fluids are a commonly prescribed treatment in the emergency department (ED). Often the prescription of a treatment to be administered in the ED prolongs the patients' length of stay (LOS). We hypothesize that IV fluids are overprescribed in the ED, mainly in patients classified as Emergency Severity Index (ESI) triaged level 3 (T3) and level 4 (T4). Methods: We conducted a quality audit to assess the appropriateness of IV fluid use in adult (age > 16 years) hemodynamically (HD) stable ESI triaged T3 and T4 patients at a tertiary hospital ED during two 6-month periods. Appropriate use of IV fluids was defined from the published literature and our own clinical experience. The primary outcome looked at was the number of patients appropriately prescribed IV fluids before and after implementing a list of indications for IV fluid use in HD stable patients. Results: Analysis showed 62.8% of T3 triaged patients and 74.2% of T4 patients were inappropriately prescribed IV fluids before the implementation of our indication list. This compared similar to 63.6% of T3 triaged patients and 71.4 % of T4 triaged patients after implementation. Median LOS of patients who received IV fluids was 3.5 and 3 hours for T3 and T4 patients, respectively. Conclusion: The data obtained in this quality audit demonstrated no difference in the use of IV fluids before and after implementing a list of indications for fluid therapy in HD stable T3 and T4 patients in our ED. We believe that developing a structured education program for caregivers in addition to patient education may help improve utilization of IV fluid based on true clinical indication.

Abstract

Ca2+ signaling is vital for the proper functioning of all cells, including cells of the cardiovascular system. Membrane receptors for many hormones trigger intracellular Ca2+ signaling via the activation of phospholipase C and production of inositol-1,4,5-trisphosphate (InsP3). Several research groups have demonstrated the expression of oxytocin (OXT) and oxytocin receptors (OXTR) in the heart and suggested a cardioprotective role of OXT against several pathological conditions.
