Aim: We address the relative importance of biogeographic regions (areas of endemism), regional climate and spatial factors on taxonomic, functional and phylogenetic beta diversity of bird assemblages in upland terra‐firme forests in the Amazon. Location: Amazon biome. Taxon: Birds. Methods: We gathered 27,498 occurrence records of 873 bird species for 115 bird assemblages distributed in all nine biogeographic regions delimited by the major Amazonian rivers. Only data from studies surveying whole communities with standard sampling methods and exhaustive sampling effort were included. We partitioned the fractions of taxonomic, functional and phylogenetic beta diversity explained by climate, biogeographic regions, spatial factors and the variation shared between them. Results: Across the entire Amazon, we found higher total taxonomic (0.68 ± 0.10) than phylogenetic (0.46 ± 0.08) β‐diversity, and low functional β‐diversity (0.34 ± 0.08). Biogeographic regions showed the highest unique contributions explaining taxonomic and phylogenetic β‐total (7% and 5%, respectively) and turnover components (7% of β‐jtu taxonomic, 7% of β‐jtu phylogenetic), but accounted for only 2% of total functional β‐diversity and trait turnover. Climatic variables explained slightly more trait nestedness (5%) compared with species and lineage turnover (3% each). Species composition was clearly distinct between biogeographical regions limited by the Amazon River, but support for the effect of other rivers was mixed. Main Conclusions: Our study highlights the significant influence of biogeographic regions and climate on species composition in the Amazon. However, our findings also suggest that the Amazon River exerts a more pronounced impact on species distribution compared with other major Amazonian rivers. Species turnover across biogeographical regions is particularly evident at the species level, with minor effects observed in functional traits, suggesting that taxonomic turnover is driven mainly by functionally redundant species. In addition, recent diversification events, occurring predominantly at the tips of the phylogeny without substantial turnover at deeper nodes, are likely responsible for the patchy distribution of species across Amazonia. The new onset of abdominal pain in a constipated patient is usually related to this problem but we should keep in mind differential diagnosis. A 9-year-old girl with history of alopecia, iron deficiency anemia and constipation, presented to the emergency department with colic epigastric pain, anorexia, vomiting, no emission of stools and no fever. She appeared pale and malnourished. The evaluation revealed a painless, hard, palpable mass in the epigastric region. Abdominal ultrasound evaluation was limited by gas trapping. The abdominal CT scan revealed an ileoileal intussusception and a gastric heterogeneous endoluminal mass, identified in the upper gastrointestinal endoscopy as a large trichobezoar, only possible to remove with gastrotomy. Another small bezoar at the jejunal level conditioned intussusception, treated with resection and anastomosis. Trichobezoars are rare in childhood. In most cases, as in this one, it reveals a psychiatric disorder that must be treated to prevent recurrence. [ABSTRACT FROM AUTHOR]

The influence of the ActiGraph® processing criteria on estimating step counts in chronic obstructive pulmonary disease (COPD) remains uncertain. This study aimed to assess the influence of filters, epoch lengths and non-wearing time (NWT) algorithms on steps/day in people with COPD. ActiGraph GT3X+ was worn on the waist for seven days. Steps were detected using different filters (normal and low-frequency extension [LFE]), epoch lengths (15s and 60s), and NWT algorithms (Choi and Troiano). Linear mixed-effects model was applied to assess the effects of filter, epoch length, NWT algorithm on steps/day. Lin's concordance correlation and Bland-Altman were used to measure agreement. A total of 136 people with COPD (107 male; 69 ± 8 years; FEV1 51 ± 17% predicted) were included. Significant differences were found between filters (p < 0.001), but not between epoch lengths or NWT algorithms. The LFE increased, on average, approximately 7500 steps/day compared to the normal filter (p < 0.001). Agreement was poor (<0.3) and proportional bias was significant when comparing steps/day computed with different filters, regardless of the epoch length and NWT algorithm. Filter choice but not epoch lengths or NWT algorithms seem to impact measurement of steps/day. The objective of this study was to explore the feasibility and preliminary effectiveness of the Lifestyle-Integrated Functional Exercise for People with Dementia (LiFE4D) on health-related physical fitness, cognitive function, physical activity, and respiratory and upper limb functions. A randomized controlled pilot study was conducted (control group: usual care; experimental group: usual care and LiFE4D). The feasibility of LiFE4D was determined considering recruitment, protocol acceptability, adherence, and safety. Measures of health-related physical fitness, cognitive function, physical activity, and respiratory and upper limb functions were assessed at the baseline and 3 months. Twelve participants (8 [66.7%] female, 82 [72.2–84] years) were included, six per group. Recruitment was challenging. LiFE4D was acceptable with excellent adherence and no major adverse events. Cardiorespiratory endurance (effect size = 1.64, 95% confidence interval [CI; 0.33, 2.95]) and balance (effect size = 1.46, 95% CI [0.19, 2.73]) improved after LiFE4D. LiFE4D seems to be feasible and safe, and it shows potential to significantly improve the health-related physical fitness of people with dementia. [ABSTRACT FROM AUTHOR]
