Background: As the demand for digital dentistry constantly increases, digital devices are gradually replacing conventional methods of recording occlusal contacts. The study aimed to assess the inter-rater reliability of occlusal contact point detection using 40 μm articulating paper, Medit i700, and OccluSense and to compare the distribution of occlusal contacts using the articulating paper and intraoral scanner. Material and Methods: The study included 25 participants aged 20 to 30 (13 women and 12 men). Photographs of contact points were taken and marked in maximum intercuspal position (MIP), in protrusive and laterotrusive movements, on working and non-working sides using 40 μm articulating paper and digital devices. The Cohen's Kappa coefficient assessed the inter-rater reliability. The Wilcoxon signed-rank test was used to compare dependent groups, articulating paper, and Medit i700. Results: The Cohen's Kappa index showed that almost perfect agreement was achieved with 40 μm articulating paper. Compared to Medit i700, the 40 μm articulating paper showed an increased mean number of contacts per tooth, except for the third molars. Conclusions: The 40 μm articulating paper has detected more overall contacts than the digital devices, particularly in the posterior areas. The current study aimed to evaluate the usefulness of cone beam computed tomography (CBCT) examination in cleft children and adolescents, the age of the first CBCT exposure, and the criteria that justified the first CBCT exposure. A number of 229 non-syndromic cleft subjects aged between 0-22 years receiving treatment in the same specialized surgical center in orofacial cleft treatment were studied. A cleft group of 64 cleft lip and palate (CLP) children with at least one CBCT exposure was identified based on CBCT records. Parameters related to diagnosis and treatment planning in cleft deformity were considered dental anomalies and bone morphology changes. The examiners assessed whether the treatment option was necessary, not necessary, or could not be evaluated at the age of CBCT exposure. A number of thirty-nine unilateral clefts and fifteen bilateral clefts were identified. Posterior palatal cleft was present in six children (mean age: 15.03±5.55 years; male/female ratio: 1/0.8). Alveolar bone morphology changes were found in 90.58% of cases; jaw relationship changes in 71.82%; nasal fossa morphology changes in 74.99%; airway obstruction in 49.98% and maxillary asymmetry in 87.48%. Orthodontic planning was necessary for 85.93% of CLP patients, and orthognathic surgery in 39.05%. The usefulness of CBCT in patients with CLP varied with age, with reduced value for the evaluation of impaction and root resorption under the age of ten. [ABSTRACT FROM AUTHOR]

8. Evaluation of Cause, Severity, Complications, and Treatment of Maxillary Canine Impaction: Dental Specialists' Perspectives.

Aim: The purpose of this study was to evaluate dental specialists' perspectives on the causes, severity, treatments, and complications of maxillary canine impaction. Methods: A non validated questionnaire was designed and used to gather information from orthodontists, prosthodontists, surgeons, and general dentists, between June and July 2022. The survey was posted online and advertised on the official website of Cluj Dentists' College. Practitioner's experience with tooth impaction, the cause of the impaction, the type of radiographic investigation, treatment options, and a rating of the degree of treatment difficulty and complications using panoramic radiography and cone-beam computed tomography (CBCT) were collected. Results: One hundred and nine dentists participated. In their practice, the maxillary canine was the most frequently encountered impacted tooth (85.32%); the most frequent etiology of impaction was lack of space (95.4%). The difficulty of a case of maxillary canine impaction on CBCT was considered lower by surgeons vs. general dentists (-30.52, 95% CI (- 60.31 to -0.74), p=0.047), in case of distance from canine cusp tip to the occlusal plane is considered in interceptive treatment (25.56, 95% CI (2.09 to 49.02), p=0.035), and decreased in case of deciduous canine extraction is considered to aid treatment (-27.15, 95%CI (-51.38 to -2.93), p=0.03). Conclusions: Differences between specialists' level of knowledge was observed, the orthodontists being followed by general dentists, surgeons, and prosthodontists. The difficulty of a case was rated higher on CBCT than on panoramic radiography. The difficulty of a case was perceived to be lower by surgeons compared to general dentists. [ABSTRACT FROM AUTHOR]

To analyze the location and intensity of occlusal contact points using three types of articulators: non adjustable, semiadjustable and digital with aiming at improving the diagnostic and treatment options in dental medicine. Material and method. For analyzing the distribution of contact points, the casts of a patient were mounted in the non adjustable and semiadjustable articulator. Intraoral scanning was performed using an intraoral scanner (Trios 3Shape) and reviewed in a virtual articulator. Occlusion obtained by the three methods was compared to the clinical situation. Results. Contact points in maximum intercuspation, propulsion and lateral movements were analyzed. The points obtained by using the non adjustable articulator have been less intense and more unprecise. By digitizing the con-tact points, the image becomes more accurate and sharp. Discussion. The semiadjustable articulator reproduces the contact points which are consistent with the clinical situation. Major differences occur when using the non adjustable articulator, which has a limited capacity of repro-ducing the clinical movements, therefore the marks are non consistent with the real clinical situation. The digital articulator seems promising in terms of eccentric movements. Conclusions. However performing an articulator may be, the clinical maximum intercuspation will never be fittingly reproduced, due to the fact that articulators are rigid systems, whereas the oral cavity has an elasticity, resulting from the mandible, teeth and periodontal ligaments. Introduction: The aims of this study were to analyze the association among (sleep and awake) bruxism, (static and dynamic) occlusion and temporomandibular joint modifications in healthy adults. Materials and methods: 60 healthy subjects (33 females and 27 males), 30 of them with bruxism, were investigated. Bruxism type, joint pain, muscle pain and/or fatigue, centric relation, dental wear, static and dynamic occlusion were analyzed. Results and discussion: Muscle pain and fatigue sensation of the masticator apparatus were higher in subjects with bruxism than in those without bruxism (3.23 vs 1.46). Joint noises and joint pain occurred with increasing frequency in subjects with bruxism. Sleep bruxism is associated with mandible lateral shift during mouth opening, joint pain and non-functional lateral guidance. A lack of occlusal involvement was found in the etiology of awake bruxism. Conclusions: Sleep bruxism is related with non-functional occlusion, while awake bruxism showed no occlusal interaction, suggesting the necessity of a different therapeutic approach. Introduction. The objectives of this study were to assess the relationship between centric relation-habitual occlusion discrepancy and temporomandibular disorder symptoms, the spatial planes in which the discrepancy is encountered and the correlation between gender and muscle pain. Materials and methods. Thirty subjects (14 females, 16 males), aged between 20-30 years, were clinically evaluated, as follows: muscle (temporalis, masseter, lateral and medial pterygoid) and temporomandibular joint examination, occlusal (static and dynamic) relationships, centric relation. Casts were mounted in centric relation using an articulator and then three-dimensionally analyzed. Results and discussion. Muscle pain was found in 57.14% females and 37.5% males. Discrepancy was higher in subjects with pain in all planes (transversal 0.92 mm vs. 0.26 mm [correlation coefficient=0.897]; sagittal 0.94 mm vs. 0.85 mm [correlation coefficient=0.249]; vertical 1.15 mm vs. 0.36 mm [correlation coefficient=0.504]) and similar in males and females. Conclusions. Transversal discrepancy was associated with muscle pain. Muscle pain and discrepancy were not correlated with gender. [ABSTRACT FROM AUTHOR]

Temporomandibular disorders (TMDs) are characterized by numerous pain manifestations. Their treatment often involves the use of an oral splint. Recent research has found a relationship between migraines, nociceptive pain and TMDs. The aim of the present study was to perform a scoping review of studies in order to evaluate the effectiveness of the various types of oral splint in the treatment of migraine or nociceptive pain. Publications were retrieved from seven databases (PubMed, Web of Science, EMBASE, Scopus, ProQuest, SpringerLink and Ovid). Out of the 15 included publications, three studies were before and after studies, with no control group, whereas the other twelve studies were clinical trials, among which two publications were crossover studies. A clear, single distinction of pain was difficult to describe. Therefore, numerous publications focused on a combination of various types of pains, including myofascial, temporomandibular joint, headaches and migraine-like symptoms, all of which mimicked TMD pain. Overall, six studies used the stabilization splint (SS), three explored the comparison between the SS and the nociceptive trigeminal inhibition splint (NTIS) and two the NTIS. The majority of publications reported a positive outcome of splint therapy. Regarding the type of oral splint usage, the most commonly used one was the SS, followed by the NTIS. The definition and assessment of pain were heterogenous in the identified articles. The findings of the current study showed that occlusal splints may help with pain management, and that effective treatment of TMD-related pain at an early stage can enhance the quality of life of patients. Histological changes in bone structure were induced at 12 weeks by experimental administration of bisphosphonates, which have been associated with osteonecrosis of the jaws. The purpose of this study was to determine the effect of local administration of bisphosphonate on bone formation in rats. Surgically created bone defects were evaluated at 12 weeks by histological examination after bisphosphonate administration. Fifteen Wistar rats that underwent surgery to create a bone defect at the right femur were divided in 3 groups: 2 experimental groups and a control group. The 1st experimental group received bisphosphonate as a single 1-mL dose into the bone defect, the 2nd experimental group received 1 mL of bisphosphonate as a 10-day fractionated dose, and the control group comprised rats who did not receive any bisphosphonate. At 12 weeks after the injection, new bone tissue was collected and a histological examination was performed. At 12 weeks after the bisphosphonate administration, bone repair processes were found in both the experimental and control groups. The bone continuity in the intervention area was ensured by a continuous layer of bone tissue. Compared with the control, the repair processes were significantly more advanced in the experimental groups, the layer of bone tissue covering the bone defect being thicker and more consolidated and the trabeculae from the Haversian canal being thicker and denser. External and internal consolidation structures were present in animals from all of the groups. Bisphosphonate had a positive influence on bone formation. The stimulatory effect of the bisphosphonate increased with the number of administered doses. [ABSTRACT FROM AUTHOR]

The experimental group comprised 15 subjects with TMD; the control group included 10 subjects without TMD. TMD signs and symptoms were evaluated according to research diagnostic criteria for temporomandibular disorders (RDC/TMD). Malocclusion severity was assessed by the dental health component (DHC) of the orthodontic treatment need index (IOTN). Skeletal pattern analysis included: skull base, maxilla-mandible relationships (SNA, SNB and FMA angle), dental relationships (overjet, overbite). Results. The SNA and SNB angles were lower in the experimental group; FMA angle was by 4° higher (p<0.05) than in the control group. The degree of overbite and overjet were increased by 2 mm and 1.6 mm in the experimental group (p<0.05). Conclusions. The temporomandibular joint status should be considered in planning orthodontic treatment in patients with skeletal class III malocclusions, skeletal pattern may represent a risk factor for temporomandibular joint disorders. The diagnosis was confirmed by magnetic resonance imaging in open/close position (FSE - fast spin echo, repetition time TR-1420 msec, echo time TE-22.3 msec, FOV-14x14 cm). Purpose: The aim of this study was to evaluate the number of maxillofacial fractures and their characteristics in trauma cases based on imaging investigations. Materials and methods: A retrospective, analytical, and observational study was performed. The research was conducted over a one-and-a-half- year period, between January 2017 to August 2018. The inclusion criteria were the presence of fracture on a radiological examination. The exclusion criteria were the absence of a maxillofacial fracture as well as patients with incomplete data. Age, gender, place of residence, diagnosis, concomitant fractures, the origin of trauma, imaging examinations, type of surgical therapy, and the number of days of hospitalization were assessed. Results: The data for this study originated from the results of observations of 139 patients hospitalized at the Oral and Maxillofacial Surgery Clinic. The sample's mean age was 35.53 years (SD: 17.45 years), and 86.3% of the subjects were males. Seventyone cases (51.08%) were from urban areas. Fracture of the mandible (n=78; 56.1%) was the most common diagnosis, followed by fracture of the zygomatic bone (n=33; 23.74%). The most common associated fractures were fractures of the mandible in multiple locations (37.05%) and the LeFort fracture type (n=7, 5%). Interhuman violence was the most prevalent etiological cause (48.2%), followed by accidental falls (23%). For the duration of the patients' hospitalization, the mean occupancy rate was 3.31 days. Treatment comprised closed fracture reduction (41.2% of patients), followed by open reduction and immobilization in 29% of the subjects. Conclusions: Implementation of current treatment approaches and developing prevention strategies are essential for trauma outcomes and patients' quality of life. [ABSTRACT FROM AUTHOR]

Background: Receiver Operator Characteristic (ROC) curve, is a graphical plot which presents the performance of a binary classifier when the discrimination cutoff is varied. The aim of this work was to create an extension for R Commander that offers a graphical user interface for Receiver Operator Characteristic tools provided by several existing command line accessible packages like pROC and ROCR. Material and Methods: The extension was built and tested with R version 3.2.0 and R Commander 2.1-7. Results: We built an extension called RcmdrPlugin.ROC that we uploaded on the CRAN servers. The extension adds a new menu called ROC, along with two submenus pROC and ROCR that broadly corresponds to commands available to access the functions of these packages. The pROC menu offers several commands: to plot a ROC curve for a dataset or for a logistic regression model, to compare paired and unpaired ROC curves, each providing the following tabs: General (to select the variables for the analysis, and options for switching cases with controls); Smoothing (allowing the user to select different types of smoothing -- binominal, density, distributions like normal, lognormal, ...); AUC (to specify the partial area under the curve (AUC) options), CI (to select the options of confidence intervals (CI) -- the level, computing method: DeLong, bootstrap, ...); Plot (for the plotting options). The ROCR dialogue window offers more options in choosing the performance measures for the plot. Aims. The aim of this paper was to assess the occlusal characteristics of a sample of patients with temporo-mandibular dysfunction. Materials and methods. 40 patients were included in this study, divided into two groups: in the first group, 20 patients were recruited from a private clinic in Cluj-Napoca all of them being diagnosed with temporo-mandibular dysfunction (TMD); the second group - the control group - included 20 subjects, chosen randomly, in the Department of Prosthodontics, Faculty of Dentistry. The inclusion criteria for the first group were: patients with TMD signs and symptoms, no history of trauma, no other organic sources of pain, no associated neuro-muscular diseases. The inclusion criteria for the control group were: clinically healthy patients, no history of trauma, no general history of disease with interest in the structures of the dento-maxillary apparatus, no orthognathic surgery and no ongoing orthodontic or prosthodontic treatment. Patients were examined clinically and the data obtained were statistically analyzed. Results. Statistical analysis revealed several significant results, such as the fact that subjects with TMDs had more frequent muscle pain (p = 0.018) and joint pain (p = 0.004). Also, for patients suffering from disc displacements without reduction, the presence of passive interference in protrusion was observed more often (p = 0.049), and for those with disc displacements with reduction, passive interference in laterality was more frequent (p = 0.010). Discussion. In the present study, interferences in maximum intercuspation (MI) and passive interferences in protrusion were found more frequently in the group of subjects with TMDs, which is consistent with the results in the literature. For subjects with disc displacements, significant differences were found between those disc displacements with reduction and, respectively, without reduction, regarding the passive interference in laterality and passive interference in protrusion, which may be due to the change of position of the articular disc and the trajectory of condylar movement during performing mandibular movements. Conclusions. No correlation has been shown between a certain type of occlusal dysfunction and a certain symptom of temporo-mandibular dysfunction. There are subjects who have occlusal dysfunctions without showing dysfunctional symptoms. Malocclusions may be correlated with dysfunctional signs. [ABSTRACT FROM AUTHOR]

Temporomandibular joint disorders cause the most common affections among adult population. Diagnosing them is an always actual research theme. Clinical examinations, very important regarding the study of temporomandibular joint disorders, have been completed during the years by different means of supplementary examination, such as axiograph, radiograph, artrograph, tomography, MRI, sonograph, T-scan etc. The purpose of this study was to compare the axiograph to the cone beam computerized tomography (CBCT). Due to the fact that the axiograph is a dynamic recording of the mandibular kinematics, and the CBCT is a static recording, with the mouth closed, intermediary and with the mouth open, establishing a bone fixed point and tracing a curve between them, mathematically determined. The study allowed the comparison of the two paths, leading to the conclusion that although the CBCT is very useful in diagnosing temporomandibular joint disorders, it cannot be used to establishing the condil's path during the functional movements; but the axiograph remaining to be used as far as precision and election are concerned. The complexity of the functional movements of the TMJ as well as the fact that the dysfunctional states are more and more frequent, have lead to the appearance of specific supplementary means of diagnosis. Among these means there is the axiography, the analysis of functional mandibular movements and modern variant, the computerized axiography. In the present article, our purpose is to present the principles that lie the base of the axiography, and also the practical steps of registering the functional mandibular movements. The computerized axiography has been developed by different manufacturers, in our case, we will present the Cadiax variant of Gamma Co, Vienna, Austria. This system was developed in two ways: the Compact (the base variant) and the Diagnostic (the complete variant). The system described by us is in this article is the Cadiax Diagnostic one. The usage of the computerized axiography - the Cadiax system, presents the numerous advantages, which reflect into the multiple choices offered by this machine, concerning the programming of the articulator, the functional analysis of mandibular movements, the documented diagnosis of the dysfunctional states of Temporo Mandibular Joint and the planning of the treatment course. 27. Therapeutic challenges in temporomandibular disorders.

- Abstract
Background and Aims: This study aimed at evaluating the etiology and treatment challenges of temporomandibular disorders (TMDs)., Methods: 160 subjects with TMDs, 38 males (23.8%) and 122 females (76.3%) were studied. A personalized coefficient was designated, which included the resolution of the main symptom, correction of secondary symptoms, patient collaboration (emotional parameter), treatment duration, and cost., Results: The most frequent cause for consultation was muscle impairment (42.5%), or limitation of mouth opening, followed by joint impairment (23.1%). Muscle pain was noticed, particularly in the masseter (57.5%) and lateral pterygoid muscles (51.9%). Tooth pain or gingival retraction was frequently associated with tooth wear (48.1%) and dental abfraction (31.3%). Remote symptomatology was dominated by otologic symptomatology. Iatrogenic etiology was highest (69.4%), followed by untreated missing teeth (66.9%). Treatment options included muscle relaxation, occlusal balancing (equilibration), kinesitherapy, medication, and swallowing re-education. Most patients benefited from four to seven different types of therapy, which resulted in a higher cost and a longer and more uncomfortable treatment. The primary symptom was relieved in 82.3% of cases, with recurrence occurring in 15.7%., Conclusion: The treatment of temporomandibular joint dysfunction is time-consuming, demanding, and intricate. Most patients required four to seven different types of therapy, which increased the expense, treatment duration, and suffering.

28. Dental occlusion characteristics in subjects with bruxism.

- Abstract
Background and Aims: Bruxism is associated with a variety of factors, some of which are oral (occlusion, joint, face shape) while others are of a systemic nature (respiratory, cardiac, neurotransmitters, stress). The relationship between bruxism and occlusion has received great attention, but it still has a lot of ambiguity. This study aimed at investigating the parameters that may affect bruxism: dental interferences or premature contacts, vicious habits, temporomandibular joint (TMJ) pain, TMJ noises, TMJ morning fatigue, snoring, obstructive sleep apnea, which may occur before bruxism, or because of chronic parafunction., Methods: We conducted an observational, analytical case-control study to determine the association between bruxism and oral cavity factors as well as general factors. Participants were chosen from a group of middle-aged Romanian population. An online questionnaire was used for data collection. A total of one hundred subjects were included in the sample. Fifty cases with bruxism were chosen as a study group, and another fifty persons of similar ages were chosen as a control group. Statistical analysis was performed using the MedCalc version 20.110 software., Results: The study comprised subjects between 18 and 42 years old. The most prevalent age range was between 20-25 years. The female-male ratio was almost similar; 62% of the participants had sleep bruxism, 10% had awake bruxism and 28% had a combined form. The arithmetic means of the daily stress levels for both research groups' scores were 3.0 for the bruxism group and 3.24 for the control group, while the work stress was 3.04 for the bruxism group and 3.41 for the control group. Dental interferences or premature contacts, vicious habits, TMJ pain, TMJ noises, TMJ morning fatigue, snoring, and obstructive sleep apnea were evaluated. The majority of subjects received an oral splint (n=26)., Conclusions: Bruxism was associated with stress, occlusal parameters (premature contacts and occlusion interferences), and joint pathologies. Bruxism was not found to be positively correlated with obstructive sleep apnea syndrome. There was a moderately significant association between bruxism and snoring. There was no evidence of an association between bruxism and vicious habits, dental aesthetics, or obstructive sleep apnea. Treatment significantly improved the quality of life, the masticatory and joint functions.

29. Oral splints in the management of nociceptive pain and migraines: A scoping review.

- Abstract
Temporomandibular disorders (TMDs) are characterized by numerous pain manifestations. Their treatment often involves the use of an oral splint. Recent research has found a relationship between migraines, nociceptive pain and TMDs. The aim of the present study was to perform a scoping review of studies in order to evaluate the effectiveness of the various types of oral splint in the treatment of migraine or nociceptive pain. Publications were retrieved from seven databases (PubMed, Web of Science, EMBASE, Scopus, ProQuest, SpringerLink and Ovid). Out of the 15 included publications, three studies were before and after studies, with no control group, whereas the other twelve studies were clinical trials, among which two publications were crossover studies. A clear, single distinction of pain was difficult to describe. Therefore, numerous publications focused on a combination of various types of pains, including myofascial, temporomandibular joint, headaches and migraine-like symptoms, all of which mimicked TMD pain. Overall, six studies used the stabilization splint (SS), three explored the comparison between the SS and the nociceptive trigeminal inhibition splint (NTIS) and two the NTIS. The majority of publications reported a positive outcome of splint therapy. Regarding the type of oral splint usage, the most commonly used one was the SS, followed by the NTIS. The definition and assessment of pain were heterogenous in the identified articles. The findings of the current study showed that occlusal splints may help with pain management, and that effective treatment of TMD-related pain at an early stage can enhance the quality of life of patients., Competing Interests: The authors declare that they have no competing interests., (Copyright: © Almăşan et al.)

- Abstract
Bisphosphonates, stable analogues of pyrophosphate, have the ability to bind to bone molecules, possessing anti-resorbtion properties influenced by the radicals linked to the carbon group of their structure. Bisphosphonates link to the hydroxyapatite of the mineral surfaces and are selectively internalized by osteoclasts, whose activity they inhibit, jeopardizing the osteoblastic activity. The purpose of this study is to determine the influence of intramedular administration (at the hip bone) of bisphosphonates on the serum values of alkaline phosphatase, total Ca, Ca2+, proteins and serum osteocalcin in a lot of experience Wistar rats. Fifteen Wistar rats of experience, five in the control group and 10 in the experimental group. All rats underwent surgery to create a bone defect with a 1.5 mm diameter bone-bur at the right femur transcortical through the medullar canal. Rats from experimental group were divided into two groups: group A, who received Zometa 1 mL single dose intramedular, intraoperative and group B, who received Zometa 1 mL in divided doses daily, 0.1 mL for 10 days. 3 mL of blood from the frontal sinus were collected from each subject at 24 hours, 14 days and 21 days postoperatively. From the blood samples were determined: alkaline phosphatase [U/L], seric proteins [g/dL], total Ca [mmol/L, mg/dL], osteocalcin [mmol/L]. The data were statistically analyzed using the ANOVA test. We found an increase in alkaline phosphatase [U/L] in all groups studied. In group B there was a significant decrease in total Ca levels [mg/dL] throughout the experiment compared with controls (11.82→10.36→9.25 mg/dL; 2.95→2.59→2.31 mmol/L; p=0.001). Ca2+ has changed significantly both in group A (1.18→1.25→1.25 mmol/L; p=0.01) and group B (1.21→1.24→1.13 mmol/L; p=0.02). Serum proteins were significantly reduced both in the control group (9.4→8.5→8.1 g/dL; p=0.03) and the experimental groups A (9.3→8.5→8.3 g/dL; p=0.01) and B (9.9→7.6→7.3 g/dL; p=0.0008). At each stage of bone development, multiple factors act in a coordinated manner that leads to increased local metabolic processes, acting both on the process of bone resorption and bone repair. Healing processes are initiated within 24 hours in both studied groups and the control group; at 14 and 21 days the bone healing processes are compromised directly proportional to the administration manner and dose of bisphosphonates.

Unlabelled: Anchorage represents a great issue that challenged orthodontists all over the years. Nowadays, with the use of microimplants, it has become possible even to induce complex teeth movements., Aim: To present a clinical case in which mesialisation of upper second molars was performed by the means of using miniimplants., Results: The present case shows how a bilateral mesialization of upper second molars using skeletal anchorage was performed by orthodontic treatment using fixed appliances and miniimplants., Conclusion: The placing site of the miniimplants plays a huge role in the moments of force produced and delivered to the teeth, thus influencing tooth movement. Miniimplants increase tooth movement control, by the use of skeletal anchorage. The success of their usage depends upon its good primary stabilization and a judicious loading.
