Modifiable (biomechanical and neuromuscular) anterior cruciate ligament (ACL) injury risk factors have been identified in laboratory settings. These risk factors were subsequently used in ACL injury prevention measures. Due to the lack of ecological validity, the use of on-field data in the ACL injury risk screening is increasingly advocated. Though, the kinematic differences between laboratory and on-field settings have never been investigated. The aim of the present study was to investigate the lower-limb kinematics of female footballers during agility movements performed both in laboratory and football field environments. Twenty-eight healthy young female talented football (soccer) players (14.9 ± 0.9 years) participated. Lower-limb joint kinematics was collected through wearable inertial sensors (Xsens Link) in three conditions: (1) laboratory setting during unanticipated sidestep cutting at 40-50°; on the football pitch (2) football-specific exercises (F-EX) and (3) football games (F-GAME). A hierarchical two-level random effect model in Statistical Parametric Mapping was used to compare joint kinematics among the conditions. Waveform consistency was investigated through Pearson’s correlation coefficient and standardized z-score vector. In-lab kinematics differed from the on-field ones, while the latter were similar in overall shape and peaks. Lower sagittal plane range of motion, greater ankle eversion, and pelvic rotation were found for on-field kinematics (p < 0.044). The largest differences were found during landing and weight acceptance. The biomechanical differences between lab and field settings suggest the application of context-related adaptations in female footballers and have implications in ACL injury prevention strategies. Highlights Talented youth female football players showed kinematical differences between the lab condition and the on-field ones, thus adopting a context-related motor strategy. Lower sagittal plane range of motion, greater ankle eversion, and pelvic rotation were found on the field. Such differences pertain to the ACL injury mechanism and prevention strategies. Preventative training should support the adoption of non-linear motor learning to stimulate greater self-organization and adaptability. It is recommended to test football players in an ecological environment to improve subsequent primary ACL injury prevention programmes.

Context: Up to 90% of pediatric athletes return to sport (RTS) after anterior cruciate ligament reconstruction (ACL-R); however, Objectives: (1) Determine which physical and patient-reported outcome measures guide clinical decision-making on RTS in pediatric athletes after ACL-R and (2) present a framework with insights from cognitive and neurophysiological domains to enhance rehabilitation outcomes. Data Sources: PubMed, CINAHL, Embrase, and Cochrane library databases and gray literature. Study Selection: Data on pediatric (Study Design: Scoping review. Level of Evidence: Level 4. Data Extraction: Details on study population, aims, methodology, intervention, outcome measures, and important results were collected in a data chart. Results: Studies included 4456 patients (mean age, 14 years). Quadriceps and hamstring strength (n = 25), knee ligament arthrometer (n = 24), and hop tests (n = 22) were the most-reported physical outcome measures guiding RTS in Conclusion: RTS clearance in pediatric ACL-R patients is not based on clear criteria. If RTS tests were performed, outcomes did not influence time of RTS. Postoperative LSI thresholds likely overestimate knee function since biomechanics are impaired despite achieving RTS criteria. RTS should be considered a continuum, and biomechanical parameters and contextual rehab should be pursued with attention to the individual, task, and environment. There is a need for psychological monitoring of the ACL-R pediatric population.

Background The outcome after ACL reconstruction (ACLR) is in general disappointing with unacceptable number of athletes that do not return to pre-injury level of sports, high re-injury rates, early development of osteoarthritis and shorter careers. Athletes after ACLR have high expectation to return to sports which is in contrast with the current outcomes. The aim of this manuscript is to present an overview of factors that are needed to be incorporated and to personalize the rehabilitation process for an athlete who has undergone an ACLR. Level of evidence 4.

Background: Limited information is available on the experiences of patients during rehabilitation after anterior cruciate ligament reconstruction (ACLR). Aim: The current study aimed to identify factors that differentiated positive and negative patient experiences during rehabilitation after ACLR. Method and Design: A survey-based study with an online platform was used to identify factors that differentiated positive and negative patient experiences during rehabilitation after ACLR. Seventy-two patients (age 27.8 [8.8] y) after ACLR participated. Data were analyzed and themes were identified by comparing categories and subcategories on similarity. Main Findings: Positive patient experiences were room for own input, supervision, attention, knowledge, honesty, and professionalism of the physiotherapist. Additionally, a varied and structured rehabilitation program, adequate facilities, and contact with other patients were identified as positive patient experiences. Negative experiences were a lack of attention, lack of professionalism of the physiotherapists, a lack of sport-specific field training, a lack of goal setting, a lack of adequate facilities, and health insurance costs. Conclusions: The current study identified factors that differentiated positive and negative patient experiences during rehabilitation after ACLR. These findings can help physiotherapists in understanding the patient experiences during rehabilitation after ACLR.

Wearable inertial sensors (WIS) facilitate the preservation of the athlete-environment relationship by allowing measurement outside the laboratory. WIS systems should be validated for team sports movements before they are used in sports performance and injury prevention research. The aim of the present study was to investigate the concurrent validity of a wearable inertial sensor system in quantifying joint kinematics during team sport movements. Ten recreationally active participants performed change-of-direction (single-leg deceleration and sidestep cut) and jump-landing (single-leg hop, single-leg crossover hop, and double-leg vertical jump) tasks while motion was recorded by nine inertial sensors (Noraxon MyoMotion, Noraxon USA Inc.) and eight motion capture cameras (Vicon Motion Systems Ltd). Validity of lower-extremity joint kinematics was assessed using measures of agreement (cross-correlation: XCORR) and error (root mean square deviation; and amplitude difference). Excellent agreement (XCORR >0.88) was found for sagittal plane kinematics in all joints and tasks. Highly variable agreement was found for frontal and transverse plane kinematics at the hip and ankle. Errors were relatively high in all planes. In conclusion, the WIS system provides valid estimates of sagittal plane joint kinematics in team sport movements. However, researchers should correct for offsets when comparing absolute joint angles between systems.

Inertial measurement units (IMUs) allow for measurements of kinematic movements outside the laboratory, persevering the athlete-environment relationship. To use IMUs in a sport-specific setting, it is necessary to validate sport-specific movements. The aim of this study was to assess the concurrent validity of the Xsens IMU system by comparing it to the Vicon optoelectronic motion system for lower-limb joint angle measurements during jump-landing and change-of-direction tasks. Ten recreational athletes performed four tasks; single-leg hop and landing, running double-leg vertical jump landing, single-leg deceleration and push off, and sidestep cut, while kinematics were recorded by 17 IMUs (Xsens Technologies B.V.) and eight motion capture cameras (Vicon Motion Systems, Ltd). Validity of lower-body joint kinematics was assessed using measures of agreement (cross-correlation: XCORR) and error (root mean square deviation and amplitude difference). Excellent agreement was found in the sagittal plane for all joints and tasks (XCORR > 0.92). Highly variable agreement was found for knee and ankle in transverse and frontal plane. Relatively high error rates were found in all joints. In conclusion, this study shows that the Xsens IMU system provides highly comparable waveforms of sagittal lower-body joint kinematics in sport-specific movements. Caution is advised interpreting frontal and transverse plane kinematics as between-system agreement highly varied.

Background: The incidence of anterior cruciate ligament (ACL) injuries is commonly reported as an annual incidence rate. There is relatively little information about the seasonal aspects of these injuries. The aim of the current study was to analyze the distribution of ACL injuries during the season in nonprofessional soccer, handball, and basketball based on a retrospective analysis of a hospital-based registry. Hypothesis: ACL injuries in soccer, handball, and basketball were more common within the first 2 months of the season in comparison with the rest of the year. Study Design: Case series. Level of Evidence: Level 4. Methods: Injury occurrence during the calendar year was divided into 6 periods of 2 months, with segment 1 (S1) representing the first 2 months of the season. For soccer, S1 corresponded to September and October. The season started 1 month later for handball and basketball, so S1 represented October and November. Chi-square tests were used to analyze the distribution of ACL injuries among segments according to gender, age, sports, and injury mechanism (contact/noncontact). Results: A total of 371 ACL injuries were included (soccer, 258, handball, 56, basketball, 57). Overall, the distribution of ACL injuries was not uniform across the segments ( P < 0.01). Almost one-third of the ACL injuries occurred in S1 (n = 104; 28%). Significant differences could be observed according to sports ( P < 0.01). There were fewer ACL injuries in S2 for soccer compared with basketball ( P < 0.05). In S5, there were significantly more ACL injuries in soccer compared with handball and basketball ( P < 0.05). Conclusion: A high occurrence of ACL injuries was reported immediately within the first 2 months of the season in nonprofessional soccer, handball, and basketball sports. Clinical Relevance: These findings indicate that ACL injury prevention programs should be started in the preseason period to allow for gradual increases of load.

Agility, a key component of team ball sports, describes an athlete´s ability to move fast in response to changing environments. While agility requires basic cognitive functions like processing speed, it also requires more complex cognitive processes like working memory and inhibition. Yet, most agility tests restrict an assessment of cognitive processes to simple reactive times that lack ecological validity. Our aim in this study was to assess agility performance by means of total time on two agility tests with matched motor demands but with both low and high cognitive demands. We tested 22 female team athletes on SpeedCourt, using a simple agility test (SAT) that measured only processing speed and a complex agility test (CAT) that required working memory and inhibition. We found excellent to good reliability for both our SAT (ICC = .79) and CAT (ICC =.70). Lower agility performance on the CAT was associated with increased agility total time and split times ( p < .05). These results demonstrated that agility performance depends on the complexity of cognitive demands. There may be interference-effects between motor and cognitive performances, reducing speed when environmental information becomes more complex. Future studies should consider agility training models that implement complex cognitive stimuli to challenge athletes according to competitive demands. This will also allow scientists and practitioners to tailor tests to talent identification, performance development and injury rehabilitation.

Context Return to running (RTR) after anterior cruciate ligament reconstruction (ACLR) is a crucial milestone. However, how and when to start a running program are uncertain. Objective To explore the feasibility of a structured program to reintroduce running after ACLR and evaluate the predictive value of potential predictors of short-term success. Design Longitudinal cohort study. Setting Local research center and participants' homes. Patients or Other Participants Thirty-five participants were recruited after ACLR. Intervention(s) Program with a progression algorithm to reintroduce running (10 running sessions in 14 days). Main Outcome Measure(s) The criterion for short-term success was no exacerbation of symptoms. Potential predictors were (1) the International Knee Documentation Committee (IKDC) subjective knee form score, (2) ACL Return to Sport after Injury questionnaire score, (3) quadriceps and hamstrings strength, (4) step-down endurance test, and (5) modified Star Excursion Balance test. Descriptive statistics were performed to study the feasibility of the RTR program, and Poisson regression analysis was used to evaluate predictors of success. Results Of the 34 participants, 33 completed the RTR program. Sixteen participants experienced some temporary exacerbation of symptoms, but only 1 had to stop the program. The initial IKDC score was the only significant predictor of a successful RTR, with an area under the receiver operating characteristic curve of 80.4%. An IKDC cut-off of 63.7/100 differentiated responders and nonresponders with the highest sensitivity and specificity (77.8% and 75.0%, respectively). A participant with an IKDC score above this threshold had a 3-fold greater chance of success. Conclusions Our results confirm the feasibility of our RTR program and progression algorithm after ACLR. Clinicians should use an IKDC score of >64 as a criterion to reintroduce running after ACLR to increase the likelihood of short-term success.

Background: A limited number of patients return to sport (RTS) after an anterior cruciate ligament reconstruction (ACLR) and patients who RTS have a relatively high risk for second ACL injury. The purpose of the current study was to compare the results of a test battery between patients who returned to the pre-injury level of sport (RTS group) and patients who did not (NO-RTS group).It was hypothesized that the RTS group showed better test results.Methods: Sixty-four patients (age 27.8 +/- 8.8 years) were included. The results of a multicomponent test battery (jump-landing task assessed with the Landing Error Scoring System (LESS), three hop tests, isokinetic strength test for quadriceps and hamstring) were compared between groups with a 2 x 2 ANOVA.Results: The RTS group showed a significantly lower LESS score (p = 0.010), significantly higher absolute scores on hop tests with both legs (injured leg: single leg hop test p = 0.013, triple leg hop test p = 0.024, side hop test p = 0.021: non-injured leg: single leg hop test p 0.011, triple leg hop test p = 0.023, side hop test p = 0.032) and significantly greater hamstring strength in the injured leg (p = 0.009 at 60 degrees/s, p = 0.012 at 180 degrees/s and p - 0.013 at 300 degrees/s). No differences in test results were identified between patients who sustained a second ACL injury and patients who did not.Conclusion: Patients after ACLR with better jump-landing patterns, hop performance and greater hamstring strength have greater likelihood for RTS. Understanding how manipulating variables with vertical jump instruction could influence kinetic variables would be beneficial for trainers and athletes. External focus (EF) instructions have been shown to result in superior motor performance compared to an internal focus (IF). In this study, we tried to determine the effects of verbal instructions using IF and EF of attention on some kinetic characteristics of three different vertical countermovement jumps (CMJs). Ninety male participants (age, 23.9 ± 2 years; height, 178.7 ± 4 cm; and mass, 74.2 ± 7.5 kg) were randomly assigned into IF (n = 30), EF (n = 30), and control groups (n = 30) and were given specific instructions. Subjects performed three kinds of CMJs on a force platform: (1) short countermovement jump (S-CMJ), (2) natural countermovement jump (N-CMJ), and (3) deep countermovement jump (D-CMJ). Landing kinetic (displacement, force, leg stiffness, and center of pressure (COP) to center of mass (COM) displacement) variables from the CMJs were assessed for pre, post training, and retention test (1 week later). The results showed that EF instruction could significantly improve vertical CMJ kinetic variables. Retention was achieved and vertical CMJ kinetic variables were maintained for subjects in the EF group. This study showed that using EF instructions improved vertical CMJ kinetics compared to IF instructions. EF is suggested to apply to CMJs for improving performance based on the exact athletic demands.

Context: Female volleyball players are more predisposed to anterior cruciate ligament injury in comparison with their male counterparts. Recent research on anterior cruciate ligament injury prevention strategies has shown the positive results of adopting the external focus (EF) of attention in sports. Objective: To determine the effect of 6-week EF instruction exercises on performance and kinetic factors associated with lower-extremity injury in landing after the volleyball blocks of female athletes. Design: Pretest and posttest control study. Setting: University research laboratory. Participants: Thirty-two female volleyball players (18–24 y old) from the same team randomly divided into experimental (n = 16) and control (n = 16) groups. Intervention: The experimental group performed a 6-week exercise program with EF instructions. The control group continued its regular volleyball team schedule. Main Outcome Measures: To assess function, single-leg triple hop test for distance was used. A force plate was used to evaluate kinetic variables including vertical ground reaction forces, the rate of loading, and dynamic postural stability index. All data were assessed at baseline and after the intervention. Background Single-leg hop tests are commonly performed in the forward direction to evaluate functional performance. However, athletes move in multiple directions during pivoting sports. The first aim of this study was to examine test–retest reliability of single-leg hop tests in the forward, medial and rotational direction in non-injured athletes. Second, the discriminative ability to detect leg asymmetries with these hop tests in anterior cruciate ligament (ACL) reconstructed athletes was determined. Methods Sixteen recreational non-injured participants (eight females, eight males; 22.4 ± 1.9 years) were tested twice (one-week interval) and performed the single hop for distance (SH), triple hop for distance (TH), medial side triple hop for distance (MSTH) and 90° medial rotation hop for distance (MRH). Intraclass correlation coefficients (ICCs), standard errors of measurement (SEM) and smallest detectable differences (SDD) were calculated. Discriminative ability was determined in 32 ACL-reconstructed participants (four females, 28 males; 24.4 ± 4.6 years; six months postoperative) who performed the same hop tests once. Results The ICCs ranged between 0.93 and 0.98. The SEM and SDD were respectively 2.6–4.1% and 7.2–11.3% of the mean hop distance of the group. The proportion (%) of ACL-reconstructed participants passing the ≥ 90% limb symmetry cut-off was 62.5 (SH), 59.4 (TH), 40.6 (MSTH) and 46.9 (MRH). Conclusion Excellent test–retest reliability of forward, medial and rotational hop tests was found. This allows clinicians to make informed interpretations of changes in hop test distances when retesting athletes. Medial and rotational hop tests are more likely to show limb asymmetries in ACL-reconstructed participants compared to forward hop tests.

Athletes who wish to resume high-level activities after an injury to the anterior cruciate ligament (ACL) are often advised to undergo surgical reconstruction. Nevertheless, ACL reconstruction (ACLR) does not equate to normal function of the knee or reduced risk of subsequent injuries. In fact, recent evidence has shown that only around half of post-ACLR patients can expect to return to competitive level of sports. A rising concern is the high rate of second ACL injuries, particularly in young athletes, with up to 20% of those returning to sport in the first year from surgery experiencing a second ACL rupture. Aside from the increased risk of second injury, patients after ACLR have an increased risk of developing early onset of osteoarthritis. Given the recent findings, it is imperative that rehabilitation after ACLR is scrutinized so the second injury preventative strategies can be optimized. Unfortunately, current ACLR rehabilitation programs may not be optimally effective in addressing deficits related to the initial injury and the subsequent surgical intervention. Motor learning to (re-)acquire motor skills and neuroplastic capacities are not sufficiently incorporated during traditional rehabilitation, attesting to the high re-injury rates. The purpose of this article is to present novel clinically integrated motor learning principles to support neuroplasticity that can improve patient functional performance and reduce the risk of second ACL injury. The following key concepts to enhance rehabilitation and prepare the patient for re-integration to sports after an ACL injury that is as safe as possible are presented: (1) external focus of attention, (2) implicit learning, (3) differential learning, (4) self-controlled learning and contextual interference. The novel motor learning principles presented in this manuscript may optimize future rehabilitation programs to reduce second ACL injury risk and early development of osteoarthritis by targeting changes in neural networks.

Background Causes of anterior cruciate ligament (ACL) injuries are multifactorial. Anterior cruciate ligament injury prevention should thus be approached from a multifactorial perspective as well. Training to resist fatigue is an underestimated aspect of prevention programs given that the presence of fatigue may play a crucial role in sustaining an ACL injury. Objectives The primary objective of this literature review was to summarize research findings relating to the kinematic and kinetic effects of fatigue on single-leg landing tasks through a systematic review and meta-analysis. Other objectives were to critically appraise current approaches to examine the effects of fatigue together with elucidating and proposing an optimized approach for measuring the role of fatigue in ACL injury prevention. Methods A systematic literature search was conducted in the databases PubMed (1978–November 2017), CINAHL (1992–November 2017), and EMBASE (1973–November 2017). The inclusion criteria were: (1) full text, (2) published in English, German, or Dutch, (3) healthy subjects, (4) average age ≥ 18 years, (5) single-leg jump landing task, (6) evaluation of the kinematics and/or kinetics of the lower extremities before and after a fatigue protocol, and (7) presentation of numerical kinematic and/or kinetic data. Participants included healthy subjects who underwent a fatigue protocol and in whom the effects of pre- and post-fatigue on three-dimensional lower extremity kinematic and kinetics were compared. Methods of data collection, patient selection, blinding, prevention of verification bias, and study design were independently assessed. Results Twenty studies were included, in which four types of single-leg tasks were examined: the single-leg drop vertical jump, the single-leg drop landing, the single-leg hop for distance, and sidestep cutting. Fatigue seemed to mostly affect initial contact (decreased angles post-fatigue) and peak (increased angles post-fatigue) hip and knee flexion. Sagittal plane variables at initial contact were mostly affected under the single-leg hop for distance and sidestep cutting conditions whilst peak angles were affected during the single-leg drop jump. Conclusions Training to resist fatigue is an underestimated aspect of prevention programs given that the presence of fatigue may play a crucial role in sustaining an ACL injury. Considering the small number of variables affected after fatigue, the question arises whether the same fatigue pathways are affected by the fatigue protocols used in the included laboratory studies as are experienced on the sports field. Background : The goal of a rehabilitation programme after anterior cruciate ligament (ACL) reconstruction is to manage a patient's goals and expectations (i.e., returning to physical activities and sports) while minimizing the risk of new injury, particularly a new ACL injury. Although general rehabilitation programmes have been proposed, some factors can lead to adapting each programme to each patient. Objective : To describe how different variables, including surgical techniques, sports participation, psycho-social and contextual factors can modify the rehabilitation programme. Methods : We performed a narrative review with input from experts in the field (level of evidence 5). Conclusions : Modifying factors of the ACL rehabilitation programme are related to the initial lesion or surgery, to sports, or to psychological or social aspects. Regarding the type of graft, the rehabilitation is mainly different in the early postoperative phase; the other phases are not graft-based but rather goal-based rehabilitation. Depending on the meniscal or cartilage repair, the rehabilitation protocol will initially take priority over the anterior cruciate ligament reconstruction protocol. The ACL reconstruction rehabilitation programme should meet the requirements of the anticipated sports, to optimize the athlete's ability to return to the expected level and minimize the risk of reinjury. Psycho-social and contextual factors must also be considered in rehabilitation care to individualize and optimize each patient's programme.

Objective External focus (EF) of attention leads to improved balance performance. Consideration of the neuromodulatory effects of EF may inform its clinical utility in addressing neuroplastic impairments after musculoskeletal injuries. We aimed to determine whether electrocortical activity and balance performance changed with attentional foci that prioritized differing sensory feedback and whether changes in electrocortical activity and balance were associated. Methods Individuals who were healthy (n = 15) performed a single-limb balance task under 3 conditions: internal focus (IF), somatosensory focus [EF with a baton (EF-baton)], and visual focus [EF with a laser (EF-laser)]. Electrocortical activity and postural sway were recorded concurrently using electroencephalography and a triaxial force plate. Electroencephalographic signals were decomposed, localized, and clustered to generate power spectral density in θ and α-2 frequency bands. Postural sway signals were analyzed with center-of-pressure sway metrics (eg, area, distance, velocity) and knee angle. The relationship between percent change in clustered brain activity and task performance metrics was assessed. Results Both EF conditions resulted in increased cortical activity and improved balance performance compared to IF. EF-laser had the largest effect, demonstrating increased frontal θ power (d = 0.64), decreased central θ power (d = −0.30), and decreased bilateral motor, bilateral parietal, and occipital α-2 power (d = −1.38 to −4.27) as well as a shorter path distance (d = −0.94) and a deeper (d = 0.70) and less variable (d = −1.15) knee angle than IF. Weak to moderate associations exist between increases in cortical activity and improved balance performance (ρ = 0.405–0.584). Conclusions EF resulted in increased cortical activity associated with cognitive, motor, somatosensory, and visual processing. EF-laser, which prioritized visual feedback, had the largest and broadest effects. Changes in cortical activity resulting from EF were independently associated with improved balance performance. Impact This study demonstrates that goal-oriented attention results in functional increases in brain activity compared to internally directed self-focus. These results suggest EF may target neurophysiologic impairments and improve balance in clinical populations.

Research of non-contact anterior cruciate ligament (ACL) inj1ury risk aims to identify modifiable risk factors that are linked to the mechanisms of injury. Information from these studies is then used in the development of injury prevention programmes. However, ACL injury risk research often leans towards methods with three limitations: 1) a poor preservation of the athlete-environment rela- tionship that limits the generalisability of results, 2) the use of a strictly biomechanical approach to injury causation that is incom- plete for the description of injury mechanisms, 3) and a reductionist analysis that neglects profound information regarding human movement. This current opinion proposes three principles from an ecological dynamics perspective that address these limitations. First, it is argued that, to improve the generalisability of findings, research requires a well-preserved athlete-environment relation- ship. Second, the merit of including behaviour and the playing situation in the model of injury causation is presented. Third, this paper advocates that research benefits from conducting non- reductionist analysis (i.e., more holistic) that provides profound information regarding human movement. Together, these princi- ples facilitate an ecological dynamics approach to injury risk research that helps to expand our understanding of injury mechan- isms and thus contributes to the development of preventative measures.

Context: Only 55% of the athletes return to competitive sports after an anterior cruciate ligament (ACL) injury. Athletes younger than 25 years who return to sports have a second injury rate of 23%. There may be a mismatch between rehabilitation contents and the demands an athlete faces after returning to sports. Current return-to-sports (RTS) tests utilize closed and predictable motor skills; however, demands on the field are different. Neurocognitive functions are essential to manage dynamic sport situations and may fluctuate after peripheral injuries. Most RTS and rehabilitation paradigms appear to lack this aspect, which might be linked to increased risk of second injury. Objective: This systematic and scoping review aims to map existing evidence about neurocognitive and neurophysiological functions in athletes, which could be linked to ACL injury in an integrated fashion and bring an extensive perspective to assessment and rehabilitation approaches. Data Sources: PubMed and Cochrane databases were searched to identify relevant studies published between 2005 and 2020 using the keywords ACL, brain, cortical, neuroplasticity, cognitive, cognition, neurocognition, and athletes. Study Selection: Studies investigating either neurocognitive or neurophysiological functions in athletes and linking these to ACL injury regardless of their design and technique were included. Study Design: Systematic review. Level of Evidence: Level 3. Data Extraction: The demographic, temporal, neurological, and behavioral data revealing possible injury-related aspects were extracted and summarized. Results: A total of 16 studies were included in this review. Deficits in different neurocognitive domains and changes in neurophysiological functions could be a predisposing risk factor for, or a consequence caused by, ACL injuries. Conclusion: Clinicians should view ACL injuries not only as a musculoskeletal but also as a neural lesion with neurocognitive and neurophysiological aspects. Rehabilitation and RTS paradigms should consider these changes for assessment and interventions after injury.

Purpose An increased posterior tibia plateau angle is associated with increased risk for anterior cruciate ligament injury and re-rupture after reconstruction. The aims of this study were to determine whether the tibia plateau angle correlates with dynamic anterior tibia translation (ATT) after an anterior cruciate ligament reconstruction and whether the tibia plateau angle correlates with aspects of knee kinematics and kinetics during jump landing. Methods Thirty-seven patients after anterior cruciate ligament reconstruction with autograft hamstring tendon were included. Knee flexion angle and knee extension moment during single leg hops for distance were determined using a motion capture system and the dynamic ATT with its embedded method. The medial and lateral posterior tibia plateau angle were measured using MRI. Moreover, passive ATT was measured using the KT-1000 arthrometer. Results A weak negative correlation was found between the maximal dynamic ATT and the medial tibia plateau angle (p = 0.028, r = − 0.36) and between the maximal knee flexion angle and the lateral tibia plateau angle (p = 0.025, r = − 0.37) during landing. Patients with a smaller lateral tibia plateau angle show larger maximal knee flexion angle during landing than the patients with larger lateral tibia plateau angle. Also, the lateral tibia plateau angle is associated the amount of with muscle activity. Conclusion The posterior medical tibia plateau angle is associated with dynamic ATT. The maximal knee flexion angle and muscle activity are associated with the posterior lateral tibia plateau angle. Level of evidence III

Despite the development of return to sport (RTS) guidelines over recent years, there is a lack of a scientific consensus on the RTS criteria used to release a patient to unrestricted sport activity after ACL reconstruction (ACLR). A proportional meta-analysis, showed that only 23% of patients after ACLR passed RTS test batteries before RTS. Although passing RTS criteria reduce the risk of subsequent graft rupture by 60%, it increases the risk of a contralateral ACL rupture by 235%. These equivocal findings in terms of validity of RTS tests after ACLR leaves clinicians with high level of uncertainty in clinical decision-making. Dingenen & Gokeler proposed that RTS should be viewed as continuum after ACL injury and ACLR. Moreover, one of the possible solutions why patients do not meet the RTS criteria is simply due to the lack of implementation of evidence- based rehabilitation in terms of specificity and training load. Too often, the end phase of the rehabilitation is not extensive or specific enough, thereby exposing athletes to specific training loads and training characteristics that they cannot handle from a physical, neurocognitive as well as from a psychological perspective. Della Villa et al. introduced the concept of an On Field Rehabilitation (OFR) model to bridge the gap between standard rehabilitation and return to training. This allows for a gradual progression of each part and thus ensures correct function and that no adverse knee reaction is noted before moving on to the next level. Returning an athlete to full participation should be a graduated continuum and not based on a set of tests at one single point in time. The above findings pertaining to the high incidence of recurrent injuries, question whether the current RTS tests provide relevant information for guiding the decision making. There is a need for more ecological valid research approaches to test an athlete returning to sports after an ACLR in more complex sports-specific demanding tests, which are more reflective of the demands while on the field. Based on a RTS continuum approach, training and tests should include physical, cognitive and psychological loads in a gradual fashion that prepare the athlete for a safer RTS

Introduction: Injuries of the anterior cruciate ligament (ACL) commonly occur during complex game situations when the athlete encounters multiple factors such as ball, opponent, field position, and game strategy (Grooms et al., 2018). Many of the current traditional injury screening programs are performed within a predictable, fixed or ‘closed’ environment which do not represent real game situations that require high neurocognitive demands (Dingenen & Gokeler, 2017; Grooms et al., 2018). A complementary approach to lab-based settings is necessary to incorporate the demands of the complex athletic environments. By using wearable sensor technology, we aim to develop an on-field injury screening test in elite youth male soccer players. Investigating the individual differences in motor coordination patterns of the players during sport-specific tasks might enhance our understanding of how ACL injuries occur. Hypotheses: We hypothesized that the motor coordination patterns of the players would be affected when they perform under different conditions manipulated with constraints (task and environmental). Methods: A football-specific test setup was created to analyse the kinematic and performance measures of a group of 17 male youth elite football players aged 15 years (height = 164 ± 9 cm, mass = 50.9± 7.4 kg). The players were grouped into two and measured on two consecutive days. All the players were instructed to complete the test setup (4 conditions, 5 trials) as fast as possible. Condition 1 includes no constraint, condition 2 includes a task constraint (football dummies), condition 3 includes an environmental constraint (stroboscopic glasses) (SENAPTEC, Beaverton, Oregon) and condition 4 includes both task and environmental constraints. 3-D kinematics of the hip, knee, ankle joints were captured using Xsens wearable full-body sensor suits (Xsens, MVN Link version, Enschede, The Netherlands). MATLAB (MATLAB R2019a, The MathWorks Inc., Massachusetts) was used to process and analyse the kinematic data. Data from condition 1 was determined as reference behavior/condition to be compared to other conditions. Kinematic data are presented in attitude vectors (ATV). Results: In total, 81% of the players demonstrated a significant difference (P < 0.05) in angles of hip, knee and ankle joints when performing under different conditions. The percentage of players with increased comparison-based joint movements as follows; condition 1 to condition 2 comparison; 41% hip flexion, 59% hip extension, 53% hip abduction, 47% hip adduction, 62% knee flexion, 38% knee extension, 59% knee abduction, 41% knee adduction, 47% ankle dorsiflexion, 53% ankle plantarflexion, condition 1 to condition 3 comparison; 35% hip flexion, 65% hip extension, 47% hip abduction, 53% hip adduction, 50% knee flexion, 50% knee extension, 41% knee abduction, 59% knee adduction, 59% ankle dorsiflexion, 41% ankle plantarflexion and condition 1 to condition 4 comparison; 31% hip flexion, 69% hip extension, 38% hip abduction, 62% hip adduction, 60% knee flexion, 40% knee extension, 44% knee abduction, 56% knee adduction, 69% ankle dorsiflexion, 31% ankle plantarflexion. Conclusion: The result of this pilot study demonstrated that manipulating task with different constraints caused significant changes in players’ motor coordination patterns which supported the hypothesis of our study. Our findings suggest to develop ACL injury screening tests in a sport-specific setting.

Objectives The primary objective of the study was to investigate rehabilitation strategies of Flemish physical therapists before and after anterior cruciate ligament (ACL) reconstruction. Secondly, we aimed to investigate physical therapists’ preferences on continuing education and evaluate their self-rated confidence and competence when treating patients before and after ACL reconstruction. Design Survey-based study. Setting Online survey platform. Participants Flemish physical therapists (n = 283). Main outcome measures The online survey consisted of a combination of 40 open- and closed-ended questions, divided across 5 sections: (1) participant demographics and clinical practice information, (2) patient population information, (3) continued education practices, (4) rehabilitation strategies, and (5) physical therapist self-rated confidence and competence to treat patients with ACL reconstruction. Results A wide variability in rehabilitation strategies were found across the whole ACL rehabilitation continuum, which were in general not in line with best available evidence. Nevertheless, the overall self-rated confidence and competence of physical therapists treating patients before and after ACL reconstruction were high. Conclusion Our findings indicate a need to improve rehabilitation practices before and after ACL reconstruction. Advanced research dissemination and implementation are required to achieve better rehabilitation outcomes.

Purpose Restricted ankle dorsiflexion can promote aberrant biomechanics associated with risk for knee injury during dynamic activities. Attentionally focused instructions have been used to improve high-risk knee biomechanics during landing tasks. Yet, it is unknown whether attentionally focused instruction can effectively improve landing patterns in the presence of a mechanical restriction on the ankle. Therefore, our purpose was to determine whether restricting ankle dorsiflexion by use of bracing mitigated the effects of attentional foci on landing biomechanics in healthy females. Methods We used a crossover design to investigate lower extremity biomechanics in 19 healthy females between the ages of 18–35 during a series of jump-landing tasks. Participants completed 6 blocks of 3 jump-landings on separate force platforms in a randomized order based on brace condition (brace, no brace) and mode of attentional foci (neutral, internal focus [IF], external focus [EF]). Attentionally focused instructions were provided immediately prior to 3 practice jump-landings, followed by 5 test jump-landings with self-controlled feedback only. Results Ankle bracing decreased peak dorsiflexion and sagittal range of motion (ROM) (mean difference: 5.7–5.8°), and peak inversion and frontal ROM (mean difference: 2.4–3.0°). However, hip flexion ROM (mean difference: 1.8°) increased compared to the no brace condition. Regardless of ankle bracing, EF instruction increased peak hip flexion (mean difference: 4.9°) and hip flexion range of motion (mean difference: 3.8–4.6°), while decreasing peak knee valgus (mean difference: 0.8–1.0°) and knee valgus moment (mean difference: 0.04 Nm/kg). Additionally, EF instruction increased peak hip abduction to a similar degree when braced (mean difference: 3.6–4.0°) and not braced (mean difference: 2.1–2.5°). Lastly, EF instruction increased hip abduction ROM only when braced (mean difference: 2.3–2.4°), but decreased peak knee valgus power only when not braced (mean difference: 0.18 W/kg). Conclusions Our findings indicate that mechanically restricting ankle dorsiflexion does not mitigate the ability of EF instruction to enhance jump-landing performance by means of improving hip and knee biomechanics in healthy females. However, our findings suggest an improved ability to control the rate of knee valgus loading when not braced. Therefore, we conclude that EF instruction remains a viable clinical strategy to improve landing patterns in the presence of restricted ankle dorsiflexion, yet this approach may be ineffective to reduce the rate of knee joint loading.

Purpose It has been reported that there is no correlation between anterior tibia translation (ATT) in passive and dynamic situations. Passive ATT (ATTp) may be different to dynamic ATT (ATTd) due to muscle activation patterns. This study aimed to investigate whether muscle activation during jumping can control ATT in healthy participants. Methods ATTp of twenty-one healthy participants was measured using a KT-1000 arthrometer. All participants performed single leg hops for distance during which ATTd, knee flexion angles and knee flexion moments were measured using a 3D motion capture system. During both tests, sEMG signals were recorded. Results A negative correlation was found between ATTp and the maximal ATTd (r = − 0.47, p = 0.028). An N-Way ANOVA showed that larger semitendinosus activity was seen when ATTd was larger, while less biceps femoris activity and rectus femoris activity were seen. Moreover, larger knee extension moment, knee flexion angle and ground reaction force in the anterior-posterior direction were seen when ATTd was larger. Conclusion Participants with more ATTp showed smaller ATTd during jump landing. Muscle activation did not contribute to reduce ATTd during impact of a jump-landing at the observed knee angles. However, subjects with large ATTp landed with less knee flexion and consequently showed less ATTd. The results of this study give information on how healthy people control knee laxity during jump-landing. Level of evidence III

Pre-seasonal assessment and performance diagnostics involve biomechanical, physiological and functional testing. These assessments help coaches and the medical team to derive information on athletes’ health and risk of injury as well as well as individual performance capacity. In case of an injury, performance diagnostics data from the uninjured state in the competitive phase of the season can reveal information about an athlete’s individual maximal functional capacity. During rehabilitation process, these values can be used to monitor the rehabilitation process and might serve as target values.

Purpose: In spite of mounting evidence indicating that concentric and eccentric knee extensor muscle contractions might have special nervous system control strategies, the differentiation of brain frequencies between concentric and eccentric movements and how the motor cortex programs this contraction has been less studied. In this study, the brain and muscle activation differences during maximum concentric and eccentric contractions were compared. Methods: Nine healthy volunteers performed 20 maximum eccentric and 20 maximum concentric knee extensor contractions. Electroencephalography (EEG) signals from sensorimotor-related cortical areas were recorded simultaneous with the electromyography (EMG) of the knee extensor muscles. In the spectral analysis the performance related power values were calculated for Theta (4-7 Hz) and Alpha (7-12 Hz). Results: The time-domain results revealed, longer time and greater cortical activity is required for the preparation of an eccentric contraction. For the eccentric task, the cortical activity was greater, but the EMG was lower in comparison to the concentric task values. Statistical analysis showed significant higher and lower Theta and Alpha power in both types of contractions compared to the resting state, respectively. Conclusion: These findings suggest that increased Theta power is associated with task complexity and focused attention and decreased Alpha power values with increased information processing in the somatosensory cortex.

Contextual factors influence how the athlete, coach, and clinicians view an injury. The main construct to define a sports injury by athletes is the consequence on sports performance. This in turn may also affect the role and responsibilities each have in the continuum of return to play (RTP). As a coach, it is recommended to have an open communication with the injured athlete before the start of the rehabilitation process, in order to set realistic goals, and to prepare the athlete to achieve these goals successfully. The RTP decision is complex and multifactorial, and probably too large to perform in only one step. Each rehabilitation exercise or phase can be considered as a small step in the direction of RTP. It is recommended to establish a healthy club culture where staff (e.g., team managers, head and assistant coaches, and members of the medical team) are encouraged to develop environments that stimulate the players to engage in adaptive stress-recovery and risk-decreasing behavior in the RTP phases.

PURPOSE: Limited evidence suggests that cross-education affords clinical benefits in the initial 8 weeks after anterior cruciate ligament (ACL) reconstruction, but it is unknown if such cross-education effects are reproducible and still present in later phases of rehabilitation. We examined whether cross-education, as an adjuvant to standard therapy, would accelerate the rehabilitation up to 26 weeks after ACL reconstruction by attenuating quadriceps weakness.METHODS: ACL-reconstructed patients were randomized into experimental (n = 22) and control groups (n = 21). Both groups received standard care after ACL reconstruction. In addition, the experimental group strength trained the quadriceps of the non-operated leg during weeks 1-12 after surgery (i.e., cross-education). Self-reported knee function was assessed with the Hughston Clinic Knee score as the primary outcome. Secondary outcomes were maximal quadriceps and hamstring strength and single leg hop distance. All outcomes were measured 29 ± 23 days prior to surgery, as a reference, and at 5-week, 12-week, and 26-week post-surgery.RESULTS: Both groups scored 12% worse on self-reported knee function 5-week post-surgery (95% CI 7-17) and showed 15% improvement 26-week post-surgery (95% CI - 20 to - 10). No cross-education effect was found. Interestingly, males scored 8-10% worse than females at each time point post-surgery. None of 33 secondary outcomes showed a cross-education effect. At 26-week post-surgery, both legs improved maximal quadriceps (5-14%) and hamstring strength (7-18%), and the non-injured leg improved 2% in hop distance. The ACL recovery was not affected by limb dominance and age.CONCLUSION: 26 weeks of standard care improved self-reported knee function and maximal leg strength relative to pre-surgery and adding cross-education did not further accelerate ACL recovery.LEVEL OF EVIDENCE: I.CLINICAL TRIAL REGISTRY NAME AND REGISTRATION: This randomized controlled clinical trial is registered at the Dutch trial register ( http://www.trialregister.nl ) under NTR4395.
