Abstract

Objective: The low level of response (LR) to alcohol, an endophenotype related to heavy drinking and alcohol problems, influences the risk for alcoholism in the context of additional life domains. This article evaluates an LR-based model of drinking patterns in 113 drinking offspring, ages 12 to 24 years, from the San Diego Prospective Study. Method: Correlations and structural equation models (SEMs) were evaluated using LR as measured from the Self-Report of the Effects of Alcohol questionnaire in the offspring. The expectations of the effects of alcohol (EXPECT), the perception of drinking in peers (PEER), the use of alcohol to cope with stress (COPE), and the drink ing quantities and alcohol-related problems (ALCOUT) were evaluated in the SEM. Results: The LR-based model worked well, with good fit characteristics and 78% of the variance of outcome explained. LR related directly to ALCOUT, with additional mediation of that relationship through EXPECT and COPE. Conclusions: The LR-based model performed well in adolescents from the San Diego Prospective Study. Knowledge of which domains mediate how LR impacts alcohol-related outcomes may be useful in developing more focused and potentially more effective prevention approaches. (J. Stud. This article identifies and assesses the impediments to implementing one aspect of community-oriented policing (COP): designated patrol assignments. The research was based on 126 interviews with three different levels of police practitioners (top, middle, and street-level) in three different southwest police departments. While this exploratory study examined several factors that could potentially impede or facilitate change, it was found that individual attitudes towards COP and pressure had the most significant effect on the process of change.

18. Assessing the impediments to organizational change in police departments.

Abstract

The process of change can be a daunting experience for public organizations. For police organizations, however, such change may be even more challenging, given that they are paramilitary, bureaucratic structures, grounded in reactive problem-solving. As demands for more effective and equitable policing continue to increase, a number of police organizations have attempted to make changes to address these issues. A research study on organizational change was undertaken in three police departments in the State of Arizona to examine the types of impediments experienced in the implementation of three change programs (Affirmative Action, Community Policing and Employee Assistance Programs). Data were collected from three levels (top, middle and street-level) of police practitioners over a four-month period. An analysis of the research data revealed that there are impediments to organizational change that remain consistent across programs and departments. Similarities across departments and programs included individual attitudes as a major factor affecting the process of change. Although communication, departmental attitudes and method of implementation were not found to be consistent impediments to change across the three programs, there were underlying themes that emerged which would suggest these factors indirectly impede the process of change. Additionally, pressure was found to be a major factor that facilitated change across programs and departments. This study ends with a discussion on how to reduce or eliminate the forces of resistance in implementing change and suggests approaches for successful change. Strategies included (1) open communication and dissemination of information on the implementation of change, (2) support and participation from all levels of the organization, (3) training to foster understanding of program goals and (4) internal and/or external pressure to ensure development and implementation of change. Future research should incorporate organizational culture in the examination of change, while focusing more on the connection between structure and process. [ABSTRACT FROM AUTHOR]

19. Association of obstructive sleep apnea with adverse pregnancy-related outcomes in military hospitals.

Abstract

Background: Obstructive sleep apnea (OSA) is associated with significant morbidity and mortality in non-obstetrical and obstetrical patients., Objectives: To estimate the prevalence of OSA and its relationship with pregnancy-related complications in a general obstetric population of Department of Defense beneficiaries receiving direct-care at military treatment facilities., Study Design: A retrospective cohort study of all women (N=305,001) who gave birth at a military treatment facility from 2008 to 2014. OSA cases were randomly selected and matched on age (3:1 ratio) to non OSA cases. Multivariable logistic regression was used to examine the risks of adverse pregnancy outcomes (cesarean delivery, gestational diabetes, gestational hypertension, preeclampsia, postoperative wound complications, hospital stay greater than five days, acute renal failure, pulmonary edema, preterm delivery, poor fetal growth, and stillbirth) between pregnant women with and without a diagnosis of OSA. Cases were identified using ICD-9 codes, while controlling for demographics, obesity, and medical comorbidities associated with OSA and the outcomes of interest., Results: We identified 266 cases of OSA (OSA rate=8.7 per 10,000; increased from 6.4 to 9.9 per 10,000 from 2009 to 2013). OSA was associated with a higher odds of cesarean delivery (AOR,1.60; 95% CI, 1.06-2.40), gestational hypertension, (AOR, 2.46; 95% CI, 1.30-4.68), preeclampsia (AOR, 2.42; 95% CI, 1.43-4.09), and preterm delivery (AOR, 1.90; 95% CI, 1.09-3.30)., Conclusions: Obstructive sleep apnea is associated with adverse maternal and fetal outcomes., (Published by Elsevier B.V.)
