2. Relationship Education during Pregnancy and Postpartum: In-Person versus Virtual Delivery of MotherWise.

Abstract

Ample research attests to the effectiveness of in-person relationship education, including education delivered to women who are pregnant or have just had a child. Relationship education delivered virtually may also be effective and hold advantages of accessibility and affordability. Few studies have compared outcomes for the same program delivered virtually and in-person, or prior to versus during the COVID-19 pandemic. The present study analyzed enrollment characteristics, satisfaction with the program, and pre-post changes in communication behaviors, relationship satisfaction, and psychological distress for women who participated in the evidence-based MotherWise program in-person and prior to the COVID-19 pandemic versus those who participated virtually after the pandemic began. The virtual and in-person programs enrolled demographically similar participants who received roughly the same amount of content. Perceptions of the program were positive across modalities and participants in both modalities demonstrated significantly reduced psychological distress from before to after the program. In-person participants reported finding the program more helpful than virtual participants on only one indicator, their ability to handle relationship conflict. Thus, while the current study does not establish equivalency across treatment modalities in efficacy, it does demonstrate the feasibility of transitioning relationship education for individuals from an in-person to a virtual setting. Abstract

Prenatal depression is a significant health issue associated with increased risk for poor mental and physical health outcomes among for both parents and their children. Having a history of childhood maltreatment is associated with increased risk for prenatal depression. Although research suggests that romantic relationship functioning likely plays a role in the links between childhood maltreatment and prenatal depression, it remains to be tested which aspects of relationship functioning modulate these associations. Using an actor–partner interdependence model framework (Kenny et al., 2006), the present study examined how specific aspects of romantic relationship functioning—relationship commitment, perceived partner support, constructive communication, and destructive conflict—moderated the links between mothers' and fathers' childhood maltreatment and depression during pregnancy. Participants included a subset of 1,135 low-income heterosexual couples (2,270 individuals) who enrolled in a randomized controlled trial of a relationship education program during pregnancy. Baseline, pretreatment data were used. Results indicated significant actor and partner effects of childhood maltreatment on prenatal depression. Further, these effects were moderated by specific indices of relationship functioning, such that the effect of one's own maltreatment on their prenatal depression was mitigated by high emotional support from partner and high constructive communication and was exacerbated by destructive conflict. Findings demonstrate that high relationship functioning can act as a protective factor, and low relationship functioning as a risk factor, in the link between one's own childhood maltreatment and their prenatal depression among both mothers and fathers. Abstract

Objective: We posit that evaluators of relationship education interventions can explore a policy relevant outcome by assessing relationship stability and conflict in a single index that is based on the literature on the effects of divorce and marital distress on children. We provide an empirical example from a randomized trial. Background: The U.S. Administration for Children and Families funds community‐based projects using relationship education with a foundational goal of fostering stable and healthy relationships. Assessing this outcome requires an approach different from separately analyzing stability and relationship quality. Method: We used data (N = 1,156 couples) from a randomized trial of Family Expectations to test the Stable Low‐Conflict Index, comparing couples assigned to the intervention to couples assigned to an untreated control group at a follow‐up 8 to 9 months postintervention. Results: Intervention couples were more likely to be in a stable, low‐conflict relationship at the follow up than control couples (b =.36, SE =.15, odds ratio = 1.44, p =.014). Conclusion: An index based on empirical precedence showed evidence of an intervention impact in a community‐based program. Implications: Evaluators of family policy linked interventions may advance the field by exploring outcomes that encapsulate aspects of both relationship stability and quality. [ABSTRACT FROM AUTHOR]

Abstract

Abstract Background Virtual Patients (VPs) are web-based representations of realistic clinical cases. They are proposed as being an optimal method for teaching clinical reasoning skills. International standards exist which define precisely what constitutes a VP. There are multiple design possibilities for VPs, however there is little formal evidence to support individual design features. The purpose of this trial is to explore the effect of two different potentially important design features on clinical reasoning skills and the student experience. These are the branching case pathways (present or absent) and structured clinical reasoning feedback (present or absent). Methods/Design This is a multi-centre randomised 2x2 factorial design study evaluating two independent variables of VP design, branching (present or absent), and structured clinical reasoning feedback (present or absent).The study will be carried out in medical student volunteers in one year group from three university medical schools in the United Kingdom, Warwick, Keele and Birmingham. There are four core musculoskeletal topics. Each case can be designed in four different ways, equating to 16 VPs required for the research. Students will be randomised to four groups, completing the four VP topics in the same order, but with each group exposed to a different VP design sequentially. All students will be exposed to the four designs. Primary outcomes are performance for each case design in a standardized fifteen item clinical reasoning assessment, integrated into each VP, which is identical for each topic. Additionally a 15-item self-reported evaluation is completed for each VP, based on a widely used EViP tool. Student patterns of use of the VPs will be recorded. In one centre, formative clinical and examination performance will be recorded, along with a self reported pre and post-intervention reasoning score, the DTI. Our power calculations indicate a sample size of 112 is required for both primary outcomes. Discussion This trial will provide robust evidence to support the effectiveness of different designs of virtual patients, based on student performance and evaluation. The cases and all learning materials will be open access and available on a Creative Commons Attribution-Share-Alike license.

Abstract

Background: Cardiovascular disease is a major comorbidity of rheumatoid arthritis (RA) and a leading cause of death. Chronic systemic inflammation involving tumour necrosis factor alpha (TNF) could contribute to endothelial activation and atherogenesis. A number of anti-TNF therapies are in current use for the treatment of RA, including certolizumab pegol (CZP), (Cimzia ®; UCB, Belgium). Anti-TNF therapy has been associated with reduced clinical cardiovascular disease risk and ameliorated vascular function in RA patients. However, the specific effects of TNF inhibitors on endothelial cell function are largely unknown. Our aim was to investigate the mechanisms underpinning CZP effects on TNF-activated human endothelial cells. Methods: Human aortic endothelial cells (HAoECs) were cultured in vitro and exposed to a) TNF alone, b) TNF plus CZP, or c) neither agent. Microarray analysis was used to examine the transcriptional profile of cells treated for 6 hrs and quantitative polymerase chain reaction (qPCR) analysed gene expression at 1, 3, 6 and 24 hrs. NF-κB localization and IκB degradation were investigated using immunocytochemistry, high content analysis and western blotting. Flow cytometry was conducted to detect microparticle release from HAoECs. Results: Transcriptional profiling revealed that while TNF alone had strong effects on endothelial gene expression, TNF and CZP in combination produced a global gene expression pattern similar to untreated control. The two most highly up-regulated genes in response to TNF treatment were adhesion molecules E-selectin and VCAM-1 (q < 0.00005). This was supported by qPCR analysis at 6 hrs (E-selectin and VCAM-1; 208.5 fold and 40.5, respectively above control) and also at 1, 3 and 24 hrs (E-selectin; 25.6, 93.5, 12.7 fold, respectively) (VCAM-1; 4.7, 47.2, 17.6 fold) (n = 3; p < 0.05). In contrast, HAoECs treated with TNF in combination with CZP exhibited control levels of E-selectin and VCAM-1 transcript (p > 0.2 compared to control; p > 0.05 compared to TNF alone). The NF-κB pathway was confirmed as a downstream target of TNF-induced HAoEC activation, via nuclear translocation of NF-κB and degradation of IκB, effects which were abolished by treatment with CZP. In addition, flow cytometry detected an increased production of endothelial microparticles in TNF-activated HAoECs, which was prevented by treatment with CZP. Conclusions: We have found at a cellular level that a clinically available TNF inhibitor, CZP reduces the expression of adhesion molecule expression, and prevents TNF-induced activation of the NF-κB pathway. Furthermore, CZP prevents the production of microparticles by activated endothelial cells. This could be central to the prevention of inflammatory environments underlying these conditions and measurement of microparticles has potential as a novel prognostic marker for future cardiovascular events in this patient group. Abstract

This paper joins the vibrant conversation on the unprecedented scale of the use of plastic in order to package, transport and aid the consumption of fresh food. Specific focus is placed how the UK government's plastic free aisle pledge, highlighted in the 2018 25-year sustainability plan, may have an impact on the already increasing levels of food waste in the UK. The current literature fails to recognise the need to evaluate the reliance consumers have on plastics in the purchase, transport and consumption of fresh food products. As a result, it is therefore difficult to evaluate the impact the UK government's plastic free aisle pledge will have on food wastage. In order to understand the possible roles, relationships and reliance consumers have on plastics in the purchase, transport and consumption of fresh food, this study followed an inductive approach. Thus, building on existing research in order to conceptualise how consumers' thoughts and behaviours interrelate with the use of plastic when purchasing, transporting and consuming fresh food and the potential impact this could have on food wastage.The findings of the study indicated consumers strongly rely on plastic packaging as a mechanism in which to preserve fresh food, in order to maintain the quality for as long as possible. Yet the results suggest consumers unconsciously rely on the information presented to them on packaging, such as best before dates. Such statements thus seem to skew consumers' views on what is edible vs. what should be thrown away. This therefore suggests there is a confusing role between plastic packaging preserving food as well as it increasing the wastage of food. The study concludes that the effect of removing plastic from fresh produce could be somewhat negative as the guidance consumers rely on is also removed. There is thus a need for further consideration with regards to the operational impacts of the removal of plastic as well as the possible perseveration techniques. A need for immediate consumer education surrounding food handling and consumption, to minimise consumer ambiguity and the growing culture of reliance on information on plastic packaging is also recommended to help reduce food wastage.
