Abstract

International audience; Introduction: Background: Endophthalmitis is still one of the most fearsome complications of cataract surgery. The most important pathogens causing post-operative phacoemulsification endophthalmitis are staphylococci. Methods: 85 patients were included in a phase II pilot study. Patients were divided in three groups, group 1 and 3 received either azithromycin 1.5% eye drops or placebo eye drops one day before surgery and for 2 days after surgery and group 2 received azithromycin 1.5% eye drops for 3 days after surgery. Treatments were administered twice a day: one drop in the morning and one in the evening. Bacterial samples were collected three times from the conjunctival cul-de-sac, at D-2 (inclusion visit), D0 (before surgery) and D5 (final visit). In this pilot study, the primary endpoint was the proportion of positive cultures on the day of surgery (D0). Ocular and systemic safety data (including tolerance) were also recorded. Results: Among patients with a positive culture in the conjunctival cul-de-sac on D-2, 57.6% were still positive at D0 in group 1, versus 92.3% in group 3 (placebo group) (p=0.035). The number of staphylococcus colonies (log-transformed) decreased between D-2 and D0 and the difference is statistically significant between the three groups (p=0.043) and between group 1 (that received azithromycin 1.5% eye drops at D0) and groups 2+3 (that did not received azithromycin 1.5% eye drops at D0) (p=0.025). This difference was emphasized between group 1 and group 3 (placebo group) (p=0.015). No safety issue was raised during the study and 100% of patients were very satisfactory or satisfactory with the global tolerance of the eye drops. Conclusions: Azithromycin 1.5% eye drops are safe, and effective in reducing the bacterial load of conjunctival cul-de-sac, including the number of staphylococcus colonies, during the peri-operative period of cataract surgery. Abstract

Radio-frequency thermokeratoplasty (RF-TKP) is a technique used to reshape the cornea curvature by means of thermal lesions using radio-frequency currents. This curvature change allows refractive disorders such as hyperopia to be corrected. A new electrode with ring geometry is proposed for RF-TKP. It was designed to create a single thermal lesion with a full-circle shape. Finite element models were developed, and the temperature distributions in the cornea were analysed for different ring electrode characteristics. The computer results indicated that the maximum temperature in the cornea was located in the vicinity of the ring electrode outer perimeter, and that the lesions had a semi-torus shape. The results also indicated that the electrode thickness, electrode radius and electrode thermal conductivity had a significant influence on the temperature distributions. In addition, in vitro experiments were performed on rabbit eyes. At 5 W power, the lesions were fully circular. Some lesions showed non-uniform characteristics along their circular path. Lesion depth depended on heating duration (60% of corneal thickness for 20 s, and 30% for 10 s). The results suggest that the critical shrinkage temperature (55-63 degrees C) was reached at the central stroma and along the entire circular path in all the cases. [ABSTRACT FROM AUTHOR]

Abstract

Background: Cataract surgery is the most common operation performed worldwide. A fixed topical corticosteroid-antibiotic combination is usually prescribed in clinical practice for 2 or more weeks to treat post surgical inflammation and prevent infection. However, this protracted schedule may increase the incidence of corticosteroid-related adverse events and notably promote antibiotic resistance. Methods: This International, multicentre, randomized, blinded-assessor, parallel-group clinical study evaluated the non-inferiority of 1-week levofloxacin/dexamethasone eye drops, followed by 1-week dexamethasone alone, vs. 2-week gold-standard tobramycin/dexamethasone (one drop QID for all schedules) to prevent and treat ocular inflammation and prevent infection after uncomplicated cataract surgery. Non-inferiority was defined as the lower limit of the 95% confidence interval (CI) around a treatment difference >–10%. The study randomized 808 patients enrolled in 53 centres (Italy, Germany, Spain and Russia). The primary endpoint was the proportion of patients without anterior chamber inflammation on day 15 defined as the end of treatment. Endophthalmitis was the key secondary endpoint. This study is registered with EudraCT code: 2018-000286-36. Results: After the end of treatment, 95.2% of the patients in the test arm vs. 94.9% of the control arm had no signs of inflammation in the anterior chamber (difference between proportions of patients = 0.028; 95% CI: −0.0275/0.0331). No case of endophthalmitis was reported. No statistically significant difference was evident in any of the other secondary endpoints. Both treatments were well tolerated. Conclusions: Non-inferiority of the new short pharmacological strategy was proven. One week of levofloxacin/dexamethasone prevents infection, ensures complete control of inflammation in almost all patients and may contain antibiotic resistance.
