In our country, the Ministry of Health is involved in the regulation and licensure of postgraduate medical education. The Turkish Medical Association plays a role in specialist training through the Coordination Board of Specialist Associations. There are two big associations that are members of the Turkish Medical Association (The Turkish Pediatric Association, The Turkish National Pediatric Association). Modern medical education in the Ottoman Empire started on 14 March 1827. The first Turkish book on newborn care and diseases was written in 1844. Lectures by a pediatrician, at the Military Medical School began in the 1900s. With the law, which was enacted on 8 November 1928 in the Republican period, a requirement of licensure in pediatrics was determined as a two-year working period. With the 1933 university reform, the İstanbul University Medical Faculty Department of Pediatrics was established. Today, Pediatrics residency training continues to be given in 62 Medical Faculties and 29 Training and Research Hospitals, according to the quota data for the 2023 Medical Specialization Examination. In 1995, the Turkish Pediatric Association and the Turkish National Pediatric Association came together and decided to work on improving Pediatric Education in Turkey. On September 14, 2003, the General Assembly of the Turkish Pediatrics Board convened for the first time. In 2019, the board of pediatrics accelerated its work with a new perspective. In 2020, the Pediatric board started accreditation of educational programs and in 2021, two levels (Knowledge and skill exams) the National board exam. [ABSTRACT FROM AUTHOR]

Aim: The aim of this study was to evaluate the cardiometabolic risk factors including vitamin D levels according to the degree of obesity in adolescents. Material and Methods: This is a retrospective cross-sectional study. A total of 363 overweight/obese adolescents aged between 11 and 18 years who were evaluated in our clinic from January 2012 to December 2015 were included in the study. The degree of obesity was calculated as the body mass index standard deviation. Hypertension, dys-lipidemia, hyperinsulinemia, hyperglycemia, insulin resistance, and vitamin D deficiency were defined as cardiometabolic risk factors. Mann-Whitney U, Chi-square, Spearman and Pearson's correlation tests, and linear regressions analyses were used for statistical analyses. Results: Of the 319 (n=319/363) adolescents, all of whose cardiometabolic risk factors were known, 267 (85.7%) had at least one cardiometabolic risk factor. The body mass index standard deviation had a positive correlation with the number of cardiometabolic risk factors (p<0.001). In the linear regression models in which sex and age were considered as covariates, an increase of one unit in the body mass index standard deviation led to an increase of 6.085 mm Hg in systolic blood pressure, 4.4 mm Hg in diastolic blood pressure, 1.59 points in HOMA-IR, 13% in insulin level, and a decrease of 2.16 ng/mL in vitamin D levels. Conclusion: In adolescents, the number of cardiometabolic risk factors increases as the degree of obesity increases. The determination of the severity of obesity can help to identify individuals at greater risk for higher blood pressure, impaired glucose metabolism, and lower serum vitamin D levels. On the other hand, the degree of obesity may not reflect the presence of abnormal lipid and glucose levels. [ABSTRACT FROM AUTHOR]

Objective: To investigate the effect of stress caused by disordered eating attitudes on bone health in obese adolescents. Methods: A cross-sectional study comprising 80 obese adolescents was performed from November 2013 to September 2014. Twentyfour- hour urinary free cortisol levels were measured as a biological marker of stress. Bone turnover was evaluated using bone-specific alkaline phosphatase, serum osteocalcin, and urinary N-telopeptide concentrations. Bone mineral density was measured using dualenergy X-ray absorptiometry. The Eating Disorder Examination Questionnaire, Dutch Eating Behavior Questionnaire, Children's Depression Inventory, and the State-Trait Anxiety Inventory for Children were used to assess eating disorders, depression, and anxiety. Psychiatric examinations were performed for binge eating disorders. Results: In the Pearson's correlation test, a positive correlation was found between the 24-hour urinary cortisol level and Dutch Eating Behavior Questionnaire total and restrained eating subscale scores (p<0.05 for both). In linear regression analyses, the Dutch Eating Behavior Questionnaire total and restrained eating subscale scores were found to be significant contributors for urinary cortisol level (β=1.008, p=0.035; β=2.296, p=0.014, respectively). The femoral neck areal bone mineral density was found to be significantly higher in subjects who had binge eating disorder compared with those without binge eating disorder (p=0.049). Conclusion: Despite the lack of apparent effects on bone turnover and bone mineral density in our obese adolescents at the time of the study, our results suggest that disordered eating attitudes, and especially restrained eating attitudes, might be a source of stress. 68 ÖZET Çocukluk çağı tüberküloz enfeksiyonunun farklı klinik tiplerin de hücresel immün sistem araştırılmıştır. Bu amaçla T lenfosit, T4 ve T8 lenfosit sayısal değerleri monoklonal antikorlar kullanılarak belirlenmiş tir. Lenfosit, T ve T lenfosit alt grupları sayısal değerlerinin tüberküloz enfeksiyonunun klinik tabloları, PPD ile değerlendirilen geç aşırı deri duyarlılığı yanıtı ve kemoterapi ile ilişkisi araştırılmıştır. Çalışma grubuna yaş dağılımı 1 yaş ile 14 yaş 3 ay arasında de ğişen 26 yeni tanı konulmuş tüberkülozlu hasta alınmıştır. Kontrol grubuna yaş dağılımı 1 yaş 4 ay ile 13 yaş 5 ay arasında değişen 14 sağlıklı çocuk alınmıştır. Çalışmanın ilk bölümünde tüberkülozlu hastaların tedavi öncesi ve tedavinin 8. haftasında saptanan lenfosit, T lenfosit, T4 ve T8 lenfosit değerleri kontrol grubu değerleri ile ve tedavi öncesi ve sonrası değerle ri kendi aralarında karşılaştırılmıştır. İkinci bölümde ise tüberkülozlu hastalar Lenzini sınıflamasına göre reaktif ve nonreaktif olmak üzere iki gruba ayrılarak veriler karşılaştırılmıştır. Üçüncü bölümde ise PPD ile değerlendirilen geç aşırı deri duyarlılığı yanıtı ile lenfosit, T lenfosit ve alt grupları arasındaki ilişki incelenmiştir. Knowledge about adolescent risk behavior has expanded in the past several decades. Risk behaviors, such as substance use, violent behavior and sexual risk behavior are behaviors that can directly or indirectly compromise well-being and health of young people. Studies showed that there are many risk factors leading to engage in risk behaviors. Studies also showed that risk behaviors can result in many negative outcomes or adverse consequences. However, there is great heterogeneity in the linkage between involvement in risk behaviors and the likelihood of adverse outcomes, and there is also great heterogeneity in the linkage between exposure to risk factors and the likelihood of involvement in risk behaviors as well. These facts lead the researchers to focus on the identification and assessment of protective factors and the concept of resilliency. Researches over the past decade also lead to changing of intervention programme strategies from risk reduction to healthy youth development. These strategies aim to provide all youth with the support, relationship, experiences, resources, and opportunities needed to become successful and competent adults. They are promising approaches for preventing or reducing a wide range of adolescent health-risk behaviors. In this article, we briefly review what we know about risk behaviors, their risk and protective factors and healthy youth development, and provide definitions of each of these concepts. Giriş: Şişmanlarda bozulmuş yeme tutumlarına sık rastlanmaktadır. Ergenlik dönemi iskelet gelişimi açısından önemli bir dönemdir. Bu nedenle şişman ergenlerde bozulmuş yeme tutumlarının ergen üzerinde yarattığı stresin kemik yapım-yıkım belirteçleri ve kemik mineral yoğunluğu üzerine etkisinin araştırılması önemlidir. Amaç: Şişman ergenlerde bozulmuş yeme tutumlarının yol açtığı stresin kemik yapım-yıkım belirteçleri ve kemik mineral yoğunluğu üzerine etkisini araştırmaktır.Gereç ve Yöntem: Kasım 2013-Eylül 2014 tarihleri arasında Cerrahpaşa Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Anabilim Dalı Adolesan Bilim Dalı polikliniği'ne başvuran, 11-18 yaş aralığında 80 şişman ergen çalışmaya alındı. Kemiğe özgül alkalen fosfataz, serum osteokalsin, idrar N-telopeptit ölçümleri ile kemik yapım ve yıkımı değerlendirildi. Yirmidört saatlik idrarda serbest kortizol düzeyi biyolojik stres belirteci olarak ölçüldü. Kemik mineral yoğunluğu çift enerji x ışını absorptiometri ile ölçüldü. Bozulmuş yeme tutumları `Yeme Bozukluğu Değerlendirme Ölçeği`, `Hollanda Yeme Davranışı Anketi` ile depresyon `Çocuklar için Depresyon Ölçeği` ile değerlendirildi ve psikiyatrik bakıları yapıldı. İstatistiksel çözümlemeler için Statistical Package for Social Sciences 15.0 versiyon istatistik paket programı kullanıldı. Kategorik değişkenlerin karşılaştırılmasında Ki-kare testi ya da Fisher exact test, sürekli değişkenlerin karşılaştırılmasında Mann-Whitney U testi ya da Student T testi kullanıldı. Değişkenler arasındaki ilişki korelasyon testi ile değerlendirildi. Çok değişkenli çözümlemelerde lineer regresyon modeli kullanıldı. Bulgular: Yirmi dört saatlik idrar serbest kortizol düzeyi ile `Hollanda yeme davranışı anketi` toplam puanı ve kısıtlama alt ölçek puanı arasında anlamlı ilişki saptandı (sırasıyla p= 0,047, p=0,011).Sonuçlar: Şişman ergenlerde kemik sağlığı ile bozulmuş yeme tutumları arasındaki ilişkinin gösterilememiş olması, bozulmuş yeme tutumlarının kemik sağlığı üzerindeki olumsuz etkilerinin uzun sürede ortaya çıkmasına ve ergenlerden oluşan bu çalışma grubunda saptanamamasına bağlı olabilir. Adolescence is a critical period for bone development. So it is important to study the effect of stress that is caused by disordered eating attitudes on markers of bone turnover and bone mineral density among obese adolescents. Objectives: To examine the effect of stress that is caused by disordered eating attitudes on markers of bone turnover and bone mineral density in obese adolescents.Materials and Method: Eighty obese adolescents, between 11- 18 years old, applying to Istanbul University Medical Faculty Adolescence Department between November 2013-September 2014 enrolled in this study. Bone turnover was evaluated with the measurement of serum bone spesific alkalen phosphatase, serum osteocalcin and urine N telopeptide. Twenty four hour urine free cortisol was measured as a marker of biological stress. Bone mineral density was measured by dual energy x ray absorptiometry. Disordered eating attitudes were evaluated with `Eating Disorder Examination Questionnaire`, `Dutch Eating Behaviour Questionnaire`; depression was evaluated with `The Children's Depression Inventory` and psychiatric evaluation was done. All analysis were conducted using Statistical Package for Social Sciences 15.0 version. `Chi- square` or `Fisher exact test` was used to compare categorical variables, `Mann- Whitney U` or `Student T test` was used to compare continous variables. The relationship between the variables was evaluated with correlation test. Lineer regression model was used for multivariate analysis. Results: Statistically significant relationships were found between twenty four hour urine free cortisol and `Dutch Eating Behaviour Questionnaire` total score and restrictive eating subscale score (p in order= 0.047, 0.011).Conclusion: In this study, no relationship between disordered eating attitudes and bone health was found; it may be due to the fact that the negative effects of disordered eating attitudes on bone health were not apparent until later years and that adolescents were the study population. So we think that further prospective studies are required to understand these relationships. Amaç: Bu çalışmanın amacı, İstanbul'da Çocuk Sağlığı ve Hastalıkları dalında tıpta uzmanlık öğrencisi olarak çalışmakta olan hekimlerin adolesan sağlığı konusundaki bilgi, beceri, tutum ve uygulamalarını, ve adolesan sağlığı konusundaki eğitim gereksinimlerini saptamaktı.Gereç ve Yöntem: İstanbul'da Çocuk Sağlığı ve Hastalıkları dalında tıpta uzmanlık eğitimi almakta olan 411 hekim kurumlarında ziyaret edilerek, soru formlarını cevaplamaları istendi. Hekimlere adolesanın sağlık sorunları ile ilgili bilgileri, becerileri, yeterlilikleri, tutumları, eğitim alma istekleri ve adolesan sağlık hizmetlerinin bir parçası olarak gizlilik ilkesi ile ilgili görüşleri soruldu. Adolesanın sağlık sorunları medikal, jinekolojik ve psikososyal sorunlar, riskli davranışlar ve, yasal ve etik sorunlar olmak üzere 5 ana grupta toplandı ve hekimlerin yanıtları bu gruplara göre oluşturulan bir skorlama sistemi ile değerlendirildi. İstatistiki değerlendirme ki kare testi, Student testi, ANOVA ve post Hoc Tukey testi, ve Pearson korelasyon testi kullanılarak yapıldı.Bulgular: Çalışmaya 411 hekimden, 344'ü katıldı (%83,6). Adolesan sağlığına ilgi duyduğunu bildiren hekimlerin oranı %27,6 (n=94) idi. Adolesanlara hizmet vermekten hoşlanan hekimlerin oranı % 2,9 idi. Hekimlerin adolesanlarla çalışma isteğini olumsuz yönde etkileyen sebepler çalıştıkları kurumda medikopsikososyal yapılanmaların bulunmaması (% 47,7), çalışma koşullarının adolesanlar için gerekli mahremiyeti sağlayamaması (% 35,9) ve kendilerini deneyimsiz hissetmeleri (%23,6) olarak saptandı. Hekimler adolesanın sağlık sorunlarıyla ilgili konularda yetersiz eğitim aldıklarını, medikal sorunlarla ilgili daha fazla hasta izlediklerini ve bu hastalar karşısında kendilerini daha yeterli hissettiklerini, ancak, jinekolojik ve psikososyal sorunlar ve, riskli davranışlarla gelen hastalar karşısında kendilerini yetersiz hissettiklerini belirttiler. Adolesanın izni olmadan gizlilik ilkesini bozmayacağını belirten hekimlerin oranı %10'dan az saptandı. Adolesan sağlığı ile ilgili konularda eğitim almış olma ile adolesan hasta izleme sıklığı, hasta karşısında kendini yeterli hissetme ve 10?14 yaş gurubu adolesan hasta izlerken gizlilik ilkesine uyma arasında anlamlı korelasyon saptandı.Tartışma: Sonuçlarımız İstanbul ilindeki Çocuk Sağlığı ve Hastalıkları tıpta uzmanlık öğrencilerinin adolesan sağlığı alanındaki bilgi ve becerilerinin artırılması, tutum ve davranışlarının değişmesi gerektiğini göstermektedir. Bu nedenle, tıpta uzmanlık eğitimi sırasında kullanılmak üzere oluşturulacak bir adolesan sağlığı rehberinin bu alanda hem eğitimin standartizasyonunu sağlayabileceğini hem de hekimlerin motivasyonunu arttırabileceğini düşünmekteyiz. Purpose: The aim of this study was to Assessment of knowledge, skills, attitudes, practices and training needs of paediatric residents working at İstanbul with regard to adolescent medicine.Method: A total of 411 paediatric residents were visited at their working places and asked to complete a questionnaire. Paediatric residents were asked to report their knowledge, perceived skills, adequacies, attitudes, practices, desire for further training on adolescent medicine regarding 21 adolescent health items grouped under 5 topics (medical, gynaecologic and psychosocial problems, risky behaviours and legal and ethical issues), and thought about confidentiality issue as part of a adolescent health services. A scoring system was applied to asses their responses. Chi-square test, Student-t test, ANOVA and post Hoc Tukey test and Pearson correlation tests were used for statistical analysis.Results: Questionnaires were responded by 344 (% 83, 6) of total 411 paediatric residents. Percentage of physicians reported to be interested in adolescent medicine was % 27, 6 (n=94). Percentage of residents reported to be pleased with care of adolescents was % 2, 9. Reasons negatively affecting physicians desire for care of adolescents were determined as absence of medico- psychosocial organisations in their institutions ( % 47,7), inability to provide essential privacy due to unfeasible conditions at work (%35,9) and perception of to be underskilled regarding adolescents (%23,6). Physicians reported to be trained inadequately about adolescent?s health issues and also reported caring adolescents with medical problems mostly and feeling more comfortable with this topic. But physicians reported to feeling inadequate regarding adolescents with gynaecologic, psychosocial problems and risky behaviours. Percentage of physicians who would be adherent to confidentiality without permission of adolescent was found to be less than % 10. There was a significant correlation between being trained about adolescent medicine and frequency of caring adolescents in daily practice, feeling adequate with adolescent patients and adherence to confidentiality with adolescents of 10-14 year-of-age.Discussion: Our results demonstrated that improvement of knowledge and skills and change in attitudes and practices of paediatric residents at İstanbul regarding adolescent medicine are required. We think that preparation of a guideline for adolescent medicine to be used in paediatric residency training programme will result in standardisation of training and increased motivation of physicians for care of adolescent patients. 96

Aim: The aim of this cross-sectional study was to determine the prevalence of the self reported health behaviors and differences in these behaviors by gender and grades in high school adolescents in İzmir, Turkey., Material and Methods: A stratified cluster sampling procedure was used for this cross-sectional study. The study sample included 2 296 students attending 22 high schools in Izmir. As a data collection instrument, some questions from the Health Behavior in School-aged Children Study 2009/2010 questionnnaire and questions which were developed by the researchers to understand behaviors of internet use in adolescents were used. Chi-square tests and Cramer's V statistics were used for statistical analyses., Results: Among the high school students, 33.8% experimented smoking, 26.3% smoked cigarette during the 30 days before the survey, 14.9% smoked cigarette regularly during the 30 days before the survey, 54.1% experimented drinking alcohol, 38.4% drunk alcohol during the 30 days before the survey, 31.6% got drunk, 10.9% were adequately physically active, 59.9% watched TV for a long period of time, 72.8% used internet for a long period of time, 48.1% ate breakfast regularly, 36.2% ate adequate amount of fruit, 14.1% ate adequate amount of vegetable, 31.3% ate candies and chocolate very often, 18% drunk soft drink very often, 30.3% were bullied, 29.9% bullied others and 41% involved in a physical fight., Conclusions: These results showed that "Adolescent friendly health services" should be generalized all over Turkey, physicians should evaluate each adolescent for his/her health behaviors in each visit and implementation of prevention programs which adopt a health promotion perspective is necessary begining from the elementary school.

- Abstract
The aim of this study was to evaluate the prevalence, behavioral patterns and correlates of regular alcohol drinking in high school students. This cross-sectional study involved the completion of a modified version of "Health Behavior in School Age Children" (HBSC 1997/1998) questionnaire by 4,153 grade 9-11 students from 26 randomly selected high schools in Istanbul. Chi-square test, Spearman correlation test and forward stepwise multiple logistic regression model were used for statistical analyses as appropriate. Overall, 61% of students were experimental drinkers, and 46% of the students were current drinkers. There was a significant difference between female and male students with respect to reporting current alcohol drinking at grade 9 and 11 (p > 0.05 for each comparison). Regular drinking was reported by 6% of students. Male students were more likely than female students to report regular drinking at each grade (p < 0.01 for each comparison). Nineteen percent of the students reported that they had been really drunk at least once during their lifetime. Malestudents were more likely than female students to report an occasion of drunkenness at each grade (p < 0.05 for each comparison). All types of drinking behavior rates tended to increase across grades for both genders (p < 0.05 for each comparison). In logistic regression analysis the following were all independently associated with regular drinking: being in grade 11, smoking cigarettes currently, lifetime drug use, bullying others, being sexually active, playing computer games > or = 4 h/week, exercising < or = 1 h/week, spending > or = four evenings with friends, at ease in talking to same gender friends, tiredness in the morning, perceived as good-looking/beautiful, higher educational level of the mother and perceived poor academic achievement. The results of this study showed that alcohol consumption is prevalent among high school students. There is therefore a need for school-based alcohol prevention programs which also deal with family and peer influences on drinking.
