Amaç: Bu çal›flmada lateral nazal duvar ve sinüs patolojilerinin distallakrimal kanal t›kan›kl›¤› etiyolojisindeki rolü araflt›r›lm›flt›r.Yöntem: Nisan 1999 ile Eylül 2003 tarihleri aras›nda endoskopikendonazal dakriosistorinostomi ve silikon entübasyonu planlanan 17kad›n ve 11 erkek hasta çal›flmaya dahil edildi. Hastalar›n lakrimal kanal t›kan›kl›¤› tan›s› için göz kliniklerinde Schirmer testi, flöreseinboya kaybolma testi, Jones I-II testleri, kanaliküler irrigasyon, kanaliküler problama, dakriosistografi, dakriosintigrafi gibi genel oftalmolojik muayeneleri yap›ld›. KBB kliniklerinde nazal kavite patolojileri için anterior rinoskopi ve diagnostik nazal endoskopik muayeneleri yap›ld›. Tüm hastalarda paranazal bilgisayarl› tomografi ile ostiomeatal kompleks hastal›¤›, etmoid hücre opasifikasyonu, konka bülloza, agger nazi hücresi varl›¤› de¤erlendirilerek 50 kontrol olgusundaki bulgular ile Fisher’in ki-kare testiyle karfl›laflt›r›ld›.Bulgular: Lakrimal kanal t›kan›kl›¤›n oldu¤u tarafta agger nazi hücresi 17 (%60.7), konka bülloza 10 (%35.7), etmoid hücre opasifikasyonu 6 (%21.4), osteomeatal kompleks hastal›¤› 4 (%14.2), bir veyadaha fazla bulgu 21 (%75) hastada saptand›. Bu lateral nazal duvar vesinüs patolojileri çal›flma grubunda kontrol grubuna oranla yüksekbulunmas›na karfl›l›k istatistiksel olarak anlaml› bulunmad›(p>0.05).Sonuç: Lateral nazal duvar ve sinüs patolojilerini distal nazolakrimalsistem t›kan›kl›¤› olan hastalarda yüksek oranda bulmam›za ra¤menetiyolojisini aç›klamakta yetersiz oldu¤u ve paranazal bilgisayarl› tomografinin bu hastalar›n de¤erlendirilmesinde artan öneme sahipolaca¤› kan›s›na vard›k., Objective: In this study, the role of lateral nasal wall and sinus pathologies in the etiology of distal lacrimal duct disease has been investigated.Methods: Seventeen female and 11 male patients who were scheduledfor endoscopic endonasal dacryocystorhinostomy and silicon tube intubation between April 1999 and September 2003 were included in the study.The patients underwent general ophthalmologic examinations such asSchirmer test, fluorescein dye disappearance test, Jones I-II tests, canalicular irrigation, canalicular probing, dacryocystography, dacryoscintigraphy for the diagnosis of lacrimal duct obstruction. In the clinics of ENT,for the detection of nasal cavity pathologies, anterior rhinoscopy anddiagnostic nasal endoscopic examinations were performed. All patientswere evaluated during paranasal computed tomographic examinationsregarding osteomeatal complex disease, ethmoid cell opacification, concha bullosa and presence of agger nasi cells and data obtained were compared with findings of 50 control subjects using Fisher’s chi-square tests.Results: On the side where lacrimal duct obstruction exists, agger nasicells were detected in 17 (60.7%) patients, concha bullosa in 10 (35.7%)patients, ethmoid cell opacification in 6 (21.4%) patients, osteomeatalcomplex disease in 4 (14.2%) patients, and one or more than one symptom were detected in 21 (75%) patients. Despite higher number of lateral nasal wall and sinus pathologies in the study group when comparedwith the control group, intergroup difference was not statistically significant (p>0.05).Conclusion: We have concluded that despite the higher rates of lateral nasal wall and sinus pathologies in patients with distal nasolacrimalsystem obstruction, its etiology has not been adequately expounded andparanasal computed tomographies will have increasing importance inthe evaluation of these patients.

Amac: Bu calismada nazal polipozisli hastalarda histopatolojik bulgularin detayli olarak tanimlanarak bu bulgularin hastaligin siddeti ve astim, alerji ve kan eozinofil duzeyi gibi bazi eslik eden faktorlerle iliskisinin arastirilmasi amaclanmistir. Yontem: Calismada 2007-2011 yillari arasinda endoskopik sinus cerrahisi uygulanan yetmis-yedi nazal polipozisli hastanin (52 erkek, 25 kadin; yas ortalamasi: 46.99±13.27) verileri incelenmistir. Yapilan retrospektif degerlendirmede endoskopik skorlari, Lund-Mackay bilgisayarli tomografi skorlari, nazal obstruksiyon semptom degerlendirme skalalari, alerji ve astim varligi ve kan eozinofil duzeyleri kaydedilmistir. Bu veriler histopatolojik bulgularla (bazal membran kalinligi, goblet hucre hiperplazisi, subepitelyal odem, submukozal gland formasyonu, eozinofilik infiltrasyon, lenfositik infiltrasyon ve polimorfonukleer infiltrasyon) karsilastirilmistir. Objective It is known that cigarette smoking causes squamous metaplasia in upper and lower airways. We hypothesized that in patients with nasal polyposis, nasal polyps of smokers might reveal some histopathological changes and there might be an increased neoplasia risk. Therefore, in this study, we aimed to explore the effects of smoking on nasal polyp histopathology and clinical measures. Study design Retrospective chart review. Setting Fatih Sultan Mehmet Education and Research Hospital. Material and methods Thirty smoker and 47 non-smoker patients with nasal polyposis who underwent endoscopic sinus surgery between 2007 and 2011 were enrolled in this study to assess effect of smoking on histopathological characteristics of nasal polyps and clinical measures such as endoscopy score, computed tomography score of Lund-Mackay, Nasal Obstruction Symptom Evaluation (NOSE) Scale, presence of allergy and asthma, and blood eosinophil level. Results No statistically significant differences were found between smoker and non-smoker groups with regard to histopathological findings. Also, there were no significant correlation defined by endoscopy, computed tomography, and NOSE scores of patients between 2 groups. Smoking did not have a significant effect on the presence of allergy or asthma. But blood eosinophil levels were found to be significantly high in the smoker group. Conclusion This is the first clinical study that investigates the effect of smoking on nasal polyp histopathology. In spite of the fact that smoking is associated with histopathological changes in respiratory mucosa, we did not find any significant change in histopathological characteristics of nasal polyps in smokers.

Objective: Bell’s palsy (BP) is an idiopathic, acute peripheral facial nevre paralysis (PFP) usually affecting only one side of the face. The etiology of Bell’s palsy is unknown but viral infections, vascular ischemia, and autoimmune disorders have all been postulated as possible mechanisms. In this study we analyze patients with PFP admitted to our hospital. Material and Methods: We evaluated epidemiological, clinical, and demographical aspects of 108 patients with PFP admitted to our hospital from April 2008 to March 2010. Results: Fifty-three patients (49,1%) were female and 55 (50,9%) were male. The age of population was between 8 and 82 (the average is 45). Twenty-five of the patients had DM and 23 had HT. Three patients were pregnant. Approximately 50% of patients applied to our clinic in summer months. Eight patients had recurrent BP. One of these patients had DM and one had HT. In our study 82,4% of the patients recovered without any sequelae. Conclusion: Epidemiological and demographical features of the patients included in this study are compatible with the literature. In spite of the foreign literature published so far we found that the frequency of disease is increased during summer months.

Amaç: Bu çalışmada kronik otitis mediada (KOM) ameliyat öncesi bilgisayarlı tomografinin (BT) endikasyonları, sonuçları ve sınırları araştırıldı ve hastalığın özellikleri ve komplikasyonlarının belirlenmesinde BT’nin klinik katkısı değerlendirildi. Hastalar ve Yöntemler: Bu çapraz kesitsel çalışmada, Ocak 2008 - Ocak 2010 tarihleri arasında KOM’si olan 50 hastanın (28 erkek, 22 kadın; ort. yaş: 34; dağılım 16-69 yıl) ameliyat öncesi BT sonuçları mastoidektomi sırasındaki cerrahi bulgular ile birlikte incelendi. Bulgular: Bilgisayarlı tomografi yumuşak doku, mastoid pnömonizasyonu, dural yükseklik ve temporal kemik yıkımı açısından, neredeyse %100 duyarlılık ile, yüksek düzeyde duyarlıydı. Lateral kanal fistüller, fasiyal kanal dehisansı, tegmen timpani erozyonu ve ossiküler bütünlüğün saptanmasına sınırlı bir katkısı vardı. Sonuç: Kronik otitis medianın ameliyat öncesi BT ile değerlendirilmesi, hastalığın derecesinin değerlendirilmesinde ve ameliyat sırasında oluşabilecek komplikasyonların önlenmesinde önemli bir kılavuzdur., Objectives: This study aims to investigate the indications, the results and the limits of preoperative computed tomography (CT) scanning in chronic otitis media (COM), and to evaluate the clinical contribution of CT in determining the nature and complications of the disease. Patients and Methods: In this cross-sectional study, the results of preoperative CT with operative findings during mastoidectomy in 50 patients with COM (28 males, 22 females; mean age 34 years; range 16 to 69 years) between January 2008 and January 2010 were analyzed. Results: Computed tomography was highly sensitive to the presence of soft tissue, mastoid pneumatization, dural height, and temporal bone destruction with nearly 100% sensitivity. Its contribution to detecting lateral canal fistulas, facial canal dehiscence, tegmen tympani erosion, and ossicular integrity was limited. Conclusion: Preoperative assessment of COM with CT produce important guidance to evaluate the extent of the disease and to prevent possible intraoperative complications.

Objectives Loss of taste function was studied pre- and postoperatively in patients who underwent myringoplasty. Patients and methods Thirty-two patients who were diagnosed with chronic otitis media and underwent myringoplasty between May 2007 and May 2009 were included in the study. Regional taste test was performed preoperatively and two weeks after the operation in all patients planned to undergo myringoplasty. For patients describing disturbance in the postoperative taste test or in the questionnaire form, the taste test was repeated at six months after the operation and the questionnaire form was completed. Results Chorda tympani was preserved in 18 patients (56.2%), traumatized in nine patients (28.1%), and cut completely in five patients (15.7%). Totally in 10 out of 32 patients (31.2%), an disturbance in the taste test was found at two weeks postoperatively. Sweetness/saltiness, bitterness, and sourness taste disturbances were found in 10, nine, and two patients, respectively. Improvements were identified in two out of 10 patients in the taste test performed at six months. The disturbances in the taste tests of eight patients continued to exist at that time. Sweetness/saltiness, bitterness, and sourness taste disturbances were found in eight, seven and two patients, respectively. Conclusion The chorda tympani nerve may be traumatized or completely cut as a complication of ear surgery. Sacrificing the nerve, where it is highly traumatized by surgical devices, reduces the duration of postoperative complaints.

We investigated the presence and frequency of Helicobacter pylori in palatine and pharyngeal tonsil tissues of children.The study included 20 patients (15 males, 5 females; mean age 8.2 years; range 5 to 13 years) with chronic or recurrent tonsillitis and/or snoring. Of these, 17 patients underwent adenoidectomy and tonsillectomy, two patients underwent adenoidectomy and tonsillectomy with bilateral ventilation tube insertion, and one patient underwent adenoidectomy with bilateral ventilation tube insertion. H. pylori colonization was sought in all tonsillar specimens by rapid urease test and immunohistochemical analysis.Rapid urease test showed H. pylori colonization in the palatine and pharyngeal tonsil specimens of one patient (5%), whereas immunohistochemical analysis showed four H. pylori-positive patients (20%), whose pharyngeal tonsil (n=2) or palatine tonsil (n=2) specimens were individually affected.The presence of H. pylori in both palatine and pharyngeal tonsil tissues of children suggests that these tissues can be a reservoir for H. pylori.
