The aim of this study was to assess the temporal evolution of external cervical resorption (ECR) defects using a volumetric quantification method.Cone-beam computed tomographic (CBCT) images of patients diagnosed with ECR who chose not to receive treatment and attended recalls were collected. ECR defects were segmented in CBCT images at baseline and recall, and their volumes were quantified. The volumetric ratio of resorption defects/teeth was calculated. Three-dimensional classification of defects at baseline and recall and the prevalence of root surface perforations ≥1 mm were determined. The Wilcoxon matched pairs signed rank test, chi-square test, and linear regression models were used to analyze the data.Fifteen patients with 20 teeth diagnosed with ECR and an average recall time of 21 months were included. Nine (45%) teeth showed a change in 3-dimensional classification at recall. The volume of resorption defects (P = .0001) and the volumetric ratio of resorption defects/teeth (P = .0001) increased over time. The prevalence of root surface perforations ≥1 mm was higher at recall (n = 17, 85%) compared with baseline (n = 9, 45%) (P = .008). Linear regression models showed significant associations between the resorption defect volume at recall compared with baseline (P .0001; 95% confidence interval, 0.053-0.081) and the volumetric ratio of resorption defects/teeth at recall compared with baseline (P .0001; 95% confidence interval, 0.205-0.356). There was no association between the volume of resorption defects at recall with the length of the recall period, sex, or age (P .05).When left untreated, ECR defects can increase in size and develop more root surface perforations. ECR has a dynamic nature, and its volumetric increase over time does not result from uniform/linear expansion of the defects.

The aim of this systematic review and meta-analysis was to investigate the overall accuracy of deep learning models in detecting periapical radiolucent lesions in dental radiographs, when compared to expert clinicians.Electronic databases of Medline (via PubMed), Embase (via Ovid), Scopus, Google Scholar, and arXiv were searched. Quality of eligible studies was assessed by using Quality Assessment and Diagnostic Accuracy Tool-2 (QUADAS-2). Quantitative analyses were conducted using hierarchical logistic regression for meta-analyses on diagnostic accuracy. Subgroup analyses on different image modalities (periapical radiographs, panoramic radiographs, cone beam computed tomographic images), and on different deep learning tasks (classification, segmentation, object detection) were conducted. Certainty of evidence was assessed by using Grading of Recommendations Assessment, Development and Evaluation (GRADE) system.A total of 932 studies were screened. Eighteen studies were included in the systematic review, out of which six studies were selected for quantitative analyses. Six studies had low risk of bias. Twelve studies had risk of bias. Pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and diagnostic odds ratio of included studies (all image modalities; all tasks) were 0.925 (95% confidence interval [CI], 0.862-0.960), 0.852 (95% CI, 0.810-0.885), 6.261 (95% CI, 4.717-8.311), 0.087 (95% CI, 0.045-0.168), and 71.692 (95% CI, 29.957-171.565), respectively. No publication bias was detected (Egger's test, p=.82). GRADE showed a 'high' certainty of evidence for the studies included in the meta-analyses.Compared to expert clinicians, deep learning showed highly accurate results in detecting periapical radiolucent lesions in dental radiographs. Most studies had risk of bias. There was a lack of prospective studies.

Revitalizing the root canals of previously treated teeth with open apices is appealing to clinicians and patients. However, there are fundamental differences in the microbiome and the microenvironment between a canal with a primary endodontic infection and a canal with a persistent endodontic infection. The aims of this report are to report a case where a previously treated tooth with an open apex and a large apical radiolucency was treated successfully using regenerative endodontic treatment (RET) and to review and critically appraise the literature on procedures and outcomes of RET that result in revitalization of canal(s) in previously treated teeth with open apices. A maxillary central incisor with poor-quality root filling, a large apical radiolucency and an open apex was retreated using RET using platelet-rich fibrin as the scaffold. After 24 months, there was complete healing of the periapical lesion and obvious radiographic signs of apical root closure. Electronic searches were performed in MEDLINE, Scopus and Embase, and the baseline, procedural and outcome data of qualified articles were collected. An assessment tool was developed to rate the quality of evidence reported in these case report/series. Nine articles, three case series and six case reports, with a total of 17 teeth of all types, were included in the reports identified. The age of patients ranged from 7 to 48 years (mean: 19.4 years). The recall period ranged from 12 to 72 months (mean: 29 months). All 17 teeth survived and were functional with healing/healed outcomes. "Apical closure" was the most common radiographic finding regarding root development. The quality of evidence using the new assessment tool was rated "Excellent" in three case reports but only "Fair" in the other six articles. The present case report, as well as the review of the literature, suggests that revitalizing the root canal system of teeth with open apices and post-treatment disease using RET is a potentially valid treatment option. However, more clinical studies with higher levels of evidence and higher quality of evidence are required to confirm the viability of this treatment approach.

To determine and compare the accuracy and efficiency of a dynamic navigation system (DNS) with a freehand (FH) technique when conducting root-end resection in a human cadaver model.Forty roots in cadaver heads were included and divided into two groups: DNS (n = 20) and FH (n = 20). Cone beam computed tomography (CBCT) scans of all teeth were taken. The drilling path and depth were planned virtually to using the X-guide software (X-Nav Technologies, Lansdale, PA, USA). Osteotomy and root-end resection were done under navigation in the DNS group, and freehand under the dental operating microscope in the FH group. Post-operative CBCTs were taken. Linear deviations, angular deflection, time of operation and number of mishaps were compared with determine the accuracy and efficiency. Shapiro-Wilk, one-way ANOVA and Fisher exact tests were used (P 0.05).Linear deviations, angular deflection and operation time were significantly less in the DNS group (P 0.05). The number of mishaps was not different between the two groups (P 0.05). Subgroup analyses revealed that the distance of5 mm from buccal cortical plate was significantly associated with lower accuracy, increased operation time and greater incidence of mishaps in the FH group (P 0.05), but not in the DNS group.The dynamic navigation system was more accurate and more efficient in root-end resection in a cadaver model than the freehand technique. The distance of the roots from the buccal cortical plate had a significant negative impact on the accuracy and efficacy of the root-end resection procedure when using the freehand technique. The dynamic navigation system has the potential to be a safe and reliable technological addition to endodontic microsurgery.

Non-Hodgkin lymphoma (NHL) of the oral cavity can present with pain, swelling and radiolucent lesion mimicking endodontic diseases. This article reports on a case of diffuse large B-cell lymphoma initially diagnosed and treated as periodontal disease and then endodontic disease in the maxillary anterior and premolar area of a 40-year old female. A cone beam computed tomography (CBCT) image of the lesion was taken. The lesion was segmented using Mimics software (Materialise NV, Lueven, Belgium). Three-dimensional models of the tumor were printed. During the surgical phase teeth #4, 6, and 7 were extracted and biopsy samples were obtained. Histopathologic examination showed invasive sheets of large, atypical, basophilic cells strongly and diffusely positive for CD20. Three-dimensional analysis, segmentation, and printing of radiolucent lesions of the jaws assists with differential diagnosis and efficient treatment. Oral health professionals can play a crucial role in the early detection and diagnosis of oral NHL, thereby preventing extensive loss of function and esthetics, and even saving lives.

There is lack of data on whether the coronavirus disease 2019 (COVID-19) pandemic was associated with changes in the etiology of pathosis in endodontic patients. The aim of this study was to determine the rate of cracks and other etiologic factors during the period of March 16th to May 31st in 2020 (COVID-19 initial outbreak) and 2021 (COVID-19 ongoing pandemic) compared with figures from the same period in 2019 (pre-COVID era) in 2 endodontists' practices.The etiologies of patients' chief complaints were determined from records of 2440 teeth (740 in 2019, 651 in 2020, and 1049 in 2021). Changes in the proportion of etiologic factors among all 3 periods were analyzed. The association between the rate of cracked teeth and patients' age and sex was determined using a logistic regression model.The rates of all etiologies collectively during the studied periods showed a significant change (P .0001). The rate of cracks significantly increased in 2020 (11.8%) (P = .0001) and 2021 (8%) (P = .0018) compared with 2019 (4.3%). The rate of persistent infections decreased in 2020 (22.3%) (P = .0013) and then increased in 2021 (27.5%) (P = .0153) compared with 2019 (30%). Cracked teeth were associated with the age group of 40-60 years (odds ratio [OR] = 1.882; 95% confidence interval [CI], 1.063-3.330) in 2020 and with age ranges of 40-60 years (OR = 2.051; 95% CI, 1.120-3.759) and60 years (OR = 2.038; 95% CI, 1.050-3.956) and male sex (OR = 1.599; 95% CI, 1.019-2.510) in 2021.The rate of cracked teeth increased during the initial outbreak of the COVID-19 pandemic and 1 year later. This study provided evidence on the association between the COVID-19 pandemic and changes in the rate and presentation of endodontic etiologic factors.

Introduction SynOss Putty (Collagen Matrix, Oakland, NJ) has shown the formation of mineralized tissues when used as a scaffold in regenerative endodontic treatment (RET) in immature human teeth. The aim of this study was to compare the outcome of RET in immature ferret teeth using 2 scaffolds: a blood clot and SynOss Putty. Methods Thirty-two immature canine teeth in 8 ferrets (95–105 days old) were divided into 4 groups: group 1, no treatment (positive control, n = 8); group 2, full pulpectomy with no further treatment (negative control, n = 8); group 3, revascularization using a blood clot (n = 8); and group 4, revascularization using a SynOss Putty scaffold (n = 8). After 3 months, the animals were euthanized, and the newly formed tissues were examined histologically. The data were statistically analyzed using chi-square and Fisher exact tests. Results Normal pulps were found in group 1. No pulp tissue was found in teeth in group 2. In group 3, the pulp tissue and the odontoblastic layer were absent, and the root canal spaces were filled with a hard tissue characterized as bonelike and cementumlike tissues. All teeth except 1 in group 4 showed no hard tissue formation and intracanal/periapical inflammation. SynOss Putty was significantly associated with a lack of tissue formation and intracanal/periapical inflammation (P Conclusions Intracanal hard tissue formation was observed in immature teeth after RET using a blood clot. No tissue regeneration was found in the majority of samples using SynOss Putty as a scaffold.

The aims of this observational study were to determine if endodontists' practices in early 2021 experienced changes in patient characteristics compared with a comparable prepandemic period and to determine whether the changes reported during the initial outbreak of coronavirus disease 2019 (COVID-19) in 2020 were reversed 1 year later.Demographic, diagnostic, and procedural data of 2657 patient visits from 2 endodontist private offices from March 16 to May 31 in 2019, 2020, and 2021 were included. Bivariate analyses and multiple logistic regression models were used to examine the impact of ongoing COVID-19 pandemic on patient data.Bivariate analyses showed that patients' self-reported pain levels and the number of visits with irreversible pulpitis in 2021 were higher than 2019 (P.05). Patients' self-reported pain, percussion pain, and palpation pain levels in 2021 were less than 2020 (P .05). Multiple logistic regression analyses showed that endodontists' practices in 2021 had an increase in the number of nonsurgical root canal treatments (odds ratio [OR] = 1.482; 95% confidence interval [CI], 1.102-1.992), and apicoectomies (OR = 2.662; 95% CI, 1.416-5.004) compared with 2019. Compared with the initial outbreak in 2020, endodontists' practices in 2021 had visits with older patients (OR = 1.288; 95% CI, 1.045-1.588), less females (OR = 0.781; 95% CI, 0.635-.960), more molars (OR = 1.389; 95% CI, 1.065-1.811), and less pain on percussion (OR = 0.438; 95% CI, 0.339-0.566).The ongoing COVID-19 pandemic was associated with an increase in the number of nonsurgical root canal treatments. Some of the changes observed during the initial outbreak in 2020, including objective pain parameters, returned to normal levels 1 year later.

The study aims to evaluate the effect of bone morphogenetic protein-2 (BMP-2) and transforming growth factor-beta 1 (TGF-β1) co-stimulation on odontogenic differentiation of human dental pulp stem cells (hDPSCs).The viability/proliferation of hDPSCs treated with BMP-2 (group B), TGF-β1 (group T), or BMP-2/TGF-β1 (group BT) were evaluated. The experiments on odontogenic differentiation were done for 14 days. The following subgroups were added to investigate the effect of co-stimulation with different timing: subgroup B1, TGF-β1 co-stimulation in the first week; subgroup B2, TGF-β1 co-stimulation in the second week; subgroup T1, BMP-2 co-stimulation in the first week; and subgroup T2, BMP-2 co-stimulation in the second week. The mineralization was assessed using alizarin red staining. The expression of following genes was assessed using quantitative real-time polymerase chain reaction: dentin sialophosphoprotein (DSPP), dentin matrix protein-1 (DMP1), osteopontin (OPN), and alkaline phosphatase.All groups showed viability similar to the control group (P .05). The greater mineralization was detected in B groups on day 14. The expressions of DSPP, DMP-1, and OPN increased on day 14 (P .05). In the combination groups, the higher expressions of DSPP and DMP-1 were observed in subgroups B1 and B2 than groups B and T (P .05).BMP-2 was the key in odontogenic differentiation of hDPSCs, which was further enhanced by co-stimulation with TGF-β1. Continuous stimulation with TGFβ-1 did not improve the differentiation of hDPSCs.Combined use of the BMP-2 and TGFβ-1 at the specific sequence can provide a tissue engineering approach for the future guided dentin regeneration.

Circulating blood is a readily available scaffold when enough bleeding cannot be induced from periapical tissues during regenerative endodontic treatments (RETs). The aim of this investigation was to compare the radiographic outcome (ie, linear and 3-dimensional volumetric) of RET using periapical blood or circulating blood as scaffolds in sheep immature mandibular incisors.Thirty-two immature sheep mandibular central incisors were randomly assigned to the following groups (n = 8): the positive control groups, the pulps were removed without any treatment; the periapical blood group, RET was performed using periapical blood as the scaffold; the circulating blood group, RET was performed using circulating blood as the scaffold; and the negative control group, intact teeth without any treatment. After 8 months, micro-computed tomographic images of mandibular blocks were taken to assess the following: root length, root thickness at the midroot and cementoenamel junction levels, and the incidence of apical closure. Root structures were segmented, and root volumes were calculated and analyzed statistically.RET using periapical blood and circulating blood resulted in increases in root length and root wall thickness at the midroot and cementoenamel junction levels, the incidence of apical closure, and root volume (P.05). There were no significant differences between the RET groups and the negative control group regarding linear measurements (ie, root length, root thickness, and apical closure) (P.05). The root volumes of the 2 RET groups were similar (P.05) and were less than those observed in the negative control group (P.05).There were no significant radiographic differences between the RET groups using periapical blood and circulating blood as scaffolds. RET resulted in less root volume compared with normal root development.

Introduction The first outbreak of coronavirus disease 2019 (COVID-19) in the United States resulted in nationwide closure of dental offices that created an oral health crisis. The aim of this observational study was to analyze and compare the characteristics of patients who visited two private Endodontics offices from March-16 to May-31, 2020, compared to the same period in 2019. Methods Demographic, diagnostic, and procedural data of 1520 (693 in 2020; 827 in 2019) patient visits were collected. Bivariate and multiple logistic regression analyses were used to assess the impact of COVID-19 outbreak on patient-related variables. Results Bivariate analyses showed that the number of patient visits decreased in April and May 2020 (P 49.5 years) (odds ratio [OR], 0.720; 95% confidence interval [CI], 0.573-0.906), more patients with kidney diseases (OR, 2.690; 95% CI, 1.143-6.331), higher levels of pain on percussion (OR, 2.277; 95% CI, 1.718-3.016), less cases with previously initiated treatment (OR, 0.242; 95% CI, 0.080-0.731), less periapical diagnosis of asymptomatic apical periodontitis (OR, 0.510; 95% CI, 0.306-0.849), higher number of non-surgical root canal treatments (OR, 2.073; 95% CI, 1.397-3.074) and apicoectomies (OR, 2.799; 95% CI, 1.367-5.729). Conclusion These findings show that the public health burden of endodontic infections was more intense during the initial outbreak of COVID-19.

Introduction Scientometric analyses outline the research output in a field and, therefore, help researchers and funding agencies to focus more on underinvestigated areas and make more efficient decisions related to public health. The aim of this study was 1. to perform a scientometric analysis to characterize the status of the research and publications in the field of regenerative endodontics and 2. to identify the most effective actors (authors, countries, and journals) and to examine their role in the development of science using a bibliometric analysis. Methods Electronic searches were done in MEDLINE and Scopus databases using specified search criteria. All articles related to the field of regenerative endodontics until December 2017 were included. The total number of publication/year and the yearly growth rate of publications were calculated. The following data were extracted and analyzed: authors, country of origin, journal, number of citations, study design, and level of evidence (LOE). The visualization maps for authorship/coauthorship, citations, and country of origin of articles were prepared and analyzed. Results A total of 694 articles were included. The average yearly growth for publications was 40.4% between 2007 and 2017. The highest number of articles were ex vivo/in vitro (n = 342, 49.27%). The distribution of articles based on LOE was as follows: LOE 1, 8.34%; LOE 2, 6.00%; LOE 3, 0.66%; LOE 4, 11.67%; and LOE 5, 73.33%. Collectively, 1820 authors contributed to the publications included in this study: 1001 authors within 40 collaborating clusters and 819 authors within 149 noncollaborating clusters. The United Sates was the lead country regarding the number of publications, citations, and international collaborations. The Journal of Endodontics published the highest number of articles. Conclusions The total number of publications in the field of regenerative endodontics shows exponential growth in the past decade. However, the proportion of articles with LOE 1 is low. The research groups around the globe are encouraged to focus their collaborative efforts on higher-quality and clinically oriented research.

Introduction Histologic examination of teeth after regenerative endodontic treatment (RET) shows that the type, quality, and quantity of tissues formed in the root canal space are not predictable. The aim of this study was to examine clinically, radiographically, and histologically the outcome of RET in immature noninfected human teeth using SynOss Putty (Collagen Matrix Inc, Oakland, NJ) as a scaffold. Methods Three pairs of maxillary/mandibular first premolars in 3 patients scheduled for extraction were included. Sensibility tests confirmed the presence of vital pulps. After informed consent, anesthesia, and rubber dam isolation, the pulps were removed. RET was performed using the following scaffolds: SynOss Putty + blood in both teeth in patient #1, SynOss Putty with or without blood in patient #2, and SynOss Putty + blood or blood only in patient #3. After a follow-up period of 2.5–7.5 months, the teeth were clinically and radiographically evaluated, extracted, and examined histologically. Results Patients remained asymptomatic after treatment. Radiographic examination of the teeth showed signs of root development after treatment. In teeth treated with SynOss Putty + blood, histologic examination showed formation of intracanal mineralized tissue around the scaffold particles solidifying with newly formed cementumlike tissue on the dentinal walls. The tooth treated with SynOss Putty without blood showed the formation of a periapical lesion. The tooth treated with a blood clot only showed tissues of periodontal origin growing into the root canal space. Conclusions SynOss Putty + blood showed a predictable pattern of tissue formation and mineralization when used as a scaffold for RET in human immature noninfected teeth. The newly formed mineralized tissue solidifies with newly formed cementum on the dentinal walls.

Examining the evolution of research parameters helps scientists to discover the well-developed and neglected aspects of knowledge. Pain after root canal treatment is a health problem affecting millions of patients. Research in this field has a meaningful impact on quality of lives. The aim of this study was to analyze the evolution of research on postoperative pain over the past 50 years.Electronic searches were performed in Scopus and MEDLINE databases to identify studies on pain after nonsurgical root canal treatments/retreatments. The full texts of eligible articles were reviewed to determine the study category and to extract and analyze the methodological variables. A series of statistical analyses were performed to determine the trend of publications based on the variable of interest over time.Four hundred twenty-four articles were included. There was a positive trend for systematic reviews, studies with sample size200, studies on single-visit treatment, and clinical trials on instrumentation and adjunct treatments (P.05). There was a negative trend for the use of numeric rating scales, studies on multiple-visit treatments, clinical trials on medication/medicament, and studies on pain in maxillary incisors (P.05). No trend was observed based on pulpal diagnosis or for studies with longer observation periods (8 weeks) (P.05).A paradigm shift is needed toward clinical studies with larger sample sizes, longer observation periods, and more focus on pulpal diagnoses associated with higher rates of postoperative pain. There is a need to view postoperative pain as an important patient-centered outcome and to develop and disseminate standard reporting guidelines for future studies.

Introduction Calcified canals present a challenge during endodontic treatments. The purpose of this study was to compare the accuracy and efficiency of a dynamic navigation system (DNS) to the freehand (FH) method for locating calcified canals in human teeth. Methods Sixty human single-rooted teeth with canal obliteration were selected and mounted in dry cadaver jaws. Based on cone-beam computed tomographic scans of the jaws, the drilling path and depth were virtually planned to use X-Guide software (X-Nav Technologies, LLC, Lansdale, PA). Access preparation was made with navigation in the DNS group and without guidance in the FH group by 2 operators with different levels of experience. Postoperative cone-beam computed tomographic scans were taken of all teeth. Linear and angular deviations and reduced dentin thickness at 2 levels were measured. The time for locating the canal, the number of mishaps, and the unsuccessful attempts were determined and analyzed. Results The mean linear and angular deviations, reduced dentin thickness (at both levels), the time for access cavity preparation, and the number of mishaps in the DNS group were significantly less than the FH group (P ≤ .05). The unsuccessful attempts were not different between the 2 groups (P > .05). The time for access preparation was significantly shorter for the board-certified endodontist in the FH group (P ≤ .05). Conclusions The DNS was more accurate and more efficient than the FH technique in locating calcified canals in human teeth. This novel DNS can help clinicians avoid catastrophic mishaps during access preparation in calcified teeth.

The purpose of this study was to investigate the physical and antibacterial properties of a resin sealer mixed with a quaternary ammonium compound, dimethylaminododecyl methacrylate (DMAHDM) and nanosilver (NAg).A pilot study was completed to determine the highest concentrations of DMAHDM and NAg that did not significantly alter the physical properties (setting time, flow, solubility, and dimensional change) of AH Plus (Dentsply Sirona, York, PA) when added to the sealer. These concentrations were selected to create a modified resin sealer (mAH Plus). A modified direct contact test evaluated antibacterial properties of AH Plus, DMAHDM + AH Plus, NAg + AH Plus, and mAH Plus at days 1, 7, and 14 against Enterococcus faecalis.Concentrations of 2.5% DMAHDM and 0.15% NAg were added to AH Plus. The flow of mAH Plus was significantly decreased but still within American National Standards Institute/American Dental Association specifications. There were no significant differences in setting time, solubility, or dimensional change. On day 1, 0.15% NAg + AH Plus, 2.5% DMAHDM + AH Plus, and mAH Plus were significantly more effective against E. faecalis compared with AH Plus (P .05). On days 7 through 14, 2.5% DMAHDM + AH Plus and mAH Plus continued to be significantly more antibacterial than AH Plus (P .05).The addition of 0.15% NAg and 2.5% DMAHDM did not adversely affect the physical properties of AH Plus, and mAH Plus was significantly more antibacterial against E. faecalis.

A series of challenging cases with unusual canal anatomy in the palatal roots of maxillary first and second molars is presented. A review of the literature was done to elucidate the prevalence of anatomic variations in the palatal canal of maxillary first and second molars. An uncertain or indefinite radiographic appearance of the palatal canal, or eccentric deviation of the master cone or previous root canal filling was considered an indication of a bifurcated palatal canal. Five maxillary molars with a bifurcated palatal canal were identified. A MEDLINE database search was performed to identify studies on the palatal canal morphology of maxillary first and second molars. Data were categorized based on the methodology used in each study. The overall prevalence of anatomic variations in the palatal canal of maxillary first and second molars was less than 2%; however, anatomic variations occurred more frequently in certain ethnic groups, reaching up to 33% in maxillary first molars and up to 14% in maxillary second molars. This case series showed that even experienced endodontic clinicians can miss a bifurcated palatal canal if they are not aware of or overlook the hidden clues for these anatomic variations. The traditional assumption of an exclusively single-canal anatomy in palatal canals of maxillary molars needs to be changed, even though it is the most prevalent anatomy. The overall low percentage of more than 1 palatal canal in maxillary molars is disturbingly misleading, because in certain ethnic groups this prevalence can be considerably higher.

Introduction The goal of regenerative endodontic treatment (RET) is to regenerate the pulp-dentin complex in immature teeth with pulp necrosis. There is no consensus in using RET in previously treated teeth. This article reports on long-term outcomes of RET in previously treated immature teeth. Methods Previously treated immature teeth with signs/symptoms of failure were included. After local anesthesia and rubber dam isolation, the root canal filling material was removed without the use of chemical solvents. The root canals were irrigated with 6% sodium hypochlorite using an EndoVac (Kerr Endodontics, Culver City, CA), and a creamy mix of metronidazole and ciprofloxacin was placed in the canals for 4 weeks. At the second visit, the antibiotic dressing was removed using 17% EDTA. Bleeding was induced into the root canal space, and SynOss Putty (Collagen Matrix Inc, Oakland, NJ) was placed. Bioceramic putty was placed over the SynOss Putty, and the teeth were restored at a subsequent visit. Results Five teeth in 4 patients were included. The patients’ ages ranged from 14–46 years. Recall examinations were performed from 20 to 72 months with an average of 54.4 months. The apical openings of the teeth ranged from 1–3.7 mm. All teeth were functional and asymptomatic at the recall visits. All periapical lesions resolved with partial or complete mineralization of the root canals, and all teeth showed partial to complete apical closure at the follow-up visits. Conclusions RET can be a viable option for nonsurgical retreatment of immature teeth. This article adds a new perspective to the field of regenerative endodontics.

Introduction Clear treatment guidelines with predictable outcomes for invasive cervical root resorption (ICRR) do not exist. Recent studies show that an internal approach for the excavation and repair of resorptive lacunae using bioactive materials might be preferred over external surgical approaches in cases with extensive ICRR. The procedure appears to interrupt and stop the resorptive process and preserves the periodontium. The aim of this case series was to report on the clinical and radiographic outcomes in cases with ICRR treated with vital pulp therapy techniques using calcium-enriched mixture cement. Methods Teeth with the diagnosis of ICRR and normal pulpal and periodontal status were included. After rubber dam isolation and the removal of existing restorations, the resorptive lacunae were accessed internally, and the contents were partially or fully removed. A vital pulp therapy technique was chosen based on the accessibility of the resorptive lacunae during excavation. Teeth were then restored and followed up. Results Six teeth with ICRR (Heithersay class 2, 3, or 4) were selected. One tooth was treated with pulp capping, 1 with partial pulpotomy, 2 with full pulpotomy, and 2 with partial pulpectomy. The average follow-up time was 19.6 months. After treatment, all teeth remained functional and asymptomatic with a normal periodontium. No progression of the resorption or development of new resorption was observed. Conclusions Internal excavation of the contents of resorptive lacunae in combination with vital pulp therapy with calcium-enriched mixture cement could be a reliable approach for the treatment of ICRR.

Class 4 invasive cervical root resorption (ICRR) presents a treatment dilemma in endodontics. The widely accepted treatment options for a class 4 ICRR are to leave these teeth untreated for as long as they are asymptomatic or extraction. This report presents a conservative approach for the management of class 4 ICRR. A 28-year-old woman was referred for root canal treatment of tooth #26. The patient had a history of orthodontic treatment. Radiographic evaluation showed class 4 ICRR that had perforated the root canal space, a radiolucent crestal bony defect, and a periapical lesion. Clinically, a deep (6-mm) probing area was found on the mesial side of the tooth that bled on probing. The tooth was sensitive to percussion. After the treatment options were discussed with the patient, she chose to save the tooth. After complete chemomechanical preparation of the root canal, the entire canal space and perforation area were filled with calcium-enriched mixture cement. No attempt was made to mechanically remove the resorptive lacuna. Twenty four months after treatment, the tooth was functional and asymptomatic, and probing was within normal limits (

This report compares and evaluates the treatment outcomes of regenerative endodontic treatment and apical plug as two accepted treatment protocols in a pair of necrotic immature maxillary central incisors of a 12-year old female. The patient was referred complaining of a dull pain and swelling in her upper lip area. She had a history of trauma to the anterior maxilla two years earlier. Both teeth were clinically diagnosed with pulp necrosis and periapical radiographs revealed that separate periapical radiolucent lesions surrounded the immature apices of both teeth. The left and right incisors were treated with apical plug and regenerative endodontic treatment, respectively, using calcium-enriched mixture (CEM) cement. The patient was followed-up for three years. During this period, both teeth were clinically asymptomatic and showed complete radiographic healing of the periapical lesions. The right central incisor showed root development. No tooth discoloration was evident. Apexification by apical plug placement and pulp regeneration are both reliable treatments for immature non-vital teeth. In order to choose the right treatment the advantages of either technique should be weighed against its drawbacks. CEM cement can be successfully applied for both purposes. This biomaterial causes less discoloration of the tooth.

Iatrogenic perforations negatively impact the outcome of endodontic treatments. Studies on prognostic factors showed that perforations in the coronal third of the root with periodontal pocket formation have an unfavorable prognosis. A 36-year-old female was referred for endodontic evaluation of tooth #13 with a history of an iatrogenic perforation, happened 3 years ago. There was a sinus tract associated with perforation, 10 mm probing on the mesial and mesio-palatal, bleeding on probing, radiolucent lesion adjacent to the perforation and complete resorption of the interdental bone between teeth #13 and #12. After the treatment options were discussed, she chose to save the tooth. The tooth was accessed under rubber dam isolation, the perforation site was cleaned and disinfected using 0.5% sodium hypochlorite and sealed with calcium-enriched mixture cement. Eighteen months after treatment the tooth was functional and asymptomatic. The probing depths were normal without bleeding on probing. Radiographically, the interdental crestal bone formed between teeth #13 and #12. Despite all negative prognostic factors in this case (i.e., perforations in the coronal third, pocket formation, and radiolucent lesion), healing was unexpectedly achieved via non-surgical repair of the perforation. Further research on biological aspects of healing in the periodontium following iatrogenic perforations are recommended.

The optimal concentration for the use of endodontic topical antibiotics is not known. The aims of this study were to determine the minimum bactericidal concentrations (MBCs) and minimum inhibitory concentrations (MICs) of metronidazole, ciprofloxacin, minocycline, Augmentin (GlaxoSmithKline, Research Triangle Park, NC), and tigecycline against common endodontic pathogens and to evaluate ex vivo the antibacterial efficacy and discoloration effect of triple antibiotic paste (TAP), Augmentin, and tigecycline at different concentrations using a slow-release hydrogel scaffold.Using the Epsilometer test method (Etest; bioMérieux USA, St Louis, MO), MICs and MBCs of selected antibiotics were determined against Fusobacterium nucleatum, Porphyromonas gingivalis, Streptococcus intermedius, and Enterococcus faecalis. Biofilms of these bacterial species were then grown in extracted single-rooted teeth anaerobically for 3 weeks. Root canals were filled with TAP, Augmentin, and tigecycline at concentrations of 1 or 0.1 mg/mL in a degradable hydrogel scaffold or pure TAP at 1 g/mL for 7 days. Coronal discoloration was evaluated spectrophotometrically at 1, 2, and 3 weeks after dressing.MIC/MBC data showed significant efficacy of tigecycline, Augmentin, and minocycline compared with the other antibiotics (P .05). Significant differences were found when comparing the log10 colony-forming units of all experimental groups (P .05). TAP at 1 g/mL had no bacterial growth but caused the greatest discoloration. Hydrogel mixtures with TAP, Augmentin, or tigecycline at 1 mg/mL significantly reduced bacterial growth and the number of positive samples compared with those at 0.1 mg/mL (P .05) with minimal discoloration.TAP, Augmentin, and tigecycline in a hydrogel at 1 mg/mL reduced bacterial growth significantly with minimal color change.

The aim of this study was to compare the onset, success rate, injection pain, and post-injection pain of mental/incisive nerve block (MINB) with that of inferior alveolar nerve block (IANB) using 4% articaine in mandibular premolars with symptomatic irreversible pulpitis. The accuracy of electrical pulp test (EPT) in determining pulpal anesthesia was also examined. The study was designed as a randomized clinical trial with two study arms—MINB and IANB. Injections were performed using a standardized technique. Root canal treatment was initiated 10 min after the injection. Success was defined as no pain or mild pain during access cavity preparation and instrumentation. Injection pain and post-injection pain (up to 7 days) were recorded. All pain ratings were done using Heft-Parker Visual Analog Scale (HP VAS). Sixty-four patients were enrolled. Newer tricalcium silicate cements (TSC) may offer biocompatibility with improved working properties. This study aimed to evaluate: (1) the occurrence of mineral deposition at the interface between dentine and two TSC (ProRoot(®) MTA and Biodentine(®)) in simulated body fluid, and (2) to investigate the nature of interfacial layer.Six root dentine segments of 1.5mm thickness were obtained from extracted human teeth and were instrumented with Gates-Glidden drills. The specimens were then randomly filled with either MTA or Biodentine. The specimens were placed in the simulated body fluid containing the same phosphate concentration as blood plasma. After 4 weeks, the specimens were examined with Scanning Electron Microscope (SEM) and Energy Disperse X-ray Spectroscopy (EDX) to measure the thickness of the interfacial layer and Ca/P ratio. Transmission Electron Microscope (TEM) and Selective Area Electron Diffraction (SAED) were conducted to examine the interface ultramicroscopically and to determine the nature of the crystalline structure within interfacial layer.The thickness of interfacial layer was significantly higher in the MTA group (14.5 μm vs 4.8 μm) (p0.001). However, there was no significant difference between MTA and Biodentine in Ca/P ratio of interfacial layer (4.1 vs 2.7) (p0.05). From TEM examination, amorphous calcium phosphate (ACP) was observed in the interface along with the surface of dentine.As an alternative to MTA, Biodentine displayed bioactivity by producing an interfacial layer on the root canal dentine even though its thickness was significantly lower than MTA. ACP was observed in the interfacial layer of both biomaterials.Biodentine could be considered as an alternative to MTA due to comparable bioactivity which creates interfacial layer between root canal dentin and Biodentine.

Although the internal anatomy of mandibular molars has been extensively studied, information about middle mesial (MM) canals is limited. The primary aim of this retrospective study was to evaluate the incidence of MM canals in mandibular first and second molars. The secondary aim was to correlate the incidence of MM canals with variables of molar type, sex, age, ethnicity, and presence of a second distal canal.All mature permanent first and second mandibular molars treated from August 2012 to May 2014 were included in the analysis. After completion of root canal instrumentation in all main canals, the clinician inspected the isthmus area of the mesial root using the dental operating microscope. If there was a catch point in this area with a file or explorer, the operator spent more time attempting to negotiate an MM canal.Seventy-five mandibular first and second molars were treated during the specified period. Fifteen (20%) teeth had negotiable MM canals. The incidence of MM canals was 32.1% in patients ≤ 20 years old, 23.8% in patients 21-40 years old, and 3.8% in patients40 years. Analysis of data revealed a significant difference in the distribution of MM canals among different age groups (P.05). The differences in the distribution of MM canals based on sex, ethnicity, molar type, and presence of a second distal canal were not significant.The incidence of negotiable MM canals overall and their frequency of identification in younger patients were higher than in previous reports.

Several clear, transparent solutions are used in endodontics. Inappropriate dispensing methods can lead to accidental injection or accidental irrigation. These accidents can cause permanent tissue damage including damage to the bone, periodontium, nerves, and vasculature. This article reports on the consequences of an accidental chloroform injection. Nonsurgical retreatment of tooth #8 was planned as part of a restorative treatment plan in a 69-year-old woman. The dentist accidentally injected chloroform instead of local anesthesia because chloroform was loaded into the anesthetic syringe. The patient experienced severe pain and swelling and soft tissue necrosis and suffered permanent sensory and motor nerve damage. A review of the literature was performed on accidents caused by improper dispensary, namely accidental injections and accidental irrigations. The data were extracted and summarized. Sodium hypochlorite, chlorhexidine, formalin, formocresol, 1:1000 adrenaline, benzalkonium chloride, and lighter fuel were accidentally injected as an intraoral nerve block or as infiltration injections. Bone and soft tissue necrosis, tooth loss, and sensory nerve damage (anesthesia and paresthesia) were the most common consequences reported. Such disastrous events can be prevented by appropriate labeling and separate dispensing methods for each solution. There is a need for disseminating information on toxicity and biocompatibility of materials/solutions used in endodontics. The authors recommend training dental students and endodontic residents on immediate and long-term therapeutic management of patients when an accidental injection or accidental irrigation occurs.

Objective: The purpose of this study was to evaluate the effectiveness of the erbium, chromium: yttrium-scandium-gallium-garnet (Er,Cr:YSGG) laser in removing debris and the smear layer using two different output powers on the apical, middle, and coronal segments of root canal walls. Background data: Previous literature has failed to evaluate the exclusive effect of Er,Cr:YSGG laser on the quality of smear layer and debris removal in all three segments of the root canal space. Methods: Sixty extracted teeth were included in the study. After instrumentation, samples were divided into three experimental groups and one positive control group with no further treatment. In group 1, a final irrigation was performed using ethylenediaminetetraacetic acid (EDTA) and sodium hypochlorite (NaOCl), sequentially. In group 2, the samples were treated with a 2.78 μm Er,Cr:YSGG laser with an output power of 1.5 W. The same laser was used in group 3, but with an output power of 2.5 W. Scanning electron microscope (SEM) images from the coronal, middle, and apical thirds of the roots were prepared and evaluated for both smear layer and debris removal by three blinded observers. Results: The results showed no differences between groups 1 and 2 regarding the quality of smear layer removal in all areas. However, the 2.5 W laser failed to remove the smear layer effectively. This article presents the successful surgical management of a failed mineral trioxide aggregate (MTA) orthograde obturation of a tooth with a history of impact trauma and perforated internal root resorption. A symptomatic maxillary lateral incisor with a history of perforation due to internal root resorption and nonsurgical repair using MTA was referred. Unintentional overfill of the defect with MTA had occurred 4 yr before the initial visit. The excess MTA had since disappeared, and a radiolucent lesion adjacent to the perforation site was evident radiographically. Surgical endodontic retreatment was performed using calcium enriched mixture (CEM) cement as a repair material. Histological examination of the lesion revealed granulation tissue with chronic inflammation, and small fragments of MTA encapsulated within fibroconnective tissue. At the one and two year follow up exams, all signs and symptoms of disease had resolved and the tooth was functional. Complete radiographic healing of the lesion was observed two years after the initial visit. This case report illustrates how the selection of an appropriate approach to treatment of a perforation can affect the long term prognosis of a tooth. In addition, extrusion of MTA into a periradicular lesion should be avoided.

Background data: Various methods are used for smear layer removal in endodontics, such as the application of Er,Cr:YSGG lasers. This laser system may influence the bond strength of resin-based sealers. Methods: Sixty single-rooted extracted teeth were selected. After root canal preparation, samples were divided into two experimental groups and one positive control group (n=20 per group). In group 1, the smear layer was removed by irrigation with ethylenediaminetetraacetic acid (EDTA) and sodium hypochlorite (NaOCl). In group 2, the smear layer was removed using a 2.78 μm Er,Cr:YSGG laser with radial firing tips (RFT3) (parameters: 1.5W, 140 μs, 20 Hz, and 15% water to 15% air ratio), moving at 2 mm/sec in an apico-coronal direction. Group 3 served as a positive control group. Five specimens from each group were selected for scanning electron microscope (SEM) observation. The remaining 45 roots were obturated with RealSeal SE/Resilon and subjected to push-out tests. Data were analyzed using one way analysis of variance (ANOVA) and Tamhane's test. Results: The results showed no significant difference between push-out bond strength of root canal fillings in the EDTA+NaOCl group and the 1.5W laser group (p>0.05). The positive control group showed the lowest push-out bond strength. Conclusions: The results of the present study indicate that the application of an Er,Cr:YSGG laser with radial firing tips did not adversely affect the push-out bond strength of RealSeal SE sealer to dentin.

Introduction True regeneration of the dental pulp-dentin complex in immature teeth with necrotic pulps has not been shown histologically. It is not known to what extent this true tissue regeneration is necessary to achieve clinically acceptable outcomes. Methods This case report describes the treatment of a patient with an immature maxillary right central incisor with a history of impact trauma and enamel-dentin crown fracture. A diagnosis of pulp necrosis with acute apical abscess was established. A regenerative endodontic protocol that used a paste containing Augmentin for 5 weeks as an intracanal medicament was used. Results Follow-ups at 9, 12, 17, and 31 months revealed complete osseous healing of the periapical lesion and formation of the root apex, but without increase in root length. Clinically, the tooth was functional, asymptomatic, and nonresponsive to pulp vitality tests. The crown discolored over time. On reentering the root canal, no tissues were observed under magnification inside the root canal space. The root canal treatment was completed with mineral trioxide aggregate obturation. Conclusions Augmentin might be an acceptable choice for root canal disinfection in regenerative endodontic procedures. The protocol for regenerative endodontic treatment is not predictable for pulp-dentin regeneration. Formation of the root apex is possible without pulp regeneration.

Contemporary pulp revascularization procedures have been successful in achieving endodontic infection control and promoting the root mineralization process. However, there is still no agreement on the methods to produce predictable clinical outcomes or optimal disinfection protocols. Furthermore, there is no evidence for actual pulp regeneration in humans or in in situ animal models. This review will outline the various factors involved in undertaking this procedure, with a special emphasis on the antimicrobial strategies employed and the available information on their efficacy. Areas in which information is lacking and research is needed will also be presented.

It is not known to what extent residual infection may interfere with the success of pulp regeneration procedures. The aim of this study was to determine, radiographically and histologically, the effect of residual bacteria on the outcome of pulp regeneration mediated by a tissue-engineered construct as compared with traditional revascularization. Periapical lesions were induced in 24 canine teeth of 6 ferrets. After disinfection with 1.25% NaOCl and triple antibiotic paste, ferret dental pulp stem cells, encapsulated in a hydrogel scaffold, were injected into half the experimental teeth. The other half were treated with the traditional revascularization protocol with a blood clot scaffold. After 3 mo, block sections of the canine teeth were imaged radiographically and processed for histologic and histobacteriologic analyses. Associations between variables of interest were evaluated through mixed effects regression models. There were no significant differences between the 2 experimental groups in radiographic root development ( P > 0.05). There was a significant association between the presence of persistent periapical radiolucency and root wall thickness ( P = 0.02). There was also no significant difference in histologic findings between the 2 experimental groups ( P > 0.05). The presence of residual bacteria was significantly associated with lack of radiographic growth ( P < 0.001). The amount of dentin-associated mineralized tissue formed in teeth with residual bacteria was significantly less than in teeth with no residual bacteria ( P < 0.001). Residual bacteria have a critical negative effect on the outcome of regenerative endodontic procedures.

SUMMARY This article describes a successful direct pulp capping of a mature symptomatic mandibular second molar in a 14-year-old girl. The patient was referred with sensitivity to cold beverages and pain on chewing on the second left mandibular molar. Clinical examinations revealed extensive coronal caries and sensitivity to percussion. Radiographically, the tooth was mature and had a widened apical periodontal ligament (PDL) and a narrow periapical lesion. The concluding diagnosis was symptomatic irreversible pulpitis with symptomatic apical periodontitis. Treatment included caries removal under rubber dam isolation, capping of exposure sites with calcium-enriched mixture (CEM) cement, and permanent coronal restoration. At three-, 10-, and 15-month follow-up, the tooth was functional, had normal response to cold test, and did not have sensitivity to percussion. The PDL space regained its normal width, and the periapical lesion healed.

Mineral trioxide aggregate (MTA) is the material of choice for apical barrier techniques during root canal treatment of teeth with open apices. However, the precise control of MTA during the placement of an apical plug is challenging. This article describes the outcomes of unintentional extrusion of MTA into the periradicular tissue during apical barrier treatment in three cases.Three cases of maxillary central incisors in adult patients with open apices were referred for treatment. After conventional access and canal preparation, MTA was placed into the apical portion of the root canals to act as an apical barrier/plug. A large increment of MTA was extruded in all cases. In Case 1, after a 4-year follow-up, the extruded MTA had resorbed and the periradicular lesion had healed. In Cases 2 and 3, after follow-up, the patients remained symptomatic and were scheduled for periradicular surgery. In Case 2, soft unset particles of MTA were present in the lesion and were curetted. In Case 3, the extruded MTA had set hard but was sandwiched between the oral mucosa and bone; its removal relieved the pain experienced on buccal palpation.Extruded MTA may not harden and may be associated with ongoing periapical irritation; Extruded set MTA when encapsulated in the mucosa and not surrounded by bony matrix may act as a mechanical irritant on palpation; Mineral trioxide aggregate should be confined to the root canal system; Teeth where MTA has been extruded beyond the foramen should be followed-up to monitor the outcome.

To compare clinical and radiographic outcomes of pulpotomy treatment using calcium-enriched mixture (CEM) cement and mineral trioxide aggregate (MTA) in carious-exposed vital immature permanent first molars. Design. Fifty-one immature molars with clinical carious exposure with symptomatic/asymptomatic pulpitis met the inclusion criteria and randomly assigned to one of the treatment groups (CEM [26 teeth; 59 roots], MTA [25 teeth; 59 roots]). After performing pulpotomy and covering the radicular pulps with the biomaterials, all teeth were permanently restored. Blinded clinical and radiographic evaluations were performed at 6 and 12 months after operation for signs of success or failure. Radiographs were evaluated for complete/partial apical closure. The data were analysed using chi-square test and generalized estimating equation (GEE) model. Results. There was no significant difference at the baseline between the two experimental groups. All available cases (49 teeth) showed pulp survival and signs of continuous root development after 12 months. Overall, complete apical closure (apexogenesis) occurred in 76.8% and 73.8% of radiographically interpreted roots in CEM cement and MTA groups, respectively. There was no statistical difference in terms of radiographic outcomes between two groups. Conclusions. Calcium-enriched mixture cement and MTA showed similar performance in pulpotomy of immature caries-exposed permanent molars.

Introduction Revascularization is a valuable treatment in immature necrotic teeth that allows the continuation of root development. In this article we describe successful revascularization treatment of 2 necrotic immature first mandibular molars. Methods The clinical and radiographic examinations showed extensive coronal caries, immature roots, and periapical radiolucencies in mandibular first molars of a 9-year-old boy and an 8-year-old girl. The exam findings suggested revascularization treatment in both cases, which was started with irrigation of the canals by using NaOCl 5.25% for 20 minutes, followed by 3 weeks of triple antibiotic (metronidazole, ciprofloxacin, and minocycline) paste dressing. Next, the antibiotic paste was removed, bleeding was induced in the canals, and calcium enriched mixture (CEM) cement was placed over blood clots. Results In radiographic and clinical follow-ups both cases were asymptomatic and functional, periapical radiolucencies were healed, and roots continued to develop. Conclusions Revascularization is a realistic treatment in immature necrotic molars. In addition, placing CEM cement as a new endodontic biomaterial over the blood clot formed inside the canals provided good seal and favorable outcomes.

In comparison to the conventional apexification using calcium hydroxide, artificial apical barrier technique is more valuable and less time consuming. This article describes successful use of calcium-enriched mixture (CEM) cement as an artificial apical barrier in open apices. In this study, 13 single-rooted teeth with necrotic pulps and open apices were treated non-surgically. After copious irrigation of the root canals with NaOCl 5.25% and gentle filing, based on need for interappointment dressing, treatments were followed by CEM cement (BioniqueDent, Tehran, Iran) apical plug insertion in the first or second appointment. All cases were then permanently restored. All subjects were followed until radiographic evidence of periradicular healing was seen (mean 14.5 months). Clinically, all cases were functional and asymptomatic and complete osseous healing was observed in all the teeth. Considering the biological properties of CEM cement, this new endodontic biomaterial might be appropriate to be used as artificial apical barrier in the open apex teeth.

Aim This article describes the apexogenesis of a mandibular right second molar in a 12-year-old girl who was referred with a history of lingering pain and pain on chewing. Summary Clinical and radiographic examinations revealed extensive coronal caries and immature roots. Treatment was performed under rubber dam isolation and included caries removal followed by pulp exposure and access cavity preparation. Pulpotomy was completed, and the remaining radicular pulp was capped with calcium enriched mixture (CEM) cement (BioniqueDent, Tehran, Iran). Clinical and radiographic examinations at 3, 6, and 12 months revealed the tooth was functional with no clinical signs or symptoms of pulpal disease. The final examination confirmed complete root development as well as formation of a calcified bridge beneath the CEM cement. Key learning point • Pulpotomy with a CEM cement allowed root development to continue.

The ferret canine tooth has been introduced as a suitable model for studying dental pulp regeneration. The aim of this study was to isolate and characterize ferret dental pulp stem cells (fDPSCs) and their differentiation potential.Dental pulp stem cells were isolated from freshly extracted ferret canine teeth. The cells were examined for the expression of stem cell markers STRO-1, CD90, CD105, and CD146. The osteo/odontogenic and adipogenic differentiation potential of fDPSCs was evaluated. Osteogenic and odontogenic marker genes were evaluated using quantitative real-time polymerase chain reaction (qRT-PCR) on days 1, 4, and 8 after osteo/odontogenic induction of fDPSCs including dentin sialophosphoprotein (DSPP), dentin matrix protein-1, osteopontin, and alkaline phosphatase. Human dental pulp cells were used as the control. The results were analyzed using 3-way analysis of variance.fDPSCs were positive for STRO1, CD90, and CD105 and negative for CD146 markers with immunohistochemistry. fDPSCs showed strong osteogenic and weak adipogenic potential. The overall expression of DSPP was not significantly different between fDPSCs and human dental pulp cells. The expression of DSPP in osteo/odontogenic media was significantly higher in fDPSCs on day 4 (P .01). The overall expression of dentin matrix protein-1, osteopontin, and alkaline phosphatase was significantly higher in fDPSCs (P = .0005).fDPSCs were positive for several markers of dental pulp stem cells resembling human DPSCs and appeared to show a stronger potential to differentiate to osteoblastic rather than odontoblastic lineage.

Introduction Inflammatory external root resorption is one of the major complications after replantation of avulsed teeth. Here we report a case of inflammatory external root resorption in a maxillary left central incisor in an 11-year old male patient that was managed and treated by using calcium enriched mixture (CEM) cement. Methods The patient’s chief complaint was mobility of avulsed and replanted maxillary left central incisor and pain in chewing on left central and lateral incisors. Radiographic examination showed progressive inflammatory external root resorption of the left central incisor with an inadequately obturated root canal treatment. Both teeth were immature and had periapical radiolucencies. Both teeth were irrigated copiously with 2.5% NaOCl and obturated with CEM cement. However, the central incisor was treated with calcium hydroxide 6 weeks before CEM cement obturation. Results The clinical and radiographic examinations at 3-, 6-, 12-, 24-, and 40-month follow-up showed that treated teeth were functional with normal mobility, the progression of the inflammatory external root resorption ceased, the resorptive lacunae were filled with newly formed bone, and periapical radiolucencies healed. Conclusions Considering the biological properties of CEM cement, especially its alkalinity and sustained calcium hydroxide release, using this novel cement for treatment of inflammatory external root resorption and obturation of immature necrotic teeth might be an applicable choice.
