Background and Aim: The COVID-19 pandemic represents a sound example of inter-individual variations in disease severity ranging from asymptomatic and mild disease to severe illness, which may end up with organ failure and ultimately death. The genetic basis of the inter-individual disparities in disease severity is an area of rigorous research worldwide. This study aimed at investigating the proposed associations between disease severity and two gene polymorphisms, namely the rs699 in angiotensinogen (AGT) gene and the rs1799752 in the angiotensin converting enzyme 1 (ACE I) gene, due to the key regulatory roles of the encoded proteins in the Renin-Angiotensin-Aldosterone System (RAAS), and their linkage to risk factors (i.e., hypertension and obesity). Study Subjects and Methods: We interviewed 54 subjects who had been previously diagnosed with COVID-19. Information on demographics, COVID-19 severity and comorbidities were collected. Patients were classified based on body mass index (BMI) into three categories; lean or normal weight (NW), overweight (OW), and obese (OB), and graded based on COVID-19 severity into mild, moderate and severe disease. Whole blood samples were drawn and genotyping was performed via electrophoresis (rs1799752) or standard sequencing (rs699) of the specific polymerase chain reactions (PCR) amplicons. Results: Our findings revealed a higher frequency (41.2%) of the rs1799752 (I) allele in the OW and OB groups compared with the NW subjects (20%) (p= 0.024). The difference was more evident when limiting comparison to young (< 50 years) OW and OB versus NW individuals (50% versus 13.9%, respectively) (p= 0.0012). Females had higher frequency (70.8%) of the TC genotype and only (12.5%) of the CC, whereas males had disparate frequencies of (33.3%) and (40%), respectively (p= 0.018). The ID genotype was dominant (62.5%) whereas the DD genotype made only (18.75%) of the youth (< 50 years) in the OW and OB groups, compared with frequencies of (27.8%) and (72.2%), respectively, in the NW individuals (p= 0.012). Furthermore, the ID and DD genotypes constituted (50.0% and 41.2%), respectively, of the moderate and severe cases versus (35.0% and 50.0%), respectively, in the mild COVID-19 patients (p= 0.04). Eleven of the 34 (32.35%) moderate and severe cases of the TC-ID haplotype compared with only two cases of 20 (10.00%) in the group of mild disease. Conclusions: Our results suggest correlations between the I allele and ID genotype with obesity, and ID genotype and TC-ID haplotype with the COVID-19 severity in a cohort of Syrian patients.

هدف البحث إلى دراسة وتحليل بعض المؤشرات الإنتاجية في المرافئ السورية وذلك بناء على البيانات المتوفرة في المجموعات الإحصائية السورية الصادرة عن المكتب المركزي للإحصاء والموقع الرسمي لوزارة النقل في الجمهورية العربية السورية، والتي تتضمن بيانات عن عدد البواخر في مرفأي اللاذقية وطرطوس، وعدد الحاويات الصادرة والواردة، إضافة إلى كمية البضائع المحملة والمفرغة، كما هدف البحث إلى تحديد طبيعة العلاقة بين كمية البضائع المتداولة في المرافئ وإجمالي التجارة الخارجية في سورية خلال الفترة 2011-2021، وتوصلت الدراسة إلى أن مجموع عدد البواخر في مرفأي اللاذقية وطرطوس قد انخفض بمتوسط معدل نمو سنوي -7.53%، كما انخفضت حركة الحاويات في المرافئ السورية بمتوسط معدل نمو سنوي -7.33%، وانخفضت كمية البضائع المتداولة بمتوسط معدل نمو سنوي -6.59%، وتبين وجود علاقة طردية ذات دلالة معنوية بين كمية البضائع المتداولة في المرافئ وإجمالي التجارة الخارجية في سورية وذلك خلال الفترة 2011-2021.

This review highlights the critical role of software sensors in advancing biosystem monitoring and control by addressing the unique challenges biological systems pose. Biosystems—from cellular interactions to ecological dynamics—are characterized by intrinsic nonlinearity, temporal variability, and uncertainty, posing significant challenges for traditional monitoring approaches. A critical challenge highlighted is that what is typically measurable may not align with what needs to be monitored. Software sensors offer a transformative approach by integrating hardware sensor data with advanced computational models, enabling the indirect estimation of hard-to-measure variables, such as stress indicators, health metrics in animals and humans, and key soil properties. This article outlines advancements in sensor technologies and their integration into model-based monitoring and control systems, leveraging the capabilities of Internet of Things (IoT) devices, wearables, remote sensing, and smart sensors. It provides an overview of common methodologies for designing software sensors, focusing on the modelling process. The discussion contrasts hypothetico-deductive (mechanistic) models with inductive (data-driven) models, illustrating the trade-offs between model accuracy and interpretability. Specific case studies are presented, showcasing software sensor applications such as the use of a Kalman filter in greenhouse control, the remote detection of soil organic matter, and sound recognition algorithms for the early detection of respiratory infections in animals. Key challenges in designing software sensors, including the complexity of biological systems, inherent temporal and individual variabilities, and the trade-offs between model simplicity and predictive performance, are also discussed. This review emphasizes the potential of software sensors to enhance decision-making and promote sustainability in agriculture, healthcare, and environmental monitoring.

هدف الدراسة: دراسة النتائج الوظيفية لعلاج كسور جسم الفخذ الجراحي عند الأطفال والتي تحقق الاندمال بسرعة وبمضاعفات أقل. المواد والطرائق: شملت الدراسة 25 طفلاً ممن راجعوا مشفى تشرين الجامعي بين عامي 2021-2022 والذين خضعوا للعلاج الجراحي باستخدام الأسياخ المرنة داخل النقي، وتمت متابعة المرضى سريرياً وشعاعياً بعد الجراحة خلال فترات محددة حتى الاندمال التام. النتائج: تمت الدراسة على 25 مريضاً وكان معظمهم من الذكور 18 (72%)، وعدد الإناث 7 (28%)، وكانت أغلب الإصابات عند الأطفال ناجمة عن السقوط 20 (80%) وباقي الإصابات تالية لحادث سير 5(20%)، وتمت متابعة المرضى سريرياً وشعاعياً بعد الجراحة وتوصلت الدراسة إلى نتائج مهمة وظيفياً وشعاعياً بالنسبة لاندمال الكسور. الخلاصة: خلصت الدراسة إلى الأهمية الكبيرة لاستخدام الأسياخ المستبطنة للنقي في علاج كسور الفخذ لما يحققه من نتائج إيجابية ويقلل من الاختلاطات.

هدف الدراسة: دراسة النتائج الوظيفية في علاج هذا النمط من الكسور الشائعة باستخدام الأسياخ المرنة بهدف تحقيق الاندمال واستعادة الوظيفة والتقليل من الاختلاطات. المواد والطرائق: تمت الدراسة على 20 طفلاً ممن قبلوا في مشفى تشرين الجامعي خلال عامي 2022-2023 والذين خضعوا للعلاج الجراحي باستخدام الأسياخ المرنة المستبطنة للنقي بعد التقييم السريري والشعاعي. تمت متابعة المرضى سريرياً وشعاعياً على فترات بعد الجراحة وإعادة التقييم السريري والشعاعي. النتائج: شملت عينة الدراسة 20 مريضاً مع غلبة للذكور 15 طفلاً مقابل 5 إناث وتراوحت الأعمار بين 8-15 سنة. وكان السبب التعرض للإصابات الرياضية وحوادث السيارات. تمت متابعة المرضى سريرياً وشعاعياً بعد الجراحة. توصلت الدراسة إلى نتائج جيدة ومهمة في استعادة وظيفة الساعد واندمال الكسور. الخلاصة: خلصت هذه الدراسة إلى أهمية استخدام الأسياخ المرنة المستبطنة للنقي في علاج هذا النمط من الكسور والحصول على نتائج إيجابية مع التقليل من الاختلاطات.

هدف الدراسة : دراسة نتائج تصنيع الرباط المتصالب الأمامي باستعمال طعم وتر المأبض تنظيرياً وتأثيرها على فترة إعادة التأهيل. المواد والطرائق: شملت هذه الدراسة ٢٢ مريضاً من المراجعين لمستشفى تشرين الجامعي في اللاذقية بين عامي ٢٠٢٠-٢٠٢٣. خضع جميع مرضى الدراسة لتصنيع الرباط المتصالب الأمامي باستعمال طعم وتر المأبض تنظيرياً بعد أن تم إجراء التقييم السريري والاستقصاءات الشعاعية المناسبة . أوصي بإعادة التأهيل لجميع المرضى بعد الجراحة وتم متابعة الحالات على فترات وإعادة التقييم السريري والوظيفي للمصابين . النتائج: تكونت عينة الدراسة من ٢٢ مريضاً ، مع غلبة الذكور بنسبة 90.9٪ ، غالبية المرضى المشاركين في الدراسة كانوا ضمن الفئة العمرية بين ١٩ – ٢٥ سنة وذلك بنسبة ٤٠,٩٪ . بناء على نتائج الفحص السريري وتقييم نتائج الاستقصاءات الشعاعية بما فيها المرنان المغناطيسي ، شكلت الإصابات المزمنة أشيع الحالات في دراستنا وذلك بنسبة تقارب ٨٢٪. تترافق أذيات الرباط المتصالب الأمامي مع أذية بنى مختلفة ضمن مفصل الركبة حيث شكلت أذيات الغضروف الهلالي الأنسي غالبية الحالات بنسبة ٦٨,٢٪ . تم متابعة المرضى بعد الإجراء الجراحي بتصنيع الرباط المتصالب الأمامي باستعمال طعم وتر المأبض عبر الجراحة التنظيرية خلال فترة إعادة التأهيل وذلك على مرحلتين ( بعد ٦ أشهر ، ثم سنة ) من الجراحة . تم استخدام معايير VAS و IKDC في التقييم الوظيفي لمفصل الركبة مع تحري وجود الألم وكانت النتائج إيجابية ، حيث سجلت دراستنا غياب الألم بعد سنة من الإصابة عند ٦٣.٦٪ من مجمل المرضى . كما توصلت الدراسة إلى وجود تحسن ملحوظ في وظائف الركبة بحسب تقييم IKDC حيث كانت النتائج ممتازة عند ١٨٪ من المرضى بعد ٦ أشهر من الجراحة لكن النسبة ارتفعت بشكل ملحوظ لتبلغ ٥٤,٥٪ بعد سنة من الجراحة. الخلاصة : أظهرت هذه الدراسة النتائج الإيجابية لعلاج تمزق الرباط المتصالب الأمامي باستعمال طعم وتر المأبض تنظيرياً ، مما يثبت دور الجراحة التنظيرية الدقيقة وأهمية التقنية الجراحية السابقة في تدبير مثل هذه الحالات.

الهدف: تقييم النتائج الوظيفية عند المرضى المعالجين وتقليل الاختلاطات التي يمكن أن تحدث. المواد والطرائق: تمت الدراسة المقطعية عند عشرين مريضاً معالجاً في الشعبة العظمية في مستشفى تشرين الجامعي يشكون من متلازمة نفق الرسغ في الأعوام 2020 و2023، حيث تم أخذ قصة مفصلة ومعلومات كافية عن كل مريض. النتائج: أظهرت النتائج شيوع هذه المتلازمة عند النساء في اليد المسيطرة وخاصةً عند الأعمار (41-60)، وأبدت تحسن الأعراض خلال الأشهر الثلاث التالية للجراحة وعودة للعمل خلال فترة 13 يوماً. وكانت الاختلاطات قليلة تمثلت بألم الضرة وورم دموي ونكس واحد. الخلاصة: شيوع متلازمة نفق الرسغ عند الإناث في الأعمار المتوسطة وفي اليد المسيطرة وقلة حدوث الاختلاطات بعد الجراحة التنظيرية.

Abstract Background The introduction of sialendoscopy has been a major step forward in providing an accurate means of diagnosing and locating submandibular sialolithiasis, also in permitting minimally invasive surgical treatment that can successfully manage those stones aiming for gland preservation. Methods We conducted a systemic review to assess the efficacy and safety of combined sialendoscopic assisted surgery in the management of submandibular sialolithiasis. This study included published medical articles from 2000 till 2022 concerning the efficacy and safety of combined sialendoscopic-assisted surgery in management of submandibular sialolithiasis searching Medline database (PubMed), Web of Science, Springer, Cochrane library, EMBASE. Fifteen articles met the inclusion criteria and were reviewed and analyzed. Results The success rate of combined sialendoscopic assisted surgery in management of submandibular sialolithiasis is 0.955% with 95% CI: (91.4% to 98.5). The overall percentage of sialadenectomy cases in the included studies is 2%. The overall percentage of complications in the included studies is 8%. Conclusion Combined sialendoscopic-assisted surgery is efficient and safe in management of submandibular sialolithiasis.

Abstract Spatial cognition is a pivotal consideration in hospital buildings. Potential circulation configuration alternatives in hospitals have been filtered to 59 significant alternatives, to be simulated using space syntax analyses for measuring connectivity, visual integration, and visual step depth. Best alternatives have been determined via comparison and correlation analysis. A survey has been conducted, comparing the preferences of professional architects with simulation outcomes. Results show that best and worst alternatives exist in triangular and circular buildings, respectively. Also, cul-de-sac corridors are argued to promote spatial cognition better than looped corridors. Notably, cognition is inversely proportional to the curvature of the circulation spaces, number of nodes, and the number of branches. The survey showed a weak perception of the best alternatives, while respondents have a better perception of highly visible locations in these alternatives.

This work developed high drug-load pellets for colon targeting in minimal steps by coupling hot-melt extrusion (HME) with a die-surface cutting pelletizer, offering a potential continuous pellet manufacturing process. Ketoprofen (KTP) was selected as a model drug for this study due to its thermal stability and severe upper gastrointestinal side effects. Low and high methoxyl grade pectins were the enzyme-triggered release matrix, and hydroxypropyl methylcellulose (HME 4 M/HME 100LV) was used as a premature release-retarding agent. The powder X-ray diffraction technique and the differential scanning calorimetry results revealed that KTP exists in the solid-solution state within the polymeric matrix after the HME step. The scanning electron micrographs of the fabricated pellets showed a smooth surface without any cracks. The lead formulation showed the lowest premature drug release (∼13%) with an extended KTP release profile over a 24 h period in the presence and absence of the release-triggering enzyme. The lead formulation was stable for 3 months at accelerated stability conditions (40 °C/75 ± 5% RH) concerning drug content, in vitro release, and thermal characteristics. In summary, coupling HME and pelletization processes could be a promising technology for developing colon-targeted drug delivery systems.

The management of R/M HNSCC is rapidly evolving with new available treatment molecules and combination modalities. Anti-EGFR cetuximab (CTX) and immune checkpoint inhibitors (ICI) can be used either alone or in combination with conventional platinum-based doublet chemotherapy (with taxanes or fluorouracil). No data have been reported to date on the association of doublet chemotherapy concomitantly with both CTX and ICI. We present a case series of patients treated with 4 cycles of quadritherapy, every 3 weeks, including paclitaxel 175 mg/m2, carboplatin AUC 5, pembrolizumab 200 mg, and weekly 250 mg/m2 CTX. All patients achieved an objective response (6 complete responses, 2 partial responses). Clinical response was fast, so 1 patient avoided an emergency tracheostomy for laryngeal dyspnea. Four patients furtherly benefited from cisplatin-based chemoradiotherapy on residual tumor sites after the response to quadritherapy. Adverse events were manageable, except for an ICI-related liver toxicity in a patient. Overall, this short series indicates that a quadruple therapy with carboplatin-paclitaxel-CTX and pembrolizumab seems to be safe and active in patients with R/M HNSCC. This observation could be confirmed through further clinical trials.

Quetiapine fumarate (QTF) was approved for the treatment of schizophrenia and acute manic episodes. QTF can also be used as an adjunctive treatment for major depressive disorders. QTF oral bioavailability is limited due to its poor aqueous solubility and pre-systemic metabolism. The objective of the current investigation was the formulation development and manufacturing of solid self-nanoemulsifying drug delivery system (S-SNEDDS) formulation through a single-step continuous hot-melt extrusion (HME) process to address these drawbacks. In this study, Capmul® MCM, Gelucire® 48/16, and propylene glycol were selected as oil, surfactant, and co-surfactant, respectively, for the preparation of S-SNEDDS. Soluplus® and Klucel™ EF (1:1) were selected as the solid carrier. Response surface methodology in the form of central composite design (CCD) was utilized in the current experimental design to develop the S-SNEDDS formulations via a continuous HME technology. The developed formulations were evaluated for self-emulsifying properties, particle size distribution, thermal behavior, crystallinity, morphology, physicochemical incompatibility, accelerated stability, and in vitro drug release studies. The globule size and emulsification time of the optimized SNEDDS formulation was 92.27 ± 3.4 nm and 3.4 ± 3.38 min. The differential scanning calorimetry (DSC) and powder X-ray diffraction (PXRD) studies revealed the amorphous nature of the drug within the formulation. There were no drug-excipient incompatibilities observed following the Fourier transform infrared (FTIR) spectroscopy. The optimized formulation showed an extended-release profile for 24 h. The optimized formulation was stable for three months (last time-point tested) at 40 °C/75% RH. Therefore, the developed S-SNEDDS formulation could be an effective oral delivery platform for QTF and could lead to better therapeutic outcomes.

Systemic lupus erythematosus (SLE) susceptibility has a strong genetic component. Genome-wide association studies (GWAS) across trans-ancestral populations show both common and distinct genetic variants of susceptibility across European and Asian ancestries, while many other ethnic populations remain underexplored. We conducted the first SLE GWAS on Egyptians–an admixed North African/Middle Eastern population–using 537 patients and 883 controls. To identify novel susceptibility loci and replicate previously known loci, we performed imputation-based association analysis with 6,382,276 SNPs while accounting for individual admixture. We validated the association analysis using adaptive permutation tests (n = 109). We identified a novel genome-wide significant locus near IRS1/miR-5702 (Pcorrected = 1.98 × 10−8) and eight novel suggestive loci (Pcorrected < 1.0 × 10−5). We also replicated (Pperm < 0.01) 97 previously known loci with at least one associated nearby SNP, with ITGAM, DEF6-PPARD and IRF5 the top three replicated loci. SNPs correlated (r2 > 0.8) with lead SNPs from four suggestive loci (ARMC9, DIAPH3, IFLDT1, and ENTPD3) were associated with differential gene expression (3.5 × 10−95 < p < 1.0 × 10−2) across diverse tissues. These loci are involved in cellular proliferation and invasion—pathways prominent in lupus and nephritis. Our study highlights the utility of GWAS in an admixed Egyptian population for delineating new genetic associations and for understanding SLE pathogenesis.
