Background: Chronic obstructive pulmonary disease (COPD) is a progressive airflow limitation that is preventable but not curable. It is associated with persistent symptoms that cause a considerable burden on individual productivity at work, and daily activities, and reduced quality of life, also burdening the healthcare system and society. Objectives: The study aims to measure the burden of COPD on patients in terms of daily activities and work productivity. It also seeks to investigate some inflammatory biomarkers' levels and their correlation with selected outcomes. Patients and Methods: A cross-sectional study on 120 stable COPD patients who were diagnosed and treated according to the GOLD guidelines at Kirkuk General Hospital's chest and Internal Medicine consultation clinics, and in private internist clinics in Kirkuk City, Iraq between January and April 2023. The Work Productivity and Activity Impairment Questionnaire (WPAI-COPD) measured the Burden of COPD. The symptom burden was assessed by the COPD assessment test (CAT). The level of IL-6 monoclonal antibodies was measured via a Roche Cobas 6000 analyzer and TNF-α using the ELISA sandwich technique. All these devices and machines were authorized by the Ministry of Health, Iraq. Results: The mean age was (54.1 ± 8.12) years, 62.5% being males and 37.5% being females; 41.7% were public sector employees, and 39.2% were self-employed. Due to COPD, the overall work loss was (45.9±18.91) days, the mean percentage of absenteeism was (10.9% ± 12.31%), activity impairment was (47.5% ±17.79%) among all patients, presenteeism was (40.8 ± 15.05%,) and COPD-related retirees were 14.2% of the study group. The mean CAT score (19.3±5.63) was high and the most troubling symptom among patients was chest tightness. WPAI-COPD scores were higher in older age and longer disease duration. The levels of biomarkers were above the reference ranges, the mean IL-6 level was (69.4 ± 35.29) pg/ml, and the mean TNF-α was (72.3 ± 22.45) pg/ ml. Conclusions: COPD patients exhibit a disease burden in terms of productivity loss at work and activity impairment that increases with aging and disease duration. COPD patients with low BMI are more prone to a decline in lung functions and to worse symptoms.

5. Impact of Gliclazide Modified Release or Glimepiride as Add-on Therapy to Metformin on Glycemic and Oxidative Stress Parameters in Type 2 Diabetic Patients

Background: Type 2 diabetes mellitus is a condition characterized by an elevation of oxidative stress, which has been implicated in diabetic progression and its vascular complications. Aim: Assessing the impact of gliclazide modified release (MR) versus glimepiride on oxidative stress markers, glycemic indices, lipid profile, and estimated glomerular filtration rate in uncontrolled type 2 diabetic patients on metformin monotherapy. Methods: This was an observational comparative study conducted in Thi-Qar specialized diabetic, endocrine, and metabolism center. Sixty-six patients were randomized into two groups based on the addition of the sulfonylureas (SUs). Group 1 (33 patients) was on gliclazide MR, whereas Group 2 (33 patients) was on glimepiride. The measured oxidative stress markers were reduced glutathione (GSH), superoxide dismutase (SOD), malondialdehyde (MDA), and protein carbonyl (PC) evaluated before and after 16 weeks of SUs addition. Results: There were significant drops in SOD (P < 0.001), MDA (P < 0.001), and PC (P = 0.001) and a significant increase in GSH (p = 0.029) levels after gliclazide MR add-on therapy. There were significant drops in SOD (P = 0.026) and MDA (P < 0.001) levels with non-significant changes in both GSH (P = 0.214) and PC (P = 0.538) after glimepiride add-on therapy. There was a significant difference in improvement of PC level (P = 0.048) in the gliclazide group compared to the glimepiride group, with a non-significant numerically higher improvement of GSH, SOD, and MDA in gliclazide MR than glimepiride. At the end of the study, there were no significant differences in glycemic control, lipid profile, or eGFR improvement between the two groups. Conclusion: Glycemic control plays a pivotal role in decreasing oxidative stress. The control of diabetes with the gliclazide-MR-metformin combination reduced oxidative stress more than the glimepiride-metformin combination, indicating its antioxidant property. This research aimed to devise a practical solution by introducing a personalized alert system tailored to individual lung function and environmental conditions, offering a holistic approach for the management of a range of chronic respiratory conditions.Methods: In response to these challenges, we developed a personalized alert system based on individual lung function tests conducted in diverse environmental conditions, as determined by air-quality sensors. Our research was substantiated through an observational pilot study involving twelve healthy participants. These participants were exposed to varying air quality, temperature, and humidity conditions, and their lung function, as indicated by peak expiratory flow (PEF) values, was monitored.Results: The study revealed pronounced variability in pulmonary responses across different environments. Leveraging these findings, we proposed a design of a personalized alarm system that monitors air quality in real-time and issues alerts under potentially unfavorable environmental conditions. Additionally, we investigated the use of basic machine learning techniques to predict PEF values in these varied environmental settings.Discussion: The proposed system offers a proactive approach for individuals, particularly those with asthma, to actively manage their respiratory health. By providing real-time monitoring and personalized alerts, it aims to minimize exposure to potential asthma triggers. Ultimately, our system seeks to empower individuals with the tools for timely intervention, potentially reducing discomfort and enhancing management of asthma symptoms.Keywords: respiratory health, real-time air-quality monitoring, lung functionality, alert system

Background and purposeAcute leptomeningeal collateral flow is vital for maintaining perfusion to penumbral tissue in acute ischemic stroke caused by large-vessel occlusion. In this study, we aimed to investigate the clinically available indicators of leptomeningeal collateral variability in embolic large-vessel occlusion.Materials and methodsAmong prospectively registered consecutive patients with acute embolic anterior circulation large-vessel occlusion treated with thrombectomy, we analyzed 108 patients admitted from January 2015 to December 2019 who underwent evaluation of leptomeningeal collateral status on pretreatment CTA. Clinical characteristics, extent of leukoaraiosis on MR imaging, embolic stroke subtype, time of imaging, occlusive thrombus characteristics, presenting stroke severity, and clinical outcome were collected. The clinical indicators of good collateral status (>50% collateral filling of the occluded territory) were analyzed using multivariate logistic regression analysis.ResultsGood collateral status was present in 67 patients (62%) and associated with independent functional outcomes at 3 months. Reduced leukoaraiosis (total Fazekas score, 0-2) was positively related to good collateral status (OR, 9.57; 95% CI, 2.49-47.75), while the cardioembolic stroke mechanism was inversely related to good collateral status (OR, 0.17; 95% CI, 0.02-0.87). In 82 patients with cardioembolic stroke, shorter thrombus length (OR, 0.91 per millimeter increase; 95% CI, 0.82-0.99) and reduced leukoaraiosis (OR, 5.79; 95% CI, 1.40-29.61) were independently related to good collateral status.ConclusionsAmong patients with embolic large-vessel occlusion, reduced leukoaraiosis, noncardiac embolism mechanisms including embolisms of arterial or undetermined origin, and shorter thrombus length in cardioembolism are indicators of good collateral flow.

A reinforced concrete beam with shape memory alloy rebar (SMA) is a novel form of smart beam used in smart seismic structural systems to reduce the effects of earthquakes while keeping nearly the same load-carrying capability as conventional concrete beams. The experimental research investigation is carried out to study beams' flexural behavior by replacing some steel rebars with shape memory alloy rebars (SMAs) in the longitudinal reinforcement zone. The experimental program included testing three beams to investigate the effects of the replacement of longitudinal steel rebar by shape memory alloy rebar on the flexure behaviour of beams. The beams were tested by the repeated load. The study was focused on determining ultimate load, maximum deflection, and load-deflection behaviour. Experimentally, the flexure behaviour beams are significantly affected by changing the number of reinforcing bars with shape memory alloys (SMA) longitudinal direction. However, for using one rebar of shape memory alloy as a longitudinal reinforcement, the replacement of one bar and two bars of shape memory alloy SMA beams have less yield load than the control beam by about (12.5%) and (37.5%) respectively. The replacement of one bar and two bars of shape memory alloy SMA beams having less ultimate load compared with reference beam by about (9.82%) and (21.94%) respectively. Because of its superelasticity quality, the introduction of superelastic SMA bars to the beam shows excellent recentering ability.

14. An experimental study of forced vibration on natural convection between closed ended concentric and eccentric annular of horizontal cylinder

An experimental study has been done into the effects of vertical mechanical vibrating, vertical eccentricity, and the Rayleigh number on natural convection heat transferring out of a horizontally enclosed, ending cylindrical annulus with a radius rate of 2.6 and an aspect ratio of (2:1). The annulus produced between two concentric and vertically eccentric circular cylinders is positioned horizontally, and its internal wall is uniformly heated while isothermally cooling the external wall. The range of present conditions for Rayleigh number is 5×〖10〗^4≤Ra≤6.48×〖10〗^6 , and Pr= 0.703, the frequency of vibration is 0f 20Hz; and the amplitude is b mm), with possible exclusion of the highest positive and negative eccentricities. Plots of the average Nusselt number variation against the Rayleigh number showed a significant increase in negative vertical eccentricity. It was found that the average Nusselt decreased as the internal cylinder changed its location vertically from negative to positive through the center, which is normally a desirable effect, but has no advantage over the concentric on the positive side. However, an increase in the Rayleigh number leads to a nearly proportional increase in the average Nusselt number and a smaller yet still substantial increase in positive eccentricity.

Our objective was to assess the PHC Physicians’ readiness and barriers to identify, screen, and respond to IPV in Saudi Arabia.Methods: A cross-sectional study recruited physicians working in PHC centers in Saudi Arabia. Data was collected using a modified online self-administered questionnaire based on the PREMIS “The Physician Readiness to Identify and Manage IPV.” The questionnaire consisted of respondent profile, perceived preparedness and knowledge, actual knowledge, practice issues, and opinion regarding barriers.Results: Among 169 PHC physicians, 60.9% had never experienced any formal IPV training. Around one-fifth of participants have a good perceived and actual knowledge, whereas one-third have a good perceived preparedness. Nearly half of the participants (46.7%) do not screen for IPV and two-thirds of them (66.3%) have never identified an IPV case during the previous 6 months. The logistic regression model showed that family physicians were 2.27 times more likely to have a good knowledge than a general practitioner, and participants with IPV training were more likely to have a good level of perceived preparedness, perceived knowledge, and more likely to perform screening of IPV.Conclusion: The low level of PHC physicians’ readiness to identify and respond to IPV is worrisome. Findings emphasize the urgent need for an IPV training program, a supportive work environment, and a clear referral system in order to help the practitioner to provide comprehensive services and ensure safety plans for abused women.Keywords: intimate partner violence, primary health care, women, family physicians, Saudi Arabia

16. An investigation of relational behaviours and supply chain risk information sharing : a mixed method study of firms in Nigeria

Supply chain risk information sharing is one of the proactive strategies for mitigating risks and making supply chains more resilient. The ability to sense threats before they disrupt the supply chain is strengthened by risk information from supply chain partners and other stakeholders who are usually not mandated to share risk information. This study responds to calls (Sheffi and Rice Jr., 2005; Juttner and Maklan, 2011; Johnson, Elliott and Drake, 2013) for advancing the research on supply chain risk management especially in the developing country context which Tukamuhabwa, Stevenson and Busby(2017) argues that the way in which threats are handled may differ. Antecedents such as trust, relationship length, commitment and reciprocity have been identified as relational enablers in the literature; however, they might not be sufficient or be the only enablers for firms to share supply chain risk information from the onset of a supply chain relationship, especially in the context of Nigeria. Apart from the cultural peculiarities of Nigeria, which affect business practices and relationships, there is on-going insurgency in the North-east and other human-made risk events that disrupt supply chains in the country. Hence, there is the need to carry out research of this kind that investigates how firms in Nigeria mitigate supply chain risk by leveraging on their informal relationships to share and receive risk information. This thesis addresses this research gap by first focussing on collecting qualitative data through semistructured interviews from supply chain managers, about the relational behaviours they leverage for supply chain risk information sharing. Data from the interviews were transcribed and coded for thematic analysis. Three propositions relating to relational closeness, relational incentive, and collective prosperity emerged. Subsequently, the result of the qualitative strand was used to develop a survey instrument and was administered to members of the Chartered Institute of Procurement and Supply in Nigeria. Data for the quantitative strand was collected through an online and self-administered questionnaire. Partial least squares structural equation modelling was used to analyse the quantitative data. The result indicated that relational closeness and collective prosperity have a significant influence on supply chain risk information sharing. However, the result does not find support for the relationship between relational incentive and supply chain risk information sharing. A mixed method discussion was further presented to explain how the quantitative findings generalised the qualitative result. The originality of this research lies in its attempt to integrate social capital and social network theories with supply chain management literature to create new knowledge of how to mitigate supply chain risk in the Nigerian context. The overall theoretical implication of this study is that it contributes to supply chain management literature by identifying new relational attributes that are vital in enhancing supply chain risk information sharing. Regarding the managerial implication, this research highlights the need to consider investing in social relationships, particularly through relational closeness and collective prosperity as a means of receiving and sharing supply chain risk information in the sample firms. Although the findings of this study are only generalised to the sample, the findings could be insightful to multinational firms operating or expanding their supply chain to Nigeria on the need to enhance relational closeness and collective prosperity for supply chain risk information sharing. In light of the findings of this study, limitations and areas for future research were outlined.

Located 50 miles west of Fort Collins, Colorado, Colorado State University’s Mountain Campus in Pingree Park hosted the 23rd annual Rocky Mountain Virology Association meeting in 2023 with 116 participants. The 3-day event at the end of September consisted of 28 talks and 43 posters that covered the topics of viral evolution and surveillance, developments in prion research, arboviruses and vector biology, host–virus interactions, and viral immunity and vaccines. This year’s Randall Jay Cohrs keynote presentation covered the topic of One Health and emerging coronaviruses. This timely discussion covered the importance of global disease surveillance, international collaboration, and trans-disciplinary research teams to prevent and control future pandemics. Peak fall colors flanked the campus and glowed along the multiple mountain peaks, allowing for pristine views while discussing science and networking, or engaging in mountain activities like fly fishing and hiking. On behalf of the Rocky Mountain Virology Association, this report summarizes select presentations from the 23rd annual meeting.

18. Development of Biofertilizer from Locally Sourced Materials

The appropriate rationing of NPK is a core problem in biofertilizer production. In this study, a 30-liter bench-scale anaerobic biodigester for biofertilizer production from solid waste was fabricated using galvanized steel sheet. About 4kg of chicken dung, 7.5 kg of wastewater treatment sludge, and 0.5 kg of banana peel are mixed, a total of 12kg, the substrate is then mixed with water in a ratio of 1:1. (w/w) 24 kg of the slurry is fed into the biodigester. The slurry is allowed to stay for 37 days at mesophilic temperature. The pH of the digestion is between 6-7, and the temperature is within the range of 25-34oC. The amount of total solid, moisture content, ash, and volatile matter of the feedstock after digestion decreases by about 31%,47%,67%, and 69%. Moreover, an increase in the amount of nitrogen content from 0.3783% to 0.6420%, phosphorous from 0.1903% to 0.2983%, and potassium from 0.1876% to 0.3153% was also observed. After digestion the biofertilizer produced has an appropriate ratio of 2:1:1. For the kinetic study, the specific growth was found to be 0.0098hr-1. Also, experimental data for microbial growth obtained from the study fitted the Monod model.

19. Thermal Performance of Double Pass Solar Air Heater With Tubular Solar Absorber

In this investigation, the effect of replacing the conventional solar absorber with a new solar absorber on the thermal performance of a double-pass solar air heater has been studied experimentally and numerically. Three configurations have been introduced, the first configuration is a double pass solar air heater with a flat plate solar absorber (DPSAHWFP) for the aim of comparison, and the second configuration is a double pass solar air heater with a tubular absorber that includes a set of tubes which are fitted perpendicularly to the direction of airflow (DPSAHWT-1), and the third configuration is double-pass solar air heater with a tubular absorber that involves set of tubes which are fitted in parallel to the direction of airflow (DPSAHWT-2). The experiments have been carried out under indoor conditions at a constant heat flux equal to 1000 W/m2 and different air mass flow rates (0.01– 0.03 kg/s). The results revealed that the air mass flow rate has a substantial impact compared to the rise in air temperature, hence, the thermal performance of solar air heater is directly proportional to increase air mass flow rate. In addition, the experimental and numerical outcomes indicated that for all air flow rates. The (DPSAHWT-2) offers higher thermal performance as compared to other models, where the maximum effective efficiency has been obtained at 0.03 kg/s equal to 80.9 %. Moreover, (DPSAHWT-2) is more efficient than DPSAHWFP and DPSAHWT-1 by 4.2 % and 9.8 % respectively.

- Abstract
Background: Disorders affecting the nervous system are diverse and include neurodevelopmental disorders, late-life neurodegeneration, and newly emergent conditions, such as cognitive impairment following COVID-19. Previous publications from the Global Burden of Disease, Injuries, and Risk Factor Study estimated the burden of 15 neurological conditions in 2015 and 2016, but these analyses did not include neurodevelopmental disorders, as defined by the International Classification of Diseases (ICD)-11, or a subset of cases of congenital, neonatal, and infectious conditions that cause neurological damage. Here, we estimate nervous system health loss caused by 37 unique conditions and their associated risk factors globally, regionally, and nationally from 1990 to 2021. Methods: We estimated mortality, prevalence, years lived with disability (YLDs), years of life lost (YLLs), and disability-adjusted life-years (DALYs), with corresponding 95% uncertainty intervals (UIs), by age and sex in 204 countries and territories, from 1990 to 2021. We included morbidity and deaths due to neurological conditions, for which health loss is directly due to damage to the CNS or peripheral nervous system. We also isolated neurological health loss from conditions for which nervous system morbidity is a consequence, but not the primary feature, including a subset of congenital conditions (ie, chromosomal anomalies and congenital birth defects), neonatal conditions (ie, jaundice, preterm birth, and sepsis), infectious diseases (ie, COVID-19, cystic echinococcosis, malaria, syphilis, and Zika virus disease), and diabetic neuropathy. By conducting a sequela-level analysis of the health outcomes for these conditions, only cases where nervous system damage occurred were included, and YLDs were recalculated to isolate the non-fatal burden directly attributable to nervous system health loss. This essay is based on the author's long-time observation of the International Relations discipline and the repeated crises it has experienced. The piece identifies 'event-drivenness ' as the structural reason behind these crises, in other words, the course of the discipline naturally follows global IR events and, depending on how transformative these events are, when responding to them, is more likely to fall into an existential crisis--with the most recent one being potentially fatal. By discussing in detail the a) science-statecraft relationship; b) scholar-practitioner disconnect; c) distortion of theories by scholars and practitioners; d) paradoxical relationship between rationality and irrationality; and e) theory-practice disconnect, the essay seeks to operationalize these crisis-generated processes when responding to major events. In order to show these crisis generation processes in detail, it uses the theories of political realism and, to a lesser degree, classical liberalism, as case reflections. As a possible solution to the reciprocal condescension between scholarship (theory-making) and statecraft (practice), the essay proposes a 'Clausewitzian' modus vivendi that aims at creating a culture of synthesis between the presumed producers and consumers of IR knowledge. 25. Evaluating the Impact of ICT on Teaching and Learning: A Study of Palestinian Students' and Teachers' Perceptions

- Abstract
This study aimed to investigate the impact of ICT on teaching and learning from the point-of-view of Palestinian students and teachers. A total of 207 school teachers and 276 students from 53 schools taking part in an ICT project in Palestine responded to a questionnaire survey. Results indicated that students in Palestinian public schools perceived ICT to have a moderate influence on their learning. Students indicated that they face frequent challenges such as: lesson duration, access to modern devices and issues with information research skills. These results contrasted with school teachers' views, which reflected a much stronger impression of the influence of ICT on teaching.

Background: Asthma is a complicated condition characterized by chronic airway inflammation and airflow restriction, resulting in various respiratory symptoms such as shortness of breath, wheezing, coughing, and chest tightness. The research intended to evaluate and compare the clinical outcomes of two interchangeable treatment regimens administered to a group of Iraqi asthmatic children. Methods: This observational clinical study was conducted on a sample of pediatric Iraqi asthmatic patients in the central pediatric hospital in Baghdad. The study enrolled asthmatic children with moderate persistent asthma, who a specialized physician had diagnosed. Patients had been allocated to two groups to receive either medium doses of beclomethasone inhaler (80 μg twice a day) for group one, or low doses (40 μg twice a day) plus montelukast chewable tablets (5 mg once daily) for group two. This study used the Child Health Survey–child version (CHS–child version) to investigate the quality of life of included patients. Also, this study investigated the inflammatory blood markers; (eosinophils and prostaglandin D2). Results: This research included 63 patients ranging in age from 7 to 11 years, with a slight male preponderance: group one patients (n=30) and group two patients (n=33). Compared with baseline levels, the study groups had considerably higher quality of life scores at the first follow-up visit ( PP>0.05). Both groups significantly improved inflammatory biomarkers at the first follow-up visit ( P Conclusion: This research found that both groups generated substantial improvements in study parameters compared with baseline values and that the second group, which included a beclomethasone inhaler with montelukast, was associated with the highest degree of improvement in terms of quality-of-life ratings.

The experimental research investigation is carried out to study the flexure behavior of beams with the replacement of shape memory alloy rebars (SMAs). The practical program included testing three beams to investigate the effects of the replacement of longitudinal steel rebar by shape memory alloy rebar on the flexure behavior of beams. The study was focused on determining ultimate load, maximum deflection, load-deflection behavior, failure mode, ductility of beams, flexural toughness, and crack pattern at failure. Experimentally, the flexure behavior beams are significantly affected by changing the number of reinforcing bars with shape memory alloys (SMA) longitudinal direction. However, for using two rebars and one rebar of shape memory alloy respectability as a replacement for steel longitudinal reinforcement, the increases in maximum deflection were 19.18% and 14.45% from the reference beam, the ultimate load decreased by 20% and 2.66% from the reference beam, the ductility increased by 56.771% and 19.049% from the reference beam.
