BACKGROUND: Scalp reconstruction requires knowledge of scalp anatomy and reconstructive options. Advances in the field have led to numerous procedures being at the disposal of the reconstructive surgeon, expanding treatment options for patients. OBJECTIVE: To provide an algorithmic approach and general guidelines to consider when deciding on which scalp surgery will optimize cosmetic and functional outcomes. METHODS & MATERIALS: Previous literature was searched for the last 20 years to provide an updated guide. RESULTS: Taking into consideration the location, size and local scalp anatomy of a presenting defect will lead to optimal surgical outcomes. Other confounding factors such as bone exposure and extremely large defects will affect decision making. An algorithmic approach has been provided in this review. CONCLUSION: While many reconstructive surgical options are available, the best ones will depend on individual presentation of scalp defects. Location and size are first line considerations while local scalp anatomy will allow for tailoring of reconstructive options. This will help to maximize cosmetic and aesthetic considerations.

\nBackground: Retinal vasculitis has heterogeneous etiologies encompassing infections, autoimmune diseases, masquerades, and idiopathic causes. The heterogeneity in the underlying clinical features and etiologies of retinal vasculitis makes its diagnosis challenging for clinicians, the workup thus becomes quite extensive, and many a times several unwarranted investigations are done to look for a possible etiology. Summary: Timely and accurate diagnosis is crucial for effective management and vision preservation. The algorithmic approach needs understanding of the phenotype, clinical features, as well as imaging biomarkers so that only customized investigations are done in order to make the timely diagnosis and initiate specific therapy wherever required. Key Message: In this review article, we shall present an algorithmic approach that combines clinical assessment, ophthalmic imaging, laboratory investigations, and targeted ancillary tests. Retinal vasculitis presents a diagnostic challenge due to its diverse etiologies and clinical manifestations. Diagnosis relies on a combination of clinical assessment, ophthalmic imaging, and tailored laboratory investigations. Clinical examination involves identifying specific vascular involvement patterns and associated findings. Ophthalmic imaging techniques such as fundus fluorescein angiography, ultra-widefield fundus photography and angiography, optical coherence tomography, and optical coherence tomography angiography imaging aid in visualizing retinal vascular changes, macular edema, neovascularization, and structural alterations. These modalities provide valuable insights into disease activity, severity, and treatment response. Laboratory diagnostics, guided by clinical and imaging findings, aim to differentiate infectious from noninfectious causes and may include serological assays, inflammatory markers, proteomic analysis, and genetic testing to uncover underlying pathophysiological mechanisms. Abstract Background Dose painting planning would be more complicated due to different levels of prescribed doses and more complex evaluation with conventional plan quality indices considering uniform dose prescription. Therefore, we tried to introduce new indices for evaluating the dose distribution conformity and homogeneity of treatment volumes based on the tumoral cell density and relative volumes of each lesion in prostate IMRT. Methods CT and MRI scans of 20 male patients having local prostate cancer were used for IMRT DP planning. Apparent diffusion coefficient (ADC) images were imported to a MATLAB program to identify lesion regions based on ADC values automatically. Regions with ADC values lower than 750 mm2/s and regions with ADC values higher than 750 and less than 1500 mm2/s were considered CTV70Gy (clinical tumor volume with 70 Gy prescribed dose), and CTV60Gy, respectively. Other regions of the prostate were considered as CTV53Gy. New plan evaluation indices based on evaluating the homogeneity (IOE(H)), and conformity (IOE(C)) were introduced, considering the relative volume of each lesion and cellular density obtained from ADC images. These indices were compared with conventional homogeneity and conformity indices and IOEs without considering cellular density. Furthermore, tumor control probability (TCP) was calculated for each patient, and the relationship of the assessed indices were evaluated with TCP values. This paper introduces an algorithmic approach to the analysis of bifurcation of limit cycles from the centers of nonlinear continuous differential systems via the averaging method. We develop three algorithms to implement the averaging method. The first algorithm allows one to transform the considered differential systems to the normal form of averaging. Here, we restricted the unperturbed term of the normal form of averaging to be identically zero. The second algorithm is used to derive the computational formulae of the averaged functions at any order. The third algorithm is based on the first two algorithms and determines the exact expressions of the averaged functions for the considered differential systems. The proposed approach is implemented in Maple and its effectiveness is shown by several examples. Moreover, we report some incorrect results in published papers on the averaging method. Mass lesions in the retroperitoneal space may be primary or secondary. Primary retroperitoneal mass lesions are relatively uncommon as compared to pathology that arises secondarily from retroperitoneal organs. These may be solid or cystic lesions. The overlapping imaging features of various solid primary retroperitoneal tumors make the diagnosis difficult, and hence, histopathology remains the mainstay of diagnosis. This paper provides a brief review of the anatomy of the retroperitoneal space and provides an algorithmic approach based on cross-sectional imaging techniques to narrow down the differential diagnosis of solid primary retroperitoneal masses encountered in the adult population.

BackgroundFree tissue transfer to the knee region in patients with chronic post-traumatic knee osteomyelitis (CTKOM) poses a great challenge to surgeons because the remaining soft tissues adjacent to defects, including vascular structures, are usually damaged by chronic inflammation and multiple debridements. Thus, we developed an algorithm to help select the optimal recipient vessels and appropriate anterolateral thigh perforator (ALTP) flap type. In addition, we performed surgery using this algorithm and achieved successful reconstructions. This study aims to review our experiences in algorithmic reconstruction and assess its efficacy.MethodsAccording to the defect size and location, our algorithm suggested the use of various-shaped ALTP flaps with centrally located perforators (Cen-ALTP flap) or eccentrically located perforators (Ecc-ALTP flap). Besides, through the algorithm, one recipient vessel was selected among three candidates, including descending branch of the lateral circumflex femoral artery (DB-LCFA), anterior tibial artery (ATA), and posterior tibial artery (PTA). Based on this algorithmic decision, we performed individualized soft tissue reconstructions of the knee in 21 patients with CTKOM, between March 2013 and June 2021. The medical records of the patients were retrospectively reviewed.ResultsThe Cen-ALTP flap (n = 15) and ATA (n = 9) were the most commonly used for reconstruction. The Cen-ALTP flap anastomosed to the ATA was most commonly selected (n = 7) using the algorithm, followed by the Cen-ALTP flap anastomosed to the DB-LCFA (n = 5), and the Cen-ALTP flap anastomosed to the PTA (n = 3). All transferred ALTP flaps survived the follow-up period. Postoperative venous congestion in two patients and hematoma in one patient were resolved by immediate treatment. The postoperative course was uneventful.ConclusionDuring free ALTP flap transfer to CTKOM-related knee defects, we could select the optimal recipient vessel and appropriate flap type using our algorithm and obtain excellent reconstructive outcomes. Therefore, we believe that our algorithm could provide helpful guidance to reconstructive surgeons on free ALTP flap transfer to reconstruct CTKOM-related soft tissue defects.

The ileum has been candidate more frequently for endoscopic biopsy compared to the past. Most of those biopsies show either completely normal tissue or non-specific changes. Nevertheless, in some diseases, ileal biopsy would be diagnostic, and in some cases, it may be the only anatomical involved location by the disease. Endoscopically, normal mucosal biopsy is unlikely to provide useful diagnostic information and is not routinely recommended. However, in the presence of ileitis, ulcers, or erosions, biopsies can be very helpful. Ileitis might be induced by various conditions including infectious diseases, vasculitis, medication-induced, ischemia, eosinophilic enteritis, tumors etc. The conclusive cause of the condition is proposed by a comprehensive clinical background and physical examination, laboratory investigations, ileocolonoscopy, and imaging findings. Ileoscopy and biopsy are mainly useful in correctly selected cases such as patients who present with inflammatory diarrhea and endoscopic lesions. The purpose of this review article is to provide a simple algorithmic approach to the ileal biopsy samples through several boxes that give diagnostic clues and an idea behind the categories of ileal disorders. This review is written based on those that were previously reported in the literature as well as the authors' experiences. We have summarized different histological patterns in the ileal biopsy specimens that can be used in the diagnosis of inflammatory disorders of the ileum. This review provides an algorithmic approach to the clinicopathological features of inflammatory disorders of the ileum with a brief discussion of some important related issues. BackgroundThe free anterolateral thigh perforator (ALTP) flap has been successfully adopted to reconstruct traumatic soft tissue defects in the lower extremities. However, the occurrence of deep vein thrombosis (DVT) in donor or recipient veins has been overlooked, and there has been no reliable guideline to manage it. Therefore, in this study, we review our cases where the ALTP flaps were transferred to traumatic lower limbs even though DVT was found in the pedicle or recipient veins. Furthermore, based on our experiences, we suggest an algorithmic approach for dealing with DVT.Patients and methodsThis study included 108 patients who underwent lower extremity reconstruction using a free ALTP flap between January 2014 and January 2021. All medical records were reviewed, including preoperative assessment data, intraoperative findings, and postoperative complications. Notably, when DVT was found in both the donor and recipient veins, we thoroughly assessed operative findings, surgical solutions, and final outcomes.ResultsSixty-one of 108 (56.4%) patients underwent computed tomographic venography (CTV) preoperatively, revealing DVT in 11 of these 61 (18%) patients. Three of these 11 patients had iliofemoral DVT, and surgery was delayed more than two weeks after detection. The remaining eight patients had calf DVT and underwent free ALTP flap transfer as scheduled. Conversely, 47 of 108 (43.6%) patients did not undergo CTV, and an occult DVT was found in five of these 47 (10.6%) patients. In two of these five patients, free flap surgery was replaced with amputation and local flap coverage. In the remaining three patients and one patient with an occult DVT that was not found on CTV, the free ALTP flap transfer was carried out. In 15 patients with DVT, free ALTP flap transfer was performed using various alternative methods for venorrhaphy. Consequently, all flaps survived, with partial necrosis occurring in two patients.ConclusionIf DVT-affected veins are appropriately managed, the free ALTP flap can be successfully transferred to the traumatic lower limb even when DVT occurs in donor or recipient veins. The author's algorithm can help surgeons overcome the insufficiency of veins for pedicle anastomosis due to DVT and avoid postoperative thromboembolic complications.

Abstract: It may be difficult to distinguish among the various dementia syndromes due to the overlap in many common clinical features across the dementias. Accurate diagnosis of dementia type is increasingly important in an era when promising disease-modifying agents can be marketed soon. In this review, we outline a clinical algorithmic approach particularly tailored to the major forms of dementia in the clinic and refined from our accumulated experience of these patients. We first present an algorithmic approach for patients presenting with predominant deficits in episodic memory, executive function, language, visuospatial, and apraxia. We then consider types of dementia that mainly cause behavioral and psychiatric changes. Finally, we illustrate clinical pearls regarding motor deficits as key associations of each syndrome. This research is part of an exploration of the opportunities that developable surfaces offer in Architecture, with a specific focus on digital fabrication. Convalute surfaces have been subject to increasing attention over recent years. This attention is related to both a search for discretization systems for free surfaces and complex shapes with flat facets or developable strips, and with the use of these geometric shapes as an element for design composition. Making connections between geometry and construction is not new, and can be identified in the very foundations that led to the formulation of descriptive geometry as a tool applied to construction. Through an in-depth review of traditional pre-computational geometry based on classical descriptive geometry, this research reinterprets traditional graphic procedures. Through instrumentalisation using computational work processes, it is now possible to achieve in practice a level of development in this area that was until relatively recently contemplated only at a theoretical level. Specifically, this research develops and reformulates the concept of developable surfaces resting on two directrices in space, that is, developable strips. The concept of correlation coefficient of intuitionistic fuzzy sets is a reliable tool in information theory with numerous applications in diverse areas. Correlation coefficients of intuitionistic fuzzy sets have been studied through two-way approach by many researchers. This approach inappropriately discarded the hesitation margins of the concerned intuitionistic fuzzy sets, which makes the results of such experiments unreliable. In this paper, we modified the correlation coefficient of intuitionistic fuzzy sets of Thao et al. [36] in a three-way approach by including the hesitation margins in the computational process to enhance reliable output through an algorithmic method. We show that the modified correlation coefficient of intuitionistic fuzzy sets is more reasonable with precise outputs than correlation coefficient method. In terms of application, we demonstrate an analysis of medical diagnosis on some selected patients via an algorithm of the novel approach coded with JAVA programming language.

Phakic intraocular lenses (IOL) are a boon for patients who want spectacle independence but are unable to get refractive correction through laser platforms due to high refractive error or certain corneal contraindications. Phakic IOL's (PIOL) have their own set of complications and challenges, the most important being getting the sizing right. This paper attempts to solve the problem of accurate sizing of PIOL's. Parameters needed for calculating the ideal size of PIOL's have been studied in a step by step manner using all possible tools depending upon the availability and preference of the surgeon. The pros and cons of using a particular tool for measurements have been highlighted along with illustrative case examples to help surgeons who are starting PIOL implantation surgery. The aim of this paper is to present new high-performance implementations of Marsa-LFIB4 which is an example of high-quality multiple recursive pseudorandom number generators. We propose an algorithmic approach that combines language-based vectorization techniques together with a new divide-and-conquer parallel method that exploits a special sparse structure of the matrix obtained from the recursive formula that defines the generator. The naval multi-function console provides various types of information to the operator. It is equipment that is key for submarine navigation, and fatal human errors can occur due to the mismatch between the console specifications and the operator's body size. This study proposes a method for deriving console specifications suitable for the body size of Korean users. The seat height, seat width, seat depth, upper edge of backrest, and worktable height were selected as the target design variables. Using six anthropometric dimensions, a mismatch equation for each target design variable was developed. Anthropometric measures of 2027 Korean males were obtained, and the optimal specifications of the console were derived via an algorithmic approach. As a result, the match rate, considering all the target design variables, was improved from 2.57% to 76.96%. In previous studies and standards, the optimal console specifications were suggested based on the anthropometric data of a specific percentile of users, and it was impossible to quantitatively confirm the suitability of the console design for the target users. However, the method used in this study calculated the match rate using the mismatch equation devised for comfortable use of the console and a large amount of anthropometric data that represented the user population, and therefore the improvement effect of the recommended specification can be directly identified when compared to the current specifications. Moreover, the methodology and results of this study could be used for deciding the specifications of multi-function consoles in several fields, including nuclear power plants or disaster situation rooms. Objective Lumbar spinal stenosis (LSS) can lead to compression of the neural and vascular elements and is becoming more common due to degenerative changes that occur because of aging processes. Symptoms may manifest as pain and discomfort that radiates to the lower leg, thigh, and/or buttocks. The traditional treatment algorithm for LSS consists of conservative management (physical therapy, medication, education, exercise), often followed by epidural steroid injections (ESIs), and when nonsurgical treatment has failed, open decompression surgery with or without fusion is considered. In this review, the variables that should be considered during the management of patients with LSS are discussed, and the role of each treatment option to provide optimal care is evaluated. Results This review leads to the creation of an evidence-based practical algorithm to aid clinicians in the management of patients with LSS. Recent international guidelines recommend rapid initiation and titration of basic treatments of heart failure but do not explain how to achieve this goal. Despite these recommendations, implementation of treatment in daily practice is poor. This may be partly explained by the profile of the patients (frailty, comorbidities), safety considerations and tolerability issues related to kydney function, low blood pressure or heart rate and hyperkalaemia. This research proposes a platform-based platooning system to maximize the participation of truck platooning considering stability of the formed platoons that arises from truck preferences of platooning partners. Preference-based platooning is of both scientific interest and practical importance to the trucking sector, which is highly fragmented especially in the US. The preferences depend on the benefits of fuel saving and schedule adjustment to coordinate the time for platoon formation. The formed platoons are stable in the sense that no two trucks in two platoons want to break away from their current platoons and platoon with each other. Tackling this operation planning problem, the proposed system involves a platform interacting with individual trucks in a way that reduces truck communication and computation burdens, mitigates truck privacy concerns, and overcomes trucks misreporting private information. The central methodological investigation of the interactive process is on how to solve the Maximum Stable Truck Platooning Participation (MS-TPP) problem, for which a two-phase algorithmic approach is proposed. The underlying idea of this approach is to progressively reduce the lengths of truck preference lists, by eliminating truck pairs that do not affect the MS-TPP solution presence and separating trucks in odd rotations – which are key constructs in this approach – from the rest of the truck population. Theoretical properties and computational complexity of this two-phase algorithmic approach are investigated, along with a comparison with an integer programming approach and extensive numerical experiments in the context of a northern Illinois road network in the US. We find that the proposed two-phase algorithmic approach is very efficient compared to the integer programming approach in forming a maximum number of truck platoons while ensuring stability. In addition, the percentage of trucks that end up platooning by solving MS-TPP is much greater than using a greedy approach. The algorithmic approach is computationally scalable for solving large problem instances. The advantages of MS-TPP, in terms of the percentage of trucks in platooning and the average utility gain, become even more prominent as we deal with a larger system with more trucks. Pericardial cysts are rare with an incidence of about 1 in every 100,000 persons and one in 10 pericardial cysts may actually be a pericardial diverticulum. Pericardial cysts and diverticula share similar developmental origin and may appear as an incidental finding in chest roentgenogram in an asymptomatic patient. CT scan is considered as best modality for diagnosis and delineation of the surrounding anatomy. Cardiac MRI is recommended in the evaluation of the compressive effects caused by the pericardial cysts. The authors recommend echocardiography for serial follow up and image guided aspiration of the pericardial cyst in presence of compressive effects leading to cardiovascular and airway symptoms. A systematic approach is desirable for management of pericardial cysts depending on size, shape and compression effects, symptoms and easy access to serial Echocardiographic follow up. However, pericardial diverticulum may not be differentiated from cysts by the above testing, and only identified at surgery.

Pinch analysis is a methodology used for minimizing energy and material consumption in engineering processes. It features the identification of the pinch point and minimum external resources. Two common established approaches are used to identify these features: the graphical approach and the algebraic method, which are time-consuming and susceptible to human and calculation errors when used for a large number of process streams. This paper presents an algorithmic procedure to heat integration based on the algebraic approach. The algorithmic procedure is explained in a didactical manner to introduce pinch analysis for students and novice researchers in the field. Matlab code is presented, which is also intended for developing a Matlab toolbox for process integration.

Photoplethysmographic (PPG) signal is getting popularity for monitoring heart rate in wearable devices because of simplicity of construction and low cost of the sensor. The task becomes very difficult due to the presence of various motion artefacts. In this study, an algorithm based on cascade and parallel combination (CPC) of adaptive filters is proposed in order to reduce the effect of motion artefacts. First, preliminary noise reduction is performed by averaging two channel PPG signals. Next in order to reduce the effect of motion artefacts, a cascaded filter structure consisting of three cascaded adaptive filter blocks is developed where three-channel accelerometer signals are used as references to motion artefacts. To further reduce the affect of noise, a scheme based on convex combination of two such cascaded adaptive noise cancelers is introduced, where two widely used adaptive filters namely recursive least squares and least mean squares filters are employed. Heart rates are estimated from the noise reduced PPG signal in spectral domain. Finally, an efficient heart rate tracking algorithm is designed based on the nature of the heart rate variability. The performance of the proposed CPC method is tested on a widely used public database. It is found that the proposed method offers very low estimation error and a smooth heart rate tracking with simple algorithmic approach.

Objective: There are limited studies of cytology diagnosis of haematopoietic and lymphoid tumours in serosal effusion except for occasional case reports. We would like to demonstrate an algorithmic approach for accurate diagnosis, especially in patients without previous history. Methods: We reviewed 36 cases of lymphoma diagnosed in serosal effusion following an algorithmic approach. Suspected tumour cells were classified into small, intermediate and large sizes and two characteristic forms of plasmacytoid and Reed Sternberg‐like on smears (step 1), followed by utilising panels of immunohistochemical markers and Epstein‐Barr encoding region in situ hybridisation on cell blocks (step 2). A panel of CD3, CD20 and Ki‐67 formed the basic workup, followed by pertinent batteries of immunostaining. Molecular tests were applied in 22 selected cases by fluorescence in situ hybridisation (step 3). Results: There were 15 diffuse large B‐cell lymphomas; 12 plasma cell myelomas; two mantle cell lymphomas; one anaplastic large cell lymphoma ALK +; one small lymphocytic lymphoma; one plasmablastic lymphoma; one peripheral T‐cell lymphoma, not otherwise specified, one extranodal NK/T‐cell lymphoma, nasal type and two T‐cell lymphoblastic lymphomas. 14 cases with previous history had complete concordance in immunophenotype between cytology and histology. Another 14 cases were primarily diagnosed in patients with initial symptom of effusion based on immunophenotyping and cytogenetic test in selected cases. Eight cases were diagnosed based on morphology alone. Conclusion: An algorithmic approach based on morphology and immunohistochemistry is the key to making an accurate diagnosis of haematopoietic and lymphoid tumours in effusion. A molecular test is also important for confirmation and prognostic prediction. Uveitis is a challenging disease to treat. Corticosteroids have been used in the treatment of uveitis for many years. Immunosuppressives are gaining momentum in recent years in the treatment of uveitis. In this article we present an overview of current treatment of uveitis and the major breakthroughs and advances in drugs and ocular drug delivery systems in the treatment of uveitis.

Nevus of Ota (oculodermal melanosis) is a dermal melanocytic hamartoma with bluish hyperpigmentation along the first and second branches of the trigeminal nerve. Extracutaneous involvement, especially ocular, has been reported. A 45-year-old male presented with malignant melanoma of the left orbit in association with nevus of Ota. Being locally invasive, a modified exenteration with frontal flap repair was done on left eye. Adjuvant chemotherapy was given after wound healing. All pigmented lesions of the eye require close monitoring to help in the early diagnosis. Since malignant transformation has been reported in oculodermal melanosis, close follow-up and patient education will facilitate early diagnosis and prompt management. This case is reported for its rarity and unusual presentation.

Intraocular inflammations are still a diagnostic challenge for ophthalmologists. It is often difficult to make a precise etiological diagnosis in certain situations. Recently, there have been several advances in the investigations of uveitis, which has helped the ophthalmologists a lot in the management of such clinical conditions. A tailored approach to laboratory diagnosis of uveitic cases should be directed by the history, patient′s symptoms and signs, and clinical examination. This review summarizes various modalities of laboratory investigations and their role in the diagnosis of uveitis.

Surgery in the management of uveitis can be divided based on indication: either for therapeutic or can be for diagnostic purposes or to manage complications. The commonest indications include: Visual rehabilitation: surgery for removal of cataract, band keratopathy, corneal scars, pupillary membranes, removal of dense vitreous membranes, management of complications: anti-glaucoma surgery, vitreous hemorrhage, retinal detachment and chronic hypotony and diagnostic: aqueous tap, vitreous biopsy, tissue biopsy (iris, choroid). In this review, we shall describe the surgical technique for visual rehabilitation and for management of complications.

Ancillary investigations are the backbone of uveitis work-up both for anterior and posterior segment diseases. They help in making the diagnosis, ruling out certain differential diagnosis and monitoring inflammation during the follow-up. This review aims to be an overview describing the role of commonly used investigations for uveitis.
