Abstract Stroke, a major global health concern often rooted in cardiac dynamics, demands precise risk evaluation for targeted intervention. Current risk models, like the $$\text {CHA}_2\text {DS}_2\text {-VASc}$$ CHA 2 DS 2 -VASc score, often lack the granularity required for personalized predictions. In this study, we present a nuanced and thorough stroke risk assessment by integrating functional insights from cardiac magnetic resonance (CMR) with patient-specific computational fluid dynamics (CFD) simulations. Our cohort, evenly split between control and stroke groups, comprises eight patients. Utilizing CINE CMR, we compute kinematic features, revealing smaller left atrial volumes for stroke patients. The incorporation of patient-specific atrial displacement into our hemodynamic simulations unveils the influence of atrial compliance on the flow fields, emphasizing the importance of LA motion in CFD simulations and challenging the conventional rigid wall assumption in hemodynamics models. Standardizing hemodynamic features with functional metrics enhances the differentiation between stroke and control cases. While standalone assessments provide limited clarity, the synergistic fusion of CMR-derived functional data and patient-informed CFD simulations offers a personalized and mechanistic understanding, distinctly segregating stroke from control cases. Specifically, our investigation reveals a crucial clinical insight: normalizing hemodynamic features based on ejection fraction fails to differentiate between stroke and control patients. Differently, when normalized with stroke volume, a clear and clinically significant distinction emerges and this holds true for both the left atrium and its appendage, providing valuable implications for precise stroke risk assessment in clinical settings. This work introduces a novel framework for seamlessly integrating hemodynamic and functional metrics, laying the groundwork for improved predictive models, and highlighting the significance of motion-informed, personalized risk assessments.

Abstract Impaired cardiac function has been described as a frequent complication of COVID-19-related pneumonia. To investigate possible underlying mechanisms, we represented the cardiovascular system by means of a lumped-parameter 0D mathematical model. The model was calibrated using clinical data, recorded in 58 patients hospitalized for COVID-19-related pneumonia, to make it patient-specific and to compute model outputs of clinical interest related to the cardiocirculatory system. We assessed, for each patient with a successful calibration, the statistical reliability of model outputs estimating the uncertainty intervals. Then, we performed a statistical analysis to compare healthy ranges and mean values (over patients) of reliable model outputs to determine which were significantly altered in COVID-19-related pneumonia. Our results showed significant increases in right ventricular systolic pressure, diastolic and mean pulmonary arterial pressure, and capillary wedge pressure. Instead, physical quantities related to the systemic circulation were not significantly altered. Remarkably, statistical analyses made on raw clinical data, without the support of a mathematical model, were unable to detect the effects of COVID-19-related pneumonia in pulmonary circulation, thus suggesting that the use of a calibrated 0D mathematical model to describe the cardiocirculatory system is an effective tool to investigate the impairments of the cardiocirculatory system associated with COVID-19.

Abstract Cardiac digital twins provide a physics and physiology informed framework to deliver personalized medicine. However, high-fidelity multi-scale cardiac models remain a barrier to adoption due to their extensive computational costs. Artificial Intelligence-based methods can make the creation of fast and accurate whole-heart digital twins feasible. We use Latent Neural Ordinary Differential Equations (LNODEs) to learn the pressure-volume dynamics of a heart failure patient. Our surrogate model is trained from 400 simulations while accounting for 43 parameters describing cell-to-organ cardiac electromechanics and cardiovascular hemodynamics. LNODEs provide a compact representation of the 3D-0D model in a latent space by means of an Artificial Neural Network that retains only 3 hidden layers with 13 neurons per layer and allows for numerical simulations of cardiac function on a single processor. We employ LNODEs to perform global sensitivity analysis and parameter estimation with uncertainty quantification in 3 hours of computations, still on a single processor.

Abstract Predicting the evolution of systems with spatio-temporal dynamics in response to external stimuli is essential for scientific progress. Traditional equations-based approaches leverage first principles through the numerical approximation of differential equations, thus demanding extensive computational resources. In contrast, data-driven approaches leverage deep learning algorithms to describe system evolution in low-dimensional spaces. We introduce an architecture, termed Latent Dynamics Network, capable of uncovering low-dimensional intrinsic dynamics in potentially non-Markovian systems. Latent Dynamics Networks automatically discover a low-dimensional manifold while learning the system dynamics, eliminating the need for training an auto-encoder and avoiding operations in the high-dimensional space. They predict the evolution, even in time-extrapolation scenarios, of space-dependent fields without relying on predetermined grids, thus enabling weight-sharing across query-points. Lightweight and easy-to-train, Latent Dynamics Networks demonstrate superior accuracy (normalized error 5 times smaller) in highly-nonlinear problems with significantly fewer trainable parameters (more than 10 times fewer) compared to state-of-the-art methods.

Abstract Background Simulating the cardiac function requires the numerical solution of multi-physics and multi-scale mathematical models. This underscores the need for streamlined, accurate, and high-performance computational tools. Despite the dedicated endeavors of various research teams, comprehensive and user-friendly software programs for cardiac simulations, capable of accurately replicating both normal and pathological conditions, are still in the process of achieving full maturity within the scientific community. Results This work introduces $$\texttt {life}^{\text{x}}$$ life x -ep, a publicly available software for numerical simulations of the electrophysiology activity of the cardiac muscle, under both normal and pathological conditions. $$\texttt {life}^{\text{x}}$$ life x -ep employs the monodomain equation to model the heart’s electrical activity. It incorporates both phenomenological and second-generation ionic models. These models are discretized using the Finite Element method on tetrahedral or hexahedral meshes. Additionally, $$\texttt {life}^{\text{x}}$$ life x -ep integrates the generation of myocardial fibers based on Laplace–Dirichlet Rule-Based Methods, previously released in Africa et al., 2023, within $$\texttt {life}^{\text{x}}$$ life x -fiber. As an alternative, users can also choose to import myofibers from a file. This paper provides a concise overview of the mathematical models and numerical methods underlying $$\texttt {life}^{\text{x}}$$ life x -ep, along with comprehensive implementation details and instructions for users. $$\texttt {life}^{\text{x}}$$ life x -ep features exceptional parallel speedup, scaling efficiently when using up to thousands of cores, and its implementation has been verified against an established benchmark problem for computational electrophysiology. We showcase the key features of $$\texttt {life}^{\text{x}}$$ life x -ep through various idealized and realistic simulations conducted in both normal and pathological scenarios. Furthermore, the software offers a user-friendly and flexible interface, simplifying the setup of simulations using self-documenting parameter files. Conclusions $$\texttt {life}^{\text{x}}$$ life x -ep provides easy access to cardiac electrophysiology simulations for a wide user community. It offers a computational tool that integrates models and accurate methods for simulating cardiac electrophysiology within a high-performance framework, while maintaining a user-friendly interface. $$\texttt {life}^{\text{x}}$$ life x -ep represents a valuable tool for conducting in silico patient-specific simulations.

In the context of SARS-CoV-2 pandemic, mathematical modelling has played a fundamental role for making forecasts, simulating scenarios and evaluating the impact of preventive political, social and pharmaceutical measures. Optimal control theory represents a useful mathematical tool to plan the vaccination campaign aimed at eradicating the pandemic as fast as possible. The aim of this work is to explore the optimal prioritisation order for planning vaccination campaigns able to achieve specific goals, as the reduction of the amount of infected, deceased and hospitalized in a given time frame, among age classes. For this purpose, we introduce an age stratified SIR-like epidemic compartmental model settled in an abstract framework for modelling two-doses vaccination campaigns and conceived with the description of COVID19 disease. Compared to other recent works, our model incorporates all stages of the COVID-19 disease, including death or recovery, without accounting for additional specific compartments that would increase computational complexity and that are not relevant for our purposes. Moreover, we introduce an optimal control framework where the model is the state problem while the vaccine doses administered are the control variables. An extensive campaign of numerical tests, featured in the Italian scenario and calibrated on available data from Dipartimento di Protezione Civile Italiana, proves that the presented framework can be a valuable tool to support the planning of vaccination campaigns. Indeed, in each considered scenario, our optimization framework guarantees noticeable improvements in terms of reducing deceased, infected or hospitalized individuals with respect to the baseline vaccination policy.

Abstract The aim of this paper is to introduce a new mathematical model that simulates myocardial blood perfusion that accounts for multiscale and multiphysics features. Our model incorporates cardiac electrophysiology, active and passive mechanics, hemodynamics, valve modeling, and a multicompartment Darcy model of perfusion. We consider a fully coupled electromechanical model of the left heart that provides input for a fully coupled Navier–Stokes–Darcy model for myocardial perfusion. The fluid dynamics problem is modeled in a left heart geometry that includes large epicardial coronaries, while the multicompartment Darcy model is set in a biventricular myocardium. Using a realistic and detailed cardiac geometry, our simulations demonstrate the biophysical fidelity of our model in describing cardiac perfusion. Specifically, we successfully validate the model reliability by comparing in-silico coronary flow rates and average myocardial blood flow with clinically established values ranges reported in relevant literature. Additionally, we investigate the impact of a regurgitant aortic valve on myocardial perfusion, and our results indicate a reduction in myocardial perfusion due to blood flow taken away by the left ventricle during diastole. To the best of our knowledge, our work represents the first instance where electromechanics, hemodynamics, and perfusion are integrated into a single computational framework.

The numerical simulation of several virtual scenarios arising in cardiac mechanics poses a computational challenge that can be alleviated if traditional full-order models (FOMs) are replaced by reduced order models (ROMs). For example, in the case of problems involving a vector of input parameters related, e.g., to material coefficients, projection-based ROMs provide mathematically rigorous physics-driven surrogate ROMs. In this work we demonstrate how, once trained, ROMs yield extremely accurate predictions (according to a prescribed tolerance) – yet cheaper than the ones provided by FOMs – of the structural deformation of the left ventricular tissue over an entire heartbeat, and of related output quantities of interest, such as the pressure-volume loop, for any desired input parameter values within a prescribed parameter range. However, the construction of ROM approximations for time-dependent cardiac mechanics is not straightforward, because of the highly nonlinear and multiscale nature of the problem, and almost never addressed. Our approach relies on the reduced basis method for parameterized partial differential equations. This technique performs a Galerkin projection onto a low-dimensional space for the displacement variable; the reduced space is built from a set of solution snapshots – obtained for different input parameter values and time instances – of the high-fidelity FOM, through the proper orthogonal decomposition technique. Then, suitable hyper-reduction techniques, such as the Discrete Empirical Interpolation Method, are exploited to efficiently handle nonlinear and parameter-dependent terms. In this work we show how a fast and reliable approximation of the time-dependent cardiac mechanical model can be achieved by a projection-based ROM, taking into account both passive and active mechanics for the left ventricle providing all the building blocks of the methodology, and highlighting those challenging aspects that are still open.

Abstract Background Modeling the whole cardiac function involves the solution of several complex multi-physics and multi-scale models that are highly computationally demanding, which call for simpler yet accurate, high-performance computational tools. Despite the efforts made by several research groups, no software for whole-heart fully-coupled cardiac simulations in the scientific community has reached full maturity yet. Results In this work we present $$\texttt {life}^{\texttt {x}}$$ life x -fiber, an innovative tool for the generation of myocardial fibers based on Laplace-Dirichlet Rule-Based Methods, which are the essential building blocks for modeling the electrophysiological, mechanical and electromechanical cardiac function, from single-chamber to whole-heart simulations. $$\texttt {life}^{\texttt {x}}$$ life x -fiber is the first publicly released module for cardiac simulations based on $$\texttt {life}^{\texttt {x}}$$ life x , an open-source, high-performance Finite Element solver for multi-physics, multi-scale and multi-domain problems developed in the framework of the iHEART project, which aims at making in silico experiments easily reproducible and accessible to a wide community of users, including those with a background in medicine or bio-engineering. Conclusions The tool presented in this document is intended to provide the scientific community with a computational tool that incorporates general state of the art models and solvers for simulating the cardiac function within a high-performance framework that exposes a user- and developer-friendly interface. This report comes with an extensive technical and mathematical documentation to welcome new users to the core structure of $$\texttt {life}^{\texttt {x}}$$ life x -fiber and to provide them with a possible approach to include the generated cardiac fibers into more sophisticated computational pipelines. In the near future, more modules will be successively published either as pre-compiled binaries for x86-64 Linux systems or as open source software.

We face a rigidity problem for the fractional $ p $-Laplace operator to extend to this new framework some tools useful for the linear case. It is known that $ (-\Delta)^s(1-|x|^{2})^s_+ $ and $ -\Delta_p(1-|x|^{\frac{p}{p-1}}) $ are constant functions in $ (-1, 1) $ for fixed $ p $ and $ s $. We evaluated $ (-\Delta_p)^s(1-|x|^{\frac{p}{p-1}})^s_+ $ proving that it is not constant in $ (-1, 1) $ for some $ p\in (1, +\infty) $ and $ s\in (0, 1) $. This conclusion is obtained numerically thanks to the use of very accurate Gaussian numerical quadrature formulas.

Several epidemiological models have been proposed to study the evolution of COVID-19 pandemic. In this paper, we propose an extension of the SUIHTER model, to analyse the COVID-19 spreading in Italy, which accounts for the vaccination campaign and the presence of new variants when they become dominant. In particular, the specific features of the variants (e.g. their increased transmission rate) and vaccines (e.g. their efficacy to prevent transmission, hospitalization and death) are modeled, based on clinical evidence. The new model is validated comparing its near-future forecast capabilities with other epidemiological models and exploring different scenario analyses.

Emerging studies address how COVID-19 infection can impact the human cardiovascular system. This relates particularly to the development of myocardial injury, acute coronary syndrome, myocarditis, arrhythmia, and heart failure. Prospective treatment approach is advised for these patients. To study the interplay between local changes (reduced contractility), global variables (peripheral resistances, heart rate) and the cardiac function, we considered a lumped parameters computational model of the cardiovascular system and a three-dimensional multiphysics model of cardiac electromechanics. Our mathematical model allows to simulate the systemic and pulmonary circulations, the four cardiac valves and the four heart chambers, through equations describing the underlying physical processes. By the assessment of conventionally relevant parameters of cardiac function obtained through our numerical simulations, we propose a computational model to effectively reveal the interactions between the cardiac and pulmonary functions in virtual subjects with normal and impaired cardiac function at baseline affected by mild or severe COVID-19.

In this paper we describe a mathematical and numerical approach that combines physics-based simulated ground motion caused by earthquakes with fragility functions to model the structural damages induced to buildings. To simulate earthquake ground motion we use the discontinuous Galerkin spectral element method to solve a three-dimensional differential model at regional scale describing the propagation of seismic waves through the earth layers up to the surface. Selected intensity measures, retrieved from the synthetic time histories, are then employed as input for a vulnerability model based on fragility functions, in order to predict building damage scenarios at urban scale. The main features and effectiveness of the proposed numerical approach are tested on the Beijing metropolitan area.

This work concerns the mathematical and numerical modeling of the heart. The aim is to enhance the understanding of the cardiac function in both physiological and pathological conditions. Along this road, a challenge arises from the multi-scale and multi-physics nature of the mathematical problem at hand. In this paper, we propose an electromechanical model that, in bi-ventricle geometries, combines the monodomain equation, the Bueno-Orovio minimal ionic model, and the Holzapfel-Ogden strain energy function for the passive myocardial tissue modelling together with the active strain approach combined with a model for the transmurally heterogeneous thickening of the myocardium. Since the distribution of the electric signal is dependent on the fibres orientation of the ventricles, we use a Laplace-Dirichlet Rule-Based algorithm to determine the myocardial fibres and sheets configuration in the whole bi-ventricle. In this paper, we study the influence of different fibre directions and incompressibility constraint and penalization on the compressibility of the material (bulk modulus) on the pressure-volume relation simulating a full heart beat. The coupled electromechanical problem is addressed by means of a fully segregated scheme. The numerical discretization is based on the Finite Element Method for the spatial discretization and on Backward Differentiation Formulas for the time discretization. The arising non-linear algebraic system coming from application of the implicit scheme is solved through the Newton method. Numerical simulations are carried out in a patient-specific biventricle geometry to highlight the most relevant results of both electrophysiology and mechanics and to compare them with physiological data and measurements. We show how various fibre configurations and bulk modulus modify relevant clinical quantities such as stroke volume, ejection fraction and ventricle contractility.

Hypertrophic Cardiomyopathy (HCM) is a pathological condition characterized by an abnormal thickening of the myocardium. When affecting the medio-basal portion of the septum, it is named Hypertrophic Obstructive Cardiomyopathy (HOCM) because it induces a flow obstruction in the left ventricular outflow tract. In any type of HCM, the myocardial function can become compromised, possibly resulting in cardiac death. In this study, we investigated with computational analysis the hemodynamics of patients with different types of HCM. The aim was quantifying the effects of this pathology on the intraventricular blood flow and pressure gradients, and providing information potentially useful to guide the indication and the modality of the surgical treatment (septal myectomy). We employed an image-based computational approach, integrating fluid dynamics simulations with geometric and functional data, reconstructed from standard cardiac cine-MRI acquisitions. We showed that with our approach we can better understand the patho-physiological behavior of intraventricular blood flow dynamics due to the abnormal morphological and functional aspect of the left ventricle. The main results of our investigation are: (a) a detailed patient-specific analysis of the blood velocity, pressure and stress distribution associated to HCM; (b) a computation-based classification of patients affected by HCM that can complement the current clinical guidelines for the diagnosis and treatment of HOCM.

The numerical simulation of multiple scenarios easily becomes computationally prohibitive for cardiac electrophysiology (EP) problems if relying on usual high-fidelity, full order models (FOMs). Likewise, the use of traditional reduced order models (ROMs) for parametrized PDEs to speed up the solution of the aforementioned problems can be problematic. This is primarily due to the strong variability characterizing the solution set and to the nonlinear nature of the input-output maps that we intend to reconstruct numerically. To enhance ROM efficiency, we proposed a new generation of non-intrusive, nonlinear ROMs, based on deep learning (DL) algorithms, such as convolutional, feedforward, and autoencoder neural networks. In the proposed DL-ROM, both the nonlinear solution manifold and the nonlinear reduced dynamics used to model the system evolution on that manifold can be learnt in a non-intrusive way thanks to DL algorithms trained on a set of FOM snapshots. DL-ROMs were shown to be able to accurately capture complex front propagation processes, both in physiological and pathological cardiac EP, very rapidly once neural networks were trained, however, at the expense of huge training costs. In this study, we show that performing a prior dimensionality reduction on FOM snapshots through randomized proper orthogonal decomposition (POD) enables to speed up training times and to decrease networks complexity. Accuracy and efficiency of this strategy, which we refer to as POD-DL-ROM, are assessed in the context of cardiac EP on an idealized left atrium (LA) geometry and considering snapshots arising from a NURBS (non-uniform rational B-splines)-based isogeometric analysis (IGA) discretization. Once the ROMs have been trained, POD-DL-ROMs can efficiently solve both physiological and pathological cardiac EP problems, for any new scenario, in real-time, even in extremely challenging contexts such as those featuring circuit re-entries, that are among the factors triggering cardiac arrhythmias.

In this paper we propose a saddle point approach to solve boundary control problems for the steady Navier-Stokes equations with mixed Dirichlet-Neumann boundary conditions, both in two and three dimensions. We provide a comprehensive theoretical framework to address (i) the well posedness analysis for the optimal control problem related to this system and (ii) the derivation of a system of first-order optimality conditions. We take advantage of a suitable treatment of boundary Dirichlet controls (and data) realized by means of Lagrange multipliers. In spite of the fact that this approach is rather common, a detailed analysis is still missing for mixed boundary conditions. We consider the minimization of quadratic cost (e.g., tracking-type or vorticity) functionals of the velocity. A descent method is then applied for numerical optimization, exploiting the Galerkin finite element method for the discretization of the state equations, the adjoint (Oseen) equations and the optimality equation. Numerical results are shown for simplified two-dimensional fluid flows in a tract of blood vessel where a bypass is inserted; to avoid to simulate the whole bypass configuration, we represent its action by a boundary velocity control.

We propose four novel mathematical models, describing the microscopic mechanisms of force generation in the cardiac muscle tissue, which are suitable for multiscale numerical simulations of cardiac electromechanics. Such models are based on a biophysically accurate representation of the regulatory and contractile proteins in the sarcomeres. Our models, unlike most of the sarcomere dynamics models that are available in the literature and that feature a comparable richness of detail, do not require the time-consuming Monte Carlo method for their numerical approximation. Conversely, the models that we propose only require the solution of a system of PDEs and/or ODEs (the most reduced of the four only involving 20 ODEs), thus entailing a significant computational efficiency. By focusing on the two models that feature the best trade-off between detail of description and identifiability of parameters, we propose a pipeline to calibrate such parameters starting from experimental measurements available in literature. Thanks to this pipeline, we calibrate these models for room-temperature rat and for body-temperature human cells. We show, by means of numerical simulations, that the proposed models correctly predict the main features of force generation, including the steady-state force-calcium and force-length relationships, the length-dependent prolongation of twitches and increase of peak force, the force-velocity relationship. Moreover, they correctly reproduce the Frank-Starling effect, when employed in multiscale 3D numerical simulation of cardiac electromechanics.

Predicting the electrical behavior of the heart, from the cellular scale to the tissue level, relies on the numerical approximation of coupled nonlinear dynamical systems. These systems describe the cardiac action potential, that is the polarization/depolarization cycle occurring at every heart beat that models the time evolution of the electrical potential across the cell membrane, as well as a set of ionic variables. Multiple solutions of these systems, corresponding to different model inputs, are required to evaluate outputs of clinical interest, such as activation maps and action potential duration. More importantly, these models feature coherent structures that propagate over time, such as wavefronts. These systems can hardly be reduced to lower dimensional problems by conventional reduced order models (ROMs) such as, e.g., the reduced basis method. This is primarily due to the low regularity of the solution manifold (with respect to the problem parameters), as well as to the nonlinear nature of the input-output maps that we intend to reconstruct numerically. To overcome this difficulty, in this paper we propose a new, nonlinear approach relying on deep learning (DL) algorithms-such as deep feedforward neural networks and convolutional autoencoders-to obtain accurate and efficient ROMs, whose dimensionality matches the number of system parameters. We show that the proposed DL-ROM framework can efficiently provide solutions to parametrized electrophysiology problems, thus enabling multi-scenario analysis in pathological cases. We investigate four challenging test cases in cardiac electrophysiology, thus demonstrating that DL-ROM outperforms classical projection-based ROMs.

Atrial fibrillation (AF) carries out a 5-fold increase in stroke risk, related to embolization of thrombi clotting in left atrium (LA). Left atrial appendage (LAA) is the site with the highest blood stasis which causes thrombus formation. About 90 % of the intracardiac thrombi in patients with cardioembolic events originally develop in the LAA. Recent studies have been focused on the association between LAA anatomical features and stroke risk and provided conflicting results. Haemodynamic and fluid dynamic information on the LA and mostly on the LAA may improve stroke risk stratification. Therefore, the aim of this study was the design and development of a workflow to quantitatively define the influence of the LAA morphology on LA hemodynamics. Five 3D LA anatomical models, obtained from real clinical data, which were clearly different as regard to LAA morphology were used. For each LAA we identified and computed several parameters describing its geometry. Then, one LA chamber model was chosen and a framework was developed to connect the different LAAs belonging to the other four patients to this model. These new anatomical models represented the computational domain for the computational fluid dynamics (CFD) study; simulations of the hemodynamics within the LA and LAA were performed in order to evaluate the interplay of the LAA shape on the blood flow characteristics in AF condition. CFD simulations were carried out for five cardiac cycles. Blood velocity, vorticity, LAA orifice velocity, residence time computed in the five models were compared and correlated with LAA morphologies. Results showed that not only complex morphologies were characterized by low velocities, low vorticity and consequently could carry a higher thrombogenic risk; even qualitatively simple morphologies showed a thrombogenic risk equal, or even higher, than more complex auricles. CFD results supported the hypothesis that LAA geometric characteristics plays a key-role in defining thromboembolic risk. LAA geometric parameters could be considered, coupled with the morphological characteristics, for a comprehensive evaluation of the regional blood stasis. The proposed procedure might address the development of a tool for patient-specific stroke risk assessment and preventive treatment in AF patients, relying on morpho-functional defintion of each LAA type.

Se revisan algunos modelos matemáticos usados para describir fenómenos complejos de la vida real. Las aplicaciones comprenden aeronaútica, hidrodinámica, medicina, problemas medioambientales, problemas que surgen en competiciones deportivas, entre otros. Para problemas medioambientales, los modelos matemáticos se usan para predecir, analizar, y, posiblemente, controlar eventos con un relevante impacto social. En deportes, los modelos matemáticos son usados a menudo para mejorar los resultados de los atletas y los diseños de vehículos en varias disciplinas.
