This second joint document, written by experts from the Brazilian Association of Allergy and Immunology (ASBAI) and Brazilian Society of Anesthesiology (SBA) concerned with perioperative anaphylaxis, aims to review the pathophysiological reaction mechanisms, triggering agents (in adults and children), and the approach for diagnosis during and after an episode of anaphylaxis. As anaphylaxis assessment is extensive, the identification of medications, antiseptics and other substances used at each setting, the comprehensive data documentation, and the use of standardized nomenclature are key points for obtaining more consistent epidemiological information on perioperative anaphylaxis. Resumo: Este segundo documento, escrito por especialistas da Associação Brasileira de Alergia e Imunologia (ASBAI) e da Sociedade Brasileira de Anestesiologia (SBA) interessados no tema anafilaxia perioperatória, tem por objetivo revisar os mecanismos fisiopatológicos, agentes desencadeantes (em adultos e crianças), assim como a abordagem diagnóstica durante e após o episódio. Por se tratar de uma avaliação abrangente, a identificação das medicações, antissépticos e outras substâncias usadas em cada região, registros detalhados e nomenclatura padronizada são pontos fundamentais para a obtenção de dados epidemiológicos mais fidedignos sobre a anafilaxia perioperatória.

4. Specific immunotherapy using Hymenoptera venom: systematic review

CONTEXT AND OBJECTIVE: The only effective treatment for patients who have severe reactions after Hymenoptera stings is venom immunotherapy. The aim of this study was to review the literature to assess the effects of venom immunotherapy among patients presenting severe reactions after Hymenoptera stings. DESIGN AND SETTING: Randomized controlled trials in the worldwide literature were reviewed. The manuscript was produced in the Discipline of Allergy and Clinical Immunology, Universidade de São Paulo (USP). METHODS: Randomized controlled trials involving venom immunotherapy versus placebo or only patient follow-up were evaluated. The risk of systemic reactions after specific immunotherapy was evaluated by calculating odds ratios (OR) and their 95% confidence intervals. RESULTS: 2,273 abstracts were identified by the keywords search. Only four studies were included in this review. The chi-square test for heterogeneity showed that two studies were homogeneous and could be included in a meta-analysis. By combining the two studies, the odds ratio became significant: 0.29 (0.10-0.87). However, analysis on the severity of the reactions after immunotherapy showed that the benefits may not be so significant because the reactions were mostly similar to or milder than the original reaction. CONCLUSIONS: Specific immunotherapy should be recommended for adults and children with moderate to severe reactions, but there is no need to prescribe it for children with skin reactions alone, especially if the exposure is very sporadic. On the other hand, the risk-benefit relation should always be assessed in each case.

5. Preoperative laboratory evaluation of patients aged over 40 years undergoing elective non-cardiac surgery

CONTEXT AND OBJECTIVE: Although it is generally agreed that a medical history and physical examination should be obtained as part of preoperative evaluation, there is still substantial controversy about the additional benefits of preoperative screening tests. The objective of the present study was to determine the percentage of abnormalities on laboratory tests among a population that underwent non-cardiac surgery and to correlate these tests with changes in preoperative evaluation management. DESIGN AND SETTING: Cross-sectional study, carried out in a University Hospital. METHODS: 991 patients aged over 40 years undergoing elective non-cardiac surgery from July 1997 to January 2000 were studied. Blood cell count, serum sodium, potassium, urea and creatinine, prothrombin, thrombin and partial thromboplastin time, electrocardiogram and chest X-ray were evaluated. RESULTS: Out of the 957 electrocardiograms performed, some type of abnormality was found in 504 cases (50.9%) and, among the 646 chest X-rays requested, 271 (42.0%) displayed alterations. Laboratory tests showed abnormal values ranging from 5.1% (prothrombin time) to 41.0% (hematocrit). Increased percentages of abnormal tests with increasing patient age were also observed. CONCLUSIONS: Although there were substantial numbers of screening test abnormalities in preoperative evaluations, these results seldom interfered in patient management.

Por se tratar de uma avaliação abrangente, a identificação das medicações, antissépticos e outras substâncias usadas em cada região, registros detalhados e nomenclatura padronizada são pontos fundamentais para a obtenção de dados epidemiológicos mais fidedignos sobre a anafilaxia perioperatória.

As anaphylaxis assessment is extensive, the identification of medications, antiseptics and other substances used at each setting, the comprehensive data documentation, and the use of standardized nomenclature are key points for obtaining more consistent epidemiological information on perioperative anaphylaxis.

The purpose of the present series of two articles was to report the most recent evidence based on the collaborative assessment between both societies. This first article will consider the updated definitions, treatment and guidelines after a perioperative crisis. The following article will discuss the major etiologic agents, how to proceed with the investigation, and the appropriate tests. 12. Clinical and Laboratory Profile of Patients With Anaphylaxis to Fire Ant Venom (Solenopsis Sp) Under Specific Subcutaneous Immunotherapy

Anaphylaxis to fire ant venoms (Solenopsis sp) is a significant cause of systemic allergic reaction caused by Hymenoptera stings in children. There are only a few reports about the safety and efficacy of specific immunotherapy. We aim evaluate clinical characteristics, IgE and IgG4 specific responses of patients undergoing immunotherapy with a whole-body extract of Solenopsis sp after one year of the maintenance phase. Thirty-three patients were enrolled due to anaphylaxis by fire ant venom (Solenopsis sp) and underwent specific venom immunotherapy. They were assessed at baseline and one year after the beginning of the maintenance phase for skin test; specific IgE and IgG4 antibodies to fire ant venom; tryptase. All patients included presented a severe anaphylactic reaction. Although two patients (6.25%) presented a tryptase level higher than 11.4 ug/ml, systemic mastocytosis was ruled out. There was no relationship between the severity of the reaction with gender, tryptase level, atopy, previous reactions, concentration of the allergen in the skin test or specific IgE level. There was an increase of the specific IgG4/IgE ratio between the two time points. Reactions were local, with only two mild systemic reactions during the build-up phase. Twenty patients had accidental stings during immunotherapy, with 3 presenting only urticaria. This study is unprecedented in evaluating clinical and laboratory data in the fire ant immunotherapy. Our results show that after one year of the maintenance phase, patients did not develop any severe reaction with only a few mild reactions and presented a significant production of specific IgG4.

- Abstract
Resumo: veneno de formiga (Solenopsis sp) é uma causa importante de anafilaxia após ferroada desses insetos. Ocorre principalmente em crianças e são poucos dados na literatura sobre tratamento específico. Objetivo: avaliamos as características clínicas e laboratoriais dos pacientes submetidos a imunoterapia com veneno de formigas (Solenopsis sp) após 1 ano da fase de manutenção. Métodos: pacientes que foram atendidos com anafilaxia por veneno de Solenopsis SP no ambulatório de Alergia e Imunologia Clínica do Hospital das Clínicas da FMUSP e foram submetidos à imunoterapia veneno específica. Esses pacientes foram avaliados no período basal e após 1 ano da fase de manutenção em relação aos seguintes parâmetros: avaliação das características clínicas; idade; atopia; gênero; reações prévias; teste cutâneo; determinação sérica de IgE e IgG4 específicas; triptase; reações adversas a Imunoterapia; exposição natural às ferroadas durante o tratamento. Resultados: 33 pacientes que apresentaram reação anafilática grave (grau III e IV de Muller) foram incluídos. A maioria morava em zona urbana e 75,9% dos pacientes tinham menos de 20 anos de idade. Não houve relação da gravidade da reação com gênero, dosagem de triptase, atopia, qualquer reação prévia, concentração do extrato alergênico no teste cutâneo ou nível de IgE específica. Houve diferença estatística significante na razão IgG4/IgE específicas no período basal e durante a imunoterapia. A maioria das reações foram locais e apenas duas reações sistêmicas ocorreram pela imunoterapia, porém foram leves e durante a fase de indução. Vinte pacientes foram ferroados acidentalmente durante o tratamento e apenas 3 apresentaram reações sistêmicas leves (urticária): 1 na fase de indução e 2 no início da fase de manutenção, não houve necessidade de uso de adrenalina intramuscular nessas reações, com melhora após antihistamínicos por via oral. Conclusões: poucos estudos no mundo avaliaram pacientes submetidos à imunoterapia com veneno de Solenopsis sp e esse estudo é inédito na avaliação de dados clínicos e laboratoriais no tratamento com veneno dessas formigas. As lições aprendidas nesse estudo nos ajudaram a entender a tanto a eficácia quanto a manutenção da tolerância ao veneno de formiga induzida pela imunoterapia específica. Seria interessante novos estudos sobre comportamento das células T específicas durante a imunoterapia com veneno de formiga. - Abstract
A hipersensibilidade a veneno de Hymenoptera representa importante problema do ponto de vista de saúde da população, uma vez que pacientes alérgicos aos componentes do veneno podem desenvolver reações graves, às vezes fatais. A única profilaxia efetiva em pacientes sensibilizados é a imunoterapia veneno específica. Objetivos: avaliar as evidências científicas a respeito dos efeitos da imunoterapia específica utilizada na profilaxia secundária das reações graves em pacientes sensibilizados a veneno de Hymenoptera, por meio da realização de uma revisão sistemática. Métodos: a estratégia de busca seguiu as recomendações do Grupo de Pele da Colaboração Cochrane. A pesquisa foi realizada nas seguintes bases de dados: MEDLINE, the Cochrane Central Register of Controlled Trials (CENTRAL, The Cochrane Library), EMBASE, LILACS, SciSEARCH e nas referências de artigos mais relevantes. Todos os ensaios clínicos controlados e randomizados envolvendo imunoterapia com veneno de Hymenoptera versus imunoterapia com placebo ou apenas seguimento dos pacientes foram avaliados. Dois revisores de forma independente (ASW e LAMF) avaliaram a elegibilidade e a qualidade metodológica de cada ensaio clínico e extraíram os dados. O risco de reações sistêmicas, conseqüentes a ferroada acidental ou a teste de provocação com o inseto responsável, após a imunoterapia específica, foi avaliado por meio do cálculo do odds ratio e do respectivo intervalo de confiança de 95%. Resultados: 2267 resumos foram identificados. A maioria dos artigos foi excluída pelas seguintes razões: os estudos não eram controlados e randomizados ou não satisfaziam alguns critérios de inclusão. Apenas quatro estudos foram analisados. A idade dos pacientes variou entre dois e 65 anos. Apenas um estudo comparou imunoterapia com veneno de formiga contra placebo (Brown et al.) e três estudos (Hunt et al., Schubert et al. e Valentine et al.) comparam imunoterapia com venenos de abelha e vespa contra placebo ou seguimento de pacientes. CONTEXT AND OBJECTIVE: The only effective treatment for patients who have severe reactions after Hymenoptera stings is venom immunotherapy. The aim of this study was to review the literature to assess the effects of venom immunotherapy among patients presenting severe reactions after Hymenoptera stings. DESIGN AND SETTING: Randomized controlled trials in the worldwide literature were reviewed. The manuscript was produced in the Discipline of Allergy and Clinical Immunology, Universidade de São Paulo (USP). METHODS: Randomized controlled trials involving venom immunotherapy versus placebo or only patient follow-up were evaluated. The risk of systemic reactions after specific immunotherapy was evaluated by calculating odds ratios (OR) and their 95% confidence intervals. RESULTS: 2,273 abstracts were identified by the keywords search. Only four studies were included in this review. The chi-square test for heterogeneity showed that two studies were homogeneous and could be included in a meta-analysis. By combining the two studies, the odds ratio became significant: 0.29 (0.10-0.87). However, analysis on the severity of the reactions after immunotherapy showed that the benefits may not be so significant because the reactions were mostly similar to or milder than the original reaction. CONCLUSIONS: Specific immunotherapy should be recommended for adults and children with moderate to severe reactions, but there is no need to prescribe it for children with skin reactions alone, especially if the exposure is very sporadic. On the other hand, the risk-benefit relation should always be assessed in each case. CONTEXTO E OBJETIVO: O único tratamento eficaz para pacientes que têm reações graves após ferroada de Hymenoptera é a imunoterapia com veneno. O objetivo deste estudo foi rever a literatura para avaliar os efeitos da imunoterapia com veneno em pacientes com reações graves após ferroada de Hymenoptera. TIPO DE ESTUDO E LOCAL: Foram revisados na literatura mundial ensaios clínicos controlados e aleatórios. O manuscrito foi realizado na Disciplina de Alergia e Imunologia Clínica, Universidade de São Paulo (USP). MÉTODOS: Ensaios clínicos controlados e aleatórios envolvendo imunoterapia com veneno de Hymenoptera versus placebo ou apenas acompanhamento dos pacientes foram avaliados. O teste qui-quadrado de heterogeneidade mostrou que dois estudos foram homogêneos e puderam ser incluídos na metanálise. Ao combinar os dois estudos, o "odds ratio" passou a ser significativo: 0.29 (0.10-0.87). Entretanto, ao analisar a gravidade das reações ocorridas após a imunoterapia, observou-se que os benefícios podem não ser tão relevantes, pois as reações foram, na grande maioria, ou mais leves ou semelhantes à reação original. CONCLUSÕES: A imunoterapia específica deve ser recomendada para adultos e crianças com reações moderadas a graves, porém não há necessidade de prescrevê-la para as crianças apenas com reações cutâneas, especialmente se a exposição é muito esporádica. No outro lado, a relação risco-benefício deve ser sempre avaliada em cada caso.

Background Double sensitization to both honeybee (Apis mellifera) and Yellow Jacket (Vespula ssp.) venom is common in up to 59% of Northern European Hymenoptera venom allergic patients and this rate is more than 50% in the United States. In Brazil yellow jacket is not a common wasp, but Polistes sp. and Polybia paulista poses the major risk for Brazilian patients. Reports about double sensitization involving honey bee and fire ant (Solenopsis invicta) are rare and there is nothing described about multi-sensitization to insects. Cross-reacting carbohydrate determinants (CCDs) are not present in Polistessp. venom and are not yet described for Polybia paulista neither Solenopsis invicta. Component-resolved analysis with recombinant species-specific major allergens may help to distinguish true double sensitization from crossreactivity, except for Polybia paulistaallergens for which these commercial tests are not yet available. Although there is no international consensus on whether immunotherapy regimens should generally include all venoms in multi-sensitized patients the recommendation is that immunotherapy (IT) should be extended to all venoms for which test results are positive and patients might potentially react to.

CONTEXT AND OBJECTIVE: Although it is generally agreed that a medical history and physical examination should be obtained as part of preoperative evaluation, there is still substantial controversy about the additional benefits of preoperative screening tests. The objective of the present study was to determine the percentage of abnormalities on laboratory tests among a population that underwent non-cardiac surgery and to correlate these tests with changes in preoperative evaluation management. DESIGN AND SETTING: Cross-sectional study, carried out in a University Hospital. METHODS: 991 patients aged over 40 years undergoing elective non-cardiac surgery from July 1997 to January 2000 were studied. Blood cell count, serum sodium, potassium, urea and creatinine, prothrombin, thrombin and partial thromboplastin time, electrocardiogram and chest X-ray were evaluated. RESULTS: Out of the 957 electrocardiograms performed, some type of abnormality was found in 504 cases (50.9%) and, among the 646 chest X-rays requested, 271 (42.0%) displayed alterations. Laboratory tests showed abnormal values ranging from 5.1% (prothrombin time) to 41.0% (hematocrit). Increased percentages of abnormal tests with increasing patient age were also observed. CONCLUSIONS: Although there were substantial numbers of screening test abnormalities in preoperative evaluations, these results seldom interfered in patient management. CONTEXTO E OBJETIVO: Embora seja consenso que uma anamnese e um exame clínico devam ser realizados como parte da avaliação pré-operatória, ainda há grande controvérsia a respeito dos benefícios adicionais de exames subsidiários de rastreamento na avaliação pré-operatória. Neste estudo objetivamos determinar a porcentagem de exames subsidiários alterados em uma população que foi submetida a cirurgia não-cardíaca e correlacionamos as anormalidades observadas nestes exames com mudanças realizadas nas condutas pré-operatórias. TIPO DE ESTUDO E LOCAL: Estudo transversal desenvolvido em Hospital Universitário. MÉTODOS: 991 pacientes com mais de 40 anos, que foram submetidos a operações não-cardíacas eletivas de julho de 1997 a janeiro de 2000 foram estudados. Foram avaliados o hemograma, as dosagens séricas de sódio, potássio, uréia e creatinina, os tempos de protrombina, trombina e tromboplastina parcial ativada, o eletrocardiograma e a radiografia de tórax. RESULTADOS: Dentre os 957 eletrocardiogramas realizados, alguma alteração foi observada em 504 (50,9%) casos e, dentre as 646 radiografias de tórax feitas, 271 (42,0%) apresentaram alterações. As porcentagens de exames de laboratório alterados variaram de 5,1% (tempo de protrombina) a 41,0% (hematócrito). Observamos, também, um aumento na porcentagem de exames alterados em pacientes mais idosos. CONCLUSÕES: Embora um grande número de exames subsidiários alterados seja observado na avaliação pré-operatória, estes resultados raramente interferem na conduta dos pacientes.
