1. 1808. Treatment Adherence and Outcomes Associated with Partial Oral Therapy vs. Intravenous Therapy in Patients with Serious Staphylococcus aureus Infections: A Comparative Effectiveness Study

Abstract

Background Randomized trials found oral step-down therapy to be as effective as intravenous (IV) therapy for infective endocarditis and bone and joint infections. However, the comparative effectiveness of IV versus oral step-down therapy for deep seated Staphylococcus aureus infections outside of the clinical trial setting is unknown. The objectives of this study were to compare treatment adherence and clinical outcomes between these two approaches in clinical practice. Methods This was a retrospective comparative effectiveness study involving adults hospitalized at an academic, safety net hospital between January 2019 to June 2021 with bacteremia, endocarditis, osteomyelitis, or septic arthritis due to S. aureus. Based on initial treatment plans, patients were categorized and analyzed in two groups: all IV or IV followed by oral step-down therapy. The co-primary outcomes were antibiotic adherence (percent of planned course received) and the proportion who completed therapy. The key secondary outcome of clinical failure was a composite of all-cause mortality, recurrent or new metastatic site of S. aureus infection, or requirement of an unplanned source control procedure. Results Of 249 patients included, 101 were in the all IV and 148 were in the oral step-down groups. Indications for treatment were osteomyelitis (72%), complicated bacteremia (25%), endocarditis (22%), septic arthritis (11%), and uncomplicated bacteremia (8%). Active substance abuse was observed in 43% of the IV group and 55% in the oral group. Between the all IV and oral step-down groups, antibiotic adherence rates did not differ significantly (Table); antibiotic therapy was completed by 93% vs 87% of patients (p = 0.13), respectively. Clinical failure occurred in 25% of the all IV group and 26% of the oral step-down group (p = 0.87). The frequency of other secondary outcomes was similar between groups; however, hospital length of stay was significantly longer in the IV group (Table). Table 1 Note: data presented as n (%) unless otherwise specified *within 6 months of hospital admission date for index infection Conclusion In clinical practice, oral step-down therapy for serious S. aureus infections was associated with similar rates of treatment adherence and completion, clinical outcomes and less health care resource utilization compared with IV therapy. Abstract

The REGAIN (Regional versus General Anesthesia for Promoting Independence after Hip Fracture) trial found similar ambulation and survival at 60 days with spinal versus general anesthesia for hip fracture surgery. Trial outcomes evaluating pain, prescription analgesic use, and patient satisfaction have not yet been reported.To compare pain, analgesic use, and satisfaction after hip fracture surgery with spinal versus general anesthesia.Preplanned secondary analysis of a pragmatic randomized trial. (ClinicalTrials.gov: NCT02507505).46 U.S. and Canadian hospitals.Patients aged 50 years or older undergoing hip fracture surgery.Spinal or general anesthesia.Pain on postoperative days 1 through 3; 60-, 180-, and 365-day pain and prescription analgesic use; and satisfaction with care.A total of 1600 patients were enrolled. The average age was 78 years, and 77% were women. A total of 73.5% (1050 of 1428) of patients reported severe pain during the first 24 hours after surgery. Worst pain over the first 24 hours after surgery was greater with spinal anesthesia (rated from 0 [no pain] to 10 [worst pain imaginable]; mean difference, 0.40 [95% CI, 0.12 to 0.68]). Pain did not differ across groups at other time points. Prescription analgesic use at 60 days occurred in 25% (141 of 563) and 18.8% (108 of 574) of patients assigned to spinal and general anesthesia, respectively (relative risk, 1.33 [CI, 1.06 to 1.65]). Abstract

Summary Engaging patients—defined broadly as individuals with lived experience of a given condition, family members, caregivers, and the organisations that represent them—as partners in research is a priority for policymakers, funders, and the public. Nonetheless, formal efforts to engage patients are absent from most studies, and models to support meaningful patient engagement in clinical anaesthesia research have not been previously described. Here, we review our experience in developing and implementing a multifaceted patient engagement strategy within the Regional Versus General Anesthesia for Promoting Independence After Hip Fracture (REGAIN) surgery trial, an ongoing randomised trial comparing spinal vs general anaesthesia for hip fracture surgery in 1600 older adults across 45 hospitals in the USA and Canada. This strategy engaged patients and their representatives at both the level of overall trial oversight and at the level of individual recruiting sites. Activities spanned a continuum ranging from events designed to elicit patients' input on key decisions to longitudinal collaborations that empowered patients to actively participate in decision-making related to trial design and management. Engagement activities were highly acceptable to participants and led to concrete changes in the design and conduct of the REGAIN trial. The REGAIN experience offers a model for future efforts to engage patients as partners in clinical anaesthesia research, and highlights potential opportunities for investigators to increase the relevance of anaesthesia studies by incorporating patient voices and perspectives into the research process.

Abstract

Background Staphylococcus aureus bloodstream infections (BSIs) are associated with increased morbidity, mortality, and healthcare costs. Severity of illness scores help stratify critically ill patients and assist providers in making decisions. The quick Pitt (qPitt) score is a reliable predictor of mortality in patients with Gram-negative BSIs (AUROC 0.85); however, limited data exist for using the qPitt in methicillin-susceptible S. aureus (MSSA) BSIs. Methods This retrospective cohort evaluated patients with MSSA BSIs. The primary outcome was the discrimination of the qPitt in predicting hospital mortality compared to the Pitt bacteremia score (PBS). Secondary outcomes were clinical failure and the predictive discrimination of the qPitt score in comparison to other severity scoring modalities. Categorical data were analyzed using chi-square test or Fisher’s exact test. Continuous data were analyzed using Student’s t-test or Mann-Whitney U. Predictive discrimination was determined by the area under receiver operating characteristic curve. Results One hundred patients were included with the mean age of 52 years (p=0.84) and a BMI of 30 kg/m2. Males were predominant (70%). Mortality occurred in 13 patients who had more ICU admissions (92% vs. 37%; p< 0.01) and longer ICU LOS (10 vs. 5 days; p=0.03) despite similar baseline comorbidities. Time to definitive therapy was longer in the mortality group than non-mortality group (28 vs. 23 hours; p=0.79) though antimicrobial use did not differ. As outlined in Figure 1, the mortality group had higher severity of illness scores. The qPitt had a ROC of 0.83, indicating high discrimination. Overall, qPitt was found to be similarly predictive than PBS, equally as predictive as qSOFA, and more predictive than SIRS and APACHE II (Figure 1). Clinical failure occurred in 23% of patients; 54% of the mortality group experienced persistent BSI vs. 11% in the non-mortality group (p< 0.01), and 3% of the non-mortality group had recurrent infection (p=1.00). Conclusion Compared to previous studies on Gram-negative BSIs, the qPitt performed similarly in MSSA BSIs. The qPitt can be considered for use in predicting mortality for patients with MSSA BSIs; however, further studies are needed to confirm these results. Abstract

An exploratory study was conducted to examine the quality of life and pain experienced by patients with pediatric cancer at home after discharge. Physical, cognitive, social, and emotional aspects of quality of life were measured and how these may be affected by age, sex, diagnosis, and pain status. The authors also characterized intensity, location, and quality of pain experienced. A sample of 33 patients participating in a larger study was selected on the basis of having pain on the day of discharge and having completed the Pediatric Quality of Life Inventory Generic, Cancer Module, Multidimensional Fatigue Scale, and the Adolescent Pediatric Pain Tool at home. Cancer diagnoses were leukemias/lymphomas (42.4%), brain/central nervous system tumors (27.3%), sarcomas (24.2%), or other (6.1%). More than half of patients reported pain (n=17; 51.5%). Patients with pain had more fatigue affecting the quality of life (P=0.01), and lower physical and emotional functioning, leading to lower overall health-related quality of life scores (P=0.011). Female individuals and adolescents reported worse emotional functioning (P=0.02 and P=0.05, respectively). Physical, cognitive, and social functioning were lowest among patients diagnosed with sarcomas (P=0.00, P=0.01, and P=0.04, respectively). It is important to understand the symptom experience of patients at home as a first step in moving towards optimal discharge teaching and treatment.

Abstract

Purpose: The purpose of this study was to explore the performance of pouching systems with respect to leakage and the development of peristomal skin complications (PSCs) in an acute care setting immediately following ostomy creation., Design: Non-experimental, retrospective cohort study., Subjects and Setting: The sample comprised 214 patients admitted for stoma-creation surgery at one of the 2 University of Pittsburgh Medical Center Presbyterian-Shadyside campuses located in the Northeastern United States (Pittsburgh, PA). Patients were seen in an in-patient care setting., Methods: Electronic health records from patients who were in the hospital for ostomy creation surgery were reviewed by wound ostomy continence nurses from each hospital. Demographic and selected clinical data were collected. The main outcome variables used to evaluate pouching system performance were leakage and PSCs. These outcomes were assessed starting at pouch placement in the operating room and at every subsequent pouch change (up to 5) through discharge or 2 weeks following surgery., Results: Use of an elastic tapeless barrier (ETB) significantly reduced the risk of leakage compared to a ceramide-infused tape-border barrier (CIB) for patients (41% reduced risk of leakage, P = .011). Use of the ETB also reduced leakage risk compared to the CIB for patients who experienced leakage 2 or more times during the observation period (31% reduction in leakage risk, P = .043). Five types of PSCs occurred during the study and statistical analysis indicated no significant differences in the number of PSCs was documented between barrier types., Conclusion: The use of an ETB significantly reduced the risk of leakage compared to a CIB., Competing Interests: The authors declare no conflicts of interests., (Copyright © 2024 The Authors.
