Summary: Background: Anti-COVID-19 hyperimmune immunoglobulin (hIG) can provide standardized and controlled antibody content. Data from controlled clinical trials using hIG for the prevention or treatment of COVID-19 outpatients have not been reported. We assessed the safety and efficacy of subcutaneous anti-COVID-19 hyperimmune immunoglobulin 20% (C19-IG20%) compared to placebo in preventing development of symptomatic COVID-19 in asymptomatic individuals with SARS-CoV-2 infection. Methods: We did a multicentre, randomized, double-blind, placebo-controlled trial, in asymptomatic unvaccinated adults (≥18 years of age) with confirmed SARS-CoV-2 infection within 5 days between April 28 and December 27, 2021. Participants were randomly assigned (1:1:1) to receive a blinded subcutaneous infusion of 10 mL with 1 g or 2 g of C19-IG20%, or an equivalent volume of saline as placebo. The primary endpoint was the proportion of participants who remained asymptomatic through day 14 after infusion. Secondary endpoints included the proportion of individuals who required oxygen supplementation, any medically attended visit, hospitalisation, or ICU, and viral load reduction and viral clearance in nasopharyngeal swabs. Safety was assessed as the proportion of patients with adverse events. The trial was terminated early due to a lack of potential benefit in the target population in a planned interim analysis conducted in December 2021. ClinicalTrials.gov registry: NCT04847141. Findings: 461 individuals (mean age 39.6 years [SD 12.8]) were randomized and received the intervention within a mean of 3.1 (SD 1.27) days from a positive SARS-CoV-2 test. In the prespecified modified intention-to-treat analysis that included only participants who received a subcutaneous infusion, the primary outcome occurred in 59.9% (91/152) of participants receiving 1 g C19-IG20%, 64.7% (99/153) receiving 2 g, and 63.5% (99/156) receiving placebo (difference in proportions 1 g C19-IG20% vs. placebo, −3.6%; 95% CI -14.6% to 7.3%, p = 0.53; 2 g C19-IG20% vs placebo, 1.1%; −9.6% to 11.9%, p = 0.85). None of the secondary clinical efficacy endpoints or virological endpoints were significantly different between study groups. Adverse event rate was similar between groups, and no severe or life-threatening adverse events related to investigational product infusion were reported. Interpretation: Our findings suggested that administration of subcutaneous human hyperimmune immunoglobulin C19-IG20% to asymptomatic individuals with SARS-CoV-2 infection was safe but did not prevent development of symptomatic COVID-19. The opinions of citizens are now being given ever-increasing consideration. Today, many government administrations have set up public participation processes as one more of the inputs required to make a decision on several aspects of governance. e-Participation initiatives make it easier for citizens to access such processes. At the present time, there is no clear and accepted field definition due to the wide diversity of theoretical proposals and the interdisciplinary nature of the initiatives, many of which have been developed ad-hoc. This paper reviews the present literature in the field of e-Participation by means of a systematic mapping of the research work carried out in the timeframe 2000–2019, together with some earlier relevant proposals in the area, with the aim of obtaining a conceptual guide to e-Participation components. This review analyses the findings and clusters the results into a conceptual e-Participation framework, which we call ePfw. The results show the diversity of the conceptualizations of many authors (25% on average) in the identification of tools, areas and levels in the field of e-participation and the almost null incorporation of fundamental aspects like trust, security, or transparency. We also found a lack of systems development (13.3%) that would prove and allow the proposed theories to be put into practice.

Category: Ankle, Diabetes, Trauma Introduction/Purpose: Fractures in diabetic patients have a well-known increased risk of complications and this makes the decision to treat these fractures either surgically or conservatively a difficult choice. However, ankle fractures are mostly treated surgically because of their pattern and the postoperative management does not differ from those ankle fractures in non-diabetic patients. The following study aims to review the evident rate of complications following the treatment of an ankle fracture in diabetic patients and their matched controls. Methods: Searches of PubMed, Scopus, Cochrane and ISI Web of Knowledge were performed for studies published between the date of database inception and March 2018. An initial selection of 202 abstracts was performed by at least 2 different reviewers, of which 77 articles were selected to complete review. After following strict inclusion and exclusion criteria, only 17 papers were admitted to the final meta-analysis. Demographics patient characteristics and incidence of the overall and specific complications were extracted from each study selected and an odds ratio with a 95% confidence interval of each complication was calculated between the diabetic and non- diabetic groups. Major complications (infection, non-union, malunion, Charcot neuroarthropathy, amputation, death) were compared not only between the two main groups but also between subgroups (complicated diabetic and non-complicated diabetic patients, surgical and orthopaedic treatment). The statistics data was analysed by Stata 15. Results: There is a significant increased rate of complications after treating an ankle fracture orthopaedically or surgically in diabetic patients (OR 1.74, IC 95% 1.67 to 1.82). This risk is considerably higher when the ankle fracture is treated surgically (OR 5.14, IC 95% 2.79 -9.58). Among the complications in diabetic patients, the rate is greater in complicated diabetic patients (neuropathy, vasculopathy) compared to the non-complicated diabetic patients (OR 8, IC 95% 2.61 - 26.31). The main complication postoperative in ankle fracture described is infection, which is 7 times higher in diabetic patients in comparison to non-diabetic patients (OR 6.9, IC 95% 3.03 -15.73). The risk of amputation and/or non-union after an ankle fracture in diabetic patients is about 0.2%. Conclusion: This meta-analysis provides evidence that diabetic patients have a significant greater risk of presenting a complication after an ankle fracture. The rate of major complications (infection, malunion, non-union, amputation and death) is by far significantly higher among those diabetic patients treated surgically and even greater among complicated diabetic patients.

El COVID-19 se introdujo rápidamente como una pandemia global, la cual necesita ser tratada con respuesta inmediatas e integradas a todos los sistemas nacionales que requieran de ellas. Con la llegada del COVID-19 el mundo vio la necesidad de respuestas oportunas y el intercambio de datos sobre ésta y futuras pandemias globales de rápida propagación. Este estudio se enfoca en predecir la incidencia del COVID-19 en Ecuador. Se realizó Minería de Datos de los registros proporcionados por instituciones públicas del estado ecuatoriano con información oficial y actualizada del COVID-19 en el Ecuador. Se experimento con modelos de regresión y memoria a largo plazo obteniendo como resultado el modelo óptimo para estimar el número de casos positivos de COVID-19. Para el modelo matemático se hizo uso del error cuadrático medio como métrica del rendimiento. Del análisis de los datos sobre el COVID-19 en Ecuador el modelo de regresión lineal predijo la incidencia con un error cuadrático medio de 0.54. siendo los factores más efectivos la incidencia de días anteriores y el número de población de cada una de las provincias afectadas.

Computer security is a fundamental part of an organization, especially in Higher Education institutions, where there is very sensitive information, capable of being vulnerable by diffeerent methods of intrusion, the most common being free access through wireless points. The main objective of this research is to analyze the impact of the open source tools in charge of managing the security information of the wireless network, such as OSSIM, a set of active and passive components used to manage events that generate tra c within the network. net. This research exposes the use of free software as a viable option of low cost to solve the problems that a ict student sta , such as lack of access to academic services, problems of wireless interconnectivity, with the purpose to restore confidence in students in the Use of the services offered by the institution for research-related development, guaranteeing free and free access to the internet. The level of dissatisfaction on the part of the students con rms the problem presented at the Technical University of Babahoyo, thus confirming the positive influence of the Open-Source tools for the institution’s wireless security.

En este artículo se muestran las ventajas de aprovechar las características de hardware, y la movilidad de los smartphones o tablets, en la recolección o actualización de información en ubicaciones geográficas extensas, en donde puede o no existir cobertura de internet para la transmisión de los datos. Se presentan las experiencias de una actualización de datos de clientes para la Empresa de Agua Potable del cantón de Pedernales, provincia de Manabí, Ecuador, mediante el uso de tablets con sistema operativo Android, y basándonos en la creación y evaluación de una aplicación móvil, el conocimiento base y el uso de SCRUM como metodología de desarrollo de software. El problema que se pretendió resolver fue la lentitud en la recolección de los datos provocados por el ingreso manual en fichas catastrales impresas y su posterior digitación en un Sistema de Información Geográfica implementado en la nube. Los resultados preliminares indicaron que, cambiando el proceso de recolección de datos por el uso de un software desarrollado para dispositivos móviles, se reducían los tiempos en el levantamiento de información de la totalidad de los predios de la ciudad.
