Objective: To evaluate acute and chronic changes seen on angiographic and histopathological studies of porcine rete mirabile, comparing those treated with the Menox liquid embolic system (LES) and those treated with the Onyx LES. Materials and Methods: Five pigs, each weighing approximately 35 kg, were submitted to rete mirabile embolization under general anesthesia and fluoroscopic guidance, with the Menox LES or Onyx LES. Four animals were treated with the Menox LES and underwent cerebral angiography, followed by euthanasia, at 1, 30, 60, and 90 days after embolization. One animal was treated with the Onyx LES underwent the same procedures at 30 days after embolization. In a subsequent histopathological analysis, we compared the Menox LES and Onyx LES in terms of the acute and chronic changes observed. Results: We observed no significant changes in blood pressure, heart rate, or electrocardiographic parameters that could be attributed to the super-selective infusion of dimethyl sulfoxide or the Menox embolic agent. Fluoroscopy showed adequate material opacity, appropriate progression to the center of the rete mirabile and complete unilateral embolization. Microcatheters were uneventfully detached from the embolized nidus. We observed mild to moderate intravascular and extravascular inflammatory responses, without histological evidence of necrotizing arteritis. There were no adverse neurovascular events. Conclusion: The Menox LES appears to be safe and effective, as well as being apparently equivalent to the Onyx LES in terms of the postprocedure angiographic and histopathological findings.

To analyze the influences of mild and severe intracranial hypertension on cerebral autoregulation (CA).Duroc piglets were monitored with an intracranial pressure (ICP) catheter. Intracranial hypertension was induced via infusion of 4 or 7 ml of saline solution by a bladder catheter that was inserted into the parietal lobe. The static cerebral autoregulation (sCA) index was evaluated via cerebral blood flow velocities (CBFv). Piglets with ICPs ≤ 25 and25 mmHg were considered as group 1 and 2, respectively. Continuous variables were evaluated using the Kolmogorov-Smirnov goodness-of-fit test. The main parameters were collected before and after ICH induction and compared using two-factor mixed-design ANOVAs with the factor of experimental group (mild and severe ICH).In group 1 (ICP ≤ 25 mmHg), there were significant differences in sCA (We conclude that intracranial hypertension may have a direct effect on sCA.

Purpose: To determine the normal optical nerve sheath (ONS) diameter ultrasonography (ONSUS) and evaluate the possible effects of drugs on ONS diameter during anesthetic induction in healthy pigs. Methods: Healthy piglets were divided into three groups: a control group, that received xylazine and ketamine (X/K); other that received xylazine, ketamine and propofol (X/K/P); and a third group that received xylazine, ketamine, and thiopental (X/K/T). The sheath diameter was assessed by ultrasonography calculating the average of three measurements of each eye from the left and right sides. Results: 118 animals were anesthetized (49 X/K 33 X/K/P and 39 X/K/T). Mean ONS sizes on both sides in each group were 0.394 ± 0.048 (X/K), 0.407 ± 0.029 (X/K/P) and 0.378 ± 0.042 cm (X/K/T) (medians of 0.400, 0.405 and 0.389, respectively). The ONS diameter varied from 0.287–0.512 cm (mean of 0.302 ± 0.039 cm). For group X/K, the mean diameter was 0.394 ± 0.048 cm. Significant differences in ONS sizes between the groups P and T (X/K/P > X/K/T, p = 0.003) were found. No statistically significant differences were detected when other groups were compared (X/K = X/K/P, p = 0.302; X/K = X/K/T, p = 0.294). Conclusions: Sedation with thiopental lead to significative ONS diameter reduction in comparison with propofol. ONSUS may be useful to evaluate responses to thiopental administration.

BackgroundIntracranial hypertension (ICH) is a common final pathway of most neurosurgical pathologies and leads to poor prognosis if not detected and treated properly. Inflammatory markers have been assessed in clinical scenarios of neurological injuries, in which systemic and brain tissue aggressions may introduce bias. There is a lack of studies under controlled settings to isolate the ICH effect on inflammation. This study aims to evaluate the effects of ICH on the serum concentration of cytokines as biomarkers of neuroinflammation in an experimental model which isolates ICH from potential confounding variables.MethodsAn established model of ICH using an intracerebral pediatric bladder catheter and a multisensor intraparenchymal catheter was used in adult pigs (Sus domesticus). The animals were randomly allocated to 2 groups based on the catheter balloon volume used to simulate the ICP increase (4 ml or 7 ml). Cytokines were measured in 4 timepoints during the experiment: (1) 15 min before balloon insufflation; (2) 5 min after insufflation; (3) 125 min after insufflation; (4) 60 min after deflation. The following cytokines were measured IL-1α; IL-1β; IL–1ra; IL-2; IL-4; IL-6; IL-8; IL-10; IL-12; IL-18; TNFα. Generalized estimating equations were modeled to compare the ICP and cytokines values between the groups along the experiment. The study sample size was powered to detect interactions between the groups and the study moments with an effect size (f) of at least 0.3. The ARRIVE checklist was followed.ResultsA total of 20 animals were studied (10 in each group, 4 ml or 7 ml balloon volume insufflation). The animal model was successful in increasing the ICP along the moments of the experiment (p p = 0,004). Background: Invasive monitoring of intracranial pressure (ICP) is currently the gold standard method for the safe diagnosis and treatment of intracranial hypertension (ICHy), but it is subject to hemorrhage, infection and malfunction. Ultrasound of the optic nerve sheath diameter (ONSD) has been applied as a non-invasive alternative that is cost effective and available at the bedside. However, ONSD time-lapse behavior in a set of ICHy and its relief by means of whether saline infusion or surgery is still unknown. The objective of this study was to correlate intracranial pressure and ultrasonography of the optic nerve sheath in an experimental animal model of ICHy, and the interval needed to ONSD to return to its baseline levels.Methods: An experimental study was conducted on 30 male and female pigs weighing about 20 kg. The diameter of the optic nerve sheath was evaluated by ultrasound at different measures of ICP given by intraventricular catheter and intracranial balloon inflation, saline infusion and balloon deflation. Laboratory and hemodynamic data were collected from the animals. ICP and ONSD values were correlated at each time point of the study.Results: All the variables obtained by ONSD ultrasonography as left optic nerve, right optic nerve and average of optic nerve sheath (AON) diameter were statistically significant to estimate ICP value. ONSD values changed immediately at balloon inflation, with a delay of 30 minutes average to return to baseline levels after balloon deflation (p = 0.016). No statistical significance was observed in ICP and ONSD values with hypertonic saline infusion. Correlation between ICP and ONSD was linear and can be estimated using the formula: -80.5 + 238.2 x AON, in this swine model. Conclusion: In the present study, ultrasound of optic nerve sheath diameter disclosed linear correlation with ICP, although a short delay in returning to its baseline levels may be observed in the case of sudden intracranial hypertension relief.

Background. Ischemic preconditioning (IPC), either direct (DIPC) or remote (RIPC), is a procedure aimed at reducing the harmful effects of ischemia-reperfusion injury. Aims. To assess the local and systemic effects of DIPC, RIPC, and both combined, in the pig liver transplant model. Methods. Twenty-four pigs underwent orthotopic liver transplantation and were divided into 4 groups according to the procedures applied: direct donor preconditioning; indirect preconditioning at the recipient and a group with direct donor and indirect recipient preconditioning. The following parameters were recorded: donor and recipient weight, graft-to-recipient weight ratio (GRWR), surgery time, hot and cold ischemia time, and intraoperative hemodynamic values. Blood samples were collected before native liver removal (BL) and at 0h, 1h, 3h, 6h, 12h, 18h, and 24h post-reperfusion and the following biochemical tests were performed: aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), gamma-glutamyl transferase (GGT), creatinine, BUN (blood urea nitrogen), lactate, total and direct bilirubin. Histopathological examination of liver, gut, kidney, and lung fragments were performed, as well as molecular analyses for expression of the apoptosis-related BAX (pro-apoptotic) and Bcl-XL (anti-apoptotic) genes, eNOS (endothelial nitric oxide synthase) gene, and IL-6 gene related to inflammatory ischemia-reperfusion injury, using real-time polymerase chain reaction (RT-PCR). Results. There were no differences between the groups regarding biochemical and histopathological parameters. We found a reduced ratio between the expression of the pro-apoptotic BAX gene and the expression of the anti-apoptotic Bcl gene in the livers of animals with IPC versus the control group. Conclusions. DIPC, RIPC or a combination of both produce local beneficial effects only at the molecular level but do not translate into biochemical or histological changes.

Background. This study aimed to analyse cerebral autoregulation (CA) during induction and treatment of intracranial hypertension (ICH) in an experimental model. Materials and Methods. Landrace and Duroc piglets were divided into mild and severe ICH groups. Four or seven millilitres of saline solution was infused into paediatric bladder catheter inserted in the parietal lobe (balloon inflation). After 1.5 h, a 3% saline solution was infused via venous catheter, and 30 min later, the bladder catheter balloon was deflated (surgery). The cerebral static autoregulation (sCA) index was evaluated using cerebral blood flow velocities (CBFV) obtained with Doppler ultrasound. Results. Balloon inflation increased ICP in both groups. The severe ICH group showed significantly lower sCA index values (p=0.001, ANOVA) after balloon inflation (ICH induction) and a higher sCA index after saline injection (p=0.02) and after surgery (p=0.04). ICP and the sCA index were inversely correlated (r=−0.68 and pr=0.74 and p Conclusion. ICH was associated with local balloon expansion, which triggered CA impairment, particularly in the severe ICH group. Moreover, ICP-reducing treatments were associated with improved CA in subjects with severe ICH.

Hypertensive intracranial haemorrhage is a common neurological emergency in clinical practice. The presence of an intracranial lesion of expansive focal nature can compress vascular structures and cause ischaemic effects. It is very common for these patients to have hypertensive peaks at admission, which may progress to rebleeding and neurological worsening. The safety of blood pressure reduction in patients with hypertension and intracranial haematomas is still a debatable subject due to lack of studies on cerebral autoregulation in this situation. The aim of this study is to discuss cerebral autoregulation in patients with spontaneous intracerebral haemorrhage based on scientific and personal evidence.

OBJECTIVE: In most cases of pediatric liver transplantation, the clinical scenario of large-for-size transplants can lead to hepatic dysfunction and a decreased blood supply to the liver graft. The objective of the present experimental investigation was to evaluate the effects of ischemic preconditioning on this clinical entity. METHODS: Eighteen pigs were divided into three groups and underwent liver transplantation: a control group, in which the weights of the donors were similar to those of the recipients, a large-for-size group, and a large-for-size + ischemic preconditioning group. Blood samples were collected from the recipients to evaluate the pH and the sodium, potassium, aspartate aminotransferase and alanine aminotransferase levels. In addition, hepatic tissue was sampled from the recipients for histological evaluation, immunohistochemical analyses to detect hepatocyte apoptosis and proliferation and molecular analyses to evaluate the gene expression of Bax (pro-apoptotic), Bcl-XL (anti-apoptotic), c-Fos and c-Jun (immediate-early genes), ischemia-reperfusion-related inflammatory cytokines (IL-1, TNF-alpha and IL-6, which is also a stimulator of hepatocyte regeneration), intracellular adhesion molecule, endothelial nitric oxide synthase (a mediator of the protective effect of ischemic preconditioning) and TGF-beta (a pro-fibrogenic cytokine). RESULTS: All animals developed acidosis. At 1 hour and 3 hours after reperfusion, the animals in the large-for-size and large-for-size + ischemic preconditioning groups had decreased serum levels of Na and increased serum levels of K and aspartate aminotransferase compared with the control group. The molecular analysis revealed higher expression of the Bax, TNF-alpha, I-CAM and TGF-beta genes in the large-for-size group compared with the control and large-for-size + ischemic preconditioning groups. Ischemic preconditioning was responsible for an increase in c-Fos, IL-1, IL-6 and e-NOS gene expression. CONCLUSION: Ischemia-reperfusion injury in this model of large-for-size liver transplantation could be partially attenuated by ischemic preconditioning.

In pediatric liver transplantations with LFS grafts, higher incidences of graft dysfunction probably occur due to IRI. It was postulated that increasing the blood supply to the graft by means of a meso-caval shunt could ameliorate the IRI. Eleven pigs underwent liver transplantation and were divided into two groups: LFS and LFS+SHUNT group. A series of flowmetric, metabolic, histologic, and molecular studies were performed. No significant metabolic differences were observed between the groups. One hour after reperfusion, portal flow was significantly lower in the recipients than in the donors, proving that the graft was maintained in low portal blood flow, although the shunt could promote a transient increase in the portal blood flow and a decrease in the arterial flow. Finally, it was verified that the shunt promoted a decrease in inflammation and steatosis scores and a decrease in the expression of the eNOS gene (responsible for the generation of nitric oxide in the vascular endothelium) and an increase in the expression of the proapoptotic gene BAX. The meso-caval shunt was responsible for some positive effects, although other deleterious flowmetric and molecular alterations also occurred.

Resumo Objetivo: Neste trabalho temos o objetivo de avaliar a acurácia do sistema de aferição da pressão intracraniana (PIC) epidural com microchip. Métodos: Foram estudados 27 suínos sob anestesia geraI, devidamente assistidos com monitoração ventilatória e hemodinâmica. Durante o experimento foi reproduzido um processo expansivo intracerebral programado no lobo frontal direito. O experimento constou de três grupos (A, B e C) com hipertensão intracraniana gerada com balão reproduzindo um hematoma intracerebral. Em todos os grupos foram calibrados os parâmetros normais: os dois sistemas de PIC foram comparados e estudados quanto à correlação dos valores aferidos. Resultados: O comportamento médio da PIC ao longo dos momentos de avaliação foi estatisticamente diferente entre os grupos (p < 0,001). A reprodução de ressangramento resultou em elevação significativa da PIC (p < 0,001). Avaliando-se a acurácia comparativa geral, verificou-se um coeficiente de correlação intraclasse de 0,8. Conclusão: O modelo de hipertensão intracraniana por balão em suínos é factível e confiável na geração de hipertensão intracraniana. O sistema de aferição de pressão intracraniana epidural apresenta elevado coeficiente de correlação com o sistema de aferição parenquimatoso na avaliação global.

Objective Intracranial hypertension (IH) develops in approximately 50% of all patients with severe traumatic brain injury (TBI). Therefore, it is very important to identify a suitable animal model to study and understand the pathophysiology of refractory IH to develop effective treatments. Methods We describe a new experimental porcine model designed to simulate expansive brain hematoma causing IH. Under anesthesia, IH was simulated with a balloon insufflation. The IH variables were measured with intracranial pressure (ICP) parenchymal monitoring, epidural, cerebral oximetry, and transcranial Doppler (TCD). Results None of the animals died during the experiment. The ICP epidural showed a slower rise compared with parenchymal ICP. We found a correlation between ICP and cerebral oximetry. Conclusion The model described here seems useful to understand some of the pathophysiological characteristics of acute IH. Objetivo A hipertensão intracraniana (HIC) ocorre em até 50% de todos os pacientes com traumatismo cranioencefálico (TCE). Por isso, é importante estabelecer um modelo animal adequado para estudar a fisiopatologia da HIC refratária, com a perspectiva de desenvolver tratamentos eficazes. Métodos Os animais foram submetidos a um protocolo padrão de anestesia. A hipertensão intracraniana foi estabelecida através de insuflação de um balão. As variáveis HIC foram medidas com a pressão intracraniana (PIC) do parênquima, oximetria, epidural e doppler transcraniano. Resultados A PIC epidural apresentou elevação mais lenta, comparada com a PIC parenquimal. Houve correlação entre a PIC e a oximetria cerebral. O registro da PIC, oximetria e índice de pulsatilidade foi realizado em todos os animais sem dificuldade. Conclusão O modelo descrito parece ser útil para a compreensão de algumas características fisiopatológicas na HIC aguda.

OBJECTIVE: The ideal ratio between liver graft mass and recipient body weight for liver transplantation in small infants is unknown; however, if this ratio is over 4%, a condition called large-for-size may occur. Experimental models of large-for-size liver transplants have not been described in the literature. In addition, orthotopic liver transplantation is marked by high morbidity and mortality rates in animals due to the clamping of the venous splanchnic system. Therefore, the objective of this study was to create a porcine model of large-for-size liver transplantation with clamping of the supraceliac aorta during the anhepatic phase as an alternative to venovenous bypass. METHOD: Fourteen pigs underwent liver transplantation with whole-liver grafts without venovenous bypass and were divided into two experimental groups: the control group, in which the weights of the donors were similar to the weights of the recipients; and the large-for-size group, in which the weights of the donors were nearly 2 times the weights of the recipients. Hemodynamic data, the results of serum biochemical analyses and histological examination of the transplanted livers were collected. RESULTS: The mortality rate in both groups was 16.5% (1/7). The animals in the large-for-size group had increased serum levels of potassium, sodium, aspartate aminotransferase and alanine aminotransferase after graft reperfusion. The histological analyses revealed that there were no significant differences between the groups. CONCLUSION: This transplant method is a feasible experimental model of large-for-size liver transplantation.

BACKGROUND Current clinical treatment methods for refractory intracranial hypertension include elevation of the decubitus, ventilation adjustment, and use of hypertonic solutions such as hypertonic saline and mannitol solutions. Previous studies have shown that hypertonic solutions are particularly effective. Although several concentrations of saline solution have been proposed, a 3% solution is the most widely used. The aim of this study was to evaluate the maintained efficacy of a 3% hypertonic saline solution in an experimental model of intracranial hypertension. MATERIAL AND METHODS A porcine model of reversible intracranial hypertension was created by inserting a balloon catheter into the brain parenchyma, which was inflated and deflated to simulate intracranial hypertension and its surgical correction. The experiment included 3 groups of animals (A, B, and C) with different balloon inflation volumes. In group B, balloons were inflated 2 times to simulate reexpansion. A 20 mL/kg bolus of 3% saline solution was infused using a pump 90 minutes after the start of balloon inflation, and the effects of intracranial pressure were evaluated 60 minutes after infusion. RESULTS No increases outside of the normal range were observed in mean serum sodium concentrations (p=0.09). In addition, we identified no differences within each group in serum sodium levels measured during hypertonic saline infusion (p=0.21). No significant reductions in intracranial pressure were observed in any of the 3 groups. CONCLUSIONS Bolus infusion of 3% hypertonic saline solution with the aid of a pump does not significantly reduce intracranial pressure in an animal model of intracranial hypertension.

OBJECTIVE: To evaluate the feasibility of using endovascular repair to treat penetrating arterial injuries with covered stents. Feasibility was examined according to the circumferential extent of the injury. INTRODUCTION: Surgical trauma often increases the risk of major morbidity and mortality associated with vascular injury, and endovascular repair has many advantages in such situations. METHODS: Twenty white male domestic pigs weighing 28-38 kg with controlled vascular injuries were divided into four equal groups according to the circumferential extent of their vascular lesion (i.e., no lesion, lesion 50%, and complete lesion). The left common carotid artery was dissected with proximal and distal control, and this procedure was followed by controlled sectioning of the arterial wall. Local manual compression was applied for 10 min and was followed by endovascular repair with the placement of a 5x50 mm VIABHAN TM covered stent using the femoral approach. We also monitored additional variables, such as the duration of the procedures (the mean was 56.3 ± 19.1 min), ultrasound parameters (e.g., maximum arterial diameter, peak systolic and diastolic velocity, and resistance index), arteriography findings, and fluctuations in vital signs (e.g., cardiac output, arterial pressure, and central venous pressure). RESULTS: The experimental procedure was found to be feasible and reproducible. Repairs were successful in all animals in the control (no lesion) and 50% group and in one pig in the complete lesion group. DISCUSSION: The endovascular repair of an arterial injury is possible, but success depends on the circumferential extent of the arterial lesion. The present experimental model, which involved endovascular techniques, highlighted important factors that must be considered in future studies involving similar animals and materials.

INTRODUÇÃO: A reperfusão que segue um insulto isquêmico aumenta o dano tecidual, promove inflamação local e, conforme o território comprometido, pode desencadear resposta inflamatória sistêmica e progredir com disfunção de múltiplos órgãos. O intestino é particularmente vulnerável ao fenômeno de isquemia e reperfusão e tem sido implicado na gênese da resposta inflamatória sistêmica e da disfunção de múltiplos órgãos em pacientes graves. Os benefícios cardiovasculares, microcirculatórios e imunomoduladores da solução salina hipertônica a 7,5% (HS) e as propriedades hemorreológicas da pentoxifilina são atraentes para modular os efeitos nocivos da reperfusão. O presente estudo avaliou os efeitos metabólicos e hemodinâmicos (sistêmicos e regionais) da infusão da solução salina hipertônica 7,5% com pentoxifilina (HS+Ptx), em modelo experimental de isquemia e reperfusão, induzido pelo clampeamento da aorta supracelíaca em suinos. MÉTODOS: Trinta e um suínos mestiços, anestesiados e sob ventilação mecânica, foram randomizados em 4 grupos: grupo controle (n=8, sem tratamento), grupo ringer com lactato (n=7; 32ml/kg), grupo HS (n=8; 4ml/kg) e grupo HS+Ptx (n=8; 4ml/kg+25mg/kg). Foram submetidos a clampeamento da aorta supracelíaca durante 30 minutos, ocasião em que receberam o tratamento tendo sido observados por 240 minutos após reperfusão. As variáveis metabólicas e hemodinâmicas sistêmicas foram obtidas por meio de um cateter posicionado na aorta torácica, carótida direita e de um cateter de Swan-Ganz na artéria pulmonar. Para a avaliação regional, foram posicionados cateteres na veia porta e hepática e fluxômetros ultrassônicos ao redor da veia porta, artéria hepática e da veia renal direita. Os níveis de lactato e as variáveis de oxigenação sistêmica e regional foram obtidos com amostras de sangue arterial, venoso misto, hepático e porta. RESULTADOS: O clampeamento da aorta promoveu um estado hiperdinâmico sistêmico caracterizado por hipertensão, taquicardia, aumento do débito cardíaco (DC) e da oferta de oxigênio sistêmico com redução do consumo, da taxa de extração sistêmica de oxigênio e do gradiente venoarterial de CO2, com paradoxal aumento do lactato e redução do excesso de base. A súbita queda dos fluxos porta, hepático e renal foi acompanhada pela redução da oferta e do consumo com aumento da extração de oxigênio, do gradiente portaarterial de CO2 e do lactato regional. Em todos os grupos, a reperfusão foi caracterizada por hipotensão imediata e sustentada, retorno do DC aos valores iniciais, mas com progressivo aumento do gradiente veno arterial de CO2 e da taxa de extração sistêmica de oxigênio. Regionalmente, houve recuperação inicial dos fluxos com redução do gradiente porta-arterial de CO2 e da taxa de extração regional de oxigênio, seguido, por redução gradual dos fluxos com alargamento do gradiente e aumento da extração regional de oxigênio no decorrer do experimento. Durante a reperfusão, os níveis de lactato sistêmico e regional retornaram aos basais. CONCLUSÕES: A isquemia e a reperfusão da aorta supracelíaca promoveram significativas alterações hemodinâmicas sistêmicas e regionais semelhantes em todos os grupos. A expansão volêmica administrada durante a isquemia promoveu discretas e transitórias melhorias sistêmicas e regionais, semelhantes nos grupos tratados, mas não evitou a magnitude da hipotensão observada durante a reperfusão. Uma discutível vantagem da utilização de menores volumes nos grupos HS, um possível benefício adicional da pentoxifilina nos fluxos, assim como o melhor momento para a administração dos fluidos necessitam melhor investigação INTRODUTION: Reperfusion, following an initial ischemic insult, increases tissue damage, causes inflammation and, depending on the affected area, can trigger the systemic inflammatory response and progress toward a multiple organ dysfunction. The intestine is particularly vulnerable to ischemia/reperfusion phenomena and has also been implicated in the genesis of systemic inflammatory syndrome and the syndrome of multiple organ dysfunctions in critically ill patients. The cardiovascular, microcirculatory and immunomodulatory systems benefit from the use of hypertonic saline solution at 7.5% (HS); the hemorheological properties of pentoxifylline are attractive to modulate the harmful effects of reperfusion. The present study evaluated the hemodynamic and metabolic effects (systemic and regional) of hypertonic 7.5% in association of hypertonic 7.5% with pentoxifylline (HS +Ptx) in experimental ischemia-reperfusion injury induced by supraceliac aortic clamping in swines. METHODS: Thirty-one crossbred pigs, anesthetized and mechanically ventilated, were randomized into 4 groups: control group (n = 8, no treatment), lactate ringer\'s group (n = 7; 32ml/kg), HS group (n = 8; 4 ml/kg) and HS+Ptx group (n = 8; 4ml/Kg+25mg/kg); they were submitted to ischemia by supraceliac aortic clamping for 30 minutes. All groups received treatment during the clamping procedure and were observed for 240 minutes post reperfusion. The systemic hemodynamic and metabolic variables were obtained through catheters positioned in the thoracic aorta, the right carotid and through a Swan-Ganz catheter in the pulmonary artery. For a regional evaluation, catheters were placed in the portal and hepatic veins and ultrassonic flowmeters were placed around the portal vein, hepatic artery and right renal vein. Lactate levels and systemic and regional oxygenation variables were obtained with samples of arterial blood, hepatic, portal and mixed venous. RESULTS: The aortic clamping produced a systemic hyperdynamic state characterized by hypertension, tachycardia, increased cardiac output (CO) and systemic oxygen delivery with the reduction of consumption and systemic oxygen extraction rate and of venous-arterial CO2 gradient with a paradoxical increase in lactate and reduction of base excess. All animals presented a sudden reduction in portal, hepatic and renal flows with oxygen delivery and consumption reduction, increase in oxygen extraction and portal-arterial CO2 gradient, and regional hiperlactatemia. In all groups, reperfusion was characterized by an immediate and maintained hypotension, while CO returned to baseline values, but by a progressive increase in the venous-arterial CO2 gradient and by an increase in the systemic oxygen extraction rate. Regionally, there was an initial flow recovery with a decrease of portal arterial CO2 gradient and regional oxygen extraction rate, followed by a gradual decrease with an increase of gradient and of regional oxygen extraction during the experiment. During reperfusion, regional and systemic lactate levels returned to baseline values. CONCLUSION: Supraceliac aortic ischemia produced significant hemodynamic (systemic and regional) alterations, which were similar in all groups. The volemic expansion produced by infused fluids during ischemia promoted discrete and transitory systemic and regional improvements similar in all groups but unable to prevent the hypotension observed during reperfusion. A possible advantage of using smaller volumes in both HS groups, an equally possible additional Ptx benefit in regional blood flows, as well as the best time for fluids infusion remains to be investigated by further studies

PURPOSE:To evaluate intestinal inflammatory and apoptotic processes after intestinal ischemia/reperfusion injury, modulated by pentoxifylline and hypertonic saline.METHODS:It was allocated into four groups (n=6), 24 male Wistar rats (200 to 250g) and submitted to intestinal ischemia for 40 min and reperfusion for 80 min: IR (did not receive any treatment); HS group (Hypertonic Saline, 4ml/kg-IV); PTX group (Pentoxifylline, 30mg/kg-IV); HS+PTX group (Hypertonic Saline and Pentoxifylline). All animals were heparinized (100U/kg). At the end of reperfusion, ileal fragments were removed and stained on hematoxylin-eosin and histochemical studies for COX-2, Bcl-2 and cleaved caspase-3.RESULTS:The values of sO2 were higher on treated groups at 40 minutes of reperfusion (p=0.0081) and 80 minutes of reperfusion (p=0.0072). Serum lactate values were lower on treated groups after 40 minutes of reperfusion (p=0.0003) and 80 minutes of reperfusion (p=0.0098). Morphologic tissue injuries showed higher grades on IR group versus other groups: HS (p=0.0006), PTX (p=0.0433) and HS+PTX (p=0.0040). The histochemical study showed lesser expression of COX-2 (p=0.0015) and Bcl-2 (p=0.0012) on HS+PTX group. A lower expression of cleaved caspase-3 was demonstrated in PTX (p=0.0090; PTXvsIR).CONCLUSION:The combined use of pentoxifylline and hypertonic saline offers best results on inflammatory and apoptotic inhibitory aspects after intestinal ischemia/reperfusion. Background Ischemia–reperfusion injury is partly responsible for morbidity in pediatric liver transplantation. Large-for-size (LFS) liver transplantation has not been fully studied in the pediatric population, and the effects of reperfusion injury may be underestimated. Materials and methods Thirteen Landrace–Large white pigs weighing 23 kg (range, 17–38 kg) underwent orthotopic liver transplantation. They were divided into two groups according to the size of the donor body: LFS and control (CTRL). After transplantation, the abdominal cavity of the recipient was kept open and portal venous flow (PVF) was measured after 1 h. The ratio of recipient PVF (PVFr) to donor PVF was used to establish correlations with ischemia and reperfusion parameters. Liver biopsies were taken 1 h after transplantation to assess ischemia and reperfusion and to quantify the gene expression of endothelial nitric oxide synthase, interleukin 6, BAX , and BCL . Results Recipient weight, total ischemia time, and warm ischemia time were similar between groups. Among hemodynamic and metabolic analyses, pH, central arteriovenous PCO 2 difference, and AST were statistically worse in the LFS group than in the CTRL group. The same was found with endothelial nitric oxide synthase (0.41 ± 0.18 versus 1.56 ± 0.78; P = 0.02) and interleukin 6 (4.66 ± 4.61 versus 16.21 ± 8.25; P = 0.02). In the LFS group, a significant decay in the PVFr was observed in comparison with the CTRL group (0.93 ± 0.08 and 0.52 ± 0.11, respectively; P Conclusions The implantation of a graft was responsible for poor hemodynamic status of the recipient 1 h after transplantation. Furthermore, the LFS group demonstrated markers of ischemia and reperfusion that were worse when compared with the CTRL group and exhibited a more significant decrease in PVF from donor to recipient.

Introdução: A hipertensão intracraniana é uma condição frequente na prática neurocirúrgica, sendo o diagnóstico precoce importante para definição de tratamento. Modelos experimentais de hipertensão intracraniana podem ser utilizados para conhecer os aspectos das condições hemodinâmicas encefálicas e estudar o efeito de novas medicações ou intervenções. Soluções osmoticamente ativas têm sido utilizadas para tratamento de hipertensão intracraniana com resultados conflitantes. Analisamos os efeitos da solução salina hipertônica a 20% e manitol a 20% sobre a pressão intracraniana aferida por cateter intraparenquimatoso e medidas ultrassonográficas da bainha do nervo óptico em modelo experimental suíno de hematoma intracerebral na fase aguda. Métodos: Foi realizado um estudo experimental em modelo animal de hipertensão intracraniana com 20 suínos de ambos os sexos com peso aproximado de 40kg. Nos grupos A (5 suínos) e B (5 suínos), foram simulados um hematoma intracraniano por meio da insuflação de uma sonda de Foley com volume final de 4ml. Nos grupos C (5 suínos) e D (5 suínos), atingimos um volume final de 7ml. Os valores de 4ml e 7ml são equivalentes a uma coleção de, aproximadamente, 80ml e 140ml em um humano adulto levando-se em consideração a proporção entre o peso do cérebro do animal (média de 75g) e o peso do cérebro de um adulto normal (1500g). Após a insuflação do balão, utilizamos manitol a 20% (grupos A e C) e solução salina hipertônica a 20% (grupos B e D). Resultados: O modelo experimental utilizado foi eficaz no aumento da pressão intracraniana. Manitol a 20% e solução salina a 20% não foram eficazes na diminuição da pressão intracraniana aferida por cateter intraparenquimatoso, mas foram eficazes na diminuição dos valores de ultrassom da bainha do nervo óptico nos grupos de 7ml sem diferença entre as soluções. Conclusões: O modelo experimental suíno de hipertensão intracraniana apresenta boa reprodutibilidade. Solução salina hipertônica a 20% diminuiu os valores de bainha do nervo óptico em modelo experimental suíno de hipertensão intracraniana sem inferioridade em relação ao manitol a 20%. Ultrassom da bainha nervo óptico é método não invasivo, de baixo custo, com facilidade de repetição e pode auxiliar na prestação de cuidados em pacientes com hipertensão intracraniana Introduction: Intracranial hypertension, a frequent neurosurgical occurrence, is a severe condition in which early diagnosis is essential to provide prompt treatment. Experimental models employing invasive and noninvasive techniques for the assessment of intracranial hypertension can be used to elucidate the hemodynamic aspects of the condition as well as the effect of new medications/interventions for its management. Osmotically active solutions have been used to treat intracranial hypertension, but with conflicting results. We analyzed the effects of 20% hypertonic saline solution and 20% mannitol on intracranial pressure, measured via an intraparenchymal catheter and ultrasound measurements of the optic nerve sheath, in an experimental porcine model of acute-phase intracerebral hematoma. Methods: This experimental study was conducted on 20 pigs of both sexes, weighing approximately 40 kg, divided into four groups. In groups A and B (five pigs each), an intracranial hematoma was simulated by insufflation through a Foley tube, to a final volume of 4 mL. In Groups C and D (five pigs each), we attained a final volume of 7 mL. The values of 4 and 7 mL correspond to a collection of approximately 80 and 140 mL in an adult human, considering the ratio between the weight of the animal\'s brain (average 75 g) and that of a standard adult brain (1,500 g). After balloon inflation, 20% Mannitol was used in groups A and C, whereas 20% hypertonic saline solution was used in groups B and D. Results: The experimental model was effective in increasing the intracranial pressure. Measured using an intraparenchymal catheter, 20% mannitol and 20% saline solution were ineffective in reducing intracranial pressure. However, they effectively reduced the optic nerve sheath ultrasound values in the 7-mL groups, with no difference between the solutions. Conclusion: The experimental porcine model of intracranial hypertension exhibited good reproducibility, wherein 20% hypertonic saline decreased the optic nerve sheath values, without a significant difference from 20% mannitol. Optic nerve sheath ultrasound is a non-invasive, low-cost, easy-to-repeat method that can help determine appropriate care for intracranial hypertension patients

Introdução: a pressão intracraniana pode ser monitorada por meio de vários métodos que podem ser invasivos ou não invasivos. A ultrassonografia do nervo óptico é uma técnica não invasiva que permite mensurar a bainha deste nervo e detectar possíveis variações no seu diâmetro. O nervo óptico faz parte do sistema nervoso central de maneira contígua e é envolvido por uma bainha. Sendo assim, elevações ou reduções da pressão intracraniana podem ser transpostas à bainha deste nervo com consequente variação do seu diâmetro. Essas variações podem ser observadas pela ultrassonografia. Objetivo: determinar, por meio da ultrassonografia, o diâmetro da bainha do nervo óptico normal e avaliar os possíveis efeitos das drogas neste diâmetro durante a indução anestésica em suínos hígidos com pressão intracraniana normal. Métodos: foram selecionados 118 suínos híbridos saudáveis (64 fêmeas) de aproximadamente 20 kg e faixa etária similar. Todos os suínos foram submetidos à anestesia geral e foram devidamente monitorados. Os animais foram divididos em três grupos conforme os medicamentos utilizados: Grupo A: utilizando medicamento pré-anestésico xilazina e quetamina; Grupo B: utilizando xilazina (X), quetamina (Q) mais Propofol (P), e Grupo C: anestesiados com xilazina, quetamina e tiopental [tionembutal (T)]. As coletas das medidas nos três grupos foram feitas pelo aparelho de ultrassom em triplicata de cada olho, com os animais em posição laterolateral. Resultados: não houve diferenças estatisticamente significantes entre sexo e peso. O Diâmetro médio da bainha do nervo óptico em ambos os lados de cada grupo foram de 0,394±0,048 cm (X/Q), 0,407±0,029 cm (X/Q/P) e 0,378±0,042 cm (X/Q/T). Considerando todos os grupos, o diâmetro da bainha do nervo óptico variou de 0,287 cm a 0,512 cm (média 0,302 ± 0,039 cm). Houve diferenças estatisticamente significativas entre os grupos P e T (P > T, p = 0,003). Não foram detectadas diferenças significativas quando outros grupos foram comparados entre si. Conclusão: o diâmetro médio da bainha do nervo óptico, considerando todos os grupos, foi 0,302 ± 0,039 cm (0,287 cm - 0,512 cm) e 0,344 cm ± 0,048 cm nos indivíduos sedados apenas com X/Q Introduction: the intracranial pressure can be monitored by various methods that may be invasive or non-invasive. Ultrasonography of the optic nerve is a technique non-invasive that allows measurement of the nerve sheath and detection of possible variations in its diameter noninvasively. The optic nerve is part of the central nervous system continuously and is surrounded by a sheath. Thus, with the increase or reduction of intracranial pressure, it can be transposed to the sheath of this nerve with consequent variation of its diameter. These variations can be observed through the ultrasound image. Objective: determine the normal optical nerve sheath diameter and to evaluate the possible effects of drugs on optical nerve sheath diameter during anesthetic induction in healthy pigs with normal intracranial pressure through ultrasound image. Methods:118 Healthy hybrid piglets from (64 female) the weighing approximately 20 kg each and of similar ages. All pigs underwent general anesthesia and were duly monitored. The animals were divided into three groups according to the medications used. Group A received preanesthetic xylazine and ketamine; Grupo B received xylazine, ketamine and propofol, and Grupo C received xylazine, ketamine, and thiopental (thionembutal). Measurements in the three groups were done by the ultrasound device in triplicate of each eye from the left and right sides. Results: There were no statistically significant differences between sex and weight. The mean optical nerve sheath sizes on both sides in each group were 0.394±0.048 cm (X/K), 0.407±0.029 cm (X/K/P) and 0.378±0.042 cm (X/K/T). Considering all the groups, the diameter of the optic nerve sheath varied from 0.287-0.512 cm (mean of 0.302±0.039 cm). There were statistically significant differences between the groups P and T (P > T, p=0.003). No statistically significant differences were detected when other groups were compared each other. Conclusion: The mean diameter of the optic nerve sheath considering all groups was 0.302±0.039 cm (0.287-0.512 cm) and 0.394±0.048 cm in the subjects only sedated with X/K

INTRODUÇÃO: A autorregulação cerebral representa um dos mecanismos fisiopatológicos incertos na hemorragia intracerebral espontânea, cujo comprometimento pode influenciar no resultado prognóstico e terapêutico. O objetivo deste trabalho é avaliar a autorregulação cerebral dinâmica em modelo suíno de hemorragia intracerebral espontânea através do índice de reatividade pressórica cerebrovascular e determinar a eficácia das intervenções clínicas e cirúrgicas. MÉTODOS: Foram estudados 21 suínos híbridos machos com idade de 3 meses. O modelo experimental simulou o efeito expansivo de uma hemorragia intracerebral espontânea de grande volume quando comparado ao cérebro humano. Foram avaliados volumes de expansão diferentes, distribuídos em três grupos com sete suínos cada. O protocolo anestésico incluiu uma monitoração hemodinâmica invasiva associada a preservação da autorregulação cerebral. Os experimentos foram submetidos a monitoração neurológica multimodal e divididos em 5 fases. O índice de reatividade pressórica cerebrovascular estimou a autorregulacão cerebral durante todas as fases, sendo as três primeiras sem intervenções terapêuticas e as duas últimas para avaliar a eficácia das intervenções salina hipertônica e cirurgia. RESULTADOS: Os grupos avaliados foram homogêneos e sem diferença estatística quanto ao comprometimento da autorregulação cerebral comparando os diferentes volumes e tempos de compressão durante as duas primeiras horas da expansão do volume intracraniano. O comprometimento do índice de reatividade pressórica cerebrovascular ocorreu em alguns experimentos influenciando nas fases de tratamento subsequentes, salina hipertônica e cirurgia. CONCLUSÕES: Volumes expansivos elevados podem comprometer a autorregulação cerebral dinâmica e apresentar desfecho terapêutico desfavorável. A intervenção clínica e cirúrgica tem benefício nos experimentos com preservação do índice de reatividade pressórica cerebrovascular INTRODUCTION: Cerebral autoregulation represents one of the uncertain pathophysiological mechanisms in spontaneous intracerebral hemorrhage, whose impairment may influence prognostic and therapeutic outcome. The aim of this study was to evaluate the dynamic cerebral autoregulation in the swine model of spontaneous intracerebral hemorrhage through the cerebrovascular reactivity index and to determine the efficacy of clinical and surgical interventions. METHODS: Twenty-one male hybrid pigs aged 3 months were studied. The experimental model simulated the expansive effect of a large intracerebral hemorrhage when compared to the human brain. Different volumes were evaluated, distributed in three groups with seven pigs each. Each experiment was divided in five phases. The anesthetic protocol included invasive hemodynamic monitoring associated with the preservation of cerebral autoregulation. Multimodallity monitoring was realised in all experiments. The cerebrovascular reactivity index estimated the cerebral autoregulation during all phases. The first three phases were without therapeutic interventions, and the last two phases were with therapeutic intervention of hypertonic saline solution and neurosurgery respectively. RESULTS: The evaluated groups were homogeneous and without statistical difference regarding the impairment of the cerebral autoregulation comparing different volumes and compression times during the first two hours of the intracranial volume expansion. CONCLUSIONS: Elevated expansive volumes may compromise dynamic cerebral autoregulation and have unfavorable therapeutic outcome. Clinical and surgical intervention had benefit in the experiments with preservation of cerebrovascular reactivity index
