- Abstract
Alcohol consumption is an integral part of social and community life worldwide. Epidemiological data show a significant increase in the consumption of mainly beer, but also other alcohols over the last 20 years. Each type of alcohol represents not only an addictive toxic substance, but also an important source of plant allergens, natural enzymes, various yeast species and flavourings, preservatives and colourings, and biogenic amines Ethanol alone may be responsible for inducing passive hyperaemia of the intestinal wall, which facilitates the penetration of other allergens. Allergic reactions induced by alcohol consumption are not common, but case reports of severe allergic reactions including anaphylaxis mean that the search for the causative agent and the consequent preparation of appropriate, individualized dietary recommendations can protect the patient from the consequences of further exposure. 4. Variabilidad polínica en la Región de Murcia: impacto en la salud y la necesidad de la vigilancia aerobiológica

La aerobiología, disciplina que analiza la dinámica de los bioaerosoles, como los granos de polen, esporas y microorganismos en el aire, juega un papel fundamental en la prevención de las enfermedades alérgicas, anticipando la aparición de síntomas en personas sensibilizadas. Este artículo examina las concentraciones de polen en el aire de los tipos polínicos mayoritarios en tres ciudades de la Región de Murcia: Cartagena, Murcia y Lorca. Las muestras fueron recolectadas con un captador tipo Hirst y analizadas bajo microscopio óptico mediante la metodología aerobiológica estandarizada, validada a nivel europeo. Los resultados muestran un incremento significativo en los índices polínicos anuales de los principales tipos polínicos en 2023, comparado con el promedio del periodo 2010-2022. A pesar de la proximidad geográfica, existe una variabilidad en los taxones predominantes entre las ciudades, lo que destaca la importancia de contar con redes de vigilancia aerobiológica robustas, así como la necesidad de un marco legislativo sólido que integre a los contaminantes biológicos con otros elementos de la contaminación atmosférica, permitiendo una mejor prevención de las enfermedades alérgicas, reduciendo costes y mejorando la calidad de vida de los pacientes afectados.

El avance en la comprensión de los procesos inmunológicos relacionados con enfermedades alérgicas y en la bioingeniería de anticuerpos ha propiciado el desarrollo de terapias biológicas específicas. Los anticuerpos monoclonales, dirigidos selectivamente hacia citocinas involucradas en la patogénesis de los procesos alérgicos, o sus receptores, han emergido como una herramienta prometedora en el tratamiento de diversas afecciones, tales como el asma, la rinitis alérgica, la urticaria y la dermatitis atópica grave. Desde la aprobación del primer anticuerpo monoclonal anti-CD3 de origen murino en 1986, se ha transitado un largo recorrido, caracterizado por la aparición de anticuerpos quiméricos, los que por estructura molecular de la inmunoglobulina es semejante a los del ser humano denominados “humanizados” y los exactamente iguales llamados “completamente humanos”. La “humanización” de los anticuerpos monoclonales ha contribuido significativamente a reducir tanto el riesgo de inmunogenicidad como la incidencia de efectos adversos, mejorando notablemente la seguridad y eficacia de estas terapias. Este artículo tiene como objetivo abordar la caracterización, desarrollo, farmacocinética, farmacodinamia y utilidad clínica de los anticuerpos monoclonales, principalmente focalizados en las enfermedades alérgicas.

6. Białko sarkoplazmatyczne wiążące wapń jako przyczyna anafilaksji po spożyciu krewetek u pacjentki z zespołem pyłkowo-pokarmowym – opis przypadku.

- Abstract
Food consumption is the second most common cause of anaphylaxis among adults. There have been reported cases after eating seafood, which the most popular are shrimps. The most common allergen of shrimp is tropomyosin. A 22 years-old female experienced dyspnoea, edema of lips, tongue, eyelids and generalized urticaria with pruritus after eating bread with shrimp paste. Also, for 4 years, the patient has symptoms of rhinitis with conjunctivitis during spring and summer months and after eating unprepared apples, nectarines, hazelnuts and walnuts experiences itching of the lips, tongue and throat. The patients provided the results of specific IgE tests which showed positive results for pollen of birch, alder, rye, timothy grass, epidermis of dog, cat, horse, rabbit, egg yolk, shrimp and Alternaria alternata. During the diagnosis, skin prick tests with extracts of food and inhalant allergens were performed. A positive result was for the allergen extract from cat fur, grass mixture, rye, alder, birch, hazel and shrimp. We also established the level of IgE specific to allergen components using the ImmunoCap ISAC method. Allergen-specific IgE was elevated for the shrimp allergen Pen m 4 (sarcoplasmic calcium binding protein), allergens of timothy grass, bermuda grass, birch and cat, as well as cross-reacting allergens of alder, hazel, apple, peach, soy and peanuts. Considering the overall clinical picture the patient was diagnosed with a shrimp allergy and seasonal allergic rhinitis caused by allergy to birch and grasses. The patient was advised to exclude seafood from her diet and provided with adrenaline, steroids and antihistamines. According to our current knowledge, this is the first reported case of anaphylaxis after eating shrimp caused by allergy to Pen m 4. [ABSTRACT FROM AUTHOR]

Proton pump inhibitors (PPIs) are one of the most commonly used drug groups in clinical practice. Despite the high prevalence of their use, symptoms of hypersensitivity to IPPs are rare and few cases of the occurrence of allergies to drugs from this group have been documented in the available literature. The most often allergic symptoms include urticaria, angioedema and near fatal anaphylaxis. Between the individual drugs of this group there are cross-reactivity reactions. 11. Dupilumab en esofagitis eosinofílica refractaria al manejo convencional en pediatría: reporte de dos casos.

- Abstract
Introduction: This publication aims to show the clinical, endoscopic, and histological responses of two pediatric patients who received dupilumab as a management strategy for eosinophilic esophagitis. Dupilumab is a monoclonal antibody that inhibits the alpha chain of the interleukin (IL)-4 and IL-13 receptors involved in the Th2 inflammatory response. The potential therapeutic role of this biological drug has been demonstrated in pediatric clinical trials in other allergic pathologies, such as atopic dermatitis and asthma, with an adequate safety and effectiveness profile. Clinical cases: Two children with a personal history of atopy, allergic rhinitis, asthma, atopic dermatitis, and food allergy began with gastrointestinal symptoms that confirmed the diagnosis of eosinophilic esophagitis. Despite the different management strategies, adequate control of the disease was not achieved, and it is considered that they benefited from management with dupilumab due to the disease evolution and the coexistence of uncontrolled atopic dermatitis. Conclusions: In recent years, various management strategies in pediatrics have been published, particularly high-dose proton pump inhibitors, topical corticosteroids, and elimination diets. However, despite these strategies, at least one-third of patients may fail to achieve remission with initial treatment, making this entity a therapeutic challenge for the gastroenterologist and pediatric allergist. Our patients received the dose recommended for their weight and age and approved for asthma and atopic dermatitis, resulting in clinical and histological remission. The improvement in gastrointestinal symptoms was accompanied by better control of asthma, rhinitis, and dermatitis. None of the patients had adverse effects of the medication. [ABSTRACT FROM AUTHOR]

13. Análisis in silico del mimetismo molecular entre Der p 23 contra fuentes alergénicas

Objetivo: Identificar, a través de análisis In Silico, el posible mimetismo molecular entre Der p 23 y antígenos de fuentes alergénicas. Métodos: Se buscó identidad entre Der p 23 y proteínas de las familias de ácaros Pyroglyphidae, Acaridae, Chortoglyphidae y Echimyopodidae, a través de PSI-BLAST, y se utilizaron PRALINE y EMBOSS para los alineamientos. Los antígenos con estructura experimental resuelta se obtuvieron de Protein Data Bank, y aquellos no informados, se generaron mediante Swiss Model Server y ALPHAFOLD 2. La predicción de epítopes se realizó con el servidor Ellipro y para la visualización de los modelos en 3D, se utilizó Pymol 2.3. Resultados: El análisis entre alérgenos de Pyroglyphidae y Der p 23, mostró identidad con la proteína parecida a endoquitinasa de D. pteronyssinus, y el dominio de unión a quitina tipo 2 de D. farinae, con identidades entre 85 y 100%, con coberturas de 100% y 75%, respectivamente. Los alérgenos Der f 23 y Der p 23 de D. farinae y D. pteronyssinu,s tuvieron una cobertura del 100% con identidades del 85,42% y 79,59%, respectivamente. Entre los alérgenos de Tyrophagus putrescentiae, se incluyeron la unión a quitina, glicoproteína específica del oviducto y Cda4p, las cuales tuvieron valores de identidad correspondientes al 40%, 42,22% y 34,78%, con valores de cobertura que no superan el 55%. No se encontraron resultados para Chortoglyphidae y Echimyopodidae. Conclusión: Existe mimetismo molecular y homología estructural entre Der P 23 y alérgenos de fuentes alérgicas de las familias Pyroglyphidae y Acaridae. Se identificaron potenciales epítopes en Der p 23, los cuales podrían presentar reactividad cruzada con las proteínas de las fuentes alergénicas estudiadas, lo cual debe ser demostrado en estudios In Vitro e In Vivo. Se necesitan trabajos In Vitro e In Vivo que demuestren los resultados obtenidos en el análisis In Silico.

14. Reactividad IgE de una proteína multi-epítope recombinante diseñada a partir de alérgenos de interés en el trópico - hallazgos preliminares

Objetivo: Diseñar una proteína multiepítope a partir de alérgenos de A. lumbricoides y APD; y evaluar preliminarmente su reactividad IgE. Métodos: Mediante herramientas computacionales se diseñó In Silico, una molécula que contiene múltiples epítopos T, de alérgenos derivados de A. lumbricoides y APD. Esta proteína multiepítope (MP1) se expresó utilizando un sistema de E. coli, y se purificó mediante cromatografía de afinidad, empleando agarosa Ni-NTA. La reactividad IgE anti-MP1 y anti-extracto de APD, se evaluó mediante Dot-Blot y ELISA indirecta, a partir de suero de pacientes alérgicos a APD, e individuos no alérgicos procedentes de Barranquilla, Colombia. Los individuos alérgicos contaron con prueba cutánea positiva a una batería estandarizada de alérgenos inhalados (EUROLINE - Ref: DP 3704-1601-1 E) e IgE específica para ácaros. Resultados: La proteína multiepítope MP1 se expresó y purificó con alta pureza. El resultado del Dot-Blot, mostró que todos los sueros de pacientes alérgicos tuvieron una reactividad IgE menor a MP1 en comparación al extracto de APD. Por ELISA, se observaron concentraciones significativamente menores de IgE anti-MP1 (Mediana: 270,86 ng/ml; RIQ: 90,3), en contraste a los niveles de IgE anti-APD (Mediana: 988,5 ng/ml; RIQ: 1117,6), en suero de pacientes alérgicos a APD. Conclusiones: Se diseñó, expresó y purificó una proteína compuesta por múltiples epítopes de alérgenos de A. lumbricoides y APD. Los resultados preliminares de Dot-Blot sugieren que esta molécula muestra propiedad hipoalergénica con una reactividad IgE muy baja, en comparación con el extracto de ácaros. Se necesita continuar con estudios funcionales para comprender mejor la respuesta inmune inducida por esta molécula.

15. A case of anaphylaxis induced by intravenous hydrocortisone.

Our case focuses on a 34-year-old woman, pregnant, with a history of allergic rhinitis treated with antihistamines as needed. She had no documented history of medication allergies. At 38 weeks she went into labor. Three days after delivery, she received intravenous hydrocortisone for postpartum pain management. During administration, the patient developed generalized urticaria, dysphonia, and dyspnea. She was brought to the emergency room and received intramuscular adrenaline and antihistamines with clinical improvement. Prick tests for methylprednisolone succinate 10 mg/ml and hydrocortisone succinate 10 mg/mL and 100 mg/ml were positive. In challenge testing, the patient tolerated dexamethasone and budesonide. A diagnosis of anaphylactic reaction to group A corticosteroids was made. The patient was instructed to avoid hydrocortisone, methylprednisolone, and prednisolone. Although rare, corticosteroid allergy has very important therapeutic consequences; therefore it is necessary to be alert and to offer a safe alternative to these patients demonstrating tolerance to other corticosteroids. Objetivo: investigar o rastreamento da hipersensibilidade metálica realizada pelos profissionais e estudantes de saúde durante a prática clínico-cirúrgico. Método: estudo exploratório-descritivo realizado com 228 profissionais e estudantes da área da saúde, por meio de questionário eletrônico semiestruturado, aplicado durante abril e maio de 2021, analisado por meio da estatística descritiva e da frequência de palavras. Resultados: 11,0% dos participantes realizaram a investigação sobre a hipersensibilidade metálica. 67,1% declararam que muito provavelmente poderiam incluir essa temática durante a triagem em seus atendimentos. Falta de conhecimento sobre alergia a metais e causas multifatoriais durante a prática clínico-cirúrgica foram citadas como barreiras para a implementação do rastreamento da hipersensibilidade metálica. Conclusão: pouco se evidencia o rastreamento da hipersensibilidade metálica em pacientes durante a prática clínico-cirúrgica dos profissionais e estudantes de saúde.

17. Inmunoglobulina E en pacientes que acuden al Hospital Dr. Rafael Rodríguez Zambrano de Manta, Ecuador

La incidencia y prevalencia de las enfermedades alérgicas han aumentado en los últimos años, por lo que se han estudiado los alergenos, desde el punto de vista inmunológico, estructural y funcional. La inmunoglobulina E es fundamental en los fenómenos alérgicos donde su concentración aumenta notoriamente. Con el fin de establecer un antecedente mediante la determinación de IgE total por el método de quimioluminiscencia como indicador de enfermedad alérgica en pacientes en el Hospital Dr. Rafael Rodríguez Zambrano, se realizó un estudio de tipo observacional, descriptivo, retrospectivo y de corte transversal en 267 pacientes que acudieron a realizarse la prueba de IgE durante el año 2019, de los cuales el sexo femenino fue el más prevalente, con una relación directa entre los valores de eosinófilos e IgE con un predominio de valores iguales o menores a 190 UI/ml .Con la prueba de chi cuadrado se obtuvo una diferencia significativa estadísticamente, predominando la asociación entre los valores normales de eosinófilos y el valor de la IgE, Spearman mostró una asociación entre ambas variables, destacando una eosinofilia en el grupo de IgE entre 1000 y 1200 UL/ml. Se concluye que la inmunoglobulina E, es una prueba fehaciente para realizar un diagnóstico de alergia

The process of domestication of animals allowed for a change in the way of human life. Farm animals provided easy and relatively constant access to milk, meat, leather and wool. Farm animals are also increasingly used as pets and companions. Unfortunately, the appearance of animals in the human environment has also provided contact with new allergens. Furry animals, such as dogs and cats, are permanently settled in our homes. Many people cannot imagine life without their favorite pet. Unfortunately, the appearance of animals in the human environment is associated with exposure to numerous allergens. Allergy to fur animals is currently a relatively common form of sensitization and affects both adults and children. The possibility of diagnosing sensitization to specific proteins that are found in animal secretions and knowledge about the biological role of these allergens allows patients to give more detailed and adequate recommendations. However, the correct interpretation of sensitization to allergen components is still a challenge for the clinician. In the first part of the review the allergens (accepted by the WHO/IUIS Allergen Nomenclature Committee) of dog, cat, guinea pig, jungle hamster, Syrian hamster, house mouse and rat were characterized. Background. Allergies have become a widespread disease in all parts of the world. Allergic diseases affect nearly 30% of the population in Hungary, of which food allergies account for a significant proportion. In this paper the prevalence of hazelnut allergy was analyzed. Material and methods. In the present study, the sensitivity to hazelnut, a specific and otherwise healthy food component, was investigated comparing it with the self-reported symptoms of the participants, and Component-Resolved Diagnostic (CRD) method. In the study a total of 229 persons were included, 87 men and 142 women. Results. Molecular allergy testing showed some serum immunoglobulin E level elevation related to hazelnut in 20 participants of which 1 individual was asymptomatic but CRD positive for hazelnut. There was positivity in 90% of the cases for the component Cor a 1, which is perhaps a consequence of a cross-reaction to the homologous component Bet v 1 of birch pollen. In 10% of cases, sensitization to components of Cor a 8 was found, which may be a consequence of cross-reactivity to the lipid transfer protein (LTP) component of other foods. Conclusions. Knowledge of hazelnut sensitization may be important for people affected, as avoiding consumption of raw hazelnut may theoretically prevent the development of hazelnut allergy. [ABSTRACT FROM AUTHOR]

24. Desensibilización a deferasirox en una paciente con hemocromatosis. Reporte de un caso.

- Abstract
Background: Deferasirox is an active iron chelator, used in the treatment of iron overload such as hemochromatosis. Up to 28% may present adverse reactions to said drug. A desensitization protocol for this drug may be useful when there are no other therapeutic options. Case report: A 52-year-old female with a diagnosis of hemochromatosis who began treatment with phlebotomy, poor response and tolerance, so it was decided to treat with deferasirox 500 mg daily, presenting symptoms of urticaria and angioedema on the third dose. Hospitalization was decided for a desensitization protocol with an initial dose of 0.6mg with a gradual increase in the dose, reaching a maintenance dose of 500 mg per day on the third day. Conclusion: The rapid desensitization protocol for Deferasirox is useful when there is no response or therapeutic alternative. [ABSTRACT FROM AUTHOR]

A 53-year-old male patient with priors of psoriasis suffered a left tibial plateau fracture and underwent open reduction and internal fixation with a titanium plating system. He had no history of atopy or contact-hypersensitivity reactions to metals. Almost 1 year later, the patient continued to have chronic pain and edema at the site of the implant. On examination, the patient had a well-healed surgical incision on the left leg without erythema or induration but with tenderness to touch and two fluctuating nodular lesions. Subsequent allergy patch testing revealed an allergy-positive reaction to nickel sulfate, titanium oxalate and sodium tetrachloropalladate. The patient was diagnosed with titanium hypersensitivity secondary to recent implantation. The patient underwent hardware removal with a resolution of the complaints. The allergic risk of titanium material is smaller than that of other metal materials. Positive patch test reactions to titanium are rare and a negative patch does not exclude the diagnosis. Preimplant patients should be asked about a history of hypersensitivity reactions to metals and patch testing should be recommended for those who have experienced such reactions. 30. Associação entre dermatite atópica e mastocitoma em cães

Foi realizado um estudo para verificar se há associação entre a ocorrência de mastocitoma cutâneo e dermatite atópica em cães atendidos de 2014 à 2018, com a casuística do Hospital Veterinário da Universidade Vila Velha. Foram avaliados os sinais clínicos e exames de pacientes atendidos com diagnóstico de mastocitoma e, com base nos critérios de Favrot, que foram criados com base nas características clínicas do paciente com dermatite atópica canina para facilitar o diagnóstico pelo médico veterinário dermatologista. Dos 26 cães com mastocitoma, 15 (58%) também eram atópicos, (sendo nove (60%) fêmeas castradas e seis (40%) machos castrados). Dos demais, ou seja, 11 animais (42%), seis (55%) eram fêmeas e cinco (45%) machos. Observou-se então nesta pesquisa que os animais atópicos foram os que mais frequentemente apresentaram mastocitoma, sugerindo uma associação entre as duas doenças. Os achados obtidos neste trabalho reforçam a associação entre elas, pois a maioria dos animais com mastocitoma cutâneo também apresentava dermatite atópica canina (n=15), pois atendiam a cinco ou mais critérios de Favrot, ao contrário do grupo controle, com outras quatro neoplasias epiteliais (carcinoma de células escamosas, carcinoma de células basais, tricoblastoma e tricoepitelioma) os quais nenhum animal (n=16) apresentou dermatite atópica canina associada.

Objetivos: Confirmar la presencia de alergia a penicilina y amoxicilina mediante pruebas de exposición in vivo, en pacientes que refieren antecedente de reacción inmediata con estos medicamentos. Métodos: Estudio observacional, transversal, descriptivo y prolectivo. Se incluyeron pacientes entre 12 y 60 años con antecedente de reacción inmediata tras administración de penicilina y/o amoxicilina. Se realizaron pruebas cutáneas por prick e intradérmicas con bencilpeniciloil polilisina y penicilina G, así como desafío oral con amoxicilina. Resultados: Se incluyeron 10 pacientes femeninos y 3 masculinos. La edad promedio fue 39 años. En 84,6% de los casos la última reacción adversa a medicamentos ocurrió 10 años atrás y en todos los casos se manifestó con urticaria. Sólo en el 38,4% se corroboró alergia a penicilina. La reacción adversa más frecuente, tras las pruebas de exposición in vivo fue prurito en el 23%. Conclusiones: Los pacientes con sospecha de alergia a penicilina se deben evaluar mediante pruebas de exposición in vivo con los determinantes mayores y menores, para corroborar o descartar reacciones alérgicas y mejorar las condiciones de tratamiento.

Objetivo: Conocer la prevalencia de la APLV con la escala CoMISS en pacientes de la consulta externa de pediatria en el instituto municipal de pensiones de chihuahua. en el periodo de marzo a mayo 2022, serie de casos. Métodos: Se realizó una búsqueda sobre las consultas de APLV en el periodo de marzo a mayo 2022, se solicitó el permiso y consentimiento informado para acceder al expediente clínico y de manera retrospectiva, se realizó estudio analítico, observacional, no experimental, descriptivo, se aplicó la escala COMISS, y formulando serie de casos. Resultados: La prevalencia de APLV es de 0.3%, Los pacientes positivos APLV no tuvieron diferencias estadisticamente significativas con los sospechosos en cuanto a edad, edad gestacional, peso al nacer, edad de la madre, atopia o tabaco. Presentando una serie de casos. Conclusiones: la prevalencia de APLV con el uso de COMISS fue del 0.3%, menor a la prevalencia a nivel mundial. Se sugiere el uso más amplio de esta escala para considerar esta patología y lograr un diagnóstico más certero.

33. Asociación entre rinitis alérgica e hiposmia

Objetivo: Determinar la asociación entre la RA y la frecuencia de hiposmia. Métodos: Estudio transversal, casos y controles de carácter censal no probabilístico en sujetos entre 14 a 50 años. Se identificó RA mediante síntomas clínicos y pruebas cutáneas positivas. La función olfatoria fue evaluada con el Sniffin Sticks Screening Test. Esta investigación fue aprobada por los Comités de Ética e Investigación del Hospital Regional “Dr. Valentín Gómez Farías” del ISSSTE: ZAPOPAN ISSSTE/CEI/488/2020. Resultados: Se incluyeron 147 pacientes, 62 con RA y 85 controles. La edad promedio fue de 33 años (35 años en RA y 31 años en controles). Como medida de asociación se calculó la razón de prevalencia estimando que aquellos con RA tienen 2.2 veces más riesgo de tener hiposmia, que los controles. Asimismo, entre ellos hubo una discrepancia calculada de 12.5509 (discrepancia esperada 3.84), y una significancia de 0.00039 (p=0.00039). Conclusión: La RA representa un proceso inflamatorio sistémico crónico que condiciona a hiposmia. Su correlación se ha informado de 23 al 48%.1 A su vez, es importante considerar el papel que juega el olfato en el reconocimiento e interacción con nuestro entorno, y consecuentemente la afección a la calidad de vida de quien lo padece. La RA incrementa significativamente el riesgo de padecer hiposmia.

Los estudios inmunológicos de esta especie indican que es una fuente alergénica importante, que puede provocar sensibilización y alergia. Se han identificado diferentes componentes alergénicos de esta planta, con diversas propiedades inmunitarias y bioquímicas, y estudiado más de diez tipos diferentes de alérgenos, cada uno con distinta capacidad de inducir síntomas alérgicos. En esta revisión informamos los avances actuales en el estudio de Hevea brasiliensis.

35. Identificación de pacientes con anafilaxia en una institución de salud de tercer nivel de Medellín, Colombia (2009-2019)

Objetivo: Identificar las características clínicas y demográficas de pacientes con anafilaxia, atendidos en un hospital de tercer nivel de Medellín, Colombia. Métodos: Estudio observacional, descriptivo, de corte transversal, al que se incluyeron pacientes con diagnóstico con anafilaxia entre 2009 y 2019. La información de los pacientes se obtuvo a partir de los expedientes clínicos, mediante un instrumento de recolección. Se realizó un análisis estadístico descriptivo, de proporciones y medidas de tendencia central de las variables de interés. Resultados: Se revisaron 1820 expedientes y se incluyeron los datos de 253 pacientes. Los agentes etiológicos más frecuentes fueron: medicamentos (52.1%), alimentos (34.7%), picadura de insectos (13.8%) y agentes no especificados (17.7%). Las manifestaciones cutáneas y respiratorias fueron las más frecuentes asociadas con anafilaxia. El 94.7% de los casos tuvo concentraciones normales de triptasa. La adrenalina, los corticosteroides y antihistamínicos fueron los fármacos de elección en el 39.9, 34.3 y 39.9% de los casos, respectivamente. Conclusiones: Las características de anafilaxia coinciden con las reportadas en la mayor parte de los estudios en Latinoamérica. Aunque existen guías mundiales de tratamiento de la anafilaxia, no suelen aplicarse de forma uniforme, lo que hace necesario adiestrar al personal de salud y desarrollar guías nacionales al respecto. Objective: To characterize demographically and clinically the patients with anaphylaxis treated in a third level health institution in Medellin, Colombia. Methods: A cross-sectional descriptive observational study was carried out, which includedpatients were diagnosed with anaphylaxis between 2009 and 2019. Information was retrieved from medical records through a collection instrument. Subsequently, a descriptive statistical analysis of proportions and measures of central tendency of the variables of interest was performed. Results: A total of 1820 records were reviewed and data from 253 patients were included. Among the reported comorbidities, drug allergy was the most prevalent (28%). The most frequent manifestations of anaphylaxis were cutaneous and respiratory. Most of the cases presented basal tryptase values < 11.4 ng/mL (94.7%). Different etiological agents (food, drugs, insects and latex) were reported, and their frequency varied according to age. Adrenaline, steroids, and antihistamines were the treatments of choice in 39.9, 34.3, and 39.9% of cases, respectively. Conclusions: The characteristics of anaphylaxis in a medical center in Colombia coincide with those reported in Latin American. The treatment of anaphylaxis is not standardized, which makes it necessary to educate the health personnel and develop national guidelines. [ABSTRACT FROM AUTHOR]

Method: an exploratory-descriptive study conducted with 228 health professionals and students, through a semi-structured electronic questionnaire, applied during April and May 2021, analyzed through descriptive statistics and the frequency of words. Results: 11.0% of participants conducted research on metallic hypersensitivity. 67.1% stated that they could very likely include this theme during screening in their care. Lack of knowledge about metal allergy and multifactorial causes during clinical-surgical practice were cited as barriers to the implementation of metallic hypersensitivity screening. Conclusion: there is little evidence of metallic hypersensitivity screening in patients during the clinical-surgical practice of health professionals and students. [ABSTRACT FROM AUTHOR]

Cow's milk protein allergy (CMPA) is the most frequent cause of food allergy in the first months of life. Despite the fact that there are different guidelines and recommendations on the management of children with CMPA, there continues to be great variability in diagnostic and therapeutic criteria in Latin America. The Food Allergy Working Group of the Latin American Society for Pediatric Gastroenterology, Hepatology and Nutrition summoned a group of Latin American experts to reach a consensus and formulate a document to unify diagnostic and therapeutic criteria for CMPA. Three teams were formed, each with a coordinator, and the members of each team developed a series of statements for their corresponding module: a) clinical manifestations and diagnosis; b) diagnostic tools, and c) treatment. A search of the medical literature was carried out to support the information presented in each module and 28 statements were then selected. The statements were discussed, after which they were evaluated by all the experts, utilizing the Delphi method. Their opinions on statement agreement or disagreement were anonymously issued. The final statements selected were those with above 75% agreement and their corresponding recommendations were formulated, resulting in the document presented herein. Resumen: La alergia a las proteínas de leche de vaca (APLV) constituye la causa más frecuente de alergia alimentaria en los primeros meses de vida. A pesar de la existencia de diferentes guías y recomendaciones sobre el manejo de niños con APLV, en Latinoamérica sigue observándose una gran variabilidad de criterios diagnósticos y terapéuticos. El grupo de trabajo de Alergia Alimentaria de la Sociedad Latinoamericana de Gastroenterología, Hepatología y Nutrición Pediátrica se dio a la tarea de convocar a un grupo de expertos de la región, realizar un consenso y elaborar un documento con el objetivo de unificar criterios diagnósticos y terapéuticos para APLV. Se dividió el grupo en tres equipos bajo un coordinador para cada equipo, y los miembros de cada grupo formularon una serie de enunciados correspondientes a uno de tres módulos diferentes: a) manifestaciones clínicas; b) herramientas diagnósticas, y c) tratamiento. Se buscó la información en la literatura médica para sustentar la información de cada uno de ellos, y posteriormente se seleccionaron 28 enunciados, los cuales fueron discutidos y posteriormente evaluados por todos los expertos, a través de método Delphi, quienes emitieron su opinión sobre acuerdo o desacuerdo sobre las mismas de forma anónima. Todos los enunciados obtuvieron porcentajes de acuerdo mayores al 75%, por lo que permanecieron, y con base en ellos se elaboraron las recomendaciones y se presentan.

Se determinó el perfil de sensibilización a aeroalérgenos más comunes en la población de Portoviejo, durante el periodo noviembre 2017 - noviembre 2018. Se llevó a cabo un estudio observacional, descriptivo, de carácter retrospectivo y corte transversal, cuyos resultados se obtuvieron de la base de datos del Laboratorio Gamma de la ciudad de Portoviejo. La muestra poblacional estuvo conformada por 112 pacientes que se realizaron el test de RAST, RIDA qLine® Allergy de alérgenos inhalantes. Los resultados obtenidos muestran que el 63,4 % presentó algún tipo de sensibilización. La mayoría de estos casos se produjeron durante el verano (96 %), en total fueron 19 aeroalérgenos que produjeron dicha sensibilización, siendo los de mayor frecuencia, Dermatophagoides pteranyssinus y Dermatophagoides farinae, seguido por polen, donde se destacaron el centeno, roble y mezclas de hierbas, con animales y hongos. Se demostró que en adultos y tercera edad existe una mayor frecuencia de sensibilización al polen de centeno comparados con los ácaros domésticos, mientras que en rango de edad infantil a adulto joven se encontró un predominio de sensibilización a ácaros de la familia Dermatophagoides.

La anafilaxia es una reacción en su mayoría de hipersensibilidad tipo I, que estimula la activación generalizada de mastocitos, y provoca un cuadro clínico multisistémico que puede ser fatal. Se estima que tiene una incidencia de 0,03-0,1% y una prevalencia de vida de 0,5- 2% en la población general. Generalmente, la reacción inmunológica ocurre posterior a la ingesta de alimentos, uso de medicamentos o picaduras de insectos, pero también se han descrito mecanismos no inmunológicos (no IgE) que actúan directamente sobre los mastocitos, llamadas en la literatura “reacciones anafilactoideas”. La anafilaxia fue descrita por Paul Portier y Charles Robert Richet en 1902 en perros, los cuales desarrollaban esta reacción posterior a la inyección repetida de veneno de anémonas (medusas). Sin embargo, esta entidad no tuvo criterios diagnósticos ni pilares de manejo estructurado hasta el año 2006. En ese año en se publicó el segundo simposio de manejo de la anafilaxia, en donde se definieron criterios diagnósticos clínicos claros y el rol fundamental de la adrenalina en su manejo; la única droga que cambia el pronóstico del paciente.

Introducción: La vacuna Pfizer-BioNTech® BNT162b2 proporciona efectividad del 95% a partir de la segunda dosis. La tasa de anafilaxia reportada de vacunas para COVID-19 es de 4.7 casos por millón de dosis administradas. Reporte de caso: Paciente femenina de 30 años, profesional de la salud, con antecedentes de rinitis alérgica, asma, reacción a productos cosméticos en los párpados y al pegamento de la cinta adhesiva (eritema, edema y prurito local). Posterior a la aplicación de la primera dosis de la vacuna Pfizer-BioNTechÒ tuvo un evento de anafilaxia grado III. Se estratificó, fenotipificó y efectuaron pruebas cutáneas con PEG-3350, que resultaron positivas. Se sugirió no aplicar la aplicar vacuna de refuerzo que contuviera polietilenglicol y se ofrecieron alternativas de tratamiento. Conclusión: Es importante efectuar la adecuada estratificación de riesgo antes de aplicar las vacunas para COVID-19 basadas en ARNm por primera vez. En caso de anafilaxia es necesario fenotipificar y ampliar el estudio con pruebas cutáneas con PEG, además de otorgar alternativas de vacunación.

- Abstract
Asthma is a chronic inflammatory disease of the airways characterised by variable and/or reversible obstruction to airflow. Standard treatments include glucocorticoids (either systemic or inhaled), short-acting beta-adrenergic agonists and long-acting beta-adrenergic agonists, and monoclonal antibodies, whereby the latter is reserved for patients with severe uncontrolled asthma (SUA). Omalizumab, mepolizumab, benralizumab and reslizumab have been shown to be effective and safe in the short term for achieving control in SUA. Additionally, it has been demonstrated that good long-term control of this disease can be achieved with omalizumab, with the drug being well-tolerated. Furthermore, dupilumab has also been incorporated to the list of available agents. Other biologics that have been investigated but which have yielded inconclusive results include lebrikizumab, tralokinumab, tezepelumab, fevipiprant and timapiprant. In the light of imminent innovations in treatment and the significant impact these are having, it is essential to consolidate our pathophysiological knowledge of SUA as well as the main drugs currently available, in addition to drugs in the pipeline. - Abstract
La esofagitis eosinofílica es una enfermedad inmunoalérgica que afecta al esófago de manera crónica, causando disfunción esofágica con síntomas como vómitos, disfagia o impactación alimentaria. Desde el primer caso diagnosticado en 1970, su prevalencia ha aumentado exponencialmente, sobre todo en aquellos países de altos ingresos. Su diagnóstico será tanto clínico (positividad en las pruebas de hipersensibilidad), como histológico (presencia de >15 eosinófilos/hpf en la biopsia de mucosa esofágica), en ausencia de otras causas de eosinofilia esofágica. La inmunoterapia específica con alérgeno ha demostrado ser efectiva tanto en niños como adultos con EoE, resultando en la mejora del 92.16% de los pacientes, cifra que asciende a 94.1% cuando esta terapia se combina con dieta de eliminación., Grado en Medicina

La esofagitis eosinofílica es una enfermedad que consiste en la disfunción del esófago debido a la infiltración de eosinófilos en la mucosa, lo que produce episodios de impactación y disfagia. La prevalencia de esta enfermedad está aumentando, y los tratamientos existentes son escasos y no son muy efectivos, con una tasa de adherencia muy baja en el caso de aplicar las dietas de eliminación y con efectos secundarios significativos en el caso del uso de corticoides. La esofagitis eosinofílica tiene un componente alérgico que hace que se produzcan exacerbaciones en ciertas épocas del año, relacionadas con el aumento del polen, hecho demostrado histológicamente con el infiltrado de alérgenos polínicos en la mucosa esofágica. Este estudio en la vida real demuestra que la dieta de evitación dirigida y la inmunoterapia alérgica de vacunas con extracto de polen, basadas en el diagnóstico mediante IgE específica, diagnóstico molecular y Prick test, son tratamientos que aseguran la remisión de la enfermedad en un alto porcentaje de los casos seleccionados para el presente estudio., Grado en Medicina

Self-reported penicillin allergy is highly prevalent. Different studies estimate that 10% of the population is labeled as such. This label, confirmed or suspected, forces us to take precautions and replace the antibiotic treatment of choice (frequently beta-lactams) with other 2nd or 3rd choice alternatives with worse overall results: side effects, resistance, costs, etc. The penicillin allergy label, once placed, remains in the medical record. It is only confirmed in less than 5% of patients, either because it has been placed inappropriately or because over time the sensitivity decreases and may disappear. Penicillin Allergy Decision Rule -PEN-FAST- is a validated and simple clinical prediction rule that estimates the risk of presenting an allergic reaction. Nowadays, the incidence of food allergies is increasing. While strict allergen avoidance remains the most important therapeutic approach, oral immunotherapy is increasingly being used to desensitize and induce tolerance in children. Oral immunotherapy is a possible treatment for food allergies, which includes the administration of gradually increasing doses of the allergen. While most children can be desensitized, or even gain sustained unresponsiveness, oral immunotherapy involves a high risk of side effects or allergic responses with a need to use epinephrine, and the long-term effectiveness is unknown. In this paper, we summarize recent clinical trials in which oral immunotherapy has been used to treat common food allergies: to cow's milk, peanuts, and chicken eggs. [ABSTRACT FROM AUTHOR]
