8. The pie‐crusting release of the medial collateral ligament in arthroscopic partial meniscectomy is associated with improved clinical outcomes without altering radiological measurements.

Abstract

Purpose: The aim of this study was to evaluate how the pie‐crusting technique affects clinical and radiological outcomes in patients undergoing arthroscopic partial meniscectomy. Methods: A total of 68 patients with Kellgren–Lawrence (K‐L) grade 2 who underwent arthroscopic partial meniscectomy between 2015 and 2021 were evaluated and divided into two groups as arthroscopic partial meniscectomy (36 patients) and arthroscopic partial meniscectomy with pie‐crusting (32 patients) according to whether the pie‐crusting technique was applied or not. All patients were evaluated at a minimum 2‐year follow‐up in terms of Lysholm score, Tegner activity score, International Knee Documentation Committee (IKDC) score and Visual Analogue Scale (VAS) score. To assess the impact of the pie‐crusting technique, radiological measurements were conducted using radiographs taken before and after pie‐crusting, as well as postoperative radiographs. Results: Lysholm, Tegner, IKDC and VAS scores exhibited statistically significant differences after surgery compared to preoperative evaluations in both groups (p < 0.05). Furthermore, these scores were significantly superior in the arthroscopic partial meniscectomy with pie‐crusting group compared to the arthroscopic partial meniscectomy group at 24 months postoperatively (p < 0.05). While the radiological measurements in the arthroscopic partial meniscectomy with pie‐crusting group showed statistically significant differences before and after pie‐crusting (p < 0.05), no significant difference was observed between before pie‐crusting and 12 and 24 months postoperatively (n.s.). Conclusion: The current study is the first to demonstrate the true effectiveness of the pie‐crusting technique. The application of the pie‐crusting technique when necessary results in a statistically significant improvement in clinical scores without affecting radiological measurements for patients undergoing arthroscopic partial meniscectomy compared to not utilising it. 15. Talon cannulated compression device as an alternative in the fixation of acetabulum posterior column fractures: A biomechanical study

Abstract

To compare the amount of displacement and rigidity at the fracture line under static ; dynamic axial loading and torsional stress of conventional cannulated screw (CS), plate screw fixation including inter-fragmentary screw (PL), and talon cannulated compression device or talon screw in other words (TS) in posterior column fracture models. Synthetic hemipelvis bone models presenting a posterior column fracture were used in this study. Group PL, CS, and TS were created with ten bone models prepared for each group for dynamic and static loading tests and another ten for torsional tests. Rigidity and displacement amounts before and after loading were measured at the reference points AL, BL, and CL, located at the acetabulum's top, middle, and bottom, respectively. Torsional tests for each group were used to calculate torsional rigidity and maximum torque values. In dynamic axial loading tests, Group CS showed more displacement than PL at the BL point (p = 0,032) and Group TS at AL (p = 0,032) and CL (p = 0,004) points. In static axial loading tests, Group CS significantly displaced more than TS at AL and CL points (p = 0,05 and p = 0,014, respectively). Group PL and Group TS exhibited similar behavior in dynamic, static axial loading tests and torsional rigidity. The maximum torque that Group PL could withstand was statistically significantly higher than the other two groups (p 0,001). Talon cannulated screws had promising results in posterior column fractures of the acetabulum, which may decline the need for open surgery for stable fixation.

32. Geriatrik kalça kırıklarında talon proksimal femoral çivileme (pfn) proksimal femoral çivi-antirotasyon (PFN-A) kadar başarılı mı?

Abstract

Amaç: PFN-A kullanılarak tedavi edilen hastaların fonksiyonel ve radyolojik sonuçlarını ve radyasyon maruziyetini Talon kilitli PFN ile karşılaştırdık. Yöntem: Çalışmaya 2014-2018 tarihleri arasında proksimal femur kırığı tanısı ile PFN yapılan 60 yaşından büyük 92 hasta (43 kadın,49 erkek; ort. yaş 75.01 yıl) dahil edildi. Retrospektif çalışmamızda hastalar Talon kilitli PFN grubu (grup 1, n=46) ve PFN-A grubu (grup 2, n=46) olmak üzere iki gruba ayrıldı. Hastaların demografik verileri, kırık tipleri, takip süreleri, kırık kaynama zamanları, radyografik bulguları, WOMAC skorları ile C-kollu skopi çekim sayıları değerlendirmeye alındı. Bulgular: Hastaların ortalama takip süreleri 11.05 aydı. Mortalite Grup I de %4.3, Grup II de %6.5 olarak bulundu. Kollodiafizer açı Grup I de 129.5±3.4, Grup II de 126.8±3.6 (p=0.01); Singh indeksi Grup I de 4.1, Grup II de 3.62 (p=0.06); kaynama Grup I de 4.62±1.06, Grup II de 5.1±1.51(p=0.68) olarak gözlendi. Abstract

Gİriş; Pes planus (düztabanlık) genel olarak ayağa yük verirkenarka ayağın valgusu, orta ayakta medial longitudinal arkın (MLA) kaybolması veön ayağın arka ayağa göre supinasyonu olarak tanımlanır [1-5].Pesplanus tanımlanması için kesin klinik veya radyografik kriterler konusundafikir birliği yoktur[9]. Bunedenle pes planusun gerçek prevalansı bilinmemektedir.Materyal Metot; SBÜ Ankara SağlıkAraştırma ve Uygulama Merkezi Etik Kurul Komisyonu onayı alınarak yapılmıştır.Kliniğimizde 2018 yılında askeri muayene amacıyla kontrollere gelen 18-22 yaş arası 5000 erkek hasta çalışmayaalınarak retrospektif olarak değerlendirildi.Tüm hastalar klinik olarak muayeneedildikten sonra her iki ayak basarak çekilen Anteroposterior ve lateral ayakgrafileri değerlendirmeye alınarak ‘Kalkaneal pitch ve Meary açıları’ ölçüldü.Klinik ve her iki radyografik test ile bilateral pes planusu olan hastalartespit edildi. Tüm hastaların yaş, boy, kilo, BMI ve radyografik ölçümlerikayıt altına alındı.Sonuç;Çalışmaya alınan 5000 olgunun tamamıerkek hastaydı. 38. Biomechanical comparison of three different fixation techniques for anterior column posterior hemitransverse acetabular fractures using anterior intrapelvic approach

Abstract

Objectives: The aim of this study was to biomechanically compare three different fixation methods for the fixation of anterior column posterior hemitransverse type acetabular fracture (ACPHT). Methods: ACPHT fracture models were created on twenty-four foam cortical shell artificial hemipelvis models. Three fixation methods were assessed - Group 1: posterior column lag screws over a suprapectineal plate; Group 2: Posterior column lag screws over a suprapectineal plate and an infrapectineal plate; Group 3: A suprapectineal plate and a vertical infrapectineal plate. Stiffness and displacement amounts of fixation methods under dynamic and static axial loading conditions were measured. Results: In the dynamic and static tests, Group 3 showed the less stable fixation compared to Group 2. There was no statistically significant difference between the stiffness values of the fixation groups. In the static tests, there was no statistically significant difference between Group 1 and 2, although less displacements were obtained in Group 2. Conclusions: A combination of posterior lag screws over a suprapectineal plate and an infrapectineal plate supporting the pelvic brim along both sides of the linea terminalis resulted in a better fixation construct than a suprapectineal plate accompanying with a vertical infrapectineal plate provide better stability with less fracture displacement., Turkish Society of Orthopaedics and Traumatology; Turkish Orthopaedics Research Council

39. SURGICAL MANAGEMENT STRATEGIES IN GARTLAND TYPE 3 SUPRACONDYLAR HUMERAL FRACTURES

Abstract

GİRİŞÇocuklarda humerus altuç kırıkları ikinci en sık görülen kırıklardır.Gartland tip 3 kırıklarının tedavisinde tam bir fikir birliği yoktur. Bu çalışmanınamacı, suprakondiler humerus kırıklarında cerrahi yönetimstratejisi belirlemektir. HASTALAR ve YÖNTEM:Ocak 2015 - Aralık 2016tarihleri arasında kliniğimizde opere edilen humerus suprakondiler Gartland tip III kırıklı53 hasta çalışmaya dahil edildi. Çalışmada hastalarındemografik verileri, kırığın oluş nedenleri, kırık tipleri, komplikasyonlar,radyolojik parametreler, kozmetik ve fonksiyonel sonuçlar değerlendirildi.BULGULAR: Hastaların 30’ u erkek23’si kadındı. Yaş ortalaması 6,5 olarak bulundu. Hastalar ortalama 4-24 aytakip edildi. Ortalama yatış süresi 3,15 gün olarak gözlendi. 3 hastadapseudovarus gözlendi.Flynn kriterlerine göre açık redüksiyon yapılmış hastalarile kapalı redüksiyon yapılmış hastalar arasında fonksiyonel ve yatış süresiaçısından anlamlı fark bulunamazken kozmetik olarak kapalı redüksiyon tedavisiaçık redüksiyona göre daha başarılı olarak gözlendi. Çapraz K teliuygulanan hastalar ile izole lateral K teli uygulananhastalar arasında da fonksiyonel ve yatış süresi açısından istatistikselanlamlı fark bulunamadı. Açık veya kapalı redüksiyon K teli ile fiksasyonyapılmış hastalarda ilk 6 saatte opere edilenler ile 12.saatten sonra opere edilenlerarasında yatış süreleri, flynn fonksiyonel sonuçları açısından anlamlı farkgözlenmedi.SONUÇ Kozmetik olarak daha avantajlı olması,komplikasyon oranlarının açık redüksiyona göre anlamlı olmaması nedeniyle kapalıredüksiyonun önceliklitercih edilmesi kanaatindeyiz. K teli konfigürasyonları arasında fonksiyonelolarak fark olmaması nedeniyle 2 adet lateral K teli ile redüksiyonunkalitesinin ve fiksasyonun stabilitesinin yeterli olduğunu düşündüğümüzvakalarda medialden K teli uygulaması gerekli değildir. Cerrahi riski yüksekhastalar içinen uygun cerrahi zaman beklenebilir., OBJECTIVE: Humeral lower extremity fractures arethe second most common fractures in children. Thereis no consensus on the treatment of Gartland type 3 fractures. The purpose of this study is to determine thesurgical management strategy for supracondylar humerus fractures. MATERIAL AND METHODS: Between January 2015 andDecember 2016, 53 patients with humeral supracondylar Gartland type IIIfractures were included in our study. Demographicdata, causes of fractures, fracture types, complications, radiologicalparameters, cosmetic and functional outcomes were evaluated in the study. RESULTS: 30 of thepatients were male and 23 were female. The mean age of patients were 6,5 years. The meanlength of hospital stay was 3,15 days. The averagefollow up was 4-24 months. Pseudovarus were observed in 3 patients. Cosmeticallyclosed reduction treatment was more successful than open reduction, althoughthere was no significant difference in functional and length of stay betweenopen reduction patients and closed reduction patients according to Flynncriteria. There was no statistically significant difference between patients who weretreated with crossed K wire and those who were treated with isolated lateral Kwire in terms of functional and length of stay. Patients who were K wire fixedwith open or closed reduction did not differ significantly in terms ofhospitalization time, flynn functional results in the first 6 hours and thosewho were oppressed after 12 hours. CONCLUSION: We believe that closed reduction is thepreferred choice because of the cosmetic advantage that complication rates arenot significant compared to open reduction. Sincethere is no functional difference between the two configurations, medial Kwires is not necessary when we think that the reduction quality and fixationstability with 2 lateral K wires are sufficient. Forpatients who are at high risk for surgery, optimal surgery time may beexpected.

Abstract

Background/aim: The aim of this study is to compare the stability and implant stresses of suprapectineal plate with infrapectineal plate in three subconfigurations of the screw types. Materials and methods: The stabilities of different fixation methods were compared by finite element analysis on six models. Three infrapectineal and three suprapectineal models each with locked, unlocked, or combined screws were employed. Three-dimensional finite element stress analysis was performed by using isotropic materials with a load of 2.3 kN applied at standing positions. Motion at the fracture line was measured on four different points located on the pubic and iliac sides of the fracture line. Results: Infrapectineal plate fixation with unlocked screws was found to be the most stable fixation method with 0.006 mm displacement of fragments in all axes at standing positions. The suprapectineal unlocked method was found to be the most unstable in standing positions with maximum displacement values of 0.46 mm vertical shear movement in the x-axis, -0.14 mm displacement in the y-axis, and -0.33 mm lateral shear in the z-axis. Conclusion: The infrapectineal unlocked plate supplies the most stable fixation with the least implant stress, contrary to the suprapectineal unlocked plate, which has the lowest stability and highest implant stresses. Abstract

Background/aim: This study aimed to generate data for the S1 and S2 alar pedicle and body and the alar orientations for both dysmorphic and normal sacra. Materials and methods: The study comprised two groups: Group N consisted of 53 normal sacra and Group D included 10 dysmorphic sacra. Various features such as alar pedicle circumference; anterior, middle, and posterior axis of the sacral ala; sacral body height and width; and sagittal thickness were measured. Results: In group N, the median anterior axis of the alae was observed to be 30° on the right and 25° on the left, the median midline axis was found to be 20° on the right and 15° on the left, and the median posterior alar axis was -15° on the right and -20° on the left. The true S1 and S2 alar pedicle circumferences were observed to be significantly smaller in group D, which demonstrated a shorter S1 alar pedicle mean circumference, significantly narrower S1 body mean width, and considerably tapered sagittal thickness. Conclusion: Our analysis indicated that dysmorphic sacra have a lower sagittal thickness and width of bodies and smaller alar pedicles, which explains the difficulties in their percutaneous fixation.
