Background: Patients with oncological diseases often have mental disorders in the form of comorbidity. The aim of this study was to research the association of cancer with the presence of symptoms of depression and anxiety in primary health care patients. Methods: This prospective observational study done in 2020 included adult users of health care at the Health Center Trstenik, Central Serbia, aged 19 and over, both sexes, with a diagnosis of oncological disease. A research instrument to assess depressive symptoms is used PHQ-9 (The Patient Health Questionnaire) questionnaire, derived from PRIME MD- and (The Primary Care Evaluation of Mental Disorders), and the Beck Anxiety Scale (BAI) to register the presence of certain anxiety symptoms. Results: The largest percentage of patients had symptoms of mild (27.2%) or moderate depression (22%), while 18% reported symptoms of major depression. The level of depression was higher in older subjects, in the presence of chronic diseases with greater limitations of activity and the presence of difficulties in performing daily activities, with a more pronounced effect of pain on activity, the presence of stress. All subjects were characterized as persons with severe anxiety (score 26-63). The level of anxiety was higher in older respondents, in the presence of long-term illness, with greater limitations of activities and difficulties in performing daily activities, with a more pronounced influence of pain on performing activities and the influence of the media. Conclusion: Caring for the mental health of cancer patients must occupy a significant part of each country's national health policy.

Cognitive impairment frequently occurs in epilepsy patients. Patients with drug-resistant epilepsy (DRE) have poor drug responsivity and higher seizure frequency which consequently lead to brain damage and may have implications on cognitive status. In the present study, we assessed a frequency and degree of cognitive impairment in 52 patients with drug-sensitive epilepsy (DSE) and 103 DRE patients at three time points (baseline, after 12 and 18 months). Degree of cognitive decline was assessed with Montreal Cognitive Assessment (MoCA) scale. We examined the possible correlation between demographic and clinical characteristics and cognitive deterioration in epilepsy patients. Patients in the DRE group had significantly lower MoCA score than patients in the DSE group at baseline (28.83 ± 2.05 vs. 29.69 ± 0.61, p = 0.003), after 12 months (27.36 ± 2.40 vs. 29.58 ± 1.22, p = 0.000) and 18 months (26.86 ± 2.73 vs. 29.33 ± 1.47, p = 0.000). Patients with DRF epilepsy had significantly lower MoCA score than patients with DSF epilepsy at three time points (28.71 ± 2.48 vs. 29.86 ± 0.35, p = 0.015; 27.22 ± 2.72 vs. 29.52 ± 1.37, p = 0.000; 26.80 ± 2.99 vs. 29.31 ± 1.56, p = 0.000). After 12 and 18 months of follow-up, patients with DRG epilepsy had significantly lower MoCA score than patients with DSG epilepsy (27.52 ± 2.01 vs. 29.65 ± 1.02, p = 0.000; 26.94 ± 2.43 vs. 29.35 ± 1.40, p = 0.000). Acute liver failure (ALF) is a rare but life-threatening illness with multiple organ failure. The short-term mortality rate exceeded 80 % despite modern approaches in treatment. Drugs, infections by hepatic viruses and toxins are the most common causes of ALF. Progressive jaundice, coagulation disorder and hepatic encephalopathy are dominated as a clinical signs of the illness. We present a case of a 36-year-old Caucasian woman hospitalized in ICU due to yellow discoloration of the skin and sclera, severe disseminated coagulopathy and hemodynamic instability. ALF is developed due to Hepatitis B Virus infection, resulting in hepatic toxicity as well as coma. General condition rapidly improved after applying of Molecular Adsorbent Recirculating System (MARS), an extracorporeal liver support system based on albumin dialysis. It is relatively expensive treatment that is used for the patient with hepatic encephalopathy grade 3 or 4 in our institution. In conclusion, an early administration of MARS significantly reveals subjective and objective clinical improvement in the case we presented.

Thrombosis of veins and venous sinus (CVT) is the rare cerebral vascular disorder which makes less than 1% of all strokes. Thrombosis of veins and venous sinuses is picturesquely called “мајоr neurological forger” since it is characterized by very varied clinical picture. Among the various causes of CVT, which can be of infective or non-infective nature, the congenital hyper coagulations especially stand out, diagnosis is based on highly sophisticated diagnostic tests. We present the case of a female patient, 36 years old, who was hospitalized at the Clinic for Neurology in Clinical Center because of the diffuse headache she had for the last few days, with milder right-sided hemiparesis and one generalized tonic-clonic epileptic seizure. With nuclear magnetic resonance (MR/2D venography) the thrombosis of the upper and lower sagittal sinuses is confirmed. By appropriate laboratory tests, as well as by confirmatory immunological and genetic analyses, the impact of the most of the factors is excluded which can contribute to the occurrence of venous thrombosis. The only pathological findings which indicated the possible congenital thrombophilia as the cause of the sagittal sinus thrombosis was the determination of the specific polymorphism of the 4G/5G gene for plasminogen activator inhibitor 1. According to our knowledge, this is the first decribed case of the possible impact of the specific polymorphism of the 4G/5G gene for plasminogen activator inhibitor of 1 on the development of cerebral venous thrombosis.

Background Cerebral small vessel disease (CSVD) and metabolic syndrome were separately associated with cognitive impairment and depression. However, whether metabolic syndrome adds to cognitive impairment and depression in patients who already have CSVD remained unanswered. Objective The aim of our study was to investigate the association of metabolic syndrome with cognitive impairment and depression in patients with CSVD who have lacunar lesions or white matter hyperintensities. Methods This prospective cohort study was conducted at Neurology Clinic, Clinical Center, Kragujevac, Serbia. Main outcomes of the study were cognitive assessment, and assessment of depression among hospitalized patients with or without CSVD. Results The study included 74 inpatients, 25 of them having lacunary infarctions, 24 with the white matter hyperintensities, and 25 control patients without CSVD. The CSVD was accompanied by impairment of cognition and depression, the patients with lacunary lesions being more cognitively impaired and more depressive than the patients with the white matter hyperintensities. The patients with CSVD who also had metabolic syndrome were more cognitively impaired and depressed than the patients with CSVD alone. Conclusions In conclusion, our study showed that metabolic syndrome is associated with further worsening of already impaired cognition and existing depression in patients with CSVD.

The aim of the study is to examine whether EEG abnormalities in patients with encephalitis might be prognostic and diagnostic factors for final epilepsy outcome and/or be correlated with the severity of the disability. The most frequent causes of encephalitis were HSV, WNV, INF V, MTB, St PN, St AU. There was a highly statistically significant positive correlation between the severity of the EEG abnormalities at the beginning of the disease (r = 0.410, p Electroencephalography in the early stages of encephalitis shows diagnostic and prognostic significance and, in combination with the overall severity of the clinical picture, could contribute to the diagnosis and assessment of outcomes and, ultimately, the correction of treatment and faster recovery of patients. This is particularly true for viral encephalitis.

Pulsatility index (PI) values in a fetal middle cerebral artery (MCA) were compared in no-risk pregnancies to examine the differences related to auditory stimulation test and pregnancy order. The study included 196 women with no-risk pregnancies selected from the database of more than 1000 pregnant women divided into two groups. Group 1 consisted of 98 nulliparous women (C1 = 98) and Group 2 consisted of 98 parous women (C2 = 98). All pregnant women were of comparable age and fetal gestational age (GA) when MCA-PI values were recorded. Measurements of PI values in fetal MCA were obtained before and immediately after the application of fetal auditory stimulation test. The MCA-PI measuring was conducted in the period between the 36th and the 41st week of GA. The results showed that PI baseline values and PI values after defined auditory stimulation were significantly different when measured in nulliparous women compared to parous women (p = 0.001, p = 0.003, respectively), while no group differences were observed in relative PI value changes due to auditory stimulation. These findings suggest that hemodynamic changes in fetal MCA caused by defined auditory stimulation measured by PI value changes may be valuable in the assessment of fetal auditory perception functionality and its development.

Optic neuritis represents an inflammatory lesion of the optic nerve, which could cause visual loss. It can be a disease isolated to the optic nerve or can be associated with a more widespread demyelinating disorder of the central nervous system, such as multiple sclerosis or neuromyelitis optica (Devic's syndrome). Optic neuritis is one of the strongest predictor for developing clinically definite multiple sclerosis. Visually evoked potentials are useful in the detection of clinical and subclinical optic neuritis and they are the noninvasive way of monitoring of the involvement of the visual system during the clinical course of multiple sclerosis. Despite some limitations, e.g. inability to localize lesions, analysis of the visually evoked potentials includes two main pathological findings, useful for diagnosis: the decrease in amplitudes, which indicates the extent of axonal damage, and the delayed latency, which is highly sensitive to focal demyelination of the optic nerve fibers. After an episode of optic neuritis, patients show significant improvement in latency p100, but it is unclear whether this improvement is caused by remyelination, by synaptic plasticity or some other mechanism.

Background: The aim of this study was to estimate the prevalence of multiple sclerosis (MS) in the district of Sumadija (central part of Serbia). Methods: All persons suffering from MS, with permanent residence in the region, were recruited. Prevalence was calculated on December 31, 2006, according to a standard procedure. Results: On December 31, 2006, one hundred ninety-four patients (72 males and 122 females) were found to have MS. We found a crude MS prevalence of 64.9/100,000 (49.3/100,000 for males and 79.9/ 100,000 for females). The highest prevalence rates were registered in the age group of 30–39 years for females and 40–49 years for males, although the age-specific rates were higher in females in all age groups. The mean age at onset was 34.2 ± 9.3 years. The average duration of MS was 8.9 ± 6.2 years. The median EDSS score was 3.9 ± 2.0. The course of MS was relapsing-remitting in 63.9% of patients, secondary progressive in 32.5%, and primary progressive in 3.6%. At MS onset, motor symptoms were present in 70.1% of patients, sensory in 40.7%, cerebellar in 21.1%, brainstem in 15.5%, visual in 22.2%, and bowel/bladder disturbances in 22.2%. Conclusions: Our results demonstrate that the MS prevalence registered in the Serbian district Sumadija is similar to that of other surrounding areas in Southeastern Europe.

We reported a case of a 22-year-old male patient with swallowing difficulties and double vision. He was diagnosed on the basis of a positive pharmacological test, damage to the postsynaptic neuromuscular junction and computed tomography of the thorax. Tests for antibodies targeted to acetylcholine receptors and to muscle-specific tyrosine kinase were negative. A partial improvement in neurological findings following treatment with pyridostigmine bromide, cyclosporine and methylprednisolone was notedadministered. Th e patient responded favourably to plasma exchange with a withdrawal of all clinical symptoms, confirming the hypothesis that humoral factors may underlie the pathogenesis of double-negative MG .

Miller Fisher syndrome (MFS) is an acute demyelinating polyneuropathy characterized by ophthalmoplegia, ataxia and areflexia. MFS is a variant of Guillain-Barre syndrome (GBS). MFS is a rare neurological disease, which comprises 5% of patients with GBS. In our work we presented 24-year-old male patient whose clinical picture was characterized by ophthalmoplegia, facial nerve diplegia, bulbar paresis, ataxia and areflexia. Albumino-cytological dissociation was determined by lumbar puncture. The patient was treated with immunomodulatory therapy in the form of intravenous immunoglobulin, followed by long-term physical therapy. One year after the onset of symptoms, there was a complete recovery of the patient. MFS syndrome is a rare disease for which there is no specific therapy and medications used are plasmapheresis and intravenous immunoglobulin. The prognosis for recovery is good, and most patients recover fully for 2-3 months, and sequelae (usually ataxia) are rare.
