1. Decolonising Data Systems: Using Jyutping or Pinyin as tonal representations of Chinese names for data linkage

Abstract

Data linkage is increasingly used in health research and policy making and is relied on for understanding health inequalities. However, linked data is only as useful as the underlying data quality, and differential linkage rates may induce selection bias in the linked data. A mechanism that selectively compromises data quality is name romanisation. Converting text of a different writing system into Latin based writing, or romanisation, has long been the standard process of representing names in character-based writing systems such as Chinese, Vietnamese, and other languages such as Swahili. Unstandardised romanisation of Chinese characters, due in part to problems of preserving the correct name orders the lack of proper phonetic representation of a tonal language, has resulted in poor linkage rates for Chinese immigrants. This opinion piece aims to suggests that the use of standardised romanisation systems for Cantonese (Jyutping) or Mandarin (Pinyin) Chinese, which incorporate tonal information, could improve linkage rates and accuracy for individuals with Chinese names. We used 771 Chinese and English names scraped from openly available sources, and compared the utility of Jyutping, Pinyin and the Hong Kong Government Romanisation system (HKG-romanisation) for representing Chinese names. We demonstrate that both Jyutping and Pinyin result in fewer errors compared with the HKG-romanisation system. We suggest that collecting and preserving people's names in their original writing systems is ethically and socially pertinent. This may inform development of language-specific pre-processing and linkage paradigms that result in more inclusive research data which better represents the targeted populations.

2. School Climate: Using a Person-Environment Fit Perspective to Inform School Improvement

Abstract

Strong and consistent findings suggest that a positive school climate is related to improved student outcomes. However, assessment of the school climate rarely considers the environmental fit (or misfit) between individuals' actual or lived experiences and their preferred environment. This study drew on a person-environment fit perspective to examine whether: students' experiences of the school climate (actual environment) differed from their views of their ideal school climate (preferred environment); the views of the actual and preferred environment differed between schools; and the actual-preferred discrepancy (as a measure of the environmental fit) was related to student wellbeing, resilience and reports of bullying. The results from the analysis of data collected from 993 upper primary school students suggest that outcomes were enhanced when the perceived environment more closely matched the preferred environment. Our study's findings support using a person-environment fit perspective alongside a socio-ecological approach to inform strategic decisions for school improvement efforts.

3. Is Parent Education a Factor in Identifying Autism/Takiwatanga in an Ethnic Cohort of Pacific Children in Aotearoa, New Zealand? A National Cross-Sectional Study Using Linked Administrative Data

Abstract

Studies have found early detection of autism is protective for an individual and their family. Fewer Pacific children are diagnosed with autism compared with non-Pacific children, and those who have significantly higher needs, suggesting an undercount of those diagnosed. The purpose of this study was to explore any association between parental education and autism in Pacific children living in Aotearoa, New Zealand. This was a national cross-sectional study using data from New Zealand's Integrated Data Infrastructure, including Pacific and non-Maori non-Pacific children in the 2013 Census. A novel method was employed to capture autism, while years of parents' education was estimated from their highest attained qualification level. Apposite regression models were used to analyse the relationship between parental education and autism. We found that autism was identified in 1.1% of Pacific children compared with 1.6% of non-Maori, non-Pacific children. A year of education among parents of Pacific children was associated with increased identification of autism (odds ratio = 1.10; 95% confidence interval = 1.06-1.15). The findings show education plays a positive role in receiving a diagnosis for Pacific autistic children but suggest a systemic failure to support Pacific parents to navigate existing health systems for themselves or their children.

The proposed methodology is well suited for complex large-dimensional problems like housing choice. The methodology retains all the properties of TTC, namely, Pareto-efficiency, individual rationality and strategy-proofness.

Abstract

Simulated learning programs are an important component of allied health education. Although the role of simulation clinical educators has been highlighted as critical for student learning within simulation, their perceptions of their role have not yet been investigated. This study aimed to explore the experiences of simulation clinical educators. Participants were ten simulation clinical educators who had supported speech-language pathology students' learning during a 5-day simulation program focussed on speech-language pathology practice with adult clients in acute hospital and rehabilitation settings. Educators participated in individual semi-structured interviews exploring their role and their perceptions of the simulation-based learning program. Data were analysed using inductive thematic analysis. Three inter-related themes emerged from participants' views. The major theme of Unique teaching and learning environment incorporated five subthemes: focus on teaching; safe learning environment; authenticity and engagement; structure and intensity of learning, and; feedback opportunities. Two additional themes were identified: Clinical educator role same but different, and Simulation bridges the gap between theory and practice. This study offers new insights into simulation clinical educators' perceptions of their role when supporting students within simulation and highlight the importance of harnessing the unique benefits of simulation as a teaching pedagogy to maximize its impacts on student learning and justify its costs.

9. Veteran Teachers' Resistance Factors to Technology Usage in the Face of Change in the Secondary Classroom in Northwest Alabama

Abstract

Technology has inserted itself into everyday life. This includes the classroom and instructional practices; however, veteran teachers face unique circumstances with technology. Veteran teachers are looking for leadership in the face of current technological changes. Using a descriptive qualitative approach, we examined the current use of technology in veteran teachers' classrooms in rural Northwest Alabama. Forty veteran teachers answered an online survey about their perceptions of technology in daily instruction, and 12 were selected via purposeful sampling to complete an interview to expound on the survey responses. Findings revealed that these veteran teachers wanted more administrative support, copious yet practical professional development sessions, and removing as many barriers as possible.

18. Small error algorithms for tropical group testing

Abstract

We consider a version of the classical group testing problem motivated by PCR testing for COVID-19. In the so-called tropical group testing model, the outcome of a test is the lowest cycle threshold (Ct) level of the individuals pooled within it, rather than a simple binary indicator variable. We introduce the tropical counterparts of three classical non-adaptive algorithms (COMP, DD and SCOMP), and analyse their behaviour through both simulations and bounds on error probabilities. By comparing the results of the tropical and classical algorithms, we gain insight into the extra information provided by learning the outcomes (Ct levels) of the tests. We show that in a limiting regime the tropical COMP algorithm requires as many tests as its classical counterpart, but that for sufficiently dense problems tropical DD can recover more information with fewer tests, and can be viewed as essentially optimal in certain regimes.

Abstract

Single-cell analyses parse the brains billions of neurons into thousands of cell-type clusters residing in different brain structures1. Many cell types mediate their functions through targeted long-distance projections allowing interactions between specific cell types. Here we used epi-retro-seq2 to link single-cell epigenomes and cell types to long-distance projections for 33,034 neurons dissected from 32 different regions projecting to 24 different targets (225 source-to-target combinations) across the whole mouse brain. We highlight uses of these data for interrogating principles relating projection types to transcriptomics and epigenomics, and for addressing hypotheses about cell types and connections related to genetics. We provide an overall synthesis with 926 statistical comparisons of discriminability of neurons projecting to each target for every source. We integrate this dataset into the larger BRAIN Initiative Cell Census Network atlas, composed of millions of neurons, to link projection cell types to consensus clusters. Integration with spatial transcriptomics further assigns projection-enriched clusters to smaller source regions than the original dissections. We exemplify this by presenting in-depth analyses of projection neurons from the hypothalamus, thalamus, hindbrain, amygdala and midbrain to provide insights into properties of those cell types, including differentially expressed genes, their associated cis-regulatory elements and transcription-factor-binding motifs, and neurotransmitter use.

Abstract

Abstract Background Marine sediments represent one of the planet’s largest carbon stores. Bottom trawl fisheries constitute the most widespread physical disturbance to seabed habitats, which exert a large influence over the oceanic carbon dioxide (CO2) sink. Recent research has sparked concern that seabed disturbance from trawling can therefore turn marine sediments into a large source of CO2, but the calculations involved carry a high degree of uncertainty. This is primarily due to a lack of quantitative understanding of how trawling mixes and resuspends sediments, how it alters bioturbation, bioirrigation, and oxygenation rates, and how these processes translate into carbon fluxes into or out of sediments. Methods The primary question addressed by this review protocol is: how does mobile bottom fishing affect benthic carbon processing and storage? This question will be split into the following secondary questions: what is the effect of mobile bottom fishing on: (i) the amount and type of carbon found in benthic sediments; (ii) the magnitude and direction of benthic-pelagic carbon fluxes; (iii) the biogeochemical, biological, and physical parameters that control the fate of benthic carbon; and (iv) the biogeochemical, biological, and physical parameters that control the fate of resuspended carbon. Literature searches will be conducted in Web of Science, SCOPUS, PROQUEST, and a range of grey and specialist sources. An initial scoping search in Web of Science informed the final search string, which has been formulated according to Population Intervention Comparator Outcome (PICO) principles. Eligible studies must contain data concerning a change in a population of interest caused by mobile bottom fishing. Eligible study designs are Before and After, Control and Impact, and Gradient studies. Studies included at full-text screening will be critically appraised, and study findings will be extracted.Extracted data will be stored in an Excel spreadsheet. Results will be reported in narrative and quantitative syntheses using a variety of visual tools including forest plots. Meta-analysis will be conducted where sufficient data exists.

23. Preparing Schools for Educational Change: Barriers and Supports -- A Systematic Literature Review

Abstract

Although the role of leadership in school improvement is widely recognized, there is less understanding of what leaders can do to support (and mitigate barriers to) the implementation of mandated educational change. This systematic literature review analyzed findings from 191 relevant primary studies published between 2000 and 2020 and identified four broad themes: developing a reform-ready school climate, comprehensive planning, preparation for implementation, and building capacity. These results offer practical information about malleable school-level factors to which school leaders can attend (before implementing mandated reform) to support the school's capacity for change.

Abstract

The Near Infrared Imager and Slitless Spectrograph instrument (NIRISS) is the Canadian Space Agency (CSA) contribution to the suite of four science instruments of JWST. As one of the three NIRISS observing modes, the Single Object Slitless Spectroscopy (SOSS) mode is tailor-made to undertake time-series observations of exoplanets to perform transit spectroscopy. The SOSS permits observing point sources between 0.6 and 2.8 um at a resolving power of 650 at 1.25 um using a slit-less cross-dispersing grism while its defocussing cylindrical lens enables observing targets as bright as J=6.7 by spreading light across 23 pixels along the cross-dispersion axis. This paper officially presents the design of the SOSS mode, its operation, characterization, and its performance, from ground-based testing and flight-based Commissioning. On-sky measurements demonstrate a peak photon conversion efficiency of 55% at 1.2 um. The first time-series on the A-type star BD+60o1753 achieves a flux stability close to the photon-noise limit, so far tested to a level of 20 parts per million on 40-minute time-scales after simply subtracting a long-term trend. Uncorrected 1/f noise residuals underneath the spectral traces add an extra source of noise equivalent to doubling the readout noise. Preliminary analysis of a HAT-P-14b transit time-series indicates that it is difficult to remove all the noise in pixels with partially saturated ramps. Overall, the SOSS delivers performance at the level required to tackle key exoplanet science programs such as detecting secondary atmospheres on terrestrial planets and measuring abundances of several chemical species in gas giants., Comment: 41 pages, 28 figures, accepted for publication in PASP

Abstract

The Near-Infrared Imager and Slitless Spectrograph (NIRISS) is the science module of the Canadian-built Fine Guidance Sensor (FGS) onboard the James Webb Space Telescope (JWST). NIRISS has four observing modes: 1) broadband imaging featuring seven of the eight NIRCam broadband filters, 2) wide-field slitless spectroscopy (WFSS) at a resolving power of $\sim$150 between 0.8 and 2.2 $\mu$m, 3) single-object cross-dispersed slitless spectroscopy (SOSS) enabling simultaneous wavelength coverage between 0.6 and 2.8 $\mu$m at R$\sim$700, a mode optimized for exoplanet spectroscopy of relatively bright ($J<6.3$) stars and 4) aperture masking interferometry (AMI) between 2.8 and 4.8 $\mu$m enabling high-contrast ($\sim10^{-3}-10^{-4}$) imaging at angular separations between 70 and 400 milliarcsec for relatively bright ($M<8$) sources. This paper presents an overview of the NIRISS instrument, its design, its scientific capabilities, and a summary of in-flight performance. NIRISS shows significantly better response shortward of $\sim2.5\,\mu$m resulting in 10-40% sensitivity improvement for broadband and low-resolution spectroscopy compared to pre-flight predictions. Two time-series observations performed during instrument commissioning in the SOSS mode yield very stable spectro-photometry performance within $\sim$10% of the expected noise. The first space-based companion detection of the tight binary star AB Dor AC through AMI was demonstrated.

Delineating the gene-regulatory programs underlying complex cell types is fundamental for understanding brain function in health and disease. Here, we comprehensively examined human brain cell epigenomes by probing DNA methylation and chromatin conformation at single-cell resolution in 517 thousand cells (399 thousand neurons and 118 thousand non-neurons) from 46 regions of three adult male brains. We identified 188 cell types and characterized their molecular signatures. Integrative analyses revealed concordant changes in DNA methylation, chromatin accessibility, chromatin organization, and gene expression across cell types, cortical areas, and basal ganglia structures. We further developed single-cell methylation barcodes that reliably predict brain cell types using the methylation status of select genomic sites. This multimodal epigenomic brain cell atlas provides new insights into the complexity of cell-type-specific gene regulation in adult human brains.

Abstract

Objectives: To identify and quantify risk factors that contribute to clusters of COVID-19 in the workplace. Methods: We identified clusters of COVID-19 cases in the workplace and investigated the characteristics of the individuals, the workplaces, the areas they work, and the methods of commute to work, through data linkages based on Middle Layer Super Output Areas (MSOAs) in England between 20/06/2021 and 20/02/2022. We estimated associations between potential risk factors and workplace clusters, adjusting for plausible confounders identified using a Directed Acyclic Graph (DAG). Results: For most industries, increased physical proximity in the workplace was associated with increased risk of COVID-19 clusters, while increased vaccination was associated with reduced risk. Commuter demographic risk factors varied across industry, but for the majority of industries, a higher proportion of black/african/caribbean ethnicities, and living in deprived areas, was associated with increased cluster risk. A higher proportion of commuters in the 60-64 age group was associated with reduced cluster risk. There were significant associations between gender, work commute methods, and staff contract type with cluster risk, but these were highly variable across industries. Conclusions: This study has used novel national data linkages to identify potential risk factors of workplace COVID-19 clusters, including possible protective effects of vaccination and increased physical distance at work. The same methodological approach can be applied to wider occupational and environmental health research.

Abstract

Twenty-six years ago a small committee report, building on earlier studies, expounded a compelling and poetic vision for the future of astronomy, calling for an infrared-optimized space telescope with an aperture of at least $4m$. With the support of their governments in the US, Europe, and Canada, 20,000 people realized that vision as the $6.5m$ James Webb Space Telescope. A generation of astronomers will celebrate their accomplishments for the life of the mission, potentially as long as 20 years, and beyond. This report and the scientific discoveries that follow are extended thank-you notes to the 20,000 team members. The telescope is working perfectly, with much better image quality than expected. In this and accompanying papers, we give a brief history, describe the observatory, outline its objectives and current observing program, and discuss the inventions and people who made it possible. Abstract

Abstract A major impediment to wildlife conservation and management, from a quantitative perspective, is dealing with high degrees of uncertainty associated with population estimates. Integrated population models (IPMs) can help alleviate that challenge, but they are often limited to narrow spatial or temporal windows owing to the financial and logistical burdens of acquiring requisite datasets. To expand the spatiotemporal scope of practical IPM implementation, we developed a novel method that expresses demographic relatedness among sampled and unsampled locations using geographic principles of spatial autocorrelation. We interpolated demographic parameters at unsampled locations using parameter estimates from data‐informed locations. Errors attributable to the interpolative process were corrected using a joint likelihood and locally recorded count data (‘cheaper’ and broadly distributed). We evaluated the spatially interpolated IPM (SIIPM) for precision and accuracy under variable levels of spatial autocorrelation using simulated data and a Leave‐One‐Out Cross‐Validation (LOOCV) technique. Conventional IPMs and state‐space models (SSM) were fit to the same simulated datasets to provide a comparative assessment of the novel method. In a final, empirical demonstration we fit the SIIPM to data collected from Greater Sage‐Grouse (Centrocercus urophasianus; sage‐grouse) populations located in Nevada, U.S.A. during 2013–2021. SIIPMs outperformed conventional IPMs when fit to data possessing moderate‐to‐high levels of spatial autocorrelation. Under moderate levels of autocorrelation, the average improvement in parameter estimation was 13.6% for survival, 65.3% for recruitment and 23.7% for rate of population change (λ). Abstract

Many revegetation projects are intended to benefit wildlife species. Yet, there are few a priori evaluations that assess the potential efficiency of restoration actions in recovering wildlife habitats. We developed a spatial vegetation–habitat recovery model to gauge the degree to which field planting strategies could be expected to recover multi-factor habitat conditions for wildlife following wildfires. We simulated a wildfire footprint, multiple sagebrush (Artemisia spp.) planting scenarios, and tracked projected vegetation growth for 15 years post-fire. We used a vegetation transition framework to track and estimate the degree to which revegetation could accelerate habitat restoration for a Greater sage-grouse (Centrocercus) population within the Great Basin, western United States. We assessed the amount of habitat 15 years post-fire to estimate the degree to which revegetation could be expected to accelerate habitat restoration. Our results highlight a potential disconnect between the expansive areas required by wide-ranging wildlife such as sage-grouse and the relatively small areas that planting treatments have created. Habitat restorations and planting strategies that are intended to benefit sage-grouse may only speed up localized habitat restoration. This study provides an example of how linked revegetation–habitat modeling approaches can scope the expected return on restoration investment for habitat improvements and support the strategic use of limited restoration resources.

38. Development and Validation of the Workplace Climate and Persistence Scale for STEM Faculty Framed in Intersectionality of Gender, Race/Ethnicity, and Socioeconomic Background

Abstract

Gender disparities in Science, Technology, Engineering, and Mathematics (STEM) faculty composition remain a major issue in U.S. higher education. Specifically, the underrepresentation of women of color has been described as a crisis. We developed the Workplace Climate and Persistence Scale (WCPS) for STEM faculty to assess departmental-level climate factors associated with faculty persistence, using a framework of the intersectionality of gender, race/ethnicity, and socioeconomic background. To validate the WCPS, we conducted analyses for construct validity, internal consistency reliability, and criterion validity evidence using data from 394 STEM faculty at three universities. Exploratory and confirmatory factor analyses resulted in 43 items loading across nine factors in a good model fit range. The nonlinear SEM reliability coefficients ranged from 0.811 to 0.971, with the overall Omega = 0.991. The correlation matrix indicated that the WCPS differentiates STEM faculty's perceptions by multiple social identities. We expect the WCPS to help administrators identify where to target approaches to improve climate and assess the effectiveness of such interventions.

Abstract

Background: Racial and ethnic minoritized people with dementia (PWD) are at high risk of disenrollment from hospice, yet little is known about the relationship between hospice quality and racial disparities in disenrollment among PWD. Objective: To assess the association between race and disenrollment between and within hospice quality categories in PWD. Design/Setting/Subjects: Retrospective cohort study of 100% Medicare beneficiaries 65+ enrolled in hospice with a principal diagnosis of dementia, July 2012-December 2017. Race and ethnicity (White/Black/Hispanic/Asian and Pacific Islander [AAPI]) was assessed with the Research Triangle Institute (RTI) algorithm. Hospice quality was assessed with the publicly-available Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey item on overall hospice rating, including a category for hospices exempt from public reporting (unrated). Results: The sample included 673,102 PWD (mean age 86, 66% female, 85% White, 7.3% Black, 6.3% Hispanic, 1.6% AAPI) enrolled in 4371 hospices nationwide. Likelihood of disenrollment was higher in hospices in the lowest quartile of quality ratings (vs. highest quartile) for both White (adjusted odds ratio [AOR] 1.12 [95% confidence interval 1.06-1.19]) and minoritized PWD (AOR range 1.2-1.3) and was substantially higher in unrated hospices (AOR range 1.8-2.0). Within both low- and high-quality hospices, minoritized PWD were more likely to be disenrolled compared with White PWD (AOR range 1.18-1.45). Conclusions: Hospice quality predicts disenrollment, but does not fully explain disparities in disenrollment for minoritized PWD. Efforts to improve racial equity in hospice should focus both on increasing equity in access to high-quality hospices and improving care for racial minoritized PWD in all hospices.

Abstract

Background: Invasive fungal infections are responsible for a large number of infections in hospitalized patients annually and are responsible for high morbidity and mortality. Familiarity with novel agents or strategies in this area can be challenging. Objectives: To identify the top 10 manuscripts on the treatment of invasive fungal infections from 2018 to 2023. Design: Modified Delphi consensus-building technique. Methods: A three-stage consensus-building approach was used comprised of (1) identifying relevant articles; (2) voting by a panel of experts to establish consensus on the importance of these articles; and (3) finalizing the list of top articles by a small group. Members of the Southeastern Research Group Endeavor network served as content experts. Publications from 2018 to 2023 were evaluated if articles met the following inclusion criteria: (1) published between 2018 and 2023, (2) contained content related to fungal infections, and (3) included an actionable intervention. Results: A total of 6518 potential publications were assessed. After applying inclusion and exclusion criteria, 82 articles were reviewed. The top 10 publications related to invasive fungal infections, selected by a panel of experts, are summarized in this manuscript and include publications related to the treatment of invasive aspergillosis, candidiasis, and cryptococcosis. Conclusion: This article highlights the selected publications and may serve as a key resource for teaching and training. Clinicians may also employ these reported interventions to identify new opportunities to optimize antifungal therapeutic strategies within one’s institution.

Abstract

Summary: Respiratory syncytial virus (RSV) causes lower respiratory tract infections with significant morbidity and mortality at the extremes of age. Vaccines based on the viral fusion protein are approved for adults over 60, but infant protection relies on passive immunity via antibody transfer or maternal vaccination. An infant vaccine that rapidly elicits protective antibodies would fulfill a critical unmet need. Antibodies arising from the VH3-21/VL1-40 gene pairing can neutralize RSV without the need for affinity maturation, making them attractive to target through vaccination. Here, we develop an anti-idiotypic monoclonal antibody (ai-mAb) immunogen that is specific for unmutated VH3-21/VL1-40 B cell receptors (BCRs). The ai-mAb efficiently engages B cells with bona fide target BCRs and does not activate off-target non-neutralizing B cells, unlike recombinant pre-fusion (preF) protein used in current RSV vaccines. These results establish proof of concept for using an ai-mAb-derived vaccine to target B cells hardwired to produce RSV-neutralizing antibodies.

Abstract

Individuals within the criminal justice system are at greater risk of substance use–related morbidity and mortality and have substantial healthcare needs. In this quasi-experimental study, we assessed utilization patterns of Massachusetts Medicaid Program (MassHealth) services and associated expenditures among drug court probationers compared to a propensity score–matched sample of traditional court probationers. Risk of reoffending, employment status, age, and living arrangement data were used to calculate propensity scores and match probationers between the two court types, producing a final sample of 271 in each court (N=542). Utilization of services and associated expenditures were analyzed using a two-part model to address the skewed distribution of the data and to control for residual differences after matching from the perspective of the payer (i.e., MassHealth). The largest categories of MassHealth spending were prescription pharmaceuticals, hospital inpatient visits, and physician visits. In the unadjusted analysis, drug court probationers exhibited greater MassHealth services utilization and expenditures than traditional court probationers. However, drug courts enrolled more females, more people at higher risk of reoffending, and more people with opioid use disorders. After controlling for differences between the two court types, the difference in MassHealth services utilization and associated expenditures did not reach statistical significance. Drug court probationers were more likely to engage with healthcare services but did not incur significantly greater expenditures than traditional court probationers after controlling for differences between the samples.

Abstract

Background and Objectives For patients receiving end-of-life care, adjusting polypharmacy and deprescription of potentially harmful medicines are important impactors on quality-of-life. Antithrombotic therapies (ATT) are used by 30-50% of patients with cancer, rising to 80% in older cancer patients, and are often continued until a quality-of-life-impacting bleeding event, or a patient is physically unable to take oral medication. We aimed to describe the use of ATT and occurrence of clinical outcomes in patients with cancer during end-of-life care. Approach Linked health and administrative data in SAIL were used to identify end-of-life cancer patients diagnosed in Wales, focusing on cancer types with an estimated 1-year survival time. Survival analysis was used to describe current ATT usage and to investigate the associations between ATT usage and major cardiovascular and bleeding events. Results 21,880 individuals were diagnosed with the pre-defined cancer types between January 2013 and December 2019, 5,660 (26%) of these were taking ATT at diagnosis. During the study period 1670 (30%) were discontinued. Median survival time of patients taking ATT at diagnosis was 169 (IQR 158-180) days, while median time to deprescription was 206 (IQR 190-224) days. Among ATT-users diagnosis, 460 (8%) and 900 (16%) experienced a major bleed or a cardiovascular event, respectively, within 1 year following diagnosis, compared to 1100 (7%) and 1930 (12%) among those who were not prescribed ATT at diagnosis. Conclusions and Implications Our research highlights the need for careful assessment and management of ATT in the interest of improving quality-of-life for patients with cancer during end-of-life care.

Abstract

Background and Objectives SARS-CoV-2 infection in children and young people (CYP) can lead to life threatening conditions including COVID-19, transmission to more vulnerable individuals, or even long COVID. Vaccination against COVID-19 reduces the chance of infection and transmission of the virus, but vaccine uptake in the UK has been shown to decrease with age. Approach We undertook a multistate-model approach to estimate hazard ratios on national cohorts constructed from linked health and administrative data, adjusting for several demographic factors. The models were applied to 3,433,483 CYP aged 5-17 years between 4th August 2021 and 31st May 2022. The results were combined in a random effects meta-analysis. Results Uptake of the first COVID-19 vaccine in CYP was lower compared to older age groups in the UK, and diminished further with subsequent doses (34%, 20% and 2% for 1st, 2nd, and booster dose respectively). Age of the CYP and vaccination status of the adults in the household were identified as important risk factors. For example, 5-11-year-olds were less likely to receive their first vaccine compared to 16-17-year-olds (adjusted Hazard Ratio [aHR]: 0.10 (95%CI: 0.06-0.19), and CYP in unvaccinated households were less likely to receive their first vaccine compared to partially vaccinated households (aHR: 0.19, 95%CI 0.13-0.29). Conclusions and Implications Our work highlights the need for targeted strategies to increase COVID-19 vaccine acceptance and uptake among CYP, especially considering the influence of parental consent and household factors. Further research could help identify risk factors in household types to investigate more direct associations, which may be influenced by factors such as living with vulnerable people, familial structure, or deprivation.

Abstract

Dravet Syndrome (DS) is a pediatric-onset epilepsy with an elevated risk of Sudden Unexpected Death in Epilepsy (SUDEP). Most individuals with DS possess mutations in the voltage-gated sodium channel gene Scn1a, expressed in both the brain and heart. Previously, mutations in Scn1a have been linked to arrhythmia. We used a Scn1a−/+ DS mouse model to investigate changes to cardiac mitochondrial function that may underlie arrhythmias and SUDEP. We detected significant alterations in mitochondrial bioenergetics that were sex-specific. Mitochondria from male Scn1a−/+ hearts had deficits in maximal (p = 0.02) and Complex II-linked respiration (p = 0.03). Male Scn1a−/+ mice were also more susceptible to cardiac arrhythmias under increased workload. When isolated cardiomyocytes were subjected to diamide, cardiomyocytes from male Scn1a−/+ hearts were less resistant to thiol oxidation. They had decreased survivability compared to Scn1a+/+ (p = 0.02) despite no whole-heart differences. Lastly, there were no changes in mitochondrial ROS production between DS and wild-type mitochondria at basal conditions, but Scn1a−/+ mitochondria accumulated more ROS during hypoxia/reperfusion. This study determines novel sex-linked differences in mitochondrial and antioxidant function in Scn1a-linked DS. Importantly, we found that male Scn1a−/+ mice are more susceptible to cardiac arrhythmias than female Scn1a−/+ mice. When developing new therapeutics to address SUDEP risk in DS, sex should be considered.

47. Estimating traffic volume and road age in Wyoming to inform resource management planning: An application with wildlife-vehicle collisions

Abstract

Road networks and their associated vehicular traffic disturb many terrestrial systems, but inventories of roads used to assess these effects often focus on the ‘where’ (e.g., local road type and density) and neglect the ‘when’ (e.g., temporal disturbance) or ‘how much’ (e.g., traffic volume disturbance). We developed annual estimates of the ‘when’ (road age) and ‘how much’ (vehicular traffic volume) across 148,172 km of highways, arterials, collectors, local, and gravel/graded roads within the state of Wyoming for the years 1986 to 2020 to provide a comprehensive dataset for future ecological investigations. We leveraged a suite of ancillary data on surface disturbances (e.g., oil & gas drilling operations, wind turbines, and open pit mines) with known establishment dates and combined them using graph theory and centrality metrics to estimate the age of each road. We then predicted traffic volume obtained from the Wyoming Department of Transportation for each year across Wyoming using a machine learning method, XGBoost, and a separate set of spatial covariates hypothesized to explain traffic patterns across large regions. We found that 132,476 km of these roads likely existed before 1986, but that 16,693 km (10.7 %) of roads have been built since 1986. Overall, our estimates of road age were 89 % accurate when assessed on a subset of 1,330 roads with high-resolution aerial imagery. Mean absolute error for predicting traffic volume ranged from 35.2 to 77.9 annual average daily traffic (aadt) for trucks and 269.2 to 516.7 aadt for all-vehicles across the 35 years. We found that mean traffic volume across the state increased by 23 % for both truck-only traffic and all vehicular traffic from 1986 to 2020. However, changes in traffic volume have varied substantially across the state (e.g., 100 % increases in volume in some areas, while other areas experienced declines of up to 1,786 %). We also illustrate a novel application of these data by predicting rates of reported wildlife-vehicle collisions (WVCs) along a subset of roads. We found evidence of a non-linear relationship that supported a threshold hypothesis for WVCs, wherein increases in traffic volume equate to increases in WVCs up to a threshold, above which increases in traffic volume result in declines in WVCs. The data provided here will enable better-informed studies of road ecology to address how roads may affect wildlife populations and key ecosystems across Wyoming.

Background: In a previous study, we identified eight types of potential barriers to bystander cardiopulmonary resuscitation (CPR) initiation and continuation until the arrival of emergency medical services (EMS) on scene, in the context of emergency calls for out-of-hospital cardiac arrest (OHCA). Many cases had multiple barriers. In this study, we aimed to estimate the independent effects of these barriers after adjusting for case characteristics. Methods: We used data for the 295 non-trauma OHCAs from the St John Western Australian (SJ-WA) OHCA Database. Excluded cases were: EMS-witnessed OHCA, callers not with/close to the patient, OHCA not recognised during the emergency call, bystander CPR in progress prior to the call and calls coded as obvious death by SJ-WA. We conducted two multivariable logistic regression models including the eight barriers (callers: 1) perceived inappropriateness of CPR, 2) emotional distress, 3) reluctance to perform CPR, 4) physical limitations, 5) access to the patient, 6) leaving the scene, 7) communication failure, and 8) on-scene distractions) and case characteristics. Results: The callers perceiving CPR as inappropriate (adjusted odds ratio [AOR] = 0.20, 0.11–0.37) and witnessed arrest (AOR = 2.88, 95% CI 1.48–5.60) were independently associated with CPR initiation. Caller distractions such as performing other tasks or relaying information to other bystanders were negatively significantly associated with callers continuing CPR to EMS arrival (AOR = 0.27, 0.10–0.73). Conclusions: Perceptions of inappropriateness and caller distractions were independent risk factors for the delivery of bystander CPR. Further research around how call-takers navigate these barriers and encourage callers should be performed.

Abstract

Understanding students' progress towards meeting grade-level expectations within an academic year is the goal of many education stakeholders so that they can make decisions to adjust instruction and improve students' learning trajectories. The purpose of this study was to explore 4th and 5th grade students' progress towards meeting expectations in writing, using a curriculum- based measurement metric. Typed writing samples in narrative, persuasive, and passaged- based informational genres were collected in a one-year longitudinal design with four time points. Multilevel change models revealed evidence of quadratic change across the school year with significant change in the narrative genre but no statistically significant change in the other genres. Type of change (fan-spread growth or mastery learning) and growth based on initial transcription, spelling, and reading abilities were also explored. BACKGROUND: The Medicare Hospice Benefit increasingly serves people dying with dementia. We sought to understand characteristics, hospice use patterns, and last-month-of-life care quality ratings among hospice enrollees with dementia coexisting with another terminal illness as compared to enrollees with a principal hospice diagnosis of dementia, and enrollees with no dementia. METHODS: We conducted a pooled cross-sectional study among decedent Medicare beneficiaries age 70+ using longitudinal data from the National Health and Aging Trends Study (NHATS) (last interview before death; after-death proxy interview) linked to Medicare hospice claims (2011-2017). We used unadjusted and adjusted regression analyses to compare characteristics of hospice enrollees with coexisting dementia to two groups: (1) enrollees with a principal dementia diagnosis, and (2) enrollees with no dementia. RESULTS: Among 1105 decedent hospice enrollees age 70+, 40% had coexisting dementia, 16% had a principal diagnosis of dementia, and 44% had no dementia. In adjusted analyses, enrollees with coexisting dementia had high rates of needing help with 3-6 activities of daily living, similar to enrollees with principal dementia (62% vs. 67%). Enrollees with coexisting dementia had high clinical needs, similar to those with no dementia, for example, 63% versus 61% had bothersome pain. Care quality was worse for enrollees with coexisting dementia versus principal dementia (e.g., 61% vs. 79% had anxiety/sadness managed) and similar to those with no dementia. Enrollees with coexisting dementia had similar hospice use patterns as those with principal diagnoses and higher rates of problematic use patterns compared to those with no dementia (e.g., 16% vs. 10% live disenrollment, p = 0.004). CONCLUSIONS: People with coexisting dementia have functional needs comparable to enrollees with principal diagnoses of dementia, and clinical needs comparable to enrollees with no dementia. Changes to hospice care models and policy may be needed to ensure appropriate dementia care.
