12. Development of Electronic Health Record System Based on Carpal Tunnel Syndrome

Abstract

Physical examination, clinical tests and electrophysiological methods are used in the diagnosis of carpal tunnel syndrome (CTS). However, in practice there are no standard clinical and electrophysiological tests for clinics and laboratories. Therefore, data fragmentation or incompatibilities may occur in Electronic Health Record (EHR) systems. Furthermore, secondary use and different biomedical research targets are not considered in these EHR systems. During routine documentation, incomplete, incorrect, inconsistent data entry and incorrect coding can be done. This study aimed to develop an EHR system that could be used in different clinics and centers in diagnosis of CTS, thus creating a standardized, high quality, predictive, preventive, personalized and real-time participatory CTS biomedical data warehouse. The CTS-based EHR system was developed using Microsoft Visual Studio C # programming language. Also during a new patient record, the system was supported by a clinical decision support system (CDS S) based on the data mining methods using WEKA program for pre-diagnosis of the CTS. This EHR system also allows clinical and electrophysiological test results as well as genetic and environmental variants to be integrated into a single database within the framework of precision medicine approachment. In addition, this system can provide a large scale accurate and complete data warehouse for secondary use purposes.

13. The Prediction of Drying Performance of Banana Rings Dried By Osmo-solar Dehydration Method.

Abstract

Solar drying is known as the oldest and most common traditional food preservation method. However, if the product is indirectly contacted with the sun light, there is negative effect in the color and nutrient values of the product. Solar dryers have been developed to utilize the heat effect of the sun to solve these problems. It can be used in combination with osmotic dehydration to increase the efficiency of the solar drying process. Osmotic dehydration is applied as a pretreatment technique in the drying process. The pretreatments to be applied before drying have important effects on the quality and operating cost of the product to be dried. In addition, the osmotic dehydration pretreatment enables to shorten the drying time and increase the drying potential. In this study, bananas with high drying temperature and high moisture content were sliced into 3 mm rings, then osmotic dehydration pre-drying was applied, and then they were dried in a solar tray dryer. Sucrose and citric acid solution in 2.5%, 5%, 7.5% and 10% (w/v) concentrations were used for osmotic dehydration treatment. In addition, samples without osmotic dehydration were dried in a solar dryer to determine the effect of pretreatment. The input variables of the drying process are solution concentration (0%, 2.5%, 5%, 7.5% and 10%), type of solution (sucrose solution and citric acid solution), osmotic dehydration time (10, 30, 60 and 90 min) and sun drying time (60, 120, 180, 240 and 300 min). Output variables were chosen as moisture ratio and shrinkage rate. The results clearly showed that both the moisture ratio and the shrinkage ratio have increased due to increased solar dryer time, solution concentration and immerse time. Osmotic dehydration was found to be effective in dried banana rings in the solar tray dryer. Gradient Boosting Machine (GBM) was used to model the drying conditions and the model was successful. The correlation coefficient (R² ) values of the GBM model were respectively found as 0.94 and 0.83 for the moisture ratio and the shrinkage ratio. [ABSTRACT FROM AUTHOR]

Fakat pratikte uygulanan klinik ve elektrofizyolojiktestlerde klinik ve laboratuvarlar için bir standart bulunmamaktadır. Bundandolayı Elektronik Sağlık Kaydı (ESK) sistemlerinde, veri parçalanması veyauyumsuzluklar meydana gelebilmektedir. Ayrıca bu ESK sistemlerinde, ikincilkullanım ve farklı biyomedikal araştırma hedefleri dikkate alınmamakta ve rutindökümantasyon işlemi sırasında, eksik, hatalı, tutarsız veri girişleri vehatalı kodlamaları yapılabilmektedir. Bu çalışma ile, KTS tanısında farklıklinik ve merkezlerce de kullanılabilecek bir ESK sisteminin geliştirilmesi veböylelikle standartlaştırılmış, kaliteli, öngörücü, önleyici,kişiselleştirilmiş ve gerçek zamanlı katılımcı bir KTS biyomedikal veriambarının oluşturulması hedeflenmiştir. KTS tabanlı ESK sistemi, MicrosoftVisual Studio C# programlama dili kullanılarak geliştirilmiştir. Ayrıca; yenihasta kaydı esnasında KTS ön tanısı için WEKA programı kullanılarak verimadenciliği yöntemine dayalı bir klinik karar destek sistemi (KKDS) iledesteklenmiştir. Geliştirilen ESK sistemi, klinik ve elektrofizyoloijk testsonuçlarının yanısıra hassas tıp yaklaşımı çerçevesinde genetik ve çevreselvaryantların da tek bir veri tabanına entegre edilmesine imkan tanımakta veikincil kullanım amacıyla geniş ölçekli doğru eksiksiz ve aynı standartta birveri ambarı sunabilmektedir.

Abstract

Spastisite, üst motor nöron lezyonu sonucu kasın aralıklı veya sürekli istemsiz aktivasyonu şeklinde kendini gösteren bir duyu-motor kontrolü bozukluğudur. Spastisitenin tüm gözlenebilen yönlerini açıklayabilen patofizyolojik mekanizma ve uygun bir ölçüm ve değerlendirme standardı yoktur. Bu tez çalışmasında, spastisitenin görüldüğü hemiplejik (inmeli) hastalar ve sağlıklı kontrol gruplarına ait alt ektremite Tibialis Anterior (TA) ve Medial Gastroknemius (MG) kaslarından ayakta, istirahat, dorsal fleksiyon ve plantar fleksyon pozisyonlarında elde edilen yüzey elektromiyografi (EMG) sinyalleri dalgacık dönüşümü (DD) ile analiz edilerek spastisitenin patofizyolojik mekanizması ve nedenselliğini uygun bir şekilde değerlendiren Bulanık Mantık Sınıflandırıcı (BMS) ile spastisitenin kantitatif ölçümünü gerçekleştirilmiştir. Tasarlanan BMS, oldukça iyi bir başarım oranı (%95,8) ile spastisitenin kendi doğasında var olan ölçüm zorluklarının ve yaygın olarak kullanılan klinik ölçeklerdeki yetersizliklerin üstesinden gelme konusunda etkili olmuştur. Ayrıca sınıflandırıcının, hemipleji ile spastisite arasındaki nedenselliği açıklayarak hasta ve sağlıklıyı ayırt etmedeki tanısal yeterlilik seviyesinin `mükemmel` olduğu görülmüştür. Spasticity is a disorder of the sensory-motor control as a result of upper motor neuron (UMN) lesion which manifests itself as intermittent or continuous involuntary activation of muscles. Pathophysiological mechanisms can not explain all observable aspects of spasticity, and there is no available measurement and a standart assessment method. In this thesis, Surface electromyography (sEMG) signals obtained from hemiplegic patients with spasticity and healthy control groups whose Tibialis Anterior (TA) and Medial Gastroknemius (MG) muscles were analyzed using wavelet transform (WT) in standing-up, relaxing, dorsal flexion and plantar flexion positions. And then quantitative measurement of spasticty was carried out by the Fuzzy Logic Classifier (FLC) which appropriately assess the pathophysiological mechanism and causality of spasticity. The designed classifier; FLC which had fairly good success rate (95,8 %), has been effective in order to overcome difficulties of measurement that naturally exist in spasticity and deficiencies of the commonly used clinical scales. In addition, this classifier which explain the causality between clinical and electrophysiological aspects in spasticity in hemiplegic patients was found to have `perfect` level of the diagnostic ability in distinguishing healthy subjects and patients. 175

Abstract

Conventional electrophysiological (EP) tests may yield ambiguous or false-negative results in some patients with signs and symptoms of carpal tunnel syndrome (CTS). Therefore, researchers tend to investigate new parameters to improve the sensitivity and specificity of EP tests. We aimed to investigate the mean and maximum power of the compound muscle action potential (CMAP) as a novel diagnostic parameter, by evaluating diagnosis and classification performance using the supervised Kohonen self-organizing map (SOM) network models. The CMAPs were analyzed using the fast Fourier transform (FFT). The mean and maximum power parameters were calculated from the power spectrum. A counter-propagation artificial neural network (CPANN), supervised Kohonen network (SKN) and XY-fused network (XYF) were compared to evaluate the classification and diagnostic performance of the parameters using the confusion matrix. The mean and maximum power of the CMAP were significantly lower in patients with CTS than in the normal group (p < 0.05), and the XYF network had the best total performance of classification with 91.4%. This study suggests that the mean and maximum power of the CMAP can be considered as less time-consuming parameters for the diagnosis of CTS without using additional EP tests which can be uncomfortable for the patient due to poor tolerance to electrical stimulation.
