Objective: To evaluate the consumption of human albumin 20% in the hospitalization services of the Hospital Militar Central during the period June - December 2019. Materials and methods: Observational, descriptive, cross-sectional and retrospective research, whose sample consisted of 1139 records provided by the computer service of hospitalized patients. Results: The average age of the patients was 78.2 years, 57.8% were 80 years old or older, 63.2% were male and 55.3% had holder status. The drug was prescribed mainly in hypoalbuminemia (27.37%) and nephrotic syndrome (23.16%). The service with greater patient care was internal medicine (50%), but those of higher prescriptions of human albumin 20% were gastroenterology (1539 bottles) and internal medicine (1276 bottles), with an average of 61.6 and 13.4 bottles per person, respectively, as well as those of higher total costs (S / 173,183.67 and S / 143,588.28, respectively). Conclusion: The consumption of human albumin 20% was prescribed mainly in male patients, 80 years of age or older, with hypoalbuminemia and nephrotic syndrome, treated in the hospitalization services for gastroenterology and internal medicine of the Central Military Hospital., Objetivo: Evaluar el consumo de albúmina humana 20% en los servicios de hospitalización del Hospital Militar Central durante el periodo junio - diciembre 2019. Metodología: Estudio tipo observacional, descriptivo, transversal y retrospectivo, cuya muestra estuvo conformada por 1139 registros proporcionados por el servicio de informática de pacientes hospitalizados. Resultados: La edad promedio de los pacientes fue 78,2 años, el 57.8% tenía 80 o más años, el 63,2% fueron varones y el 55.3% tuvieron condición de titular. El medicamento fue prescrito principalmente en hipoalbuminemia (27.37%) y síndrome nefrótico (23.16%). El servicio con mayor atención de pacientes fue medicina interna (50%), pero los de mayores prescripciones de albúmina humana 20% fueron gastroenterología (1539 frascos) y medicina interna (1276 frascos), con un promedio de 61,6 y 13,4 frascos por persona, respectivamente, así como los de mayores costos totales (S/ 173,183.67 y S/ 143,588.28, respectivamente). Conclusión: El consumo de albúmina humana 20% fue prescrito principalmente en pacientes varones, de 80 a más años de edad, con hipoalbuminemia y síndrome nefrótico, atendidos en los servicios de hospitalización por gastroenterología y medicina interna del Hospital Militar Central.

It courses with metabolic and dyslipidemic alterations, involving total cholesterol and fractions, triglycerides and nonesterified fatty acids (NEFAs). The cellular mechanisms involved in its pathogenesis include the inhibition of the enzyme sodium-potassium adenosine triphosphatase and lipotoxicity. Both mechanisms are aggravated by the reduction of serum levels of albumin, a molecule that plays a fundamental role in the adsorption of lipid molecules. In this observational study, we determined the serum concentrations of bilirubin, creatinine and albumin, and, by High Performance Liquid Chromatography, the serum concentrations of long chain NEFAs (C16: C18) during the period of hospitalization of 27 patients with Weil's syndrome, five of whom progressed to death. Significant correlations were found between the length of hospitalization and serum concentrations of biochemical markers of severity (bilirubin, creatinine, albumin), NEFAs, oleic acid and linoleic acid, and the oleic acid: albumin molar ratio, with r (Pearson) of de -0,7981, -0,7699, 0,9014, -0,8795 -0,9816, -0,9694, -0,9821 respectively. The oleic acid :albumin molar ratio and oleic-plus-linoleic acid: albumin molar ratio were significantly higher in the patients who progressed to death, whereas in the cured patients this ratio decreased to levels that were similar to those found in the control group. Roc Curve analysis for the acid oleic: albumin molar ratio proved a good predictive test, with value of cutting 0.705 associated with greater specificity and prognostic sensitivity. Our results suggest that parenteral administration of human albumine may interfere positively in the rescue of biochemical balance of oleic acid: albumin molar ratio and oleic-plus-linoleic acid: albumin molar ratio and be a therapeutic tool for severe cases of leptospirosis. Coordenação de Aperfeiçoamento de Pessoal de Nível Superior A instabilidade relacionada às proteínas, tanto estruturalmente quanto quimicamente, compromete a sua utilização em formulações como um agente terapêutico, isto se deve a um curto tempo de vida quando são submetidas a um estresse químico ou físico. Para melhorar a estabilidade de fármacos de origem protéica foi desenvolvida uma modificação química em aminoácidos e proteínas denominada peguilação. A caracterização termoanalítica de proteínas bioativas e seus produtos peguilados tem aplicação limitada na indústria farmacêutica em comparação com outras técnicas analíticas. O presente trabalho realizou estudos de caracterização térmica da forma natural e modificada por peguilação da albumina humana sérica (HSA). As técnicas térmicas envolvidas foram: Termogravimetria (TG), calorimetria exploratória diferencial (DSC) e calorimetria exploratória diferencial fotovisual (DSC fotovisual). Os estudos termogravimétricos dinâmicos comprovaram que as formas peguiladas da albumina humana sérica apresentaram uma melhor estabilidade térmica nas razões de aquecimento de 10 oC/min, 20 oC/min, 40 oC/min. As curvas calorimétricas obtidas na razão de aquecimento de 10 oC/min mostraram diferenças entre os produtos peguilados e não peguilados em relação aos eventos endotérmicos e exotérmicos. A preparação da albumina humana peguilada (HSA-PEG) (82-85% de rendimento) ocorreu através de uma reação de substituição nucleofílica. A reação ocorre a partir do metoxipolietilenoglicol Propionaldeído (75% rendimento) e o grupo alfa-amino do aminoácido livre da HSA em meio ácido com o agente redutor hidreto de metal complexo.
