Palliative care in Romania is a relatively new emerging field. Challenges of this specialty, its uniqueness and the several problems posed by a particular category of patients, the chronically ill patients, make this medical specialty a particularly complex one in certain situations, requiring a multidisciplinary team well-prepared in terms of theory and practice. Unlike in the case of most medical specialties, particular emphasis is laid on the “human” side of medical care, this specialty introducing the concept of “total pain”.

Introduction: Gallbladder hydrops is an increase in the volume of the gallbladder without any inflammatory sign, bacterial infection or the presence of any abnormalities of the biliary ducts or of the gallbladder.

The aim of the work is to study the possibility of replacing wood sawdust pellets with grain at the heating systems. Due to the energy crisis experienced in the winter of 2022-2023, when the price of used pellets increased by 300% compared to the previous year, when in some periods wooden pallets disappeared from commerce, the study of the replacement of the energy sources with which the small power domestic boiler were designed to operate on wooden pellets is justified. It is known that corn has a high calorific value. Due to this fact in the present work the possibility of using corn and the effects of its use on boiler with pellets that do not have in the specifications the fact that they can also work with any granular biomass are studied. Introduction: One of the biggest medical problems for clinicians is to establish antibiotic treatment. Irrational prescribing and abuse is an explanation of failures in the treatment of infectious diseases. This fact has led to the emergence of antibiotic resistance, which can seriously threaten public health. The purpose of our study was to identify antibiotic prescribing practices of doctors of various specializations, as well as to identify knowledge and attitudes related to microbial resistance. Materials and Methods: The study was based on a questionnaire which was distributed to doctors of different medical and surgical specialties in state hospitals. Results: The questionnaire was filled out by 110 physicians (response rate 84.6%). 73.6% of physicians found that too many prescriptions of antibiotics are issued needlessly. Most antibiotics used by doctors were cephalosporins, followed by aminopenicillins. Some of the factors involved in bacterial resistance include self-medication, prescribing inappropriate antibiotics, and increased and prolonged hospitalization. The problem of antibiotic resistance is a major one (89.1%). Conclusions: This study identified medical concepts related to antibiotics and bacterial resistance. Limitation of bacterial resistance to antibiotics is necessary and can be accomplished by reducing the prescription of antibiotics, and by informing doctors (treatment protocols, conferences) to establish an effective antibiotic treatment. The purpose of this paper is to design the mechanism for lowering and lifting the car platform using CAD software (Inventor). The used mechanism to drive the car platform is the quadrilateral mechanism due to the low cost of its compared to a gear train. Operating a mechanism can be done manually by applying a force on frame attached on the crank or by operating through a small winch. Kinematic analysis was performed using the software mentioned above. Lowering the platform to ground level is necessary in some cases to be loaded heavy objects onto the platform when are not available forklifts and other lifting mechanisms. [ABSTRACT FROM AUTHOR]

The purpose of this paper is to make the geometric model to the Archimedean spiral, curve often used in practice. Although it is a widely used, in AutoCAD there is no command with which to represent the Archimedean spiral. The method used for determining the mathematical relationships that define the spiral is the coordinate transformation, and for the calculation of the points which define it, is use AutoLISP, representation being made in AutoCAD. The result of this work is to develop an AutoLISP program which can represent the Archimedean spiral, presented a method that can be applied to the representation of any curves used in the technique. The purpose of this work is to demonstrate that by milling the archimedian cylindrical worm on CNC machines using frontal-cylindrical milling tools are obtained roughness higher than those obtained by classical methods of turning. Now worms produced from improving steel are finishing through turning and worms produced from case-hardening steel are finish through grinding. Surface roughness obtained by turning is comprised in the range between 3.2 -6.3 μm and the surface roughness obtained by milling is in the range 1.6-3.2 μm. The worm producers are interested in increasing the possibilities of processing the worms on the CNC machines with specific tools with productivity and as well as higher accuracy. The experiment which form the basis of the work has been done on CNC machine, OKUMA MULTUS B300 by processing a batch of archimedian worms with ma = 10, z1 = 2 with the use of a mills FRANKE, code 2887A.010. The paper demonstrates that surface roughness obtained by milling through the use of a frontal- cylindrical milling tool from carbide mono-block is comparable to that obtained by grinding. Introduction: Hepcidin is a regulatory protein in iron metabolism; we do not know the role in chronic kidney disease anemia. Methods: 22 patients with CKD anemia and 15 patients with CKD without anemia were investigated. CKD anemia-inclusion criteria: over 18 years, hemoglobin ≤12 g/dl for women and ≤13 g/dl for men, no treatment for anemia 6 months before enrollment, glomerular filtration rate (eGFR) <60 ml/min/1.73m² and stable creatinine three months before enrollment. Exclusion criteria: infection, bleeding, malignancy, systemic or liver disease, immunosuppression, renal replacement therapy. CKD without anemia-inclusion criteria: over 18 years, no anemia or treatment for anemia, CKD with stable creatinine values three months before enrollment. Exclusion criteria: medical conditions known to have a role in the development of polycythemia. Hepcidin-25 and ferritin were measured by ELISA method. Erythropoietin (EPO), tumor necrosis factor (TNF)-α, interleukin (IL)-6 were evaluated using chemiluminescent enzyme immunometric assays. Unpaired T test, Pearson correlation and multiple regression were used for statistical analysis. Results: Hemoglobin values were significantly lower in anemia group. There were no differences in terms of eGFR, age, body mass index, serum hepcidin, erythropoietin, fibrinogen, IL-6, and TNF-α between CKD patients with and without anemia. Serum hepcidin correlated positively with ferritin (r=0.45 p<0.05), TNF-α (r=0.54, p<0.05) and negatively with erythropoietin (r=-0.51, p<0.05). Multiple linear regression analysis demonstrated that TNF-α is an independent predictor of serum hepcidin in our patients (p=0.003, R=0.71). Conclusion: We found no differences in serum hepcidin, erythropoietin and inflammatory markers in non-dialysis CKD patients with and without anemia. [ABSTRACT FROM AUTHOR]
