Infectious diseases may lead to ocular complications including uveitis, an ocular inflammatory condition with potentially sight-threatening sequelae, and conjunctivitis, inflammation of the conjunctiva. Emerging infectious pathogens with known ocular findings include Ebola virus, Zika virus, Avian influenza virus, Nipah virus, severe acute respiratory syndrome coronaviruses, and Dengue virus. Re-emerging pathogens with ocular findings include Toxoplasma gondii and Plasmodium species that lead to malaria. The concept of One Health involves a collaborative and interdisciplinary approach to achieve optimal health outcomes by combining human, animal, and environmental health factors. This approach examines the interconnected and often complex human-pathogen-intermediate host interactions in infectious diseases that may also result in ocular disease, including uveitis and conjunctivitis. Through a comprehensive review of the literature, we review the ophthalmic findings of emerging infectious diseases, pathogenesis, and One Health perspectives that provide further insight into the disease state. While eye care providers and vision researchers may often focus on key local aspects of disease process and management, additional perspective on host-pathogen-reservoir life cycles and transmission considerations, including environmental factors, may offer greater insight to improve outcomes for affected individuals and stakeholders. more...

PURPOSE: The OlympiA randomized phase III trial compared 1 year of olaparib (OL) or placebo (PL) as adjuvant therapy in patients with germline BRCA1/2, high-risk human epidermal growth factor receptor 2-negative early breast cancer after completing (neo)adjuvant chemotherapy ([N]ACT), surgery, and radiotherapy. The patient-reported outcome primary hypothesis was that OL-treated patients may experience greater fatigue during treatment. METHODS: Data were collected before random assignment, and at 6, 12, 18, and 24 months. The primary end point was fatigue, measured with the Functional Assessment of Chronic Illness Therapy-Fatigue scale. Secondary end points, assessed with the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire, Core 30 item, included nausea and vomiting (NV), diarrhea, and multiple functional domains. Abstract Background This case represents a rare presentation of bilateral corneoscleral perforation secondary to presumed ocular mycobacterium tuberculosis infection with the goal of reporting a case of bilateral corneoscleral perforation in the setting of a positive interferon-γ release assay (IGRA) test. Findings A 27-year-old patient presented with five months of redness, worsening eye pain, and five days of decreasing vision. Visual acuity (VA) was counting fingers bilaterally. Intraocular pressure (IOP) was 10 mmHg and 19 mmHg in the right and left eye, respectively. A slit lamp examination disclosed conjunctival injection, corneal haze, bilateral mutton-fat keratic precipitates, and a hypopyon in both eyes. The right eye had a 1.5 mm × 1.5 mm Seidel-positive corneal perforation with uveal prolapse. Similarly, the left eye had a 0.5 mm × 0.5 mm Seidel-negative inferior corneal perforation with uveal plugging. The chest X-ray showed a left-sided pleural effusion. Conclusion Computed tomography (CT) of the face and sinuses showed bilateral circumferential globe thickening. IGRA was positive. All other rheumatologic and infectious workups were negative, including HIV, ACE, ANA, ANCA, CRP, anti-scleroderma antibody and HCV. The patient was treated with intravenous methylprednisolone and seven months of rifampin, isoniazid, pyrazinamide, and ethambutol. Abstract The threat of climate change has renewed interest in the responses of communities and ecosystems to warming, with changes in size spectra expected to signify fundamental shifts in the structure and dynamics of these multispecies systems. While substantial empirical evidence has accumulated in recent years on such changes, we still lack general insights due to a limited coverage of warming scenarios that span spatial and temporal scales of relevance to natural systems. We addressed this gap by conducting an extensive freshwater mesocosm experiment across 36 large field mesocosms exposed to intergenerational warming treatments of up to +8 °C above ambient levels. We found a nonlinear decrease in the overall mean body size of zooplankton with warming, with a 57% reduction at +8 °C. This pattern was broadly consistent over two tested seasons and major taxonomic groups. We also detected some breakpoints in the community-level size-temperature relationship, indicating that the system’s response shifts noticeably above a certain level of warming. These results underscore the need to capture intergenerational responses to large gradients in warming at appropriate scales in time and space in order to better understand the effects of warming on natural communities and ecosystems. more...

Abstract Background Avian malaria is caused by diverse parasite species of the genus Plasmodium, and it affects various bird species. The occurrence of this disease in some wild bird species is sparsely documented due to the scarce availability of samples. Hence the pathogenicity in some hosts is not completely known. In addition, feral birds may act as reservoirs bridging the transmission cycle from wild migratory birds to domestic and zoo-kept bird species. Case presentation An owner of pigeons adopted a feral pigeon (Columba livia forma domestica) and housed it together with his other pet-pigeons. The bird died unexpectedly a few weeks after a surgical procedure and necropsy revealed a severely anaemic carcass, with pale organs and hydropericardium. Histopathologic analysis revealed inflammatory infiltrates in the lung and liver, and monocytes and Kupffer cells contained haemozoin pigment indicative of phagocytosis of Plasmodium-infected erythrocytes. A high erythrocytic infection rate of 18% was evident in tissues and blood vessels in various organs. Furthermore, the thyroid had masses classified as thyroid carcinomas. Immunohistochemistry with anti- Plasmodium falciparum HSP70 antibody revealed positive signals in erythrocytes and intravascular leucocytes. Further microscopy analysis using a Hemacolor-stained impression smear revealed a high parasitaemia with an asynchronous infection showing all erythrocytic stages. Molecular diagnosis by PCR identified Plasmodium relictum, lineage GRW11 as the aetiological agent. The bird presented died most likely due to an acute infection as evidenced by the high blood parasitaemia, leading to major erythrocyte destruction. Further analyses of feral pigeons (n = 22) did not reveal any additional cases of Plasmodium infections. Conclusion This study reports the first mortality associated with P. relictum lineage GRW11. The study supports previous studies, suggesting that Plasmodium infections are not frequent in pigeons. Host conditions like immunosuppression due to the tumour may have influenced the infection outcome in this fatal case. Use of anti-P. falciparum HSP70 antibody for detection of P. relictum antigens for immune assays in blood and tissue samples will be a useful tool for future studies. more...

Uma das estratégias para melhorar a eficiência nas instituições é a gestão por processos, que inclui a criação de uma estrutura formal, o Escritório de Processos, prática pouco utilizada na gestão em saúde pública. Nesse sentido, esta pesquisa objetivou apresentar um estudo de caso para implantação de um Escritório de Processos na Secretaria de Estado de Saúde do Paraná. Para tanto, de maneira descritiva exploratória, aplicou-se um método que perpassa sete fases. Os resultados apontam que o contexto institucional e técnico favorece a implementação, enquanto o ambiente político pode trazer desafios, como resistências a mudanças. O escritório terá como objetivos alinhar estratégias, melhorar práticas de gestão e capacitar equipes, sendo estruturado em divisões especializadas em modelagem e governança de processos. Tal implantação reforça a gestão por processos como ferramenta potente para transformar a gestão em pública de saúde, com vistas a proporcionar serviços de qualidade à população. more...

In recent years, Internet of Things (IoT) technologies have been applied to the most diverse types of applications, highlighting their use in Smart Grids (SGs), such as smart metering, demand response, fault detection and renewable energy integration, as well as in Smart Cities, such as smart lighting, smart parking and environmental monitoring. A technology that has been widely used in these contexts is the Wireless Smart Ubiquitous Networks Field Area Network (Wi-SUN FAN) standard, which defines the implementation and behavior of interoperable networks based on open standards such as IEEE 802.15.4 and the IPv6 Routing Protocol for Low-Power and Lossy Networks (RPL). The adopted standards allow Wi-SUN FAN networks to take advantage of features such as security, frequency hopping, multi-hop packet routing, and IPv6 addressing, ensuring a wide coverage area and immunity against electromagnetic interference. Despite all the advantages, a shortcoming of Wi-SUN FAN networks is the long network formation time. In this article, we propose a mechanism called Fast Routing Recovery (FRR), which aims to accelerate the reconnection process in networks that use the RPL protocol by memorizing information about the nodes’ preferred parents. Seafood is a crucial global food source, but its role as a reservoir for antibiotic-resistant pathogens is concerning. This study focuses on the prevalence of antibiotic resistance in retail seafood, using Enterococcus spp. as an indicator. Seven categories of raw seafood were collected from retail markets in Central Italy. Out of 422 samples, 288 tested positive for Enterococcus spp., resulting in a prevalence rate of 68.25% (CI95%: 63.66%-72.51%). The most common species were E. faecalis (48%) and E. faecium (17.4%). Enterococcus spp. was most frequently found in cephalopods, salmon, bivalves, and crustaceans than in sea bass and bream. The odds of Enterococcus spp. recovery were higher in frozen than in fresh samples, while there was no difference between farm-raised and wild-caught seafood. A subset of 102 isolates was selected for antibiotic resistance testing, showing the highest resistance rates to quinupristin/dalfopristin (52.94%) and tetracycline (27.45%). Over 70% of isolates were multidrug-resistant. Additionally, strains resistant to vancomycin and oxazolidinones, two last-resort antimicrobials, were detected. Vancomycin resistance was observed in E. casseliflavus and E. gallinarum, which are naturally resistant due to the vanC chromosomal cluster. The optrA gene, responsible for resistance to oxazolidinones and phenicols, was found in E. thailandicus and E. faecium, where it was located on to a transferable plasmid. In conclusion, this study confirms the widespread presence of Enterococcus spp. in retail seafood. The detection of multi-resistant isolates and resistance to last-resort antimicrobials highlights significant human health risks, stressing the importance of including Enterococcus spp. in seafood antibiotic resistance surveillance. more...

Pancreatic cancer is characterized by extensive resistance to conventional therapies, making clinical management a challenge. Here we map the epigenetic dependencies of cancer stem cells, cells that preferentially evade therapy and drive progression, and identify SWI/SNF complex member SMARCD3 as a regulator of pancreatic cancer cells. Although SWI/SNF subunits often act as tumor suppressors, we show that SMARCD3 is amplified in cancer, enriched in pancreatic cancer stem cells and upregulated in the human disease. Diverse genetic mouse models of pancreatic cancer and stage-specific Smarcd3 deletion reveal that Smarcd3 loss preferentially impacts established tumors, improving survival especially in context of chemotherapy. Mechanistically, SMARCD3 acts with FOXA1 to control lipid and fatty acid metabolism, programs associated with therapy resistance and poor prognosis in cancer. These data identify SMARCD3 as an epigenetic modulator responsible for establishing the metabolic landscape in aggressive pancreatic cancer cells and a potential target for new therapies. more...

In the present study, LiFePO4/CNF self-standing cathodes for LIBs are synthesized by electrospinning. A lower active material amount (12.3 and 34.5 wt%) is used, compared to the conventional tape-casted cathodes (70–85 wt%). The characterization techniques (XRPD, SEM, TEM, EDS, Raman spectroscopy, and thermogravimetry) confirm that the olivine-type structure of LiFePO4 is maintained in the binder-free electrodes, and the active material is homogeneously dispersed into and within the carbon nanofibers. The electrochemical investigation demonstrates that higher Li+ diffusion coefficients (1.36 × 10−11 cm2/s) and improved reversibility are reached for free-standing electrodes, compared to the LiFePO4 tape-casted cathode (80 wt% of active material) appositely prepared for comparison. The 34.5 wt% LiFePO4 self-standing cathode displays a lower capacity fading, good reversibility and stability, enhanced capacity values at C-rates higher than 5C, and a good lifespan when cycled 1000 cycles at 1C and further cycled up to 20C, compared to the tape-casted counterpart. Notably, the improved electrochemical performances are obtained by only the 34.5 wt% of active material. Summary: Background: Mucositis is a common and highly impactful side effect of conventional and emerging cancer therapy and thus the subject of intense investigation. Although common practice, mucositis assessment is heterogeneously adopted and poorly guided, impacting evidence synthesis and translation. The Multinational Association of Supportive Care in Cancer (MASCC) Mucositis Study Group (MSG) therefore aimed to establish expert recommendations for how existing mucositis assessment tools should be used, in clinical care and trials contexts, to improve the consistency of mucositis assessment. Methods: This study was conducted over two stages (January 2022–July 2023). The first phase involved a survey to MASCC-MSG members (January 2022–May 2022), capturing current practices, challenges and preferences. These then informed the second phase, in which a set of initial recommendations were prepared and refined using the Delphi method (February 2023–May 2023). Consensus was defined as agreement on a parameter by >80% of respondents. Findings: Seventy-two MASCC-MSG members completed the first phase of the study (37 females, 34 males, mainly oral care specialists). High variability was noted in the use of mucositis assessment tools, with a high reliance on clinician assessment compared to patient reported outcome measures (PROMs, 47% vs 3%, 37% used a combination). The World Health Organization (WHO) and Common Terminology Criteria for Adverse Events (CTCAE) scales were most commonly used to assess mucositis across multiple settings. Initial recommendations were reviewed by experienced MSG members and following two rounds of Delphi survey consensus was achieved in 91 of 100 recommendations. For example, in patients receiving chemotherapy, the recommended tool for clinician assessment in clinical practice is WHO for oral mucositis (89.5% consensus), and WHO or CTCAE for gastrointestinal mucositis (85.7% consensus). The recommended PROM in clinical trials is OMD/WQ for oral mucositis (93.3% consensus), and PRO-CTCAE for gastrointestinal mucositis (83.3% consensus). Interpretation: These new recommendations provide much needed guidance on mucositis assessment and may be applied in both clinical practice and research to streamline comparison and synthesis of global data sets, thus accelerating translation of new knowledge into clinical practice. Abstract The Arctic polar nights bring extreme environmental conditions characterised by cold and darkness, which challenge the survival of organisms in the Arctic. Additionally, multiple anthropogenic stressors can amplify the pressure on the fragile Arctic ecosystems during this period. Determining how multiple anthropogenic stressors may affect the survival of Arctic life is crucial for ecological risk assessments and management, but this topic is understudied. For the first time, our study investigates the complex interactions of multiple stressors, exploring stressor temporal dynamics and exposure duration on a key Arctic copepod Calanus glacialis during the polar nights. We conducted experiments with pulse (intermittent) and press (continuous) exposure scenarios, involving microplastics, pyrene and warming in a fully factorial design. We observed significant effects on copepod survival, with pronounced impacts during later stressor phases. We also detected two‐way interactions between microplastics and pyrene, as well as pyrene and warming, further intensified with the presence of a third stressor. Continuous stressor exposure for 9 days (press‐temporal scenario) led to greater reductions in copepod survival compared to the pulse‐temporal scenario, characterised by two 3‐day stressor exposure phases. Notably, the inclusion of recovery phases, free from stressor exposure, positively influenced copepod survival, highlighting the importance of temporal exposure dynamics. We did not find behaviour to be affected by the different treatments. Our findings underscore the intricate interactions amongst multiple stressors and their temporal patterns in shaping the vulnerability of overwintering Arctic copepods with crucial implications for managing Arctic aquatic ecosystems under the fastest rate of ongoing climate change on earth. more...

Background: Patients suffering from coronavirus disease-19 (COVID-19)-related interstitial pneumonia have variable outcomes, and the risk factors for a more severe course have yet to be comprehensively identified. Cohort studies have suggested that coronary artery calcium (CAC), as estimated at chest computed tomography (CT) scan, correlated with patient outcomes. However, given that the prevalence of CAC is gender- and age-dependent, the influence of baseline confounders cannot be completely excluded. Methods: We designed a retrospective, multicenter case-control study including patients with COVID-19, with severe course cases selected based on death within 30 days or requiring invasive ventilation, whereas controls were age- and sex-matched patients surviving up to 30 days without invasive ventilation. The primary outcome was the analysis of moderate-to-severe CAC prevalence between cases and controls. Results: A total of 65 cases and 130 controls were included in the study. Cases had a significantly higher median pulmonary severity score at chest CT scan compared to controls (10 vs. 8, respectively; p = 0.0001), as well as a higher CAC score (5 vs. 2; p = 0.009). The prevalence of moderate-to-severe CAC in cases was significantly greater (41.5% vs. 23.8%; p = 0.013), a difference mainly driven by a higher prevalence in those who died within 30 days (p = 0.000), rather than those requiring invasive ventilation (p = 0.847). White blood cell count, moderate-to-severe CAC, the need for antibiotic therapy, and severe pneumonia at CT scan were independent primary endpoint predictors. Conclusions: This case-control study demonstrated that the CAC burden was higher in COVID-19 patients who did not survive 30 days or who required mechanical ventilation, and CAC played an independent prognostic role. more...
